Treatment of pars planitis: I. Cryotherapy

Editor’s comment. Multiple designations for the same clinical entity usually betray ignorance of pathogenesis and confusion about therapy. Take, for example, “Mortimer’s Malady.” Jonathan Hutchison first described it in 1869 as “papillary psoriasis” of the hands, but after examining Mrs. Mortimer and nine other patients, his penchant for precision required “lupus vulgaris multiplex non-ulcerans et non-serpigeneous.” Besnier settled for the simpler “lupus pernio,” Caeser Boeck for “multiple benign skin sarcoid,” and Heerfordt for “uveo-parotid fever.” By 1916, Schaumann had identified these syndromes as manifestations of the same disseminated granulomatosis, “lymphogranuloma benigna.” Eponymic obligations have given us “Besnier-Boeck-Schaumann disease,” not to mention the six other men so honored. We now call Mortimer’s malady “sarcoidosis,” and still have identified neither etiology nor completely effective therapy. Peripheral uveitis tits this pattern: “pars planitis, angiohyalitis, cyclitis, peripheral uveoretinitis,” and we have been spared eponyms! Initially classified as a granulomatous uveitis in the extreme fundus periphery with multiple causes, the disorder has assumed its own identity over the past 25 years as an idiopathic, often bilateral, retinal vasculitis in young people which produces peripheral vitreous organization. Chronic recurrences produce secondary macular edema, posterior subcapsular cataract, glaucoma, and retinal detachment, and treatment often has been unsatisfactory. Drs. Aaberg and Schlaegel relate here their successful experiences with cryotherapy and systematic corticosteroids, respectively, and recommend practical guidelines for management. We commend their diligent long-term study of this imprecisely defined, unpredictable disease. The next step should be a randomized controlled clinical trial of therapy in pars planitis which, while difficult, could provide the ophthalmologist an even firmer basis for treatment. SARCOID ARTHRITIS. A REVIEW.

Jonathan Hutchinson, in 1877, in what is generally considered to be the first reported instance of sarcoidosis, remarked that, "I was inclined to consider the skin disease as essentially connected with gout."¹Twelve years later, in his classic description of uveoparotid fever, Heerfordt described an unexplained transient knee effusion in one of his patients.²In more recent times, Schaumann, whose name is often linked with that of Boeck in the nomenclature of sarcoidosis, described joint abnormalities in the disease.³Despite these classic reports, it was not until the last decade that arthritis has been considered to be more than a rare and coincidental finding in patients with sarcoidosis. Although many isolated cases had been noted previously,^4-16in 1952 Myers et al¹⁷were the first to describe in detail a series of patients with significant articular disease, and then, in 1959, Sokoloff and Bunim¹⁸reported their experience with sarcoid arthritis

Introduction The systemic nature of sarcoidosis has been recognized for many years,1and its cutaneous, visceral, and lymphohematopoietic manifestations are well documented.2Peripheral3and cranial nerve involvement are not infrequent, the latter being present in half of the patients with uveoparotid fever.4Invasion of the central nervous system, however, is unusual. It was first demonstrated pathologically in 1907, by Tillgren, in a case of sarcoid chorioependymitis.5Twenty-one clinically well-documented cases of sarcoidosis of the central nervous system have been published since in which the diagnosis was substantiated at necropsy and 7 cases in which it was confirmed by surgical biopsy. Of these 29 cases of central nervous system sarcoidosis, 10 clinically simulated brain tumor.5-14 The following case is of interest because it presented a clinical picture of multiple brain tumors associated with disturbances of the hypothalamic function. SARCOIDOSIS WITH INVOLVEMENT OF THE NERVOUS SYSTEM

SINCE Boeck¹in 1899 first described the skin lesions that have since come to bear his name, a large number of accounts of the disease have appeared in the literature. The disease is known by a variety of names, of which sarcoidosis, benign lymphogranulomatosis, and Besnier-Boeck-Schaumann disease are the commonest. The condition described by Heerfordt²under the title of "febris uveo-parotidea subchronica," later known as Heerfordt's syndrome, has now come to be recognized as a special manifestation of the same disease. The early history of the disease has been fully reviewed by Hunter,³Pautrier,⁴Gravesen,⁵Leitner,⁶and others. Although Boeck was the first to describe the histological characteristics of the skin lesions, Hutchinson (cited by Hunter³) had already published a clinical description of such a case in 1875 and of two others in 1898. It was Schaumann,⁷however, in 1914, who first

DESTRUCTION of the pituitary gland by giant-cell granulomata is a poorly understood, seldom recognized cause of pituitary insufficiency. While these granulomata are readily apparent pathologically, their cause is obscure. They have been considered to be sarcoid or tuberculosis. 1 A syphilitic causation has often been questioned. 2 Some have considered these lesions to be a distinct, specific disease of unknown origin. 3 It is probable that four etiological processes are involved in the reported cases of this disease. In the literature 16 cases have been associated with typical uveoparotid fever, osteitis multiplex cystica, lupus pernio, hyperglobulinemia, or other characteristics of Besnier-Boeck-Schaumann sarcoidosis. In some of these cases, furthermore, the asteroid and Schaumann bodies which are so suggestive of sarcoidosis have been demonstrated. 4 Occasional reported cases may represent a healed syphilitic process: In the case of Oelbaum and Wainwright the association of pituitary granulomata with the almost pathognomonic syphilitic lesions of interstitial keratitis and

SINCE Boeck 1 first published his original description of sarcoid, in 1899, approximately 1,500 cases have been recorded in the literature. The disease has been reported from many European countries and in increasing numbers from this country, where a high percentage of patients are Negroes. In general, the disease is a chronic, low-grade, granulomatous process of unknown cause, with fever, malaise, loss of weight, anorexia, and special symptoms or signs referable to involvement of one or more organs of the body. Common manifestations are lesions of the skin, enlargement of lymph nodes, parotitis, visual loss associated with uveitis (the last two being sometimes referred to as uveoparotid fever), and lesions in the lung, often found on routine chest films without symptoms. In the fifty years which have passed since Caesar Boeck (1) first described an apparently rare dermatologic condition which he chose to call “multiple benign sarkoid of the skin,” this disease has come to be recognized as a generalized systemic disorder of interest and importance not only to dermatologists but to physicians in virtually all branches of medical practice. On occasion sarcoid lesions apparently confined to the skin are encountered, but the consistency with which deep-seated sarcoidosis occurs with or without skin alterations warrants thorough investigation of all organ-systems of patients in whom the disease is suspected. The evolution of sarcoidosis as we now recognize it has been carefully documented by many writers, notably Hunter (2) and Longcope and Pierson (3). Their accounts of Hutchinson's “Mortimer's malady,” Besnier's lupus pernio, Boeck's multiple benign sarcoid, Heerfordt's uveoparotid fever, Schaumann's lymphogranuloma benignum, Jüngling's osteitis tuberculosa multiplex cystica, and certain instances of the Mikulicz syndrome, and how all these apparently specific diseases were gradually recognized as different manifestations of a single clinicopathologic entity constitute one of the most fascinating chapters in the history of medicine. Nor has the chapter yet ended, for not only do each few years bring reports of sarcoidosis involving portions of the body in which it has not hitherto been described and new tests for the determination of its presence (4, 5), but the etiology of the condition never has been satisfactorily established. Schaumann (6, 7), who made the first real attempt to correlate the various clinical and pathological manifestations of sarcoidosis, insists that the disease is primarily one of the lymphohematopoietic system, with or without involvement of the skin, and that the reporting of lesions in other portions of the body represents a failure to distinguish “what is essential and what is accidental.” Like Besnier (8), Boeck (1), and Jüngling (9), as well as the majority of modern authorities, Schaumann has consistently maintained that sarcoidosis is a peculiar manifestation of tuberculosis. One can find considerable support for this theory in the contributions of various laboratory research workers. For example, Jadassohn (10) has produced sarcoid-like lesions in the skin of rats by injection of tubercle bacilli, and Florence Sabin (11) and her co-workers have obtained non-caseating “hard” tubercles typical of sarcoid by injecting only the lipoid fraction of tubercle bacilli into laboratory animals. On the other hand, it has been found that sarcoid-like lesions can be produced locally by the injection of other organisms (12) as well as by numerous inert substances such as grass (12) and silica (13).

X-Ray Therapy of Boeck's Sarcoid

It has been generally accepted that sarcoidosis or benign lymphogranulomatosis is a specific disease of unknown etiology, of which uveoparotid fever is just another manifestation (8, 13). Although the etiologic agent is unknown, the tubercle bacillus has been accused most frequently with little direct evidence to support the suspicion that sarcoid may represent a peculiar form of benign tuberculosis. Snapper states that Besnier-Boeck's disease must be considered as a peculiar reaction of the reticulo-endothelium with formation of pseudo tubercles under the influence of an unknown virus (13). The disease runs a chronic, relapsing course and may produce comparatively mild constitutional symptoms but sometimes causes great damage to many structures. Reisner (12) stated that the organs involved, in the order of frequency, were the lymph nodes, skin, bones, spleen, liver, eyes, parotid gland, mucous membrane of the respiratory tract, lacrimal glands, and possibly the serous membranes. Involvement of the nervous system and also of the stomach has been reported. Fibrosis in the lungs may lead to myocardial failure, and lesions in the eye may result in permanent blindness. Treatment reported by various observers includes x-ray therapy, arsenic medication, ultraviolet and Finsen light, and esters of chaulmoogra oil, none of which has been successful. However, the tendency of the disease to undergo spontaneous regression makes evaluation of any therapy difficult. Roentgen therapy was used by Bernstein and Oppenheimer (1) in 4 out of 6 cases with encouraging results. They felt that the earlier phases of the disease were more likely to respond to x-ray treatment. Katz and co-workers (5) noted a reduction of hilar nodes in one case treated with x-ray therapy. Pohle (11) was impressed especially by the response of the mediastinal lesions to x-ray treatment. However, according to the literature, spontaneous regression is seen most frequently in the pulmonary and mediastinal lesions of sarcoid. Pohle stated that clinical improvement in nearly all cases preceded objective evidence of regression, which appeared two to four months after roentgen therapy was started. He urged radiologists to treat more cases of sarcoid with x-ray in order to determine with greater accuracy the efficacy of this type of treatment. Material Eleven cases of Boeck's sarcoid were treated in the Radiotherapy Department of the Presbyterian Hospital (New York). Of these, 10 were microscopically proved by biopsy. Although no biopsy was obtained on Case 3, the diagnosis was based on the radiological evidence in the chest and hands and on the general course of the disease. The accompanying table illustrates the inconsistencies of the disease. The ages range from twenty-two to fifty years. Six patients were women and 5 were men. Seven patients were Negroes and 4 were white.

Pulmonary Sarcoidosis: The Early Roentgen Findings

The pulmonary changes in sarcoidosis are now recognized as constituting one of the most conspicuous manifestations of this bizarre and no longer rare disease. They are usually described as being of one of the following types: (a) lymph node enlargement, (b) miliary or nodular lung densities, or (c) a combination of nodal enlargement and pulmonary infiltration. Are there any particular lesions suggestive or characteristic of early pulmonary sarcoid disease? In an attempt to determine this point, we studied a series of cases of sarcoidosis which we have been privileged to see during the last several years (33 of which were proved by biopsy or necropsy) and reviewed much of the now voluminous literature on the subject. Since sarcoidosis is a disease of protean manifestations and constant changeability, we believe it will be well to consider briefly its salient features before analyzing the purely pulmonic aspects. Clinical Aspects of Sarcoidosis Sarcoidosis is a generalized disease of unknown origin, in which characteristic histologic changes are found in different organs and tissues (17, 19). It is usually of insidious onset and tends to run a chronic, relapsing course. Fairly mild constitutional symptoms are manifested in most cases, and quite severe, even fatal, signs in a few. It is discovered most commonly between the ages of twenty and thirty years, but cases have been reported in patients as young as two months (19) and as old as eighty years (11). It may develop in persons of any race, but seems to occur with somewhat greater frequency in Negroes than in others (12, 13, 14). Although a widespread, disseminated disease, it is often observed in a phase when involvement of only one system is apparent; for example, lesions of the skin, the eyes, or the lungs. The clinical picture varies from case to case, and from time to time in the same case. The victims may have no complaint or may complain of fever, fatigue, or slight dyspnea. Peripheral lymphadenopathy is present at some stage of the disease in about 90 per cent of cases, the nodes being painless, discrete, and movable. Pulmonary involvement (parenchymal or nodal) occurs in a similar percentage of cases. Cutaneous and ocular lesions are seen in about 40 per cent of cases. The usual skin lesions are described as sharply defined, brownish nodules (cutaneous or subcutaneous) distributed over the face or extremities (12). The common ocular finding is a chronic bilateral granulomatous iritis, or iridocyclitis, without much redness or pain. This may regress spontaneously or progress to complete blindness. Hepatomegaly and splenomegaly are fairly frequent. Cardiac and pericardial involvement has been noted. As in Hodgkin's disease, any tissue or structure can be invaded. Infiltration of the salivary apparatus may lead to “Mikulicz's disease,” and of the parotid glands and eyes to “uveo-parotid fever.”

Editorial—Pulmonary Sarcoidosis

Sarcoidosis is a chronic indolent disease of unknown etiology. It may affect every tissue or organ of the body, or it may be limited to certain groups, such as the parotids and eyes, or the lungs and lymph glands, or the skin and bones. It was originally described as a skin lesion (lupus pernio), by Jonathan Hutchinson who called it Mortimer's malady, Mortimer being the name of the original patient. Another manifestation in the eyes and parotids was described as uveoparotitis or Heerfordt's syndrome, while the bone changes go under the name of Jüngling's disease or osteitis tuberculosa multiplex cystica. Identification of a similar underlying pathology for these varied syndromes was made by Boeck, Besnier, Tenneson, and Schaumann, all of whose names have been attached to the disease. Fortunately sarcoidosis is now accepted as a universal name.

Approximately 100 cases have been reported in the literature, the majority in Scandinavian and German journals of ophthalmology. No previous case has been so diagnosed at the Wisconsin General Hospital. The essential features are bilateral inflammatory lesions of the uveal tract and bilateral parotitis, often associated with paralyses of the cranial nerves, usually the peripheral portion of the seventh. A rather comprehensive review of the literature reveals that approximately 100 such cases have been reported, less than 20 of which come from America and the remainder from foreign countries, chiefly Scandinavia, England and Germany.
