Radiological predictors of outcomes in hip arthroscopy for femoroacetabular impingement.

Hip arthroscopy has gained prominence as a primary surgical intervention for symptomatic femoroacetabular impingement (FAI). This study aimed to identify radiological features, and their combinations, that predict the outcome of hip arthroscopy for FAI. A prognostic cross-sectional cohort study was conducted involving patients from a single centre who underwent hip arthroscopy between January 2013 and April 2021. Radiological metrics measured on conventional radiographs and magnetic resonance arthrography were systematically assessed. The study analyzed the relationship between these metrics and complication rates, revision rates, and patient-reported outcomes. Out of 810 identified hip arthroscopies, 359 hips were included in the study. Radiological risk factors associated with unsatisfactory outcomes after cam resection included a dysplastic posterior wall, Tönnis grade 2 or higher, and over-correction of the α angle. The presence of acetabular retroversion and dysplasia were also significant predictors for worse surgical outcomes. Notably, over-correction of both cam and pincer deformities resulted in poorer outcomes than under-correction. We recommend caution in performing hip arthroscopy in patients who have three positive acetabular retroversion signs. Acetabular dysplasia with a lateral centre-edge angle of less than 20° should not be treated with isolated hip arthroscopy. Acetabular rim-trimming should be avoided in patients with borderline dysplasia, and care should be taken to avoid over-correction of a cam deformity and/or pincer deformity.

Translation and cross-cultural adaptation of the Russian version of the Hip Preservation Surgery Expectations Survey

There are currently no validated tools for evaluating preoperative expectations from hip preservation surgery to use in Russian-speaking patients. The objective of this study was to translate and adapt the Hip Preservation Surgery Expectations Survey and to assess the reliability of it’s Russian version. The process of adaptation included forward and backward translation and pilot testing. The reliability of the final Russian version of the survey was assessed in 40 patients undergoing hip arthroscopy for femoroacetabular impingement. The Cronbach's alpha was 0.945 and the intraclass correlation coefficient was 0.82 (95% CI, 0.56-0.93), indicating a high level of intraclass consistency and re-test reliability. The floor and ceiling effects were not detected. The median score was 87 (0-100). The Russian version of the Hip Preservation Surgery Expectations Survey is a useful and simple tool for assessing and recording preoperative expectations in a structured way, and it can be used in both clinical practice and research.

Association Between Symptoms of Anxiety and Depression, Hip Pathology, and Patient-Reported Outcomes After Hip Arthroscopy for Femoroacetabular Impingement

Background: In patients with femoroacetabular impingement (FAI), mental health has been implicated in both symptom severity and postoperative outcomes. However, there are limited data regarding the independent influences of baseline mental health and hip pathology on patient-reported outcomes over time after hip arthroscopy. Purpose: To evaluate the association between mental health and structural hip pathology with pain, hip function, and quality of life (QOL). Study Design: Cohort study; Level of evidence, 3. Methods: Patient records from a single surgeon’s hip outcomes registry were retrospectively reviewed. Mental health was evaluated using the Patient-Reported Outcomes Measurement Information System Anxiety and Depression scores. Pain was evaluated with the Single Assessment Numeric Evaluation score for Activities of Daily Living (SANE-ADL), while hip-related QOL was evaluated with the 12-item International Hip Outcome Tool (iHOT-12). Hip function was assessed with the Hip Outcome Score (HOS) Sport-Specific (SS) and ADL subscales. Separate mixed models were used to predict pain, QOL, and hip function, including hip pathology measures (size of labral tear, grade of chondral damage, preoperative alpha angle), anxiety, depression, and time as fixed effects and individuals as a random effect. Results: A total of 312 patients were included in this study. The preoperative alpha angle, degree of intraoperative cartilage damage, and size of the labral tear were not associated with pain or QOL (P > .05 for all). However, higher levels of anxiety and depression were significantly associated with lower SANE-ADL scores (estimate ± SE) (anxiety: –0.59 ± 0.07, P < .0001; depression: –0.64 ± 0.08, P < .0001), iHOT-12 scores (anxiety: –0.72 ± 0.07, P < .0001; depression: –0.72 ± 0.08, P < .0001), HOS-SS scores (anxiety: –0.68 ± 0.09, P < .0001; depression: –0.57 ± 0.10, P < .0001), and HOS-ADL scores (anxiety: –0.43 ± 0.05, P < .0001; depression: –0.43 ± 0.06, P < .0001). Conclusion: Patients had similar improvements in pain scores, QOL, and hip function after hip arthroscopy for FAI irrespective of their degree of hip pathology. Additionally, preoperative symptoms of anxiety and depression symptoms were associated with greater pain, decreased QOL, and worse hip function both pre- and postoperatively, independent of the degree of hip pathology. This suggests that efforts to directly address symptoms of anxiety and depression may improve outcomes after hip arthroscopy.

EARLY OUTCOMES OF HIP ARTHROSCOPY FOR FEMOROACETABULAR IMPINGEMENT: A STUDY OF 7,511 CASES USING THE NAHR DATASET

This study used a national registry to assess the outcomes of hip arthroscopy (HA) for the treatment femoroacetabular impingement (FAI).All HAs for FAI recorded in the UK Non-Arthroplasty Hip Registry (NAHR) between January 2012 and September 2023 were identified. Cases were grouped according to the index procedure performed for FAI (cam, pincer, or mixed). Patient outcomes captured included the International Hip Outcome Tool (iHOT)-12.7,511 HAs were identified; 4,583 cam (61%), 675 pincer (9%), 2,253 mixed (30%). Mean age (34.8) was similar between groups. There was a greater proportion of females in the pincer group (75%) compared to cam (52%) and mixed (50%). A higher proportion of patients had a recorded cartilage injury in association with a cam lesion compared to pincer. The pincer group had poorer mean pre-op iHOT-12 scores (31.6 \[95%CI 29.9 to 33.3\]; n=364) compared to cam (33.7 \[95%CI 32.1 to 34.4\]; n=3,941) and achieved significantly lower scores at 12 months (pincer = 52.6 (50.2 to 55); n=249, cam = 58.3 (57.1 to 59.5); n=1,679).Overall, significant (p &lt; 0.0001) iHOT-12 and EQ-5D improvement vs baseline pre-operative scores were achieved for all FAI subtypes at 6 months (overall mean iHOT-12 improvement +26.0 \[95%CI 25.0 to 26.9\]; n=2,983) and maintained out to 12 months (+26.2 \[25.1 to 27.2\]; n=2,760) at which point 67% and 48% of patients continued to demonstrate a score improvement greater than or equal to the minimum clinically important difference (&gt;/=13 points) and substantial clinical benefit (&gt;/=28 points) for iHOT-12 respectively.This study demonstrates excellent early functional outcomes following HA undertaken for FAI in a large national registry.

Preoperative opioid use is associated with worse preoperative patient-reported outcomes in hip arthroscopy patients

ObjectivesThe purpose of this study was to define the rate of preoperative opioid use among patients undergoing hip arthroscopy, ascertain which clinical factors are associated with opioid use, and assess the effect of preoperative opioid usage on preoperative patient-reported outcome (PRO) measures. MethodsA single institution orthopedic registry was retrospectively analyzed for patients undergoing hip arthroscopy for femoroacetabular impingement (FAI) syndrome with or without labral tear between 2015 and 2022. Patients completed Patient-Reported Outcomes Measurement Information System (PROMIS) in six domains, Numeric Pain Scores (NPS), and Musculoskeletal Outcomes Data Evaluation and Management System expectations domain preoperatively. Patients’ charts were reviewed to determine demographic factors and identify any active opioid prescription within 6 weeks before surgery. Bivariate analysis was used to determine associations between preoperative opioid use and baseline PROs. Statistically significant bivariate associations were further tested by multivariate analysis to determine independent predictors. ResultsA total of 123 patients were included (age 39.7 ​± ​12.0 years; 87 females; body mass index 27.4 ​± ​5.7 kg/m2). There were 21 patients (17%) using opioids preoperatively. Prior orthopedic or other surgery and lower education level were associated with preoperative opioid use. Patients with preoperative opioid use scored statistically significantly worse compared with those without preoperative opioid use on baseline PROMIS Physical Function (38.6 vs 40.5, p ​= ​0.01), Pain Interference (65.9 vs 60.2, p ​= ​0.001), Fatigue (60.7 vs 51.6, p ​= ​0.005), Social Satisfaction (38.2 vs 43.2, p ​= ​0.007), and Depression (54.2 vs 48.8, p ​= ​0.01). Preoperative opioid use was also associated with statistically significantly worse preoperative NPS for both the operative hip (6.3 vs 4.6, p ​= ​0.003) and whole body (3.0 vs 1.4, p ​= ​0.008). Preoperative opioid use was an independent predictor of worse baseline PROMIS Pain Interference, Fatigue, Social Satisfaction, and NPS for the operative hip. ConclusionPatients using opioids preoperatively had worse baseline PROs for physical function, pain, social satisfaction, and depression than those not using opioids preoperatively. When controlling for confounding variables, preoperative opioid use was independently predictive of worse baseline pain, fatigue, and social satisfaction. Association Between Chondrolabral Junction Breakdown and Conversion to Total Hip Arthroplasty After Hip Arthroscopy for Symptomatic Labral Tears: Minimum 8-Year Follow-up

Background: Arthroscopic treatment of femoroacetabular impingement (FAI) and symptomatic labral tears confers short- to midterm benefits, yet further long-term evidence is needed. Moreover, despite the physiological and biomechanical significance of the chondrolabral junction (CLJ), the clinical implications of damage to this transition zone remain understudied. Purpose: To (1) report minimum 8-year survivorship and patient-reported outcome measures after hip arthroscopy for FAI and (2) characterize associations between outcomes and patient characteristics (age, body mass index, sex), pathological parameters (Tönnis angle, alpha angle, type of FAI, CLJ breakdown), and procedures performed (labral management, FAI treatment, microfracture). Study Design: Cohort study; Level of evidence, 3. Methods: This retrospective cohort study included patients who underwent primary hip arthroscopy for symptomatic labral tears secondary to FAI by a single surgeon between 2002 and 2013. All patients were ≥18 years of age with minimum 8-year follow-up and available preoperative radiographs. The primary outcome was conversion to total hip arthroplasty (THA), and secondary outcomes included revision arthroscopy, patient-reported outcome measures, and patient satisfaction. CLJ breakdown was assessed using the Beck classification. Kaplan-Meier estimates and weighted Cox regression were used to estimate 10-year survivorship (no conversion to THA) and identify risk factors associated with THA conversion. Results: In this study of 174 hips (50.6% female; mean age, 37.8 ± 11.2 years) with mean follow-up of 11.1 ± 2.5 years, the 10-year survivorship rate was 81.6% (95% CI, 75.9%-87.7%). Conversion to THA occurred at a mean 4.7 ± 3.8 years postoperatively. Unadjusted analyses revealed several variables significantly associated with THA conversion, including older age; higher body mass index; higher Tönnis grade; labral debridement; and advanced breakdown of the CLJ, labrum, or articular cartilage. Survivorship at 10 years was inferior in patients exhibiting severe (43.6%; 95% CI, 31.9%-59.7%) versus mild (97.9%; 95% CI, 95.1%-100%) breakdown of the CLJ (P < .001). Multivariable analysis identified worsening CLJ breakdown (weighted hazard ratio per 1-unit increase, 6.41; 95% CI, 3.11-13.24), older age (1.09; 95% CI, 1.04-1.14), and higher Tönnis grade (4.59; 95% CI, 2.13-9.90) as independent negative prognosticators (P < .001 for all). Conclusion: Although most patients achieved favorable minimum 8-year outcomes, several pre- and intraoperative factors were associated with THA conversion; of these, worse CLJ breakdown, higher Tönnis grade, and older age were the strongest predictors.

The Effect of Pelvic Incidence on Outcomes After Hip Arthroscopy for Femoroacetabular Impingement and Acetabular Labral Tears

Background: In the setting of femoroacetabular impingement (FAI), decompression osteoplasties reconcile deleterious loading patterns caused by cam and pincer lesions. However, native variations of spinopelvic sagittal alignment may continue to perpetuate detrimental effects on the labrum, chondrolabral junction, and articular cartilage after hip arthroscopy. Purpose: To evaluate the effect of pelvic incidence (PI) on postoperative outcomes after hip arthroscopy for acetabular labral tears in the setting of FAI. Study Design: Cohort study; Level of evidence, 3. Methods: A retrospective query of prospectively collected data identified patients ≥18 years of age who underwent primary hip arthroscopy for FAI and acetabular labral tears between February 2014 and January 2022, with 3-, 6-, 12-, and 24-month follow-ups. Measurements for PI, pelvic tilt (PT), sacral slope (SS), and acetabular version were obtained via advanced diagnostic imaging. Patients were stratified into low-PI (<45°), moderate-PI (45°≤ PI ≤ 60°), and high-PI (>60°) cohorts. Patient-reported outcome measures (PROMs), clinically meaningful outcomes (ie, minimal clinically important difference, Patient Acceptable Symptom State, substantial clinical benefit, and maximal outcome improvement), visual analog scale (VAS) pain scores, and patient satisfaction were compared across cohorts. Results: A total of 74 patients met eligibility criteria and were stratified into low-PI (n = 28), moderate-PI (n = 31), and high-PI (n = 15) cohorts. Correspondingly, patients with high PI displayed significantly greater values for PT (P = .001), SS (P < .001), acetabular version (P < .001), and acetabular inclination (P = .049). By the 12- and 24-month follow-ups, the high-PI cohort was found to have significantly inferior PROMs, VAS pain scores, rates of clinically meaningful outcome achievement, and satisfaction relative to patients with moderate and/or low PI. No significant differences were found between cohorts regarding rates of revision arthroscopy, subsequent spine surgery, or conversion to total hip arthroplasty. Conclusion: After hip arthroscopy, patients with a high PI (>60°) exhibited inferior PROMs, rates of achieving clinically meaningful thresholds, and satisfaction at 12 and 24 months relative to patients with low or moderate PI. Conversely, the outcomes of patients with low PI (<45°) were found to match the trajectory of those with a neutral spinopelvic alignment (45°≤ PI ≤ 60°). These findings highlight the importance of analyzing spinopelvic parameters preoperatively to prognosticate outcomes before hip arthroscopy for acetabular labral tears and FAI.

Objectives: Left untreated, femoroacetabular impingement (FAI) poses an elevated risk for the early progression of degenerative joint disease. As a result, the utilization of hip arthroscopy has increased to counteract the maladaptive effects of FAI, as femoroacetabular osteoplasties have been shown to reconcile deleterious loading patterns caused by CAM and pincer lesions. However, variations in the sagittal alignment of the lumbosacral spine also pose an elevated risk for the early progression of degenerative joint disease. While femoroacetabular osteoplasties improve degenerative loading patterns caused by CAM and pincer lesions, variations in the sagittal alignment of the lumbosacral spine may continue to generate deleterious effects on the labrum, chondrolabral junction, and articular cartilage following hip arthroscopy for acetabular labral tears and FAI. Thus, the purpose of this study was to evaluate the impact of pelvic incidence (PI) on patient-reported outcomes measures (PROMs) and rates of achieving clinically meaningful outcomes following primary hip arthroscopy for acetabular labral tears and FAI. Methods: A retrospective review of a prospectively collected, single-surgeon database was performed to identify patients ≥18 years of age who underwent primary hip arthroscopy for the treatment of labral tears secondary to FAI with a minimum of 12 months of follow-up. Patients with a history of ipsilateral hip or spine surgery, absence of preoperative lumbar spine imaging, radiographic evidence of hip dysplasia or advanced hip osteoarthritis, and contraindicated spinal pathologies were excluded. PI, pelvic tilt (PT), sacral slope (SS), and acetabular version were measured on 2-D supine lumbar images, and patients were stratified into low PI (PI &lt; 45°), moderate PI (45° ≤ PI ≤ 60°), and high PI (PI &gt; 60°) cohorts. PROMs, VAS Pain, and frequencies of achieving clinically meaningful outcomes were compared between groups. PROMs and frequencies of achieving clinically meaningful outcomes were compared between cohorts using one-way analysis of variance tests and Fisher’s exact tests, respectively. Additionally, a sensitivity analysis using a linear mixed-effects model with time as a continuous variable was carried out to evaluate the weighted differences in PROM averages between cohorts. Results: A total of 74 patients undergoing primary hip arthroscopy for FAI and acetabular labral tears met eligibility criteria and were subsequently stratified into low ( n=28), moderate ( n=31), and high ( n=15) PI cohorts. The mean ± SD length of follow-up was 25.16 ± 2.91 months, and no significant differences were observed between cohorts in patient demographics. However, as expected, patients in the high PI cohort displayed significantly larger mean values for PT ( P=0.001), SS ( P&lt;0.001), acetabular version ( P&lt;.001), and acetabular inclination ( P=0.049). Moreover, despite no differences in preoperative outcome scores between cohorts, the high PI cohort displayed significantly inferior scores for all PROMs and VAS pain across 6-, 12, and 24-month follow-ups. This finding also extended to our sensitivity analysis, with the high PI cohort displaying significantly lower weighted averages in scores across all PROMs relative to the moderate PI cohort ( P≤.001 for all). Additionally, the high PI cohort demonstrated significantly lower rates of clinically meaningful outcome achievement and overall patient satisfaction at 12- and 24-month follow-ups compared to the moderate and/or low PI cohorts. No significant differences were found between cohorts in terms of revision hip arthroscopy, subsequent spine surgery, or conversion to THA. Conclusions: Following hip arthroscopy to address acetabular labral tears, patients with a high PI exhibit significantly inferior PROMs relative to those with moderate and low PI. Thus, despite addressing FAI deformities, variations in lumbosacral alignment may continue to impact patient outcomes. Although the current study design limits our ability to infer causality, the significantly greater acetabular version and inclination values observed in the high PI cohort lead us to speculate that the sagittal imbalance in the setting of a high PI places increased mechanical stress on the anterior-superior acetabulum, which portends poor outcomes following hip arthroscopy. Overall, our findings highlight the critical importance of analyzing pelvic alignment in order to prognosticate outcomes following hip arthroscopy for acetabular labral tears in the setting of FAI.

Capsular Management at the Time of Hip Arthroscopy for Femoroacetabular Impingement Syndrome Varies With Geography and Surgeon Subspecialty Training: A Cross-Sectional, Multinational Surgeon Survey

PurposeTo perform a multi-national survey and identify patterns in capsular management at the time of hip arthroscopy. MethodsAn anonymous, non-validated survey was distributed by the American Orthopedic Society for Sports Medicine, Arthroscopy Association of North America, European Society of Sports Traumatology, Knee Surgery, and Arthroscopy, International Society for Hip Arthroscopy, and Turkish Society of Sports Traumatology, Arthroscopy, and Knee Surgery. Questions were broken down into 6 categories: demographics, capsulotomy preference, traction stitches, capsular closure, postoperative rehabilitation, and postoperative complications. ResultsThe survey was completed by 157 surgeons. Surgeons who used a half or full T-type capsulotomy had 2.4 higher odds of using traction sutures for managing both the peripheral and central compartment during hip arthroscopy for femoroacetabular impingement (FAIS) (p=0.024). Surgeons who felt that there was sufficient literature regarding the importance of hip capsule closure had 1.9 higher odds of routinely performing complete closure of the capsule (p=0.044). Additionally, surgeons who practiced in the United States had 8.1 higher odds of routinely closing the capsule relative to international surgeons (p<0.001). Moreover, those who received hip arthroscopy training in residency or fellowship had 2.4 higher odds of closing the capsule completely compared to surgeons who did not have exposure to hip arthroscopy during their training (p=0.009). ConclusionGeographic and surgeon-related variables correlate to capsular management preferences during hip arthroscopy. Surgeons who utilized a half or full T-capsulotomy more often utilized traction stitches for managing both the peripheral and central compartment. Surgeons performing routine capsular closure are more likely to believe that sufficient evidence is available to support the practice, with surgeons in the United States more likely to perform routine capsular closure in comparison to their international colleagues.

Hip Arthroscopy in Recreational Athletes: Risk Factors For Failure to Return to Sport and Factors That Influence Psychological Readiness to Return to Sport

Objective: High level athletes achieve high rates of return to play and improved patient reported outcomes (PROMs) after hip arthroscopy for femoroacetabular impingement (FAI). However, few studies have evaluated return to sport rates in recreational athletes, risk factors for failure to return to sport, and the effect of psychological readiness to return to sport (RTS) within this population. We hypothesize that recreational athletes undergoing hip arthroscopy for FAI achieve high PROMs and rates of RTS and that psychological readiness is a risk factor for failure to RTS. Additionally, we hypothesize that male athletes and younger age are positively associated with psychological readiness to RTS. Methods: Retrospective review of prospectively collected data in our institution's Center for Hip Pain and Preservation Outcomes Registry identified recreational athletes with a minimum of 12 months follow-up, who underwent hip arthroscopy for FAI between 2008-2018. The primary outcome was RTS. The secondary outcomes were ---Hip-RSI (SF) score, modified Harris Hip Score (mHHS), Hip Outcome Score Activities of Daily Living (HOS-ADL), Hip Outcome Score Sport (HOS-Sport), and International Hip Outcome Tool (iHOT33). Results: 84.8% of recreational athletes returned to sport with 70.52% returning to the same level of play. Athletes who failed to RTS were younger (p-value=0.0427), last participated 9-12 months prior to surgery (32.26%) or had stopped participating in sports altogether (70.97%) (p-value=0.0002 and p-value=0.0013). Hip-RSI Score was a statistically significant predictor of failure to RTS at one and two years respectively (OR 0.915 (95%CI 0.87, 0.962) and OR 0.904 (95%CI 0.818, 0.999)). Increasing age had a positive effect on psychological readiness to RTS (0.5004-point increase in RSI score (p-value=0.0002)) while sex, BMI, additional ipsilateral surgery, and last participation in sport were not predictive of psychological readiness (p-values&gt;0.05). Conclusions: Recreational athletes undergoing hip arthroscopy achieve high rates of RTS, return to the same level of play, and demonstrate significant improvements in PROMs. The Hip-RSI-score was a statistically significant predictor of failure to RTS at 1 and 2 years post operatively. Lastly, higher psychological readiness for return to sport was seen in older recreational athletes undergoing hip arthroscopy for FAI.

Safety of a Novel Capsular Closure Device in Hip Arthroscopy for Femoroacetabular Impingement

Abstract Introduction: The utilization of hip arthroscopy for the management of femoroacetabular impingement has increased. Capsular closure has been shown to maintain biomechanical stability postoperatively compared to unrepaired capsules. The novel Cap-Fix device (Smith and Nephew, Watford, UK) was developed to aid in capsular closure both by improving the placement of suture and decreasing the number of steps required for capsulotomy. This study aimed to evaluate the safety of the Cap-Fix device for use in capsulotomy and capsular repair following hip arthroscopy. Materials and Methods: A retrospective review of 30 patients undergoing hip arthroscopy for the treatment of femoroacetabular impingement (FAI) with capsular repair using the Cap-Fix 45° or 70° Suture Passer was performed. The Hip Disability and Osteoarthritis Outcome Score for Joint Replacement (HOOS JR) and Patient-Reported Outcomes Measurement Information System-Physical Function (PROMIS-PF) measures were used to evaluate patient-reported outcomes at the baseline and the follow-up intervals. Outcomes of interest included complications and patient-reported outcomes at 2-week, 6-week, and 3-month follow-up. Results: All patients completed 2- and 6-week follow-up, and 28 (93.3%) completed 3-month follow-up. Two patients experienced complications; one with pain requiring glucocorticoid injection at 6 weeks postoperatively, and another with a 12-mm capsular defect found on 3-month postoperative magnetic resonance imaging requiring repeat arthroscopic debridement, revision labral repair, and capsular plication. By 3 months postoperatively, statistically significant improvement in HOOS JR but not PROMIS-PF scores was seen. Conclusion: The Cap-Fix device appears to be safe for use in capsulotomy and subsequent capsular repair during hip arthroscopy for FAI.

Risk factors for conversion to arthroplasty after hip arthroscopy for femoroacetabular impingement

Background: Hip arthroscopy is a surgical procedure that is increasingly used worldwide, mostly in the management of femoroacetabular impingement (FAI). The purpose of this study is to evaluate risk factors for conversion of hip arthroscopy to THA within 3 years after the procedure. Methods: Hip arthroscopies for FAI performed at our institution between 2013 and 2019 were reviewed. Inclusion criteria included a minimum 3-year follow-up or a conversion to THA within 3 years. Patient characteristics and radiographic evaluation was made for each patient. This collected data were compared in the two groups Total hip arthroplasty vs non total hip arthroplasty (THA vs non-THA). Results: A total of 136 patients met the inclusion criteria (69 men, 67 women, mean age 38,21 years). During the follow-up period, 22 underwent a THA in less than 3 years after the procedure (16.2%). A statistically significant association was found between conversion to THA and the Tönnis grade (p&lt;0.05), Kellgren-Lawrence grade (p&lt;0.05) and the joint space (p&lt;0.05). It was also found that the patient's age, body mass index (BMI), race and sex did not add independent predictive information. Conclusions: Our analysis demonstrates a strong and significant association between the radiographic evaluation of osteoarthritis (Tönnis grade, Kellgren-Lawrence grade and joint space narrowing) and the risk of early conversion to THA. Other patient characteristics, like patient's age, BMI, race and sex, doesn´t seem to increase the risk.

Hip Arthroscopy for Femoroacetabular Impingement Is Associated With Improved Sexual Function And Quality of Life

PurposeThe purpose of this study was to investigate changes in sexual function and activity following arthroscopic hip surgery for femoroacetabular impingement using the United Kingdom Non-Arthroplasty Hip Registry dataset. Sub-analyses were performed between males and females, and patients over 40 and under 40 years old. MethodsPatients who had arthroscopic hip surgery between 1st January 2012 and 31st October 2020, were aged over 16, and completed the relevant PROMs were included. Question 9 of the iHOT-12 (SAQ) refers to problems with sexual activity, and responses to this were compared pre-operatively and at 6-months and 12-months post-operatively. Sub-analyses were also performed, including SAQ scores by patients’ sex or age. SAQ scores were correlated with EQ-5D-5L scores using Spearman’s rank coefficient. ResultsSAQ was answered by 2547 patients pre-operatively and at 6-months post-operatively (62.3% female, median age 36.2, IQR 29-44 years) and by 2314 at 12-months (61.9% female, median age 36.2, IQR 29-44 years). Scores for sexual activity increased from 35.0 pre-operatively to 70.0 at 6-months (p<0.001), maintained at 12-months (p<0.001). Female patients demonstrated a significantly greater improvement in their scores for sexual function from pre-operatively (median 30.0, IQR 14-50) to 6-months (median 60, IQR 28-86, p<0.001) and 12-months (median 62.0, IQR 29-90, p<0.001), compared to male patients (pre-operative median 50.0, IQR 25-84; 6-month median 80, IQR 45-97; 12-month median 80, IQR 41-98). The effect of age on improvements in sexual function did not demonstrate a significant difference. A significant positive correlation was found between improvements in sexual function and quality of life, as measured by the EQ-5D-5L, at 6-months and 12-months (p<0.001). ConclusionHip arthroscopy for symptomatic FAI produces an improvement in sexual function and activity. Scores for sexual function improved regardless of patient age or sex; however, female patients experienced a greater improvement in sexual function than males.

PurposeTo determine outcomes after hip arthroscopy (HA) for femoroacetabular impingement (FAI) at a minimun 10-year follow up and identified risk factors for revision surgery. MethodsRetrospective study of a prospective database of patients undergoing HA between January 2010 and December 2011. Rim chondral injuries were evaluated using the acetabular labral articular disruptions system (ALAD). Clinical and radiological data was obtained. Cumulative survival was estimated by Kaplan–Meier and a multivariate Cox proportional hazards model. ResultsSeventy-four patients were included in the study. Mean follow-up was 132 months. There was statistically significant improvement from preoperative PROs at latest follow-up. Seventeen patients (23.9%) underwent revision surgery. Odds ratio for revision surgery was 8.08 in a patient above 40 years old, 7.57 in a patient Tönnis greater than 1, and 4.25 in a patient ALAD 2–3. Cumulative survivorship rate at 10 years was 77.8%, with a 45.4% for patients with Tönnis grade greater than 1 in front of 85.2% for patients with Tönnis grade of 1 or less (p<.001). Risk factor for revision surgery was preoperative degree of osteoarthrosis (p=.02). ConclusionCumulative survivorship rate at 10 years was 45.4% for patients with Tönnis grade greater than 1 in front of 85.2% for patients with Tönnis grade of 1 or less (p<.001). Age, chondral injuries, and degree of osteoarthrosis would increase the risk for revision surgery.

Benefits of a Postoperative Hip Orthosis After Routine Arthroscopy of the Hip: A Retrospective Cohort Study.

No consensus currently exists among orthopaedic surgeons regarding the benefits of hip orthosis after routine hip arthroscopy. To compare patient-reported outcome measures (PROMs) and reoperation rates between patients who were braced versus those who were not braced after routine hip arthroscopy. Cohort study; Level of evidence, 3. A retrospective review was conducted of 193 patients who underwent hip arthroscopy for femoroacetabular impingement (FAI) from January 1, 2018, to December 31, 2021, by 2 orthopedic surgeons at a single institution. Patients before July 1, 2019, were immobilized in a hip orthosis after hip arthroscopy (braced group; n = 101), whereas those after July 1, 2019, were not (nonbraced group; n = 92). Baseline PROMs (visual analog scale for pain, modified Harris Hip Score, Single Assessment Numeric Evaluation, and Veterans Rand 12-Item Health Survey [VR-12] Physical Component Summary and Mental Component Summary) were obtained for all patients and were repeated postoperatively at 2 weeks, 4 weeks, 3 months, 6 months, 1 year, and 2 years. The study groups were compared to evaluate differences in PROMs over time and 2-year postoperative reoperation rates. Group comparisons were also stratified by patient sex. There were no significant differences on any PROM between the braced and nonbraced cohorts at any timepoint. There were also no significant group differences in reoperation rates, with 8 braced patients (7.9%) undergoing reoperation and 1 nonbraced patient (2.3%) undergoing reoperation (P = .208). In the sex-stratified analyses, nonbraced male patients had significantly higher VAS pain and lower VR-12 Mental Component Summary scores at 6 months postoperatively compared with braced male patients (P = .043 and .026, respectively). The study findings suggested that the use of an orthosis after routine hip arthroscopy for FAI does not improve patient-reported outcomes or negatively affect the 2-year reoperation rate. Postoperative bracing increases perioperative cost, and by foregoing routine bracing, patients may avoid the morbidity associated with wearing a brace for a prolonged period.

Association of Hip Dysplasia With Trochlear Dysplasia in Skeletally Mature Patients.

Developmental dysplasia of the hip (DDH) and trochlear dysplasia (TD) are distinct pathologies with several important features in common. In addition to shared risk factors, both forms of dysplasia cause abnormal joint kinematics and force transmission, predisposing patients to pain, injuries to cartilage and soft tissue stabilizers, and ultimately arthritis. To evaluate for an association between hip dysplasia and TD in skeletally mature patients with symptomatic hip dysplasia. Cross-sectional study; Level of evidence, 3. A total of 48 patients with DDH who underwent periacetabular osteotomy were compared with 48 sex-matched patients who underwent hip arthroscopy for femoroacetabular impingement (FAI) between July 2014 and February 2021. All patients were skeletally mature. The Tönnis angle and lateral center-edge angle were measured on preoperative pelvis radiographs. Femoral version, trochlear depth, lateral trochlear inclination (LTI), tibial tubercle-trochlear groove distance (TTTG-d), and posterior lateral condylar angle (PLCA) were measured on preoperative magnetic resonance imaging scans of the symptomatic hip and ipsilateral knee. Continuous variables were compared between the patient groups using 2-sample t tests. Interobserver reliability was measured using the intraclass correlation coefficient. Patients with DDH demonstrated a reduced trochlear depth compared with patients with FAI (3.6 vs 4.6 mm; P < .001). There were no differences between groups in femoral anteversion, LTI, TTTG-d, or PLCA. Two (4.2%) patients with FAI and 17 (35.4%) patients with DDH had a trochlear depth <3 mm (P < .001). One (2.1%) patient with FAI and 7 (14.6%) patients with DDH had an LTI <11° (P = .027). There was no difference between groups in frequency of a convex proximal trochlea, patient-reported ipsilateral knee pain, or ipsilateral knee procedures. Patients with DDH had reduced trochlear depth compared with patients with FAI, demonstrating a higher incidence of dysplastic trochlear features that may predispose patients to patellofemoral joint disease. Further research is needed to determine whether screening at-risk patients and treating TD will help to prevent symptomatic patellofemoral disease.

Postoperative weight-bearing restrictions andrehabilitation protocols after hip arthroscopy forfemoroacetabular impingement: a systematic review.

Despite recent increased interest in hip arthroscopy for the management of femoroacetabular impingement (FAI), there is little evidence to guide weight-bearing recommendations and rehabilitation postoperatively. The primary objective of this study was to determine if sufficient evidence exists to recommend specific weight-bearing restrictions postoperatively. This study was registered with PROSPERO (CRD42021247741). PubMed, MEDLINE and Embase were searched on 3 March 2023 for Level I-IV studies including patients over the age of 18 years, with a minimum 1-year follow-up and reporting of a weight-bearing status, a patient-reported outcome measure (PROM) and a clinical outcome. Meta-analysis was precluded due to heterogeneity in the included studies, and a descriptive analysis was undertaken. Methodological quality and risk of bias were assessed with the methodological index for non-randomized studies (MINORS). Twenty-four studies including 2231 patients who underwent hip arthroscopy for treatment of FAI were included (follow-up interval 33.2 ± 24.7 months). Most articles (62.5%) were case series. There were seven terms describing weight-bearing recommendations, with 83% being some variation of 'partial weight-bearing'. Eight PROMs were reported, with 83% using the modified Harris Hip Score and 87.5% of studies reporting reoperation rates. Only 75% of studies reported rehabilitation protocols. The average MINORS score was 11.07 ± 1.10 out of 16 for non-comparative studies and 18.22 ± 1.48 out of 24 for comparative studies. The reporting of weight-bearing status, clinical outcomes, PROMs and rehabilitation parameters remains poor. At present, sufficient comparative evidence does not exist to make specific weight-bearing recommendation postoperatively.
