Sensor-Acquired Reachable Workspace (RWS) Correlates with Activities of Daily Living (ADL) Function in Stroke as Measured by Functional Independence Measure (FIM) Self-Care.

Optimal upper extremity motor control and range of motion are necessary to achieve even the basic activities of daily living (ADL) function. Stroke, with resulting hemiparesis, can significantly and negatively impact an individual's ADL function. Functional Independence Measure (FIM) self-care score can provide an assessment of what aspects and to what degree ADL functions are impaired. FIM self-care assessment can also track changes in ADL function during stroke recovery and rehabilitation. Recently, the sensor-acquired 3D motion analysis of stroke patients' upper extremity has shown promise as a potential alternative to assess ADL function. This observational study evaluates whether the sensor-acquired upper extremity reachable workspace (RWS) measure correlates with clinician-evaluated FIM self-care score in stroke patients. Seventeen patients with stroke were enrolled in the study. FIM self-care, NeuroQoL upper extremity, and reachable workspace outcome measures (relative surface area, RSA) were collected upon rehabilitation hospital admission, at discharge, and at the 3-month visit. Pearson and Spearman's rank correlation coefficients as well as multiple linear regression analyses were used to determine the relationships between FIM self-care, NeuroQoL, and reachable workspace RSAs. Moderately strong correlation between total reachable workspace RSA and total FIM self-care score at discharge and at 3 months were noted (r = 0.619, r = 0.661, p < 0.05), and similarly strong correlation was also noted with the upper extremity NeuroQoL total score (r = 0.690, r = 0.815, p < 0.05). Multiple linear regression analyses revealed a change in average bilateral total RSA of 0.1 unit from admission to the 3-month follow-up correlated with a respective change in the FIM self-care score of 2.011 points (95%CI: 0.663-3.360). Longitudinal improvement in ADL function during stroke rehabilitation and recovery process is correlated with improvement in reachable workspace.

Construct Validity of the Stroke Upper-Limb Capacity Scale as a Measure of Upper Extremity Capacity

<h3>Research Objectives</h3> The Stroke Upper-Limb Capacity Scale (SULCS) is a standardized assessment of 10 upper extremity (UE) functional tasks, but there are little data on construct validity in acute stroke. Using International Classification of Function definitions, we hypothesize: H1 – a stronger relationship between the SULCS and another activity measure (Functional Independence Measure Selfcare (FIM-SC)), as compared to the SULCS and an impairment measure Motricity Index-UE (MI-UE)); H2 – a stronger association between the SULCS and UE items on the FIM (eating, grooming and bathing (FIM-E, FIM-G, FIM-Ba, respectively)) compared to FIM items not dependent on UE function (walking, bowel and expression (FIM-W, FIM-Bo, FIM-Exp, respectively)). <h3>Design</h3> Cross-sectional. <h3>Setting</h3> Academic inpatient rehabilitation unit. <h3>Participants</h3> N=341/488 (70%) admissions with complete data, age=70±15 years, time post stroke=10±9 days, length of stay=14±8 days, female=47.2%, Admission Total FIM=60±20, Admission SULCS=5.5±3.6, Admission MI-UE=65±35. <h3>Interventions</h3> N/A. <h3>Main Outcome Measures</h3> SULCS, FIM and MI-UE. <h3>Results</h3> H1: SULCS correlated significantly with FIM-SC and MI-UE (ρ=0.74 and ρ=0.85, respectively; (p< 0.001)). H2: SULCS correlated significantly with upper extremity selfcare items (FIM-E/ρ=0.61, FIM-G/ρ=0.56 and FIM-Ba/ρ=0.60), and significantly but more weakly with non-upper extremity items (FIM-Bo/ρ=0.47, FIM-W/ρ=0.57 and FIM-Exp/ρ=0.37). All p< 0.001. <h3>Conclusions</h3> Contrary to H1, SULCS was similarly and strongly correlated with MI-UE and FIM. The inability to compensate for hemiparesis with the intact extremity on both SULCS and MI-UE may explain this unexpected finding. H2 was generally supported, as SULCS demonstrated stronger correlations with UE selfcare items compared to non-UE items. The correlation between SULCS and FIM-W is likely due to hemiparesis in stroke commonly affecting upper and lower extremities. Relationships Between Functionality, Depression, and Anxiety With Community Integration and Quality of Life in Chronic Traumatic Spinal Cord Injury.

The aims of the study were (1) to identify relationships between functional and psychological aspects with community integration and quality of life assessments in people with chronic traumatic spinal cord injury and (2) to analyze clinical and demographic predictors of quality of life dimensions. This is an observational cohort study, and correlation coefficients were calculated between the Functional Independence Measure, the Hospital Anxiety and Depression Scale, the Community Integration Questionnaire, and the World Health Organization Quality of Life-BREF dimensions (physical [D1], psychological [D2], social [D3], and environmental [D4]). Quality of life predictors were identified using multiple linear regression analyses. Nine hundred seventy-five people with traumatic spinal cord injury assessed since 2007-2020 were included. The Community Integration Questionnaire home integration correlated strongly with the Functional Independence Measure self-care (r = 0.74) and transfers (r = 0.62) for participants with tetraplegia. The specific Hospital Anxiety and Depression Scale items (known as the anhedonia subscale) correlated strongly with D1 (r = -0.65), D2 (r = -0.69), D3 (r = -0.53), and D4 (r = -0.51) for participants with paraplegia and D1 (r = -0.53), D2 (r = -0.61), D3 (r = -0.47), and D4 (r = -0.53) for participants with tetraplegia. The Hospital Anxiety and Depression Scale-depression was the most relevant predictor of D1 (β = -0.61) and D2 (β = -0.76). The Functional Independence Measure transfers and self-care were strongly associated with the Community Integration Questionnaire home integration (in participants with tetraplegia). Anhedonia was strongly related to all four World Health Organization Quality of Life-BREF Scale dimensions, being the Hospital Anxiety and Depression Scale-depression the most relevant predictor of D1 and D2. Type and Timing of Rehabilitation Following Acute and Subacute Spinal Cord Injury: A Systematic Review.

Objectives:The objective of this study was to conduct a systematic review of the literature to address the following clinical questions: In adult patients with acute and subacute complete or incomplete traumatic SCI, (1) does the time interval between injury and commencing rehabilitation affect outcome?; (2) what is the comparative effectiveness of different rehabilitation strategies, including different intensities and durations of treatment?; (3) are there patient or injury characteristics that affect the efficacy of rehabilitation?; and (4) what is the cost-effectiveness of various rehabilitation strategies?Methods:A systematic search was conducted for literature published through March 31, 2015 that evaluated rehabilitation strategies in adults with acute or subacute traumatic SCI at any level. Studies were critically appraised individually and the overall strength of evidence was evaluated using methods proposed by the GRADE (Grades of Recommendation Assessment, Development and Evaluation) working group.Results:The search strategy yielded 384 articles, 19 of which met our inclusion criteria. Based on our results, there was no difference between body weight–supported treadmill training and conventional rehabilitation with respect to improvements in Functional Independence Measure (FIM) Locomotor score, Lower Extremity Motor Scores, the distance walked in 6 minutes or gait velocity over 15.2 m. Functional electrical therapy resulted in slightly better FIM Motor, FIM Self-Care, and Spinal Cord Independence Measure Self-Care subscores compared with conventional occupational therapy. Comparisons using the Toronto Rehabilitation Institute Hand Function Test demonstrated no differences between groups in 7 of 9 domains. There were no clinically important differences in Maximal Lean Test, Maximal Sidewards Reach Test, T-shirt Test, or the Canadian Occupational Performance Measure between unsupported sitting training and standard in-patient rehabilitation.Conclusion:The current evidence base for rehabilitation following acute and subacute spinal cord injury is limited. Methodological challenges have contributed to this and further research is still needed.

Self-esteem level and stability, admission functional status, and depressive symptoms in acute inpatient stroke rehabilitation.

Explore the relationship of self-esteem level, self-esteem stability, and admission functional status on discharge depressive symptoms in acute stroke rehabilitation. One hundred twenty stroke survivors serially completed a measure of state self-esteem during inpatient rehabilitation and completed a measure of depressive symptoms at discharge. Functional status was rated at admission using the Functional Independence Measure (FIM). Regressions explored main effects and interactions of self-esteem level and stability and admission FIM self-care, mobility, and cognitive functioning on discharge depressive symptoms. After controlling for potential moderating variables, self-esteem level interacted with FIM self-care and cognitive functioning to predict discharge depressive symptoms, such that survivors with lower self-rated self-esteem and poorer functional status indicated higher levels of depressive symptoms. Self-esteem stability interacted with FIM mobility functioning, such that self-esteem instability in the presence of lower mobility functioning at admission was related to higher depressive symptoms at discharge. These results suggest that self-esteem variables may moderate the relationship between functional status and depressive symptoms. Self-esteem level and stability may differentially moderate functional domains, although this conclusion requires further empirical support.

Multilevel modeling of self-esteem change during acute inpatient stroke rehabilitation.

To explore self-esteem change during inpatient stroke rehabilitation and moderators of change. One hundred twenty survivors of stroke serially completed the State Self-Esteem Scale (SSES) during inpatient rehabilitation, as well as measures of mood and perceived recovery as potential moderators of change. Age, gender, prior stroke, prestroke depression, stroke laterality, and admission Functional Independence Measure (FIM) self-care, mobility, and cognitive scores were also included as moderators. Multilevel modeling of the repeated administrations of the SSES indicated that self-esteem significantly improved during rehabilitation. Female gender, left hemisphere stroke, prior stroke, and lower admission FIM cognitive scores were associated with lower self-esteem ratings at admission, but only age and admission FIM self-care and mobility scores were associated with self-esteem change. Older individuals showed less self-esteem improvement than younger individuals, and higher self-care and mobility scores at admission were associated with greater self-esteem improvement. While mood change significantly covaried with self-esteem, the rate of mood change did not appear to influence rate of self-esteem change. Greater improvement in self-esteem over time was related to lower levels of perceived recovery, but this was likely because of the relationship between perceived recovery and self-esteem at rehabilitation admission. These results suggest that self-esteem improves during inpatient rehabilitation, and this change may be partially dependent on functional status. Implications for facilitating self-esteem change by the clinician are discussed, as well as future research directions.

Motor impairment as a predictor of functional recovery and guide to rehabilitation treatment after stroke.

This study tests three hypotheses relevant for the efficient use of rehabilitation services after stroke: (a) the severity of initial motor impairment after stroke predicts discharge motor impairment and self-care mobility scores; (b) identification of those unlikely to show improvement in motor impairment can focus rehabilitzation efforts on use of compensatory techniques and assist devices; and (c) improvement in self-care mobility scores without change in motor impairment, balance, or cognition is a quantitative estimate of the value of teaching compensatory techniques and use of assist devices. We studied 171 sequential patients previously independent in the community who were admitted for inpatient rehabilitation within 17 +/- 12 SD days of an initial, unilateral, hemispheric, ischemic stroke. Impairment was assessed using the Fugl-Meyer upper limb motor (ULM), lower limb motor (LLM), and upper plus lower limb total motor (TM) subscores. Disability was assessed using the Functional Independence Measure (FIM), FIM self-care (FIMS), FIM mobility (FIMM), and FIM self-care plus FIM mobility (FIMSM) subscores. Spearman correlation coefficients tested strength of association between dependent and independent variables, stepwise linear regression tested the effects of clinically relevant co-variables, and positive and negative predictive values (PPV, NPV) assessed the clinical relevance of outcome-prediction models. The highest correlations observed were between admission TM scores and the following discharge scores: TM (R = 0.92; p < 0.01), ULM (R = 0.91; p < 0.01), LLM (R = 0.82; p < 0.01), FIMSM (R = 0.67; p < 0.01), FIMM (R = 0.67; p < 0.001), FIM (R = 0.58; p < 0.0001). An admission TM score in the lowest quartile had a PPV of 0.74 for a discharge ULM score in the lowest quartile. An admission TM score in the highest quartile had a PPV of 0.86 for a discharge ULM score in the highest quartile. Similar but weaker PPVs were seen for admission TM scores and discharge LLM scores. Patients without significant change in TM scores (< or = 2 points) had a 17 +/- 9 SD improvement in FIMSM scores. Admission motor impairment scores (a) predict discharge impairment and activities of daily living mobility functional outcome; and (b) guide treatment toward improving motor impairment versus use of compensatory techniques and assistive devices. The use of compensatory techniques and assistive devices, without change in motor impairment, is associated with a 17 +/- 9 SD improvement in FIMSM score.

Admission motor impairment as a predictor of physical disability after stroke rehabilitation.

The purpose of this study is to investigate the Fugl-Meyer Motor Impairment Scale and physical disability as measured by the Functional Independence Measure (FIM) as predictors of physical independence after stroke inpatient rehabilitation. Forty-eight patients admitted to a rehabilitation hospital within 6 wk of their first unifocal stroke were prospectively studied. Upper and lower extremity Fugl-Meyer scores were obtained near rehabilitation admission. FIMs were obtained on admission and discharge. Discharge FIM self-care was predicted best by admission FIM total (rs = 0.75; P < 0.0001), although admission FIM self-care, other FIM components, and upper- and lower-extremity Fugl-Meyer scores were also good predictors. Discharge FIM mobility and locomotion correlated well with their admission counterparts (rs = 0.54 and 0.45; P < 0.001 and < 0.01, respectively) but were even more strongly related to admission Fugl-Meyer scores (rs = 0.63 and 0.74, respectively; P < 0.0001). As expected, cognitive and psychosocial components of the FIM at discharge correlated poorly with admission Fugl-Meyer. These data confirm the strong motor impairment basis for the physical disability subdimensions of the FIM and suggest that dependency in physical activities of daily living after stroke is primarily determined by degree of motor impairment.
