Development of a multivariable clinical prediction model for liposomal doxorubicin-induced cardiotoxicity in adult breast cancer patients: a retrospective multicenter study

BackgroundThe clinical use of anthracyclines is limited by the risk of cardiotoxicity. So, we aim to develop a clinical prediction model for liposomal doxorubicin-induced cardiotoxicity in adult breast cancer patients.MethodsWe designed a multicenter retrospective cohort study. A total of 257 hospitalized breast cancer patients treated with doxorubicin liposomes were finally enrolled in the study, including 58 patients from Beijing Friendship Hospital and 199 from Beijing Cancer Hospital. In all, 32 cases developed cardiotoxicity, including 4 at the Beijing Friendship Hospital and 28 at the Beijing Cancer Hospital. The study involved breast cancer patients with no pre-existing heart disease, whose clinical data were collected from their medical records. All patients underwent electrocardiogram (ECG) and/or left ventricular ejection fraction (LVEF) measurements prior to treatment with doxorubicin liposomes. Patients were clinically assessed after each cycle of treatment, and ECG and/or LVEF measurements were performed at least once after treatment. Liposomal doxorubicin-induced cardiotoxicity was defined when one of the following three conditions was met: (I) a reduction in LVEF of at least 5% from the baseline and the absolute value was less than 55%, accompanied by congestive heart failure (CHF) symptoms or signs; (II) a reduction in LVEF of at least 10% to an absolute value of less than 55%, without CHF symptoms or signs; (III) the definite diagnosis of CHF. Variables associated with cardiotoxicity were identified by univariate and multivariate logistic regression, and the consistency and differentiation of the final model were evaluated.ResultsIn our final model, age [odds ratio (OR): 5.626, 95% confidence interval (CI): 2.321 to 13.639], cancer metastasis (OR: 3.873, 95% CI: 1.220 to 12.299), paclitaxel (OR: 3.601, 95% CI: 1.010 to 12.843), and hypertension (OR: 2.435, 95% CI: 1.046 to 5.671) were significantly associated with cardiotoxicity. The final model was tested for Hosmer-Lemeshow goodness-of-fit, the χ2 was 2.696 and the P value was 0.747, and the resultant predictive model had an area under the receiver operating characteristic (ROC; AUC) curve of 0.781.ConclusionsThis study established a risk prediction model for liposomal doxorubicin-induced cardiotoxicity in breast cancer patients and performed a stratified risk scores

Association between breast cancer and hepatitis C: A joint study of hospitalized patients and nationwide cohorts

Whether hepatitis C virus (HCV) infection is associated with breast cancer risk remains elusive, and we aimed to elucidate it. A nationwide population-based cohort study of the Taiwan National Health Insurance Research Database (TNHIRD) was conducted. Additionally, breast cancer risk factors, and HCV core expression were surveyed in breast cancer patients of a tertiary care center. Three TNHIRD cohorts (1:4:4, propensity score-matched, 2003-2012), including HCV-treated (3646 HCV-infected females with interferon-based therapy ≥6 months), HCV-untreated (n=14,584) and HCV-uninfected (n=14,584) cohorts, were enrolled. The HCV-untreated cohort had the highest 9-year breast cancer cumulative incidence (2.017%; 95% confidence interval [CI]: 1.382%-2.846%), while the HCV-treated (1.073%; 0.414%-2.356%), and HCV-uninfected (1.453%; 0.785%-2.486%) cohorts showed no difference. Untreated HCV infection (hazard ratio [HR]: 1.701; 95% CI: 1.205%-2.400), urban residency (1.658, 1.183-2.323), and baseline cardiovascular events (1.920; 1.005-3.668) were associated with incident breast cancers. The interaction analysis showed that particularly among patients <49 years, HCV infection was associated with breast cancer development (2.193; 1.097-4.384). Of 12,170 hospitalized breast cancer patients, 4.90% were HCV Ab-positive. HCV Ab-positive patients were older (60.92+/-10.82 vs 53.91+/-11.38 years, P < 0.0001) and had a higher body mass index (25.39+/-5.1 vs 24.5+/-4.3 kg/m2, P = 0.007), rates of diabetes (30.60 vs 19.98%, P < 0.0001), hypertension (46.9 vs 30.39%, P < 0.0001), dyslipidemia (25.52 vs 20.28%, P = 0.031), and hyperuricemia (11.38 vs 5.52%, P < 0.0001) than their counterparts. No HCV core-positive cells were demonstrated in breast cancer tissues. Conclusions: Untreated HCV infection, urbanization, and cardiovascular events were potential risk factors for breast cancer. The HCV-associated risk was most prominent among patients <49 years, might not be associated with in situ HCV core-related oncogenesis but with metabolic alterations, and was reversed by anti-HCV therapy.

Chemotherapy-induced neutropenia among breast Cancer patients in a tertiary care hospital: Risk and consequences.

To identify the risk factors that may predispose breast cancer patients to Chemotherapy-induced neutropenia (CIN) and its associated complications for the years 2018 and 2020. CIN is an established complication of breast cancer treatment. Clinical Pharmacists can play an important role in the treatment of CIN through involvement in risk assessment to identify patients for oral antimicrobial therapy, drug therapy monitoring, and development of suitable guidelines or policies. A retrospective study was performed by collecting data of 72 breast cancer patients for the last two years from department of Medical Oncology in a tertiary care hospital. The overall occurrence of CIN was 59.7% in our study population. Out of 72 patients studied, 43 patients were found to be neutropenic. Using Pearson Chi square test, chemotherapy-induced neutropenia was associated with older age (over 60 y) (p < 0.038), diabetes mellitus (p < 0.050), tumour stage IIIa (p < 0.024), AC (p < 0.051) and taxane chemotherapy regimens (p < 0.041). Febrile neutropenia occurred in 37.28% of patients and the incidence of infection-related mortality [severe septicaemia] was 3.38%. The incorporation of clinical pharmacist must be brought into practice in our country for providing proper guidance to the patient on CIN and its complications. By identifying risk factors for neutropenia, the safe management of CIN may be possible in patients with breast cancer.

Neutrophil-lymphocyte Ratio as Independent Prognostic Factor among Breast Cancer Patients in a Tertiary Care Hospital, Kolkata, India

Introduction: The most important prognostic factor for breast cancer is tumour stage and tumour grade. However, the assessment of the above parameters are time-consuming and require expertise. Thus evaluation of the prognosis of breast cancer is still limited to tertiary care hospitals, with appropriate facilities for histopathological techniques. Recently, inflammatory blood markers have shown a role as a prognostic factor. Out of all the inflammatory blood markers, neutrophil-lymphocyte ratio has emerged as the most useful. Abundant evidence suggests the role of NLR as an adverse prognostic factor in breast cancer. NLR is simple and inexpensive. It can be easily obtained, as the differential count of every patient is done routinely. Thus it can act as an indicator of high-risk patients who are likely to show poor prognosis. Though NLR has been found to play a role in prognosis prediction in breast cancer, much is unknown in this field. Aim: To assess the effectiveness of Neutrophil-Lymphocyte Ratio (NLR) as an independent prognostic parameter among breast cancer patients in a tertiary care hospital in Kolkata, India. Materials and Methods: This observational cross-sectional study was conducted in Department of Pathology, Nil Ratan Sircar Medical College and Hospital, Kolkata, West Bengal, India, where 140 female patients undergoing mastectomy for breast cancer were studied from 1st February 2019 to 31st January 2020. The clinicopathological parameters, histopathological parameters, and molecular subtypes were evaluated. NLR was calculated and related with the other prognostic parameters. Data entry was done in Microsoft Excel and analysis was done using Statistical Package for Social Sciences (SPSS) software version 20.0. One way Analysis of Variance (ANOVA) test and Chi-square test was conducted to assess the relationship of NLR with various other prognostic factors. Results: Out of 140 patients, 78 patients showed NLR values within 1.8 to 3.33. Higher NLR (&gt;3.33) was associated with poor prognostic factors like higher T stage (T4) {17 (53.1%)}, higher stage (stage III) 95 (67.8%), skin involvement 19 (47.5%), Lymphovascular involvement in 23 (39.7%), perineural involvement in 10 (71.4%) and in patients with HER2 positive molecular subtype in 8 (5.7%). Conclusion: This study suggests that a high NLR value was associated with poor prognosis in breast cancer patients. Thus, it can be used as an independent marker of poor prognosis and can help guide the treatment of breast cancer patients. The more we study the role of NLR, the more useful it will be in predicting the course of breast cancer as early as possible.

Analysis of Influencing Factors on Hospitalization Expenses of Patients with Breast Malignant Tumor Undergoing Surgery: Based on the Neural Network and Support Vector Machine.

Objective Analyze the influencing factors of hospitalization expenses of breast cancer patients in a tertiary hospital in Chengdu and provide a basis and suggestion for controlling the unreasonable increase of medical expenses. Methods The first pages of all inpatient medical records of patients with breast malignant tumor from 2017 to 2020 were extracted, and the descriptive analysis, single-factor analysis, and multifactor analysis were conducted by using the statistical method and data mining method to explore the influencing factors of hospitalization expenses. Results In 2017–2020, the average hospitalization cost and the average surgical treatment cost increased year by year, and the number of operations, actual hospitalization days, and CCI were the important influencing factors. Conclusion It is suggested to strengthen the supervision of medical rationality and eliminate the waste of medical resources; and we should improve the efficiency of diagnosis and treatment services, so as to shorten the actual length of hospitalization; at the same time, the combination of DRG grouping and fine management can be used to control the hospitalization expenses.

Axillary lymph node status according to location, size, number and grading among breast cancer patients in tertiary care hospitals, Bangladesh

Background: There are a good number of tumor-related features available to predict the prognosis of breast cancer and its stage of severity. Axillary lymph node status according to the location, size, number, and grading among breast cancer patients have a potential prognostic value in the course of breast cancer treatment, mortality, and morbidity. We have very limited researched-based information regarding these issues. Aim of the study: The aim of this study was to assess the axillary LN status according to location and grade of tumor also size and number of axillary LN among breast cancer patients. Methods: This was axillary LN status according to the location of a prospective observational study conducted in the Department of Surgery &amp; Oncology, Dhaka Medical College Hospital, BSMMU, and NICRH Dhaka, Bangladesh during the period from January 2015 to December 2016. In total 100 patients of several age groups with breast cancer, patients attended, diagnosed, and treated in the mentioned hospital were selected as the study subjects. Proper clinical examination and ultrasonography for the assessment of axillary lymph node status were done for every participant. All data were processed, analyzed, and disseminated by MS Office and SPSS program as per need. Results: In location analysis, we observed, most of the tumors were located in the upper outer quadrant (57%) followed by 22.0%, 17.0%, and 4.0% were located in the upper inner quadrant, lower outer quadrant, and in other locations respectively. Most of the tumors were of Grade II (67%) followed by 21.0% and 12.0% were of grade I and grade III respectively. Maximum 52.5% patients of this study had a lymph node with the size of 1-2 cm followed by 25.0% and 22.5% patients had lymph nodes with the size of &gt;2cm and 9. Both clinical examination and USG showed positive results in increasing the number of lymph nodes. Conclusion: In analyzing the location, size, number, and grading of axillary lymph node among breast cancer patients by using clinical examination and USG findings, we found statistically significant correlations in the size and number of the lymph node. So, we can conclude that the location &amp; grade of tumor and number &amp; size of axillary lymph node among breast cancer patients are the most potent indicators of severity and status of breast cancer.

Exploring the employment readiness and return to work status of breast cancer patients and related factors

ObjectiveTo investigate the employment status, employment readiness, and other factors affecting the ease or difficulty with which breast cancer patients effect their return to work (RTW). MethodsThis study adopted a mixed-method design, recruiting participants from among breast cancer patients in a cancer hospital in Hunan from December 2018 to June 2019. We approached 300 individuals, 192 of whom ultimately participated in this study. The quantitative part of the study involved several scales: the Patient Health Questionnaire-9 (PHQ-9), the Brief Fatigue Inventory (BFI), the Work Ability Index (WAI), and the Lam Assessment of Employment Readiness (LASER). The qualitative part involved a set of open-ended questions and written responses collected from 41 participants who had already returned to work at the time of data collection. Their written responses mainly concerned factors influencing RTW. ResultsForty-one breast cancer patients had returned to work. The results reported a median total Cognitive Symptom Checklist score of 9.00 (6.00, 15.25), a median WAI score of 5.00 (3.50, 9.75), a median BFI score of 26.00 (14.75, 42.00), a median total PHQ-9 score of 8.00 (5.25, 17.00), and a LASER score of 50.35 ± 11.90. Multiple regression analysis showed that the participants’ cancer stage, cognitive limitations, depression, fatigue, and work ability were significant predictors of employment readiness (P < 0.05). Exploring the qualitative data, we found that higher skill levels, better social support, and a flexible work schedule facilitated RTW; stress, lack of confidence in one’s work skills, depression, and fatigue are all possible barriers to RTW. ConclusionThe findings indicate that breast cancer patients have a low level of employment readiness. Nurses and other healthcare providers can develop relevant interventions to promote employment readiness and ultimately achieve RTW in this study population.

The Impact of Socioeconomic Status on the Psychological Distress of Breast Cancer Patients in Kolkata"

Background: Breast cancer patients experience a variety of psychological symptoms such as anxiety, depression that affect their quality of life. The present study probed whether the socioeconomic status had any significance on psychological distress and quality of life of breast cancer patients in India.Method: This was a cross-sectional and observational study, conducted over 111 breast cancer patients in a tertiary cancer hospital, Kolkata. Subjects were eligible to participate and they were of over 23rd years of age and had a histologically confirmed diagnosis of breast cancer had no history of recurrence or metastasis, and had no previous psychological problems. Age, marital status, residence, education, occupation and family income were assesses for patients’ socio economic status. Psychological symptoms and quality of life were assessed by using validated tools. Means and standard deviations of each outcome were compared by socioeconomic status and multivariate linear regression models for evaluating the association between socioeconomic status, psychological distress and quality of lifeResults: The patient group was categorized into three groups based on their income level as follows: ≤500=27.9%), 500-1000=51.4% and high income = ≥1000=20.7%. A total of 111 breast cancer patients participated in the study; the mean age of participants was 49.7 (SE=1.31).After using multivariate logistic regression with fully adjusted models, the patients who were living alone had significantly higher level of depression and poor quality of life compared to those who are employed. Higher education and family income both positively associated with quality of life after adjusting for age, marital status and occupation.Conclusion: The findings revealed that the family income have a significant impact on the psychological distress of the cancer patients.

A Dance Program to Manage a Fatigue-Sleep Disturbance-Depression Symptom Cluster among Breast Cancer Patients Receiving Adjuvant Chemotherapy: A Feasibility Study

Objective:Fatigue, sleep disturbance, and depression (FSD) are highly prevalent among breast cancer (BC) patients undergoing adjuvant chemotherapy, constituting a common symptom cluster (i.e., FSD cluster). Exercise is effective in relieving fatigue, alleviating sleep disturbance, and improving the quality of life (QoL) during adjuvant chemotherapy among these women. Therefore, this study aimed to assess the feasibility and acceptability of a dance program with social support elements for managing this symptom cluster.Methods:A prospective randomized controlled design combined with a semi-structured interview was utilized. Twenty consented eligible participants were randomly assigned to a dance group or an attention control group after baseline assessment but before chemotherapy. The feasibility of recruitment, retention, intervention, and data collection procedures and the acceptability of exercise modality, hospital-based instruction, and home-based practice were collected by retrieving information from research documents and interviews by the principal investigator 4 weeks after the start of intervention.Results:The recruitment and retention rates were 86.96% and 100.00%, respectively. Participants took about 30–40 min to complete the questionnaire. All the items were comprehensible. The baseline characteristics were comparable between groups, indicating that randomization was successful. The implementation procedure progressed smoothly. Dancing was considered interesting and easy, and participants would like to practice at home. Only minor adjustments would be needed for future studies.Conclusions:This study demonstrated that the proposed dance program was feasible and acceptable for BC patients in hospital and home settings. A full-scale study is warranted to examine its effects on managing the FSD cluster and promoting QoL.

Chemotherapy induced neutropenia and febrile neutropenia among breast cancer patients in a tertiary hospital in Nigeria

PurposeThis study assessed the incidence of chemotherapy-induced neutropenia and febrile neutropenia (FN) while identifying their associated factors.MethodsA prospective cross-sectional study was conducted among 113 female chemotherapy-naïve breast cancer patients over a 2-year period. Socio-demographic, clinical and haematological data were obtained via semi-structured interviews and from medical case files. Blood samples for complete blood count parameters were collected 2 weeks after each course of chemotherapy. The National Cancer Institute Common Terminology Criteria for Adverse Events version 4.03 was used to assess FN, neutropenia and their severity.ResultsThe incidence of neutropenia and FN among the patients was 31.9% and 5.3%, respectively. Throughout all courses of chemotherapy (n = 502), there were 57 (11.4%) neutropenic episodes with 6.6% mild, 3.4% moderate and 1.4% severe neutropenia. The incidence of neutropenia decreased with increasing chemotherapy courses, with a rate of 14.2% and 4.9% after the first and last course, respectively. Factors associated with the risk of developing neutropenia include increasing age (p = 0.014), Eastern Cooperative Oncology Group performance score ≥ 1 at presentation (p = 0.033) and presence of bone metastasis (p = 0.002).ConclusionOne in three breast cancer patients in this study developed neutropenia while on chemotherapy but no independent risk factors were identified for FN among these patients. This study has, therefore, provided the preliminary data necessary for further independent validation of the identified risk factors for FN in a more robust and well-designed study within our clinical practice setting in Nigeria.

To Determine the Prevalence of Obesity in Breast Cancer Patients

Background: Breast Cancer is one of the leading causes of death worldwide. Pakistan alone has the highest rate of Breast Cancer than any other Asian country as approximately 90000 new cases are diagnosed every year out of which 40000 dies. Obesity is also a critical public health problem growing with every passing year in Pakistan and worldwide. Research studies are being conducted worldwide regarding the relation between the two problems. Objective: The objective of this study is to determine the prevalence of obesity in breast cancer patients in a tertiary care hospital in Karachi, Pakistan. Methods: BMI is used as a screening tool for overweight and obesity. According to World Health Organization, a body mass index (BMI) over 25 is considered overweight, and over 30 is obese. A retrospective analysis of 262 patients diagnosed with Breast Cancer during 2019 and 2020 was performed. Patients’ hospital records in Oncology were reviewed. The weight in kilograms and height in centimeters of patients were reviewed. Their BMI was calculated and recorded using the SPSS system. Results: The median BMI was 28.25 kg/m2 with an interquartile range of 25.15 - 31.99 kg/m2. Nearly 80% of the study participants had body mass index higher than normal levels (n=203, 77.5%) and out of them approximately half were overweight (n=104, 51.2%) and the remaining were obese (n=99, 48.7%). Conclusion: We conclude from our study that body mass index is positively correlated with breast cancer occurrence and thus the proportion of females having BMI &gt;= 25 was significantly higher among patients.

Targeted genome sequencing data of young women breast cancer patients in Cipto Mangunkusumo national hospital, Jakarta.

Breast cancer is the most common cancer in women, accounting for approximately 25% of all cancer cases worldwide. Some breast cancer patients are genetically predisposed to genes involved in genomic stability. We report the targeted genome sequencing data of 24 young women (aged below 45 years) breast cancer patients admitted to Cipto Mangunkusumo National Hospital, Jakarta, Indonesia. These data will be useful in detecting the genome markers of breast cancer and in deciding the diagnostics and therapies. DNA sequences were obtained using the Illumina NextSeq 500 platform. Anxiety, depression and perceived stress among breast cancer patients: single institute experience

BackgroundStatistics reveal that the number of women diagnosed with breast cancer is increasing in Egypt. It is seen as a terrifying disease due to its high mortality rate and its impacts on the self-image and the sexual relationship. Many of its patients experience psychological reactions and may have psychiatric morbidities. The present study aimed to identify the prevalence and associated psychosocial factors of anxiety, depressive disorders, and perceived stress among breast cancer patients in Menoufia university hospitals. This cross-sectional study was conducted in the Clinical Oncology Department, Menoufia University. Sixty patients were subjected to questionnaires for socio-demographic data, structured psychiatric clinical interview to screen for psychiatric diagnoses, Beck Depression Inventory (BDI-II) for measuring the emotional, cognitive and motivational symptoms of depression, Manifest Anxiety Scale to assess the degree of anxiety, and Perceived Stress Scale (PSS-10) to assess stress level.ResultsThe prevalence of depressive symptoms, anxiety symptoms, and perceived stress were 68.6%, 73.3%, and 78.1% respectively. Moderate to severe anxiety, depression, and stress were more prevalent among advanced disease patients, patients who underwent surgery, married patients, patients who were living in rural areas, illiterate, and those without satisfactory income but without statistically significant difference except for the effect of occupation on the anxiety state as unemployed patients had significantly higher prevalence of moderate to severe anxiety (100%) than employed patients p = 0.003.ConclusionDepressive disorders, anxiety, and perceived stress are common psychiatric disorders among the studied breast cancer patients. Understanding these common psychiatric disorders and associated stress can help to plan for their management.

Impact of posttraumatic growth on the quality of life in woman with breast cancer

Cancer is an abnormal cell condition that can grow faster than usual condition which makes it difficult to control. Posttraumatic Growth is a reactive psychopathological response to traumatic events. Someone who has been diagnosed with breast cancer will experience physical, psychological, and social change as well as difficulties in both sexual and daily activities. This situation is going to be affect the quality of life of patients. This study aims to look at the relationship of posttraumatic growth and the quality of life of breast cancer patients in Makassar general hospital (RSUD). This research was quantitative research with an observational analytic approach and cross-sectional study design. The research site was in RSUD Makassar. The sampling technique used was a total sampling technique in which 34 respondents were selected. The Chi-Square test results confirmed that there were significant influences of factors such as social relationship new opportunities internal strength spiritual development the expected life value and posttraumatic growth on the quality of life of patients with Ca. Mammae. Therefore, for further research in the similar scope, it is expected for future researchers to conduct research in diverse cultural settings or by adding other psychological perspectives to be discussed in the research.

Abstract P2-12-13: Patient reported outcome measures in breast cancer patients in a Brazilian private hospital

Abstract Introduction: Breast cancer is the leading cause of death from cancer among women with a high incidence in Southern Brazil (crude rate estimated at 114.25/100,000 for 2018). Since survival of patients with breast cancer is improving all over the world, patient reported outcomes (PROs) are of great importance for evaluating treatment decisions and outcomes. The International Consortium for Health Outcomes Measurement (ICHOM) developed standard sets of health outcome measures for specific medical conditions and breast cancer was among the first conditions covered. We recently adopted ICHOM in our institution as a standard set of health outcome measures for several medical conditions, including breast cancer. This study aimed to add knowledge on PROMs within a Brazilian breast cancer population sample from a private hospital. Material and methods: All participants were recruited prospectively and asked to complete the PROMs as proposed in the international ICHOM breast cancer outcome set after diagnosis and before treatment of breast cancer. The generic PROM EORTC-QLQ-C30 and the disease specific-PROM EORTC-QLQ-BR23 modules were applied. Scores range from 0 to 100; higher scores represent a higher quality of life, except for ‘Breast symptoms’/‘Arm symptoms’, ‘Pain’ and ‘Fatigue’.Normative scores of the EORTC-QLQ-C30 and references scores for the EORTC-QLQ-BR23 were used. Results: We obtained data from the first 164 patients who entered the ICHOM. Median age was 55.8 years; 94% were Caucasian, 66% married, 17% premonopausal and 16% had in situ ductal carcinoma and 84% had invasive carcinoma. Depression was present at baseline in 17% of patients. The worst scores in QLQ-BR23 were related to sexual function and to the perspectives patients had related to their future (Table 1). Emotional function was the lowest in QLQ-C30 (Table 1). Discussion and conclusion: Breast cancer diagnosis is associated with emotional, physical and social distress. ICHOM is a tool to guide treatment and discuss quality and functioning throughout care by the multidisciplinary health team. We found a low score for sexual function among our patients after the diagnosis of breast cancer (before the initiation of oncologic treatment). This drives our attention to establish medical and psychological interventions before starting treatment, since surgery, chemotherapy and hormonal therapy may worsen all aspects of sexuality. It is important to note that these results are initial and after future prospective analyzes we may determine the influence of treatment on quality of life scores. Table 1. Anthracycline Chemotherapy in Treatuing Advanced Breast Cancer and its effect on Estradiol and Tumor Size

To investigate the therapeutic effect of anthracycline chemotherapy in advanced breast cancer and its influence on estradiol and tumor size, 136 breast cancer patients in Shan Xi Bethune Hospital were divided into anthracycline chemotherapy group and non anthracycline chemotherapy group. The clinical effects of anthracycline chemotherapy group and non-anthracycline chemotherapy group were observed. The levels of estrone, estradiol and follicle stimulating hormone before and after treatment were measured. The tumor size, adverse reactions and 2-y survival rate of anthracycline chemotherapy group and non anthracycline chemotherapy group were measured after 1-3 courses of treatment. The total effective rate of anthracycline chemotherapy group and non-anthracycline chemotherapy group was 64.7 and 23.5 %, respectively and the total effective rate of non anthracycline chemotherapy group was significantly lower than that of anthracycline chemotherapy group (p 0.05). After treatment, the tumor volume of each group gradually decreased with increased time of drug exposure and the degree of tumor reduction in the non-anthracycline chemotherapy group was lower than that of the anthracycline chemotherapy group (p 0.05). After 2 y of follow-up, the survival rate of anthracycline chemotherapy group was significantly higher than that of non-anthracycline chemotherapy group (p<0.05). The effect of anthracycline on advanced breast cancer was better, the level of serum estradiol decreased, and the volume of tumor decreased.

Validity of distress thermometer for screening of anxiety and depression in family caregivers of Chinese breast cancer patients receiving postoperative chemotherapy

ObjectiveFamily caregivers (FCs) of breast cancer patients play a vital role throughout the treatment process. Psychological distress of FCs is common and often ignored. A simple and effective instrument for screening psychological distress would help in selecting those FCs requiring special attention and intervention. Here, the validity of distress thermometer (DT) in FCs of Chinese breast cancer patients receiving postoperative chemotherapy was assessed, and the prevalence of anxiety and depression was evaluated.MethodsWe recruited 200 FCs of hospitalized breast cancer patients in this cross-sectional descriptive study. Before the first cycle of adjuvant chemotherapy, the levels of anxiety and depression among FCs were assessed using DT and Hospital Anxiety and Depression Scale (HADS). In total, 191 valid cases were analyzed. HADS was used as the diagnostic standard to assess the effectiveness of DT as a screening tool for anxiety and depression as well as to analyze the diagnostic efficiency of DT at various cutoff points.ResultsThe definitive prevalence of both anxiety and depression was 8.90%. The mean level of anxiety and depression among FCs was 5.64±3.69 and 5.09±3.85, respectively, both of which were significantly higher than corresponding Chinese norms (P<0.01). The areas under receiver operating characteristic curves of DT for the diagnoses of FCs’ anxiety and depression were 0.904 and 0.885, respectively. A cutoff value of 5 produced the best diagnostic effects of DT for anxiety and depression.ConclusionsThe levels of both anxiety and depression were higher in the FCs of Chinese breast cancer patients receiving postoperative chemotherapy than the national norm. DT might be an effective tool to initially screen psychological distress among FCs. This process could be integrated into the palliative care of breast cancer patients and warrant further research.

191P - Imaging-proven venous thromboembolism among breast cancer patients in a tertiary hospital in the Philippines from 2010-2015

Background: Venous thromboembolism (VTE) is a common cause of mortality in cancer patients. This paper looks into prevalent characteristic of breast cancer in association with VTE, if any. Methods: Hospital patient database were gathered from 2010-2015 with Venous Dupplex Scan or Computed Tomographic Pulmonary Angiography. Each result were reviewed for final conclusion bearing acute pulmonary embolism or acute thrombosis. Only the first VTE event after the diagnosis of cancer was considered. Results: There were 10,380 CTPA/Venous duplex scan performed for the year 2010-2015, where 916 (8.8%) had positive venous thromboembolism. Of the patients who had positive for venous thromboembolism, 208 (2%) had malignancy. Among all the malignancy-associated venous, majority 17.9% (n = 37) had breast cancer. Mean age for Breast cancer patients who had positive VTE was 60.9(46-84) with majority 64.8% having normal BMI. Most had VTE of lower extremities 75.67%, presented with swelling of extremities 48.6%. Twenty-one patients (57.7%) had VTE within one year of cancer diagnosis, 8(21.6%) had VTE more than two years from cancer diagnosis. Sixteen (43.2%) were alive for more than two years from VTE diagnosis, 14 (37.8%) died within 3 months to one years from VTE diagnosis. During the time of VTE diagnosis, active cancer management were chemotherapy 17 (45.9%), hormonal therapy 12 (32.4%), surgery 4(10.8%), while 4(10.8%) had VTE as the presenting symptom of cancer diagnosis. Among those receiving chemotherapy, 11(64.7%) had VTE during chemotherapy days while 35% had VTE with one month after completion of chemotherapy. Those with hormonal therapy 83.3% had aromatase inhibitor, while only 5.4% received tamoxifen. Conclusions: Breast cancer patients with VTE have normal BMI, presenting as swelling of the lower extremity. VTE presented within one year of cancer diagnosis and were mostly alive for more than two years. Active cancer management during VTE diagnosis was mostly during chemotherapy and hormonal with aromatase inhibitor. Legal entity responsible for the study: The authors. Langer's axillary arch lymph node metastasis in breast cancer patients: A prospective clinical study

Background and objectivesLatissimus dorsi, an anatomical landmark for axillary lymph node dissection, was reported to harbor an anatomical variation named Langer's axillary arch (LAA). However, the incidence and clinical significance of LAA in breast cancer remain obscure. MethodsWe conducted a six-year prospective study, including 1724 breast cancer patients in Qilu hospital between January 2012 to February 2018. All patients received ALND were inspected for existence of LAA. All the surgeries were completed with the involvement of same experienced surgeon. Once the LAA was identified, all the lymph nodes located lateral to it, named LAA's lymph node, were dissected and collected for pathologic examination. ResultsAmong 1724 breast cancer patients, LAA was identified in 132 patients (7.66%). 120 out of the 132 patients (90.91%) had at least two LAA's lymph nodes. 21 out of 120 patients (17.50%) were confirmed with cancer cell metastasis in LAA's lymph nodes. Among the 23 patients received sentinel lymph node tracing, sentinel lymph node was located lateral to LAA in 3 patients (13.04%). ConclusionsOur results indicated that it is of great importance for surgeons to correctly ascertain LAA, and it should be taken as a nonnegligible parts during ALND or SLNB.
