Analysis of Ethical and Legal Aspects in Implementing a Medical Record System in Hospitals on Patient Health Practices

Implementing a medical record system in a hospital plays an integral role in managing patient health information. Findings regarding breaches of confidentiality, inaccurate information, inappropriate access, delays in patient access, and non-transparency of information are the main focus. Ethical implications involve the risk of losing patient trust and the establishment of a positive doctor-patient relationship, while legal implications include potential lawsuits by patients and administrative sanctions against medical record system administrators. Online Course Design Review: Quality e-Learning Practices in Higher Education

This article presents the importance of online course revision to improve best practices in distance higher education, increase student success and raise retention rates. A year-long, systematic, unified approach to online course review was conducted by a committee of Humanities professors. The aim of this paper is to share the significant enhancements made to an online course design, using the 7th Edition of Quality Matters Higher Education Rubric to boost student engagement, performance, and satisfaction. This study discusses how each standard was applied to refine an online course. The purpose of the revision was to systematically address the standards as proposed by the QM rubric. Findings revealed areas to strengthen in each of the eight general standards resulting in the actualization of the course design to be more accessible and practical for student use. A few of the specific enhancements because of the revision were the creation of a course content map, clear and predictable design, weekly email reminders, and rubrics. Student feedback confirmed positive outcomes after the successful implementation of best practices using the QM rubric.

Open educational resources (OER) management: implications for policymakers on the best practices in an open education university libraries in Nigeria

PurposeThe study aimed to identify the best practices for open educational resources (OER) management in Nigerian open education university libraries and the implications for policymakers.Design/methodology/approachA mixed-methods approach was used, which included a survey questionnaire administered via Google Forms to academic librarians and other library staff of the National Open University of Nigeria (NOUN). The study’s population was 398 and 273 participants responded to the survey. The study included all academic librarians and other library staff representing the six geopolitical zones in Nigeria. The data were analyzed using both qualitative and quantitative methods. The study’s results were presented using frequency counts, percentages tables and the qualitative data analysis was done thematically.FindingsThe findings of the study are consistent with existing literature on best practices for managing OER, which emphasized the importance of establishing clear policies and strategies for OER management practices, such as guidelines for acquiring, creating, curating and disseminating OER.Originality/valueThe originality of the study concludes by recommending that policymakers should develop policies that support the implementation of best practices in OER management to improve access to educational resources and reduce costs for students.

Flushing and locking management related to central venous catheter occlusion rate among adult patients in acute care: a best practice implementation project.

A central venous catheter (CVC)-a type of central venous access device (CVAD)-is the most common intervention for critical illnesses. Obstruction of the CVC can lead to fatal consequences. Thus, it is critical to maintain catheter lumen patency. The CVC occlusion rate in a hospital in Taiwan was 33%. This project aimed to decrease the CVC occlusion rate in acute care. This project was conceptually informed by the JBI Evidence-Based Model of Healthcare, in particular, the conceptualization of evidence implementation as inclusive of context analysis, implementation, and evaluation of outcomes using evidence-based quality indicators. As part of the seven-phase implementation process, we used audit and feedback in a pre- and post-test design to measure baseline compliance. Based on the initial audit, we developed an implementation strategy responsive to the identified gaps in compliance. We then undertook a final audit to measure changes in compliance to evaluate our implementation effects. The JBI software, PACES, and the situational analysis software, GRiP, were used for data collection and implementation planning. There were six evidence-based criteria, with a sample size of 30 nurses for each criterion. The team carried out the project from September 2022 to January 2023. Post-implementation audit compliance rates increased to 100% for the following criteria: the organization had a standardized flushing and locking solution protocol (0%), the CVAD lumen was locked upon completion of the final flush (10%), a single-dose system was used for flushing and locking the CVAD (60%), and preservative-free 0.9% sodium chloride was used to flush the CVAD (60%). Furthermore, the CVC occlusion rate decreased from 33% to 5%. The project successfully decreased the rate of CVC occlusion and increased the competence of nurses in acute care settings. The implementation of best practices in clinical care should focus on leadership, cross-department coordination, education, and innovation.

Implementation of Best Practices in Pancreatic Cancer Care in the Netherlands: A Stepped-Wedge Randomized Clinical Trial.

Implementation of new cancer treatment strategies as recommended by evidence-based guidelines is often slow and suboptimal. To improve the implementation of guideline-based best practices in the Netherlands in pancreatic cancer care and assess the impact on survival. This multicenter, stepped-wedge cluster randomized trial compared enhanced implementation of best practices with usual care in consecutive patients with all stages of pancreatic cancer. It took place from May 22, 2018 through July 9, 2020. Data were analyzed from April 1, 2022, through February 1, 2023. It included all patients in the Netherlands with pathologically or clinically diagnosed pancreatic ductal adenocarcinoma. This study reports 1-year follow-up (or shorter in case of deceased patients). The 5 best practices included optimal use of perioperative chemotherapy, palliative chemotherapy, pancreatic enzyme replacement therapy (PERT), referral to a dietician, and use of metal stents in patients with biliary obstruction. A 6-week implementation period was completed, in a randomized order, in all 17 Dutch networks for pancreatic cancer care. The primary outcome was 1-year survival. Secondary outcomes included adherence to best practices and quality of life (European Organisation for Research and Treatment of Cancer [EORTC] global health score). Overall, 5887 patients with pancreatic cancer (median age, 72.0 [IQR, 64.0-79.0] years; 50% female) were enrolled, 2641 before and 2939 after implementation of best practices (307 during wash-in period). One-year survival was 24% vs 23% (hazard ratio, 0.98, 95% CI, 0.88-1.08). There was no difference in the use of neoadjuvant chemotherapy (11% vs 11%), adjuvant chemotherapy (48% vs 51%), and referral to a dietician (59% vs 63%), while the use of palliative chemotherapy (24% vs 30%; odds ratio [OR], 1.38; 95% CI, 1.10-1.74), PERT (34% vs 45%; OR, 1.64; 95% CI, 1.28-2.11), and metal biliary stents increased (74% vs 83%; OR, 1.78; 95% CI, 1.13-2.80). The EORTC global health score did not improve (area under the curve, 43.9 vs 42.8; median difference, -1.09, 95% CI, -3.05 to 0.94). In this randomized clinical trial, implementation of 5 best practices in pancreatic cancer care did not improve 1-year survival and quality of life. The finding that most patients received no tumor-directed treatment paired with the poor survival highlights the need for more personalized treatment options. 163 Knowledge and Implementation of Tobacco Control Practices in Rural Louisiana Community Health Centers

OBJECTIVES/GOALS: Tobacco use remains a significant public health problem in rural America. Community health centers (CHCs) can help reduce the burden of tobacco use in rural areas, but we know little about their knowledge and implementation of best practices for tobacco control. This study assessed the knowledge and existence of tobacco control practices in rural CHCs. METHODS/STUDY POPULATION: Using a cross-sectional study design, we electronically surveyed health administrators and providers (n=64) in six rural CHCs in Louisiana between March 2021 and June 2023. The assessment measured 1) knowledge of the U.S. Public Health Service Clinical Practice Guideline for Treating Tobacco Use, 2) the priority given to smoking cessation programming, 3) the presence of best practices for tobacco control programming, such as having a tobacco control champion and team, 4) establishment of treatment and smoke-free campus policies, and 5) existence of referral procedure to external cessation services. We used descriptive statistics to characterize survey participants and responses. RESULTS/ANTICIPATED RESULTS: Most of the respondents were female (80%), White (61.8%), between 35 and 49 years of age (48.1%), and non-smokers (72.7%). Only half (51.6%) reported knowledge of the guideline for treating tobacco use among all respondents. Only a third (32.8%) said their health center gave smoking cessation high priority relative to other health priorities. Only a third (35.9%) reported having a tobacco champion; less than a fifth (18.8%) said they had a tobacco control team at their health center. Although all health centers had a smoke-free campus policy, about a quarter (23.4%) were unaware of the policy. Less than a quarter (23.4%) reported having a written policy for smoking cessation treatment at their health center, and less than half (46.7%) knew about cessation services to which they could refer patients. DISCUSSION/SIGNIFICANCE: Rural CHCs had limited knowledge of the guideline for tobacco use treatment. Smoking cessation lacked priority, and the prevalence of tobacco control best practices implementation was low. Rural CHCs must improve their implementation of guideline-recommend policies and clinical strategies to promote treatment and reduce the burden of tobacco use.

UTILIZATION OF DIGITAL-BASED LEARNING MEDIA IN THE INDEPENDENT CURRICULUM IN ELEMENTARY SCHOOLS

The utilization of digital-based learning media in elementary schools is becoming increasingly important in supporting the implementation of the Free Curriculum. Digital media such as interactive multimedia, e-learning, instructional videos, augmented reality (AR), and virtual reality (VR) have the potential to enhance active student participation, facilitate contextual and meaningful learning, and develop 21st-century skills. However, its implementation faces challenges such as limited technological infrastructure, educator skill shortages, lack of quality content, and insufficient stakeholder support. This research deeply examines the utilization of digital-based learning media in the Free Curriculum at Indonesian elementary schools, identifies influencing factors, and proposes optimization strategies. The method used is a literature review by analyzing and synthesizing information from relevant literature sources. The research results show that optimizing the utilization of digital media requires a comprehensive approach, including infrastructure development, educator training, provision of quality content, stakeholder collaboration, supportive policies, adequate funding, and the implementation of best practices. This research contributes to the development of effective implementation strategies to support the use of digital media in elementary school learning and to achieve the goals of the Free Curriculum in creating meaningful learning processes and developing 21st-century skills.

Perspective: Beyond the Mediterranean Diet—Exploring Latin American, Asian, and African Heritage Diets as Cultural Models of Healthy Eating

The Mediterranean diet is a well-studied cultural model of healthy eating, yet research on healthy models from other cultures and cuisines has been limited. This perspective article summarizes the components of traditional Latin American, Asian, and African heritage diets, their association with diet quality and markers of health, and implications for nutrition programs and policy. Though these diets differ in specific foods and flavors, we present a common thread that emphasizes healthful plant foods and that is consistent with high dietary quality and low rates of major causes of disability and deaths. In this perspective, we propose that nutrition interventions that incorporate these cultural models of healthy eating show promise, though further research is needed to determine health outcomes and best practices for implementation.

Assessing nitrate groundwater hotspots in Europe reveals an inadequate designation of Nitrate Vulnerable Zones

Monitoring networks show that the European Union Nitrates Directive (ND) has had mixed success in reducing nitrate concentrations in groundwater. By combining machine learning and monitored nitrate concentrations (1992–2019), we estimate the total area of nitrate hotspots in Europe to be 401,000 km2, with 47% occurring outside of Nitrate Vulnerable Zones (NVZs). We also found contrasting increasing or decreasing trends, varying per country and time periods. We estimate that only 5% of the 122,000 km2 of hotspots in 2019 will meet nitrate quality standards by 2040 and that these may be offset by the appearance of new hotspots. Our results reveal that the effectiveness of the ND is limited by both time-lags between the implementation of good practices and pollution reduction and an inadequate designation of NVZs. Substantial improvements in the designation and regulation of NVZs are necessary, as well as in the quality of monitoring stations in terms of spatial density and information available concerning sampling depth, if the objectives of EU legislation to protect groundwater are to be achieved.

Lifecycle of surgical devices: Global, environmental, and regulatory considerations.

Technological advancements, improved surgical access, and heightened demand for surgery have fueled unprecedented device and supply turnover impelling wealthy hospitals to upgrade continually and sell, donate, recycle, or dispose of used, expired, antiquated, or surplus goods. This paper reviews the issues related to device and supply lifecycles and discusses the opportunities and challenges they present for sustainable surgical growth in low- and middle-income (LMICs) countries. This review found, in LMICs countries, regulatory disparities persist that limit effective harmonization secondary to highly variable national policies and a lack of prioritized enforcement. Heterogeneity in the regulatory landscape, specifically in the classification, nomenclature, and identification of medical devices, encumbers effective regulation and distribution. Once devices are sold, donated, or reused in LMICs countries, complexities arise in regulatory compliance, maintenance, and appropriate use of these technologies. At the end of the lifecycle, waste management poses significant obstacles with limited resources hindering the implementation of best practices. There are major disparities in access to quality surgical equipment and supplies around the world. Improved communication between relevant stakeholders and harmonization of manufacture and disposal regulations will be needed to ensure adequate and appropriate responses to these challenges. Introduction to Privileged Access Management

Privileged Access Management (PAM) plays a crucial role in modern cybersecurity. Deficits in blood culture collection in the emergency department if sepsis is suspected: results of a retrospective cohort study.

Blood cultures (BCs) are key for pathogen detection in septic patients. We investigated the extent to which sampling was performed and what factors were associated with the absence of general or inadequate BC sampling. We conducted a retrospective cohort study of hospitalized patients with sepsis admitted to one of three EDs in 2018. Primary outcome was the extent of general BC collection of at least 1 set. Secondary outcome was the extent of adequate BC sampling, defined as ≥ 2 sets before antibiotic therapy (AT). Multivariable logistic regression analysis was performed to identify factors associated with deficits in both outcomes. 1143 patients were analyzed. BCs were collected from 946 patients. Single BCs were taken from 520 patients, ≥ 2 sets from 426 patients. Overall, ≥ 2 BCs were taken from 349 patients before AT. BC sampling before AT occurred significantly more frequently when ≥ 2 BC sets were taken rather than a single one (81.9%, versus 68.4%, p < 0.001) and this also led to the highest pathogen detection rate in our cohort (65.6%). A body temperature of ≥ 38°C was the a supporting factor for general and adequate BC collection in all three EDs. Retrospective analysis of 533 patients showed that the qSOFA score had no influence on general or adequate BC collection. Data on everyday clinical practice in the pre-analytical phase of microbiological diagnostics shows considerable deficits and indicates the need for more implementation of best practice. The variations identified in BC sampling between EDs should be further investigated.

Enhancing “HyFlex” Instruction: Best Practices, Criticisms, and Results from a Case Study in Graduate Education

HyFlex learning (a course modality in which students have the week-to-week option to attend either virtually or in-person) has gained prominence due to the increasing demand for flexible education options, especially in master’s programs for working professionals. It is frequently touted as a solution to programs that want to include in-person requirements but struggle with enrollment challenges and need to increase flexibility. This paper discusses the conversion of a traditionally in-person course to a HyFlex Course and the adaption of several active-learning modules to the new course modality, answering the following research questions: 1) What are some identifiable best practices for HyFlex instruction? 2) What type of active learning exercises are appropriate for HyFlex classrooms, and what active learning exercises are difficult to curate in a HyFlex environment? 3) What are the equity concerns in a HyFlex classroom, and how can instructors ensure that students have access to the same high-quality education? This research, which consists of a literature review of best practices and a case study applying lessons to the conversion of active-learning modules to HyFlex practice, found mixed results regarding student success in HyFlex courses, emphasizing the importance of addressing the unique challenges posed by this modality. Some issues that are discussed include concerns about lack of consistency, technical issues, difficulty engaging asynchronously, increased instructor workload, management challenges, and the need for effective communication. In conclusion, it finds that HyFlex teaching offers flexibility but presents challenges that educators and institutions must address to create a successful learning environment that meets the needs of all students. Awareness of potential issues and the implementation of best practices can help mitigate these challenges and enhance the effectiveness of HyFlex instruction.

A Study to Establish the Relationship between Hand Hygiene Compliance and Hospital Associated Infections in an ICU Setup of a Tertiary Care Teaching Hospital

Proper hand hygiene is especially important in a hospital setting where patients may have weakened immune systems and are more susceptible to infections. It is essential for healthcare professionals to follow strict hand hygiene protocols to ensure the safety and well-being of both patients and staff. Hand hygiene audits should be conducted regularly to assess compliance with these protocols and identify areas for improvement. These audits can help identify any gaps in hand hygiene practices and provide opportunities for additional training or education for healthcare professionals. By consistently monitoring and reinforcing proper hand hygiene, hospitals can minimize the spread of infections and create a safer environment for everyone involved in patient care. The present study provides an overview of the hand hygiene audits conducted at a tertiary care hospital in Pune, India, and highlights the areas for improvement and implementation of best practices.

Microbiological quality of raw milk: community expansion tanks in Alagoas

Milk is a highly nutritious food consumed by a large part of the population, and due to its nutritional composition, its quality can be altered by the amount of microorganisms present. The objective of this study was to evaluate the microbiological quality of raw milk from community expansion tanks in the state of Alagoas. A total of 160 milk samples were aseptically collected from the three mesoregions of the state for microbiological analysis. It was observed that 10.83% (13/120) of the samples had Staphylococcus coagulase-positive counts lower than 105 CFU/mL, and 10.83% (13/120) had counts higher than 105 CFU/mL. For coliforms, 18.8% (30/160) were found to have counts less than 1.1×103 MPN/mL, while 81.2% (130/160) of the samples had values higher than 1.1×103 MPN/mL. No Salmonella spp. or Listeria spp. were found in any of the samples. Aeromonas spp. was found in 28.7% (46/160) of the samples. For mesophilic microorganisms, 15% (24/160) of the samples were within the standard required by legislation, while 85% (136/160) were outside the established limit of up to 6×105 CFU/mL. Psychrotrophic microorganism counts were higher than 5×106 CFU/mL in 41.9% (67/160) of the samples analyzed. It was concluded from this study that the high microorganism counts in the analyzed milk and the inadequate temperature of the tanks at the time of collection can lead to the production of a low-quality product due to the deteriorative or pathogenic action of the microorganism. Therefore, the implementation of good practices during milking, transportation, and storage of refrigerated raw milk is suggested to prevent contamination of the raw material, ensuring a product with certified quality.

Best practices for implementation of Kamishibai cards in the healthcare setting to improve nursing-sensitive indicator associated outcomes: An integrative review.

Many adverse events are identified as nursing-sensitive indicators (NSIs) and have evidence-based care bundles known to reduce risk of occurrence. Kamishibai cards are a tool from the manufacturing industry used for practice auditing and improvements. Use of Kamishibai cards is believed to be common in the healthcare setting, but true evidence-based guidelines do not yet exist to guide their implementation. The aim of this integrative review was to identify best practices around the implementation of Kamishibai cards in the healthcare setting for improvement in NSI-associated outcomes. Eleven nurses at three facilities worked through the evidence using the Johns Hopkins Evidence-Based Practice Model. Ten articles were included for this review. Broad themes included direct observation with non-punitive and timely feedback, clearly visualized results, use of evidence-based care bundles, pre-implementation education, and both leadership and frontline-staff involvement. All facilities showed improvement in NSI-associated outcomes after the implementation of K-cards. In health care, K-cards can be implemented and designed with additional focus on the bundles of care they are intended to audit and staff support, but further evidence would better define guidelines around implementation.

USE OF SONG MEDIA IN LEARNING AS AN EFFORT TO INCREASE STUDENTS' LEARNING MOTIVATION ON THE SUBJECTS OF SCIENCE

This Best Practice writing aims to describe the use of song media to improve students' understanding of the material taught by the teacher. The subjects of this Best Practice writing were 25 students in class IV A of Wirasakti Atambua State Elementary School, West Atambua District. Writing this Best Practice uses a descriptive method. The achievement of Best Practice activities is 100%. This is because all activities have been carried out and the output obtained is in line with expectations. The use of song media to increase student motivation towards the material taught by the teacher is effective. The results of the implementation of Best Practice that have been implemented are: 1). Learning activities using song media related to the material studied every day are effective. 2). The use of song media in learning really helps students' understanding of the material being taught because it can be sung repeatedly both in class and at home. 3). Based on the learning that has been carried out, differences can be felt between students who carry out learning using song media and using the lecture learning model. 4). The activities that have been designed in the RPP (Teaching Module) can be implemented fully in accordance with what has been planned. The increase in students' understanding of the material being taught can be seen from the achievement of student learning outcomes, through the results of the pre-test and post-test that have been carried out by the teacher, where in the pre-test of the total number of students in class IV A, only 7 students or 2.24% who are able to achieve the KKM value. Meanwhile, in the post test results, there was a significant increase, namely 25 students, 7 students or 2.24% achieved the KKM score and 18 students or 92.8% were able to achieve a score above the KKM score.

The effectiveness of an interdisciplinary team in the VESTA Project at UnB

To analyze the effectiveness of the actions of an interdisciplinary team from the VESTA respirator research group. This is a cross-sectional, descriptive study with a quantitative approach. The participants were members of the interdisciplinary team of the VESTA Project at UnB. Data was collected using the Research Electronic Data Capture - REDcap software platform between May and August 2023. The questionnaire consisted of: a characterization form, the Jefferson Scale of Attitudes Related to Interprofessional Collaboration (EJARCI) and the Team Climate Scale for the Hospital Context in Brazil (ECECHB). The results were analyzed individually and jointly using graphs downloaded from the REDcap platform and exported to Microsoft Excel in order to apply statistics. The sample consisted of twelve participants from different areas of knowledge. The professionals who took part in the research work in the areas of biological sciences n=1 (8.3%), exact sciences n=1 (8.3%), engineering n=1 (8.3%), human sciences n=1 (8.3%), but the vast majority are in the field of health sciences n=8 (66.7%). The analysis of the EJARCI showed that all the participants had attitudes in favor of interprofessional collaboration, and there were no significant differences in the responses in relation to professional area or level of education. In relation to the ECECHB, a generally positive perception was seen, with the item with the highest score being support for new ideas, with an average of 34.03; and the item with the lowest score being task orientation, with an average of 23.75. Multiprofessional and interprofessional work contributes positively to the implementation of good practices and the improvement of health care.
