The Influence Of Use Of Montage Media On The Creativity Of Children In Group B RA Ash Shiddiq Cileunyi Bandung District

This research is based on the problems found during the initial observation at RA Ash Shiddiq Group B, where children's creativity has not yet developed optimally, with most children unable to coordinate eye and hand movements. This is due to the lack of supporting media in learning activities, as the focus is more on the use of pencils and paper, as well as worksheets. The aim of this research is to determine the effect of using montage media on the creativity of children in Group B of RA Ash Shiddiq Cileunyi. This research uses a quantitative approach with a quasi-experimental research method, implemented using a nonequivalent control group design. The sample used consists of all children in Group B of RA Ash Shiddiq, totaling 28 children divided into two groups: Group B1 with 13 children as the control group and Group B2 with 15 children as the experimental group. The results of the research show that there is an influence of montage media on the creativity of children in Group B of RA Ash Shiddiq Cileunyi, Bandung Regency. Based on the results of hypothesis testing, it is obtained that at a significance level of 5%. Thus, the null hypothesis is rejected and the alternative hypothesis is accepted.

Iron Status in Male Type 2 Diabetic Patients and Its Association with Their Glycemic Status

Background & objective: Alteration in iron metabolism may occur in diabetic patients especially those who have poor glycemic control. Serum iron is involved in free radical formation associated with hyperglycemia and causes diabetic complications. The present study was undertaken to observe the iron status and its relation with glycemic control in male type 2 diabetic patients. Methods: This cross-sectional analytical study was carried out in the Department of Physiology, Sir Salimullah Medical College (SSMC), Dhaka from July 2017 to June 2018. A total of 48 diagnosed male type 2 diabetic patients (cases) were recruited from the Out-patient Department (OPD) of Endocrinology based on predefined eligibility criteria. Of them, 24 had good glycemic control (HbA1c level < 7%) (Group B1) and 24 had poor glycemic control (HbA1c level ≥ 7%) (Group B2). To compare the outcome (serum iron status) of these diabetic patients, another 48 age- and BMI-matched healthy non-diabetic male subjects were selected from personal contact as control. While diabetes and glycemic status (fasting blood glucose level, and HbA1c) of the diabetic patients were exposure variables, serum iron, serum total iron binding capacity (TIBC), transferrin saturation, and serum ferritin were the outcome variables. Results: The mean age of the subjects of either study group was around 43 and there was no significant difference between the groups concerning age (p = 0.610). The distribution of BMI and serum creatinine between groups was also similar (p = 0.135 and p = 0.134 respectively). The cases had a significantly higher level of serum iron, transferrin saturation, and an insignificantly lower total iron binding capacity (TIBC) than their non-diabetic peers (p = 0.045, p = 0.036, and p = 0.061 respectively). A comparison of outcome variables among the three groups shows that the groups were significantly heterogeneous in terms of serum iron, TIBC, transferrin saturation, haemoglobin level (p = 0.032, p = 0.012, p = 0.008 and p = 0.018 respectively). The associations were more conspicuous in patients with poor glycemic status. Correlation analyses revealed that while there was a significant linear correlation between serum iron and HbA1c (r = + 0.376, p < 0.01), it was negatively correlated with serum TIBC (r = -0.478, p < 0.001). The glycemic status (HbA1c) was also found to be linearly correlated with serum ferritin and transferrin saturation (r = + 0.354, p = 0.013 and r = + 0.462, p < 0.001 respectively). Conclusion: The study concluded that type 2 diabetic patients have a significantly higher level of serum iron, transferrin saturation, and a lower total iron binding capacity (TIBC) than the non-diabetic healthy subjects, all of which may lead to reduced synthesis of hemoglobin. A Comparative Evaluation of Removable Partial Denture Prostheses and Dental Caries Status among Diabetic and Nondiabetic Patients

Abstract Introduction Oral health is an integral part of general health. Systemic diseases such as diabetic condition and local factors have an impact on oral health of individuals. Dental caries is one of the most common oral health conditions affecting 60 to 90% of the population. Acrylic removable partial denture (RPD) is one of the most widely accepted means of tooth replacement. Its insertion in the mouth leads to altering the oral environment and increases plaque formation, particularly on tooth surface of those in contact with the acrylic partial denture facilitating the initiation of caries. Aim The aim of the study was to evaluate the effect of acrylic RPDs on caries prevalence in diabetic and nondiabetic patients. Methodology This comparative study was performed on 400 patients of age between 20 and 64 years visiting JSS Dental College and Hospital in Mysuru. Patients participating in this study were explained regarding the survey analysis and informed consent was taken. It was a questionnaire-based clinical study. Inclusion and exclusion criteria were considered for the selection of the study sample and categorize the patients into one of the following groups, with prostheses (Group A) and without prostheses (Group B) and subgroups diabetic patients with prostheses (Group A1) and nondiabetic patients with prostheses (Group A2). Diabetic patients without prostheses (Group B1) and nondiabetic patients without prostheses (Group B2). Sociodemographic information of each patient was recorded, and clinical examination was performed for decayed, missing, and filled teeth (DMFT) using DMFT index. Results A statistically significant high prevalence of dental caries was observed among nondiabetic patients with acrylic RPD (Group A2). Conclusion Acrylic RPD and diabetes had least role in caries prevalence.

Bentonite-supplemented diets improved fish performance ammonia excretion haemato-biochemical analyses immunity antioxidants and histological characteristics of European seabass Dicentrarchus labrax

The purpose of this research was to examine the potential effects of bentonite (BN) supplemented diets on growth, feed utilization, blood biochemistry, and histomorphology of Dicentrarchus labrax. Six treatments in triplicate were tested: B0, B0.5, B1.0, B1.5, B3.0, and B4.5, which represented fish groups fed diets supplemented with 0, 0.5, 1, 1.5, 3, and 4.5% BN, respectively. For 84 days, juveniles’ seabass (initial weight = 32.73 g) were fed diets containing 46% protein, three times daily at 3% of body weight. With a 5% daily water exchange, underground seawater (32 ppt) was used. Findings revealed significant improvements in water quality (TAN and NH3), growth (FW, WG and SGR) and feed utilization (FCR, PER and PPV) in fish fed BN-supplemented diets, with the best values in favor of the B1.5 group. Additional enhancements in kidney function indicators (urea and uric acid) and liver enzymes were observed in fish of the BN-treated groups along with a decrease in cholesterol level in the B1.5 group. Further improvements in fish innate immunity (hemoglobin, red blood cells, glucose, total protein, globulin, and immunoglobulin IgM), antioxidant activity (total antioxidative capacity and catalase), and decreased cortisol levels in fish of the BN-treated groups. Histological examinations of the anterior and posterior intestines and liver in groups B1.5 and B3 revealed the healthiest organs. This study recommends BN at a concentration of 1.5% as a feed additive in the Dicentrarchus labrax diet.

Upaya Meningkatkan Kemampuan Membaca Permulaan Anak Kelompok B1 melalui Metode Bercerita Gambar Berseri di TK ABA Brosot II Kulon Progo

Beginning reading is a skill and cognitive process. The storytelling method is a learning strategy that can provide learning experiences to kindergarten students. A series of images is a series of images that form a story. The formulation of the problem in this research is how to improve the beginning reading skills of group B1 children through the serial picture storytelling method at the ABA Brosot II Kulon Progo Kindergarten? The aim is to find out how to improve the beginning reading skills of group B1 children through the serial picture storytelling method at the ABA Brosot II Kulon Progo Kindergarten. The type of research is Classroom Action Research (PTK). The instrument used was an observation sheet, the sample size was 13 children and the data was analyzed using a percentage formula. The results of the study showed an increase in children's initial reading abilities, from pre-cycle 35%, cycle I to 46%, and cycle II to 81%. Based on these data, it can be concluded that the serial picture storytelling method can improve the beginning reading skills of children in group B1 at ABA Brosot II Kindergarten, Kulon Progo.

Urinary exosomal mRNAs as biomarkers for predicting the therapeutic effect of renin-angiotensin system inhibitors in IgA nephropathy patients

BackgroundRenin-angiotensin system inhibitors (RASi) treatment is the basic therapy for IgA nephropathy (IgAN) patients. However, there is few of biomarker that can predict the efficacy of RASi. This study aimed to find urinary exosomal mRNAs related to the therapeutic effect of RASi in the treatment of proteinuria in IgAN patients. Methods: We divided IgAN patients in screening cohort into A1 (proteinuria increase at 3 months), B1 (proteinuria decrease less than 50 % at 3 months), C1 (proteinuria decrease more than 50 % at 3 months) groups according to changes of proteinuria after treatment. The urinary exosomes were collected before biopsy, RNAs were extracted and analyzed with the microarray assay. The candidate genes were screened by differentially expressed genes (DEGs) analysis and then validated by quantitative real-time polymerase chain reaction (qPCR) in a validation cohort. A receiver operating characteristic (ROC) curve was used to evaluate gene performance in predicting therapeutic effect on RASi reducing proteinuria in IgAN patients. Results: ECE1 and PDE1A mRNAs were significantly different among the three groups, and were gradually decreased among A1, B1 and C1 groups. In the validation cohort, the level of urinary exosomal ECE1 and PDE1A mRNAs were also significantly lower in A2 group compared with C2 group(ECE1, P < 0.001;PDE1A, P < 0.01). Besides, the level of ECE1 mRNA was also lower in B2 group compared with C2 group (P < 0.01). The ROC curve verified that urinary exosomal ECE1 and PDE1A gene level predicted RASi efficacy in IgAN patients with area under curve (AUC) 0.68 and 0.63 respectively. Conclusion: Urinary exosomal ECE1 and PDE1A mRNAs expression can serve as potential biomarkers for predicting the RASi efficacy to reduce proteinuria in IgAN patients.

The effectiveness of rehabilitation in vulvovaginal atrophy

Objective: to evaluate the effectiveness and safety of rehabilitation programmes in patients with vulvovaginal atrophy (VVA).Material and methods. The study involved 350 patients with VVA in surgical (n=140) and natural (n=140) menopause, the control group included 70 women without VVA. Patients with VVA were distributed into groups receiving complex “active” rehabilitation (surgical menopause: group 1A, natural menopause: group 2A) and “passive” rehabilitation (surgical menopause: group 1B, natural menopause: group 2B). Body mass index (BMI), vaginal pH, carbohydrate profile with calculation of insulin resistance index (HOMA-IR), inflammatory markers leptin, interleukin-6, tumour necrosis factor alpha, serum magnesium, and safety profile with evaluation of adverse events related to rehabilitation measures were assessed over 24 months.Results. BMI decreased significantly in groups 1A and 2A. Vaginal pH at 1-year follow-up decreased in groups 1A and 2A, but increased in groups 2A and 2B. BMI was higher than normal in all patients with VVA, as well as in the control group, demonstrating preobesity (groups 1A, 2A, control group) or first-degree obesity (initially in group 1B and throughout follow-up in group 2B). HOMA-IR initially reflected postoperative insulin resistance in groups 1A and 2A, but in those receiving complex “active” rehabilitation the dynamics of its reduction was the most pronounced. The concentration of inflammatory markers was initially increased compared to the control group and decreased over time in groups 1A and 1B, but only leptin levels reached the control group values by the end of the study. Serum concentration of magnesium levels was 0.71±0.12 mmol/l in group 1A, 0.71±0.10 mmol/l in group 2A, 0.76±0.08 mmol/l in group 1B, 0.72±0.17 mmol/l in group 2B (magnesium deficiency). When organic magnesium salts in combination with pyridoxine were supplemented in groups 1A and 1B, serum magnesium levels were restored to normal from the 3rd month of follow-up. No adverse events were registered.Conclusion. The personalised programme of complex “active” rehabilitation in patients with VVA is more effective than “passive” rehabilitation with comparable safety profile. Its implementation into routine practice will contribute to the improvement of the quality of health care for such patients.

Analysis of Antioxidant Capacity, Total Phenolic and Total Flavonoid Contents in Boric Acid applied Camellia sinensis L.

Consumption of Camellia sinensis L (tea), a popular beverage, is very common today. In addition to consuming tea as a beverage, it is suggested that it can also be added to other foods to increase their antioxidant activities. This study aimed to determine the antioxidant capacity, total phenolic, and total flavonoid contents of extracts prepared in different solvents of tea grown in soil treated with boric acid. The area in Rize/Turkey was divided into 4 groups. No application was made to the control group (B0 group). Boric acid prepared in sodium tetraborate buffer was applied to the other three areas as a single dose at concentrations of 100 (group B1), 300 (group B3), and 500 (group B5) mg/m2. The obtained tea leaves were ground and infused in water, 20% ethanol, and 50% ethanol by the classical infusion method. The extracts obtained after infusion were analyzed for total phenolic contents, flavonoid content, and antioxidant capacity. It was determined that 20% and 50% ethanol were a better solvent than water in terms of total phenolic content, flavonoid content, and antioxidant capacity. Although the total phenolic contents of tea leaves grown in soils treated with different doses of boric acid did not change, differences were observed in their flavonoid contents and antioxidant capacities. As a result, we can say that boric acid should be at a certain dose to improve the quality of the tea plant. In addition, different solvents can be used to reveal more of the tea content.

Effect of femtosecond laser-assisted phacoemulsification on corneal endothelium and prognosis of diabetic patients with cataract with different nuclear hardness

Objective: To assess the impact of femtosecond laser-assisted phacoemulsification on corneal endothelial characteristics and prognosis among patients with type 2 diabetes and age-related cataracts, considering varying nuclear hardness. Methods: This non-randomized controlled trial involved 161 patients (161 eyes) with type 2 diabetes undergoing cataract extraction at Weifang Eye Hospital between March 2020 and December 2022. The cohort comprised 73 males and 88 females, with an average age of (65.9±5.23) years. Based on patient preference, 101 individuals underwent conventional phacoemulsification (group A), while 60 chose femtosecond laser-assisted phacoemulsification (group B). Patients were further stratified based on Emery-Little grade of lens nuclei into A1/B1 (grade Ⅰ and Ⅱ), A2/B2 (grade Ⅲ), and A3/B3 (grade Ⅳ) subgroups. The study compared effective phacoemulsification time (EPT), cumulative energy release of phacoemulsification (CDE), central corneal thickness (CCT), endothelial cell density (ECD), coefficient of variation (CV), and hexagon cell ratio (HEX) before and after surgery at 1 day, 1 week, 1 month, and 3 months. Results: The intraoperative EPT of patients in groups A and B were (6.52±4.93) and (5.63±4.31)s, respectively, and the CDE were 11.57%±5.21% and 10.68%±6.02%, respectively. The differences between them were not statistically significant (all P>0.05).There were no significant differences in EPT and CDE between groups A1 and B1 (all P>0.05), and there were statistically significant differences between groups A2 and B2, A3 and B3 (all P<0.05).The postoperative CCT was increased in both groups.There were no statistically significant differences in CCT between A1 and B1 groups at different time after surgery (all P>0.05), and there were statistically significant differences in CCT between A2 and B2, A3 and B3 groups at 1 day and 1 week after surgery (all P<0.05), and group B was significantly lower than group A.There was no significant difference between 1 month and 3 months after surgery (P>0.05).ECD was reduced in both groups.There was no statistically significant difference in ECD at different time of operation between A1 and B1 groups (all P>0.05), while there was statistically significant difference in ECD at 1 day, 1 week, 1 month and 3 months after operation in A2 and B2, A3 and B3 groups (all P<0.05). Group B was significantly better than group A. There was no significant difference in coefficient of variation and HEX between the two groups at different time after surgery (P>0.05). Conclusion: Femtosecond laser-assisted phacoemulsification demonstrates benefits in preserving corneal endothelial cells, reducing early postoperative corneal edema, and minimizing corneal injury in type 2 diabetes patients with cataracts of high nuclear hardness.

TOWARDS UNDERSTANDING MÜLLER-WEISS DISEASE: A UNIVERSAL PLATFORM

BackgroundThe only existing classification of Müller-Weiss Disease (MWD), based solely on Méary's angle, serves neither as guide for prognosis nor treatment. This accounts for lack of gold standard in its management.MethodsNavicular compression, medial extrusion, Kite's angle and metatarsal lengths were measured on all radiographs of 95 feet with MWD. Joints involved, presence and location of navicular fracture were recorded.ResultsWe identified three distinct groups. Group 1 comprises 11 “early-onset” MWD feet, aged 9 to 29 years. These had the greatest compression and medial extrusion, and lowest Kite's angles. All except 1 were index minus and had a lateral navicular fracture. None has required surgery to date. Only 1 has moderate talonavicular joint (TNJ) degeneration.Group 2 comprises 23 “Müller-Weissoid” feet with radiologically normal navicular in their fifties and developing MWD, on average, 4.5 years later. These had the lowest compression and extrusion, and highest Kite's angles. None had complete fracture. All had TNJ arthritis, with early changes at lateral naviculocuneiform joint (NCJ) in 43%. Group 3 “late-onset” MWD, presenting in the sixth decade, is subdivided into 3 sub-groups. Only TNJ is involved in group 3A (16). Group 3B denotes affection of TNJ more than NCJ (20). In group 3C “reverse Müller-Weiss disease”, which affects NCJ more than TNJ (25), second metatarsal overlength is highest of all groups. No difference in age, compression, extrusion and Kite's angle exists among the 3 subgroups. No fracture occurred in group 3A compared to 65% and 32% in groups 3B and 3C, respectively.ConclusionsWith a need to compare like-for-like pathology, the proposed classification provides a common platform for reporting outcomes of different treatment modalities, operative or nonoperative. We theorize pathogenetic pathways in the different groups and propose systematic surgical approaches for each category.

The class B heat shock factor RcHsf17 from Rosa chinensis enhances basal thermotolerance in Rosa rugosa

Rosa rugosa, a species of notable economic importance, faces cultivation challenges attributed to its vulnerability to thermal stress. In this study, we report the molecular cloning and functional characterization of RcHsf17, a heat shock transcription factor (Hsf) from group B2, derived from the stress-resilient cultivar of Chinese rose, R. chinensis ‘Slater's Crimson China’. RcHsf17, which localizes to the nucleus, is responsive to heat stress and various stress-related stimuli, such as abscisic acid (ABA), jasmonic acid (JA), salicylic acid (SA), and hydrogen peroxide (H2O2). The overexpression of RcHsf17 in transgenic Arabidopsis thaliana and R. rugosa significantly enhanced their thermotolerance. In R. rugosa, overexpression of RcHsf17 led to cell wall thickening, sustained photosynthetic efficiency, and decreased levels of reactive oxygen species (ROS) and malondialdehyde (MDA). Transcriptome analysis revealed that RcHsf17 upregulated the expression of other Hsfs such as HsfA3 and HsfB1, small heat shock protein genes (sHSPs), and genes involved in cell wall biosynthesis under normal conditions. Under heat stress condition, there is a significant upregulation of HsfA6B, gene associated with photosynthesis, and genes responsible for encoding antioxidant enzymes. These findings collectively emphasize the pivotal role of RcHsf17 in conferring thermotolerance to R. rugosa, through mechanisms that include strengthening cell wall integrity, alleviating heat-induced photosynthetic decline, and enhancing the efficiency of ROS detoxification pathways.

Staging of psychiatric disorders in patients with limited and advanced/metastatic solid cancer: A pilot study.

e22545 Background: 38% of cancer patients (pts) receive diagnosis of major depressive disorder. According with the clinimetric approach of the staging of psychiatric disease, the 5-step model of unipolar depression allows for the evaluation of the extent of the disorder, its severity, and characteristics at a specific time in the course of the disease. This study was aimed: a) to assess mental health and quality of life of cancer patients with limited (LS) and advanced/metastatic (AS) solid cancer through the join evaluation of the staging of psychiatric disorders; b) to explore the potential role of severity of oncological disease in anxious/depressive disease development. Methods: A cross-sectional study was conducted at the Oncology Unit of Luigi Sacco Hospital of Milan during the first session of a psychological support path. Patients’ cancer diagnosis and stage, as well as regular engagement in work, leisure and social activities were explored. Hospital and Depression Anxiety Scale (HADS), the Revised Illness Perception Questionnaire (IPQ-R) and Mental Health Continuum Short-Form (MHC-SF) were also administered. Results: 104 pts were included: F 86, M 18, median age 57,5 (27-84), 30% AS, and 70% LS. Pts’ anxious/depressive symptoms were assessed by HADS scoring and they were divided into 3 groups according to the model of unipolar depression: Group 1a (prodromal phase – no symptoms), Group 1b (prodromal phase – mood symptoms) and Group 2 (anxious/depressive disorder). Pts in group 2 were significantly more frequently AS pts, while in group 1a pts in LS stage (χ² = 7.78, p &lt; .05). Based on MHC-SF scores, flourishing and languishing were significantly more frequently reported by pts in group 1a (55,5% and 46% of pts, respectively), and moderate mental health in group 2 (45,9% of pts, χ² = 18.1, p &lt; .001). Regression analysis highlighted non-hobbyist vs. hobbyists, AS vs. LS cancer pts, asthenic vs. non asthenic pts, and moderate vs flourishing metal health were more likely to receive a provisional anxious depressive disorder diagnosis (Stage 2). No association emerged for other physical symptoms, work, and socialization. Conclusions: AS pts with asthenia, no engagement in structured activities and moderate mental health had a higher risk of developing anxious/depressive disorders. Screening psychiatric disorders in cancer pts may improve the physician's ability to recognize earlier phase of psychopathological disease and guide pts to tailored treatment. These may prevent progression of psychiatric disease and promote mental health and positive quality of life.

Comparative evaluation of increase in temperature on the external root surface of teeth during retrieval of broken NiTi instrument using two ultrasonic tips and two power settings: An in vitro study.

An unfavorable event that can hinder endodontic treatment and affect the outcome of root canal treatment is the separation of endodontic instruments. Endodontic instrument separation can occur due to clinical or metallurgical factors. Friction between the ultrasonic tip and the remaining dentin generates heat, which is subsequently transferred to the external root surface. Elevated temperatures exceeding 10°C above body temperature for more than a minute may result in injury to periodontal or bone tissue. The aim of this study was to evaluate and compare temperature rise on the external root surface of teeth during retrieval of broken NiTi instrument using two different ultrasonic tips and two power settings. In each group, a sample size of 8 was sufficient to attain a statistical power exceeding 90%, enabling the detection of a minimum mean difference of 0.9204 (delta) through a one-way ANOVA test at a 95% confidence level (alpha 0.05). After access opening and working length determination, samples were randomly distributed into two groups - Group 1 (A and B) - ProUltra tip at high and low power settings and Group 2 (A and B) - Cric Dental IR3 at high and low power settings. The temperature rise was measured using K-type thermocouple thermometer. The comparisons were analyzed using the Kruskal-Wallis test with pairwise comparisons using the Dunn's test. Group 1A and Group 1B resulted in lower heat generation compared to Group 2A and 2B and its difference was statistically significant (P < 0.05). Minimum temperature rise is seen in the ProUltra group at lower power settings (Group 1A) at the apical level and maximum temperature rise is seen in the Cric Dental IR3 group at higher power settings (Group 2B) at the middle third level. It was found that there is a significant temperature rise seen when ultrasonic tips are used for the retrieval of separated files, especially at higher power settings. The ProUltra tip demonstrated the lowest temperature rise at lower power settings, particularly at the apical level, whereas the IR3 Cric Dental tip exhibited the highest temperature rise, notably at higher power settings and the middle third level.

The Influence of Weaving Activities on Self-Identity Development in Kindergarten: Exploring Early Childhood Enrichment

The fine motor development of children aged 5-6 years at the ABA Kindergarten Kedai Manggeng Aceh Barat Daya experiences several obstacles in the development of their fine motor skills, and this is indicated by the child's hands still being stiff when holding a pencil and cutting not according to the pattern. So, this research aims to determine the effect of weaving activities with coconut leaves on the development of the identity of children aged 5-6 years at the ABA Kedai Manggeng Kindergarten, Aceh Barat Daya. This quantitative research uses a pre-experimental design through approach one group pre-test post-test design. The sample used was all group B1 students (29 people) taken using random sampling techniques. Data collected based on observation results obtained an average value pretest amounted to 52.06%, and the average value posttest obtained 75.10%, so the resulting normality test is 0.233 &gt; 0.05. Based on this data, the value of tcount &gt; ttable is 16.949 &gt; 1.04841, So it can be stated that Ha accepted and HO. Based on these data, it can be concluded that weaving activities can influence the development of children's identity at the Kedai Manggeng ABA Kindergarten, Aceh Barat Daya. .

Live birth rate per fresh embryo transfer and cumulative live birth rate in patients with PCOS under the POSEIDON classification: a retrospective study.

Ovarian stimulation (OS) for in vitro fertilization (IVF)/intracytoplasmic sperm injection (ICSI) in women with PCOS often results in multiple follicular development, yet some individuals experience poor or suboptimal responses. Limited data exist regarding the impact of poor/suboptimal ovarian response on pregnancy outcomes in women with PCOS. The aim of this study was to evaluate whether the live birth rate (LBR) per fresh embryo transfer and cumulative live birth rate (CLBR) per aspiration cycle differ in women with PCOS defined by the Patient-Oriented Strategy Encompassing IndividualizeD Oocyte Number (POSEIDON) criteria. A retrospective study involving 2,377 women with PCOS who underwent their first IVF/ICSI cycle at Sun Yat-sen Memorial Hospital from January 2011 to December 2020 was used. Patients were categorized into four groups based on age, antral follicle count, and the number of oocytes retrieved, according to the POSEIDON criteria. The LBR and CLBR were compared among these groups. Logistic regression analysis was performed to assess whether the POSEIDON criteria served as independent risk factors and identify factors associated with POSEIDON. For patients <35 years old, there was no significant difference in the clinical pregnancy rate between POSEIDON and non-POSEIDON patients, whereas POSEIDON patients exhibited lower rates of implantation and live birth. POSEIDON Group 1a displayed lower rates of implantation, clinical pregnancy, and live birth. However, no significant differences were observed in the rates of clinical pregnancy and live birth between POSEIDON Group 1b and non-POSEIDON groups. For patients ≥35 years old, there were no significant differences in the rates of implantation, clinical pregnancy, and live birth between POSEIDON and non-POSEIDON patients. CLBRs were significantly lower in POSEIDON Groups 1 and 2, compared with the non-POSEIDON groups. The levels of body mass index (BMI), follicle-stimulating hormone (FSH), and antral follicle count (AFC) were associated with POSEIDON hypo-response. POSEIDON was found to be associated with lower CLBR, but not LBR per fresh embryo transfer. In patients with PCOS, an unexpected suboptimal response can achieve a fair LBR per fresh embryo transfer. However, CLBR per aspirated cycle in POSEIDON patients was lower than that of normal responders. BMI, basal FSH level, and AFC were independent factors associated with POSEIDON. Our study provides data for decision-making in women with PCOS after an unexpected poor/suboptimal response to OS.

The Effect of Educational Game Tools Rotating Series Story Cards on The Speaking Ability of Children 5-6 Years Old at TKIT Shalih Child Character Padang

The purpose of this study was to analyze the extent of the influence of the use of Rotating Series Story Cards on the speaking ability of children aged 5-6 years at TKIT Karakter Anak Shalih Padang. The type of research used is quantitative experimental research (Quasi Experimental Design), with a Nonequivalent Posttest-Only Control Group Deign approach), The research population is 5-6 year old children in Group B1 as the experimental class and B2 as the control class with a total of 30 students. Data collection was carried out by observation and checklists. Data analysis was carried out using SPSS 24.0, using tests and hypotheses. Based on the results of the study, it shows that there is a significant effect of using educational game tools for rotating series story cards on the speaking ability of children aged 5-6 years at TKIT Karakter Anak Shalih Padang, it can be seen from the statistical test results Based on the results of the Independent Sample T-test test, it can be seen that the significance value is 0.171 indicating that 0.171 ≥ 0.01 then ????0 is accepted. So it can be concluded that there is an effect of using educational game tools Rotating Series Story Cards on the speaking ability of children aged 5-6 years in TKIT Karakter Anak Shalih Padang.

Effect of non-surgical periodontal therapy on salivary and gingival crevicular fluid concentration of visfatin in periodontal health and disease

Background and objectiveVisfatin, a pleotropic mediator mostly produced by visceral fat, is crucial in controlling the immunological and defensive systems. It serves the roles of a cytokine, an enzyme involved in energy metabolism, and a growth factor. The objective of the present study was to assess the impact of non-surgical periodontal therapy (scaling and root planing) on visfatin concentrations in saliva and gingival crevicular fluid in individuals with Periodontitis (stage-II grade-A) Materials and methods54 individuals were divided into Group A (Periodontally Healthy) and Group B1(Periodontitis baseline) based on periodontal parameters including plaque index (PI), gingival index (GI), probing pocket depth (PPD), clinical attachment level (CAL), and radiographic parameters. After NSPT (SRP), Group B1 patients were recalled after 4 weeks, constituting Group B2 (post NSPT group B1). At baseline and 4 weeks after non-surgical periodontal therapy (SRP), all clinical parameters, salivary and GCF samples were recorded. An ELISA kit was used to measure the levels of visfatin. Using the paired t-test, unpaired t-test, and Pearson's correlation coefficient, data were analysed using SPSS 15. ResultsAfter non-surgical periodontal treatment (SRP), the mean salivary and gingival crevicular fluid concentration of visfatin considerably decreased to a level comparable to periodontal health. In all groups, GCF visfatin concentration was higher than salivary concentration of visfatin. In periodontitis patients, visfatin concentration in GCF was 1.5 times higher than in saliva. ConclusionThe results of this investigation suggest a direct correlation between salivary and gingival crevicular fluid visfatin concentration and periodontal tissue inflammation and disease activity.

Electrophysiology-guided algorithm for patients with conduction abnormalities post TAVR: one-year follow-up

Abstract Introduction Transcatheter aortic valve replacement (TAVR) revolutionized the treatment of aortic stenosis [1]. Although TAVR is an effective treatment for AS patients, it is associated with high incidence of new-onset conduction disturbances and pacemaker implantations. The two most common conduction disturbances after TAVR are new onset left bundle branch block (LBBB) and high-degree atrioventricular block (HDAVB) [2]. While patients with HDAVB require a permanent pacemaker implantation (PPM), the management of LBBB is not completely established yet. According to the ESC 2021 guidelines, a PPM is indicated in patients with persistent HDAVB or new onset alternating BBB after TAVR. Yet, the recommendations regarding other conduction abnormalities are not well defined. In patients with persistent new onset LBBB, ambulatory continuous ECG monitoring or electrophysiology study (EPS) are recommended [3]. Purpose To evaluate the clinical outcomes of patients with conduction disorders after TAVR who were managed according to electrophysiology-guided algorithm (Figure 1), including mortality, late pacemaker’s implantations, and heart failure hospitalizations. Predictors of periprocedural LBBB and HDAVB were also assessed. Methods Retrospective population-based cohort study including all patients who developed new onset LBBB after TAVR at a tertiary University Medical Center from October 1, 2018, to December 31, 2022, and were managed according to specific electrophysiology-guided algorithm. Results The study cohort included 230 consecutive patients who underwent TAVR. Mean age was 80.3 and 56.5% were females (Figure 2). Twenty patients (8.7%) developed HDAVB and required a PPM, whereas 40 patients (17.4%) developed persistent LBBB. According to the algorithm, 8 patients (20%) with a QRS&amp;lt;130ms were discharged without PPM [group A], 20 patients (50%) with a QRS of 130-160ms underwent EPS, and 12 patients (30%) with a QRS&amp;gt;160ms [Group C] received a PPM. Among the EPS group: 9 patients (22.5%) with HV&amp;gt;65ms underwent PPM [group B2], while 11 patients with HV&amp;lt;65ms (27.5%) were discharged without PPM [group B1]. During a one-year follow-up only one patient required late PPM [group B1]. Mortality was higher among the subgroups with PPM [B2 and C] compared to the subgroups without a PPM [A and B1]: 4 vs. 1 patient accordingly. Conclusions The presented electrophysiology-guided algorithm for patients with persistent LBBB after TAVR appears safe and efficient, with a modest incidence of PPM (17.6% of the total cohort) during hospitalization and a minimal need for late PPM at one year follow-up. Importantly, there was no excess of mortality among patients who were discharged without a pacemaker implantation. Thus, we suggest that the presented algorithm may serve as a reliable strategy to distinguish between individuals with new onset persistent LBBB after TAVR who require PPM compared to those in whom a PPM can be safely avoided.

Ablation of persistent atrial fibrillation based on atrial electrogram duration map: methodology and clinical outcomes from the AEDUM pilot study

Abstract Background Catheter ablation of persistent atrial fibrillation (PsAF) represents a challenge for the electrophysiologist and there are still divergences regarding the best ablative approach to adopt. Objectives Create a new map of the duration of atrial bipolar electrograms (Atrial Electrogram DUration Map, AEDUM) to recognize a functional substrate during sinus rhythm and guide a patient-tailored ablative strategy for PsAF. Methods 40 PsAF subjects were assigned in a 1:1 ratio to either for PVI alone (Group B1) or PVI+AEDUM areas ablation (Group B2). A cohort of 15 patients without AF history undergoing left-sided accessory pathway ablation was used as a control group (Group A). In all patients, voltage and AEDUM maps were created during sinus rhythm. The minimum follow-up was 12-months, with rhythm monitoring via 48-h ECG Holter or by implantable cardiac device. Results Electrogram (EGM) duration was higher in Group B than in Group A (49±16.2ms vs 34.2±3.8ms; p-value&amp;lt;0.001). In Group B the mean cumulative AEDUM area was 21.8±8.2cm2; no difference between the two subgroups was observed (22.3±9.1cm2 vs 21.2±7.2cm2; p-value=0.45). The overall bipolar voltage recorded inside the AEDUM areas was lower than in the remaining atrial areas [median:1.30mV (IQR:0.71–2.38mV) vs 1.54mV (IQR:0.79–2.97mV); p-value:&amp;lt;0.001)]. Low voltage areas (&amp;lt;0.5mV) were recorded in three (7.5%) patients in Group B. During the follow-up [median 511 days(376-845days)] patients who underwent PVI-only experienced more AF recurrence than those receiving a tailored approach (65% vs 35%; p-value=0.04). The purpose of this study is to reduce aggressiveness in early childhood, especially 5-6 years of age. This research is a quasi-experiment research with Nonequivalent control group design, data collection techniques using observation and documentation. The population in this study were 2 classes, namely group B1 and group B2 with random sampling technique, each of which had 15 students. Data collection techniques using observation and documentation. The instrument measurement scale uses a Likert scale. The data analysis technique uses the t test, namely independent sample t-test and paired sampe-test to determine the comparison when the pretest and postest. The data was processed using the help of SPPS for windows to determine whether the hypothesis was rejected or accepted. The results showed that after treatment for 5 times, it can be seen from the results of calculations with SPSS which obtained a value of 0.000 which means the sig value &lt;0.05. This means that there is an effect of reinforcement techniques in reducing the level of aggressiveness of Ramadani natural school children. Forms of aggressiveness in children such as pushing, grabbing, ignoring teacher calls, hitting.
