Doctor-patient communication: a study of junior house officers.

This study evaluates junior house officers' perceptions of their communication skills with cancer patients; the usefulness of their undergraduate communication skills training; and their sources of emotional support. All 42 junior house officers employed at Guy's and Lewisham Hospitals in August 1994 were interviewed using a study-specific, semi-structured interview. Sixty-seven per cent of junior house officers felt they had adequate communication skills in relation to medical issues, but only 36% felt they had adequate skills in relation to psychological issues. Thirty-one per cent of doctors reported that they never, or nearly never, enquired about the emotional adjustment of dying patients. Lack of time was the most commonly reported reason for avoiding asking about psychological problems (62% of junior house officers), followed by wishing to avoid awkward questions (51%) and inadequacy of skills to deal with such issues (44%). Ninety-eight per cent of junior house officers had attended the 1-week undergraduate communication skills course at Guy's and St Thomas's Hospital Medical School (UMDS). Sixty-seven per cent of those who had attended found the course helpful and 62% felt they would benefit from further training as junior house officers. Seventy-four per cent felt they could discuss their work-related concerns with colleagues and 95% felt they could talk to friends. In contrast only about 9% felt they could, if needed, talk to a counsellor. Although the majority of the junior house officers reported benefit from their communication skills training, the course does not appear to be meeting all their communication training needs. Junior house officers require further training opportunities at the undergraduate and postgraduate levels. Traditional counselling services for junior house officers may not be meeting their support needs.

Under any new contractual arrangements core and non-core services should be separated and the in hours and out of hours parts of the contract should be split. These decisions emerged after last week's meeting of the General Medical Services Committee, which was devoted to discussing various options in open forum and in working groups. As a result of the 1990 general practitioner contract and the NHS reforms there is increasing support among general practitioners for other contractual options. And the chairman of the General Medical Services Committee, Dr Ian Bogle, has always insisted that the recent out of hours agreement was only an interim measure. Each of the eight working groups completed a questionnaire, which included questions on the independent contractor status, salaried employment, commissioning for the provision of general medical services, and patient access to general practitioner services, as well as one on priorities for negotiations--separation of core and non-core services came top of the list. The results of the day's deliberations will be drawn together and taken back to the GMSC before being sent out to local medical committees (LMCs) for consultation. Several members supported the idea of a special LMC conference in the spring of 1996, but Dr Bogle said that the important thing was to get the document right. The debates were helped by contributions from the professor of health policy at Imperial College, University of London, Professor Nick Bosanquet, and the visiting professor of general practice at Guy's and St Thomas's Hospital Medical School, Professor Marshall Marinker. The development of forensic medicine in the United Kingdom from the 18th century.

Forensic medicine in the United Kingdom includes both forensic pathology and clinical forensic medicine on the living. It began at the end of the 18th century, long after its development in Germany, Italy, France, and other countries in Europe. Initial beginnings were in Scotland, where a program began at the University of Edinburgh with the establishment of a chair in Forensic Medicine by Prof. Andrew Duncan Sr. The development in England began in London's Kings College Medical School with a chair held by Prof. William A. Guy. Later chairs in Forensic Medicine were established in Glasgow, Aberdeen, and in London, where Forensic Medicine was taught at St. Mary's Hospital Medical School, Guy's Hospital Medical School, London Hospital Medical School, Charing Cross Hospital Medical School, St. Thomas Hospital Medical School, and St. George's Hospital Medical School. In other cities in England, Wales, and Northern Ireland, departments were founded in Leeds, Manchester, Cardiff, and Belfast. Many textbooks were prepared during this time by professors from these medical schools and by others working in nonacademic areas. The development of coroner activities and those of the police surgeons is also part of the study of forensic medicine.

Teaching medicine in general practice: the Guy's experience.

The development of teaching in general practice at Guy's Hospital Medical School is described. Important features of the current programme (a new programme for the United Schools of Guy's and St Thomas' Hospitals will come into effect this year) are the emphasis on learning directly from patients and the active role and responsibility given to clinical students. Students welcome the opportunities to see patients first, to deal with undifferentiated problems, to work with one clinical teacher, to put to use knowledge and skills and to test themselves as clinicians. In these circumstances they gain confidence and display the human qualities required of doctors. An acceptable service to patients, the essential basis for effective clinical teaching, requires the general practitioner teachers devote more of their time to service than to clinical teaching.

Synthesis of two components of human complement, beta 1H and C3bINA, during fetal life.

The levels of beta 1H and C3bINA were estimated in sera from human fetuses, cord and maternal samples. Both components of complement were detected in fetuses more than 12 weeks old. The serum concentrations tended to increase with the gestational age. The mean levels of beta 1H and C3bINA in cord sera were near 54 and 61% of the mean values in sera from normal adults. Elevated levels of beta 1H were observed in maternal sera at the end of the gestational period. When the levels of beta 1H and C3bINA were compared with those of two other components of complement, it was confirmed that the mean levels of C9 were low in fetal and newborn sera, while the serum concentrations of both C9 and Factor B were elevated in maternal samples. Newly synthesised beta 1H was detected in the culture fluids of fetal liver and peritoneal cells, as judged by the incorporation of labelled aminoacids and the autoradiography of he specific immunoprecipitates in agar gel.

Migraine: a biochemical headache?

MARTIN CROOK Department of Biochemistry, Guy's Hospital Medical School, London S E l 9 R T, U.K. It has been estimated that about 11% of the United Kingdom population at some time has suffered from a migraine attack. These attacks can take various forms and to encompass all these variations migraine is usefully defined as a recurrent headache with two or more of the following five factors: (a) unilateral headache; (b) nausea; (c) visual or other neurological disturbance; (d) family history of migraine; (e) history of bilious attacks, travel sickness, asthma, eczema or hay fever (Thrush, 1978). Disturbance of cerebral blood vessels seems to be important in the pathogenesis of migraine. Initially, the aura or prodromal phase occurs associated with vasoconstriction of intracerebral arteries leading to cerebral ischaemia and resulting in cerebral malfunction, such as hand numbness or disorders of vision. This is succeeded by vasodilatation of extracerebral arteries causing headache and tenderness (Dukes & Vieth, 1964). It is hardly surprising, therefore, that vasoactive compounds are strongly implicated in provoking a migraine attack, one of the leading contenders being About 99% of the total blood 5-HT is contained in the platelets. The platelet itself lacks the enzymes 5-hydroxytryptophan decarboxylase and tryptophan 5-hydroxylase and consequently cannot synthesize 5-HT. However, the enzymes of 5-HT catabolism, namely aldehyde dehydrogenase and monoamine oxidase, are present (Fig. I). Platelets take up 5-HT actively with the 5-HT uptake being proportional to the platelet ATP content. Now one of the physiological actions of 5-HT is to cause platelet aggregation; 5-HT produces reversible aggregation of platelets and also potentiates the aggregation elicited by ADP and adrenaline. The aggregation promoted by 5-HT is related to the availability of platelet 5-HT uptake sites, aggregation occurring when the uptake site is empty (Hilton & Cumings, 1972). Platelet aggregation results in 5-HT release. Also capable of releasing platelet 5-HT are antigen-antibody complexes, blood-clotting factors, sympathomimetic amines and some fatty acids (Paasonen, 1965).

Physics

It is tempting for a lecturer in physics to present medical students with a course which, though sound and vigorous, is more suited to a physicist or engineer. At Guy's Hospital Medical School, physics is taught at a level which the student can follow and the relevance of the subject to the practice of medicine is made clear.

At Guy's Hospital Medical School the student's preconception of the relationship between medicine and society is challenged in a clinical situation and during a series of seminars. By confronting students with their own ideas rather than those of the teacher, learning is facilitated.
