A 24-year-Old Boy with Previously Treated and Aberrant Root Canal Anatomy in Mandibular Left First Premolar Tooth

Perfect cleaning and shaping is the key to successful root canal treatment. Therefore, a careful understanding of the internal anatomy and structure of the root canal system is an important factor when performing cleaning and cosmetic procedures. Mandibular premolars are among the most complicated teeth to treat with root canals due to their abnormal root canal anatomy. Furthermore, it varies in different ethnic groups. Therefore, it is needed to evaluate and search for an abnormal root canal anatomy in a failed root canal. In light of these observations, this case report provides a case of previously root canal treated mandibular first premolar tooth which was failed due to different root canal patterns. The tooth was subjected to re-root canal treatment and after completion of the therapy, the tooth was symptomless and patient was referred to the Department of Prosthodontics for a full veneer crown.

Comparison Between Maximum Bite Force on First Molar with Prosthesis and Contralateral First Molar Without Prosthesis

Background: Bite force is an indicator of the functional state of the masticatory system that results from the action of jaw elevator muscles modified by the craniomandibular biomechanics. Bite force measurement is essential in dental treatment as an extreme bite force on tooth may affect health of periodontium and increase risk of prosthesis fracture. A patient with low biting force may be helpful for the successful long-term outcome of prosthesis. A variety of devices has been used to record human bite force. Nowadays, sensitive electronic devices are used those are both accurate and precise enough. Recently, T-scan has been more popular to record the pattern of occlusion. The T-scan system precisely and dynamically records the occlusal contact time, force, and area in percentage. T-scan identify both force and timing which are the most fundamental parameters for measuring occlusion. The present study was designed to compare maximum bite force on first molar with prosthesis and contralateral first molar without prosthesis by using T-scan. Objectives: To compare maximum bite force (MBF) on first molar with prosthesis and contralateral first molar without prosthesis. Materials and Methods: This quasi-experimental type study was conducted in the Department of Prosthodontics, BSMMU. The patients were selected by a consecutive sampling technique, requiring permanent restoration in endodontically treated first molar on one side and natural first molar on contralateral side. The selected tooth was prepared for full veneer crown following standard procedure with armamentarium. Symptomless patients up to 3 months were measured bite force on cemented prosthesis and contralateral natural tooth. Bite force was measured with T-scan for three times with 30 sec interval and highest value was recorded in a data collection sheet for each patient to compare maximum bite force on first molar with prosthesis and contralateral first molar without prosthesis in the same patient. Result: The maximum bite force at the first molar without prosthesis was 14.30 ±4.2% and first molar with prosthesis was 12.05 ±3.55%. Tooth without prosthesis maximum bite force was significantly more than the tooth with prosthesis. Conclusion: Maximum bite force was significantly higher in tooth without prosthesis than the tooth with prosthesis. The difference of maximum bite force was not statistically significant in relation to gender, age and BMI.

Impact of marginal defects on the accuracy of automated finish line detection in tooth preparation.

The present study aimed to evaluate the accuracy of automated detection of preparation finish lines in teeth with defective margins. An extracted first molar was prepared for a full veneer crown, and marginal defects were created and scanned (discontinuity of finish line: 0.5, 1.0, and 1.5 mm; additional line angle: connected, partially connected, and disconnected). Six virtual defect models were entered into CAD software and the preparation finish line was designated by 20 clinicians (CAD-experienced group: n = 10; CAD-inexperienced group: n = 10) using the automated finish line detection method. The accuracy of automatic detection was evaluated by calculating the 3D deviation of the registered finish line. The Kruskal-Wallis and Mann-Whitney U tests were used for between-group comparisons (α = 0.05). The deviation values of the registered finish lines were significantly different according to conditions with different amounts of finish line discontinuity (P < 0.001). There was no statistical difference in the deviation of the registered finish line between models with additional line angles around the margin. Moreover, no statistical difference was found in the results between CAD-experienced and CAD-inexperienced operators. The accuracy of automated finish line detection for tooth preparation can differ when the finish line is discontinuous. The presence of an additional line angle around the preparation margin and prior experience in dental CAD software do not affect the accuracy of automated finish line detection.

Evaluation of the Quality of Undergraduate Full Veneer Crown Preparations at a UK Dental Teaching Hospital.

To determine the quality of full veneer crown preparations produced by supervised undergraduate students at a UK dental teaching hospital. One hundred and eighty-five scanned digital dies between October 2019 and March 2021 were obtained. Using cross-sections in four planes, the total angle of convergence, abutment height, margin design and margin depth were evaluated. Statistical comparisons were made by plane, location, material-type, tooth-type, and inter- and intra-arch positions. Across all preparations the mean total angle of convergence was 24.8° ± 11.7°. and mean abutment height was 3.6 mm ± 1.0 mm. Mandibular and molar teeth were significantly more tapered (P ⟨0.001) and exhibited significantly shorter abutment heights (P ⟨0.001). Chamfer margins were the most frequently observed and mean margin depths ranged from 0.49-1.06 mm. The compliance to recommended taught parameters were 28.1%, 42.7%, 34.1% and 6.5% for total angle of convergence, abutment height, margin design and margin depth, respectively. The findings from this study suggest that compliance to taught parameters is poor, especially for molar teeth, and demonstrates the use of digital software in guiding future research and teaching.

Clinical evaluation of crown and bridge prosthesis placed in patients attended in the Bangabandhu Sheikh Mujib Medical University

The proper diagnosis and clinical assessment are essential when dealing with the complications of fixed restorations in dental prosthesis. This cross-sectional study was carried out to evaluate clinically the crown and bridge prosthesis among the patients attending in the Department of Prosthodontics, Bangabandhu Sheikh Mujib Medical University, Dhaka, Bangladesh. A total of 50 patients with full veneer crowns or bridges were selected. Clinical assessment for the contour of the prosthesis, nature of the contact of the prosthesis, type of occlusal contact in ICP were done. Among the study subjects, 44% and 52% of the prostheses were over-contoured buccally and lingually, respectively. The prostheses contacted with adjacent tooth mesially at occlusal one-third in 58%, middle one-third in 40% &amp; cervical one-third in 2% cases. The prostheses contacted with adjacent tooth distally at occlusal one-third in 52%, middle one-third in 44% &amp; nothing abnormality detected in 4%. About half of prostheses (54%) contacted with an opposite tooth at the level of central fossa whereas 40% at buccal to central fossa and only a few numbers (6%) at lingual/palatal to central fossa. The nature of proximal contact was normal in 52%, tight in 36%, and open in 12% of the prostheses. As the prostheses cemented in the patient's mouth were not up to the accepted clinical standards, so enrichment of knowledge about prosthetics are needed and responsibilities should be ensured. Micro-CT analysis of 3D printed provisional crowns fitting.

The provisional crowns (PR) play an important role during the course of fixed prosthesis treatment. The fitting of PR varies and depends on various factors. The aim of the study was to evaluate the marginal and internal fit of PR produced by three-dimensional (3D) printing technique and to compare those with PR made by the milling technique by using micro computed tomography (μCT) on three commonly used finish line designs. Ninety study models were made using metal die of #14 tooth (i.e. maxillary 1st premolar molar) prepared for full veneer crown with three different finish line chamfer (C), rounded shoulder (RS) and rounded shoulder with bevel (RSB). PR was fabricated on each study model, using milling (MiL group, n= 45), and 3D printing technique (3D-P group, n= 45). Marginal and internal fit of each study model was measured by micro computed tomography, at 7 Zones Pr-1- Pr-7 on each finish line. Recorded data were statistically analysed by one-way analysis of variance (ANOVA) and using Dunnett t-Test (p> 0.05). The mean gap at margins was minimum for provisional crowns in 3D-P group in any finish line with minimum in rounded shoulder with bevel at zone Pr-1 30.9 ± 5.3 and at zone Pr-7 32.7 ± 5.3. In the axial region, i.e. zone Pr-2, the smallest gap was recorded in the 3D-P group and in the occlusal region, for zone Pr-3, 4 and 5, the maximum gap was recorded in the MiL group. The provisional crowns fabricated by 3D printed technique have better marginal and internal fit than the provisional crowns made by milling technique.

Impact of Full Veneer Crown Restoration on the Periodontal Health of Posterior Teeth

Background: Periodontal health plays a vital role in the longevity of prosthodontic restorations. Full veneer crown (FVC) is one of the most common modalities in restoration of single tooth for preservation of function and health of oral tissues. Objectives : The study was done to critically evaluate the impact of FVC restoration providing standard procedures on the periodontal health of posterior teeth, also to observe the changes of periodontal health in terms of their status of gingival index, plaque index, periodontal pocket depth, bleeding on probing in different stages of treatment. Materials &amp; Methods: This prospective study evaluated the impact of FVC on periodontal health of 30 endodontically treated posterior teeth with FVC and 30 contralateral natural teeth of 19 patients in different stages of treatment for 2 years duration in the Dept. of Prosthodontics, Bangabandhu Sheikh Mujib Medical University (BSMMU) Hospital. To assess the periodontal health gingival index, plaque index, periodontal pocket depth, bleeding on probing were used. Collected data were analyzed using SPSS software (version 22) according to the study Objectives Results: The study Result showed that the mean differences of gingival index and plaque index between abutments and contraleteral natural teeth were non significant (p &gt; 0.05) at baseline, 4th month visit; but the differences were significant (p &lt; 0.05) at 8th month, 12th month visits. Regarding periodontal pocket depth the mean differences between abutments and contraleteral natural teeth found non significant (p &gt; 0.05) throughout the follow up period. Same thing observed in bleeding on probing except 12th month follow up visit, where highly significant (p &lt; 0.01) mean differences observed. Also the study found that the gingival index, plaque index, bleeding on probing, periodontal pocket depth increased a bit in 4th,8th,12th month visits gradually in abutments, which was almost same in contralateral natural teeth in case of gingival index, periodontal pocket depth; and increased a bit in case of plaque index, bleeding on probing. Conclusion : If the FVC is fabricated maintaining all standard procedures equigingival margin with proper marginal adaptation, oral hygiene as well as general health of the patient is maintained although less but there is a chance of detrimental effects on periodontal health in comparison to contralateral natural teeth. Implementation of CAD/CAM program in a nonface-to-face classroom environment due to the COVID-19 pandemic.

This study aimed to evaluate a nonface-to-face crown designing module in a preclinical dental course. Free dental planning software (Blue Sky Plan) was installed on the personal computers of dental college students, and a #46 full veneer crown designing practice was performed individually. An online survey was conducted on the computers' specification and main usage of the students, the practice process, and results. Statistical analysis was conducted to analyze the association between variables, such as "operating system," "central processing unit ," "number of cores," "random-access memory (RAM)," "graphic card," and task performance. Of the D2 students, 75.4% (52 of 69) responded to the survey. Overall, 96% of the respondents used their computers, and all respondents had no problem running the program. Most of the students marked their level of computer literacy as intermediate and had purchased the computers for the purpose of performing light work. The most common specifications of the computer were Intel i5, quad core, 8 GB RAM, and Windows 10. Students had little experience with computer-aided design/computer-aided manufacturing before the class. The relationship between computer specifications and task performance was not statistically significant. Overall, students with intermediate-level computer literacy used computers with less than the recommended specifications of the program; however, they were able to run the program and individually proceed with modules to submit results. Using an individually available crown designing program can provide an opportunity to diversify curricula and broaden students' perspectives even under circumstances like the COVID-19 pandemic that limits intimate face-to face classes.

Management of Fractured Endodontically Treated Tooth by Fibre Optic Composite Post-Core-Crown: A Case Report

The restoration of endodontically treated tooth is a subject that has been evaluated and discussed widely in the dental literature. The goal of dental treatment is to provide optimal oral health, aesthetics and functions. The successful treatment of a badly broken endodontically treated tooth depends not only on the success of the endodontic treatment but also on good prosthetic reconstruction. The prosthetic treatment on a seriously damaged, endodontically treated tooth often requires an endodontic post as an additional retention element for core build up prior to crown restoration. This case report presents a male patient of 22 years of age with prosthetic crown fracture of endodontically treated maxillary left central incisor tooth. The following clinical case represents the clinical steps involved in the utilization of a fibre-reinforced composite post-core in a maxillary left central incisor and the subsequent treatment with a full veneer crown.&#x0D; Central Medical College Journal Vol 5 No 2 Jul 2021 PP 126-130

Gingival health status of patients treated with full veneer crown and contra lateral natural tooth : A comparative study

Aim: The aim of this study was to compare the gingival health of patients treated with full veneer crown (FVC) restoration with the contralateral natural tooth of that patients in different stages of treatment. Materials and methods: This study compared the gingival health status of 30 endodontically treated posterior teeth with FVC and 30 contralateral natural teeth of 19 patients in terms of gingival index, and plaque index. These patients have been examined from July 2017 to June 2019 in the Dept. of Prosthodontics, Bangabandhu Sheikh Mujib Medical University (BSMMU) Hospital. The gingival health was assessed with gingival index and plaque index by Leo H in different stages of treatment, also compared with the contralateral natural teeth. Collected data were analyzed using SPSS software (version 22) according to the study objectives. Results: Tooth with FVC restoration found high gingival index and plaque index compared to a contralateral natural tooth in different stages of treatment. The mean differences of the gingival index were not significant at baseline, 4th month visits, but it was significant, and highly significant at 8th month, 12th month follow-up visits respectively. The mean differences in plaque index were found significant in all visits except 12th month visit. The gingival index and plaque index increased from baseline to 4th, 8th,12th month visits gradually both in abutments and contralateral natural teeth. Conclusion: If the FVC is fabricated with maintaining proper anatomical contour, equigingival margin with proper marginal adaptation, despite maintaining oral hygiene as well as the general health of patients there is a chance of development of adverse effects on gingival health in comparison to the contralateral natural tooth. Analysis of surface characteristics of (Y, Nb)-TZP after finishing and polishing.

This in vitro study aimed to evaluate the surface characteristics of a full veneer crown fabricated chairside (CS) from a (Y, Nb)-TZP zirconia block in response to conventional zirconia grinding and polishing. Zirconia crowns (n = 40) were first prepared and divided into two groups of materials: Labside (LS) and CS, after which each specimen went through a five-step grinding and polishing procedure. Following each surface treatment, surface characteristics were analyzed using confocal laser microscopy (CLSM), average surface roughness (Ra) values were processed from the profile data through Gaussian filtering, and X-ray diffraction pattern analysis was performed to evaluate the monoclinic (M) phase content. Then, a representative specimen was selected for field-emission scanning electron microscopy (FE-SEM), followed by a final analysis of the roughness and X-ray diffraction of the specimens using the independent t-test and repeated measures analysis of variance (RM-ANOVA). In every group, polishing significantly reduced the Ra values (P < .001). There was no significant difference in Ra between the polished state CS and LS. Furthermore, CLSM and FE-SEM investigations revealed that even though grain exposure was visible in CS specimens throughout the as-delivered and ground states, the exposure was reduced after polishing. Moreover, while no phase transformation was visible in the LS, phase transformation was visible in CS after every surface treatment, with the M phase content of the CS group showing a significant reduction after polishing (P < .001). Within the limits of this study, clinically acceptable level of surface finishing of (Y, Nb)-TZP can be achieved after conventional zirconia polishing sequence.

Assess the Radiological Change of Post Extraction Socket after Using Beta Tricalcium Phosphate for Understanding of Bone Formation

Background: Bone healing is an important subject in various fields of dentistry such as prosthesis, implant, surgery, etc. Numerous studies and researches have been done on tooth socket healing process however, most of these studies are histological evaluations and less are radiographic studies. Aim: The aim the Study was the Radiological Change of Post Extraction Socket after Using Beta Tricalcium Phosphate for Understanding of Bone Formation. Methods: The experimental Study was conducted in the department of oral and maxillofacial of in Bangabandhu Sheikh Mujib Medical University Hospital (BSMMU), Dhaka, Bangladesh to find out the common indications of Post Extraction Socket. 20 patients (Group-A) whose full veneer crown were prepared on Alloplastic bone substitute and another 20 patients (Group-B) whose were without alloplastic bone substitute. 40 cases were randomly selected for the study whose common indication of Radiological Change of Post Extraction Socket after Using Beta Tricalcium Phosphate. Clinical examination and evaluation were done from October 2010 to February 2011. Other necessary investigations were done if clinically indicated and to prepare the patient for anesthesia. Statistical analysis of the results was obtained by using window-based computer software devised with Statistical Packages for Social Sciences (SPSS-22). Results: Table shows the distribution of the age of the patients. Majority of the patients 22(55.0%) were of 18-25 years age group and another 15(37.5%) were at their 26-35 age group. Mean age of the patients was 25.97±4.99 years. Conclusion: Both QDSR and LRA methods can be used in evaluation of the rate of bone formation in the tooth socket but the former method is more accurate.

Effects of Modified Crown Contour on Periodontal Health

Introduction: The contour of artificial crowns has and continues to be highly controversial. Much of the controversy centers on whether the gingival sulcus is really in need of protection from buccal and/or lingual convexities, or whether a flatter emergence profile affords "self-cleansing" muscle action. Evidence is reviewed in this part which suggests that over-contoured crown is probably more detrimental to gingival health than under-contoured crown. Objective: The purpose of this study was to evaluate the effects of modified crown contour (over-contoured and under-contoured) on periodontal health. Methods: This study was conducted in the department of Prosthodontics, Bangabandhu Sheikh Mujib Medical University, Dhaka, Bangladesh for the duration of one and half year to find out the successful artificial crown contour that is harmonious with the periodontium. Selected mandibular molar teeth were prepared for full veneer crown. 50 patients (Group-A) whose full veneer crown was fabricated with under-contouring form and another 50 patients (Group-B) whose full veneer crown was fabricated with over-contouring form and cemented on respected teeth. Clinical examination and evaluation were done after three (3) weeks, six (6) weeks, three (3) months, and six (6) months of post prosthesis follow up period. Data were collected on the basis of following parameters like gingival index, periodontal index, periodontal pocket depth, and gingival recession in a predesign data collection sheet. Statistical analysis (Chi- Square Test and unpaired t-test) was done to find out the significance value (P&lt;0.05). Results: It is observed that the result was highly significant (p&lt;.001) between two groups regarding Gingival index and Periodontal index but not significantly difference regarding Periodontal pocket depth or Gingival recession. Correlation of soft tissue biotype with pink aesthetic score in single full veneer crown.

It is of interest to document the correlation of soft tissue biotype with pink esthetic score in single full veneer crown in Indian population. Hence, a Cross-sectional, descriptive study was conducted in an institution, on randomly selected individuals from a data collection of 86000 patient data. Scalloped and thin gingival biotype was present in 62.1 % patients and flat and thick was present in 37.9% individuals according to Anon and Ross Classification. Pink esthetic score didn't give any significant value in single crown cases where 85% cases had a good pink aesthetic score. Thus, the rightness of the PES index for the objective outcome assessment of the esthetic dimension of anterior single-tooth crown was confirmed. However, many randomized clinical trials are needed to further validate and refine this index for its clinical use in prosthetic rehabilitation.

Age-related radiomorphometric changes on panoramic radiographs.

ObjectivesWe aimed to analyse age‐related anatomical changes in teeth and mandibular structures using panoramic radiographs.Materials and MethodsWe included 471 subjects aged 13–70 years (mean, 35.12 ± 18.72 years). Panoramic radiographs were used to record intraoral condition and radiomorphometric parameters. After grouping the subjects by age decade, descriptive statistics and analysis of variance were performed to assess age‐related patterns.ResultsThe number of missing teeth, endodontically treated teeth, full veneer crowns, and implant prosthesis increased with age (all p < .05). The prevalence of periodontitis significantly increased after the 40s and was the highest in the 60s (57.1%). The maxillary canine root was the longest in the 10s and 20s (p < .001). With age, the mandibular canal and mental foramen moved towards the alveolar bone crest, on the opposite side of the mandibular inferior border. The pulp area and pulp‐to‐tooth ratio of maxillary/mandibular first molars were significantly higher in the 10s and 20s than in other age groups (all p < .05).ConclusionsWe provided comprehensive information on age‐related anatomical changes in teeth and mandibular structures based on panoramic radiographs. Various radiographic parameters showed specific changes with increasing age. Assessing these age‐related changes can be useful in determining an individual's age, and may aid in medico‐legal and forensic judgments.

Analysis of Factors Causing Extraction of Endodontically Treated Teeth-A Retroprospective Study

Aim: To study and analyze the various reasons for extraction of endodontically treated teeth. Material and Method: 150 Endodontically treated teeth were reviewed, and the following reasons were recorded: patient’s age, gender, type of tooth; presence and type of coronal restoration; motive of consultation; reasons for extraction. Results: Pain was the primary reason for consultation .Mandibular first molar was the most common tooth involved followed by maxillary first molars. coronal failure where the main reason for extraction of endodontically treated teeth, this was due to neglegance in replacing permanent restoration where only 16% of the cases had either full veneer crown or post and core, followed by periodontal disease, vertical root fracture and non restorable caries. Conclusion: A good coronal restoration is required for a successful long term outcomes.

Fracture resistance of endodontically treated teeth restored with various esthetic posts.

Dental esthetic materials are constantly introduced to meet the increasing esthetic demand in contemporary dental practice. To test the fracture resistance of endodontically treated teeth (ETT) restored with different esthetic post materials like fiber-reinforced composite post (FRC), polyether ether ketone (PEEK), and polymer infiltrated ceramic (PIC). Thirty-six human root canal treated single-rooted premolar teeth were decoronated 3 mm above the cemento-enamel junction and prepared to receive the post. They were randomly divided into three groups (n= 12) to be restored with FRC, PEEK, and PIC. After appropriate surface treatment, they were cemented with self-adhesive luting cement and restored with full veneer crowns. The samples were thermocycled, subjected to a compressive static load at 45∘ angulation until fracture. Obtained data were analyzed by one-way ANOVA and Tukey's HSD post hoc comparison test. The results indicate that the ETT restored with PEEK post had the maximum fracture load (1929.94 N), followed by PIC endodontic post group (1810.65N), and FRC post (1715.68N). Meanwhile, ETT restored with FRC showed a predominantly favorable fracture, whereas PEEK restored teeth had a more unfavorable fracture. Of all the esthetic post materials, the group for which PEEK endodontic post was used displayed higher fracture resistance.

Endocrown bridge: An alternate way to restore endodontically treated tooth

Endocrown restorations are single unit prostheses fabricated from metal. These restorations are indicated for endodontically treated teeth, molars especially, that have significant loss of coronal structure. Endocrowns gain their retention from the coronal portion integrated into the apical projection that fills the pulp chamber space, and possibly the root canal entrances. In this case report a new style Fixed Partial Denture is introduced. Here mandibular second molar had compromised tooth structure and hence Endocrown was planned with the tooth. For replacement of missing first molar a three unit fixed partial prosthesis was planned. This unique three unit prosthesis had Endocrown on mandibular second molar and a full veneer crown on mandibular second premolar as an abutment fabricated in metal. This innovative design of Fixed Partial Denture is better suited in cases where there is compromised tooth structure and the tooth is strategically important and has to be retained.

Comparative evaluation of gingival displacement using retraction cord impregnated with Astringedent®, Magic foam and Expasyl: an in vivostudy

Background: Gingival retraction helps in achieving good quality impressions. These are needed for a precision fit and long-term success with fixed prosthodontic restorations. Aim: This study aimed to evaluate the clinical efficiency of gingival displacement obtained using Expasyl, Magic foam cord and Medicated retraction cord. Materials and methods: One hundred twenty patients with the requirement of full veneer crowns were selected. They were divided into three groups, forty subjects in each group (twenty subjects by each operator) namely Expasyl, Magic foam cord and Medicated retraction cord. The impressions obtained before and after placing the retraction system were poured with type IV die stone. The casts obtained before and after placing the retraction system were coded in a blind fashion to avoid the influence of the operator. The casts were viewed under tool maker microscope “10X” magnification for the amount of both depth and width of gingival displacement. Results: Mesial, distal, mid-buccal, mid-lingual were taken as reference points and for Medicated retraction cord, Expasyl and Magic foam cord the mean values are 0.50mm, 0.49mm and 0.29mm respectively in horizontal displacement and 0.56mm, 0.47mm and 0.31mm respectively in vertical displacement. One way ANOVA was used to calculate the p-value and multiple range test by the Tukey-HSD analysis to identify significant groups at 5% level. The level of significance for all tests was set as p < 0.05. Conclusion: Within the limitations of this study, Magic foam cord showed the ease of placement followed by Expasyl retraction system and Medicated retraction cord. Clinical significance: Gingival retraction helps in achieving good quality impressions. These are needed for a precision fit and long-term success with fixed prosthodontic restorations. Selecting techniques and materials that produce transient retraction and dry field without irreversible damages to the tissues is of utmost importance.
