Identification of Occupational Safety and Health Hazards at Paint and Welding Workshop using the HIRARC Method

Occupational Health and Safety (OHS) is important for every company to pay attention to. Preventing work accidents can be done by knowing the existing risks, one of which is through the Hazard Identification, Risk Assessment, and Risk Control (HIRARC) method. Body Repair, Paint, and Welding Workshop "Family 99" is a small to medium-sized entrepreneurship that operates in car repair, paint, and welding services. The research aims to analyze the potential dangers found in the Body Repair, Paint and Welding Workshop "Family 99". Hazard identification was conducted in the workshop work area, with research respondents totaling three active workers. The data collection employed observation and interviews with a descriptive approach. According to the research results obtained from a total of 14 work stages analyzed, two stages had the highest risk value, namely the paint peeling stage with a risk value of 12, and the first, second, and third sanding stages with a risk value of 9. Controls that can be carried out to reduce risks include administrative controls and the use of Personal Protective Equipment (PPE) for workers to work safely and comfortably.

Risk Analysis and Occupational Health and Safety Assessment Series (OHSAS) at Hospital X Surabaya

A hospital is a facility that generally raises the degree of public health. More and more human involvement makes hospitals have great potential to cause harm. To control the risk of accidents, it is necessary to identify hazards that exist in the workplace and evaluate the level of risk and control. One of the first stages in risk management is HIRARC (Hazard Identification Risk Assessment and Risk Control). The existence of HIRARC makes it easier to compile a risk register or hospital risk register in each work unit. This study aims to analyze the application of the Occupational Health and Safety Management System (OHSAS) at Hospital X Surabaya based on HIRARC. This type of research uses qualitative research with a research sample of 78 respondents. Qualitative variables in this research used for analyze risk management and identify risk control through the process of observation and interviews. Qualitative data analysis is carried out through identification of initial conditions, exposure of interview results and qualitative content analysis. The results of the study stated that the implementation of occupational safety and health management has been carried out at Hospital X Surabaya. OHS risk management is very important in preventing workplace accidents and improving performance. In this study, the HIRARC approach (Hazard Identification, Risk Assessment and Risk Control) found three levels of risk in hospitals, namely high risk, moderate risk and low risk. The results showed that the implementation of OHS risk management was used to identify, evaluate, and control OHS risk. The thing that cannot be avoided from work activities is a risk. Based on this, it is expected that there will be an increase in understanding and awareness related to OHS in hospitals, the application of risk control, routine evaluations and good cooperation and coordination between the OHS team and health workers for the implementation of OHSAS

Penerapan Manajemen Risiko Kecelakaan Kerja pada Proyek Penggantian Jembatan Tukad Ayung Denpasar, Bali

Management of occupational accident risk involves systematic, planned, structured, and comprehensive efforts to mitigate the factors that contribute to the occurrence of work accidents. This study aims to examine the risk of work accidents in the Tukad Ayung Bridge replacement project, especially at the stage of installing concrete girders. Qualitative and quantitative data from primary and secondary sources were used, collected through observation, interviews, questionnaires, literature review, and documentation analysis. The Hazard Identification, Risk Assessment, and Risk Control (HIRARC) method is used for data analysis. The findings revealed 24 types of work accident risks at seven stages of concrete block installation in the project including the concrete girder preparation stage, girder truss making, erection (setting segmental girder), prestressed steel installation (strand), cabel strand stressing work, concrete girder tendon grouting, and diaphragm installation stage. The results of the risk analysis on the concrete girder installation work on the Tukad Ayung Bridge replacement project, are in the moderate category, which is as many as 12 risks. Furthermore, as many as 11 risks are included in the high category. Meanwhile, in the extreme category as much as 1 risk.

ANALISA RESIKO KESELAMATAN KERJA KARYAWAN PRODUKSI SEPATU DIVISI NIKE DENGAN METODE HIRARC (HAZARD IDENTIFICATION, RISK ASSESSMENT, AND RISK CONTROL) DI PT. NIKOMAS GEMILANG

Occupational Safety and Health (K3) is an issue that must be considered by all organizations, for example in industry. Based on observations and interviews of the production department at the Nike Division of PT. Nikomas Gemilang, There have been work accidents ranging from light to severe categories. The Hazard Identification, Risk Assessment and Risk Control (HIRARC) method in the production process with hazard identification steps (Hazard Identification), risk assessment (Risk Assessment) classifies based on the severity of the hazard posed (Severity). The last step is to multiply the Likelihood value and Severity value that will be mapped into the Risk Matrix. Hazard identification using Job Safety Analysis (JSA) 33 potential hazards were obtained in the production section at PT. Nikomas Resplendent Nike Division. There are 3 types of hazards that exist in the entire production process, namely: ergonomic hazards, physical hazards and chemical hazards. From the results of the risk assessment, it was found that the risk in the Low category was 11 (34%) potential, while in the Medium category there were 10 (30%) potentials, the High category as many as 8 (24%) potentials and the Very High category 4 (12%) potentials. Control that can be done to reduce risk is the use of Personal Protective Equipment (PPE) for workers, so that workers can safely and comfortably work and are installed warning signs in every corner of the room to prevent accidents

Analisis K3 pada Mesin Milling, Mesin Bubut dan Ruangan Produksi di PT. Z Menggunakan Metode HIRARC

Occupational safety and health play a crucial role in maintaining productivity, sustainability, and the reputation of a company. Unsafe or unhealthy working conditions can lead to workplace accidents, injuries, or even fatalities. Additionally, the costs associated with work-related accidents or illnesses can be significant, including medical expenses, workers' compensation, and lost productivity. Therefore, it is important for companies to have an effective occupational safety and health program to protect their employees. One method used to analyze and manage occupational safety and health risks is HIRARC (Hazard Identification, Risk Assessment, and Risk Control). In this report, we will discuss the background of occupational safety and health and explain the steps of HIRARC in identifying, assessing, and controlling occupational safety and health risks in the workplace. The HIRARC method serves as an effort to prevent or minimize workplace accidents and includes a combination of Hazard Identification, followed by Risk Assessment to evaluate potential hazards in milling machines, lathe machines, and production areas, and finally Risk Control to implement measures that can minimize workplace accidents in the working environment. Work accident risk analysis using HIRARC and FTA methods (Case study: Suwarno Meubel)

Suwarno Meubel is a business engaged in furniture production, which has risks related to occupational health and safety including workers carrying heavy wood manually, cutting wood with a table saw, painting wood with a spray gun, and so on. The purpose of this study is to find out the potential hazards that exist in each job and find the root causes of work accidents and then provide suggestions for improvements. The research method used is Hazard Identification Risk Assessment and Risk Control (HIRARC) to identify hazard risks and risk assessments for each job, after that, jobs with extreme values will be further analyzed using the Fault Tree Analysis method (FTA). The results of this study found 9 low categories of hazard risks, 12 moderate hazard categories, 4 high hazard categories, and 1 extreme hazard category. Risks in the extreme category were analyzed and found that there were 7 basic events, namely fatigue, lack of focus, lack of training, a limited number of PPE (goggles), no safety equipment on the machine, and no work procedures. The proposed controls are in the form of administrative control, PPE, and engineering.

Analysis of Occupational Safety and Health Cost Plan Construction on The Project “Preservasi Jalan dan Jembatan Surabaya – Sidoarjo”

Indonesia is one of the developing countries which currently has a lot of development underway. Problems related to occupational health and safety are still often ignored, especially in East Java province, this is demonstrated by the still high number of construction work accidents in 2021 in East Java province, namely 26% of the total construction work accident cases in Indonesia. This research was carried out on the project “Preservasi Jalan dan Jembatan Surabaya – Sidoarjo”. The method of this research is hazard identification risk assessment and risk control (HIRARC) and quantitative descriptive method. The results of this research show that the amount of occupational health and safety costs is based on the owner's estimate which is guided by the Minister for Public Works and Housing Regulation Number 10 of 2021 concerning Construction Safety Management Systems is IDR. 664,667,000.00 where The value of 4,04 % of occupational safety and health costs has met the ideal standard from circular letter number 10/SE/M/2018 of the Minister for Public Works and Housing that is between 1.5% - 2.5% of the total project value.

Analysis of Health and Safety in The Production Floor of CV. XYZ Using Job Safety Analysis Method and the Hazard Identification Risk Assesment and Risk Control Approach

Occupational Health and Safety (OHS) is a critical factor in the smooth operation of a company, involving machinery, tools, the work environment, raw materials and their processing, as well as the employees themselves. The company's issues regarding OHS culture and the inadequate use of Personal Protective Equipment (PPE) can lead to the risk of workplace accidents when employees perform their tasks. This research utilizes the Job Safety Analysis (JSA) method, which plays a role in risk identification, and the Hazard Identification, Risk Assessment, and Risk Control (HIRARC) approach to determine the direction for the implementation of risk management and OHS in the company. This study focuses on the production process of CV. XYZ, with 35 employees. Primary data were obtained through direct observation and interviews, which were then analyzed based on potential hazards discovered. The hazard identification results in the production floor revealed a total of 71 hazards, which include 23 potential hazards in the casting process, 26 potential hazards in the machining process, and 22 potential hazards in the finishing process. Based on these potential hazards, they can be classified into risk categories, including 27 (38%) potential hazards categorized as low risk, 30 (42%) potential hazards categorized as moderate risk, and 4 (6%) potential hazards categorized as high risk. Risk control measures include elimination, substitution, technical controls, administrative controls, and the use of PPE.

ANALISIS RISIKO KERJA PADA PROSES PEMBUBUTAN MENGGUNAKAN METODE HIRARC

Poor management of occupational safety and health (OSH) has an impact on occupational accidents. This results in losses for both the individual worker and the company. Therefore, the occupational hazard accident risk analysis was carried out using the HIRARC (Hazard Identification Risk Assessment and Risk Control) method. Data collection was conducted from the business owners of SSBC (Sanggar Sarana Baja Cakra) Welding and Turning Shop, located in Cakranegara, Mataram, West Nusa Tenggara. There are 3 work activities for risk identification, namely the clamping process, the engraving process and the drilling process. The results of the analysis show that the highest level of occupational risk occurs during clamping and engraving process. For drilling, it's in low level of risk. As for risk management, it is mainly done for high-risk level. Immediate improvement is needed by providing personal protective equipment (PPE) to workers to reduce these risks. Based on these results, business owners can do evaluation on his business so occupational accidents can be in the workplace.

Risiko Proyek Konstruksi pada Tahap Pelaksanaan Pembangunan Jalan Tol (Studi Kasus: Proyek Pembangunan Jalan Tol Solo – Jogja)

The Solo – Jogja Toll Road Development Project is a large-scale project with a large potential risk, especially at the project implementation stage. This research aims to find risks, obtain the level of risk by conducting a risk assessment, and recommend control measures against possible risks. The research method used in this study is HIRARC (Hazard Identification, Risk Assessment, and Risk Control). Purposive sampling was used to collect data from 30 respondents using a questionnaire based on certain criteria. The testing of this research instrument is in the form of validity and reliability tests using SPSS. In this research, 5 risk groups were identified during the implementation stage of the Solo - Jogja Toll Road Construction Project, including project risk, technical risk, natural risk, human risk, and occupational health and safety risk. The results of the analysis explained that the project has a risk range from low to high levels. The risk included in the high-level category is the risk of damage to roads around the project due to heavy equipment, such as dump trucks, project materials carriers, and trucks mixer. The risk can be minimized by creating a safe project access road, so that project activities do not cause further damage to the road.

Identifikasi Bahaya dengan Metode Hirarc Pada Kegiatan Hose Connection di Departement Jetty PT XYZ

The Jetty Department is a department that supports production because it is the initial point of entry for raw materials for production activities which are brought by a fleet of ships from outside Java. In the Jetty Department there are many types of work that have a risk of danger, one of the jobs that will be identified as dangerous in this research is hose connection activities. This research was carried out to identify hazards or dangers that could occur to workers, assess the risks, and finally be able to control the risk of danger using the HIRARC (Hazard Identification, Risk Assessment, and Risk Control) method. The research results show that in the risk assessment, there are 4 Low Risks, which means the risk is still acceptable, 5 Moderate Risks, which means the risk is still acceptable but needs additional risk control, and 3 high risks, which means the risk is not accepted, the risk needs to be reduced immediately. The proposed risk is to always use PPE that is appropriate to the job, and use work support tools such as supports and heavy forklifts. As well as always focusing on carrying out work, you must always be alert and responsive to areas or work environments that can pose risks and have good mutual coordination or communication between related workers.

ANALISA KESEHATAN DAN KESELAMATAN KERJA (K3) MENGGUNAKAN METODE HAZARD IDENTIFICATION RISK ASSASSEMENT AND RISK CONTROL (HIRARC) PADA PEMBANGUNAN GERBANG BANYURAN TANJUNG JATI UNIT 5 DAN 6 DI PT. SUPRACO INDONESIA

HSE is a system that must be owned by every company to ensure that operational activities carried out do not harm anyone. HSE is also known as K3, namely occupational health and safety. HSE itself stands for health, safety and environment. In order to achieve the objectives of the K3 program, various elements and resources need to be used, so that in the application of K3 it needs to be organized, and regulated into a K3 management system (SMK3). This study uses the Hazard Identification Risk Assessment and Risk Control (HIRARC) Method is a series of hazard identification processes in routine and non-routine activities. HIRARC is an effort to prevent and reduce the potential for work accidents, avoiding and minimizing risks that occur appropriately by avoiding and minimizing the risk of work accidents and controlling them in order to carry out the process of activities so that the process becomes safe. Hazard identification, risk assessment and control are part of the risk management system which is the basis of the Occupational Health and Safety Management System (SMK3), which consists of hazard identification, risk assessment and risk control.

Implementation of the Hazard Identification, Risk Assessment and Risk Control (HIRARC) Method on Erection Girder Work of South Japek II Toll Road Construction Project Package 3

The development of infrastructure is progressing rapidly, particularly for National Strategic Projects, such as the construction of the Jakarta-Cikampek Toll Road (Japek) South II Package 3, which is currently in progress. This toll road project also includes the construction of several interchanges that utilize girders in the process. The project commenced during a challenging time when Indonesia was facing the adverse effects of the Covid-19 pandemic, adding an additional obstacle to infrastructure work. Girder Erection Work, in particular, has recorded various types of accidents over the past few years. In response, the Ministry of Public Works and Public Housing has issued Ministerial Regulation No. 10 of 2021, which provides guidelines for Construction Safety Management Systems (SMKK). By employing the Hazard Identification, Risk Assessment, and Risk Control (HIRARC) method, it is anticipated that construction practitioners can effectively manage risks and reduce workplace accidents in this project. Additionally, the Probability Impact Matrix (PIM) will be utilized to identify the most likely hazards and risks that could significantly impact the project's implementation in the event of an accident. In this study, out of the 57 variables validated by experts, the respondents identified 17 risks classified as high-risk and determined 8 main/dominant risks in the girder erection work of the Jakarta-Cikampek Toll Road Japek South II Package 3 Project. Subsequently, control measures and mitigations are planned for these 8 main risks.

Hazard Risk Analysis in the Laboratory of Qolbu Insan Mulia (QIM) Hospital Using the Hazard Identification Risk Assessment and Risk Control (HIRARC) Approach

Activities in hospitals have risks originating from physical, chemical, biological, ergonomic, psychosocial, and waste factors. Qolbu Insan Mulia Hospital is a health service facility where sick and healthy people gather, which allows work-related accidents and occupational diseases to occur. The laboratory of Qolbu Insan Mulia Hospital is part of the medical support services, which provides hematology, serology, immunology, urine, blood chemistry, drugs, endocrinology, faces, blood transfusion, tumor marker, microbiology, and electrolyte testing services. This study aimed to determine K3 risk management using hazard identification risk assessment and risk control (HIRARC). This research is qualitative research. The techniques used in data collection were field observation, document review, and in-depth interviews. The research results show that in the laboratory there are 17 sources of potential danger consisting of physical, chemical, biological, ergonomic, mechanical, electrical, psychosocial, and waste hazards. In conclusion based on a high-level risk level of 71%, medium 23%, and low 6%, while the control efforts are in the form of administration, manipulation, and PPE.

AN EVALUATION OF OCCUPATIONAL SAFETY AND HEALTH HAZARDS AMONG FISHERMEN IN PONTIAN BESAR, JOHOR

Malaysian fisheries industry has dynamic and competitive development and it is not obtained without the support and sacrifices by fishermen as a backbone of this sector. This study was carried out to assess the occupational safety and health hazards of fishermen in Pontian Besar, Johor. The objectives of this study were to evaluate the current state and to propose the prevention measure for occupational safety and health issues among fishermen. The respondent consist of registered fishermen and unregistered fishermen. The tools of the research used are questionnaire and Hazard Identification Risk Assessment and Risk Control (HIRARC) form which involves mixed method. The researchers found twenty hazard identification based on job task of the fishermen that exposed to some hazards such as ergonomic, biological, physical, psychosocial, and natural. Six out of twenty hazards (30%) are considered as low risk and seven out of twenty (35%) respectively are recognized as medium and high risk. The results of the study shows that fishermen are highly exposed to ergonomic hazard. All the suggestions for the prevention measures for the risk control of the hazards are from the officers of Pontian Area Fishermen’s Association. This research is expected to raise awareness of the occupational safety and health hazards among fishermen and will be referenced to future researcher, policy makers, and other parties to take part in development of future plans.

Potential Risk of Work Accident in The Production Process of Gudeg Chicken using The Failure Mode and Effect Analysis (FMEA) Method

Every agricultural industry has a potential risk of work accidents, one of which is at the Micro, Small and Medium Enterprises (MSMEs) level in Yogyakarta. Gudeg producers as one of the MSMEs in the traditional culinary also have various types of work accident risks in their production process, including in the process of producing Gudeg Chicken. Overall, the process of producing Gudeg Chicken includes sorting, washing, cooking, and distribution. The results of the initial observations indicated that workers in sorting division had the complaints of back pain and several work accidents have occurred in the production division of Gudeg Chicken. Hence, it is deemed necessary to analyze the risk of work accidents occurred. The analysis of work accident risks was done using the Failure Mode and Effect Analysis (FMEA) method by considering severity, occurrence, and detection values to obtain a Risk Priority Number (RPN). The results were then analyzed using a Pareto diagram to determine the work accident with the highest risk. Recommendations for improvement were given using the Hazard Identification Risk Assessment and Risk Control (HIRARC). The results showed that the highest risk of work accidents was related to the position of sitting that is not ergonomic in the process of sorting Gudeg Chicken. The elimination of these risks does not need to be carried out, but it needs to do substitution of production areas, engineering controls with modifications towards the tables or chairs of employees, administrative controls, and the use of Personal Protective Equipment.

Risk Analysis of Occupational Health and Safety Using Hazard Identification, Risk Assessment and Risk Control (HIRARC) Method (Case Study in PT Barokah Galangan Perkasa)

PT Barokah Galangan Perkasa is a company located in East Kalimantan that is engaged in the manufacturing, repair, and maintenance of ships. The welding activity is the most dominant in shipbuilding, but the implementation of the occupational health and safety (OHS) system is not optimal. There have been several work accidents in the upper accommodation and cargo oil tank areas, such as falling from a height, being hit by welding sparks, and an electrical short circuit. The working conditions in this area are quite risky. Therefore, research on risk analysis is needed. HIRARC is a method for identifying a problem that is assessed at risk based on the likelihood and severity where the results of the assessment can determine the level of risk and give risk control for each work activity. Research results show that there were 40 potential hazards in the upper accommodation area with 77% low risk level, 12% moderate risk level, 8% high risk level, and 3% very high risk level. While in the cargo oil tank area there was 37 potentials hazard with 84% low risk level, 13% moderate risk level, and 3% high risk level. By knowing the risk level, companies can identify work activities that require prioritized improvement. They can also determine appropriate control to prevent the occurrence of such risks. Risk control in the upper accommodation area can be achieved through the implementation of administrative controls, utilization of PPE, and technical engineering controls.

KAJIAN PENGENDALIAN RESIKO KESELAMATAN DAN KESEHATAN KERJA DENGAN METODE HAZARD INDENTIFICATION RISK ASSESSMENT AND RISK CONTROL ( HIRARC ) DI PT. LANGKAT NUSANTARA KEPONG

PT. Langkat Nusantara Kepong is one of the KSO companies engaged in the oil palm plantation business. In carrying out the operational activities of one PKS unit, PT. Langkat Nusantara Kepong is supported by more than 100 workers who interact directly with machines and other work equipment. Every workplace contains a high potential for hazards, so a prevention and control effort is needed to prevent work accidents. The occurrence of work accidents is caused by the actions of people who do not comply with work safety (unsafe actions) and environmental conditions or processes from unsafe systems (unsafe conditions). Hazard Identification Risk Assessment and Risk Control (HIRARC) is an effort to prevent work accidents and OHS risks. This research was conducted using a qualitative descriptive method which describes the identification and assessment of work risks. Data collection regarding identification and risk assessment was analyzed with HIRARC. HIRARC Results at PKS Gohor Lama PT. Langkat Nusantara Kepong that the highest number of risk levels is at the boiler station.

Analisis Identifikasi Bahaya K3 dengan Metode HIRARC pada Unit Recycle Industri Tekstil Nonwoven

Occupational Safety and Health (K3) is a program created by employees and employers as an effort to prevent work-related accidents and diseases by identifying things that have the potential to cause work-related accidents and diseases as well as anticipatory actions in the event of work-related accidents and diseases. The method used in this study is the HIRARC (Hazard Identification, Risk Assessment, and Risk Control) method. This method consists of a series of OSH implementations which include hazard identification, risk assessment, and determination of control measures based on the data collected. The results of the risk assessment are used as a reference in the implementation of risk control. This study uses surveys and interviews to collect data to identify potential hazards in the recycle unit, risk assessment and risk control are carried out by observing and identifying as well as interviewing workers and company owners in the recycle unit. From the results of the identification of the recycle unit that has been carried out, there are several potential hazards that can be identified and the risk assessment and risk categories in the recycle unit can be identified. Based on the results of the study, there were 31 potential hazards in the recycle unit, of which 19.3% were in the low risk category, 61.2% were in the moderate risk category, and 16.6% were in the high risk category. Identification of hazards carried out in the work process in the recycle unit, potential hazards contained in the recycle unit, namely; exposed wires, inhalation of dust, exposure to noise, exposure to hot engines, and exposure to hot polymer spills.

Occupational Health and Safety Risk Analysis Using AS/NZS Standard 4360:2004 in a Fish Meatball Industry

The Indonesian fishing industry developed by 1.4% in 2020, compared to the previous year, as shown by the existence of around 773 seafood factories. Such growth caused the emergence of different working conditions due to the use of complex machines. In 2019, the industrialization process generated an immense potential for accidents on occupational health as the mishaps increased from 60 to 270 cases. For this reason, this study aimed to identify the value and level of risk, using HIRARC (Hazard Identification Risk Assessment and Risk Control) following AS/NZS 4360:2004 standards. The study was conducted at the fish meatball company located in East Java, for six months. The results showed that the company could manage the occupational health and safety, as indicated by the reduction of the levels of risk from acceptable (33.4%), priority 3 (14.3%), priority 2 risk (38.1%), priority 1 risk (9.5%), and very high risk (4.8%) to acceptable (76%) and priority 3 (24%). The risk assessment matrix had shifted from yellow (moderate) to green (low). Some recommendations implemented in the workplace included creating a safety organization, organizing training sessions among employees, fostering a safety culture, applying ergonomic principles, and controlling work hours.
