Crohn's Disease of the Esophagus

SummaryEsophageul involvement wilh C'rohn's disease has been rarely reported and pathologic documentation of granulomutous disease is often missing. A 12‐year‐old hoy who presented initially with dysphagia, odynophagia, and weight loss was found to have granulomatous esophagitis, gastritis, and subsequent colitis by endoscopic examination. Esophageal manometry showed a hypertensive lower esophageal sphincter with normal peristalsis and sphincter relaxation. The esophageal symptoms responded to oral steroids and sulfasalazine without any specific treatment for acid peptic disease. This case is the youngest reported patient with Crohn's disease of the esophagus. A review of the medical literature illustrates salient clinical, radiographic, endoscopic, and pathologic features of esophageal involvement in Crohn's disease. This case and the summarized cases emphasize the potential significance of esophageal symptoms in patients with Crohn's disease.

Perforated Gastric Tuberculosis: A Rare Complication of a Rare Disease

A 30-year old physician presented with acute abdominal findings compatible with acute appendicitis. Chest roentgenogram was normal. After appendectomy, the patient slowly improved. Microscopically the appendix displayed only peri-appendicitis, prompting upper gastrointestinal and computerized tomographic examinations. The latter revealed a lesser sac abscess which was drained. Despite drainage, the abscess recurred and the patient was reexplored. A minute perforation of the distal stomach, communicating with the lesser space, was closed and the area widely drained. The patient developed a low-output gastrocutaneous fistula which failed to close with conservative treatment. The patient remained well clinically, even with fistulous involvement of the right colon. Endoscopic biopsy revealed granulomatous gastritis without acid-fast bacilli. Several months later a large inguinal lymph node was excised and found to have caseating granulomas with acid-fast bacilli. All fistulous drainage resolved after initiating antituberculous therapy and the patient has remained well. This case appears to be the first report of gastric tuberculosis causing an acute abdomen due to perforation. This rare curable condition should be considered when evaluating gastric pathology.

Gastritis in Lake Tanganyika cichlids (Tropheus duboisii)

Necrotic and granulomatous gastritis is described in Lake Tanganyika cichlids. Clostridium hastiforme and flagellated protozoa were both associated with the reaction but the significance of either is unknown. Nevertheless, treatment of surviving fish with ampicillin was carried out and mortalities ceased. The possible involvement of an unsuitable diet as a predisposing factor is discussed.

Spirorchidiasis in loggerhead sea turtles (Caretta caretta): pathology.

Loggerhead sea turtles (Caretta caretta) from the Atlantic seaboard (Florida to Massachusetts) were examined at the Marine Pathology Laboratory, University of Rhode Island, from March through December, 1980. Three genera of blood flukes (spirorchids) were found in 14 (33%) of the 43 turtles. Gross signs in heavily infected animals included cachexia, anemia and enteritis. Histopathological lesions were similar to those present in homeotherms with schistosomiasis. Granulomatous gastritis, enteritis, hepatitis, pneumonitis, and nephritis were present. Acute and chronic vasculitis accompanied metastasis of eggs. Infected animals had severe hepatic hemosiderosis, indicative of the anemia observed grossly. Evidence is presented that spirorchidiasis is prevelent in sub-adult loggerhead sea turtles, is responsible for extensive lesions and may be responsible for significant debilitation and mortality.

Ileojejunitis in a Pair of Identical Twins

A pair of identical twin brothers with identical white and red blood cell antigens and diffuse ileojejunitis is reported. One brother initially presented with granulomatous gastritis, presumably Crohn's disease of the stomach. The relationship of diffuse ileojejunitis to Crohn's disease and the suspected genetic predisposition to develop Crohn's disease are briefly discussed.

Granulomatous gastritis, iron deficiency, vitamin B12 malabsorption and immunoglobulin deficiency

Summary A 24-year-old male with recurrent iron deficiency anaemia was found to have a generalized reduction in his immunoglobulin levels, granulomatous gastritis and impaired vitamin B12 absorption corrected by Intrinsic Factor. There is discussion of the possibility that cell mediated mechanisms may have been involved in the cause of the vitamin B12 malabsorption.

Crohn's disease of the stomach. A case report and review of the literature.

Crohn's disease of the stomach is a rare but well-established clinical and pathologic entity. It is not justifiable to assume that isolated granulomatous gastritis is Crohn's disease, although this has been reported as its first manifestation (6, 9). In the stomach the antrum is most commonly involved, and narrowing, rigidity, alteration of mucosal pattern, ulceration, and striking hypomotility are the usual radiologic features. Duodenal involvement is a frequent concomitant. When combined with the typical ileal or colonic lesion, a presumptive clinical diagnosis on the basis of radiologic findings can be made with confidence. Occasionally, as in our patient, the diagnosis can be confirmed histologically by endoscopic biopsy. On the basis of short-term followup, gastric involvement does not appear to portend an unusually bad prognosis. Surgical intervention is recommended only for complications such as perforation or gastric outlet obstruction.
