2nd International Symposium on Molecular Diagnostics and Skin Gene Therapy

The 2nd International Symposium on Molecular Diagnostics and Skin Gene Therapy was held on a beautiful spring weekend at one of the youngest universities in the world, the Heinrich-Heine University, founded 1965. The conference attracted more than 340 participants from 24 different countries, including ethicists, basic researchers, virologists, immunologists, molecular biologists and clinicians, as well as numerous students. The event was organised under the auspices of the European Gene Therapy Society by U Hengge of the Department of Dermatology at Heinrich-Heine University. As Dr J Glorioso, President of the American Society for Gene Therapy, pointed out in his welcome address, the skin is a new target for gene therapy, with the potential for in vivo gene transfer to the central nervous system or to the lymphoid organs, for example. As such, Düsseldorf was a perfect location for such a meeting, because the most important biomedical research investments in Germany have been in neurology and dermatology. Many scientific challenges facing this young field of medicine were presented, but there was also much discussion of ethical, regulatory and safety issues with respect to gene therapy.

Implicit Association Test: Separating Transsituationally Stable and Variable Components of Attitudes toward Gay Men

Therapeutic experience of 65 cases with organic hyperinsulinism.

The treatment for insulinoma is enucleation or resection, which are associated with various degrees of morbidity, including fistulas, pseudocysts, and necessity for reoperations. We evaluated the outcome of 65 patients operated on for organic hyperinsulinism at an experienced university hospital (Department of Surgery, Heinrich-Heine University, Düsseldorf, Germany) between 1990 and 1998. Procedures included enucleation (n=37), left-sided resection (n=19), combination of enucleation from the head and left-sided resection (n=4), subtotal left pancreatectomy (n=2), and pancreaticoduodenectomy (n=3). In 47 out of the 65 patients (72%), the expected benign solitary insulinoma was found. Seven patients had malignant tumors and another eight had multiple tumors (including two with multiple endocrine neoplasia type 1 (MEN-1)), and three had nesidioblastosis. In total, 20 patients (31%) developed postoperative drainage of high amylase containing fluid, which in all but three cases resolved with conservative therapy. The three patients who developed fistulas and the three patients who developed pseudocysts underwent reoperative surgery. Seventeen (41%) of the patients who underwent enucleation (10 from the pancreatic head; 50%) and six (25%) of the resected patients demonstrated these complications. Postoperative glucose metabolism was normalized in all patients. Resection, especially in the head of the pancreas, demonstrates lower complication rates than enucleation. We propose resection for tumors that are large, malignant, situated close to pancreatic duct, and are multiple, including in MEN-1. For tumors in the head, pancreaticoduodenectomy may be an occasional choice.

We compared the plantar melanoma cases from 1990 to 1997 in Kumamoto University and Heinrich-Heine University in a retrospective study. Out of 112 melanoma patients in Kumamoto University, 47 cases (42%) were plantar melanoma, while out of 925 melanoma patients, 68 cases (7%) were plantar melanoma in Heinrich-Heine University. There were no differences in patient's mean age and sex ratio between Kumamoto University and Heinrich-Heine University. However, there were some differences in mean Clark level, mean tumor thickness, patients's delay and the recurrence ratio between these two Universities. Plantar melanoma in Kumamoto University showed those of cases with a thin Clark level, thin tumor thickness, short patients's delay and low recurrence rate. These data suggest that public education on plantar melanoma was more prevailing in Japan and public education will help early detection leading to a good prognosis. Effect of age on excess mortality in obesity.

The effect of age on excess mortality from all causes associated with obesity is controversial. Few studies have investigated the association between body mass index (BMI, calculated as weight in kilograms divided by the square of height in meters), age, and mortality, with sufficient numbers of subjects at all levels of obesity. To assess the effect of age on the excess mortality associated with all degrees of obesity. Prospective cohort study. A total of 6193 obese patients with mean (SD) BMI of 36.6 (6.1) kg/m2 and mean (SD) age of 40.4 (12.9) years who had been referred to the obesity clinic of Heinrich-Heine University, Düsseldorf, Germany, between 1961 and 1994. Median follow-up time was 14.8 years. All-cause mortality through 1994 among 6053 patients for whom follow-up data were available (1028 deaths) analyzed as standardized mortality ratios (SMRs) using the male-female population of the geographic region (North Rhine Westphalia) as reference. The cohort was grouped into approximate quartiles according to age (18-29, 30-39, 40-49, and 50-74 years) and BMI (25 to <32, 32 to <36, 36 to <40, and > or =40 kg/m2) at baseline. The SMRs showed a significant excess mortality with an SMR for men of 1.67 (95% confidence interval, 1.51-1.85; P<.001) and an SMR for women of 1.45 (95% confidence interval, 1.34-1.57; P<.001). The excess mortality associated with obesity declined with age. For men, the SMRs of the 4 age groups were 2.46, 2.30, 1.99, and 1.31, respectively; for women, they were 1.81, 2.10, 1.70, and 1.26, respectively (Poisson trend test, P<.001). The SMRs increased with BMI but, within each BMI group, the SMRs decreased with age. The lowest SMRs (for men, 1.01; for women, 0.91) were obtained for patients older than 50 years with BMIs of 25 to less than 32 kg/m2. Thus, older men and women at a BMI range of 25 to less than 32 kg/m2 had no excess mortality. The highest SMRs (for men, 4.22; for women, 3.79) were calculated for the patients aged 18 to 29 years with a BMI of 40 kg/m2 or higher. In this large cohort of obese persons, risk of death increased with body weight, but obesity-related excess mortality declined with age at all levels of obesity.

Pediatric Hematology and Oncology at the Heinrich-Heine University Medical School, DÜSSELDORF, Germany

The Heinrich-Heine University Medical Center is a large tertiary care hospital, a major medical research facility, the largest teaching hospital of the Heinrich-Heine University Medical School, and a training facility for both nursing and physiotherapy schools. In 1974, the Department of Pediatric Hematology and Oncology was established as a separate department within the University's Center of Child Health, serving a catchment area of 3 million inhabitants in the northwest of the federal state of Northrhine-Westfalia. With approximately 85 newly diagnosed cases of malignant illness and 25 bone marrow transplantations per annum, the department is one of the larger ones in Germany. The self-understanding of this unit reaches beyond immediate care of patients, however. The department attempts to provide psychosocial support to families, patients, and caregivers. Furthermore, it takes its responsibility as a teaching and research facility quite seriously, aiming to add to our understanding of cancer with the goal of improving the outcome of childhood cancer.

Comparison of a Lactate- Versus Acetate-Based Hemofiltration Replacement Fluid in Patients with Acute Renal Failure

The objective of the study was to determine the impact of a lactate- and an acetate-based hemofiltration replacement fluid (HF) on the acid–base status in patients with acute renal failure (ARF) and continuous venovenous hemofiltration (CVVH). The prospective, cohort study was carried out in the intensive care unit of the Heinrich-Heine University Hospital, Düsseldorf, FRG. Subjects were 84 critically ill patients with ARF and CVVH. Fifty-two patients were subjected to lactate-based (group 1) and 32 to acetate-based hemofiltration (group 2). Thirty-eight patients had a septic, 46 a cardiovascular origin of the ARF. Creatinine, BUN, serum bicarbonate, arterial pH, lactate and APACHE II score were noted daily. Mean CVVH duration was 9.8 ± 8.1 days; mortality was 65%. The groups did not differ with regard to the main clinical parameters. Lacate-based hemofiltration led to significantly higher serum bicarbonate and arterial pH values as compared to the acetate-based hemofiltration. Baseline serum bicarbonate values were 23.3 ± 8.3 mmol/L in group 1 and 21.6 ± 4.3 mmol/L in group 2 (NS); values at 48 h after initiating CVVH treatment were 25.7 ± 3.8 mmol/L and 20.6 ± 3.1 mmol/L, respectively (p < 0.001). Arterial pH prior to CVVH treatment was 7.36 ± 0.1 in group 1 and 7.34 ± 0.1 in group 2 (NS), and 7.43 ± 0.07 versus 7.37 ± 0.06 (p < 0.001) on day 2. These findings were maintained throughout therapy. While a lack of increase in serum bicarbonate and arterial pH was correlated to a poor prognosis in lactate-based hemofiltration, no such observation could be made in acetate-based hemofiltration. Septic patients did not differ in their acid–base status from nonseptic patients. Lactic acidosis occurred in 8 septic patients irrespective of the substitution fluid. All 8 patients died. There was a significant increase in HCO3 and arterial pH values in lactate-based as compared to acetate-based HF.

A giant in the field: Albert Montgomery Kligman

all of this if one has no friends, particularly when one gets older. Living the lust of life with a full academic and scientific career, and being surrounded by warm friendship, that is particularly true for Albert Montgomery Kligman. Al, as his friends call him, has never left Philadelphia, Pa, where he was born on March 17,1916, except for his innumerable trips throughout the world. He went to medical school in Philadelphia, and was trained at the famous Department of Dermatology at the University of Pennsylvania. He became, and still is, an outstanding and famous teacher and scientist at this institution.1,2Philadelphia became a Mecca for pilgrims who searched for postgraduate scientific training in dermatology. The magnet is Albert Montgomery Kligman, who has attracted and educated generations of dermatologists. He is without a doubt a gi¬ ant in the field, as were others, like Stephen Rothman. His impact on dermatology is seen not only in the United States, but particularly in Europe, Asia, and South America. Albert has a particular relationship to European derma¬ tology. He has made enormous contributions to the univer¬ sities on the continent. Particularly deep roots connect the University of Pennsylvania with Germany. When the old and famous Duhring laboratory was still in place, Albert's small office was the navel of training in experimental der¬ matology. Although his academic children are numerous, I can only speak for his German offspring: Herbert Goldschmidt (Munich, now Philadelphia); Enno Christophers (Kiel); Reinhard Breit (Munich); Gerd Plewig (Dusseldorf); Ulrich Berendes (Weinheim); Erhard Holzle (Dusseldorf); Peter Frosch (Dortmund); Percy Lehmann (Dusseldorf); and Rainer Gollhausen (Munich). His academic honors are too numerous to mention. Un¬ like others, Albert is always good for seconds, not onlywhen it comes to food. He was surprised twice with a Festschrift. One was prepared in honor of his 65th birthday in a splen¬ did edition of the oldest academic journal of the German Dermatological Society, the Archives for Dermatologica! Research, founded in 1869 as Archiv fur Dermatologie und Syphilis1; and again with a Festschrift on the occasion of his 70th birthday, this time in the most prestigious Journal of Investigative Dermatology.' He has received both an MD and a PhD degree; and a Doctor Honoris Causa (DrHC) from the University of Utrecht, the Netherlands, in 1968. Albert was celebrated again and was awarded his second DoctorHonoris Causa by the Faculty ofMedicine, HeinrichHeine University of Dusseldorf (Germany).
