Suture Suspension Arthroplasty for Thumb Carpometacarpal Arthritis Reconstruction: Long-term Outcome: Level 4 Evidence

A simple reconstructive method for treating advanced arthritis of the thumb carpometacarpal (CMC) joint with suture suspension arthroplasty (SSA) is a reliable, low morbidity procedure, providing a simpler alternative with similar long-term outcomes, compared to trapeziectomy procedures, with or without ligament reconstruction. SSA technique for thumb CMC arthritis reconstruction consists of trapeziectomy via a single volar incision and a four-strand intra-articular suture (0 gauge non-absorbable) suspension sling, creating a “hammock” between the distal-most insertions of the abductor pollicis longus and the flexor carpi radialis. This construct is pulled firmly, tethering the base of the thumb towards the base of the index metacarpal, correcting the subluxation deformity and maintaining arthroplasty space. All procedures were performed by the senior author, averaging 22 minutes operative time, followed by 14 days of post-operative immobilization, then short opponens splinting for four weeks and rehabilitation protocol (1DelSignore J.L. Long-term evaluation of 117 thumbs (average 89.4 months) with strength measurement and QuickDASH questionnaire, were analyzed with a paired t test. Final Xrays were obtained. 117 out of 153 thumbs in 110 patients (76% recall) were available for long-term follow up averaging 89.4 months (80-104 months). 96 patients were female, 21 male. Statistically significant (P < 0.05) improvement in pain relief, patient satisfaction, grip, key and tip pinch strength, and functional improvement was noted in the majority of patients. Average QuickDASH score (0.3) at final follow up indicated good to excellent satisfaction and functional use in the majority of patients. Xray evidence of subsidence averaged 30% (0-90%). One patient (0.8%) required revision surgery. Flexor carpi radialis rupture occurred in 7 (6%) thumbs after a sudden stress activity around 4-6 weeks post op, requiring no additional treatment. •Suture suspension arthroplasty for thumb CMC arthritis reconstruction is a simple, reliable method. Patient advantages include lower morbidity as a result of a single incision without tendon sacrificing, and avoidance of potential complications related to multiple incisions, tendon harvesting, drill hole formation, hardware implantation, pin fixation, or prolonged immobilization. Most patients are able to resume normal, unrestricted use at 12 weeks post-operative.•This method is cost-effective, utilizing less operative time, and does not require additional cost of implants or anchors.•Long-term results of suture suspension arthroplasty show similar outcomes as described with other methods of trapeziectomy, with or without ligament reconstruction (2Vermeulen G.M. Evaluation of In Vivo Properties of Velosorb™ Fast Versus Vicryl™ Rapide in a Functional Rabbit Model of Intradermic Suture and Episiotomy

ABSTRACTObjective: Fast-absorbing polyglactin 910 is commonly used to suture skin and vaginal incisions. In vitro studies have shown similar tensile strength properties between Velosorb™ Fast and Vicryl™ Rapide (Al-Qattan MM. J Hand Surg Br Eur. 2005;30(1):90–91). This randomized study compared Velosorb™ Fast (Covidien) with Vicryl™ Rapide (Ethicon) in a rabbit model of intradermic suture and episiotomy. Materials and Methods: This randomized prospective study was performed on 20 New Zealand rabbits, in which we realized a mediolateral perineo-vaginal incision and two vertical skin incisions of about 8 cm on the abdominal wall. The sutures were randomized between Velosorb™ Fast (Covidien) and Vicryl™ Rapide (Ethicon), and each rabbit was treated with the same suture on both vaginal incision and left abdominal incision, while the right abdominal incision was closed with an alternate suture. A macroscopic examination was performed on days 5 and 11. On day 11, animals were sacrificed for histological (Badylak quantitative score) and electron microscopic examinations. Differences were considered significant at p < .05. Results: All of the rabbits survived and were included in the final results. Fifty-one sutures were visible by histologic analysis (27 in the Velosorb™ Fast group and 24 in the Vicryl™ Rapide group). The power of the study was ≥80%. There was no significant difference between the two groups based on the total histologic Badylak score analysis or the Badylak sub-score analysis. Conclusions: Velosorb™ Fast seems to be as efficient as Vicryl™ Rapide for use in perineal and skin closure.

Performance Characteristics of the Verbal QuickDASH

To quantify the performance of the verbally administered Quick Disabilities of the Arm, Shoulder, and Hand (QuickDASH) questionnaire by assessing its replication of self-administered scores, its test-retest reliability, and its rate of scorable completion compared with its self-administered, written administration. Fifty patients presenting for initial visits to a hand clinic were enrolled regardless of diagnosis. All patients completed a written and a verbal QuickDASH 1 day apart (25 patients written first; 25 patients verbal first). Intraclass correlation coefficients quantified the verbal questionnaire's ability to reproduce written scores. Participants verbally completed the questionnaire a final time, 5 months later, to assess test-retest reliability. To quantify the usability of survey data, we compared percentages of scorable surveys between written and verbally administered QuickDASH questionnaires in this study and in prior studies within our division. The intraclass correlation coefficient between the 2 QuickDASH administration types for the entire sample was 0.91. Across all participants, there was a minimal change in mean score from a patient's written QuickDASH to that patient's first verbal QuickDASH score. Scoring consistency between QuickDASH administrations was similar for each administration sequence (phone followed by written vs. written followed by phone) and by diagnosis. Test-retest reliability between the 2 verbal administrations demonstrated good reliability and a minimal difference between scores. In this study, no written or verbal surveys were incomplete. Reviewing our practice, 17% of 258 written questionnaires produced unscorable data compared with 0% of 239 verbally administered surveys. Our results indicate that verbal administration of the QuickDASH replicates clinically relevant scores of the written QuickDASH, has good test-retest performance, andmay minimize unusable data. These data allow researchers greater flexibility in gatheringpatient outcome data in both retrospective and prospective studies. Diagnostic II.

Citation Accuracy for Scientific Articles Published in Journal of Hand Surgery (American) in 2011: Not a clinical study

Previously, reference listings in the Journal of Hand Surgery–American (J Hand Surg Am) were reviewed for accuracy and improvement was associated with a more stringent reporting process at the time of manuscript submission.1 The accuracy of citations in supporting a statement of fact, however, has not been evaluated. The purpose of this study was to evaluate the accuracy of citation listing and the ability of the reference to support a statement in scientific articles published in J Hand Surg Am in 2011.
