O-BN07 Transesophageal Endoscopic Resection of Mediastinal Cysts

Abstract Background Primary mediastinal cysts are infrequent lesions derived from a variety of mediastinal organs or structures. Complete surgical resection is the treatment of choice even in asymptomatic patients to prevent severe adverse events (AEs) and establish the diagnosis. Transesophageal endoscopic resection of benign mediastinal tumors has been proven feasible. The aim of this study is to evaluate the feasibility, safety and efficacy of transesophageal endoscopic surgery for mediastinal cysts. Methods From January 2016 to May 2021, patients with mediastinal cysts who underwent transesophageal endoscopic resection were retrospectively included. Clinicopathological characteristics, procedure-related parameters, AEs, and follow-up outcomes were analyzed. Results A total of 10 patients with mediastinal cysts were included in this study. The mean cyst size was 3.3±1.3 cm. Histopathology revealed 3 bronchogenic cysts (30.0%), 4 esophageal duplication cysts (40.0%), 2 gastroenteric cysts (20.0%), and 1 lymphatic cyst (10.0%). All procedures were performed uneventfully. En bloc resection was achieved in 6 patients (60.0%). Aggressive resection was avoided to prevent damage to the surrounding vital organs. Mean procedure time was 63.4±36.5 min. No major pneumothorax, bleeding, mucosal injury or fistula occurred. One patient had a transient febrile episode (>38.5 °C). Mean postoperative hospital stay was 2.7±0.9 days. No residual or recurrent lesions were observed in any patient during a mean follow-up period of 29.8±19.5 months. Conclusions Transesophageal endoscopic surgery appears to be a feasible, safe, effective and much less invasive approach for mediastinal cyst resection. Larger prospective studies are required to fully assess the efficacy and safety of this novel technique.

Multiple Bronchogenic and Gastroenteric Cysts Arising from the Stomach in a Patient with Abdominal Pain

Bronchogenic cysts arising from the stomach are uncommon. We discuss a young female patient with presumed enteric duplication cysts who was found to have three bronchogenic and gastroenteric cysts upon pathologic review. We discuss the pathophysiology of bronchogenic cysts and their malignant potential.

Anterior mediastinal gastroenteric cyst containing pancreatic tissue influenced the diabetes mellitus status

The mediastinal gastroenteric cyst is a rare developmental cyst. In general, the majority of mediastinal gastroenteric cysts are recognized in infancy and are commonly located in the posterior mediastinum. In addition, gastroenteric cysts occasionally contain pancreatic tissue; however, no studies have reported that these cysts influenced the diabetes mellitus status of the patient. We report here an extremely rare case of an adult gastroenteric cyst with pancreatic tissue originating from the anterior mediastinum, influencing the severity of diabetes mellitus in a patient. This case report presents two remarkable findings. First, the location of the gastroenteric cyst (anterior mediastinum) and the patient's age at the time of detection (adulthood) were extremely rare. Secondly, the present case is also unusual in terms of glycometabolism. As this tumour decreased in size, the glycosylated haemoglobin value increased, which suggested a worsening of the patient's diabetes mellitus. The diabetes mellitus further worsened after removal of the tumour.

Mediastinal Chest Masses in Children

In children, as in adults, mediastinal chest masses are considered on the basis of their location in the mediastinum: i.e., anterior, middle, or posterior. Most of these masses are first detected on plain chest films but then are investigated with CT, MRI, and occasionally nuclear scintigraphy. Nuclear scintigraphy is used primarily for lesions suspected of being thyroid in origin or gastroenteric cysts containing gastric mucosa. For the most part, plain chest films provide information regarding mass location, with subsequent information derived from CT scans, MR images, and ultrasound imaging.

Usually, plain chest films provide basic information regarding the location of the mass, with subsequent information derived from CT, MR, and ultrasound imaging. This article (the first part of a two-part series) deals with anterior mediastinal chest masses in children. Middle and posterior mediastinal chest masses in children will be discussed as the second part of the series in Volume 33, Number 14.

Mediastinal gastroenteric cyst in a neonate containing respiratory‐type epithelium and pancreatic tissue

Mediastinal gastroenteric cyst is an uncommon congenital malformation and a distinct histopathological entity within the family of foregut duplication cysts. This lesion is mainly encountered in neonates and infants. Histologically, it is characterized by double-layered smooth muscle wall and gastric lining mucosa. We report on a case of a 2-day-old girl, with a posterior mediastinal cystic mass associated with T3-T4 hemivertebrae, presenting with severe respiratory distress. The cyst was multilocular, surgically removed, and histopathologic analysis revealed that it was of gastroenteric type. However, in numerous areas of the lesion, respiratory-type epithelium was observed, as well as pancreatic tissue. After removal of the lesion the patient made an uneventful recovery and shows no signs of long-term pulmonary sequelae. We failed to demonstrate in the available literature the presence of this variable epithelial lining within a single mediastinal foregut cyst. In addition, pancreatic tissue within an intrathoracic enteric cyst has been reported only twice.

A giant gastroenteric cyst associated with pectus excavatum and compression of the thoracic duct: a case report

Gastroenteric cysts are unusual developmental cysts encountered in the posterior mediastinum of patients. They are usually lined by alimentary (squamous or enteric) epithelium and contain gastric mucosa and, rarely, pancreatic tissue. Gastroenteric cysts are usually associated with vertebral abnormalities and less often with complete situs inversus. We present a case of a gastroenteric cyst with pectus excavatum and compression of the thoracic duct.

Alimentary tract duplications.

Duplications of the alimentary tract may be defined as those cystic and tubular structures found in relation to normal bowel whose walls resemble intestine and consist of smooth muscle with alimentary tract mucosa. Synonyms for duplication include enterocystoma, enterogenous cyst, enteric cyst, giant diverticulum, ileum duplex, gastroenteric cyst, gastrogenous cyst, and reduplication of the intestine. Atypical and ectopic Meckel's diverticula are usually duplications. Single reports and reports of small numbers of cases in the literature suggest that duplications occur infrequently. This report concerns 25 cases in which one or more alimentary tract duplications were present. were Negro and 22 were white. There were 12 male and 13 female patients. Duplications were discovered or confirmed at surgery in 19 cases. Duplications were discovered at necropsy in six patients. In three instances (Table 1) it was small and had been asymptomatic.
