IntroductionSarcoidosis is a chronic granulomatous disease that is characterised by its histopathology showing the presence of non-necrotising granulomas in several organs. Cutaneous lesions of sarcoidosis are polymorphic, delaying its correct diagnosis. Material and methodsA review was performed on patients from the Department of Dermatology, Hospital Universitario Doctor Peset, Valencia, with a diagnosis of sarcoidosis from January 1, 2008, to September 1, 2016, confirmed by histopathology. Patients with other granulomatous diseases were excluded. ResultsA total of 15 patients: 8 patients had systemic disease, and 7 patients limited cutaneous disease. The female: male ratio was 2:1, with a mean age of 49 years.The predominant clinical form was subcutaneous (30% of cases), and was more associated with a systemic disease, than any other variant. This was followed by maculopapular sarcoidosis (25% of cases), and was more associated with limited cutaneous diseases.Angiotensin-converting enzyme and 24-hour urine calcium were normal in every patient with limited cutaneous sarcoidosis. However, in patients with systemic sarcoidosis, angiotensin-converting enzyme levels were high in 75% of patients, and calcium levels were high in 37% cases. ConclusionA review is presented of patients with a diagnosis of sarcoidosis in this department over the last eight years, as well as an analysis of their epidemiological, clinical, therapeutic, and prognostic features.Subcutaneous sarcoidosis was the most frequent clinical form, and was more associated with a systemic disease than any other variant. Subsequently, special attention needs to be paid to this variant of clinical onset. This should also be the case for others such as, scar-like, plaque-like or when several forms co-exist, which may also be associated to systemic sarcoidosis.There are some analytical parameters that may help clinicians to predict a possible systemic affection, such as angiotensin-converting enzyme and 24-hour urine calcium. In this series, angiotensin-converting enzyme levels showed high sensitivity and specificity.
