Surpoids et obésité de l’adulte : 14 messages clés pour améliorer les pratiques: Overweight and obesity in adults: 14 key messages to improve practices

This guide describes the care, support and overall follow-up of adults with overweight or obesity. It reasserts the importance of early identification, coupled with a lifestyle assessment. It details the role of professionals (health, social and medico-social field, adapted physical activity, occupational health) and looks at the situations in which the person’s primary care physician may consult them.This work was conducted jointly with the French national health insurance fund, in the context of the healthcare system transformation strategy, to meet the objectives of the “Ma santé 2022” [“My Health 2022”] program. It was produced with the assistance of a multidisciplinary working group and patient association representatives drawing on an analysis on the international literature.To help professionals assimilate this work, the HAS proposes a summary of the critical points in the care pathway, 14 key messages to improve practices, 8 toolkit guides reiterating the role of each professional and the arrangements for sharing information with the primary care physician.© 2023 Société francophone nutrition clinique et métabolisme (SFNCM).Published by Elsevier Masson SAS. 6.M. Workshop: Private equity investment in Europe’s primary care sector

Abstract This workshop explores the growing role of private equity funds in Europe's primary care sector. Based on a rapid response report of the European Observatory on Health Systems to the French National Health Insurance Fund (Caisse nationale de l’assurance maladie, CNAM), it offers a platform for sharing examples from across Europe, facilitating cross-country learning through country presentations and audience participation. Private equity investments in Europe’ health sector have increased exponentially in recent years, including in primary care, but this trend has so far received very little attention. This is surprising, as this development has the potential to change the healthcare landscape in Europe dramatically. The short investment and resale horizon of private equity funds and the emergence of practice chains and monopolistic structures are of particular concern. Ultimately, the growing private equity involvement in Europe's primary care sector could undermine equitable access to health care, reduce physicians’ autonomy, affect local or regional cooperation, erode data protection and raise health care costs. However, private equity investments may also be associated with benefits, such as providing needed capital or enabling employment for young physicians who want to work part-time or do not want to run a practice as entrepreneur. So far, there is little transparent information on ownership structures and very little research on the impact of private equity investments on outcomes, such as equity, access and quality of care. This workshop aims to provide a snapshot of trends in Europe and to inform current debates on how regulators should best respond. It provides an overview of the situation in four European countries (Germany, the Netherlands, Ireland and Sweden) and of policy responses so far. The workshop will provide opportunity for the audience to comment on and discuss the country presentations, share experience in their own countries, and consider policy options in Europe to improve transparency on ownership structures and ensure the interests of patients, the public, and health workers are protected. The workshop will be of interest to researchers, practitioners and policy-makers working on primary care in Europe. Key messages • The workshop provides a forum for discussing the growing role of private equity investment in Europe's primary care sector. • It explores regulatory options to safeguard the interests of patients, health workers and the public.

Facilitating human papillomavirus vaccination pathways by extending vaccination competencies to community pharmacists: A cross-sectional survey on the acceptability and expectations among healthcare professionals and parents

BackgroundHuman papillomavirus (HPV) vaccination coverage rate (VCR) remains low in France (37.4% in girls in 2021). The French health authority recommended in 2022 to extend vaccination competencies to additional healthcare providers (HCPs), including community pharmacists (CPs). ObjectivesTo understand the acceptability by general practitioners (GPs), CPs and parents of adolescents of extending vaccination competencies and to identify benefits and barriers of new vaccination pathways. MethodsThis cross-sectional research used a qualitative and quantitative approach. For the quantitative survey, GPs, CPs and parents of adolescents eligible to HPV vaccination completed an online questionnaire. Participants were asked to imagine themselves in different pathways and evaluate them. ResultsA total of 200 GPs, 201 CPs and 800 parents were included. The level of acceptability of extending vaccination competencies to other HCPs was high in CPs (86% rated ≥7/10), but low in GPs (35%) and intermediate in parents (61%). Parents ranked first (44%) a pathway where GPs prescribed while CPs vaccinated because GPs inspire confidence as vaccine prescribers (80%) and parents prefer to be informed on vaccination by them (80%). CPs ranked first (42%) a scenario where they vaccinated after invitation of adolescents from the French National Health Insurance Fund (NHIS). They emphasized the simplicity of this scenario (94%) and the potential increase of VCR (91%), but asked to be more informed on HPV vaccination (77%) and favored television (83%) for communication campaigns. ConclusionsGPs and parents, in contrast with community pharmacists, were only moderately supportive of the extension the vaccination competencies. Confidence in the HCP remains the primary factor for adherence to a vaccination pathway beyond the simplicity of the pathway. Training of CPs, traceability tool, support from authorities and communication campaigns are levers that will support CPs in their new role and contribute to increase parents' acceptability toward CPs.

The associative approach, a force for progress in a context of Covid-19

The pioneering national association "TousPartenairesCovid" dedicated to Covid long has the singularity of operating in 2.0 and cultivating transversality. In two years, it has built up an unprecedented database based on online surveys, worked with the French National Authority for Health and regional health agencies, and designed digital tools including the "Covid long" adult, child and adolescent referral algorithm in partnership with the French National Health Insurance Fund. Very early on, it proposed positioning the coordination support systems- territorial support platforms as post-Covid cells. Today, its flexibility allows it to address other issues, such as facilitating the return to work of patients with long-standing Covid, or modeling care in medical deserts through hybridization between healthcare professionals and emerging e-health tools.

Basket clinical trial design for targeted therapies for cancer: a French National Authority for Health statement for health technology assessment.

During the past decade, health technology assessment bodies have faced new challenges in establishing the benefits of new drugs for individuals and health-care systems. A topic of increasing importance to the field of oncology is the so-called agnostic regulatory approval of targeted therapies for cancer (independent of tumour location and histology) granted on the basis of basket trials. Basket trials in oncology offer the advantage of simultaneously evaluating treatments for multiple tumours, even rare cancers, in a single clinical trial. To address the novel challenges introduced by these trials, an interdisciplinary panel was convened on behalf of the Transparency Committee of the French National Authority for Health to clarify an approach designed to guarantee a transparent, reproducible, and fair assessment of histology-agnostic treatments for reimbursement by the French National Health Insurance Fund. The requirements of this approach include the need for randomisation, clinically relevant endpoints, appropriate correction for multiple significance testing, characterisation of subgroup heterogeneity, and validation of underlying biomarker assays. A prospectively designated external control is encouraged when the implementation of a direct comparison is deemed infeasible. We also underline the importance of recording outcomes from basket trials in a registry for use as future external controls.

Long-term risk of hip or forearm fractures in older occasional users of benzodiazepines.

This article sought to study the association between patterns of benzodiazepine (BZD) use and the risk of hip and forearm fractures in people aged 50 and 75 years or more. In a representative cohort of the French National Health Insurance Fund of individuals aged 50 years or older (n = 106 437), we followed up BZD dispensing (reflecting their patterns of use) and the most frequent fall-related fractures (hip and forearm) for 8 years. We used joint latent class models to simultaneously identify BZD dispensing trajectories and the risk of fractures in the entire cohort and in those 75 years or older). We used a survival model to estimate the adjusted hazard ratios (aHRs) between these trajectories and the risk of fractures. In the entire cohort, we identified 5 BZD trajectories: non-users (76.7% of the cohort); occasional users (15.2%); decreasing users (2.6%); late increasing users (3.0%); and early increasing users (2.4%). Compared with non-users, fracture risk was not increased in either occasional users (aHR = 0.99, 95% confidence interval [CI] 0.99-1.00) or in decreasing users (aHR = 0.90, 95% CI 0.74-1.08). It was significantly higher in early increasing users (aHR = 1.86, 95% CI 1.62-2.14) and in late increasing users (aHR = 1.39, 95% CI 1.15-1.60). We observed similar trajectories and risk levels in the people older than 75 years. Occasional BZD use, which is compatible with current recommendations, was not associated with an excess risk of the most frequent fall-related fractures in people older than 50 or 75 years.

Seasonal influenza vaccination among people with diabetes: influence of patients’ characteristics and healthcare use on behavioral changes

ABSTRACT Annual seasonal influenza vaccination (SIV) is recommended for people with diabetes, but vaccine coverage remains low. We estimated the probabilities of stopping or starting SIV, their correlates, and the expected time spent in the vaccinated state over 10 seasons for different patient profiles. We set up a retrospective cohort study of patients with diabetes in 2006 (n = 16,026), identified in a representative sample of beneficiaries of the French National Health Insurance Fund. We followed them up over 10 seasons (2005/06–2015/16). We used a Markov model to estimate transition probabilities and a proportional hazards model to study covariates. Between two consecutive seasons, the probabilities of starting (0.17) or stopping (0.09) SIV were lower than those of remaining vaccinated (0.91) or unvaccinated (0.83). Men, older patients, those with type 1 diabetes, treated diabetes or more comorbidities, frequent contacts with doctors, and with any hospital stay for diabetes or influenza during the last year were more likely to start and/or less likely to stop SIV. The mean expected number of seasons with SIV uptake over 10 seasons (range: 2.6–7.9) was lowest for women <65 years with untreated diabetes and highest for men ≥65 years with type 1 diabetes. Contacts with doctors and some clinical events may play a key role in SIV adoption. Healthcare workers have a crucial role in reducing missed opportunities for SIV. The existence of empirical patient profiles with different patterns of SIV uptake should encourage their use of tailored educational approaches about SIV to address patients’ vaccine hesitancy.

L’effet du non-recours aux soins médicaux et dentaires sur les dépenses de santé des personnes ayant une maladie chronique

BackgroundPeople with chronic disease often have dental (especially periodontal) disorders. Nevertheless, people with chronic disease seek dental care less often than others. We wanted to know if there is a relationship between the consumption of medical care and the consumption of dental care, and if so if the relationship is especially strong for people with chronic disease. MethodsWe conducted a longitudinal study that combined two data-sets: consumption data from the French National Health Insurance Fund and health and socioeconomic welfare data collected with a dedicated national survey. We studied healthcare expenditure and analyzed the association between healthcare consumption, health status and healthcare expenditure over a four-year period (2010–2013). ResultsPeople who did not seek medical or dental care in 2010 exhibited irregular consumer behavior thereafter. This pattern was particularly evident among those with chronic disease whose healthcare expenditures did not stabilize during the study period compared with the rest of the study population. Among people who did not seek medical care in 2010, variation in average dental care expenditure was 91% in people with chronic disease versus 42% for those without chronic disease. Lack of medical care during the first year of the study was also associated with greater expenditure-delay in people with chronic disease (77%) compared with 15% in people without chronic disease. ConclusionThe lack of medical or dental care in 2010 for people with chronic disease did not lead to an increase in medical and dental consumption in the following years. The catch-up delay was longer than four years. This highlights a problem of monitoring and identifies a marginalized population within the healthcare system.

Inequality in income change among cancer survivors five years after diagnosis: Evidence from a French national survey.

Worldwide, around 18 million people receive a cancer diagnosis each year, most of whom survive long enough to face additional cancer-related costs. In France, most costs directly related to cancer are covered by the National Health Insurance Fund, and cancer patients can receive treatments without paying advance fees. In this context, the costs faced by cancer survivors are mostly social costs. Drawing on fundamental cause theory, this study aimed to explore the socially-differentiated evolution of cancer survivor’s income five years after diagnosis. Our study draws on the findings of VICAN5, a French national survey that was conducted in 2015/2016 in a representative sample of 4,174 cancer survivors to obtain information on living conditions five years after diagnosis, and that was restricted to 12 tumour sites accounting for 88% of global cancer incidence in France. We used the multiple imputation method and the Heckman selection model to identify the factors associated with a decrease in household income per consumption unit (HICU), while accounting for missing data. Among survivors still working five years after diagnosis, 17.6% reported lower income at survey than at diagnosis. After adjustment for socio-demographic and medical characteristics, the decrease in HICU was more frequent in women, singles, low educated survivors, and survivors with reduced working time. Finally, subjective measures of income variation and economic well-being were a useful complement to objective measures since 31.6% of cancer survivors still working five years after diagnosis reported a perceived decrease in household income. In conclusion, inequalities in economic well-being persist long after diagnosis in France, and this despite the fact that most cancer-related costs are covered by the French National Health Insurance Fund. Consequently, more attention should be paid to cancer patients with low socio-economic status to help reduce inequalities in post-diagnosis living conditions.

Trajectories of seasonal influenza vaccine uptake among French people with diabetes: a nationwide retrospective cohort study, 2006\u20132015

BackgroundAnnual seasonal influenza vaccination (SIV) is recommended for people with diabetes, but their SIV rates remain far below public health targets. We aimed to identify temporal trajectories of SIV uptake over a 10-year period among French people with diabetes and describe their clinical characteristics.MethodsWe identified patients with diabetes in 2006 among a permanent, representative sample of beneficiaries of the French National Health Insurance Fund. We followed them up over 10 seasons (2005/06–2015/16), using SIV reimbursement claims and group-based trajectory modelling to identify SIV trajectories and to study sociodemographic, clinical, and healthcare utilization characteristics associated with the trajectories.ResultsWe identified six trajectories. Of the 15,766 patients included in the model, 4344 (28%) belonged to the “continuously vaccinated” trajectory and 4728 (30%) to the “never vaccinated” one. Two other trajectories showed a “progressive decrease” (2832, 18%) or sharp “postpandemic decrease” (1627, 10%) in uptake. The last two trajectories (totalling 2235 patients, 14%) showed an early or delayed “increase” in uptake. Compared to “continuously vaccinated” patients, those in the “progressively decreasing” trajectory were older and those in all other trajectories were younger with fewer comorbidities at inclusion. Worsening diabetes and comorbidities during follow-up were associated with the “increasing” trajectories.ConclusionsMost patients with diabetes had been continuously vaccinated or never vaccinated and thus had stable SIV behaviours. Others adopted or abandoned SIV. These behaviour shifts might be due to increasing age, health events, or contextual factors (e.g., controversies about vaccine safety or efficacy). Healthcare professionals and stakeholders should develop tailored strategies that take each group’s specificities into account.

Compensation claims for work-related musculoskeletal disorders among hairdressers in France, 2010–2016

Background. Hairdressers in France experience occupational illness as well as stressful working conditions which can result in permanent incapacity mainly due to work-related musculoskeletal disorders (WRMSDs), yet WRMSDs in this workforce remain largely unstudied. Objective. The aim of this study was to analyze trends in compensation claims for WRMSDs among hairdressers. Methods. Data concerning gender, age, permanent incapacity, working experience and lost work days (LWD) of claimants were extracted from the French National Health Insurance Fund for Salaried Workers. Results. The claim rate increased non-significantly by 12.8% during the study period. The incidence rate of permanent incapacity increased significantly by 16.0%. In proportion, significant differences were observed between age groups, with age 35–49 years ranking first (45.8%), and also in claimants with working experience > 10 years (43.1%). Overall, there were 666,461 LWD during the study period with a significant increase of 16.2% (p < 0.001). Conclusion. Although the claims rate did not increase significantly, the incidence rate of permanent incapacity increased significantly and some groups at risk have been indentified.

Trends in seasonal influenza vaccine coverage of target groups in France, 2006/07 to 2015/16: Impact of recommendations and 2009 influenza A(H1N1) pandemic.

Background and aimsSeasonal influenza vaccination (SIV) uptake (SIVU) rates in France are below target. We (i) describe trends in French SIVU over 10 consecutive seasons among different target groups and (ii) examine the effects of the 2009 influenza A(H1N1) pandemic and the publication of new SIV recommendations in 2011 and 2013.MethodsOur study was based on records of vaccines delivered in community pharmacies for a permanent, representative sample of 805,000 beneficiaries of the French National Health Insurance Fund. For the first objective, we analysed SIVU rate trends among ≥ 65 year olds as well as among < 65 year olds with each of the following conditions: diabetes, respiratory, cardiovascular, neuromuscular, or chronic liver disease. For the second goal, we computed segmented log-binomial regression analyses.ResultsAfter the 2009 pandemic, except for the target group with liver diseases, where the difference was not statistically significant, SIVU fell significantly in all groups during the 2010/11 season, remaining relatively stable until 2015/16 in groups not targeted by new recommendations. Crude SIVU rates in 2015/16 were 48% (43,950/91,794) for ≥ 65 year olds and between 16% (407/2,565) and 29% (873/3,056) for < 65 year olds depending on their condition. SIVU increased modestly after new recommendations were published, but only in patients newly eligible for a free vaccine voucher.ConclusionsOur results suggest: (i) a prolonged confidence crisis in SIV, initially impelled by the 2009 pandemic vaccination campaign; (ii) that new recommendations are ineffective without additional measures. Interventional research in this field is a priority.

Trajectories of seasonal influenza vaccination uptake in French people with diabetes from 2006 to 2015

Introduction Seasonal influenza vaccine (SIV) is recommended in France for people aged 65 years or older and those with clinical risk factors, including diabetes. The French National Health Insurance Fund (NHIF) can identity these individuals and send them a voucher so they can obtain the vaccine free of charge. Despite this, SIV coverage remains below the target of 75% and has progressively decreased since the 2009 pandemic. We aimed to identify temporal trajectories (T) over a 10-year period of SIV uptake (SIVU) among patients with diabetes and to describe their correlates. Methods We identified patients with diabetes in 2006 (n = 17,259) among a representative sample of French NHIF beneficiaries. We followed them from 2006 through 2015, using SIV reimbursement claims and group-based trajectory modeling to identify SIVU-T and drug reimbursement claims to assess diabetes severity and comorbidity status. A multinomial logistic regression allowed us to study characteristics associated with the SIVU-T. Results We found 6 SIVU-T: (1) “never” T (prevalence: 32%): people with quasi null SIVU probabilities (≤ 5%); (2) “late increasing” T (4%): SIVU probability varying from ≤ 10% before 2011/12 to 85% in 2015/16; (3) “early increasing” T (8%): probability varying from ≤ 10% in 2006/07 to ≥ 80% starting in 2009/10; (4) “regular” T (33%): probability always ≥ 95%; (5) “progressively non-vaccinated” T (14%): probability decreasing from 90% in 2006/07 to 20% in 2015/16; (6) “post-pandemic decreasing” T (9%): probability decreasing right after the 2009/10 season. Overall, compared to the “never” T group, people in all other trajectories had poorer health at inclusion and/or over the study period (severe diabetes, high comorbidity score), people in the “late increasing” T group were more likely to have received newly free vaccination vouchers and/or changed general practitioners (GPs) during follow-up, and those in the “early increasing” T more likely to have received newly free vaccination vouchers and to have been hospitalized for an influenza-like illness during the follow-up. Those with “regular” T were slightly older than those with “never” T and less likely to have been hospitalized due to diabetes; those with a “progressively non-vaccinated” T were oldest (mean age at inclusion = 80 ± 7 years versus 65 ± 14 in the study population). Finally, people with a “post-pandemic decreasing” T were more frequently women, more likely to have been hospitalized due to an influenza-like illness, and to have changed GPs during the follow-up. Conclusions SIVU behavior was stable in most people with diabetes over the study period: one third were vaccinated regularly and another third never. The latter were globally healthier than the other groups and may feel less vulnerable to it, despite their clinical risk. About 25% became less inclined toward vaccination in one of two different patterns: the “progressively non-vaccinated” T may reflect the patient and/or healthcare professional's doubts about the benefits of SIV after a certain age (in part due to immunoscenescence); those in the “post-pandemic decreasing” T may have lost confidence in SIV after controversies in France during the 2009 mass vaccination campaign against the pandemic. About 10% became more likely to be vaccinated during the study period; our results suggest that receiving free vouchers for the first time might have triggered or fostered this behavioral change. Changing GPs and hospitalization for an influenza-like illness were associated with both increasing and decreasing trajectories. These events may represent key opportunities to foster or prevent behavioral changes toward SIV. Further research is needed to better understand the chronology of these events and potential causal pathways. These results should help stakeholders to adapt public health interventions to specific subgroups.

Trajectory analysis of anxiolytic dispensing over 10 years among new users aged 50 and older.

To identify temporal trajectories of anxiolytic benzodiazepine (A-BZD) use over 10 years among new A-BZD users aged 50 and older and describe treatment patterns and demographic and clinical characteristics associated with each trajectory. A representative cohort of the French national health insurance fund users was tracked from 2006 through 2015. We used latent class mixed models to identify the trajectories. We observed four trajectories among new users (no A-BZD dispensing in 2005) plus one non-use trajectory. The proportion of occasional use among users was 60%; early increasing use, 10%; late increasing use, 17%; and increasing/decreasing use, 13%. Prevalence of occasional use decreased with age in women, but not men. Duration of treatment episodes and doses differed between trajectories. Multiple regression analyses with occasional use as the reference showed that the other three trajectories shared characteristics (age, coprescriptions of other psychotropic drugs, and more general practitioner consultations) but differed by the presence at inclusion or occurrence during follow-up of psychiatric, neurodegenerative, and somatic conditions. We found four different long-term temporal trajectories in new A-BZD users (occasional, early increasing, late increasing, and increasing/decreasing use). Difficulties quitting or reducing consumption may be very different for each trajectory, requiring tailored care approaches.

Effect of restaurant types on compensation claims for work-related musculoskeletal disorders in France

Objectives. The economic burden of work-related musculoskeletal disorders (WMSDs) is industry specific. The objective was to analyse compensation claims for WMSDs among restaurant workers in France taking into account the type of restaurant. Methods. Data for 2014 were obtained from the French National Health Insurance Fund for Salaried Workers. A chi-square test was used to compare claims, incapacity and lost work day rates in different types of restaurant. Results. Prevalence for WMSDs differed significantly by the type of kitchen: collective restaurant (7.2/1000 workers), traditional restaurant (1.7/1000), and fast food restaurant (1.0/1000). There were more male claimants in traditional restaurant workers (51.0%) than in collective restaurant workers (40.1%) but more female claimants in collective restaurant workers (50.1%) than in traditional restaurant workers (39.4%). Permanent incapacity was significantly more prevalent in collective restaurant workers (49.6%), than in traditional (41.9%) and fast food (8.5%) restaurant workers. In collective restaurant workers, as a percentage, claims, permanent incapacity and lost work days increased with age range or work experience. Conclusions. Prevalence for WMSDs and therefore incapacity and lost work days are different according to the type of restaurant. Preventive strategies should take these differences into consideration.

Impact of a primary care national policy on HIV screening in France: a longitudinal analysis between 2006 and 2013.

Early diagnosis of HIV infection is a major public health issue worldwide. In 2009, the French National Authority for Health (Haute Autorité de Santé) developed specific guidelines and recommended mass screening of 15-70-year-olds across the general population. The guidelines were supported by communication directed at healthcare professionals, especially GPs. To assess the impact of the national mass screening policy on HIV testing. The study used data from the French National Health Insurance Fund database, from January 2006 to December 2013. Males and females aged 15-70 years, excluding HIV-positive individuals and pregnant females, were followed up throughout the 2006-2013 period. During the study period, 2 176 657 person-years and a total of 329 748 different individuals were followed up. Standardised and non-standardised rates of HIV screening were calculated for each year; the impact of the policy was assessed using adjusted segmented regression analyses. Overall, annual HIV screening rates increased over the study period, from 4.2% (95% confidence interval [CI] = 4.2 to 4.3) in 2006 to 5.8% (95% CI = 5.7 to 5.9) in 2013 with a more pronounced trend after 2010 (P<0.0001). This increase was more significant for those who regularly consulted a GP. For these individuals, the policy led to a 20.4% increase (95% CI = 17.0 to 23.8) in HIV screening in 2013 compared with only a 4.5% increase (95% CI = 4.4 to 4.5) for those who did not consult a GP regularly in 2013. The results show that the mass screening policy coordinated by GPs had a significant impact on HIV testing in France, which could result in positive impacts on public and individual health outcomes.

Characteristics of patients and physicians correlated with regular influenza vaccination in patients treated for type 2 diabetes: a follow-up study from 2008 to 2011 in southeastern France

We studied a cohort of 110 823 patients treated with oral hypoglycaemic agents for type 2 diabetes in southeastern France from 1 January 2008 to 31 December 2011, to identify influenza vaccination coverage trends and the patient and physician correlates of influenza vaccine (IFV) uptake. We used French national health insurance fund (NHIF) databases to identify these patients and collect data on their IFV reimbursement claims (IFVC) and patient and physician characteristics. We used multilevel multivariate polytomous logistic regressions to test the correlates of IFVC. Between 2008 and 2011 the annual IFVC rate varied from 33.7% to 32.3% in the 18–64 age group and from 69.5% to 61.1% in the 65 + age group, among whom we saw a clear trend towards reduced vaccination after 2008. In the younger group, the probability of regular vaccination each year from 2008 to 2011 increased with diabetes severity and duration, comorbidities, and the number of general practitioner and nurse visits; it was higher among patients seeing endocrinologists and lower among low-income patients than in other patients. In the older group, there was no association with either diabetes severity or physician specialty. These results suggest different patterns of correlates of influenza vaccination according to age. Endocrinologists might help to improve IFV uptake in the younger group of patients with type 2 diabetes. Communication strategies regarding influenza vaccination should be adapted to age, and collaboration between healthcare professionals should be reinforced to achieve vaccination objectives for these patients.

BackgroundUnderreporting of occupational diseases related to asbestos exposure remains a matter of concern in France. The aim of this study was to evaluate the number of claims for compensation for asbestos-related non-malignant pulmonary or pleural occupational disease in subjects having undergone a chest CT-scan in a multiregional screening programme. MethodsAmong the 5444 voluntary retired asbestos-exposed subjects recruited in four regions between 2003 and 2005 who had undergone a chest CT-scan, the number of claims for compensation for an asbestos-related pulmonary or pleural benign disease was analysed in 2006 and 2010. ResultsFollowing CT-scan screening, 17.2% of participants were acknowledged as presenting with an asbestos-related non-malignant occupational disease, essentially pleural plaques, by the French National Health Insurance fund. Underreporting decreased as duration of follow-up after CT-scan increased. Nevertheless, 4 years after CT-scan, underreporting was still as high as 36% for subjects identified as presenting with pleural plaques. Mean duration between the date of CT-scan and the date of recognition as occupational disease was 7.4 months, shorter in cases where screening was coordinated by specialized centres. ConclusionA plan of action for an easier claiming process for compensation of asbestos-related diseases is desired. This could probably be obtained through improved sensitization of physicians engaged in the follow-up of asbestos-exposed subjects, and by standardization of the interpretation and reporting of asbestos-related abnormalities observed on chest CT-scans.
