Endoscopic Submucosal Dissection for a Giant Well-Differentiated Liposarcoma Originated from Hypopharynx.

Hypopharyngeal liposarcoma is an extremely rare entity, and yet fewer than 40 cases have been reported in the literature. Lacking the possibility of distant metastases, local surgical resection with a clear margin can achieve the purpose of treatment. Endoscopic submucosal dissection has been widely used in the treatment of early upper gastrointestinal cancer and superficial hypopharyngeal cancer. Here, we present a case of a giant tumor originated from the hypopharynx that received primary resection by endoscopic submucosal dissection using a flexible upper gastroscope.

Liposarcoma is the most common form of soft-tissue sarcoma, only 3-8% have a head and neck location. They are commonly found in the retroperitoneum and on the limbs, laryngeal and hypopharyngeal presentation are extremely rare and less than 40 cases have been reported in literature. We present the case of a 50-year-old man with no prior history, who developed two contiguous hypopharyngeal liposarcomas.

Introduction: Liposarcomas are infrequent tumors. Only 5.6% of these tumors occur in the head and neck. Primary liposarcomas of the larynx comprise ∼10% to 15% of head and neck liposarcomas. Multiple surgeries and recurrences history are common. Case summary: We present the case of a 33-year-old male whose illness started at 16 years of age, and who underwent multiple surgeries due to multiple recurrence (six) by a liposarcoma in the neck, all treated with organ preservation. In 2014 patient arrived with a big recurrence a total pharyngolaryngectomy with a jejunal are flap was performed. after 18 month of the follow up the patient comes to the visits and refered developed headache, ataxia and shortness of breath, that is why a MRI of the brain and CT of the torax shown intraxial lesions in the brain and multiple bilateral pulmonary nodules compatible with metastatic disease, We propose palliative treatment with radiotherapy but the patient refuse treatment so he was sent to palliative care. Discussion: The mainstay of treatment is surgical resection. Multiple recurrence is the rule but metastasis to the brain was not reported before. The role of radiotherapy is not well know. Chemotherapy until now showed no sustancial benefit.

Liposarcoma of Hypopharynx and Esophagus: a Unique Entity?

Liposarcoma is the most common soft tissue sarcoma in adults. It represents approximately 20% of all mesenchymal malignancies. It most frequently involves retroperitoneum, trunk, and extremities. Hypopharyngeal and esophageal localization of liposarcoma is extremely rare. We performed a systematic review of literature and reported 26 and 33 cases of hypopharyngeal and esophageal liposarcoma. We analyzed natural history, imaging features, histology, treatment, and prognosis, with a specific focus to similarities and differences between tumors of hypopharynx and esophagus. Hypopharyngeal and esophageal liposarcomas have more similarities than differences. Incidence has a peak at 6th and 7th decades. The diagnostic procedures are barium swallow, endoscopic examination, and CT/MR imaging. Well-differentiated liposarcoma represents the most frequent histological subtype. Surgical excision is the main treatment. Endoscopic resection can be useful for pedunculated tumors of hypopharynx and cervical esophagus. Differences between hypopharyngeal and esophageal liposarcoma are represented by local recurrence rate (greater for hypopharyngeal tumors), number of giant tumors, and time to recurrence (greater for esophageal tumors). Finally, liposarcomas of distal esophagus need more extended approaches. Liposarcomas of hypopharynx and cervical esophagus could be considered a unique pathological entity, with similar features and treatment options. Survival rate is dependent on histological type and location. Local recurrence is common, especially for hypopharyngeal liposarcoma, while the risk of lymph node or distant metastasis is very low. Patients should undergo regular examinations to rule out local recurrence, also for a long time, especially for esophageal tumors.

Laryngeal Liposarcoma: Report of a Case

Laryngeal and hypopharyngeal liposarcomas are fairly rare tumors, with only 30 convincing cases reported to date. These tumors usually arise in the supraglottic area, and only two cases have been reported to affect the true vocal cord. They behave in an indolent fashion with multiple local recurrences and only rarely cause the patient's death. Our case highlights the natural history of this entity. A 62-year-old man presented because of air-way obstruction. A CT scan discovered a large 5 cm polypoid mass in the right aryepiglottic fold. The patient had already undergone three previous operations for the same reason in another clinic, with histopathological diagnoses of fibrovascular polyps. In the last recurrence, only a careful search for lipoblasts in the surgical specimen allowed us to identify this lesion as a low-grade well-differentiated liposarcoma. Complete resection was impossible in this case, despite total laryngectomy.

Liposarcomas of the larynx and hypopharynx: A clinicopathologic study of eight new cases and a review of the literature

Laryngeal and hypopharyngeal liposarcomas were studied in seven men and one woman. Patient age ranged from 25 to 81 years, with a median of 64 years. Symptoms included dysphagia, airway obstruction, and the sensation of a foreign body in the back of the throat. Histologically, seven of the tumors were of the biologically favorable types, either representing well-differentiated (lipoma-like) liposarcomas or myxoid liposarcomas. One tumor was a pleomorphic liposarcoma. Six of the eight patients had one or more episodes of recurrent tumor. Surgery is the treatment of choice and can include conservative (organ-sparing) procedures. However, to eradicate the tumor completely and thereby prevent recurrent disease, open surgical approaches (i.e., lateral pharyngotomy), rather than endoscopic techniques should be employed. The morbidity rate for laryngeal and hypopharyngeal liposarcomas is high because these tumors tend to recur over extended periods of time. In this study, however, there were no instances of metastatic disease, and no deaths were attributed to liposarcoma. Prospectively, laryngeal and hypopharyngeal well-differentiated (lipoma-like) liposarcoma is a difficult clinical and histopathologic diagnosis to establish. Often, this diagnosis is made only after one or more episodes of recurrent disease.

Laryngeal and Hypopharyngeal Liposarcoma A Clinicopathologic Study of 10 Cases with a Comparison to Soft-Tissue Counterparts

We report the clinicopathologic findings of 10 cases of primary laryngeal and hypopharyngeal liposarcoma. The tumors occurred in patients ranging in age from 37 to 77 years. They showed a marked male to female predominance of 9 to 1. The tumors, which occurred exclusively in the supraglottic larynx or hypopharynx (pyriform sinus), caused airway obstruction; this was the most common presenting symptom. All of the cases were well-differentiated liposarcomas (grade I). Atypical cells, scattered lipoblasts, and infiltration differentiated malignant laryngeal fatty tumors from benign ones. To date, none of these tumors has metastasized; however, multiple recurrences are common. Surgery is the treatment of choice; radiotherapy should be decided on an individual basis.
