Critical success factors influencing construction safety program implementation in developing countries

The construction industry has always been considered as one of the most hazardous industries globally. The condition is worse in developing countries due to poor implementation of safety management. To overcome this problem, there is a need to focus on improving the implementation of safety programs. This paper aims to identify the factors that have the potential to influence the implementation of safety programs in construction projects in developing countries. An extensive review of literature found 21 critical success factors. Sixteen semi-structured interviews with experts in the Iraqi construction industry was conducted to gain in-depth insight and understanding of these factors. The interviewees confirmed the relevance of the success factors and emphasized on the importance of several factors, including management commitment, safety training, the enforcement of safety rules and regulations, and stakeholder collaboration. Technology is a new factor identified by the interviewees. The findings can used in the Iraqi construction industries and in developing countries to support the implementation of safety programs.

Implementation of Technosphere and Fire Safety Program (TFSP) to Increase Production Efficiency and Minimize Losses

Technosphere and Fire Safety Program (TFSP) is a production program whose main goal is to achieve maximum compliance with environmental laws, meet the requirements of safety standards and norms, maintain production equipment throughout its entire life cycle by engaging and motivating participation in this program throughout work force. The main three goals of the TFSP are: minimization of manufacturing defects, breakdowns and emergencies. According to this program, these goals can be achieved by taking measures aimed at improving the efficiency of equipment, creating an autonomous maintenance program, creating a planned maintenance system, organizing training courses for workers and developing an enterprise management system. The article considers the possibility of introducing technosphere and fire safety programs (TFSP) in industry. At the first stage, possible losses and factors contributing to these losses were determined. The general indicators of technosphere safety are affected by eight major production losses associated with equipment, which include equipment breakdowns, adjustment and commissioning, component replacement, production line start-up, idling, speed, manufacturing defects, and unplanned shutdowns. At the second stage of the introduction of the TFSP, a scheduled maintenance program was proposed with the aim of creating a smooth production process with minimizing technosphere risks.

Investigation of Safety Factors through Frequency and Severity Importance Index in Iranian Construction Project

The high rate of injuries in Tehran construction indicated that safety program may be neglected. This research has been conducted to identify root causes of accidents in Tehran construction projects and to propose ways to mitigate them. Accident causation factors divided in two groups based on reviewing literatures as follow, distal and proximal factors. The analysis on distal factors shows that the distal factors of accident causation in Tehran are result because of many issues and causes. Some of the serious factors are ‘Cash flow problems’, ‘inadequate budget for supervision’, ‘Lack of relevant experience’, ‘Recruit untrained operatives’ and ‘Reduce project budget’. By analyzing successful safety program implementation factors, the view of respondents indicates that general management in Tehran companies, are not completely committed to safety program and try to neglect safety. Moreover, the departments of labour’s inspectors regularly neglect inspections. On the other hand, lack of workers participation in safety program, has the potential for causing accidents which cause the safety program unsuccessful. Finally, for mitigating and preventing accidents in Tehran construction site, some recommendations have been made as follow: ‘Management should assign adequate budget for designing Section’, ‘Management should have enough and relevant experience and knowledge about the projects’ and ‘Management should recruit trained operative who has enough skill relevant with projects.

Abstract No. 635 Historical review of dosimetric trends in interventional radiology

To review trends in radiation dose indices from fluoroscopically-guided interventions (FGI) at an NCI-designated cancer center over the past decade. Dose indices including fluoroscopy time (FT), reference air kerma (Ka,r), and kerma area product (PKA) for FGI performed between July 2009 and May 2019 were extracted from an institutional database. Data was evaluated using Python including calculation of derived indices average x-ray field size (PKA/Ka,r) and procedural dose rate (Ka,r/FT). Error checking was performed on data prior to 2016, which was manually entered. Institutional procedure names were mapped to the ACR Common lexicon. Over 34,000 procedures were evaluated in aggregate and, for procedures with 100 or more instances, stratified by procedure. Procedures were classified by complexity on a scale of 1 (least) to 3 (most). Notable events in the institution were included in the evaluation ti [Editor's note: Incomplete sentence as per original submission.] Overall downward trends were observed for all indices with steeper decreases in Ka,r and procedural dose rate observed after implementation of an FGI patient safety program in 2009 and after a move to a new procedural area with new fluoroscopes in 2016. Average field size increased in early 2010 before decreasing steeply until 2016, when it began to increase before decreasing again in late 2017. Similar trends were observed in many individual procedures, with the strongest downward trends observed for nephrostomy exchange (average field size, slope = -5.1 cm2, r = -0.77); nephrostomy placement (log[Ka,r], slope = -0.021, r = -0.63); and nephrostomy exchange (log[procedural dose rate], slope = -0.013 Gy/s, r = -0.56). The trends depended on procedure complexity. With one exception, average field size decreased strongly for the most complex procedures. Ka,r trended downward for all of the least and most complex procedures, but not for all procedures of intermediate complexity. Procedural dose rate trended downward for all of the most complex procedures but not for all of the least complex procedures. Over the past decade, dose indices for FGI have trended downwards, with stronger trends observed around educational interventions and the installation of new equipment.

Development of Safety Plan Based on Work Breakdown Structure to Determine Safety Costs for Building Construction Projects. Case Study: Lower Structure Building

The construction industry has an important role in the economic development of the country, especially developing countries. However, along with the increase in the construction sector, work accidents in the construction sector also increased. This accident has a negative impact on the project, such as loss of life, material, time, and others. The risk of workplace accidents can be prevented through a well-designed safety management system. The prevention of the workplace accidents can be done by identifying potential hazards that occur in each activity contained in the work breakdown structure (WBS). Identification of potential hazards is needed to be planned well so that the construction companies can take the best preventive action against any potential hazards that exist in each activity of the construction projects, so the safety performance can be improved. After the potential hazards have been identified, then the preventive actions are taken to control the hazards to minimize the safety risk rate. The implementation of safety programs requires costs. But there is no separate budget in this sector that make the implementation of the safety management is still not optimal. Thus, the financing of the safety management system was taken from the project budget, which led to a reduction in the profits of construction companies. This study aims to develop the cost of safety management system based on work breakdown structure on the building lower structure work. The results of this study are the standardized WBS of the lower structure of buildings, potential hazard sources in the lower structure of the building construction projects, the preventive action to each potential hazards in each activity, and finding the safety cost components based on work breakdown structure with the aim of improving the quality of the occupational health and safety management system and reducing work accidents in the building construction projects.

Multiple Interventions for Improving Food Safety Practices in 2 Small Beef Abattoirs of Honduras and Associated Impacts on Risk-Mitigation Management.

Mitigation of risk for Shiga toxin-producing Escherichia coli (STEC) and Salmonella contamination was evaluated after a multiple-intervention approach (comprising food safety education and training, implementation of customized food safety practices and programs, and environmental monitoring programs with audits and corrective actions) in 2 small Honduran beef abattoirs. Previously, neither abattoir had food safety programs in place nor were they subjected to strict food safety regulatory surveillance. Abattoirs A and B were sampled on 4 nonconsecutive months each. Swab samples of abattoir A (n = 160, 40 samples per sampling date) and abattoir B (n = 78, 16-22 samples per sampling date) were taken from direct and indirect food contact surfaces, screened by BAX real-time polymerase chain reaction (PCR) assays and confirmed using immunomagnetic separation, selective media, and latex agglutination. In abattoir A, Salmonella presence was negligible, whereas presumptive STECs were present in 10%, 12.5%, 0%, and 5% of the environmental samples respective to each sampling month, indicating a reduction of STEC (P = .06) by the third and fourth sampling months. Conversely, presumptive STEC presence was negligible in abattoir B, whereas Salmonella presence for each sampling month was of 5.6%, 6.3%, 27.3%, and 0.0%, respectively. Upon the increased pathogen presence detected on the third sampling month, additional actions were taken to reinforce the implementation of food safety practices and programs, which resulted in a Salmonella reduction to 0% by the fourth sampling month (P = .013). The satisfactory results strongly suggest that a multiple-intervention approach is crucial to improve food safety in this type of premises.

An intercontinental analysis of food safety culture in view of food safety governance and national values

Taking food safety culture into account is a promising way to improve food safety performance in the food industry. Food safety culture (FS-culture) research is expanding from an organisational perspective to include characteristics of the internal and external company environment. In this study, the prevailing food safety culture in 17 food companies from four countries on three continents (Africa, Asia and Europe) was assessed in view of food safety governance and national values. The internal environment characteristics, i.e. food safety vision, food safety program and food production system vulnerability, were also assessed. Statistical analysis revealed little variation in FS-culture scores between the companies within the same country. Overall the FS-culture for Greek and Zambian companies was scored proactive, while for Chinese and Tanzanian companies an active score was achieved. Both the internal and external company environment seemed to influence the prevailing FS-culture. Cluster analysis showed that Tanzanian and Zambian companies exhibited similarities in the implementation of food safety programs, and in their national values and food safety governance as compared to Greece and China. Food safety governance was reflected in the food safety programs and supportiveness of the organisation to food safety and hygiene. All cultural dimensions were correlated with risk perceptions, with masculinity and long-term orientation also significantly correlated with the enabling conditions and attitude. Understanding how national values and food safety governance approaches differently influence food safety culture is expected to enable formulation of best approaches tailored for companies operating in countries with different company environments, to improve food safety performance.

The implementation of occupational safety and health carried out by all employees must be oriented to completion of work. This study was designed to describe work safety and work safety at PT. Indonesian Railways. Data collection techniques used in this research are conducting field studies by direct monitoring, interviews, and documentation. Analysis of the data used in this study is an interactive analysis that begins with data reduction, data presentation and conclusion conclusions obtained about the facts that occur in the field. Research results Obtained from that, the implementation of occupational safety and health programs at PT. Indonesian Railways was not carried out due to lack of facilities and the minimal number of participants participating in the training.

Developing strategies for patient safety implementation: a national study in Iran.

Today, healthcare organizations focus mainly on development and implementation of patient safety strategic plan to improve quality and ensure safety of provided services. The purpose of this paper is to recommend potential strategies for successful implementation of patient safety program in Iranian hospitals based on a strengths, weaknesses, opportunities, threats (SWOT) analysis. In this qualitative study, key informant interviews and documentation review were done to identify strength and weakness points of Iranian hospitals in addition to opportunities and threats facing them in successful implementation of a patient safety program. Accordingly, the research team formulated main patient safety strategies and consequently prioritized them based on Quantitative Strategic Planning Matrix (QSPM) matrix. The study recommended some of the potential patient safety strategies including provision of education for employees, promoting a safety culture in hospitals, managerial support and accountability, creating a safe and high-quality delivery environment, developing national legislations for hospital staff to comply with patient safety standards and developing a continuous monitoring system for quality improvement and patient safety activities to ensure the achievement of predetermined goals. Developing a comprehensive and integrated strategic plan for patient safety based on accurate information about the health system's weaknesses, strengths, opportunities and threats and trying to implement the plan in accordance with patient safety principles can help hospitals achieve great success. Ministry of Health and Medical Education (MOHME) conducted a national study to recommend potential strategies for successful implementation of patient safety in Iranian hospitals based on a SWOT analysis and QSPM matrix.

The purpose of this study is to determine the most priority and most important of food safety program in oil & gas industrial .Data analysis method used is descriptive analysis based on expert justification and assessed using SAST (Strategic Assumption Surfacing & Testing) and AHP (Analytic Hierarchy Process). The result of SAST analysis shows that program inspection and monitoring to food service facility (food producer) is most certain and most importance. Based on assessed by AHP analysis, the first priority of food safety program is also inspection & monitoring with a value 0.275, followed by food safety training (0.207), campaign & awareness (0.184), HACCP system (0.136), public involvement (0.108) and the last is law enforcement (0.090). The consistency value ratio (CR) is 0.01 less than 0.1.

Implementation of Safety Standards for Youth Sports Leagues: The "Safe Stars" Example in Tennessee.

We intend for this to serve as a roadmap for others to adopt and institute a statewide philosophy that emphasizes the importance of a safe, positive, and high-quality experience for children while holding their respective youth sports communities accountable for its achievement. Currently, very few leagues meet the recommended minimum standards of safety. Continued advancement of strategic partnerships as well as research into the program's impact on sports participation, health, and safety will be some of the critical next steps in realizing the full value of this program.

Identifying factors and mitigation measures of safety practices for sustainable building construction

Sustainability is a balanced approach that puts equal focus on the environment, economy and society. Research suggests that safety issues is an integral dimension of social sustainability and considered as the major concern in the construction industry. Despite the rapid advancement of technology, implementation and establishment of safety programs, it is revealed that the rate of the fatalities and injuries in this industry is extremely high. However, in this sector achieving a safe working environment is an ongoing challenge. Therefore, this study aims at identifying and assessing the potential factors of safety practices in sustainable construction. A review of the current state-of-the-practices in the area of construction safety would be indispensable for establishing the critical factors and mitigation measures to share innovative research findings and gain access to future research trends. From the literature analysis thirty-seven (37) factors and twenty (20) mitigation measures were identified and presented a database of the findings of construction safety. The findings of this study can serve as motivation for researchers and practitioners to work on the next generation of studies and the development of future effective measures, thus ensuring a safe construction environment.

Solar Ultraviolet Radiation Exposure among Outdoor Workers in Three Canadian Provinces.

Solar ultraviolet radiation (UVR) exposure places outdoor workers at risk of skin cancer and exposure is difficult to control. In response, the Sun Safety at Work Canada (SSAWC) project was undertaken (2014-2016). The purpose of this substudy was to characterize the UVR exposure levels of outdoor workers in the SSAWC project. Thirteen workplaces in the provinces of British Columbia, Ontario, and Nova Scotia participated in an exposure monitoring campaign (late summer/early fall 2016). Study participants were workers from power utilities and municipalities. Participants wore a UVR measurement badge (light-sensitive polysulfone plastic) on their wrist, shoulder, or hardhat. Badge calibration and absorbance measurements were performed in the AusSun Research Lab. Personal UVR doses are presented as standard erythemal doses (SED) and compared with the internationally recommended exposure limit (1.3 SED), as well as to the total available UVR by date. Generalized linear models were used to examine determinants of solar UVR for personal UVR dose (for both SED and percent of ambient UVR). Models considered badge placement, date, province, industry, main job task, and the hours spent outdoors. Mean personal UVR dose of participating workers was 6.1 SED (nearly 5× the recommended limit). Just 14% of workers experienced 'acceptable' levels of solar radiation; 10% were exposed at >10 times the limit. In univariate analyses, workers in Ontario had the highest levels (mean 7.3 SED), but even in the lowest exposed province (British Columbia), the mean personal UVR dose was 4.5 SED. Utility workers had double the exposure of municipal workers (10.4 and 5.5 SED, respectively). In the determinants of exposure models, the differences by province were muted, but utility line workers and those in general maintenance had higher predicted exposures. Those who wore their badge on their hardhat also had higher values of SED in the fully adjusted determinants models. Solar ultraviolet overexposure among outdoor workers is a concern, even in a country like Canada with relatively low ambient UVR. Implementation of sun safety programs should be supported in an effort to reduce exposure in this vulnerable group of workers.

SAFETY AND HEALTH ANALISYS IMPLEMENTATION OF CONSTRUCTION PROJECTS WORK SAFETY IN STUDY ACEH PROVINCY

Material losses suffered by the company due to the applied program and appropriate safety management among other things, cessation of work which resulted in reduced production and increased frequency of repair or rehabilitation equipment/supplies. The purpose of this study was to assess the level of understanding, awareness, and the factors and constraints of the implementation of safety management programs work by the company to the contractor. To achieve this end, it is the goal of research is to identify the level of understanding and awareness of the parties to the contractor (contractor) on safety at construction project sites, identify the factors and constraints of the implementation of safety management program at the project site construction work, and to identify risks to safety that occurred construction project location. The research data obtained through a questionnaire about the respondent's profile and project descriptions, safety program implementation checklists, and questionnaires Perceptions of contractor safety programs. The results show a variable level of understanding and awareness of the parties to the contractor (contractor) on safety at construction project sites, the most dominant subvariabel selected respondents is subvariabel "Provision of First Aid boxes Accident Victim (P3K)" with a mean value of 5.000 and variance 0.000 . In the variable patterns of program implementation and management of safety is commonly done by the company to the contractor (contractor) subvariabel "Restore the Operating System on the Safe state" with 4.4462 mean value and variance 0.407. On the identification of variable risks to safety that occurred at the site of construction projects on the implementation of construction works of sub variables "machine, for example engine powerhouse" with a mean value of 4.7846 with 0.172 variant, "The working environment (outside the building, inside buildings, and under ground) "with a mean value of 4.7846 with 0.172 variant," The cut on the surface", "Burns", and “Below members of the body" with a mean value of 4.7846 with variance 0.172, are subvariabel-subvariabel as the most dominant.

Segurança do paciente: notificação de incidentes na Unidade de Terapia Intensiva

OBJETIVO: descrever a notificação de incidentes na unidade de terapia intensiva após implementação do Programa Nacional de Segurança do Paciente. MÉTODO: estudo retrospectivo, do tipo descritivo de abordagem quantitativa, realizado na unidade de terapia intensiva de um Hospital Privado do município de Salvador, Bahia, Brasil, nos meses de novembro e dezembro de 2016. RESULTADOS: a amostra de incidentes foi de 210 notificações, que revelaram 80% (n=168) por erro de medicação. Foram encontradas 11% (n=23) notificações por exteriorização de dispositivos; 4,8% (n=10) por flebite e um total de 4,3% (n=9) referentes à lesão por pressão. Não existiram notificações de queda no período estudado. CONCLUSÃO: O levantamento das notificações dos incidentes, mesmo existindo subnotificações, evidencia o tipo de atendimento prestado pela organização e a preocupação com a segurança do paciente

A demonstration project on the impact of safety culture on infection control practices in hemodialysis

Infections among hemodialysis patients continues to be major causes of morbidity and mortality despite advances in the science of infection prevention. Many infections are potentially preventable, yet research suggests that evidence-based interventions are not uniformly practiced in dialysis settings. The purpose of the project was to reduce the risk of infection among hemodialysis patients in an outpatient dialysis clinic in upstate New York through the development of an enhanced patient safety culture. A survey was used to assess the safety culture of a large outpatient dialysis program. A Comprehensive Unit-based Safety Program was instituted to enhance infection prevention practices. Evidence-based checklists and audit tools were used to track staff adherence to protocols. Scores on the survey were strongly correlated with bloodstream infection rates. Adherence to infection control standards improved when the End Stage Renal Disease Safety Program was implemented, with audits improving from 27%-82% of procedures performed correctly. Bloodstream infection rates decreased from 2.33-1.07 events per 100 patient months, and the standardized infection ratios decreased from 1.960-0.985 in the 12-months after implementation. The Comprehensive Unit-based Safety Program model and implementation of the safety program may be effective in improving the culture of safety and adherence to evidence-based practices in hemodialysis. Enhanced patient safety culture is correlated with improved patient outcomes.

Successful implementation of a medication safety program for Aboriginal Health Practitioners in rural Australia.

This study describes the development and implementation of a medication safety program for Aboriginal Health Practitioners practising in rural Australia. A two-stage process was used to develop and implement the medication safety program. Initially, a total of eight semi-structured interviews was undertaken with Aboriginal Health Practitioners to identify the main challenges faced while implementing medication safety in the organisation. This was followed by the development of a culturally appropriate medication safety program. Aboriginal Community Controlled Health Service. Aboriginal Health Practitioners. The participants' knowledge, confidence, behaviour, and utilisation of medication safety developed resources. The development and implementation of the medication safety program in the Aboriginal Community Controlled Health Service consisted of addressing the barriers to medication safety cited by the Aboriginal Health Practitioner from the interviews, providing face-to-face educational sessions and developing a culturally appropriate educational resource to address the identified gaps. The program developed was evaluated by 17 Aboriginal Health Practitioners who took part in the study. The evaluation of Aboriginal Health Practitioners' knowledge, confidence, behaviour, utilisation of the medication safety program and resources was undertaken using an anonymous survey. A total of 31 participants completed the survey: 17 before the training and 14 at 6 months post-training. The data analysis, using t test, revealed a statistically significant change in the Aboriginal Health Practitioners' knowledge, confidence, behaviour and utilisation. The success of the implementation of a collaborative medication safety program within the Aboriginal Community Controlled Health Service is dependent on understanding the barriers to medication safety in the workplace associated within the organisation and emphasising a wide culture of patient safety.

Approach for an automated safety configuration for robot applications

Configuration and implementation of safety programs for robot applications have to face the common requirements of changeability and flexibility. The paper points out the challenges for safety and that dynamic reconfiguration and automated compilation of safety programs are not state of the art and not supported by international standards. On the other side HRC technologies and collaborative production scenarios, lead to more required knowledge and a higher complexity in creating safety programs. This paper gives an overview of the state of the art, how safety programs for robot applications are generated and what approaches and methods can be found to automate the program generation. The throughout this paper introduced approach for an automated safety configuration for robot applications is based on the information model of the application. The model consists of product, process, resource and hazard information. Combined with a safety function model and safety standards, it results in a proposed safety state machine as safety program. The throughout this paper introduced safety states are explained by an application example. The safety program is structured by safety states and implemented in a conventional way.

THE RELATIONSHIP OF THE WORK SAFETY PROGRAMS IMPLEMENTATION WITH THE PRODUCTIVITY OF EMPLOYEES IN PACKER PARTNERS PT. SEMEN BOSOWA MAROS

The application of work safety programs is one form of the company's efforts to create safe and healthy workplace conditions for the workforce, so as to reduce absenteeism and increase employee productivity.This study aims to find out how the "Relationship between Work Safety Program Implementation and Employee Productivity in the Packer Section of PT. Semen Bosowa Maros”. This type of research is a type of quantitative research, namely an analytical survey. This study uses an analytic international design with a cross sectional study approach. With a sample of 48 respondents in the cement filling packer section with sampling, the total sampling method. Data obtained from the results of questionnaires, observations, and interviews. Then the data will be analyzed using chy-square analysis and fisher's excat to see the relationship between the dependent variable and the independent variable. Fisher's exact statistical test results obtained p value = 0.650> 0.05, this shows that there is no relationship between the knowledge of safety signs with the achievement of cement filling targets, obtained p value = 0.369> 0.05 which indicates that no there is a relationship between the frequency of health and safety training with the achievement of cement filling targets. With the chy-square statistical test, the value of p = 0.04 <0.05 indicates that there is a relationship between the use of personal protective equipment and the achievement of cement filling targets. It is necessary to install safety signs in the packer section to improve and foster awareness of the workforce in protecting themselves, improve K3 training and provide strict sanctions to workers who are not disciplined in using complete personal protective equipment.
