Zygomatic and Conventional Implants for Management of Severe Alveolar Atrophy in Partial Edentulous Maxilla and Completely Edentulous Mandible

The geriatric population, atrophic maxilla is a common condition.1.Severely resorbed maxilla is challenging for the installation of conventional osseointegrated implants. 2. To reduce the complications associated with bone grafting procedures and to simplify the rehabilitation of atrophicmaxilla, zygomatic implants play an essential role. 3. With the use of zygomatic implants the wait for osseous graft maturation is eliminated saving the treatment time and money.4.In this case report,nasal floor lift for anterior implants was performed with placement of bilateral zygomatic implants and conventional implants.

CONDIÇÕES DE ARMAZENAGEM E QUALIDADE DA MATÉRIA PRIMA: ESTUDO DE CASO EM UMA DERIVADORA DE ALIMENTOS A BASE DE MILHO

The storage process, in some way, has a direct influence on the production and status of the derived final product. It was possible to verify in this case study that the storage system of the company is deficient, needing improvements, especially when entering the raw material for production. Such problems, such as impurities, broken and burned grains, are generated in the warehouse itself, due to lack of prudence and coordination. Findings that contradict the objective of storage that is the conservation of the particularities of the grains and that may detract from the attributes of the end product in terms of taste, physical and chemical characteristics and visual conditions. Among the possible causes of the disturbances are the lack of maintenance of the machines, inefficient aeration and thermometry gauging. As a suggestion, besides the improvements on the mentioned problems, the implantation of management and strategy of inventories, of programs and quality control, development of the producing agents, planning and methods of solution of problems. The decline in the quality of the raw material, corn, can result in chain effect, damaging the final product, thus damaging the financial results of the organization.

Choice of implants in management of fractures in patients with post-polio residual paralysis

Introduction: Poliomyelitis is on the verge of eradication. Fractures in polio survivors present unique challenges; the bone is often small, deformed, osteoporotic and hypovascularized. Aim: Our aim was to assess the varied fracture patterns and the challenges faced in fixation of fractures in polio affected limbs and possible solutions to overcome them. Material & methods: All cases were managed at RMMCH, Annamalai University. Each and every case was unique in its own terms with variable clinical and radiological presentation. Varied challenges were managed in different manner with respect to implant choice for optimal outcome. Results: Fracture union and callus formation were evaluated on radiographs taken at each postoperative visit. Functional outcome assessments included range of motion and early return to activities of daily living. All fractures healed except for one nonunion due to conservative management. Conclusion: Anatomical locking plate may not be ideal for these patients as the plates may be larger and overhang the malformed bone. Hence nails with smaller diameter and locking compression plates designed for other skeletal regions may give better results than the conventional plates and nails. Fractures in the polio-affected limb are not a common entity and are difficult to manage using a common fracture management protocols and need to be customized for individual patients to suit their needs and bones with all implants ready for use.

Combined Special Capsular Tension Ring and Toric IOL Implantation for Management of Astigmatism and High Axial Myopia with Cataracts

Aim: This study aimed to compare the effects of toric intraocular lens (IOL) implantation with a capsular tension ring and toric IOL implantation only in patients with axial myopic astigmatism who had undergone cataract surgery. Methods: Of 34 patients with axial myopia, 16 patients who had received IOL and capsular tension ring (CTR) implantation were included in the combined group and 18 patients who received toric IOL implantation only were included in the simple group. Uncorrected visual acuity (UCVA) and best-corrected visual acuity (BCVA) were evaluated by measuring subjective refraction, residual astigmatism, and the toric IOL axis six months post-surgery. Results: At six months postoperatively, the UCVA for the combined and simple groups was 4.6 ± 0.1 and 4.5 ± 0.2, respectively, a statistically significant difference (t = 3.531, P<0.05). The toric IOL in all of the cases was located in the capsular sac, but there were more cases with IOL rotation (12 eyes) in the simple group than in the combined group (4 eyes). The rotation angles were 20°~30° (one eye), 10°~20° (four eyes), and <10° (seven eyes) compared with 2°~5° (four eyes). The residual astigmatism was –0.50 ± 0.25 D in the combined group, not a significant difference from the predicted residual astigmatism (–0.35 ± 0.13 D). There was a significant difference in the simple group (–1.25 ± 0.33 D) when the predicted residual astigmatism was compared (–0.37 ± 0.11 D) (t = –9.511, P < 0.01). Conclusions: In patients with axial myopic astigmatism, CTR can effectively increase the rotational stability of a toric IOL, achieving improvement in corneal astigmatism and visual acuity.

Alveolar Nerve Repositioning with Rescue Implants for Management of Previous Treatment. A Clinical Report

The goal of modern implant dentistry is to return patients to oral health in a rapid and predictable fashion, following a diagnostically driven treatment plan. If only a limited number of implants can be placed, or some fail and the prosthetic phase of implant dentistry is chosen to complete the patient's treatment, the final outcome may result in partial patient satisfaction and is commonly referred to as a "compromise." Previous All-on-4 implant treatment for the patient presented here resulted in a compromise, with an inadequate support system for the mandibular prosthesis and a maxillary complete denture with poor esthetics. The patient was unable to function adequately and also was disappointed with the resulting appearance. Correction of the compromised treatment consisted of bilateral inferior alveolar nerve elevation and repositioning without bone removal for lateral transposition, to gain room for rescue implants for a totally implant-supported and stabilized prosthesis. Treatment time to return the patient to satisfactory comfort, function, facial esthetics, and speech was approximately 2 weeks. The definitive mandibular prosthesis was designed for total implant support and stability with patient retrievability. Adequate space between the mandibular bar system and the soft tissue created a high water bridge effect for self-cleansing. Following a short interim mandibular healing period, the maxillary sinuses were bilaterally grafted to compensate for bone inadequacies and deficiencies for future maxillary implant reconstruction.
