Association of Alopecia Areata with Sensorineural Hearing Loss: A Systematic Review and Meta-analysis

Background: Immune-mediated melanocyte-related pathogenesis in alopecia areata (AA) may cause sensorineural hearing loss (SNHL). However, the relation between AA and SNHL has been unclear. Therefore, we aimed to investigate this association between AA and SNHL. Methods: We performed a systematic review and searched MEDLINE and Embase on July 25, 2022, for cross-sectional, case-control, or cohort studies that examined the association of AA with SNHL. The Newcastle-Ottawa Scale was used to evaluate their risk of bias. A random-effects model meta-analysis was performed to obtain the mean differences in frequency-specific hearing thresholds between AA patients and age-matched healthy controls and the pooled odds ratio for SNHL in relation to AA. Results: We included 5 case-control studies and 1 cohort study, with none of them rated with high risk of biases. The meta-analysis showed AA patients had significantly higher mean differences in pure-tone hearing thresholds at 4,000 Hz and 12,000–12,500 Hz. The meta-analysis also found increased odds for SNHL among patients with AA (OR: 3.18; 95% CI: 2.06–4.89; I² = 0%). Conclusions: AA is associated with an increase of SNHL, especially at high frequencies. Otologic consultation may be indicated if AA patients present with hearing loss or tinnitus.

Refinement of systemic guinea pig deafening in hearing research: Sensorineural hearing loss induced by co-administration of kanamycin and furosemide via the leg veins.

Auditory disabilities have a large impact on the human population worldwide. Research into understanding and treating hearing disabilities has increased significantly in recent years. One of the most relevant animal species in this context is the guinea pig, which has to be deafened to study several of the hearing pathologies and develop novel therapies. Applying kanamycin subcutaneously and furosemide intravenously is a long-established method in hearing research, leading to permanent hearing loss without surgical intervention at the ear. The intravenous application of furosemide requires invasive surgery in the cervical area of the animals to expose the jugular vein, since a relatively large volume (1 ml per 500 g body weight) must be injected over a period of about 2.5 min. We have established a gentler alternative by applying the furosemide by puncture of the leg veins. For this, custom-made cannula-needle devices were built to allow the vein puncture and subsequent slow injection of the furosemide. This approach was tested in 11 guinea pigs through the foreleg via the cephalic antebrachial vein and through the hind leg via the saphenous vein. Frequency-specific hearing thresholds were measured before and after the procedure to verify normal hearing and successful deafening, respectively. The novel approach of systemic deafening was successfully implemented in 10 out of 11 animals. The Vena saphena was best suited to the application. Since the animals' condition, post leg vein application, was better in comparison to animals deafened by exposure of the Vena jugularis, the postulated refinement that reduced animal stress was deemed successful.

Comparison of Auditory Steady-State Responses With Conventional Audiometry in Older Adults.

Behavioral measures, such as pure-tone audiometry (PTA), are commonly used to determine hearing thresholds, however, PTA does not always provide reliable hearing information in difficult to test individuals. Therefore, objective measures of hearing sensitivity that require little-to-no active participation from an individual are needed to facilitate the detection and treatment of hearing loss in difficult to test people. Investigation of the reliability of the auditory steady-state response (ASSR) for measuring hearing thresholds in older adults is limited. This study aimed to investigate if ASSR can be a reliable, objective measure of frequency specific hearing thresholds in older adults. Hearing thresholds were tested at 500 Hz, 1000 Hz, 2000 Hz, and 4000 Hz in 50 participants aged between 60 and 85 years old, using automated PTA and ASSR. Hearing thresholds obtained from PTA and ASSR were found to be significantly correlated (p < .001) in a cohort consisting of participants with normal hearing or mild hearing loss. ASSR thresholds were significantly higher as compared to PTA thresholds, but for the majority of cases the difference remained within the clinically acceptable range (15 dB). This study provides some evidence to suggest that ASSR can be a valuable tool for estimating objective frequency-specific hearing thresholds in older adults and indicate that ASSR could be useful in creating hearing treatment plans for older adults who are unable to complete behavioral PTA. Further research on older adults is required to improve the methodological features of ASSR to increase consistency and reliability, as well as minimize some of the limitations associated with this technique.

Association of vitiligo with high-frequency sensorineural hearing loss: a systematic review and meta-analysis.

The association between vitiligo and hearing loss has been noted but the specific frequencies and degrees of hearing impairment remain unclear. The objective of this systematic review was to investigate the relationship between vitiligo and hearing thresholds at various specific frequencies. We followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. We searched MEDLINE and Embase for relevant studies from inception to 10th April 2021. Case-control studies, cross-sectional, or cohort studies that compared the frequency-specific hearing thresholds between vitiligo patients and age-matched non-vitiligo controls were included. There were neither language nor geographic limitations. We used the Newcastle-Ottawa Scale to assess the risk of bias of included studies. The DerSimonian and Laird random-effects model was utilized in meta-analyses due to expected clinical heterogeneity. We included 9 case-control studies with 371 vitiligo patients and 349 controls, which were rated with low or unclear risk. We found neither relevant cross-sectional nor cohort studies. The meta-analysis showed that when compared with controls, vitiligo patients had significantly higher pure-tone hearing thresholds at 2000, 4000, and 8000 Hz. In conclusions, vitiligo patients are prone to high-frequency sensorineural hearing loss.

Assessing Agreement between Frequency-Specific Chirp Auditory Steady-State Response and Pure Tone Audiometry in Adults by Intraclass Correlation Coefficient

Introduction: Chirp auditory steady-state response (ASSR) can be used to assess frequency-specific hearing thresholds. However, its reliability has not been confirmed yet. The purpose of this proposed study is to analyze the agreement of thresholds measured by chirp-ASSR and pure tone audiometry (PTA) to investigate the value of chirp-ASSR in hearing threshold evaluation. Methods: Participants with normal hearing (age: 18–66, 108 ears) and patients with sensorineural hearing loss (age: 22–82, 75 ears) were tested using PTA and chirp-ASSR at 0.5, 1, 2, and 4 kHz, respectively. Intraclass correlation coefficient (ICC) and Bland-Altman plot were introduced to analyze the agreement between the 2 methods. Results: One-hundred eight participants underwent both chirp-ASSR and PTA to estimate their thresholds. The ICCs yielded by these 2 methods are 0.757, 0.893, 0.883, and 0.921 (p < 0.001) at 0.5, 1, 2, and 4 kHz carrier frequency, respectively. However, there is a significant difference between the 2 methods at 2 kHz: the mean value of the ASSR thresholds is 5.27 dB HL higher than the value of PTA thresholds. Additionally, the 95% limits of agreement range from −27.48 to 26.66 dB HL at 0.5 kHz, from −18.19 to 17.87 dB HL at 1 kHz, from −12.01 to 22.55 dB HL at 2 kHz, and from −21.29 to 19.17 dB HL at 4 kHz, which are large enough to affect clinical decision-making. Conclusion: In this study, we have confirmed good to excellent correlation between chirp-ASSR and PTA in threshold estimation for adults with and without hearing loss. The degree of correlations is higher for participants with hearing loss and for measurements at high frequencies. However, significant systematic difference and large limits of agreement between the 2 methods have been found. These findings show that chirp-ASSR can be treated as a supplementary method to PTA when evaluating the hearing level, but the 2 methods are not interchangeable due to their systematic difference and large limits of agreement.

Hearing loss in patients with systemic lupus erythematosus: A systematic review and meta-analysis.

To determine the prevalence of hearing loss (HL) in patients with systemic lupus erythematosus (SLE), describe frequency-specific hearing threshold changes in this patient population as compared to age-matched control, and compare the clinical and serological profiles of patients with SLE with and without HL. A systematic review querying four databases (PubMed, Web of Science, Scopus, and Cochrane) was performed. Meta-analysis of available data was performed to determine the overall prevalence and odds ratio (OR) of HL, and compare mean differences in frequency-specific hearing thresholds between patients with SLE and control. Additionally, meta-analysis of proportions allowed for comparison of disease features present in patients with SLE with and without sensorineural HL. This review included 17 studies reporting on 1326 patients (635 with SLE and 691 control). The pooled prevalence of HL in patients with SLE was 27%. In comparison to control, patients with SLE had a significantly higher odds of HL (OR 14.6, 95% CI: 8.5 to 25.0). Mean air-conduction hearing thresholds in patients with SLE were significantly elevated relative to control at 125 and 250 Hz. Mean bone-conduction hearing thresholds were significantly elevated in patients with SLE across all measured frequencies except at 3000 and 6000 Hz compared to control. Disease features did not significantly differ between patients with SLE with and without HL. Compared to age-matched control, patients with SLE have increased odds of HL, which primarily manifests at low frequencies. Therefore, this patient population requires greater audiologic attention.

Objective frequency-specific hearing thresholds definition for medicolegal purposes in case of occupational NIHL: ASSR outperforms CERA

Audiological use of the 40 Hz-ASSR (auditory steady state responses) could be valuable for objectively estimating the frequency-specific threshold in adults undergoing an expertise examination for medicolegal and/or compensation purposes. The present prospective study was set up to clarify the relationship between the thresholds obtained by cortical evoked response audiometry (CERA) and by 40 Hz-ASSR, in the same ears, within a large homogeneous sample of 164 subjects (328 ears) with NIHL and well documented exposure to noise. All these subjects claimed financial compensation for occupational NIHL, and there was a suspicion of exaggeration of the reported NIHLs. ASSR thresholds show a good correlation with the CERA thresholds. However, a systematic shift is noticed, ASSR thresholds being on average (1–2 – 3 kHz) 4.38 dB lower (i.e. showing less hearing loss) than CERA thresholds. Moreover, the binaural multiple ASSR technique allows a considerable time gain when compared to the CERA.

Hearing Loss in Patients With Ankylosing Spondylitis: A Systematic Review and Metaanalysis

To determine the prevalence of hearing loss (HL) in patients with ankylosing spondylitis (AS) and to describe frequency-specific hearing threshold changes in this patient population compared to patients without AS. A systematic review querying 4 databases (PubMed, OVID Medline, Scopus, Cochrane) was performed to identify studies evaluating HL in patients with AS. Metaanalysis was performed to identify overall prevalence rate and OR of HL, as well as to compare mean differences in frequency-specific hearing thresholds between patients with and without AS. Our metaanalysis included 14 studies and 1083 patients (598 with AS vs 485 without AS). The pooled prevalence of HL in patients with AS was 42.4% (95% CI 29.2-56.2). Patients with AS had a significantly higher OR of HL than patients without AS (OR 4.65, 95% CI 2.73-7.91). Mean differences in pure-tone hearing thresholds ranged from 0-5 decibels (dB) for frequencies of 0.25-4 kHz, and from 5-15 dB for frequencies of 6-16 kHz. Patients with AS have higher odds of having HL than patients without AS. The AS population also presents with significantly impaired hearing thresholds across all conventional and extended pure-tone frequencies. This may manifest as slight to moderate HL. Results of this systematic review might justify increased attention to audiologic manifestations of patients with AS.

Consumption of betel quid contributes to sensorineural hearing impairment through arecoline-induced oxidative stress

Betel quid is one of the most widely used psychoactive substances, and is consumed by approximately 10% of the world’s population. In addition to its carcinogenicity, betel quid has also been reported to affect many organs, including the brain, heart, lungs, gastrointestinal tract, and reproductive organs. As betel quid contains several neurotoxic ingredients, we hypothesize that it also possesses ototoxicity and may lead to sensorineural hearing impairment (SNHI). In this study, we investigated the contribution of betel quid consumption to SNHI in a large clinical cohort, and validated the pathogenetic mechanisms in ex vivo tissue explants. We enrolled a total of 2364 volunteers, and determined their audiologic results based on Z-scores converted from their original frequency-specific hearing thresholds. Using generalized linear regression, we identified a positive correlation between betel quid consumption and the Z-scores across different frequencies. Subsequently, we explored the toxicity of arecoline, the main neuroactive component of betel quid, on tissue explants from murine cochleae. Arecoline reduced cell activity in the explant cultures and induced apoptosis in the hair cells, probably through the effects of oxidative stress. These findings have expanded the potential hazards of betel quid to common neurological disorders, and provide insights into preventive strategies against SNHI caused by neurotoxic substances.

Sex-specific enlarged vestibular aqueduct morphology and audiometry.

Enlargement of the vestibular aqueduct (EVA) is one of the most common congenital malformations in pediatric patients presenting with sensorineural or mixed hearing loss. The relationship between vestibular aqueduct (VA) morphology and hearing loss across sex is not well characterized. This study assesses VA morphology and frequency-specific hearing thresholds with sex as the primary predictor of interest. A retrospective, longitudinal, and repeated-measures study was used. 47 patients at an academic tertiary care center with hearing loss and a record of CT scan of the internal auditory canal were candidates, and included upon meeting EVA criteria after confirmatory measurements of vestibular aqueduct midpoint and operculum widths. Audiometric measures included pure-tone average and frequency-specific thresholds. Of the 47 patients (23 female and 24 male), 79 total ears were affected by EVA; the median age at diagnosis was 6.60 years. After comparing morphological measurements between sexes, ears from female patients were observed to have a greater average operculum width (3.25 vs. 2.70 mm for males, p = 0.006) and a greater average VA midpoint width (2.80 vs. 1.90 mm for males, p = 0.004). After adjusting for morphology, male patients' ears had pure-tone average thresholds 17.6 dB greater than female patients' ears (95% CI, 3.8 to 31.3 dB). Though females seem to have greater enlargement of the vestibular aqueduct, this difference does not extend to hearing loss. Therefore, our results indicate that criteria for EVA diagnoses may benefit from re-evaluation. Further exploration into morphological and audiometric discrepancies across sex may help inform both clinician and patient expectations.

Comparison of thresholds estimation in adults with conductive hearing loss and normal hearing adults using auditory steady state response evoked by narrow band CE-chirps

Background: Narrowband CE Chirps (NB CE-chirps) were used to provide the characteristics of compensation for the cochlear traveling wave delay and frequency specificity.Aim: The aim was to compare the objective frequency-specific hearing thresholds estimation by the use of NB CE-chirps ASSR and behavioural thresholds in normal hearing subjects and patients with the conductive hearing loss.Subjects and methods: The cases group comprised 35 ears of 25 patients with the conductive hearing loss with varying degree from mild to severe in severity and their age ranging from 15 to 60 years. They were compared with 20 ears of 10 adult control subjects with normal hearing thresholds, whose age ranged from 19 to 42 years. All cases and controls were subjected to pure-tone audiometry (PTA), tympanometry and auditory steady-state response (ASSR), using 4 NB CE-chirp for air conduction (AC) and bone conduction (BC) threshold estimation.Results: AC-ASSR estimated thresholds were statistically significantly higher than AC PTA thresholds at 500, 1000, 2000 and 4000 Hz in both the cases and the control groups. BC ASSR estimated thresholds were statistically significantly lower than BC PTA thresholds only at 500 Hz in the cases and statistically significantly higher than BC PTA thresholds only at 2000 Hz in the controls. There was a statistically significant positive correlation between AC-ASSR and AC PTA, and between ABG-ASSR and ABG PTA in the cases group.Conclusions: Auditory steady-state responses with NB CE-chirps is suitable tool for objective frequency-specific hearing threshold detection, that can be used for estimation of behavioural bone conduction thresholds in adults with conductive hearing loss, with more accuracy in the mid frequencies, and can estimate behavioural air conduction thresholds and air-bone gap more accurately in the higher frequencies 2000, 4000 Hz more than at 500 and 1000 Hz.

Comparison of frequency-Specific hearing thresholds between pure-tone audiometry and auditory steady-state response

Introduction: Hearing assessment in difficult to assess persons gives inconsistent response in free field hearing and pure-tone audiometry (PTA). We have been performing brainstem evoked response audiometry (BERA) as an objective assessment in such cases. BERA has limitations that it is not frequency specific, measures thresholds in higher frequencies only, signal intensity levels cannot be higher than 90 dB and presents the data in waveform which needs to be interpreted by experienced audiologist to deduce correct information. All these limitations can be negated with use of auditory steady-state response (ASSR) which can present intensity levels till 120 dB, is frequency specific, even for lower frequencies and has automated response and deduction analysis. The aim of our study was to compare pure-tone thresholds with ASSR thresholds in normal hearing and hearing loss subjects with an objective to validate the ASSR as a tool for objective hearing assessment. Materials and Methods: In this prospective cross-sectional study of 70 adults (n = 140 ears) of both genders, with and without hearing complaints, Group 1 consisted of 24 individuals of normal hearing, Group 2 consisted of 25 individuals of sensorineural hearing loss, and 21 individuals with conductive hearing loss in Group 3. The patients were evaluated with PTA and ASSR test. The mean pure tone and ASSR thresholds for each frequency were calculated and analyzed using the SPSS IBM software. Results: The mean difference of ASSR with PTA was noted at 9.92 dB in the right ear and 10 dB in the left ear. A high significant correlation was noted between ASSR and PTA with P(0.0005) across all the three groups. The relationship between the PTA and ASSR measurement for measuring average hearing threshold levels is described by the equation: PTA = ASSR mean × 0.99–9.92. Conclusion: ASSR is an excellent tool to predict accurate level of frequency-specific hearing threshold. It has constant relationship with pure tone thresholds. ASSR can predict true hearing thresholds in “difficult to assess” patients such as children and malingerers. This can also be used in the prediction of disability assessment objectively.

Hearing Trajectory in Children with Congenital Cytomegalovirus Infection.

Objectives To compare hearing trajectories among children with symptomatic and asymptomatic congenital cytomegalovirus infection through age 18 years and to identify brain abnormalities associated with sensorineural hearing loss (SNHL) in asymptomatic case patients. Study Design Longitudinal prospective cohort study. Setting Tertiary medical center. Subjects and Methods The study included 96 case patients (4 symptomatic and 92 asymptomatic) identified through hospital-based newborn cytomegalovirus screening from 1982 to 1992 and 72 symptomatic case patients identified through referrals from 1993 to 2005. We used growth curve modeling to analyze hearing thresholds (0.5-8 kHz) by ear with increasing age and Cox regression to determine abnormal findings on head computed tomography scan associated with SNHL (hearing threshold ≥25 dB in any audiometric frequency) among asymptomatic case patients. Results Fifty-six (74%) symptomatic and 20 (22%) asymptomatic case patients had SNHL: congenital/early-onset SNHL was diagnosed in 78 (51%) and 10 (5%) ears, respectively, and delayed-onset SNHL in 25 (17%) and 20 (11%) ears; 49 (32%) and 154 (84%) ears had normal hearing. In affected ears, all frequency-specific hearing thresholds worsened with age. Congenital/early-onset SNHL was significantly worse (severe-profound range, >70 dB) than delayed-onset SNHL (mild-moderate range, 26-55 db). Frequency-specific hearing thresholds were significantly different between symptomatic and asymptomatic case patients at 0.5 to 1 kHz but not at higher frequencies (2-8 kHz). Among asymptomatic case patients, white matter lucency was significantly associated with SNHL by age 5 years (hazard ratio, 4.4; 95% CI, 1.3-15.6). Conclusion Congenital/early-onset SNHL frequently resulted in severe to profound loss in symptomatic and asymptomatic case patients. White matter lucency in asymptomatic case patients was significantly associated with SNHL by age 5 years.

Binaural Interaction Effects of 30-50 Hz Auditory Steady State Responses.

Auditory stimuli modulated by modulation frequencies within the 30 to 50 Hz region evoke auditory steady state responses (ASSRs) with high signal to noise ratios in adults, and can be used to determine the frequency-specific hearing thresholds of adults who are unable to give behavioral feedback reliably. To measure ASSRs as efficiently as possible a multiple stimulus paradigm can be used, stimulating both ears simultaneously. The response strength of 30 to 50Hz ASSRs is, however, affected when both ears are stimulated simultaneously. The aim of the present study is to gain insight in the measurement efficiency of 30 to 50 Hz ASSRs evoked with a 2-ear stimulation paradigm, by systematically investigating the binaural interaction effects of 30 to 50 Hz ASSRs in normal-hearing adults. ASSRs were obtained with a 64-channel EEG system in 23 normal-hearing adults. All participants participated in one diotic, multiple dichotic, and multiple monaural conditions. Stimuli consisted of a modulated one-octave noise band, centered at 1 kHz, and presented at 70 dB SPL. The diotic condition contained 40 Hz modulated stimuli presented to both ears. In the dichotic conditions, the modulation frequency of the left ear stimulus was kept constant at 40 Hz, while the stimulus at the right ear was either the unmodulated or modulated carrier. In case of the modulated carrier, the modulation frequency varied between 30 and 50 Hz in steps of 2 Hz across conditions. The monaural conditions consisted of all stimuli included in the diotic and dichotic conditions. Modulation frequencies ≥36 Hz resulted in prominent ASSRs in all participants for the monaural conditions. A significant enhancement effect was observed (average: ~3 dB) in the diotic condition, whereas a significant reduction effect was observed in the dichotic conditions. There was no distinct effect of the temporal characteristics of the stimuli on the amount of reduction. The attenuation was in 33% of the cases >3 dB for ASSRs evoked with modulation frequencies ≥40 Hz and 50% for ASSRs evoked with modulation frequencies ≤36 Hz. Binaural interaction effects as observed in the diotic condition are similar to the binaural interaction effects of middle latency responses as reported in the literature, suggesting that these responses share a same underlying mechanism. Our data also indicated that 30 to 50 Hz ASSRs are attenuated when presented dichotically and that this attenuation is independent of the stimulus characteristics as used in the present study. These findings are important as they give insight in how binaural interaction affects the measurement efficiency. The 2-ear stimulation paradigm of the present study was, for the most optimal modulation frequencies (i.e., ≥40 Hz), more efficient than a 1-ear sequential stimulation paradigm in 66% of the cases.

Establishing the presence of cochlear implant related artifact during sound-field recording of the auditory steady state response: A comparison between normal hearing adults, cochlear implant recipients, and a cochlear implanted human cadaver

The Auditory Steady State Response (ASSR) is an objective technique permitting dichotic, frequency-specific hearing threshold estimation. Electrical ASSR (eASSR) has the potential for auditory assessment in cochlear implant (CI) users through direct CI electrode stimulation. However, sound-field ASSR (sASSR) provides a more natural listening environment for CI users. Electrophysiological recordings could be affected by CI related artifact. The present study investigated the possibility of CI-artifact during sASSR recording in 10 CI subjects at 0.5, 1, 2 and 4 kHz. Results were compared with 15 NH controls. We investigated the mean threshold differences between the sASSR thresholds and the behavioral thresholds for each group at the four frequencies. Correlation between the sASSR thresholds and behavioral thresholds were computed. Applications of the NTID speech recognition test (NSRT®)

Objective: This study evaluated the diagnostic capabilities of an adaptive speech recognition protocol (NSRT®) that can be self-administered in non-clinical venues by listeners using internet-based software. Design: All participants were given an audiological evaluation, including pure-tone testing, and responded to the NSRT administered in quiet and + 5 dB SNR listening conditions. The NSRT test materials are sentence-length utterances containing phonetic contrasts, primarily minimal pairs. Study sample: Subjects were 123 adults with normal hearing to moderately severe sensorineural hearing loss (mean age = 55 years, SD = 23). Results: Performance on the NSRT is strongly related to pure-tone thresholds. Linear regression analyses support the utility of the NSRT as a proxy for clinically-obtained hearing thresholds across the octave frequencies 0.5 to 8 kHz, primarily for individuals in the − 10 to 55 dB HL range. Other NSRT results are linked to analyses of phonetic errors and components of aural rehabilitation. Conclusions: Among its numerous results, the NSRT yields quantitative predictions of frequency-specific hearing thresholds, provides insight into the phonetic errors that affect speech understanding in adults who suffer from sensorineural hearing loss, primarily in the − 10 to 55 dB HL range, and has implications for the design of individualized auditory training programs.

Contribution of adiponectin and its type 1 receptor to age-related hearing impairment

Age-related hearing impairment (ARHI) is a complex neurodegenerative disorder caused by a combination of environmental and genetic factors. We have reported previously that obesity increases the risk for ARHI, and that plasma levels of adiponectin are associated with ARHI. In the present study, we further explored the role of adiponectin in the pathophysiology of ARHI by investigating the genotypes of ADIPOQ and ADIPOR1, the genes of adiponectin and its type 1 receptor, respectively. A total of 1682 volunteers were enrolled, and their audiological phenotypes were determined according to the z scores converted from their original frequency-specific hearing thresholds. A total of 9 tag-single nucleotide polymorphisms (tagSNPs) in ADIPOQ and 4 tagSNPs in ADIPOR1 were genotyped, and the genotypes were correlated to the audiological phenotypes under the assumption of various inheritance models. Significant associations were identified between certain ADIPOQ tagSNPs and z scores under dominant, codominant, or additive models, whereas no association was identified between ADIPOR1 tagSNPs and z scores. The associations between ADIPOQ tagSNPs and z scores appear to exist only in subjects with specific ADIPOR1 genotypes, indicating an interaction between adiponectin and AdipoR1. Measurement of plasma adiponectin in 736 subjects revealed that ADIPOQ genotypes might exert their effects on hearing levels via modulation of plasma adiponectin levels. Subsequently, we confirmed the expression of AdipoR1 in the inner ear of mice, and demonstrated antiapoptotic effects of adiponectin in cochlear explant cultures. These results provide insights into the physiological function and potential clinical implications of adiponectin against ARHI.

Prediction of frequency-specific hearing threshold using chirp auditory brainstem response in infants with hearing losses

ObjectivesTo investigate the clinical usefulness of the LS-chirp auditory brainstem response for estimation of behavioral thresholds in young children with mild to severe hearing losses. Methods68 infants (136 ears) aged 6–12 months (mean age=9.2 months) with bilateral mild to severe hearing losses were studied at Children's Hospital of Fudan University. In all cases, the children were referred for LS-chirp ABR and visual reinforcement audiometric (VRA) measurements. The low-frequency band chirp (LF-chirp) thresholds (frequency band=0.1–0.85kHz) were compared to the average VRA thresholds (frequency band=0.25–0.5kHz), whereas the high-frequency band chirp (HF-chirp) thresholds (frequency band=1–10kHz) were compared to the average VRA thresholds (frequency band=1–4kHz) using statistical correlation coefficient values. ResultsThe LS-chirp ABR thresholds are very close to behavioral hearing levels. The mean differences between chirp-ABR and VRA thresholds were within 5dBHL for all measurements. The smallest mean threshold difference (<3dBHL) was obtained for the severe hearing loss group. The correlation coefficient values (r) were 0.97 at low-frequency and high-frequency bands. For each carrier frequency, the best correlations between chirp-ABR thresholds and VRA thresholds were obtained at VRA frequency of 0.25kHz/LF-chirp (r=0.98) and VRA frequency of 1kHz/HF-chirp (r=0.98). ConclusionsThis study demonstrates the effectiveness using chirp-ABR predicted frequency-specific thresholds, especially of low and middle frequencies. LS-chirp ABR thresholds determined behavioral thresholds in patients with severe hearing losses were better than for mild hearing losses. The use of a chirp-ABR testing ensures higher sensitivity and accuracy than that of auditory stead-state evoked response (ASSR) for measuring frequency-specific thresholds in young children.

Comparative study between pure tone audiometry and auditory steady-state responses in normal hearing subjects

IntroductionAuditory steady-state responses (ASSR) are an important tool to detect objectively frequency-specific hearing thresholds. Pure-tone audiometry is the gold-standard for hearing evaluation, although sometimes it may be inconclusive, especially in children and uncooperative adults. AimCompare pure tone thresholds (PT) with ASSR thresholds in normal hearing subjects. Materials and methodsIn this prospective cross-sectional study we included 26 adults (n = 52 ears) of both genders, without any hearing complaints or otologic diseases and normal pure-tone thresholds. All subjects had clinical history, otomicroscopy, audiometry and immitance measurements. This evaluation was followed by the ASSR test. The mean pure-tone and ASSR thresholds for each frequency were calculated. ResultsThe mean difference between PT and ASSR thresholdswas 7,12 for 500Hz, 7,6 for 1000Hz, 8,27 for 2000Hz and 9,71dB for 4000Hz. There were no difference between PT and ASSR means at either frequency. ConclusionASSR thresholds were comparable to pure-tone thresholds in normal hearing adults. Hearing threshold estimation by auditory steady-state responses with narrow-band chirps and adaptive stimulus patterns: implementation in clinical routine

Objective hearing threshold estimation by auditory steady-state responses (ASSR) can be accelerated by the use of narrow-band chirps and adaptive stimulus patterns. This modification has been examined in only a few clinical studies. In this study, clinical data is validated and extended, and the applicability of the method in audiological diagnostics routine is examined. In 60 patients (normal hearing and hearing impaired), ASSR and pure tone audiometry (PTA) thresholds were compared. ASSR were evoked by binaural multi-frequent narrow-band chirps with adaptive stimulus patterns. The precision and required testing time for hearing threshold estimation were determined. The average differences between ASSR and PTA thresholds were 18, 12, 17 and 19 dB for normal hearing (PTA ≤ 20 dB) and 5, 9, 9 and 11 dB for hearing impaired (PTA > 20 dB) at the frequencies of 500, 1,000, 2,000 and 4,000 Hz, respectively, and the differences were significant in all frequencies with the exception of 1 kHz. Correlation coefficients between ASSR and PTA thresholds were 0.36, 0.47, 0.54 and 0.51 for normal hearing and 0.73, 0.74, 0.72 and 0.71 for hearing impaired at 500, 1,000, 2,000 and 4,000 Hz, respectively. Mean ASSR testing time was 33 ± 8 min. In conclusion, auditory steady-state responses with narrow-band-chirps and adaptive stimulus patterns is an efficient method for objective frequency-specific hearing threshold estimation. Precision of threshold estimation is most limited for slighter hearing loss at 500 Hz. The required testing time is acceptable for the application in everyday clinical routine.
