Negative health impacts of navigating the healthcare system for musculoskeletal conditions: A scoping review protocol.

Musculoskeletal (MSK) conditions, particularly shoulders, knees, and the low back issues, place a significant burden on individuals, society, and healthcare systems. There is a lack of attention to negative health effects impacting patients because of their interactions to access appropriate diagnostics, assessments, and treatments. This scoping review intends to search and synthesize peer-reviewed evidence on the negative health impacts associated with navigating the healthcare system for MSK care. A scoping review will be conducted following the PRISMA guidelines for Scoping Reviews and Arksey and O'Malley's 5-step process. Six databases will be searched with no time or geographic limits. Included articles must meet all the following criteria: 1) the patients must be adults, 2) patients must be seeking care for their knee, low-back, or shoulder condition, 3) interacted with the healthcare system, and 4) experienced health impacts due to navigating the healthcare system. Information from each article will be charted in a pre-determined extraction. This protocol aims to share our methods ahead of analysis to increase rigour and transparency. The scoping review results will better elucidate the health impacts of the inaccessibility of high-quality care for MSK conditions. The findings also aim to inform the development of patient-centered outcomes to evaluate alterations to the current MSK pathways.

Understanding the performance of geographic limits on Web of Science Core Collection databases, using the United Kingdom as an example.

To consider the approaches within Web of Science Core Collection (WoSCC) databases for limiting geographically. To compare the limits to an adaptation of NICE's UK MEDLINE filter for use on WoSCC databases. We tested and appraised the inbuilt functions and search field options that support identification by countries/regions and affiliations. We compared these with an adapted filter to identify healthcare research on or about the UK. We calculated the recall of the inbuilt limits and filter using 177 studies and investigated why records were missed. We also calculated the percentage reduction of the overall number-needed-to-screen (ONNS). Inbuilt limits within WoSCC enable identification of research from specific countries/regions or affiliations if there is data in the address field. Refining by affiliations allows retrieval of research where affiliations are in the 200 or 500 most frequent for a set of results. An adaptation of the UK MEDLINE filter achieved an average of 97% recall. ONNS was significantly reduced using the filter. However, studies where the countries or regions are only mentioned within the full text or other non-searchable fields will be missed. Information specialists should consider how inbuilt geographic limits operate on WoSCC and whether these are suitable for their research. The adapted filter can sensitively limit to the UK and could be useful for systematic reviews due to its high recall and ability to significantly reduce ONNS. Geographic filters can be feasible to adapt for use on WoSCC databases (where similar search fields are used between platforms).

The ecological and evolutionary dynamics of inselbergs.

Islands are fundamental model systems in ecology, biogeography, and evolutionary biology. However, terrestrial islands, unlike their aquatic counterparts, have received comparatively less attention. Among these land islands, inselbergs (i.e. isolated rock outcrops with diverse lithologies and a modest topographical prominence) stand out as iconic examples distributed worldwide across global biomes. Due to their durable lithology, inselbergs change slowly, persisting for tens of millions of years. In this review, we propose a biological definition for inselbergs that captures three fundamental characteristics of inselbergs from the perspective of biota. These are old age, isolation and the presence of unique microhabitats that are rare or absent in the surrounding matrix, fostering distinct communities often with unique and endemic biota. We synthesise the state of the art and formulate a set of testable hypotheses to deepen our understanding of the origins and maintenance of diversity on inselbergs, which are increasingly exposed to anthropogenic threats. By offering different habitats compared to the surrounding habitat matrix (e.g. moist microhabitats in dryland landscapes and xeric environments in humid tropical landscapes), inselbergs may allow specific lineages to thrive beyond their typical geographical limits. Particularly in drylands and degraded landscapes, inselbergs may not just provide different habitats but also act as ecological refuges or evolutionary refugia by providing a wider range of potential microhabitats than the surrounding matrix, enhancing resilience and promoting regional biodiversity. The central role of the matrix ensures that the ecological and evolutionary dynamics of inselbergs differ from those of true islands such as oceanic islands. Given that inselberg biota coexist within a terrestrial matrix, interactions between inselberg and matrix populations impact each other significantly. Over evolutionary timescales, matrix species may contract to inselberg refugia, preserving lineages while cycles of isolation and reconnection may drive speciation via a species pump. Although inselberg biodiversity has been studied predominantly from an island biogeography perspective, we argue that depending on the spatial scale, habitat specificity and mobility of the organisms considered, a range of different theories and paradigms can help explain the biogeography and local distribution patterns of different taxonomic and functional groups of inselberg species.

Strategies used to deliver bad news to the family of unexpected and sudden death victims: a scoping review protocol

Objective: This scoping review aims to map the strategies used during the communication of bad news to families of unexpected and sudden death victims from the perspective of those receiving the news. Introduction: The strategies used in communicating a person’s death to their family, especially in unexpected and sudden situations, can have a profound impact on the grief management process. This communication is often carried out by a health professional (doctor, nurse, or paramedic), but may also be carried out by a police officer, depending on the context in which the situation has occurred (in or out of hospital). Inclusion criteria: This scoping review will include studies on families of victims of unexpected and sudden death. All studies focusing on the strategies used to communicate bad news of unexpected and sudden death face to face, implemented by any professional in an intra-hospital or extra-hospital context, from the perspective of those who received the news, will be considered. Methods: The JBI methodology for scoping reviews will be followed. An initial search will be conducted, followed by a second search for published and unpublished studies in major health-related electronic databases. Studies published in English, Spanish, French, and Portuguese will be included, with no geographical, cultural, or time limits. Data selection, extraction, and synthesis will be performed independently by 2 reviewers and will include details of populations, study methods, and strategies used. Shifting alpine plant distributions with global change: Testing the environmental matching hypothesis

ABSTRACT Species facing novel temperature, precipitation, and nitrogen (N) deposition regimes must move or adapt to persist. For long-lived plants, a primary form of climate acclimation is through shifting geographic range limits or establishing in favorable microclimates. One commonly assumed but rarely tested hypothesis is that these shifts can be predicted by environmental matching: that the environmental characteristics that define a current distribution should predict how a population will shift with environmental changes. To test this hypothesis, we transplanted four alpine and two subalpine plant species into environments with experimentally increased temperature, snow, and N. We predicted that species would perform best when environmental change matched their geographic distributional characteristics: increased temperature, snow, and N (two subalpine species), increased temperature (two dry meadow specialists), and increased snow (two snowbed specialists). Our results provided limited support for the environmental matching hypothesis. Snowbed specialists did not benefit from increased snow, dry meadow specialists' performance did not consistently differ among the treatments, and subalpine plants' survival was not affected by treatments while their growth response was variable among species. Our results suggest that global change effects will vary among species and distributional shifts are not easily predicted by species environmental preference.

Crimean-Congo Hemorrhagic Fever Virus: An Emerging Threat in Europe with a Focus on Epidemiology in Spain.

Crimean-Congo hemorrhagic fever (CCHF) is a tick-borne disease transmitted by ticks of the genus Hyalomma and caused by a virus of the Nairoviridae family. In humans, the virus can generate different clinical presentations that can range from asymptomatic to mild illness or produce an hemorrhagic fever with a mortality rate of approximately 30%. The virus pathogenicity and the lack of effective treatment or vaccine for its prevention make it an agent of concern from a public health point of view. The main transmission route is tick bites, so people most exposed to this risk are more likely to become infected. Another risk group are veterinarians and livestock farmers who are in contact with the blood and other fluids of animals that are mostly asymptomatic. Finally, due to its first phase with a non-characteristic symptomatology, there exists a risk of nosocomial infection. It is endemic in Africa, the Balkans, the Middle East, and those Asian countries south of the 50th parallel north, the geographical limit of the main vector. Recently, autochthonous cases have been observed in areas of Europe where the virus was not previously present. Human cases have been detected in Greece, Bulgaria, and Spain. Spain is one of the most affected countries, with a total of 17 autochthonous cases detected since 2013. In other countries, such as France, the virus is present in ticks and animals but has not spread to humans. A high-quality epidemiological surveillance system in these countries is essential to avoid the expansion of this virus to new areas and to limit the impact of current cases.

How Digital Nomad Communities Can Empower Rural Revitalisation

The potential of digital nomad communities to support sustainable development and rural revival is examined in this research. Digital nomads are remote workers who travel and work from different places using technology. They provide skills, creativity, and cross-cultural interchange to rural areas. The desire for flexible lifestyles and worldwide trends toward remote work are congruent with the emergence of digital nomadism. Conversely, rural rejuvenation tackles issues including depopulation and economic downturns in rural areas, which are frequently made worse by urbanization and globalization. Rural communities can benefit from the new technology, varied businesses, and cultural enrichment that digital nomads can bring. Nonetheless, issues like geographical limits and infrastructure shortcomings must be resolved. The effective integration of digital nomads into rural areas requires policy support, teamwork, and new approaches. Rural communities may support economic growth, cultural preservation, and sustainable development by utilizing the skills and resources of digital nomads.

Distribution and host ranges of Ceratitis rosa and Ceratitis quilicii (Diptera: Tephritidae) in South Africa.

Two fruit fly (Diptera: Tephritidae) species of economic importance: Ceratitis rosa Karsch and Ceratitis quilicii De Meyer, Mwatawala & Virgilio are present in South Africa. The two species were considered as one species prior to 2016, but were subsequently separated. In this study, the distribution and abundance of the two species were quantified in seven provinces in South Africa through trapping with Enriched Ginger Oil as an attractant. Trapping was conducted over three seasons across two years (2020 and 2021): late summer, autumn-winter, and spring-early summer. Host ranges of the two species were investigated by fruit sampling in and outside of trapping sites. Ceratitis quilicii was more widely distributed than C. rosa with the latter being recorded in only three north-eastern provinces. There were geographical limits for both species with no records of them in Northern Cape Province. Catches of C. quilicii were higher in summer with average temperatures varying from 15 to 27°C while for C. rosa, catches remained low and consistent between seasons. Ceratitis quilicii catches decreased at lower rates than those of C. rosa at temperatures below 15°C. The two species were reared from 13 plant species from nine families. Four of these hosts were infested by both C. quilicii and C. rosa in the same province where they occurred. Preferred hosts of the two species belonged to the Myrtaceae family. The characterisation of the distribution, abundance and host ranges of these pests will provide a baseline for pest status determination and implementation of management actions.

An Integrative Review Exploring Womens' Experiences of Retraumatization Within Perinatal Services.

Evidence indicates that retraumatization has a detrimental effect for those women who are accessing perinatal services. One in five women worldwide has a history of childhood adversity. Between 18% and 34% of women experience trauma, which is a well-known risk factor for the onset of chronic mental health disorders. There is a lack of evidence on women's experiences on retraumatization in perinatal care settings and how to prevent retraumatization from occurring. The purpose of this study was to conduct an integrative review on women experiences of retraumatization to determine preventive measures within perinatal services. This integrative review followed Whittemore and Knafl's 5-stage framework as it allows for the inclusion and integration of diverse research methodologies into an overall synthesis of the evidence. A systematic search of 5 databases was conducted (Web of Science, MEDLINE, CINAHL, ASSIA, and PsychINFO) with no date, language, or geographical limits set due to the paucity of research published in this subject area. This review was conducted and reported according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. Fifteen studies met the inclusion criteria and were included in the thematic synthesis. The review identified that participants across the studies had a history of child sexual abuse, sexual abuse, and rape. Three main themes plus subthemes were identified: (1) activating (subthemes: positions in labor, intimate procedures, communications with health care professionals, loss of control); (2) outcomes (subtheme: emotional responses); and (3) interventions reducing or preventing retraumatization (subthemes: role of the health care professional, screening for abuse and history of trauma). Our findings demonstrate that women are experiencing retraumatization in perinatal services, and there is evidence of formalized approaches being applied in clinical settings to prevent retraumatization from occurring. This study is the first to examine the factors that contribute to retraumatization in perinatal services and make recommendations to reduce the harmful practices in place in perinatal care settings.

Birds optimize fruit size consumed near their geographic range limits.

Animals can adjust their diet to maximize energy or nutritional intake. For example, birds often target fruits that match their beak size because those fruits can be consumed more efficiently. We hypothesized that pressure to optimize diet-measured as matching between fruit and beak size-increases under stressful environments, such as those that determine species' range edges. Using fruit-consumption and trait information for 97 frugivorous bird and 831 plant species across six continents, we demonstrate that birds feed more frequently on closely size-matched fruits near their geographic range limits. This pattern was particularly strong for highly frugivorous birds, whereas opportunistic frugivores showed no such tendency. These findings highlight how frugivore interactions might respond to stressful conditions and reveal that trait matching may not predict resource use consistently.

Institutional model of commercial real estate market

The study considers institutions of the commercial real estate market with the aim to develop a descriptive institutional model. Institutional boundaries of the commercial real estate market are outlined. Time limits are determined by the equalization of supply and demand through the dynamics of rental rates. Geographical limits, despite the local character of the market, are determined by the participation of the largest developers. Commodity boundaries form the intended purpose of commercial real estate related to its use by lessees for business. This allows the interconnection of commercial real estate institutions to be considered from the perspective of value creation. By means of land relations, the initial conditions are determined by the institution of the state, which also establishes registration requirements. The developer connects the land supply with the rental market by developing a concept of the object, thus influencing related industries (developers, producers of construction materials). The institution of commercial real estate management ensures the interests of the developer. The composition of its participants is diverse, allowing the investor’s offer to be communicated to potential tenants as a value. The institute of accompanying participants of the commercial real estate market is mainly involved in information exchange between its other subjects; however, this institute is responsible for making the constituent links of the market sustainable. The lessee forms the final product using the value created by the developer (lessor) in the form of the intended purpose and individual properties of the object. Rental rates affect the profitability of commercial real estate, thereby determining the developer’s demand for capital. The results obtained can be used by both developers and researchers. The conclusion is made about the limited institutional role of the investor due to irrational capital raising and transferring these costs to the lessee. The reasons lie both in the strategies of most developers and in the desire to reduce risks through the preferred use of debt capital.

In defence of the entity of Macaronesia as a biogeographical region.

Since its coinage ca. 1850 AD by Philip Barker Webb, the biogeographical region of Macaronesia, consisting of the North Atlantic volcanic archipelagos of the Azores, Madeira with the tiny Selvagens, the Canaries and Cabo Verde, and for some authors different continental coastal strips, has been under dispute. Herein, after a brief introduction on the terminology and purpose of regionalism, we recover the origins of the Macaronesia name, concept and geographical adscription, as well as its biogeographical implications and how different authors have positioned themselves, using distinct terrestrial or marine floristic and/or faunistic taxa distributions and relationships for accepting or rejecting the existence of this biogeographical region. Four main issues related to Macaronesia are thoroughly discussed: (i) its independence from the Mediterranean phytogeographical region; (ii) discrepancies according to different taxa analysed; (iii) its geographical limits and the role of the continental enclave(s), and, (iv) the validity of the phytogeographical region level. We conclude that Macaronesia has its own identity and a sound phytogeographical foundation, and that this is mainly based on three different floristic components that are shared by the Macaronesian core (Madeira and the Canaries) and the outermost archipelagos (Azores and Cabo Verde). These floristic components are: (i) the Palaeotropical-Tethyan Geoflora, formerly much more widely distributed in Europe and North Africa and currently restricted to the three northern archipelagos (the Azores, Madeira and the Canaries); (ii) the African Rand Flora, still extant in the coastal margins of Africa and Arabia, and present in the southern archipelagos (Madeira, the Canaries and Cabo Verde), and (iii) the Macaronesian neoendemic floristic component, represented in all the archipelagos, a result of allopatric diversification promoted by isolation of Mediterranean ancestors that manage to colonize Central Macaronesia and, from there, the outer archipelagos. Finally, a differentiating floristic component recently colonized the different archipelagos from the nearest continental coast, providing them with different biogeographic flavours.

Is adaptation associated with long-term persistence beyond a geographic range limit?

Adaptation to new habitats might facilitate species' range shifts in response to climate change. In 2005, we transplanted experimental populations of coastal dune plant Camissoniopsis cheiranthifolia into 4 sites within and 1 site beyond its poleward range limit. Beyond-range transplants had high fitness but often delayed reproduction. To test for adaptation associated with experimental range expansion, we transplanted descendants from beyond- and within-range populations after 10 generations in situ into 2 sites within the range, 1 at the range edge, and 2 sites beyond the range. We expected to detect adaptation to beyond-range conditions due to substantial genetic variation within experimental populations and environmental variation among sites. However, individuals from beyond-range experimental populations were not fitter than those from within the range when planted at either beyond-range site, indicating no adaptation to the beyond-range site or beyond-range environments in general. Beyond-range descendants also did not suffer lower fitness within the range. Although reproduction was again delayed beyond the range, late reproduction was not favored more strongly beyond than within the range, and beyond-range descendants did not delay reproduction more than within-range descendants. Persistence in beyond-range environments may not require adaptation, which could allow a rapid response to climate change.

Migration patterns of the Japanese sardine in the Sea of Japan by combining the microscale stable isotope analysis of otoliths and an ocean data assimilation model

The Japanese sardine (Sardinops sagax melanostictus) is a small pelagic fish found in the Sea of Japan, the marginal sea of the western North Pacific. It is an important species for regional fisheries, but their dispersal and migration patterns during early life stages remain unclear. In this study, we analyzed the stable oxygen isotope ratios of otoliths of young-of-the-year (age 0) Japanese sardines collected from the northern offshore and southern coastal areas of the Sea of Japan in 2015 and 2016. The ontogenetic shifts of the geographic distribution were estimated by comparing the profiles of life-long isotope ratios and temporally varying isoscape, which was calculated using the temperature and salinity fields produced by an ocean data assimilation model. Individuals that were collected in the northern and southern areas hatched and stayed in the southern areas (west offshore of Kyushu) until late June, and thereafter, they can be distinguished into two groups: one that migrated northward at shallow layer and one that stayed around the southern area in the deep layer. A comparison of somatic growth trajectories of the two groups, which was reconstructed based on otolith microstructure analysis, suggested that individuals that migrated northward had significantly larger body lengths in late June than those that stayed in the southern area. These results indicate that young-of-the-year Japanese sardines that hatched in the southern area may have been forced to choose one of two strategies to avoid extremely high water temperatures within seasonal and geographical limits. These include migrating northward or moving to deeper layers. Our results indicate that the environmental variabilities in the southern area could critically impact sardine population dynamics in the Sea of Japan.

Investigating the impact of temperature on growth rate of the root rot fungus, Gymnopus fusipes.

Gymnopus fusipes is an understudied root rot pathogen associated with multiple tree species and is linked to episodes of oak decline across the United Kingdom and Europe. Although the reported distribution of G. fusipes is broad, many observations rely solely on visual identification of fruiting bodies, which can be unreliable, and lack confirmation by molecular and/or isolation data to verify this broad ecological range. Given the paucity of information regarding the true ecological distribution of G. fusipes, it is difficult to predict and model the potential distribution of the species under both current and future climate scenarios. In this study, to determine the growth capabilities of G. fusipes across a range of ecologically relevant temperatures, five geographically diverse isolates of G. fusipes were grown at five different temperatures ranging from 4-37°C, to determine the optimal temperature for G. fusipes growth, and to establish whether geographically diverse isolates exhibit local adaptation to temperature tolerance. Incubation temperature had a significant effect on G. fusipes growth rate, with 25°C representing the optimum (P<0.001). Isolates had differing growth rates at each of the temperatures, with an isolate from the UK having the highest overall growth rate across all five temperatures tested (P<0.001), and at the optimum, increased by a mean value of over 4915 mm2. Local adaptation to temperature tolerance was not found in the isolates tested. These data demonstrate the optimal incubation temperature for future laboratory studies on G. fusipes and provide the first data on the growth rate of this pathogen across ecologically relevant climate ranges that may inform land managers, modellers, and policy makers in predicting the current and potentially future geographical limits of this widespread root rot pathogen.

Forecasting climate change impacts on neotropical Myotis: Insights from ecological niche models for conservation strategies.

Myotis originated during the Oligocene in Eurasia and has become one of the most diverse bat genera, with over 140 species. In the case of neotropical Myotis, there is a high degree of phenotypic conservatism. This means that the taxonomic and geographic limits of several species are not well understood, which constrains detailed studies on their ecology and evolution and how to effectively protect these species. Similar to other organisms, bats may respond to climate change by moving to different areas, adapting to new conditions, or going extinct. Ecological niche models have become established as an efficient and widely used method for interpolating (and sometimes extrapolating) species' distributions and offer an effective tool for identifying species conservation requirements and forecasting how global environmental changes may affect species distribution. How species respond to climate change is a key point for understanding their vulnerability and designing effective conservation strategies in the future. Thus, here, we assessed the impacts of climate change on the past and future distributions of two phylogenetically related species, Myotis ruber and Myotis keaysi. The results showed that the species are influenced by changes in temperature, and for M. ruber, precipitation also becomes important. Furthermore, M. ruber appears to have been more flexible to decreases in temperature that occurred in the past, which allowed it to expand its areas of environmental suitability, unlike M. keaysi, which decreased and concentrated these areas. However, despite a drastic decrease in the spatial area of environmental suitability of these species in the future, there are areas of potential climate stability that have been maintained since the Pleistocene, indicating where conservation efforts need to be concentrated in the future.

Frontex: Transcending the EU Borders

ABSTRACT Does the work of Frontex transform borderlands? The article aims to explore the role of the European Border and Coast Guard Agency (Frontex) beyond the EU borders. Challenging the dominant portrayal of Frontex as a technical border control “tool” deployed at the EU borders, this article unpacks Frontex’s impact outside the EU geographic space. Applying the “cultures of border control” analytical framework and analyzing Frontex’s cooperation with third countries, it casts light on Frontex’s cultural impact in the Western Balkan region, using Albania and Serbia as case studies. The article argues that Frontex is expanding and diffusing the EU border control culture beyond the EU borders. This transcends Frontex’s institutional mandate set by its creators and the technical nature of cooperation agreed by its third partners. Actually, through its cultural impact, it both solidifies and expands its role and salience in border control, escaping the EU geographic limits.

Uncovering mangrove range limits using very high resolution satellite imagery to detect fine‐scale mangrove and saltmarsh habitats in dynamic coastal ecotones

AbstractMangroves are important ecosystems for coastal biodiversity, resilience and carbon dynamics that are being threatened globally by human pressures and the impacts of climate change. Yet, at several geographic range limits in tropical–temperate transition zones, mangrove ecosystems are expanding poleward in response to changing macroclimatic drivers. Mangroves near range limits often grow to smaller statures and form dynamic, patchy distributions with other coastal habitats, which are difficult to map using moderate‐resolution (30‐m) satellite imagery. As a result, many of these mangrove areas are missing in global distribution maps. To better map small, scrub mangroves, we tested Landsat (30‐m) and Sentinel (10‐m) against very high resolution (VHR) Planet (3‐m) and WorldView (1.8‐m) imagery and assessed the accuracy of machine learning classification approaches in discerning current (2022) mangrove and saltmarsh from other coastal habitats in a rapidly changing ecotone along the east coast of Florida, USA. Our aim is to (1) quantify the mappable differences in landscape composition and complexity, class dominance and spatial properties of mangrove and saltmarsh patches due to image resolution; and (2) to resolve mapping uncertainties in the region. We found that the ability of Landsat to map mangrove distributions at the leading range edge was hampered by the size and extent of mangrove stands being too small for detection (50% accuracy). WorldView was the most successful in discerning mangroves from other wetland habitats (84% accuracy), closely followed by Planet (82%) and Sentinel (81%). With WorldView, we detected 800 ha of mangroves within the Florida range‐limit study area, 35% more mangroves than were detected with Planet, 114% more than Sentinel and 537% more than Landsat. Higher‐resolution imagery helped reveal additional variability in landscape metrics quantifying diversity, spatial configuration and connectedness among mangrove and saltmarsh habitats at the landscape, class and patch scales. Overall, VHR satellite imagery improved our ability to map mangroves at range limits and can help supplement moderate‐resolution global distributions and outdated regional maps.

Forest Flora of Nichlaul-IV. Trees

This paper enlists the trees found in Nichlaul forest, Gorakhpur Division (U.P.). Geographical limits, topography, edaphic and climatic factors of the area, its vegetational composition, injurious factors, phenological behaviour of the common plants and an artificial key to the families as well as to the species are given. In all, 109 tree species belonging to 91 genera and 44 families are included, giving field observations, local names, habit, habitat, dominance and sociability. Flowering and fruiting seasons as well as collection numbers of plants are indicated.
