Comparing silicone-coated self-adhesive absorbent polyurethane films with transparent absorbent films for bilateral hip dressing: a prospective randomized controlled trial

BackgroundSilicone-coated self-adhesive absorbent (SSA) and transparent films with absorbent (TFA) dressings are reportedly effective postoperative knee surgery dressings; however, there have been no direct comparative studies on these two innovative dressings over the hip areas. In this study, we aimed to compare user satisfaction and potential complications between TFA and SSA dressings for the hip area.MethodsThis prospective randomized controlled trial was conducted at a tertiary hospital. The hip side to receive the polyurethane film with SSA dressing (Mepilex® Border Post-Op) was randomly allocated. The other side of the hip was covered with TFA (OPSITE Post-Op). Participants were scheduled for follow-ups 7 and 14 days after the initial application. Between-group outcomes were compared using a two-sample t-test or Wilcoxon signed-rank test for continuous variables and McNemar’s chi-square test for categorical variables.ResultsThirty-two participants (30 − 60 years) without a history of hip surgery were included in the study. The participants were predominantly female, with a mean age of 42.8 years. Pain, difficulties in daily activities, and satisfaction scores were similar between the groups. However, moisture accumulation was significantly higher with the TFA dressing (37.9% vs. 13.8%, p < 0.01), with more dressing failures (34.5% vs. 20.7%, p = 0.016) and complications (37.9% vs. 17.2%, p = 0.012) at the 14-day follow-up than with the SSA dressing.ConclusionsSSA dressings are preferable for hip wound care because of better moisture management, fewer dressing changes required, and fewer complications if applied for > 7 days. Both dressings offered high user satisfaction, minimal pain, and minor difficulties in daily activities.

Artificial intelligence and machine learning algorithms in diagnosis and therapy of the ankle joint.

The recent advancement of computational systems provides fast information exchange and the collection of large amounts of data. Growing number of those systems allow for effective processing of huge amounts of information, utilizing advanced algorithms that are called artificial intelligence (AI). AI has been used for many years, and the number of its applications is growing in various areas. Such solutions are also being developed increasingly in medicine, including orthopedics and radiology, to support the diagnostic and therapeutic processes. Progress in this area is particularly targeted at the skeletal sites that most often require intervention, such as the hip or knee area, with modest interest in the ankle joint. The ankle is one of the most complicated human joints, and therapeutic procedures for its treatment are relatively common. One of the solutions used in the event of serious ankle joint damage is arthroplasty. This review summarizes the current state of AI applications for the diagnosis and therapy of the ankle joint, focusing on trends and achievements in ankle joint arthroplasty and contemporary orthopedic AI solutions. Ideas from other fields of medical diagnostics or orthopedic surgery that may be utilized in the diagnosis and treatment of ankle joint are also discussed.

The Russian version of the Hip Preservation Surgery Expectations Survey: translation and cross-cultural adaptation

Background. Patients’ preoperative expectations are one of the most significant factors affecting the satisfaction rates after organ-preserving surgeries performed in the hip area. To date, there are no validated Russian-language methods to assess the expectations from hip preservation surgeries. The aim of the study is — to carry out cross-cultural adaptation of the Hip preservation surgery expectations survey, as well as to assess the validity of its Russian version. Methods. In order to preserve validity of the survey, we carried out its cross-cultural adaptation including forward translation, back translation, and pilot testing (pretesting). Validity of the final Russian-language version was assessed by interviewing 40 patients, which underwent hip joint arthroscopy due to femoroacetabular impingement. Results. Cronbach’s α has accounted for 0.945, indicating a high level of internal consistency. Intra-class correlation coefficient (ICC) has amounted to 0.82 (95% CI: 0.56-0.93), which corresponds to good correlation and demonstrates retest reliability. The “ceiling” and “floor” effects have not been detected. Conclusions. The Russian version of the Hip preservation surgery expectations survey is a useful and simple tool to assess preoperative expectations of patients with diseases of the hip joint. The survey makes it possible to structure and document the expectations and may be used in clinical practice and scientific studies.

Preventive machine learning models incorporating health checkup data and hair mineral analysis for low bone mass identification

Machine learning (ML) models have been increasingly employed to predict osteoporosis. However, the incorporation of hair minerals into ML models remains unexplored. This study aimed to develop ML models for predicting low bone mass (LBM) using health checkup data and hair mineral analysis. A total of 1206 postmenopausal women and 820 men aged 50 years or older at a health promotion center were included in this study. LBM was defined as a T-score below − 1 at the lumbar, femur neck, or total hip area. The proportion of individuals with LBM was 59.4% (n = 1205). The features used in the models comprised 50 health checkup items and 22 hair minerals. The ML algorithms employed were Extreme Gradient Boosting (XGB), Random Forest (RF), Gradient Boosting (GB), and Adaptive Boosting (AdaBoost). The subjects were divided into training and test datasets with an 80:20 ratio. The area under the receiver operating characteristic curve (AUROC), accuracy, sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV) and an F1 score were evaluated to measure the performances of the models. Through 50 repetitions, the mean (standard deviation) AUROC for LBM was 0.744 (± 0.021) for XGB, the highest among the models, followed by 0.737 (± 0.023) for AdaBoost, and 0.733 (± 0.023) for GB, and 0.732 (± 0.021) for RF. The XGB model had an accuracy of 68.7%, sensitivity of 80.7%, specificity of 51.1%, PPV of 70.9%, NPV of 64.3%, and an F1 score of 0.754. However, these performance metrics did not demonstrate notable differences among the models. The XGB model identified sulfur, sodium, mercury, copper, magnesium, arsenic, and phosphate as crucial hair mineral features. The study findings emphasize the significance of employing ML algorithms for predicting LBM. Integrating health checkup data and hair mineral analysis into these models may provide valuable insights into identifying individuals at risk of LBM.

Harvesting of autologous bone graft for the mandible: from the hip or the jaw?

A 61-year-old edentulous patient presented with dental problems. Examination revealed a very narrow alveolar process in the symphysis area of the chin, with moderate vertical resorption. The decision was made for bone augmentation by means of harvesting an autologous bone graft from the alveolar process. This method can be carried out in an outpatient or day surgery unit and is much more cost effective and less invasive than harvesting an autologous bone graft from the hip area.

Polymyalgia Rheumatica Presenting as Nocturnal Pyrexia of Unknown Origin: A Case Report

Background. Polymyalgia rheumatica (PMR) is a rheumatic disorder characterized by musculoskeletal stiffness and pain, primarily affecting the shoulder, neck, and hip areas. It is more common in females, with the peak incidence usually after the age of 70. Case Report. A 74-year-old man presented with a two-month history of low-grade nocturnal fever up to 100oF (37.7oC) which did not respond to multiple courses of antibiotics. There was unintentional weight loss of 5 kg and mild shoulder stiffness. The patient had a history of Type 2 diabetes mellitus treated with 34 units of insulin (Humulin 70/30) daily and active smoking of 40 pack-years. On examination, mild tenderness of both shoulder girdle muscles and discomfort on external rotation were noted. The initial blood work-up revealed raised erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP) levels. Serologies for syphilis, human immunodeficiency virus (HIV), hepatitis B virus (HBV), and hepatitis C virus (HCV) were negative. Given the negative results of infective and malignancy screening, along with raised inflammatory markers and mild shoulder stiffness, a diagnosis of PMR was made and a trial of prednisolone was initiated, resulting in the resolution of symptoms. Conclusions. The present case report highlights the importance of thoroughly investigating all differential diagnoses of pyrexia of unknown origin, regardless of the patient’s ethnic origin, to facilitate timely diagnosis.

Weight-Bearing Restrictions Following Total Hip Arthroplasty in a Patient with Upper Limb Amputation - a Care Compliant Case Report and Review of The Literature

We present the case of a 71 year male patient with right upper limb amputation who presented to an orthopaedic clinical hospital for increased pain in the hip area associated with decreased range of motion which interfered with most of his daily activities. Patient was evaluated in an interdisciplinary team by rehabilitation and orthopaedic specialists and was scheduled for surgery after a pre rehabilitation period. Acute phase included the first week post-surgery in which early mobilization was initiated with transfer training and verticalization. Weight-bearing as tolerated was indicated on the operated limb from postoperative day 1 and the use of an axillary crutch on the functional upper limb. Walking was supervised by a physical-therapist in the first days post-surgery. Neuromuscular electrical stimulation was associated in order to promote muscle strength and static cryotherapy was applied every 4 hours on the operated limb. At day 7 the patient was discharged with a functional autonomy sufficient for his home activities. The patient was given a precise protocol to follow until control a reevaluation at 6 weeks postsurgery. By the end of the 8 week post-surgery the patient was able to demonstrate reliable gait patterns and was permitted to wean assistive devices. At 6 month follow up the patient was able to restore all functional activities with satisfying results regarding functional scores. Our case report suggests that THA can be indicated for a broader spectrum of patients without posing a major risk regarding the rehabilitation program if supervised properly in an interdisciplinary team.

Factors related to adolescent women's readiness for menarche

Menarche is the first natural bleeding occurring in teenage women where blood comes out of the vagina or can be called the first menstruation. Menarche is an early sign of other changes such as breast growth, hair growth in the pubic and axillary areas, and fat distribution in the hip area. Objective: This research determines the factors related to the readiness of young women to face menarche in young women at Baitul Aziz High School, Percut Sei Tuan District, Deli Serdang Regency in 2023. This type of research is an Analytical Survey of researchers who try to explore how and why this phenomenon occurs and this research uses a cross-sectional approach, namely finding out what factors are related to the readiness of young women to face menarche. The sample used was 40 people. Data analysis in this study was univariate and bivariate analysis using the Chi-square test with a=0.05. Based on the results of the Chi-square statistical test, this research found that there was a relationship between the readiness of young women to face menarche, namely the knowledge factor with a p-value = 0.001, the attitude factor with a p-value = 0.000, and the family support factor with a p-value -value= 0.003 so all Ho are rejected (p&lt;0.05). Baitul Aziz High School, Percut Sei Tuan subdistrict, Deli Serdang district, to foster more motivation, enthusiasm and increase knowledge when facing menstruation for the first time (menarche).

Денситометрическая оценка минеральной плотности костной ткани у пациентов молодого возраста с лимфомой Ходжкина

Hodgkin’s lymphoma is more often diagnosed in young patients. One of the late complications of pathogenetic therapy of Hodgkin’s lymphoma is osteopenia/osteoporosis and the increased risk of low-energy fractures. Conducting densitometry in young patients in order to assess the bone tissue condition is advisable, but its clinical assessment remains difficult. At the same time, early diagnosis and prevention of bone mineral density decrease in patients with Hodgkin’s lymphoma who received antitumor therapy are very relevant. The purpose — to study the features of densitometric indicators for the verification of osteopenia / osteoporosis in young patients with Hodgkin’s lymphoma. Material and methods. 63 people were included in the study, 33 of them were patients with Hodgkin’s lymphoma who received pathogenetic therapy, and 30 people the control group — healthy volunteers. Bone mineral density was measured in all patients using the HOLOGIC (Hologic Inc., Bedford) in the lumbar spine (L1–L4) and in the hip area (the total area of the hip and hip neck). Criteria such as minimum bone mineral density, T-criteria and Z-criteria for young patients with Hodgkin’s lymphoma were determined. Results. In the group of patients with Hodgkin’s lymphoma, a decrease in bone mineral density is significantly more often recorded in all measurement areas, as well as the Z-criterion in the lumbar spine. Regretfully, in young patients the T-criterion can only be used given a more intensive mineralization of bone tissue than in older people and the presence of additional risk factors for osteoporosis (PCTs, glucocorticosteroids, decreased androgenic and fertile function). In patients with HL who received pathogenetic therapy, a decrease in the T-criterion to osteopenia is more often observed in the proximal femur. Conclusion. In many cases, modern capabilities for osteoporosis diagnostic allow correcting mineral metabolism in bone tissue and preventing the development of complications in young patients with Hodgkin’s lymphoma.

Whole-Body Electrical Muscle Stimulation As a Way To Increasing a Comfort of Life for Patient with Frozen Shoulder History

Purpose: This research aims to present the effects of a whole-body electrical muscle stimulation (WB-EMS) physical training program on body composition and subjective pain experience for the 50 years old man A 71-day program was designed for the patient who completed 6 months of physiotherapy and rehabilitation phase after arthroscopy of a frozen shoulder, as an alternative way to complete recovery and increase comfort of life. Methods: A WB-EMS trainer supervised a subject as he worked out, following a detailed physical training program displayed on a projector screen. The subject exercised by imitating the movements shown in a previously recorded video, while the trainer carefully observed the subject. All data was collected (body composition and circumference, quality of life, subjective shoulder pain and perceived exertion questionnaire)and exercises performed based on programmed training were made in the laboratory with access to appropriate equipment. Results: Comparing day 0 (before training) and day 71, an upward trend was observed for muscle mass +5.82 kg (+19%) and segmental body composition data for lean mass +6.38 kg (+13%) in total. A downward trend was observed for segmental body composition data for fat mass –5.60 kg (-24%) in total. Body circumference data decreased for the hip, waist, and thigh -4.4cm (-2%) in total. Body circumference data increased for the arm, calf, and chest +8.7cm (+4%) in total. Our results indicated that WB-EMS physical training program can reduce fat (especially hip and waist area) and obtain gain mass, especially muscle mass for older subjects without pain. Quality of life index increase from 115 points (67%) to 152 points 89%. Conclusions: The WB-EMS physical training program was a joint shoulder-friendly form of physical exercise during which the patient reported no pain. Subject achieved the training goal: reducing fat mass and gaining muscle mass which will increase his comfort of life and perhaps prevents other age-related diseases 50+. Because of the study’s-controlled environment, this training could likely be adapted for successful home use with careful attention to detail, especially having the trainer’s guidance readily accessible.

Experience of Avascular Necrosis of Hip Joint due to Unjustified Use of Steroid: A Case Series Study

The use of steroids, whether in herbal or synthetic form, for purposes such as weight gain, appetite enhancement, and pain relief is a prevalent practice in Bangladesh. Lacking adherence to pharmacological guidelines, such as those outlined by the American College of Rheumatology (ACR) poses a significant health risk. Steroid-induced osteonecrosis is one in many adverse effects that consuming OTC steroid causes. AVN is characterized by the death of bone tissue due to the prolonged use or high-dose administration of corticosteroids. Commonly affected areas include the hip joints, knees, and shoulders. However, this seemingly innocuous trend has prompted concern, particularly at the Dhaka Specialized Pain Management and Research Centre, where a notable pattern has emerged. From July 2022 to June 2023, a total of six reported cases highlighted a worrisome trend of individuals encountering health issues due to the misuse or prolonged use of over-the-counter (OTC) steroids. The majority of the reported cases were male, predominantly in their 2nd to 3rd decade of life. The common thread among these cases was the history of oral steroid ingestion, either in the form of herbal or synthetic compounds. The patients presented with complaints of pain in the hip and groin area, particularly during walking. Diagnostic imaging, including X-ray and MRI of the hip joints, consistently revealed a classical picture of Avascular Necrosis (AVN), underlining the potential association between steroid use and this debilitating condition. The cases reported at the Dhaka Specialized Pain Management and Research Centre serve as a stark reminder of the potential consequences, such as AVN, that can arise from the uninformed or unsupervised use of steroids.

Periprosthetic Knee and Hip Infection — Is It Possible to Compare Treatment Outcomes?

Background. It is known that the outcomes of patients treatment with periprosthetic joint infection (PJI) are significantly influenced by the state of the patient’s body, the nature of the pathogen, the state of tissues in the area of the infectious focus and the treatment tactics. However, topographic and anatomical features of the blood supply to the knee and hip joints, as well as the volume of soft tissues, can affect the spectrum of pathogens, manifestations of the infectious process and, as a consequence, the effectiveness of treatment.&#x0D; The aim of the study was to conduct a comparative analysis of the somatic status of patients, the etiology of the infectious process and the effectiveness of treatment of PJI depending on its localization.&#x0D; Methods. A single-center retrospective study was conducted. The cases of 337 patients were studied — 119 patients with knee and 218 patients with hip PJI who underwent treatment with a two-stage technique during the period from 2007 to 2017. A comparative analysis of the PJI pathogens structure and concomitant pathology between patients with hip and knee PJI was carried out. The frequency of infection relief in the groups following the first stage was counted.&#x0D; Results. Recurrence was diagnosed four times more frequently in patients with hip PJI compared to knee PJI. Risk factors for recurrence were gram-negative pathogens, microbial associations, and fistulous forms of the infectious process. Localization of the infectious process in the hip area was associated with a statistically significant greater volume of blood loss during the prolonged debridement surgery.&#x0D; Conclusion. Recurrences of PJI occur more frequently in the hip area compared to the knee joint. Further analysis of critical factors in recurrence development is necessary for potential intervention.

Arterial vascularization of the pelvic limb of the reindeer fetus

The reindeer is a representative of the order artiodactyls, most of the individuals inhabit the territory of the Northern hemisphere. It is a significant hunting and commercial species, and reindeer husbandry is the main branch of animal husbandry in the conditions of the Far North. The morphology of the reindeer has always been of interest to researchers. In different years, the architectonics of the vascular bed of the reindeer have been studied by many researchers [3, 4]. However, in the available literature sources, we have not found data concerning arterial blood supply to organs and tissues in reindeer fetuses. In the prenatal period of development, there is a restructuring of organs and tissues, and with them the vascular bed. The aim of the study was to study the arterial vascularization of the pelvic limb in a reindeer fetus, to give morphometric characteristics to the arteries. The studies were conducted on 5 reindeer fetuses at the age of 4.0-4.5 months. The fruits were delivered to the Department of Animal Anatomy of the St. Petersburg State University of Veterinary Medicine from reindeer herding farms of the Republic of Sakha (Yakutia). The fruits were extracted after the slaughter of vazhenok for household needs. Classical methods were chosen as research methods: fine anatomical dissection, morphometry, photographing and vasorentgenography. During the study, it was found that the main arterial trunk of the pelvic limb in a reindeer fetus is the external iliac artery. Skeletotopy and angioarchitectonics of the main arteries of the pelvic limb of the reindeer fetus have typical patterns characteristic of adult animals. Morphometric parameters of the arteries are consistent with the topographic position of the muscle groups – a greater number of large arterial branches are noted in the hip area. The presence of collateral blood flow in the foot area is noted – the vessels of the dorsal and plantar surfaces are connected to each other due to the perforating metatarsal and perforating metatarsal arteries, as well as at the confluence of the axial and non-axial finger arteries.

REMOVAL OF RETAINED BULLETS FROM THE HIP JOINT IN CIVILIAN GUNSHOT INJURIES

Removal of bullets retained within joints is indicated to prevent mechanical blockade, 3rd body wear and resultant arthritis, plus lead arthropathy and systemic lead poisoning. The literature is sparse on this subject, with mostly sporadic case reports utilizing hip arthroscopy. We report on the largest series of removal of bullets from the hip joints using open surgical.We reviewed prospectively collected data of patients who presented to a single institution with civilian gunshot injuries that breached the hip joint between 01 January 2009 and 31 December 2022.We included all cases where the bullet was retained within the hip joint area. Exclusion criteria: cases where the hip joint was not breached, bullets were not retained around the hip area or cases with isolated acetabulum involvement.One hundred and eighteen (118) patients were identified. One patient was excluded as the bullet embedded in the femur neck was sustained 10 years earlier. Of the remaining 117 patients, 70 had retained bullets around the hip joint. In 44 patients we undertook bullet removal using the followingsurgical hip dislocation (n = 18), hip arthrotomy (n = 18), removal at site of fracture fixation/replacement (n = 2), posterior wall osteotomy (n = 1), direct removal without capsulotomy (tractotomy) (n = 5).In 26 patients we did not remove bullets for the following reasons: final location was extra-capsular embedded in the soft tissues (n=17), clinical decision to not remove (n=4), patients’ clinical condition did not allow for further surgery (n= 4) and patient refusal (n=1). No patients underwent hip arthroscopy.With adequate pre-operative imaging and surgical planning, safe surgical removal of retained bullets in the hip joint can be achieved without the use of hip arthroscopy; using the traditional open surgical approaches of arthrotomy, tractotomy and surgical hip dislocation.

Ethnosains of The Javanese Community "The Body Funeral Process" as A Resource of Basic Science Learning

The purpose of this study is to reveal the natural science concept of the funeral process for the bodies of the Javanese people. This research is a qualitative research method of phenomonology. This research involved the Brebes community who carried out this activity. Data collection techniques used are interviews and scientific documents. The research instrument used was interviews. The validity of the data was obtained using the triangulation method. Data analysis techniques namely data reduction, presentation and drawing conclusions. The results of this study the process of burial of the bodies of the Brebes community includes 4 (four) activities: (1) bathing, (2) shrouding, (3) lighting, and (4) burying. The funeral process for bodies can be reconstructed into formal science/science at school. The depth of the grave is the standard measurement, which is 160 cm and 2 meters long and the height of the pile of soil is 75 cm. The circle that is placed on the head of the deceased serves as a pillow, 7 of which are placed between the neck and head, the hip area, and the feet. The body is laid on its side and facing the Qibla. The wood used as “tenger”/tombstone is of the teak/albasiar type. The body was closed using a board. The type of soil used for graves that has a loose texture. The types of flowers sprinkled over the grave are jasmine, magnolia, and roses. The process of treating the corpse has been carried out from generation to generation. The burial process of the corpse can be used as a learning resource for basic science courses in science teaching.

Designing for Unique Female Figures: A Study in the Cape Coast Metropolis

This study looks into challenges custom designers face in constructing garments for unique female figures and the satisfaction derived in terms of fit and comfort by the unique female figures, namely: 'A,' 'V,' 'H' and 'O' body shapes, in the Cape Coast Metropolis, Ghana. A total of 120 participants, comprising 60 custom designers and 60 unique figures, were purposively sampled and data were collected using observational checklists and structured interviews. The study reveals that garment shaping is the most challenging process custom designers face in garment making for unique figures. Unique figures were barely satisfied with the overall garment fit of their custom-made garments. The 'A' body shapes were most displeased, whereas the 'H' body shapes were most pleased with the fit of their custom-made garments. This study observes that custom designers' adjustment methods are not suitable for the shoulders, bust, waist, hip, buttock, thigh and armhole areas of the garment resulting in fit problems for their customers. Also, custom designers are not aware of some garment areas, such as the bust, armhole, sleeve length, waist, hip, thigh and buttock, which give problems to their clients. It is recommended that designers should always be abreast with how to manipulate patterns or incorporate adaptation techniques while cutting out fabric so that garments fit different body types.

Design and application of a kind of anti-bedsore turning pad

Patients who are bedridden are for a long-time prone to develop bedsores, especially in the hip and sacral areas and limbs, which causes eczema, ulcers, infection and other complications, resulting in pain and more medical costs. Therefore, the medical staff of the Second Affiliated Hospital of Zunyi Medical University designed and developed a kind of anti-bedsore turning pad, and has obtained the national utility model patent (ZL 2021 2 3004923.9), which is suitable for various long-term bedridden patients. The anti-bedsore turning pad includes the center axis of the turning pad, and ventilation pad 1 and ventilation pad 2 designed on the left and right of turning pad center axis. Under the ventilation pad 1 and the ventilation pad 2, the air pad 1 and the air pad 2 are respectively designed. There is a bedspread connected with ventilation pad 1 and ventilation pad 2 on the inflatable pad 1 and the inflatable pad 2. Through the design of inflatable pad 1 and inflatable pad 2, the left and right of the anti-bedsore turning pad can be lowered or raised independently, which is convenient for the patient's body to tilt and turn over, and can significantly reduce the number of nursing staff and the burden of nursing staff when turning over. In addition, it is convenient to replace the force site at any time and reduce the occurrence of pressure ulcers caused by long-term pressure on the force site. Through the design of ventilation cushion 1 and ventilation cushion 2, the internal gas flow of the turning pad can be made, and the ventilation between the patient and the turning pad can be kept dry, so as to reduce the occurrence of eczema, ulcers or infection and other complications, and ultimately reduce the occurrence of bedsores. In addition, through the design of the most superficial limb pad, the patient's limb can be appropriately elevated or massaged, which increases the comfort of the patient. The anti-bedsore turning pad is simple and effective, and can be widely used in long-term bedridden patients.

Gynoid Fat Distribution and Adipocyte Trapping May Explain Virological Failure With Intramuscular Long-Acting Cabotegravir and Rilpivirine.

Intramuscular long-acting antiretroviral drugs can improve adherence to lifelong antiretroviral treatment. Nevertheless, adipose tissue thickness and distribution play a critical role with injectable drugs. We describe a virological failure with cabotegravir and rilpivirine in a Black African woman with human immunodeficiency virus type 1 with gynoid fat distribution (ie, adipose tissue prevailing in the pelvis and hip area) and body mass index <30 kg/m2.

Association between obesity and osteoarthritis in the South Korean older population: A nationwide population-based study.

There are few studies on the association between obesity and radiologically-confirmed osteoarthritis (OA) in the South Korean older population. We investigated the association between obesity and radiologically-confirmed OA in a nationally-representative sample of the South Korean older population. The study population comprised 5811 participants (2530 men and 3281 women) aged ≥60 years selected from the 2010 to 2012 Korea National Health and Nutrition Examination Survey. Radiographic OA was defined as Kellgren-Lawrence grade ≥ 2 on either the knee or hip area in radiographic images. The odds ratios and 95% confidence intervals for OA were determined using multiple logistic regression analyses after adjusting for confounding factors. Overall, 7.9% and 29.6% of older men and women had OA, respectively. A U-shaped curve with the nadir in the appropriate body weight (body mass index 18.5-23 kg/m2) revealed that 9.0%, 6.8%, 8.1%, and 9.1% of older men and 24.5%, 21.6%, 27.1%, and 38.4% of older women who were underweight, normal weight, overweight, and obese, respectively, had OA. Compared with normal-weight people, the odds ratios (95% confidence intervals) for OA in obese subjects were 1.73 (1.13-2.64) and 2.76 (2.13-3.56) for older men and women, respectively, after adjusting for age, comorbidities, lifestyle behaviors, and socioeconomic status. Obesity was significantly associated with an increased risk of OA in the South Korean older population. This finding suggests that efforts to maintain appropriate body weight and reduce excessive body weight should be considered to reduce the risk of OA in older adults.

Sex-specific bone and muscular morphological features in ischiofemoral impingement: A three-dimensional study.

The study aimed to investigate how hip bone and muscular morphology features differ between ischiofemoral impingement (IFI) patients and healthy subjects among males and females. Three-dimensional models were reconstructed based on magnetic resonance imaging images from IFI patients and healthy subjects of different sexes. Bone morphological parameters and the cross-sectional area of the hip abductors were measured. The diameter and angle of the pelvis were compared between patients and healthy subjects. Bone parameters of the hip and cross-sectional area of the hip abductors were compared between affected and healthy hips. The comparison results of some parameters were significant for females but not males. For females, the comparison results of pelvis parameters showed that the anteroposterior diameter of the pelvic inlet (p=0.001) and intertuberous distance (p < 0.001) were both larger in IFI patients than in healthy subjects. Additionally, the comparison results of hip parameters showed that the neck shaft angle (p < 0.001) and the cross-sectional area of the gluteus medius (p < 0.001) and gluteus minimus (p=0.005) were smaller, while the cross-sectional area of the tensor fasciae latae (p < 0.001) was significantly larger in affected hips. Morphological changes in IFI patients demonstrated sexual dimorphism, including bone and muscular morphology. Differences in the anteroposterior diameter of the pelvic inlet, intertuberous distance, neck shaft angle, gluteus medius, and gluteus minimus may explain why females are more susceptible to IFI.
