Reattachment of Fractured Maxillary Central Incisor Crown Using Fiber Post: A Case Report.


 ABSTRACT: Dental fractures comprise the most frequent form of traumatic dental injury and often require an immediate procedure for their treatment. Among the treatments, the restoration of fractured teeth by reattachment of the tooth fragment to its tooth remnant is a viable clinical procedure to treat tooth fracture due to trauma because it is considered a highly conservative technique.

Interdisciplinary orthodontic camouflage treatment with extraction of fractured maxillary central incisor and tooth substitution to treat skeletal bimaxillary protrusion- A case report

Malformed or fractured central incisors with poor prognosis could be the choice of teeth for extraction when space is required for orthodontic treatment. This case report refers to a 14-year-old girl who had a fractured upper left central incisor associated with bimaxillary protrusion. She expressed her concern to correct her dental appearance and enhance her smile aesthetics. The patient had class I malocclusion with bimaxillary proclination and severe crowding in the lower arch. Due to the increased space requirement, extraction of teeth was required as part of orthodontic treatment. Patient had a class 2 skeletal profile with prominent upper front teeth and therefore increased space requirement in the upper arch in order to retract the prominent teeth to achieve favourable aesthetics. The choice of teeth for extraction were 14,34,44 and 21. In the second quadrant the central incisor was extracted due to its poor prognosis, and the lateral incisor, canine and first premolar were substituted for the central incisor, lateral incisor and canine respectively. In the lower dental arch the first premolars were extracted. The end of treatment objectives- class 1 molars, canines, incisors were achieved with greatly improved smile aesthetics. Minimally Invasive Restoration of Fractured Maxillary Central Incisors with Partial Laminate Veneers- A Case Series

Porcelain laminate veneers are one of the most preferred treatment methods today, due to the increasing interest in minimally invasive approach and their superior aesthetic properties. With the appreciation of the minimally invasive approach and the developing adhesive procedures, the partial laminate veneers have become more popular in dental field. In these case series, the teeth were restored with partial laminate veneers by only intervening in the fractured area. In this case series, the teeth that were restored with partial laminate veneers were prepared with different techniques. Preparation was performed without bevel in enamel margin in the case 1, with incisal overlap and bevel to create more natural transition on tooth surface in the case 2. In the case 3, preparation with incisal overlap was performed with preparation of the palatal surface of the tooth. The optical impression was made with the digital camera of the Chairside Economical Restoration of Esthetic Ceramic (CEREC) acquisition unit (Omnicam CEREC Sirona, Bensheim, Germany), in the first case. In the other cases, impression was taken with elastomeric impression materials. In addition, restorations were fabricated from feldspathic porcelain and lithium disilicate porcelain. In all the cases, restoration was cemented with resin cements. The following cases present the minimally invasive treatment of maxillary central incisors with a partial laminate veneers fabricated from different ceramic materials. New materials, procedures, and techniques provide patients an improved quality of life and a greater self-esteem with creating a confident aesthetic smile.

Unconventional Endodontics for the Aesthetic Rehabilitation of Fractured, Discolored and Rotated Maxillary Central Incisor

Management of traumatic injuries to the teeth is a challenge to the practicing dentist. It has no prescriptive method for occurring, possesses no significant predictable pattern of intensity or extensiveness and occurring at times when dentists are least prepared for it. Ninety-two percent of traumatic injuries results in fracture of maxillary permanent central incisors because of its protrusive and anterior positioning. The young permanent maxillary central incisor root canal chamber is large and tapered. This case report describes the management of Ellis Class III fractured tooth in a young permanent maxillary central incisor by an innovative clinical procedure.

Reattachment of Crown in Anterior Fractured Tooth-A Case Report

In dentistry, traumatic injuries are most common in the anterior teeth due to its anatomic position. The various treatment modalities have been used depending on the extent of injury on teeth and supporting structures. Esthetic concerns are the most important challenges in modern dental practice. Adhesive dentistry is an excellent approach regarding esthetics, especially for fractured anterior teeth. This case report describes the successful treatment of reattachment procedure in a 17-year-old male patient presented with a fractured maxillary left central incisor.

Noncomplicated Surgical Removal of Fractured Maxillary Central Incisor from Lower Lip and Its Reattachment: A Case Report

AbstractTraumatic injuries to maxillary anterior teeth while playing are common in children and adolescents. Sequelae of such trauma include broken, lost, aspirated, and swallowed tooth. At times, the broken tooth fragments get embedded in the adjoining soft tissue and frequently get neglected during diagnosis. The present case report discusses the case of a 13-year-old adolescent who had broken tooth fragment emerged in the lower lip and its surgical excision to prevent detrimental reactions and scarring.

REHABILITATION OF FRACTURED CENTRAL INCISOR WITH RICHMOND CROWN- A CASE REPORT

Rehabilitation of badly broken endodontically treated teeth is a common problem in restorative dentistry. Tooth with less remaining crown height is indicated for post and core followed by crown to restore normal anatomy, function and esthetics. Patients having reduced interocclusal clearance and are most difficult to manage. Richmond crown is a feasible approach for such cases that can be performed with less incisal clearance as it accommodates post, core and crown thickness. In this case report, diagnosis and treatment planning for a case with fracture of maxillary central incisor along with fabrication technique of Richmond crown has been discussed. Reattachment of a Dehydrated Tooth Fragment Using Retentive Screws

Coronal fracture of the anterior teeth is mainly common among children and adolescents. Though diverse treatment modalities are available, tooth fragment reattachment is generally considered a viable treatment option due to simplicity, natural aesthetics, and functional success. This paper presents a case of a 10.5-year-old female patient with a fractured maxillary central incisor with a dehydrated fragment. The dehydrated fragmented part was reattached with the help of retentive holes using an adhesive bonding agent and a resin composite cement. Follow-up at 15 months showed that the tooth was vital and functional with natural aesthetics.

Endodontic management of blunderbuss canal with open apex and Ellis Class IV fracture using mineral trioxide aggregate and compo-post

The permanent teeth with open apex, blunderbuss canal, and Ellis Class IV fracture are difficult to treat as a traditional root canal procedure. Apexification is induction of apical closure, the treatment of choice for necrotic teeth with immature or resorbed root. Mineral trioxide aggregate (MTA) has been proposed as a potential material for formation of hard tissue apical barrier. Use of anatomical post results in better prognosis of the present case. This case report presents endodontic management of a fractured maxillary right central incisor with apexification and compo-post to support weak canal and to provide tooth structure. A 21-year-old male patient came to the Department of Conservative Dentistry and Endodontics, Jaipur Dental College and Hospital, Jaipur, with the chief complaint of broken upper front tooth due to accidental fall (6–7 years). Dental history of discontinued endodontic treatment 1-year back was observed. Treatment includes two phases – endodontic phase and re-construction phase followed by full-crown porcelain-fused metal prosthesis. This case report discussed an improved option of application of chairside customized made fiber posts are preferred as they stimulate the root canal anatomy, provides increased retention and adaptation and also esthetics.

Tooth fragment reattachment

Trauma to maxillary anterior teeth is very common in children. Reattachment of the original tooth fragment to the fractured tooth forms a relatively quick, biologic and esthetic restoration. Presented here is a case of ten year old female with fractured maxillary central incisor restored by tooth fragment reattachment, thereby, maintaining natural wear resistance and esthetics of the patient.

Management of a traumatized and luxated maxillary incisor with customized computed-aided design and computer-aided manufacturing zirconia post–core and crown

The incidence of maxillary anteriors being injured during any assault to the facial region is nearly 37% owing to its position in the arch. Treatment planning always depends on the clinical scenario. Coronal fracture of teeth involving the pulp may need conventional endodontic treatment and aesthetic reconstruction. When there is not enough tooth structure to retain a crown, it may often need post–core to retain final post endodontic restoration. Various types and advancements of post-and-core systems have been documented in the literature over the years, with an excellent long-term prognosis. Although the traditional custom cast post-and-core system has been considered to be cumbersome and time-consuming, it has an added advantage of allowing modification in the angle of insertion and the position of the core to obtain optimal aesthetics. “BIOLOGIC RESTORATION:” A CASE REPORT OF A NOVEL METHOD FOR RESTORATION OF A FRACTURED ANTERIOR TOOTH

Anterior tooth fracture, as a result of traumatic injuries, is a common occurrence in routine dental practice. Trauma to the anterior teeth affects the physical and psychological well-being of the patient and hence, requires quick functional and esthetic repair. In spite of the advances in material science, none of the materials completely simulate the physical and mechanical properties of natural tooth structure. Henceforth, a biological restoration seems to be a successful biocompatible and cost-effective alternative approach for treating such cases. Here, we report the clinical case of esthetic and functional reconstruction of a fractured maxillary right central incisor by means of “biological restoration” using Homogenous fragment bonding associated with the cementation of biopins produced from human dentin in a 32-year-old male patient.

Clinical management of a complicated crown-root fracture of maxillary central incisor in a 12-year-old patient

Traumatic injuries to permanent maxillary anterior teeth are seen commonly in children. Esthetic and functional rehabilitation of these teeth is important. A 12-year-old male patient reported to the Department of Paedodontics and Preventive Dentistry with fracture of the permanent maxillary right central incisor. Clinical and radiographic examinations revealed the presence of a complicated crown-root fracture. This case report describes the management of complicated crown-root fracture by endodontic therapy, orthodontic extrusion, fiber post placement, core build-up with composite, and esthetic restoration using polycarbonate crown.

Management of a crown-root fracture: A novel technique with interdisciplinary approach

Anterior teeth with subgingival fractures require a complex treatment plan that addresses biologic, functional and aesthetic factors. This case report describes the management of a crown-root fractured maxillary left central incisor. An interdisciplinary approach was used to restore the tooth due to the complex nature of the treatment. Orthodontic extrusion was performed to move the fracture line above the alveolar bone and periodontal surgery to recontour the altered gingival margin. Finally, the incisor was restored performing a root canal retreatment with a fiber post and a full ceramic crown. The treatment resulted in secured periodontal health and good aesthetics. Partial Pulpotomy of Immature Anterior Permanent Teeth with Complicated Crown Fractures: Report of Two Cases

Aim: This report aims to present the application of partial pulpotomy procedures in fractured maxillary central incisors with immature roots. The application of this treatment approach in fractured immature permanent teeth is advantageous but a number of challenges exist particularly when the affected teeth are severed with complicated fractures
 Methodology: Two cases of complicated crown fractures are reported. A 7-year-old male patient with broken maxillary left central incisor tooth and 8-year-old female patient with fractured maxillary right and left central incisor teeth were referred to the clinic after trauma. Partial pulpotomies were performed and MTA was applied directly onto the pulps.
 Results: Hard tissue barrier was observed after treatments.
 Conclusions: Partial pulpotomy is a valid treatment approach in which part of an exposed vital pulp is removed while preserving the vitality and function of the remaining part.
