IMPACT OF PATIENT DEMOGRAPHICS, CLINICAL CHARACTERISTICS, AND SOCIAL DETERMINANTS OF HEALTH ON ADVANCED THERAPY MEDICATION ADHERENCE AND PERSISTENCE IN ULCERATIVE COLITIS ACROSS UNITED STATES PAYER TYPES

Abstract INTRODUCTION Social determinants of health (SDOH) factors such as insurance status, race, ethnicity, and socioeconomic status have been associated with clinical outcomes in ulcerative colitis (UC). We assessed the impact of demographic, clinical, and SDOH factors on advanced therapy medication adherence and persistence in patients (pts) with UC. METHODS Pts with UC who initiated an advanced therapy (Janus kinase inhibitor, sphingosine-1-phosphate receptor agonist, or biologic) were identified using Commercial and Medicaid Managed Care (MMC) claims data from the MORE2 Registry (2017–2022) and Medicare Fee-for-Service (FFS) (2017–2021) claims. Community-level SDOH variables were linked using pt ZIP codes at index date (first advanced UC therapy prescription date from May 01, 2018 up to Jun 30, 2021). Adherence (proportion of days covered [PDC]) and persistence (duration) of treatment were assessed for 1 year post-index. Logistic regression analyses evaluated the associations between demographic, clinical, and SDOH factors and adherence/persistence. RESULTS The number of pts included in the Medicare FFS, Commercial, and MMC plan types were 9,158, 6,706, and 3,170, respectively. Mean PDC were 69%, 72%, and 64% (Fig. a), respectively, and the corresponding proportions of pts with 1-year persistence were 53%, 43%, and 31%, respectively (Fig. b). PDC by percentile is shown in Fig. c. Female pts had lower odds of adherence vs male pts in the Medicare FFS plan only (odds ratio [OR] 0.89; Table). A higher Charlson Comorbidity Index Score was associated with lower odds of adherence in the Medicare FFS and Commercial plans only (OR for both: 0.94; Table). In the 1 year post-index period, a corticosteroid prescription was associated with lower odds of adherence across plan types (OR 0.57–0.66; Table); a 5-aminosalicylic acid prescription was associated with higher odds of adherence in the Medicare FFS and Commercial plans only (OR 1.12–1.15; Table). A mental health condition diagnosis in the 1 year prior to and post-index was associated with lower adherence vs those without among pts with Medicare FFS and Commercial coverage (OR 0.85–0.87; Table). Community-level SDOH variables assessed had no association with adherence (Table); persistence model results were similar (data not shown). CONCLUSION For pts initiating an advanced therapy, there are still unmet treatment needs as demonstrated by mean PDC <80% and full persistence ≤53% across plan types. Adherence and persistence were lower among pts on MMC vs Medicare FFS or Commercial plans. The impact of demographic variables on adherence varied depending on plan type. Higher comorbidity burden, mental health diagnosis, and concomitant steroid use were generally associated with lower adherence to advanced UC therapies. Community-level SDOH variables were not associated with adherence in these populations.

Newly Developed Sex-Specific Z Score Model for Coronary Artery Diameter in a Pediatric Population.

This study aimed to generate a Z score calculation model for coronary artery diameter of normal children and adolescents to be adopted as the standard calculation method with consensus in clinical practice. This study was a retrospective, multicenter study that collected data from multiple institutions across South Korea. Data were analyzed to determine the model that best fit the relationship between the diameter of coronary arteries and independent demographic parameters. Linear, power, logarithmic, exponential, and square root polynomial models were tested for best fit. Data of 2,030 subjects were collected from 16 institutions. Separate calculation models for each sex were developed because the impact of demographic variables on the diameter of coronary arteries differs according to sex. The final model was the polynomial formula with an exponential relationship between the diameter of coronary arteries and body surface area using the DuBois formula. A new coronary artery diameter Z score model was developed and is anticipated to be applicable in clinical practice. The new model will help establish a consensus-based Z score model.

The short-, medium-, and long-term prevalence of physical health comorbidities in first-episode psychosis: a systematic review protocol

Background Individuals with first-episode psychosis (FEP) face an increased risk of physical comorbidities, notably cardiovascular diseases, metabolic disorders, respiratory disorders, and certain types of cancer. Previous reviews report pooled physical health prevalence from chronic psychosis and FEP groups. By contrast, this review will focus on antipsychotic-naïve FEP cohorts and incorporate data from observational longitudinal studies and antipsychotic intervention studies to understand the progression of physical health comorbidities from the onset to later stages of psychosis. This review aims to systematically examine the short-, medium-, and long-term period prevalence of these comorbidities in FEP and variations related to demographic factors. Methods A systematic review will be conducted using the PRISMA and MOOSE guidelines. Medline, Embase, PsycINFO, and CINAHL+, as well as Clinical Trials gov.uk, OpenGrey, WHO International Clinical Trials Registry Platform, Current Controlled Trials, United States National Institute of Health Trials Registry, and the Irish Health Repository, will be searched from inception. Longitudinal studies exploring physical health outcomes in FEP cohorts and antipsychotic intervention studies monitoring health outcomes in antipsychotic naïve FEP individuals will be eligible for inclusion. Two reviewers will independently screen titles, abstracts, and full-text articles. Bias in individual studies will be assessed using the JBI Critical Appraisal Checklist. A meta-analysis of the short-, medium-, and long-term prevalence of cardiovascular, metabolic, cancer, and respiratory outcomes and a narrative synthesis will be conducted. If possible, a meta-regression on the impact of demographic variables will be conducted. Conclusions This systematic review will clarify the progression of physical health comorbidities in FEP, informing early intervention strategies and policies for this population. Subsequent findings will be submitted to a leading journal, supplemented by a recovery education module for patient groups and a lay summary for wider dissemination. Registration The study was registered in PROSPERO, the International Prospective Register of Systematic Reviews (CRD42023431072; 17/06/2023).

Does fear of re-injury affect the self-perceived level of lower limb functionality among soccer players with ACL reconstruction?: A cross-sectional study.

The fear of re-injury may persist after anterior cruciate ligament (ACL) reconstruction (ACLR) in professional soccer players (PSPs) even after a successful return to sport (RTS). This study aimed to determine the extent of this fear of re-injury and the impact of demographic variables and this fear of re-injury on the lower extremity functional scale (LEFS) scores in PSPs who had completed a successful RTS following ACLR. A cross-sectional survey design was used. Sixty-seven PSPs who had successfully RTS after ACLR, completed a demographic information sheet, the Athlete Fear-Avoidance Questionnaire (AFAQ), and the LEFS. The average AFAQ scores indicated low fear (M = 10.2, SD = 6.7), while high LEFS scores were observed (M = 67.1, SD = 12.4). There were significant strong negative correlations between LEFS and body mass index (BMI; rs [65] = -0.501, P = .001) and AFAQ and BMI (rs [65] = -0.378, P = .001). A hierarchical linear regression analysis found AFAQ to be a significant predictor of LEFS (b = -0.92, s.e. = 0.19, P = .001), with a total variance (adjusted R2) of 32.9%. PSPs' fear of re-injury post-ACLR significantly reduced their perceived levels of lower limb functionality. This study highlights the need to further explore and develop strategies to manage PSP fear of re-injury to improve sports-related performance post-ACLR.

Impact of Demographic Variables on Recovery After Total Hip Arthroplasty

BackgroundSeveral patient factors affect recovery after total hip arthroplasty (THA). However, the impact of these variables on patient-reported outcome measure recovery curves following THA has not been defined. Our goal was to quantify the influence of multiple variables on recovery after primary THA. MethodsThere were 1,724 patients in a multicenter study included. Variables included sex, race/ethnicity, anxiety/depression, body mass index, tobacco, and preoperative opioid use. The Hip disability and Osteoarthritis Score for Joint Replacement (HOOS JR) was recorded at multiple time points. Recovery curves were created using longitudinal estimating equations. ResultsPatients who were women, obese, or smokers demonstrated lower HOOS JR scores at all time points. Preoperative opioid use was also correlated with lower HOOS JR scores, but this difference diminished after 6 months. Black patients demonstrated lower HOOS JR scores compared to Caucasians, and this relative difference increased out to 1-year postoperatively (P = .018). Hispanics also had lower HOOS JR scores, but scores recovered at similar rates compared to non-Hispanics. Patients who had only anxiety or depression had similar HOOS JR scores compared to patients who did not have anxiety or depression. However, patients who had both anxiety and depression had lower HOOS JR scores compared to patients who had neither (P = .049), and this relative difference became greater at 1-year postoperatively (P = .002). ConclusionsSeveral factors including race/ethnicity, opioid use, and mental health influence recovery trajectory following THA. This information helps provide more individualized counseling about expectations after THA and focus targeted interventions to improve outcomes in at-risk groups.

The influence of online multiplayer games on social capital and interdependent well-being in Japan

This study examines the effects of online game behaviors and players' characteristics on social capital and sense of belonging generated in multiplayer online games, and then on interdependent well-being. Hypotheses testing was performed using the partial least squares structural equation modeling based on an online survey conducted in 2021 (n = 383). Communication with close persons, acquaintances, and strangers all impact bridging social capital and bonding social capital, respectively, with the effect of communication with strangers being the strongest. Both extroversion and materialism positively impact bridging social capital and bonding social capital, while the impact of demographic variables is not found. Moreover, communication with close persons, strangers, bridging social capital, bonding social capital, and materialism positively impact belonging. Finally, belonging positively impacts interdependent well-being, and bridging social capital positively impacts interdependent well-being through belonging. This study shows the importance of interaction with strangers, and those players could develop broader and deeper social relationships in online games. Moreover, this study is the first to examine players' communication with players across different relationship types to clarify the social effects of in-game behaviors on interdependent well-being.

The Emotional Intelligence and Social Media Addiction in Communication Undergraduate Students in Turkey: The Impact of Emotional Intelligence, Demographic Variables and Social Media Use Habits on Social Media Addiction

This study’s aim is to investigate the relationship between emotional intelligence (EI) and social media addiction (SMA) in Communication undergraduate students in Turkey. In addition to EI, the impact of demographic variables and social media use habits on SMA were investigated. For the study, quantitative method was chosen, and an online questionnaire was conducted on 317 Communication undergraduate students in Turkey with 301 participants being the final sample from different cities in Turkey. As a result of the study, analyses indicated that EI and SMA were related at medium and low levels. The EI partly predicted SMA. It was determined with Path analysis that intrapersonal skills, dealing with stress and adaptability could be statistically significant predictors of SMA sub-dimensions virtual tolerance and virtual communication at a negative level, and time spent on social media, number of posts on social media could have a significant impact on students’ SMA at a positive level. This research differs from other research conducted in Turkey in terms of SMA being examined with the focus of EI.

Impact of demographic variables on consumers’ adoption of e-banking services in Ganjam District of Odisha: An empirical investigation

The primary objective of this research is to determine demographic factors that influence the adoption of internet banking services to improve the situation. As banks are spending a lot of money and time on technology to provide easy and better banking services, hence it becomes critical to examine whether technological advances improve the extent of customer satisfaction. An empirical study has been carried out with the help of 372 respondents in the district of Ganjam, Odisha, India who have access to technology-based banking services. The results of the chi-square test show that there are relationships between demographic factors and various technological banking services provided by the banks to the customers. By using factor analysis and multiple regression analysis it is also inferred from the study that the intense use of technology has negative effects on users' openness to technology-based banking services. This study has clarified many demographical factors which affect the acceptance of technological banking services among the customers in the Ganjam district and has suggested the required measures to adopt more customer-friendly e-banking services.

Quality of Life, Lifestyle Behaviors, and Well-Being in Patients With Cancer: The Impact of Demographic Variables

Background: Cancer can challenge all aspects of a person’s quality of life (QoL) and well-being, including physical, psychological, social and spiritual. QoL has long been a central question in cancer research.  Optimal QoL has become a goal in health care. It can be argued that the improvement of QoL should be a primary goal of cancer care. Lifestyle factors have been repeatedly identified as cancer risk factors.  Unhealthy lifestyle behaviors are associated with serious illnesses such as cancer. How lifestyle behaviors affect the cancer control continuum have not been well-studied. Objective: This study evaluates the relation between lifestyle behaviors, perceived quality of life, well-being and tries to expand knowledge regarding the correlation between them. Methodology: 105 oncology outpatients who have been submitted in pain therapy in the Pain Unit of an Ongological Hospital in Greece in 2021, were administrated the World Health Organization OoL instrument (WHOQOL-BREF) the WHO (10 items) Well-Being Index  and an improvised questionnaire with Lifestyle indices. The present study is a cross-sectional, descriptive, correlational research design. Results: This study suggests that male patients’ well-being scores were significantly higher. The patients also reported higher scores in the psychological and environmental domain of QoL but they scored significantly lower in the dimensions of physical health, smoking and alcohol use of Lifestyle. Older cancer patients reported lower levels on the well-being  and also lower scores on the QoL in the physical, psychological and social relationships domains. They also reported less on the lifestyle variables, particularly on physical health(p=>.05),food consumption(p=>.05), observing weight control (p=.04)and exercise and well-being(p=0.02). More educated patients reported significantly higher scores in the psychological and environment domains of QoL and presented a significantly higher scores in physical health, smoking and alcohol use of lifestyle indices indicating more healthy lifestyle patterns. Conclusions: Findings provide evidence that demographic variables such as being male, younger more educated relate to a more preserved QoL. Regarding lifestyle patterns being female, younger and more educated relate to a more healthy behaviors.

Use of next-generation sequencing (NGS) to study the impact of demographic variables on prevalence of driver mutations in non-small cell lung cancer (NSCLC).

e21030 Background: The management of NSCLC is rapidly evolving with the discovery of new driver mutations and the introduction of targeted therapies (TT). The list of targets with FDA-approved TT is estimated to cover over half of the cases of non-squamous NSCLC. The prevalence of driver mutations varies significantly depending on several demographic variables such as smoking status, race, and sex. This abstract presents data obtained from a unique, racially diverse cohort of patients in the Washington, DC metropolitan area. Methods: Retrospective review of circulating tumor DNA NGS data from 2 commercial testing companies (Guardant and Tempus) and demographic data from our health records on 66 patients with NSCLC treated at the George Washington University Medical Faculty Associates between October 2017 and November 2021 was performed. Demographic information including age, sex, race, and smoking history, as well as histology, stage at diagnosis, and driver mutations were collected. Only patients with non-squamous NSCLC and identifiable mutations were included. Results: Mean age was 67. Most patients in this cohort were female (56.1%, male 43.9%), African-American (62.1%, Caucasian 25.8%, African 9.1%, Asian 3.0%), had smoking history (former smoker 48.5%, current smoker 25.8%, never smoker 25.8%), had adenocarcinoma (78.8%, other NSCLC (not squamous cell carcinoma or adenocarcinoma) 21.2%) with advanced stage (stage 4 62.1%, stage 3 28.8%, stage 1 3.0%, unknown 6.1%). Mutations are listed as follows: KRAS 28.8% (G12C accounted for 58% of all KRAS mutations), EGFR 19.7% (exons 19 and 21 mutations such as del19 and L858R accounted for 69%), MET 9.1% (MET amplification made up 83%), Her2 3.0% (2/66), ALK 3.0% (2/66), RET 1.5% (1/66), non-targetable mutations 34.8% (23/66). A table at the end of this abstract shows the frequency of three most prevalent mutations in this cohort distinguished by race, sex, and smoking status. Conclusions: KRAS was the most prevalent driver mutation in this cohort, followed by EGFR and MET. In concordance with the existing literature, EGFR was more prevalent in non-smokers and females, while KRAS was more common among smokers. Interestingly, a larger percentage of Africans had EGFR mutations compared to African-Americans. Given the small sample size, however, a larger scale study is needed to investigate this further. These findings show that demographic factors influence the prevalence of driver mutations which can have therapeutic implications in patients with NSCLC.[Table: see text]

How well does the discrepancy between semantic and letter verbal fluency performance distinguish Alzheimer’s dementia from typical aging?

ABSTRACT In Alzheimer’s dementia (AD), greater declines in semantic fluency (SF) relative to letter fluency (LF) have been assumed to reflect semantic disintegration. However, the same pattern is observed in typical aging and neurodegenerative disorders besides AD. We examined this assumption by comparing different aspects of SF and LF performance in older adults with and without dementia, and identifying which verbal fluency measures most clearly distinguish AD from typical aging. Verbal fluency data were compared from 109 individuals with AD and 66 typically aging adults. Correct items, clusters, and errors were analyzed using both raw counts and proportions. Regression analyses examined Task-by-Group interactions and the impact of demographic variables on verbal fluency measures. ROC analyses examined the sensitivity and specificity of the different outcome measures. In regressions, interactions were found for raw but not proportional data, indicating that different group patterns were driven largely by the number of correct items produced. Similarly, in ROC analyses, raw SF totals showed stronger discriminability between groups than either raw discrepancy scores (SF–LF) or discrepancy ratios (SF/LF). Age and cognitive status (MMSE) were the strongest individual predictors of performance. Findings suggest that AD entails quantitative declines in verbal fluency, but qualitatively similar patterns of performance relative to typically aging adults. Thus, SF declines in AD seem to be at least partially attributable to an exaggeration of the underlying mechanisms common to typical aging, and do not necessarily implicate semantic disintegration.

Consumer Expectation of Flavored Water Function, Sensory Quality, and Sugar Reduction, and the Impact of Demographic Variables and Woman Consumer Segment.

This study aimed to investigate consumer expectation of flavored water and potential consumer segments. The results showed flavored water was ranked the fourth most popular drink, after plain water, tea, and coffee, by 901 participants. Consumers highly expected functional flavored water with refreshing (87.4% selection), thirst-quenching (73.7%), and tasty (65.7%) qualities, containing vitamins, minerals, and antioxidants, and providing energy. Expected flavored water sensory qualities included temperature (62.4%), flavor (52.4%), and sweet taste (47.4%); lemon, berry, and lime flavors were most preferred, while bitterness, irritation, astringency, and sourness were least preferred. Pure sugar and honey were rated highest as the sweeteners for flavored water. Likewise, consumers were mostly concerned with taste followed by calories. Single demographic variables (age, reported health condition, drinking frequency, educational level) significantly influenced (p ≤ 0.05) flavored water function, sensory quality, and sugar reduction expectations. Females had higher expectation of flavored water’s refreshing and antioxidant functions. Cluster analysis revealed two consumer segments. The younger, low-education, self-reportedly less healthy cluster (mainly college students) expected various functions and flavors such as low temperature, cooling taste, diverse flavors, and sweet taste (and disliked bitterness). The older, educated, employed, self-reportedly healthy cluster had lower expectations of flavored water functions, were less sensitive to bitterness, and preferred no sweetness or little sweetness. These findings provide informative data to establish marketing and sales strategies for promoting flavored water.

A Study of Quality of Banking Services

For maintaining competitiveness in the market, the pursuit of service quality for service providers is necessary. Customer satisfaction is necessary for acquiring customer loyalty and having better financial controls. Service quality on banks is done on several studies including USA, Canada and Tunisia, Malaysia, five Balkan countries: Albania, Greece, FRYOM, Bulgaria and Serbia and India. The research gap in the current literature is that very few studies have been done on the impact of demographic variables on the perception of service quality. The objective of this article is to examine the effect of type of bank i.e. public and private sector banks on perception of quality of banking services. The instrument used for evaluating service quality was Bank Service Quality created by Bahia and Nantel (2000). This instrument consists of 31 items which spans 6 dimensions: access; effectiveness and assurance; tangibles; price; services portfolio and reliability. In the study, perception was only measured with the items of perception only. A quantitative research approach and well-structured survey questionnaire was used in the study. Convenience sampling was used with 51 questionnaires and google forms were used for collecting the data. Data collected was analysed using SPSS version 16. The highest score for both public and private sector banks was for “effectiveness and assurance” dimension. The lowest score for public and private sector banks was for “reliability” dimension. ANOVA was used to check whether there were significant differences among public and private sector banks for six dimensions. The result shows that there are non-significant differences among perceptions of public sector and private sector banks on all six dimensions. There are nonsignificant differences on all 31 items in the case of public sector and private sector banks. The study shows that respondents of both public and private sector banks perceive that service quality of banks is not as per their expectations. The limitation of this study is that convenience sampling has been used in the study. Moreover, it would not represent the perception of service quality of banks in the whole of India. A Quantitative Study of Mathematics Identity and Achievement among LatinX Secondary School Students

ABSTRACT Using data from the National Center of Educational Statistics’ (NCES) 2009 High School Longitudinal Study, this article studies the relationship between mathematics identity and mathematics success for LatinX students by relying on critical race theory and intersectionality as theoretical frameworks. The quantitative analysis relies on hierarchical regression modeling to examine the relative impact of demographic variables, school characteristics, parents social capital, and parental involvement on the mathematics grade point averages at the conclusion of 11th grade of a national sample of LatinX students. The article concludes with a discussion of the ways in which we as educators, policy makers, and researchers can work toward supporting positive mathematics identity development and, by extension, mathematical attainment and success for LatinX students. Specifically, the article discusses ways in which the experiences, cultures and abilities of these students can be acknowledged, celebrated, and built upon.

An Analysis of Utilization of Maternal Health Services with Respect to Information Received and Impact of Demographics in Punjab, India

The value information leads to effective utilisation of services. The study identifies the gap in information about maternal health services and their utilisation. The information which has led to utilisation of services can be assiduously labelled as value information or value awareness. The relationship has been explored between awareness about maternal health services and service utilisation among Janani Suraksha Yojana (JSY) beneficiaries in Punjab, a high-performing state in India as per NRHM. Further, an assessment of impact of demographics on beneficiaries with information and those who utilised the services has been explored. Information received and utilisation of ante-natal checkups (ANC), delivery care and post-natal checkups (PNC) services have been studied for two districts in the state of Punjab, India. Chi-square test has been applied to know the association between the maternal health services and the select districts with respect to information and services utilised. Impact of demographic variables such as age, education, income and caste have been evaluated for information and utilisation of maternal health services. The dispersed information that has not triggered in utilisation of services by JSY beneficiaries have been used as an ingredient to explore and understand the deterring factors in non-utilisation of maternal care services in the rural areas of developing country like India. Results highlight that a significant association exists between the utilisation of maternal health services among the select districts in the state of Punjab ( χ2 = 8.73, p-value = 0.0127); however, there is no association between the informed beneficiaries and the districts for the maternal health services. No impact of demographic variables has been found on the beneficiaries with information about maternal health services and those who utilised the services. The gap in awareness and utilisation of services throws light on the lurking concerns of unrecognised benefits, uncaring attitude towards health, prejudices about institutional delivery and PNC being considered an unimportant service among the beneficiaries.

Demographic variables and their impact on the level of job performance of employees of the Ministry of Labor in the Sultanate of Oman: المتغيرات الديمغرافية وأثرها على مستوى الأداء الوظيفي لدى موظفي وزارة العمل في سلطنة عُمان

The current study aimed to identify the reality of job performance and the impact of demographic variables on the level of job performance among employees of the Ministry of Labor in the Sultanate of Oman. The study has followed the descriptive analytical approach, and the questionnaire was selected as an essential means of data collection. The (337) questionnaires were distributed to the study sample from various administrative divisions of the Ministry, and the data was analyzed by the Statistical Package of Social sciences (SPSS). The results of the study revealed that the level of job performance among employees of the Ministry of Labor in Oman is very high. The results also showed that there are no statistically significant differences between the following variables (age, scientific qualification, career level, and number of years of service) and level of job performance, except the gender variable, where statistical differences were in favour of the male sample.

This study aims to investigate the relationship between delegation and organizational loyalty of workers in the General Administration of Islamic Tadamon Bank, and find out the impact of demographic variables, represented by: gender, academic qualifications, years of service, and training courses on delegation& organizational loyalty. To achieve the objectives of the study and testing of hypotheses, a special questionnaire were developed to gather data from a sample, which contained (99) employees, and the number of questionnaires were subject to analysis (85), representing (85.9%) of the total questionnaires distributed. The study found a strong positive correlation between delegation and organizational loyalty which is statistically significant at the level of significance of (0.01), and show a significant impact statistically significant to the independent variable of delegation on the dependent variable which is the organizational loyalty, as well as nonexistence the of statistically significant differences between the views of members of the research sample towards delegation and organizational loyalty attributable to the variables of: gender, academic qualification, years of service, and training courses the level of significance values which were calculated for variables, were larger than the tabular value (0.05).
