INTRODUCTION: Colon polyps arising from within a diverticulum are rare with only a few cases reported to date. Endoscopic resection of such polyps carries a high risk of perforation largely due to muscular hypertrophy associated with diverticular disease. Surgical intervention is the traditional approach for patients with these findings but carries an increased risk and cost. In this case we present successful excision of an adenomatous polyp located entirely within the lumen of a diverticulum using endoscopic mucosal resection (EMR). Only few cases exist documenting successful removal of a polyp within a colonic diverticulum by non-surgical means. CASE DESCRIPTION/METHODS: A 73-year-old woman presented for surveillance colonoscopy. During the procedure she was found to have a 1 cm polyp arising from within a diverticulum in the proximal transverse colon. ORISE gel was injected within the diverticulum raising the polyp out, allowing for successful removal by one-piece snare polypectomy technique without any complications. Two Endo-clips were prophylactically deployed. Pathology was significant for tubular adenoma without high grade dysplasia or malignancy. DISCUSSION: Submucosal lift is one of the most significant advances made in polypectomy. This technique allows for resection of large, flat and complicated lesions while allowing sparing of submucosal tissue. Initially, this technique was performed with saline solution but now Eleview and ORISE solutions are approved by the FDA for these procedures.
