Building a resilient health system in Ghana- Lessons from COVID-19 and beyond

The COVID-19 pandemic has exposed weaknesses in the health system in Africa and beyond. There were global health system failures such as the lack of international collaboration, lack of equity in vaccine distribution, poor data accuracy and sharing, and massive global economic losses. Locally, the breakdown and inefficiencies of surveillance networks contributed to the spread of COVID-19. This was worsened by years of neglect and lack of investment in our surveillance systems. The COVID-19 pandemic directly or indirectly affected all spheres of life and affected everyone. Ghana fought the COVID-19 pandemic in the way it knows best. There were some failures but also some very important successes and lessons we learned. This is all garnered towards building a resilient health system capable of resisting the next health event. This editorial focuses on the scope and range of COVID-19 on the health of the population and how it can aid Ghana to build a more resilient healthcare system.

Characterizing the dynamics of multi-scale global high impact weather events

The quantitative characterization and prediction of localized severe weather events that emerge as coherences generated by the highly non-linear interacting multivariate dynamics of global weather systems poses a significant challenge whose solution is increasingly important in the face of climate change where weather extremes are on the rise. As weather measurement systems (multiband satellite, radar, etc) continue to dramatically improve, increasingly complex time-dependent multivariate 3D datasets offer the potential to inform such problems but pose an increasingly daunting computational challenge. Here we describe the application to global weather systems of a novel computational method called the Entropy Field Decomposition (EFD) capable of efficiently characterizing coherent spatiotemporal structures in non-linear multivariate interacting physical systems. Using the EFD derived system configurations, we demonstrate the application of a second novel computational method called Space-Time Information Trajectories (STITs) that reveal how spatiotemporal coherences are dynamically connected. The method is demonstrated on the specific phenomenon known as atmospheric rivers (ARs) which are a prime example of a highly coherent, in both space and time, severe weather phenomenon whose generation and persistence are influenced by weather dynamics on a wide range of spatial and temporal scales. The EFD reveals how the interacting wind vector field and humidity scalar field couple to produce ARs, while the resulting STITS reveal the linkage between ARs and large-scale planetary circulations. The focus on ARs is also motivated by their devastating social and economic effects that have made them the subject of increasing scientific investigation to which the EFD may offer new insights. The application of EFD and STITs to the broader range of severe weather events is discussed.

Generation of state‐dependent ensemble perturbations based on time‐varying seawater density for GloSea5 initialization

AbstractIn this study, we developed a flow‐dependent oceanic initialization system for initializing the oceanic temperature and salinity in the Global Seasonal forecast system version 5 (GloSea5). Our algorithm overcomes the limitation of stationary perturbations for Ensemble Optimal Interpolation (EnOI) by spreading observed information along isopycnal lines to create three‐dimensional snapshot density states. The proposed algorithm, which we call state‐dependent ensemble‐based EnOI (SD‐EnOI), takes into account changes in the background error covariance over time without relying on ensemble model simulations. To evaluate the quality of the oceanic initial conditions (ICs) produced by SD‐EnOI, we compared them with those generated by the Global Ocean Data Assimilation and Prediction System version 1 (GODAPS1) operated by the Korea Meteorological Agency (KMA) throughout January 2017 to December 2017. Our findings show that the thermal construction of the SD‐EnOI ICs is more realistic than that of GODAPS1, particularly in the tropical Pacific region. The strong warm bias in sea surface temperature (SST) and the shallow mixed‐layer depth bias observed in the GODAPS1 ICs are not shown in SD‐EnOI. Due to the more realistic oceanic thermal structure present in the SD‐EnOI ICs, their use in retrospective forecast experiments resulted in a systematic reduction in climatological SST drift in the central‐eastern Pacific for forecasts up to four lead months compared to using GODAPS1 ICs. This demonstrates the significant impact of the initialization process on the quality of dynamical seasonal forecasts.

Low- and middle-income countries’ external debt cycle and global economic prospect

The rapid growth of peripheral countries has brought significant changes to the global economic and financial system, making it crucial to examine the relationship between their cyclical debt fluctuations and the world economy. Using the Bayesian dynamic factor model, this paper documents the presence of a set of common factors, known as global debt cycles, in the external debt accumulation of the low- and middle-income countries (LMICs). The interesting finding is that this debt cycles differ from well-known factors for global financial cycles in capital flows, such as U.S. monetary policy and global risk aversion. Even after controlling the factors for global financial cycles, our results reveal a significantly negative predictive effect of estimated debt cycles on global economic growth. Moreover, we demonstrate that this negative effect is primarily dominated by LMICs occupying prominent positions in international debt market networks, especially during the post-2008 financial crisis period. These findings emphasize the increasing importance of peripheral countries in the world system, which has been largely ignored in existing literature.

Innovation of the global international ship registry system: Evolution process and future reforms

The innovation and governance of the global international ship registry system have become key to achieving sustainable development for maritime nations worldwide. For a long time, many maritime nations have chosen to fly flags of convenience for commercial or political reasons, causing major maritime nations to lose effective control over their fleets. To counteract the negative impacts of flags of convenience, major maritime nations have established international ship registry systems. This paper analyzes the development process and basic characteristics of the global international ship registry system, points out the main problems in its implementation, and proposes major directions for innovation and governance of the international ship registry system worldwide. By reviewing the development history of the global international ship registry system, evaluating key issues in its implementation, and constructing future reform and governance directions, this paper aims to provide policy innovation suggestions for the ship registry policy reforms of global maritime nations.

Practices of Trans-National Corporations: The Need to Change Global Economic and Political Norms; Comment on "National Public Health Surveillance of Corporations in Key Unhealthy Commodity Industries – A Scoping Review and Framework Synthesis"

Trans-national corporations (TNCs) are recognised as having an adverse impact on public health through the marketing and sale of unhealthy products. In addition to this some of their practices affect health, including taxation avoidance, lobbying politicians to gain favourable legislative and regulatory environments for their operations, and failure to abide by occupational health and safety standards. We argue that while considering the individual practices of commercial actors is crucial the true public health harms are only evident when the synergistic impacts of the practices are considered. We also note that the practices are supported by a global economic and political system which operates in the favour of commercial actors rather than the health of people and the planet. Consequently there needs to be a norm change by which norms and power are shifted away from commercial interests and externalities of commercial practices are no longer outsourced to the public purse.

Assessing the Impact of Derivatives on the 2008 Financial Crisis and the Effectiveness of Subsequent Regulatory Reforms

The 2008 financial crisis was a global financial catastrophe that reflected the misuse of financial derivatives in financial markets, such as credit default swaps, asset-backed securities and collateralized debt obligations, exposing significant weaknesses in the global financial system. These financial instruments were designed to hedge against debt risk, but their lack of transparency and massive misuse ultimately led to the accumulation of systemic risk that accelerated the financial crisis. Since then, regulatory authorities around the world have realized the necessity for stronger regulatory frameworks, and a series of significant reforms have been implemented, such as the Dodd-Frank Wall Street Reform and Consumer Protection Act in the United States, the European Market Infrastructure Regulation in the European Union, and the Basel III international regulatory framework, with the objectives of increasing transparency in financial markets and improving the risk management measures of financial institutions. This paper will explore the role of derivatives in the 2008 financial crisis and examine the effectiveness of these regulatory measures introduced after the crisis in enhancing stability of financial system.

Current Development and Future Outlook of China CBDC (e-CNY): A Literature Review

The great success of Bitcoin has drawn the attention of the world to the field of digital currency. Stablecoins and CBDC have come out right after cryptocurrencies. This study focuses on analyzing CBDC, especially China CBDC (e-CNY), elaborating and comprehending arguments from several outstanding essays in this field. In this study, the author summarizes the development history of e-CNY, concludes the competitive advantages of e-CNY over other types of payment methods, points out the potential shortcomings of e-CNY, and implements possible solutions. In order to provide reliable suggestions and recommendations, the author has compared e-CNY with several existing CBDC from different countries and regions in the world, including CBDC from Singapore, Canada, and England. In the short run, China CBDC can enhance finance monitoring and reduce M0 supply. In the long run, it could stimulate the internationalization of the RMB and boost the evolution of the global monetary system.

Combinatorial control of Pseudomonas aeruginosa biofilm development by quorum-sensing and nutrient-sensing regulators.

The human pathogen Pseudomonas aeruginosa, a leading cause of hospital-acquired infections, inhabits and forms sessile antibiotic-resistant communities called biofilms in a wide range of biotic and abiotic environments. In this study, we examined how two global sensory signaling pathways-the RhlR quorum-sensing system and the CbrA/CbrB nutritional adaptation system-intersect to control biofilm development. Previous work has shown that individually these two systems repress biofilm formation. Here, we used biofilm analyses, RNA-seq, and reporter assays to explore the combined effect of information flow through RhlR and CbrA on biofilm development. We find that the ΔrhlRΔcbrA double mutant exhibits a biofilm morphology and an associated transcriptional response distinct from wildtype and the parent ΔrhlR and ΔcbrA mutants indicating codominance of each signaling pathway. The ΔrhlRΔcbrA mutant gains suppressor mutations that allow biofilm expansion; these mutations map to the crc gene resulting in loss of function of the carbon catabolite repression protein Crc. Furthermore, the combined absence of RhlR and CbrA leads to a drastic reduction in the abundance of the Crc antagonist small RNA CrcZ. Thus, CrcZ acts as the molecular convergence point for quorum- and nutrient-sensing cues. We find that in the absence of antagonism by CrcZ, Crc promotes the expression of biofilm matrix components-Pel exopolysaccharide, and CupB and CupC fimbriae. Therefore, this study uncovers a regulatory link between nutritional adaption and quorum sensing with potential implications for anti-biofilm targeting strategies.IMPORTANCEBacteria often form multicellular communities encased in an extracytoplasmic matrix called biofilms. Biofilm development is controlled by various environmental stimuli that are decoded and converted into appropriate cellular responses. To understand how information from two distinct stimuli is integrated, we used biofilm formation in the human pathogen Pseudomonas aeruginosa as a model and studied the intersection of two global sensory signaling pathways-quorum sensing and nutritional adaptation. Global transcriptomics on biofilm cells and reporter assays suggest parallel regulation of biofilms by each pathway that converges on the abundance of a small RNA antagonist of the carbon catabolite repression protein, Crc. We find a new role of Crc as it modulates the expression of biofilm matrix components in response to the environment. These results expand our understanding of the genetic regulatory strategies that allow P. aeruginosa to successfully develop biofilm communities.

The Evolution of Futures and Options Markets: From Agricultural Roots to High-Frequency Trading

The evolution of futures and options markets from simple agricultural hedging tools to complex financial instruments has been marked by significant technological advancements, particularly the advent of electronic trading and high-frequency trading (HFT). This paper traces the development of these markets, highlighting key milestones and innovations that have shaped their current form. High-frequency trading, while enhancing liquidity and reducing transaction costs, has also introduced significant challenges, including increased market volatility, the potential for market manipulation, and systemic risks, as seen in events like the 2010 Flash Crash. This research provides a comprehensive analysis of the legal and regulatory frameworks governing futures and options markets, with a focus on both Indian and international contexts. It examines the effectiveness of existing regulations in addressing the challenges posed by modern trading practices and explores the need for continuous adaptation in regulatory approaches. Through a detailed review of relevant case laws, statutory provisions, and scholarly literature, the paper identifies gaps in current regulatory practices and proposes recommendations to enhance market transparency, fairness, and stability. The study concludes that while the evolution of futures and options markets has brought undeniable benefits in terms of efficiency and risk management, these markets must be carefully regulated to mitigate the risks associated with high-frequency and algorithmic trading. Adaptive regulatory frameworks, enhanced transparency measures, and international cooperation are essential for ensuring that these markets continue to contribute positively to the global financial system.

Geochemical records of mudflat sediments from southern Saurashtra, Western India: Implications for Holocene climate and global teleconnection

The heat transfer from the low latitudes to high latitudes is responsible for maintaining the earth’s climate dynamics. Thus, deciphering the possible mechanism driving the variability of the Indian summer monsoon (ISM) during the Holocene Epoch has been critical to understand the hydroclimatic changes of the low latitudes. Despite several efforts, the teleconnection of ISM with the global climate dynamics remains under-represented and poorly understood. The present study aims to delineate the ISM variability and its possible forcing mechanism from western India (Gujarat). In this study, a sediment core (~65 cm long) was raised from the Jaffrabad mudflat (MIT) in western Gujarat. The sediment samples were subjected to geochemical analysis to investigate paleomonsoon, paleo-sediment source and paleoweathering changes. The results show that, with the addition of intermediate sources, the sediments were principally derived from the hinterland’s Deccan basalts. Further, the study suggested a warm and wet climate due to strong ISM during 10,650−5500 cal yr BP associated with the solar as well as orbital forcings. The weak monsoon during 5500−2700 cal yr BP has been linked with southward migration of the Intertropical Convergence Zone (ITCZ) along with the increased El Niño-like conditions. Further, the wavelet analysis revealed that a combined influence of solar, orbital and North Atlantic forcings led to monsoon variability along western India, during the Holocene Epoch. By reconciling the geochemical proxies, the present study has implications in the reconstruction of paleomonsoon and establishing the possible teleconnection with the global climate system.

Unraveling the AKT/ERK cascade and its role in Parkinson disease.

Parkinson disease represents a significant and growing burden on global healthcare systems, necessitating a deeper understanding of their underlying molecular mechanisms for the development of effective treatments. The AKT and ERK pathways play crucial roles in the disease, influencing multiple cellular pathways that support neuronal survival. Researchers have made notable progress in uncovering how these pathways are controlled by upstream kinases and how their downstream effects contribute to cell signalling. However, as we delve deeper into their intricacies, we encounter increasing complexity, compounded by the convergence of multiple signalling pathways. Many of their targets overlap with those of other kinases, and they not only affect specific substrates but also influence entire signalling networks. This review explores the intricate interplay of the AKT/ERK pathways with several other signalling cascades, including oxidative stress, endoplasmic reticulum stress, calcium homeostasis, inflammation, and autophagy, in the context of Parkinson disease. We discuss how dysregulation of these pathways contributes to disease progression and neuronal dysfunction, highlighting potential therapeutic targets for intervention. By elucidating the complex network of interactions between the AKT/ERK pathways and other signalling cascades, this review aims to provide insights into the pathogenesis of Parkinson disease and describe the development of novel therapeutic strategies.

Reducing climate change impacts from the global food system through diet shifts

How much and what we eat and where it is produced can create huge differences in GHG emissions. On the basis of detailed household-expenditure data, we evaluate the unequal distribution of dietary emissions from 140 food products in 139 countries or areas and further model changes in emissions of global diet shifts. Within countries, consumer groups with higher expenditures generally cause more dietary emissions due to higher red meat and dairy intake. Such inequality is more pronounced in low-income countries. The present global annual dietary emissions would fall by 17% with the worldwide adoption of the EAT-Lancet planetary health diet, primarily attributed to shifts from red meat to legumes and nuts as principal protein sources. More than half (56.9%) of the global population, which is presently overconsuming, would save 32.4% of global emissions through diet shifts, offsetting the 15.4% increase in global emissions from presently underconsuming populations moving towards healthier diets.

A thermodynamic framework for ductile phase-field fracture and gradient-enhanced crystal plasticity

This study addresses ductile fracture of single grains in metals by modeling of the formation and propagation of transgranular cracks. A proposed model integrates gradient-extended hardening, phase-field modeling for fracture, and crystal plasticity. It is presented in a thermodynamical framework in large deformation kinematics and accounts for damage irreversibility. A micromorphic approach for variationally and thermodynamically consistent damage irreversibility is adopted. The main objective of this work is to analyze the capability of the proposed model to describe transgranular crack propagation. Further, the micromorphic approach for damage irreversibility is evaluated in the context of the presented ductile phase-field model. This is done by analyzing the impact of gradient-enhanced hardening considering micro-free and micro-hard boundary conditions, studying the effect of the micromorphic regularization parameter, evaluating the performance of the model in ratcheting loading and testing its capability to predict three-dimensional crack propagation. In order to solve the fully coupled global and local equation systems, a staggered solution scheme that extends to the local level is presented.

Examining the effects of anxiety and education level on mental health: The role of spiritual intelligence as an intervening variable in post COVID-19 patients in Indonesia

Background The COVID-19 pandemic has profoundly affected global health systems and daily life, exacerbating vulnerabilities, particularly in Indonesia. This study investigates the impact of anxiety and education level on mental health, with spiritual intelligence as an intervening variable among post-COVID-19 patients in Indonesia. Methods A cross-sectional design was employed, analyzing data from 390 post-COVID-19 patients in Indonesia. Structured questionnaires were used to assess anxiety, education level, spiritual intelligence, and mental health. Results The findings indicate that anxiety significantly affects both spiritual intelligence and mental health, with spiritual intelligence acting as a mediating factor. Additionally, higher educational attainment is positively associated with enhanced spiritual intelligence and improved mental health outcomes. Conclusions The study underscores the importance of incorporating spiritual development practices into mental health interventions and educational programs to boost resilience and overall well-being in the post-pandemic era. These practices can help mitigate the adverse effects of anxiety and educational disparities on mental health.

How viruses spread across space and time: forecasting pandemic progression by modelling geographico-temporal interactions

The COVID-19 pandemic has revealed severe flaws in the global healthcare systems ability to respond to unexpected health catastrophes. Much of the confusion and mishandling of the situation could be attributed to the failure in accurately predicting the spread of the virus across geographical locations. A global resource shortage in essential medical supplies and equipment, such as personal protective equipment (PPE) and ventilators, led to a compromised global supply chain. As a result, resources could not be allocated as needed to curb the spread of the pathogen in the most efficacious way. Although forecast models and machine learning algorithms have served as invaluable tools in devising effective response strategies, a large majority of these models were limited by their ability to describe the intricate interactions that underlie the spatio-temporal dynamics of viral proliferation. To address this issue, we employed a vector autoregression model to help capture the evolution of the disease across both the spatial and the temporal axes. Unlike traditional autoregression models, the present model is able to account for statistical regularities that exist both within a given region, and between geographical locations. Our results demonstrate that this approach accurately described the relationships across domestic and international localities throughout the evolution of the disease.

Subnational Environmental Policy: Trends and Issues

Policies relevant to many key sociological processes are often subnational, enacted at the regional, state/provincial, and/or local levels. This applies notably in the politics of the environmental state, where public and private subnational environmental policies (SNEPs) have major consequences for managing climate change, addressing environmental injustices, regulating land uses, greening energy markets, limiting pollution, and much more. While sociologists focus more on national policies, diverse sociological contributions emphasize the importance of SNEPs and their origins, diffusion, implementation, and sources of backlash. We begin by providing a typology of SNEPs. Next, we highlight not only environmental sociology (with its particular attention to climate change and energy) but also the sociologies of social movements, politics, the economy, science, risk, and organizations, which have each offered unique perspectives. Finally, we outline an agenda for how sociologists can further elaborate a distinctive perspective that highlights inequality, valuation, diffusion, scale shifts, and venue-shopping up to national and global policy systems.

Performance Evaluation of Structural Health Monitoring System Applied to Full-Size Composite Wing Spar via Probability of Detection Techniques.

Probability of detection (POD) is an acknowledged mean of evaluation for many investigations aiming at detecting some specific property of a subject of interest. For instance, it has had many applications for Non-Destructive Evaluation (NDE), aimed at identifying defects within structural architectures, and can easily be used for structural health monitoring (SHM) systems, meant as a compact and more integrated evolution of the former technology. In this paper, a probability of detection analysis is performed to estimate the reliability of an SHM system, applied to a wing box composite spar for bonding line quality assessment. Such a system is based on distributed fiber optics deployed on the reference component at specific locations for detecting strains; the attained data are then processed by a proprietary algorithm whose capability was already tested and reported in previous works, even at full-scale level. A finite element (FE) model, previously validated by experimental results, is used to simulate the presence of damage areas, whose effect is to modify strain transfer between adjacent parts. Numerical data are used to verify the capability of the SHM system in revealing the presence of the modeled physical discontinuities with respect to a specific set of loads, running along the beam up to cover its complete extension. The POD is then estimated through the analysis of the collected data sets, wide enough to assess the global SHM system performance. The results of this study eventually aim at improving the current strategies adopted for SHM for bonding analysis by identifying the intimate behavior of the system assessed at the date. The activities herein reported have been carried out within the RESUME project.

A time-space varying distributed unit hydrograph (TS-DUH) for operational flash flood forecasting using publicly-available datasets

Although flash floods originate in upstream (headwater) catchments causing hazards locally, they often occur at short time scales and affect widely while relatively small size catchments respond to intense rainfall systems. Due to the heavy cost of computing and parameterization, high resolution spatially distributed hydrological models are not effectively deployed for global or large-scale flash flood forecasting. This research proposes a Time-Space varying Distributed Unit Hydrograph (TS-DUH) based on publicly-available data for efficient global scale flash flood forecasting. TS-DUH estimates the runoff travel time (and flow velocity) from each location to the catchment outlet based on topographic and hydroclimatic characteristics, while it delineates the runoff-drainage process by emphasizing the heterogeneous and dynamic runoff contribution corresponding to rainfall and soil moisture variations. The excess rainfall is estimated by two methods: the Soil Conservation Service’s (SCS) curve (CN-Runoff), and the widely used Global Flood Monitoring System (GFMS) which provides both long-term (2000-present) well-archived and real-time online global runoff (GFMS-Runoff) prediction based on a distributed hydrological model. TS-DUH is first validated based on 6,324 flash flood events from 281 small-to-medium-sized catchments broadly distributed across the CONUS, using “perfect” precipitation input (i.e., NLDAS-2 for this study). TS-DUH calibrated with GFMS-Runoff shows better performance than using CN-Runoff, with a probability of detection (POD) value of 0.9 for all the events and 71% of them having a KGE metric above 0.5. More importantly, with the real-time satellite rainfall-driven GFMS-Runoff, the long-term (2003–2020) TS-DUH simulation shows better performance than GFMS, indicating promising value in global flash flood prediction of the TS-DUH-integrated GFMS.

Investigation and Finite Element Analysis of The Distal Weight-Bearing Implant

General Background: Osseointegration. a critical advancement in prosthetics, significantly benefits individuals with transfemoral amputations by enhancing their quality of life through innovative implant systems. Specific Background: The study examines a novel distal weight-bearing implant from 17 global systems, featuring a composite nanocoating of hydroxyapatite and silica, evaluated through finite element analysis and mechanical testing. Knowledge Gap: Research on nanocoating's impact on mechanical performance and its integration into advanced prosthetic designs is limited, despite extensive exploration of various implant systems. Aims: The study evaluates the distal weight-bearing implant's effectiveness, focusing on the nanocoating's role in shock absorption and mechanical stability during various gait cycle phases. Results: The design process involved creating a Ti-6Al-4V femoral stem and UHMWPE spacer, with the implant subjected to FEA under gait cycle conditions. Nanocoated samples demonstrated effective shock absorption, though with slightly reduced mechanical properties. The implant’s performance was evaluated for heel strike, midstance, and pre-swing phases, showing adequate load-bearing capacity within safe thresholds. Novelty: This study introduces a detailed analysis of nanocoating impacts on implant performance and integrates biomechanical forces into FEA for enhanced prosthetic design evaluation. Implications: Research indicates nanocoating enhances shock absorption, but further studies are needed to balance mechanical properties with biocompatibility and biological response, potentially improving amputee care outcomes.
