Assessment of Microgap and Bacterial Leakage of Two Types of Internal Implant-Abutment Union.

The current research was done to assess microbial seepage of two types of internal implant-abutment connections. Twenty dental implants are categorized into two groups. Group A fixtures with an internal hexagonal geometry and group B fixtures with a tri-lobe internal connection. All implant-abutment assemblies underwent a three-week incubation period at 37°C in sterile tubes containing 5 mL of Staphylococcus aureus broth culture. Through the use of Gram stain and biochemical processes, the resultant colonies were recognized. The mean Log10 colony forming unit (CFU) in group A was 8.4 and in group B was 7.2. The variation between both groups was found to be considerable (P < 0.05). Microgap was more in group B compared to group A. Bacteria may infiltrate the small area between the implant and the abutment. Compared to dental implant fixtures with a tri-lobe internal connection, there was a noticeably higher Log10 CFU in dental implant fixtures with an internal hexagonal geometry.

Biomechanical Analysis of Titanium Dental Implants in the All-on-4 Treatment with Different Implant-Abutment Connections: A Three-Dimensional Finite Element Study.

The type of implant-abutment connection is one of the factors influencing the distribution of occlusal forces. This study aims to investigate the biomechanical performance of the mandibular all-on-4 treatment with different implant-abutment connections. Two connection types with 30° abutments and 18-mm implant fixtures were chosen for the posterior implants of the all-on-4 assembly. For the external hexagon connection (EHC) group, the implants with 4 mm in diameter were used. For the internal hexagon connection (IHC) group, we selected implants with 4.3 mm in diameter. A vertical force of 190 N was applied to the cantilever region. The FEA results indicated that the most stressed region in the two groups was prosthetic screws, followed by multi-unit abutments (MUAs). The lowest values of von Mises stress were both observed on the bone. The peak stress value of the implant screw and implant fixture in the EHC group were 37.75% and 33.03% lower than the IHC group, respectively. For stress distribution patterns, the load force tended to be concentrated at locations where components were interconnected. The EHC and IHC are clinically durable under the tested loading conditions, but the prosthetic screws and MUAs can be the weak point on the posterior implant within the mandibular all-on-four assembly. The peak stress values of implant screw and implant fixture in the EHC groups were lower than the IHC group.

Influence of Mechanical Cycling on Torque Values of Tapped-In and Screw-In Implant-Supported Crowns.

This study investigated the influence of mechanical cycling on screwed-in and tappedin implants restored with screw-retained metallic crowns. Three implant-abutmentcrown systems were evaluated: T1 (multi abutment) and T2 (standard abutment) received tapped-in abutments and S received a screwed-in abutment. The specimens were subjected to two million cycles of 0-150 N load, at 2 Hz, 30° inclination in a dry medium, and torque evaluation. Survival and removal torque were analyzed using chisquare, ANOVA, followed by Tukey's test. Differences between installation and removal torque were determined using a T-test for dependent samples. Analyses were performed in SPSS, considering α = 0.05. All specimens survived mechanical cycling in S, 40% in T1, 80% in T2 (p=0.008). Failures occurred due to loosening of the crown screw. A significant decrease in torque (p=0.000) was found. Group T1 had the lowest removal torque (1.6 ± 0.84 N.Cm²), followed by T2 (3 ± 1.49 N.Cm²) and S (6.3 ± 1.16 N. Cm²), and a statistical difference was found between Groups T1 and S. Both types of implantabutment connections were stable and can be considered for rehabilitative treatment, but failure and removal torque were influenced by the design of prosthetic abutment. Crowns were more susceptible to becoming loose in tapped-in systems.

Implant-Abutment Connections and Their Effect on Implant Survival Rates and Changes in Marginal Bone Levels (Δ): A Systematic Review and Meta-Analysis of 45,347 Oral Implants.

Purpose: To quantify the cumulative oral implant survival rates and changes in radiographic bone levels based on the configuration of the implant-abutment connection type over time. Materials and Methods: An electronic literature search was conducted in four databases (PubMed/MEDLINE, Cochrane Library, Web of Science, and Embase), and records were refereed by two independent reviewers based on the inclusion criteria. Data from included articles were grouped by implant-abutment connection type into four categories ([1] external hex; [2] bone level, internal, narrow cone < 45 degrees; [3] bone level, internal wide cone ≥ 45 degrees or flat; and [4] tissue level) and duration of follow-up (short-term 1 to 2 years, mid-term 2 to 5 years, and long-term > 5 years). Meta-analyses were performed for cumulative survival rate (CSR) and changes in marginal bone level (ΔMBL) from baseline (loading) to last reported follow-up. Studies were split or merged as appropriate based on the implants and follow-up duration in the study and trial design. The study was compiled under PRISMA 2020 guidelines and registered in the PROSPERO database. Results: A total of 3,082 articles were screened. Full-text review of 465 articles resulted in a total of 270 articles (representing 16,448 subjects with 45,347 implants) included for quantitative synthesis and analysis. Mean ΔMBL (95% CI) was as follows: short-term external hex = 0.68 mm (0.57, 0.79); short-term bone level, internal, narrow cone < 45 degrees = 0.34 mm (0.25, 0.43); short-term bone level, internal wide cone ≥ 45 degrees = 0.63 mm (0.52, 0.74); short-term tissue level = 0.42 mm (0.27, 0.56); mid-term external hex = 1.03 mm (0.72, 1.34); mid-term bone level, internal, narrow cone < 45 degrees = 0.45 mm (0.34, 0.56); mid-term bone level, internal wide cone ≥ 45 degrees = 0.73 mm (0.58, 0.88); mid-term tissue level = 0.4 mm (0.21, 0.61); long-term external hex = 0.98 mm, 0.70, 1.25); long-term bone level, internal, narrow cone < 45 degrees = 0.44 mm (0.31, 0.57); long-term bone level, internal wide cone ≥ 45 degrees = 0.95 mm (0.68, 1.22); and long-term tissue level = 0.43 mm (0.24, 0.61). CSRs (95% CI) were: short-term external hex = 97% (96%, 98%); short-term bone level, internal, narrow cone < 45 degrees = 99% (99%, 99%); short-term bone level, internal wide cone ≥ 45 degrees = 98% (98%, 99%); short-term tissue level = 99% (98%, 100%); mid-term external hex = 97% (96%, 98%); mid-term bone level, internal, narrow cone < 45 degrees = 98% (98%, 99%); mid-term bone level, internal wide cone ≥ 45 degrees = 99% (98%, 99%); mid-term tissue level = 98% (97%, 99%); long-term external hex = 96% (95%, 98%); long-term bone level, internal, narrow cone < 45 degrees = 98% (98%, 99%); long-term bone level, internal wide cone ≥ 45 degrees = 99% (98%, 100%); and long-term tissue level = 99% (98%, 100%). Conclusion: The configuration of the implant-abutment interface has a measurable effect on the ΔMBL over time. These changes can be observed over a period of at least 3 to 5 years. At all measured time intervals, similar ΔMBL was noted for external hex and internal wide cone ≥ 45-degree connections, as were internal, narrow cone < 45-degree and tissue-level connections.

Stress Distributions Created by Short and Regular Implants Placed in the Anterior Maxilla at Different Angles: A Finite Element Analysis

Although short implants are seen as alternative treatments that require additional surgical techniques in posterior region, they can be applied to anterior maxilla and various studies are required on this subject. The purpose of this study was to examine and compare the peak von Mises stress distributions in the crown, implant and abutment by using finite element analysis (FEA). Besides, a comparison of the implant-abutment connection types in the short implant with the FEA method was established. A short implant (4 × 5 mm) with a taper-lock connection and a regular implant (4 × 9 mm) with a screw connection were used in maxillary central incisor tooth area. Three different titanium abutments with 0?, 15? and 25? angles were used for abutments. In addition, in order to determine whether the stress change in short implants is due to the length of the implant-abutment connection, a screw was designed for a short implant and it was also evaluated in the same three angles. A total of three groups and nine models were generated. 114.6N load was applied to the cingulum area of the crown at an angle of 135? to the long axis of the crowns. A torque load of 25 Ncm was applied to the regular and short implant screw. Von Mises stress distributions of implants, abutments and crowns were evaluated by using FEA. Increased angle in implants increased von Mises stress values of implant, abutment and crown. Screw connection was found higher at all angles in short implants. Close values were found at different angles in taper-lock short implant crowns. The length and the angle in the bone of implant with the type of implant-abutment connection results in the accumulated stress values. Clinical Implications Taper implant-abutment connection system was found to be more promising in terms of stress accumulation in crowns. Although the amount of stress on the abutment increased due to the length of the implant in short implants, taper implant-abutment connection system slightly reduced related to this increase.

Implant – Abutment Connections : A Review on Marginal Bone Loss

Dental implants have been used for replacement of missing teeth. Various factors are considered to determine the success rate of dental implants. One of the important factors determining the success rate is peri-implant bone loss that occurs subsequently after implant placement . A variety of etiological factors have been listed responsible for peri-implant bone loss. One among the etiological factors for bone loss pertaining to implant geometry is implant-abutment connection type. Implant abutment connection type should be designed to minimize the bone loss around implants. Hence it is important to analyze and compare the bone changes that occur around implants with different connection types to choose an appropriate type for achieving maximum success rates.

Evaluation of Peri-implant Bone Change and its Risk Factors in Patients Presenting to a Medical Center: A 7-year Cross-sectional Study

&lt;p&gt;Abstract: Objective: To evaluate the amount of peri-implant bone loss detected on digital panoramic radiographs in patient presenting to Chi-Mei medical center and to determine the factors associated with peri-implant bone loss. Materials and Methods: The panoramic radiographs of patient who presenting to Chi-Mei medical center (CMH) form year of 2008 to 2014 were screened. Those with at least one dental implant were selected and further evaluated. Submerged implants, implants with healing cap, failure implants, and implants with blurred vision were excluded. For patient level, sex, age and number of implants were recorded. For each implant obscured on the radiographs, location, type of implant-abutment connection, type of prosthesis, implant placed at CMH or non-CMH were recorded. The amount of peri-implant bone loss (PIBL) was assessed at the mesial and distal aspect using magnifying option. Clinical features of patients and implants were separately presented by descriptive statistics. Two multiple logistic regression models were also performed. Results: A total of 34.83% (650/1866) implants with detectable bone loss were identified. According to the result of chi-square test, age, implant-abutment connections and type of prosthesis were separately shown a significant influence on PIBL. Patients older than 25 years old have greater than 2.5 times risk of developing bone loss than younger patients. Comparing to the bone level with butt-joint connections, tissue level and bone level with platform switch connection separately significantly decrease 93.5% and 59.2% risk of bone loss. For the type of prosthesis evaluation, fix partial denture, overdenture and full mouth fix complete denture increased 1.97, 2.39 and 1.53 times risk of bone loss. Conclusion: Among these patients presenting to CMH with implants, the prevalence rate of peri-implant bone loss was 34.83% in implant level. Age, different implant-abutment connection designs and type of prosthesis could play a major role in peri-implant bone loss.&lt;/p&gt; &lt;p&gt;&amp;nbsp;&lt;/p&gt;

Antimicrobial Efficacy and Permeability of Various Sealing Materials in Two Different Types of Implant-Abutment Connections.

The presence of a microgap along an implant–abutment connection (IAC) is considered the main disadvantage of two-piece implant systems. Its existence may lead to mechanical and biological complications. Different IAC designs have been developed to minimise microleakage through the microgap and to increase the stability of prosthodontic abutments. Furthermore, different sealing materials have appeared on the market to seal the gap at the IAC. The purpose of this study was to evaluate the antimicrobial efficacy and permeability of different materials designed to seal the microgap, and their behaviour in conical and straight types of internal IACs. One hundred dental implants with original prosthodontic abutments were divided into two groups of fifty implants according to the type of IAC. Three different sealing materials (GapSeal, Flow.sil, and Oxysafe gel) were applied in the test subgroups. The contamination of implant–abutment assemblies was performed by a joint suspension containing Candida albicans and Staphylococcus aureus. It was concluded that the IAC type had no significant influence on microleakage regarding microbial infection. No significant difference was found between the various sealing agents. Only one sealing agent (GapSeal) was found to significantly prevent microleakage. A complete hermetic seal was not achieved with any of the sealing agents tested in this study.

Effect of the geometry of butt-joint implant-supported restorations on the fatigue life of prosthetic screws

Statement of problemDental implant geometry affects the mechanical performance and fatigue behavior of butt-joint implant-supported restorations. However, failure of the implant component has been generally studied by ignoring the prosthetic screw, which is frequently the critical restoration component. PurposeThe purpose of this in vitro study was to evaluate the effect of 3 main implant geometric parameters: the implant body diameter, the platform diameter, and the implant-abutment connection type (external versus internal butt-joint) on the fatigue life of the prosthetic screw. The experimental values were further compared with the theoretical ones obtained by using a previously published methodology. Material and methodsFour different designs of direct-to-implant dental restorations from the manufacturer BTI were tested. Forty-eight fatigue tests were performed in an axial fatigue testing machine according to the International Organization for Standardization (ISO) 14801. Linear regression models, 95% interval confidence bands for the linear regression, and 95% prediction intervals of the fatigue load-life (F-N) results were obtained and compared through an analysis of covariance (ANCOVA) to determine the influence of the 3 parameters under study on the fatigue behavior (α=.05). ResultsLinear regression models showed a statistical difference (P<.001) when the implant body diameter was increased by 1 mm; an average 3.5-fold increase in fatigue life was observed. Increasing the implant abutment connection diameter by 1.4 mm also showed a significant difference (P<.001), leading to 7-fold longer fatigue life on average. No significant statistical evidence was found to demonstrate a difference in fatigue life between internal and external implant-abutment connection types. ConclusionsIncreasing the implant platform and body diameter significantly improved (P<.001) the fatigue life of the prosthetic screw, whereas external and internal connections provided similar results. In addition, experimental results proved the accuracy of the fatigue life prediction methodology.

Stability of implant-abutment connection in three different systems after fatigue test.

Abutment screw loosening of implant-supported prosthesis causes a mismatch between the abutment and the implant. This screw loosening is influenced by the implant-abutment connection type, however, with contradictory results reported in different studies. The present study evaluates the stability of abutment-implant connections in three different systems before and after the fatigue test. Thirty implants (4.3 mm in diameter and 12 mm in length) were divided into three groups of 10: Implantium, Zimmer, and straight internal hexagonal connection (SIC) implants. Two torques of 35 Ncm with an interval of 10 min were applied, followed by measuring removal torque value (RTV). The samples were re-torqued and then underwent a simulation of 1-year chewing clinical performance of dental implant under axial force of 400 N, with a frequency of 8 Hz (one million cycles). After fatigue test, the RTV was calculated and recorded. The mean RTVs obtained before and after cyclic load were analyzed by SPSS version 22 software using multivariate analysis. Significant differences in RTV and role of cyclic loading were found between SIC and Implantium groups (P = 0.006 and 0.021, respectively), as well as between Zimmer and SIC groups (P = 0.032 and 0.006, respectively), but not between Zimmer and Implantium groups (P = 0.771 and 0.248, respectively). The type of connection could affect the screw loosening, the preload loss, and the implant component stability. SIC group revealed the highest RTVs before and after cyclic loading.

Long-term effect of implant-abutment connection type on marginal bone loss and survival of dental implants.

This study aimed to compare the long-term survival rate and peri-implant marginal bone loss between different types of dental implant-abutment connections. Implants with external or internal abutment connections, which were fitted at Gangneung-Wonju National University Dental Hospital from November 2011 to December 2015 and followed up for >5 years, were retrospectively investigated. Cumulative survival rates were evaluated for >5 years, and peri-implant marginal bone loss was evaluated at 1- and 5-year follow-up examinations after functional loading. The 8-year cumulative survival rates were 93.3% and 90.7% in the external and internal connection types, respectively (P=0.353). The mean values of marginal bone loss were 1.23 mm (external) and 0.72 mm (internal) (P<0.001) after 1 year of loading, and 1.20 mm and 1.00 mm for external and internal abutment connections, respectively (P=0.137) after 5 years. Implant length (longer, P=0.018), smoking status (heavy, P=0.001), and prosthetic type (bridge, P=0.004) were associated with significantly greater marginal bone loss, and the use of screw-cement-retained prosthesis was significantly associated (P=0.027) with less marginal bone loss. There was no significant difference in the cumulative survival rate between implants with external and internal abutment connections. After 1 year of loading, marginal bone loss was greater around the implants with an external abutment connection. However, no significant difference between the external and internal connection groups was found after 5 years. Both types of abutment connections are viable treatment options for the reconstruction of partially edentulous ridges.

Four-implant-supported overdenture treatment in the maxilla. Part I: A randomized controlled split mouth trial assessing the effect of microthreads and abutment connection type on 4 years peri-implant health.

According to literature, peri-implant bone loss is minimized on implants with microthreaded neck design and internal type of abutment connection. However, most clinical studies may be biased due to confounding factors. This nonblinded RCT assessed the effect of implant neck (microthreaded vs non-microthreaded) as well as the type of abutment connection (internal conical vs external flat-to-flat) on peri-implant bone stability and peri-implant health after at least 36 months. Twenty-five patients were treated with a maxillary implant-supported bar-retained overdenture on four different implant types: internal connection with microthreads (I-MT), internal connection without microthreads (I-NMT), external connection with microthreads (E-MT), and external connection without microthreads (E-NMT). To control confounding factors, all other design features were similar. A linear mixed-model analysis or mixed-model logistic regression analysis was used to determine the effect of implant type on bone level, probing pocket depth, bleeding on probing, and plaque. Four out of 98 implants (4.1%) placed in 25 patients failed during provisionalization and were replaced. Mean overall bone loss after 6months was 0.39 mm (SD 0.62, range 0.00-3.48) with limited additional bone loss of 0.04 mm (SD 0.54, range -1.80-1.63) after at least 3 years. Microthreads or connection type had no effect on the bone level, probing pocket depth, bleeding on probing, nor plaque. With 96% of implant survival, the maxillary overdenture supported with a bar on four implants yield a predictable outcome and the implant-abutment connection type (internal vs external) and implant neck design (microthreaded vs non-microthreaded) have no influence on peri-implant bone remodeling after initial bone remodeling nor up to 4 years of function. Peri-implant bone levels are within international success standards and peri-implant health is indicative of absence of peri-implantitis.

Risk Indicators Affecting the Survival of the Mandibular First Molar Adjacent to an Implant at the Mandibular Second Molar Site: A Retrospective Study.

This study aimed to compare the survival of mandibular first molars (MnM1s) adjacent to implants placed in mandibular second molar sites (ImM2s) with MnM1s adjacent to mandibular second molars (MnM2s) and to investigate risk indicators affecting the survival of MnM1s adjacent to ImM2s. A total of 144 patients who had MnM1s adjacent to ImM2s and MnM1s adjacent to MnM2s on the contralateral side were included in this study. Clinical variables and radiographic bone levels were evaluated. The survival of MnM1s adjacent to ImM2s or MnM2s was evaluated using a Kaplan–Meier analysis and Cox proportional hazards model. The 5-year cumulative survival rates of MnM1s adjacent to ImM2s and MnM2s were 85% and 95%, respectively. MnM1s adjacent to ImM2s of the internal implant-abutment connection type had higher multivariate hazard ratios (HR) for loss. MnM1s that had antagonists with implant-supported prostheses also had higher HR for loss. The multivariate HR for the loss of MnM1s adjacent to ImM2s with peri-implant mucositis was 3.74 times higher than MnM1s adjacent to healthy ImM2s. This study demonstrated several risk indicators affecting the survival of MnM1s adjacent to ImM2s. It is suggested that supportive periodontal and peri-implant therapy combined with meticulous occlusal adjustment can prolong the survival of MnM1s and ImM2s.

Effects of Implant-Abutment Connection Type and Inter-Implant Distance on Inter-Implant Bone Stress and Microgap: Three-Dimensional Finite Element Analysis.

The attainment of a good aesthetic outcome in dental implant treatment requires inter-implant papilla reconstruction, which is very difficult to perform. Maintenance of the inter-implant bone is essential for maintenance of the inter-implant papilla. The aim of this study was to investigate the mechanical influences of the implant–abutment connection type and inter-implant distance on the inter-implant bone by using three-dimensional finite element analysis. Three computer-aided design models of two-piece implants were designed: external connection (EC), internal connection (IC), and conical connection (CC). In each model, two identical implants were placed with inter-implant distances of 3.0, 2.5, and 2.0 mm. The maximum principal stress and microgap were evaluated. The stress values of the inter-implant bone decreased in the following order: IC, EC, and CC. The microgap decreased in the following order: EC, IC, and CC. Regardless of the type of implant–abutment connection, the stress of the inter-implant bone increased as the inter-implant distance decreased. The microgap barely changed as the inter-implant distance decreased. A CC implant is a mechanically advantageous implant–abutment connection type for maintenance of the inter-implant bone. With an inter-implant distance of less than 3.0 mm, use of a CC implant might suppress absorption of the inter-implant bone.

Effects of implant diameter, implant-abutment connection type, and bone density on the biomechanical stability of implant components and bone: A finite element analysis study

Statement of problemVarious kinds of implants of different diameters and connection types are used for patients with a range of bone densities and tooth sizes. However, comprehensive studies simultaneously analyzing the biomechanical effects of different diameters, connection types, and bone densities are scarce. PurposeThe purpose of this 3-dimensional finite element analysis study was to evaluate the stress and strain distribution on implants, abutments, and surrounding bones depending on different diameters, connection types, and bone densities. Material and methodsTwelve 3-dimensional models of the implant, restoration, and surrounding bone were simulated in the mandibular first molar region, including 2 bone densities (low, high), 2 implant-abutment connection types (internal tissue level, internal bone level), and 3 implant diameters (3.5 mm, 4.0 mm, and 4.5 mm). The occlusal force was 200 N axially and 100 N obliquely. Statistical analysis was performed using the general linear model univariate procedure with partial eta squared (ηp2) (α=.05). ResultsFor bone tissue, low-density bone induced a larger maximum and minimum principal strain (in magnitude) than high-density bone (P<.001). As the implant diameter increased, the volume of the cancellous bone in low-density bone at the atrophy region (strain<200 με) increased (P<.001). For implant and abutment, the internal bone-level connection type was associated with increased peak stress as compared with the tissue-level connection type (P<.001). For all models, the stress distribution on the implant complex was influenced by implant diameter (P<.001): a decrease in implant diameter increased the stress concentration. ConclusionsThe implant connection type had a greater impact on the stress of the implant and abutment than the diameter. A tissue-level connection was more advantageous than a bone-level connection in terms of stress distribution of the implant and abutment. Bone density was the most influential factor on bone strain. The selection of dental implants should be made considering these factors and other important factors including tooth size.

Internal Hexagon vs Conical Implant-Abutment Connections: Evaluation of 3-Year Postloading Outcomes.

Different types of internal implant-abutment connections, namely hexagon and conical, have been used for implant restoration. However, data regarding the benefits of these internal connections in terms of clinical outcomes are scarce. Accordingly, the aim of this study was to compare radiographic marginal bone loss (RMBL) and associated implant complications between implants with internal hexagon (IH) connections and those with internal conical (IC) connections. Forty-nine patients with 98 implants (2 per patient) placed in the posterior mandible were recruited. All implants were inserted in pairs into solid D2 bone according to a randomized sequence; the first patient received an IH connection implant on the mesial side, while the second patient received an IC connection implant on the mesial side. Each patient received 1 implant with an IH connection and 1 with an IC connection, placed side by side. Four months after placement, all implants were loaded with single screw-retained ceramic restorations with IH or IC connections. RMBL and complications, including implant/prosthesis failure, were recorded at the time of implant loading (baseline) and at 6, 12, and 36 months after loading. The results revealed no significant between-group differences in RMBL (P = .74), gingival bleeding on probing (P = .29), and complications (P = .32). Thus, the type of internal implant-abutment connection did not affect clinical outcomes, including RMBL and implant/prosthesis failure. Future studies should additionally evaluate long-term prosthesis-related complications, such as screw loosening and fracture, between the 2 types of internal connections.

Review of the Mechanical Behavior of Different Implant-Abutment Connections.

Introduction: Different implant–abutment connections have been developed to reduce mechanical and biological failure. The most frequent complications are loss of preload, screw loosening, abutment or implant fracture, deformations at the different interfaces, and bacterial microleakage. Aim: To review the evidence indicating whether the implant–abutment connection type is significant regarding the following issues: (1) maintenance of the preload in static and dynamic in vitro studies; (2) assessment of possible deformations at the implant–abutment interfaces, after repeated application of the tightening torque; (3) evaluation of the sealing capability of different implant connections against microleakage. Materials and Methods: In June 2020, an electronic literature search was performed in Medline, EBSCO host, and PubMed databases. The search was focused on the ability of different implant connections to maintain preload, resist deformation after tightening and retightening, and prevent microleakage. The related titles and abstracts available in English were screened, and the articles that fulfilled the inclusion criteria were selected for full-text reading. Results: The literature search conducted for this review initially resulted in 68 articles, among which 19 articles and 1 systematic review fulfilled the criteria for inclusion. The studies were divided according to the three proposed objectives, with some studies falling into more than one category (maintenance of preload, surface abutment–implant deformation, and resistance to microleakage). Conclusions: Conical abutment appears to result in fewer mechanical complications, such as screw loosening or fractures, and higher torque preservation. After SEM evaluation, damage was observed in the threads of the abutment screws, before and after loading in internal and external connections. Internal hexagon implants and predominantly internal conical (Morse taper) implants showed less microleakage in dynamic loading conditions. We suggest further studies to guarantee excellence in methodological quality.

Clinical study on screw loosening in dental implant prostheses: a 6-year retrospective study.

ObjectivesIn this study, we determined the incidence and pattern of screw loosening in patients who received dental implants.Materials and MethodsPatients who received implants between January 2008 and October 2013 and completed their prosthetic rehabilitation were evaluated for the incidence, frequency, and onset of screw loosening using dental charts and radiographs. The association between each factor and screw loosening was analyzed using the chi-square test and a multivariate analysis with binary logistic regression models (P<0.05).ResultsTotal 1,928 implants were placed in 837 patients (448 males, 389 females), whose follow-up period after loading varied from 0.25 to 70 months (mean period, 31.5 months). Screw loosening occurred in 7.2% of implants. Most cases occurred less than six months after loading. Among those, 22.3% experienced recurrent screw loosening. Screw loosening was most common in the molar region (8.5%) and frequently associated with an implant diameter of ≥5 mm (14.2%). External implant–abutment connections (8.9%) and screw-retained implant prostheses (10.1%) showed higher incidence of problems than internal implant–abutment connections and cement-retained implants, respectively. Screw loosening was most common in implant prostheses with single crowns (14.0%).ConclusionWithin the limits of the current study, we conclude that the incidence of screw loosening differs significantly according to the position of implant placement, the type of implant and manufacturer, implant diameter, the type of implant–abutment connection, the type of retention in the implant prosthesis, and the type of implant prosthesis.

Fracture resistance after implantoplasty in three implant-abutment connection designs.

Background To assess the effect of implantoplasty and implant-abutment design on the fracture resistance and macroscopic morphology of narrow-diameter (3.5 mm) dental implants. Material and Methods Screw-shaped titanium dental implants (n = 48) were studied in vitro. Three groups (n = 16) were established, based on implant-abutment connection type: external hexagon, internal hexagon and conical. Eight implants from each group were subjected to an implantoplasty procedure; the remaining 8 implants served as controls. Implant wall thickness was recorded. All samples were subjected to a static strength test. Results The mean wall thickness reductions varied between 106.46 and 153.75 µm. The mean fracture strengths for the control and test groups were, respectively, 1211.90±89.95 N and 873.11±92.37 N in the external hexagon implants; 918.41±97.19 N and 661.29±58.03 N in the internal hexagon implants; and 1058.67±114.05 N and 747.32±90.05 N in the conical connection implants. Implant wall thickness and fracture resistance (P < 0.001) showed a positive correlation. Fracture strength was influenced by both implantoplasty (P < 0.001) and connection type (P < 0.001). Conclusions Implantoplasty in diameter-reduced implants decreases implant wall thickness and fracture resistance, and varies depending on the implant-abutment connection. Internal hexagon and conical connection implants seem to be more prone to fracture after implantoplasty. Effect of Implant-Abutment Connection Type on Bone Around Dental Implants in Long-Term Observation: Internal Cone Versus Internal Hex.

The purpose of this study was to evaluate the influence of the implant-abutment connection type on the bone level around dental implants in long-term observation and the survival rate for the different types of implant-abutment connections. Two groups of implants made of titanium grade 23 alloy and with sandblasted and acid-etched (SLA) surface were included in the study: (a) the internal hex implant-abutment connection group (480 SPI dental implants; Alpha-Bio Tec, Petach Tikwa, Israel, 184 patients) and (b) the internal cone implant-abutment connection group (60 C1 dental implants; MIS Implant Technologies, Shlomi, Israel, 34 patients). Certain inclusion and exclusion criteria were applied. Marginal bone loss (MBL) around the dental implants was measured in intraoral radiographs taken with parallel technique with a film holder and by bite recording index. X-rays were performed at the moment of functional loading, and at 12, 24, 36, and 60 months after loading. The digital analysis was conducted using Dental Studio 2.0 computer software. Average MBL was significantly lower in the conical connection compared with internal hex group-0.68 ± 0.59 versus 0.99 ± 0.89 mm (12 months), 0.78 ± 0.80 versus 1.12 ± 1.00 mm (24 months), 0.83 ± 0.87 versus 1.22 ± 1.03 mm (36 months), and 0.96 ± 1.02 versus 1.30 ± 1.15 mm (60 months after loading). Both groups of implants achieved a 100% survival rate. The internal cone connection reduced bone resorption compared with the internal hex. Both groups of implants had a 100% survival rate.
