ACTH-Dependent Cushing Syndrome Treated with Homeopathy: A Case Report

Abstract Background Cushing's disease is an endocrine disorder caused by endogenous hypercortisolism resulting from an adrenocorticotropic hormone (ACTH)-secreting pituitary tumour. The average incidence of new cases is 2.4 cases per million people per year. The first-line treatment for Cushing's disease is selective transsphenoidal pituitary surgery; however, the risk of recurrence after the initial surgery is high and remains so for many decades after surgery. There is paucity of scientific evidence regarding Cushing disease in homoeopathy. Method This case study describes a case of ACTH-dependent Cushing's syndrome with fasting plasma cortisol serum levels and 24 hours urinary free cortisol levels were 22.7 ug% and more than 1000 μg/24 hour, respectively, and decreased after 1 mg and 8 mg overnight dexamethasone. Mercurius in ascending fifty millesimal potency was prescribed as an individualised homeopathic medicine on the basis of symptoms. Result The patient improved symptomatically as evidenced by a marked reduction in weight and cortisol levels in the normal range under continued homoeopathic treatment. Conclusion Homoeopathy can be effectively used to decrease cortisol levels in Cushing disease and prevent hypercortisolism-associated comorbidities such as infection, diabetes, obesity, osteoporosis and cardiovascular disease.

Homeopathic and conventional treatments for acute upper respiratory tract infections (aURTI): Real-World cohort study on recurrence and antibiotic prescriptions.

The study evaluated the recurrence of acute upper respiratory tract infections (aURTI) and the number of antibiotic prescriptions within 12-month follow-up in patients prescribed with either homeopathic medicines or medicines from one of four conventional medication classes for aURTI therapy. This explorative cohort study used real-world electronic healthcare data from the Disease Analyzer database (IQVIA). Included were patients of all ages from Germany with an index diagnosis of aURTI between 2010 and 2018, who had prescriptions for either homeopathic, conventional cough &amp; cold, nasal, or throat medicines, or nonopioid analgesics on the day of diagnosis or within six days afterwards. aURTI recurrence was assessed by multivariable logistic regression, the number of antibiotic prescriptions by multivariable negative binomial regression. From 3,628,295 patients with aURTI diagnosis initially identified in the database in the relevant time interval, a total of 610,118 patients, fulfilling the in- and not violating the exclusion criteria, were retained for analysis. In the multivariate analyses on all patients, prescriptions of nasal medicines were associated with a significant, slightly higher (OR: 1.18, CI: 1.10-1.26, p<0.001) risk of aURTI recurrence compared to homeopathic medicines within 12 months. Prescriptions of cough &amp; cold (OR: 0.92, CI: 0.86-0.97, statistically significant, p=0.005) as well as throat medicines (OR: 0.93, CI: 0.86-1.01, p=0.086), and nonopioid analgesics (OR: 0.95, CI: 0.89-1.02, p=0.181) were associated with slightly lower risk of aURTI recurrence compared to homeopathic medicines. In the analysis of the age-dependent subgroups, there were some deviations from the overall population in terms of statistical significance; however, the directions of the effect estimates were unchanged. Almost all results of negative binomial regression analyses assessing differences in the frequency of antibiotic prescriptions during follow-up, both in all patients and in the age-dependent subgroups were statistically significant in favor of homeopathic medicines. The study demonstrated that follow-up recurrence and antibiotic prescriptions in patients with uncomplicated aURTI are at least comparable between patients treated with homeopathic and conventional medicines in real-world practice. Despite some methodological limitations inherent to the used database, the results of this study indicate that homeopathic medicines present a valuable therapeutic option for managing aURTI.

Presentation and Analysis of the Online Evidence Gap Map, “Clinical Effectiveness of Homeopathy”

Abstract Introduction and Objective This study presents and analyzes the content of an online Evidence Gap Map (EGM), “Clinical Effectiveness of Homeopathy, 1st Edition”, which graphically represents evidence from systematic reviews (SRs) of human clinical trials in homeopathy that were published from 1991 to 2021. The EGM was built according to the guidelines of the International Initiative for Impact Evaluation (3ie), adapted for complex interventions, to represent visually relevant evidence and research gaps. Presentation of Evidence Gap Map Content The interventions, classified as rows, are characterized in the EGM not only by the homeopathic therapeutic strategy but also by individual medicines and potencies. The outcomes are listed in columns. The associations between both are represented by “bubbles” that refer to the corresponding SRs and whose colors indicate their quality level. Synthesis of Evidence Gap Map Content From 239 citations identified, 51 SRs underwent complete characterization for the final construction of the EGM. A further in-depth quality analysis of each SR was performed to list the studied intervention/outcome associations described in each mentioned trial. At this step, those trials considered to have a high risk of bias by the authors of each SR were excluded from the EGM inputs. Analysis of Evidence Gap Map Content The methodological quality of each of the EGM's SRs was evaluated using the AMSTAR-2 criteria for level of confidence in an SR's results. Intervention–outcome associations were categorized into one of five effects of a homeopathic intervention: positive, potentially positive, ineffective, inconclusive, or negative. Findings The EGM presents research evidence across a wide range of medical conditions, with substantial heterogeneity of homeopathic interventions and clinical outcomes. Forty-two of the 51 SRs yielded inconclusive findings. AMSTAR-2 analysis identified seven high-quality SRs, in which reliable primary studies presenting positive or potentially positive results for different categories of homeopathy are cited and related to specific clinical conditions: fibromyalgia, otitis media, diarrhea, respiratory infections, menopausal syndrome, irritable bowel syndrome, and attention deficit hyperactivity disorder in children. Conclusion The EGM “Clinical Effectiveness of Homeopathy, 1st Edition” presents visually relevant research evidence that is scattered across a large number of medical conditions, showing substantial heterogeneity of homeopathic interventions, clinical outcomes, and research quality. To enhance the precision and relevance of future research, we recommend that the individualized homeopathic approach under investigation be standardized to the greatest extent feasible, and to use measures of well-being and quality of life as primary outcomes.

An evidence-based case of bilateral nephrolithiasis treated with individualised homoeopathic medicine: A case report

Nephrolithiasis, or renal calculi, is a painful and common urinary disorder affecting millions worldwide. Conventional treatments primarily focus on pain management and calculus removal, but concerns about recurrence have led many patients to seek alternative therapies. Homoeopathy, a holistic medical system, offers an intriguing approach to managing nephrolithiasis. A 35-year-old patient presented with bilateral flank pain radiating up to the groin with an ultrasonography report of non-obstructive bilateral renal calculi. He was treated effectively with the homoeopathic medicine Thuja, which was selected according to case totality. This case shows the efficacy of homoeopathic medicines in addressing and expelling bilateral renal calculi. A large-scale randomised trial is recommended to assess the efficacy of individualised homoeopathic treatment.

MONARCH Inventory for Causal Attribution in Homeopathy Case Reports: Explanation and Elaboration.

The Modified Naranjo Criteria for Homeopathy (MONARCH) inventory is a novel and useful tool for assessing the likelihood of a causal relationship between the homeopathic intervention and the ultimate clinical outcome. To explain and elaborate on the use of the MONARCH inventory to improve its consistency of use and thereby elevate the overall quality and evidentiary value of homeopathy case reports. Each of the 10 MONARCH inventory domains is explained and elaborated with the aid of references from published literature and hypothetical clinical situations. The MONARCH total score can range from -6 to +13; within that range, an arithmetic total of +6 is available from the scores of four critical domains, which should be given prominence in the overall consideration of causality. In reporting a clinical case, great importance should be placed on the narrative of the MONARCH evaluation process, domain by domain. The application of MONARCH allows us to evaluate the likelihood of a causal relationship between the intervention and the observed clinical outcomes in successfully treated cases, thereby augmenting their evidentiary value.

Sonication Enhancement of Capsaicin Formation in Callus of Chili Pepper, Capsicum annuum L.

The current study investigates the induction of callus from leaf explants of chili pepper Capsicum annuum L. coupled with the isolation of capsaicin from alcoholic extracts. To determine which isolated alkaloid has a positive reaction, the DragenDroff test is used. Alkaloid is identified using conventional diagnostic techniques, such as measuring the absorbance values of the isolated alkaloid with an ultraviolet spectrophotometer, the alkaloid is identified. The results show a complete identity among them, and with control. Thin layer chromatography data showed a 0.8 cm distance between one location from each tested sample with the same rate, which is 0.8 cm from the control’s rate flow value. The chemical structure of studied samples is subsequently determined using nuclear magnetic resonance, which reveals similarities between the isolated alkaloid’s structure and standard capsaicin. A quantitative analysis of the isolated alkaloids revealed variations in the amounts for generated explants relative to other explants. This study shows that fruits are the most effective source of alkaloids. It’s interesting to note that the composition of the explant and the sonicated callus are identical. Since capsaicin discovery, it is used as a homeopathic remedy to treat burning pain using the concept of “treating like with like” or counterirritant, relieve minor pain associated with rheumatoid arthritis or muscle sprains and strains and due to large consumption of this fruit recently, the current study done to find out the structure and quantity.
