'It's far from the norm': breastfeeding beyond 1 year in the Republic of Ireland.

Breastfeeding is the optimal form of nutrition for infants and young children. The World Health Organization recommends that babies are breastfed exclusively for the first 6 months of life, and up to the age of 2 years or beyond in combination with complementary food. Breastfeeding initiation and continuation rates are suboptimal globally and very low in the Republic of Ireland where health promotion initiatives and healthcare professional support predominantly focus on the important phase of initiation and early months of the breastfeeding journey. This qualitative descriptive study aimed to explore the experiences of women who chose to breastfeed their children beyond 1 year of age in the Republic of Ireland. Fourteen women participated in semi-structured interviews. Interviews were transcribed verbatim and thematic analysis was conducted. The analysis generated three overarching themes: (1) Influences on breastfeeding beyond 1 year, (2) Sustaining breastfeeding and (3) Benefits of breastfeeding beyond 1 year. Family, friends, peers, culture and commercial milk formula marketing had an influence on breastfeeding journeys. Support, determination, knowledge, bed-sharing and Covid-19 pandemic social restrictions helped to sustain breastfeeding beyond 1 year. Benefits of breastfeeding beyond 1 year such as nutrition, strengthening of emotional bonds, development of a parenting tool, and protection of child and maternal health were identified. Our findings support the need for discussions and further research on the normalization of breastfeeding beyond 1 year in the Republic of Ireland, targeted health promotion initiatives and education programmes for healthcare professionals on supporting the continuation of breastfeeding.

Administering Intravenous Fluid in the Immediate Postpartum Period Minimizes Ultimate Formula Feeding.

Background: Human milk is the optimal form of nutrition for infants. Interventions associated with a decreased rate of exposure to infant formula are of benefit. We hypothesized that intravenous (IV) fluids could be associated with a reduction of infant formula when infants are separated from their mother. Methods: Retrospective chart review of term infants admitted to Rady Children's Neonatal Intensive Care Units (NICUs) for possible sepsis who received antibiotic treatment but did not develop sepsis. IV fluid use and formula exposure were analyzed. Results: Six hundred twenty-eight infants met the inclusion criteria. Around 25.6% received IV fluids and 74.4% did not. IV fluid administration was associated with a significantly reduced chance of formula exposure while in the NICU (odds ratio: 0.694, 95% confidence interval: 0.484-0.993; *p = 0.046). Conclusion: This retrospective chart review found that infants transferred to the NICU and separated from their mothers due to chorioamnionitis exposure who received IV fluids were significantly less likely to receive any infant formula than infants who were not administered IV fluids.

A comprehensive review on infant formula: nutritional and functional constituents, recent trends in processing and its impact on infants' gut microbiota.

Human milk is considered the most valuable form of nutrition for infants for their growth, development and function. So far, there are still some cases where feeding human milk is not feasible. As a result, the market for infant formula is widely increasing, and formula feeding become an alternative or substitute for breastfeeding. The nutritional value of the formula can be improved by adding functional bioactive compounds like probiotics, prebiotics, human milk oligosaccharides, vitamins, minerals, taurine, inositol, osteopontin, lactoferrin, gangliosides, carnitine etc. For processing of infant formula, diverse thermal and non-thermal technologies have been employed. Infant formula can be either in powdered form, which requires reconstitution with water or in ready-to-feed liquid form, among which powder form is readily available, shelf-stable and vastly marketed. Infants' gut microbiota is a complex ecosystem and the nutrient composition of infant formula is recognized to have a lasting effect on it. Likewise, the gut microbiota establishment closely parallels with host immune development and growth. Therefore, it must be contemplated as an important factor for consideration while developing formulas. In this review, we have focused on the formulation and manufacturing of safe and nutritious infant formula equivalent to human milk or aligning with the infant's needs and its ultimate impact on infants' gut microbiota.

Maternal drugs and breastfeeding: Risk assessment from pharmacokinetics to safety evidence - A contribution from the ConcePTION project

Human milk is the most appropriate form of nutrition for infants while taking medication during the postpartum period is common. Discontinuation of breastfeeding is sometimes wrongly recommended for fear of adverse effects in the breastfed infant whereas only a few drugs are strictly contraindicated while breastfeeding. Most drugs are transferred from the mother's blood to the milk, but the breastfed infant usually ingests a small drug amount through human milk. As population-based evidence is still scarce on safety of drugs during breastfeeding, risk assessment relies on the little clinical evidence available and on pharmacokinetic principles, as well as on specialized sources of information that are essential for clinical decision-making. Risk assessment should not only be based on the drug's potential risk for the breastfed infant but should always take into account the benefits associated to breastfeeding, the risks of untreated maternal disease and the maternal willingness to breastfeed. Identifying situations with potential for drug accumulation in the breastfed infant is decisive while assessing the risk. Health care providers should always assume that mothers will be concerned and use risk communication as a key to ensure medication adherence and prevent unnecessary interruption of breastfeeding. When a mother still expresses concerns, decision support algorithms may facilitate communication and some strategies can be offered to minimize the drug exposure in the breastfed infant even when clinically not justified.

Breastfeeding Knowledge and Attitudes among Midwifery Diploma Students in Jordan: A Descriptive Study.

ABSTRACTBackground: Healthcare providers widely support breastfeeding as the optimal form of nutrition for infants. Midwives play a vital role in assisting the mothers to initiate and continue breastfeeding. Therefore, they must acquire proper knowledge accompanied by positive attitudes toward breastfeeding during college education. The main aim of the present study was to assess the knowledge and attitudes of diploma midwifery students toward breastfeeding.Methods: This is a descriptive cross-sectional study. A short version of the Australian Breastfeeding Knowledge and Attitude Questionnaire (ABKAQ-SF) was distributed among 72 diploma midwifery students from one community college in July 2018, using convenience sampling. Data were analyzed using SPSS software, and the statistical significance was set at P<0.05 level.Results: Results revealed neutral attitudes (a mean score of 3.02±0.36 out of 5), coupled with a lack of breastfeeding knowledge (mean score was 10.07±2.38 out of 22), particularly knowledge regarding breastfeeding problem management. Midwifery students’ knowledge and attitudes scores were significantly correlated with one another (r=0.68, P=0.001). Demographic variables such as age, year of study, residency place, or personal breastfeeding experiences affected neither the students’ breastfeeding knowledge nor their attitudes.Conclusion: The findings of this study highlight the need for reforming the curricula of midwifery colleges. Midwifery curricula should provide in-depth knowledge of human lactation physiology and management and give students the basic skills they need to assist breastfeeding women. At the same time, it should focus on the development of supportive and positive attitudes toward breastfeeding.

Prevalence and determinant factors of exclusive breastfeeding practices among mothers in Marrakesh province, Morocco: A cross- sectional survey

BACKGROUND: Exclusive breastfeeding is the best form of nutrition for infants during the first 6 months of life. Nevertheless, the practice of breastfeeding is currently declining throughout the world. OBJECTIVE: The objective of this study was to assess the prevalence of exclusive breastfeeding (EBF) practice and its associated factors among mothers living in Marrakesh province, Morocco. METHODS: The data were collected using an interviewer administered questionnaire. Both bivariate and multivariate logistic regression analyses were used to identify factors associated with exclusive breastfeeding practice. RESULTS: Prevalence of exclusive breastfeeding was 50.2%. Mothers with age more than 30 years, residing in rural area, receiving counseling related breastfeeding during antenatal follow up and having knowledge about duration of breastfeeding were more likely to practice exclusive breastfeeding than their counterparts. CONCLUSION: Although the prevalence of breastfeeding in Marrakesh was higher than the last national survey report but it is still insufficient. These findings have to stress authorities and deciders to sensitize mothers and future mothers to increase exclusive breastfeeding practice notably through the involvement of health professionals.

Maternal Zearalenone exposure impacted ovarian follicle formation and development of suckled offspring

Zearalenone (ZEN) is a secondary metabolite, which is mainly produced by Fusarium fungi and exists in various feeds and agricultural products. Recently, an increasing amount of data has shown that ZEN, as an estrogen-like hormone, can have harmful effects on the female reproductive system, especially on oogenesis and folliculogenesis. Breast milk is considered to be the ideal form of nutrition for infants; however, there are some records of contaminants in food, such as mycotoxins, which may be transferred from maternal blood to milk. In this study, we investigated the toxic effects of breast milk on folliculogenesis in offspring following maternal ZEN exposure. Our results showed that maternal ZEN exposure significantly inhibited the process of primordial follicle (PF) assembly and reduced the number of PFs in suckled offspring's ovaries. In addition, RNA-seq analysis showed that RIG-I-like receptor (RLRs) signaling pathways were activated after exposed to ZEN, which increased the expression levels of DNA damage (γ-H2AX, RAD51, and PARP1) and apoptosis related protein (BAX/BCL2 and Caspase-3). Finally, ZEN exposure interfered with follicular development, as evidenced by the reduced percentages of oocyte maturation and embryonic development when the offspring grew to adolescence. It is worth noting that maternal ZEN exposure disrupted the tri-methylation levels of H3K4, H3K9, and H3K27 in the offspring's oocytes. Our results indicated that maternal ZEN exposure affected ovarian development in offspring through the breast milk, which may be detrimental to their reproductive capability in adult life.

Mothers Who Engage in Long-Term Informal Milk Sharing.

Human milk has repeatedly been shown to be the best form of infant nutrition. Many reasons may prevent a family from providing human milk to their child, leaving them to seek other options. One alternative is informal milk sharing, where individuals and families find donor milk through online communities. We present perspectives from three mothers who used informal milk sharing for at least 6 months. Common themes identified are sadness, guilt, and stigmatization of informal milk sharing. Implications for practice include providing families and practitioners with resources for learning about informal milk sharing and navigating the community safely.

Dietary Intake of Mycotoxin Susceptible Foods by Brazilian Nursing Mothers

Background: The human milk has several health benefits to the mother and child, and it is the ideal form of nutrition for infants. However, some food contaminants such as mycotoxins can be transferred from the mother to the child through breast milk. Objective: To access the dietary intake of mycotoxin susceptible foods by nursing mothers. Methods: The dietary intake of mycotoxin susceptible foods was investigated by interviewing nursing mothers (n = 86), who were registered donors of the Human Milk Bank (HMB) of Blumenau (Brazil), through a food frequency questionnaire (FFQ). The mothers were contacted and those who agreed to answer the FFQ were recruited for the study group. The interview was conducted individually, face to face with each mother, by a trained researcher. The FFQ was divided into three food Groups (vegetables, animal and beverages, respectively), considered most prone to mycotoxins contamination. Socio-demographic characteristics of the mothers were also obtained. Results: The FFQ revealed a high weekly intake of foods susceptible to fungi and mycotoxin contamination. Consumption of wheat, coffee and cow’s milk was reported by 52, 44 and 51% of the mothers, respectively. The consumption of those foods occurred daily to more than 7 times per week. Conclusion: The study revealed that Brazilian nursing mothers frequently consume foods susceptible to mycotoxin contamination, especially wheat and its by-products. Brazilian health, environmental and agriculture authorities should pay permanent attention by applying monitoring programs to grains and cereals - both imported and domestically produced - as they are highly susceptible to fungi and mycotoxin contamination, which can be transferred to infants via breastfeeding, apart from harming the general population health.

MODERN ASPECTS OF LACTATION SUPPORT AND INFANTS BREASTFEEDING IN THE ACTIVITY OF A FAMILY DOCTOR

Introduction. Breastfeeding is a natural form of infant nutrition during evolution that determines the optimal formation of baby's health, its physical, mental and intellectual development. The aim of the research. To learn the influence of social, informational and psychological factors on the duration of breastfeeding. Materials and methods. 200 mother-child pairs were examined. Research results. According to the survey, it was found that 86,4 % of children were breastfed in the first month of life. At 6 months, ration contained breast milk approximately 58,5 %, up to 1 year - 29,0 % of the examined children. A children health study, depending on the duration of breastfeeding, showed that healthy children were longer on exceptionally breastfeeding and breastfeeding than those who had any disease during the first year of life. The median duration of exclusive breastfeeding for healthy infants was 3 months and for infants with some disease - 1 month. The breastfeeding support issues in the work of the family doctor, the role of professional advisory help of medical workers for the formation of long and sufficient lactation were discussed. It is established that the use of modern perinatal technologies for pregnancy, childbirth and newborn provides only breastfeeding up to 6 months and optimal breastfeeding. It has found out that the breastfeeding education for mothers, psychological supportive ambience has a positive effect on the duration of breastfeeding and the health of the child. Conclusions. Lactation and breastfeeding is complicated process, but completely controllаble.

Abstract C50: Applying the Stages of Change Model to breastfeeding in African-American women: A qualitative study

Abstract Background: Breastfeeding is the optimal form of infant nutrition, and has numerous health benefits for both infant and mother. One such maternal health benefit is decreased likelihood of breast cancer. Breastfeeding trends in the United States have improved in the past decade. However, racial disparities remain, with African-American women having the lowest breastfeeding rates among all racial/ethnic groups. Although breastfeeding trends in African-American women have also improved greatly, current rates do not meet Healthy People 2020 objectives. According to the CDC, African-American women are more likely to develop an aggressive form of breast cancer (triple negative) and also have a higher mortality rate from breast cancer. In addition, breast cancer has a tendency to onset at younger ages in African-American women. Thus, efforts to improve breastfeeding initiation and duration rates in this population can help alleviate breast cancer disparities. The Stages of Change model is one behavioral health theory useful in gauging readiness to adopt a preventive health behavior. There is a gap in the breastfeeding literature regarding the usefulness of this theory for breastfeeding in African-American women. The current research described here helps to fill that gap. Study Question: How can the Stages of Change model be used to increase motivation and readiness to initiate and sustain breastfeeding in African-American women? Methods: A secondary data analysis was conducted of qualitative data from a dissertation exploring the long-term breastfeeding experiences of African-American women. In the first study, 3 focus group discussions were conducted with 23 WIC breastfeeding peer counselors (PCs) to better understand breastfeeding norms and behavior of the African-American women they serve. Focus groups were conducted at community health clinics and lasted approximately 90 minutes. In the second study, semi-structured individual interviews were conducted with 11 African-American mothers who were WIC participants (identified as positive deviants), who had breastfed one child for at least 6 months, and the breastfed child was younger than 2 years old. Interviews were conducted in participant homes or public settings (ie. library, café) and lasted approximately 60 minutes. Both focus groups and interviews were audio-recorded and conversations were transcribed. Data was coded by two researchers and analyzed using thematic analysis techniques. The Stages of Change model was used as a guide in the secondary data analysis. Results: WIC PC's and interview participants described historical and socio-cultural complexities influencing African-American women's breastfeeding behaviors. Four themes developed across both studies about African-American women's motivation and readiness to breastfeed using the Stages of Change model and strategies useful to encourage breastfeeding at each stage of behavioral change. 1) Pre-contemplation Stage: Increasing Breastfeeding Awareness (preconception and pregnancy); 2) Contemplation &amp; Preparation Stages: Breastfeeding Decision-Making (pregnancy and childbirth); 3) Action Stage: Initial Breastfeeding Triumphs and Trials (childbirth and early postpartum period); 4) Maintenance Stage: Committing to Long-term Breastfeeding (extended postpartum period). Conclusions: Findings indicate opportunities to enhance breastfeeding education and strengthen breastfeeding support for African-American women at each stage of behavioral change. Lessons learned can be used in designing culturally-tailored breastfeeding interventions for African-American women and also a survey tool to assess women's readiness to breastfeed. Healthcare providers and researchers should be aware of the unique historical and socio-cultural complexities influencing African-American women's breastfeeding behaviors when developing interventions. Wide Variability in Caloric Density of Expressed Human Milk Can Lead to Major Underestimation or Overestimation of Nutrient Content.

Very-low-birth-weight infants continue to face significant difficulties with postnatal growth. Human milk is the optimal form of nutrition for infants but may exhibit variation in nutrient content. This study aimed to perform macronutrient analysis on expressed human milk from mothers whose babies are hospitalized in the neonatal intensive care unit. Up to five human milk samples per participant were analyzed for protein, carbohydrate, and fat content using reference chemical analyses (Kjeldahl for protein, high pressure liquid chromatography for carbohydrates, and Mojonnier for fat). Calorie content was calculated. A total of 64 samples from 24 participants was analyzed. Wide variability was found in calorie, protein, carbohydrate, and fat composition. The authors found an average of 17.9 kcal/ounce, with only 34% of samples falling within 10% of the expected caloric density. The assumption that human milk contains 20 kcal/ounce is no longer supported based on this study. This supports promoting an individualized nutrition strategy as a crucial aspect to optimal nutrition.

Biomonitoring of Mycotoxins in Human Breast Milk: Current State and Future Perspectives.

Human breast milk is considered as the best and ideal form of nutrition for infants. However, food contaminants such as mycotoxins, which may be transferred from maternal blood to milk, are poorly described. Mycotoxins are a major group of natural toxins frequently detected in foods. Here, we review the current state-of-the-art in the monitoring of mycotoxins in human breast milk, i.e., knowledge on occurrence, metabolism, and analytical assays utilized for their quantification. We highlight that most of the data captured to date have not been verified with the precision now capable utilizing LC-MS/MS and LC-HRMS approaches. One concern is that some studies may overestimate individual measures, and most cannot capture the patterns and levels of mycotoxin mixtures. We propose accurate assessment as a priority, especially for aflatoxins, fumonisins, ochratoxin A, zearalenone, and deoxynivalenol as well as their major metabolites. However, also so-called emerging toxins such as citrinin, the enniatins, beauvericin, aurofusarin, or Alternaria toxins should be considered to evaluate their potential relevance. Key requirements for analytical quality assurance are identified and discussed to guide future developments in this area. Moreover, research needs including investigations of lactational transfer rates, the role of human metabolism for bioactivation or detoxification, and an evaluation of potential combinatory effects of different mycotoxins are pointed out. It is hoped that LC-MS based multianalyte methods will enable more accurate, rapid and affordable human biomonitoring approaches that support informed decisions for maternal and infant health.

Minimizing Patient Safety Events Through a Multidisciplinary Approach to Human Milk Management

Optimization of nutrition is key in infant development and it is well documented that human milk is the ideal form of nutrition for infants during the first year of life. However, unaltered maternal human milk may not be sufficient to meet the nutritional demands of premature infants. Under these circumstances, addition of a fortifier to the maternal human milk has become standard practice. In one newborn/infant intensive care unit (N/IICU) located in Philadelphia, fortification and modification of human milk is handled within the Human Milk Management Center (HMMC). A multidisciplinary team rounds weekly on all infants receiving fortified or modified human milk. Feeding orders and maternal milk supply are discussed to ensure the feeding is ordered correctly, the final caloric concentration is correct and a backup feeding order is in place for low maternal milk supply. Through this multidisciplinary discussion, the team has been able to identify and resolve potential errors from reaching the patient. Improvement in patient safety while providing human milk remains of utmost importance and the central goal of the HMMC collaborative.

Novel approaches to improve the intrinsic microbiological safety of powdered infant milk formula.

Human milk is recognised as the best form of nutrition for infants. However; in instances where breast-feeding is not possible, unsuitable or inadequate, infant milk formulae are used as breast milk substitutes. These formulae are designed to provide infants with optimum nutrition for normal growth and development and are available in either powdered or liquid forms. Powdered infant formula is widely used for convenience and economic reasons. However; current manufacturing processes are not capable of producing a sterile powdered infant formula. Due to their immature immune systems and permeable gastro-intestinal tracts, infants can be more susceptible to infection via foodborne pathogenic bacteria than other age-groups. Consumption of powdered infant formula contaminated by pathogenic microbes can be a cause of serious illness. In this review paper, we discuss the current manufacturing practices present in the infant formula industry, the pathogens of greatest concern, Cronobacter and Salmonella and methods of improving the intrinsic safety of powdered infant formula via the addition of antimicrobials such as: bioactive peptides; organic acids; probiotics and prebiotics.

Prevalence and factors associated with exclusive breastfeeding at 6 months of life in Tehran: a population-based study

Exclusive breastfeeding is the best form of nutrition for infants in the first 6 months of life. The aim of this study was to determine the prevalence of exclusive breastfeeding in Tehran, Islamic Republic of Iran in the first 6 months of life, and the factors that influence it. In a population-based, cross-sectional study 538 mothers with children aged 6-24 months completed an interview questionnaire. Only 46.5% of mothers exclusively breastfed their infant in the first 6 months of life. In multivariate analysis formula supplementation in the hospital (OR = 0.41, 95% CI: 0.17-0.95) and mother receiving conflicting infant feeding advice (OR = 0.53, 95% CI: 0.37-0.78) had a negative effect on exclusive breastfeeding. Mother's intention to exclusively breastfeed (OR = 5.85, 95% CI: 2.88-11.9) and infant having first breast contact 6-30 minutes after delivery (OR = 2.35, 95% CI: 1.17-4.72) had positive effects on exclusive breastfeeding.

Breastfeeding at NICU Discharge

Human milk is the optimal form of nutrition for infants, especially sick or compromised infants, yet international data suggest that breastfeeding (feeding at the breast) and the use of expressed human milk (mother's and donor's milk) are limited in patients cared for in the neonatal intensive care unit (NICU). The goal of this study was to examine feeding status at hospital discharge among high risk infants. We used the 1991 World Health Organization infant feeding definitions, applied to the 72 hour period preceding discharge from the NICU. The study sample consisted of all high risk infants discharged from July 1, 2005, to June 30, 2006 from 13 Italian NICUs. Data on infant feeding in the last 72 hours were collected at discharge from the medical records. We recorded data from 2948 subjects with a median gestational age of 35 weeks (IQR, 32-38), a median birth weight of 2200 g (IQR, 1630-2920) and a median length of stay of 16 days (IQR, 8-33). At discharge, 28% of all infants were fed exclusively with human milk: 31%, 25%, 22% and 33% respectively in the <1500 g, 1500-2000 g, 2000-2499 g and ≥ 2500 g birth weight categories. The proportion of infants not fed with human milk varied from 6 to 82% across different centers. Our study found limited breastfeeding and use of human milk among the NICU infants at discharge. At discharge, infants with a birth weight 1500-2499 g were fed exclusively with human milk less than those in higher or lower birth weight categories.

Care of the HIV-Exposed Child—To Breast Feed or Not?

Breast milk is the best form of nutrition for infants. However, it places infants born to HIV infected women at risk of acquiring HIV infection. Total avoidance of breastfeeding is the only way of completely avoiding HIV transmission through breast milk. However, this strategy has not proved to be viable in resource constrained nations. A high infection related mortality and morbidity, and a high prevalence of malnutrition have been observed in HIV exposed infants on replacement feeding. Exclusive breastfeeding for the first six months followed by rapid weaning has also not proved to be a good strategy, with a surge in infection related morbidity and malnutrition reported after weaning. Current evidence indicates that continued use of ARV prophylaxis to mother/infant starting during pregnancy and continued to cover the entire duration of breastfeeding offers the infant best chance of HIV free survival, combining the benefits of breast milk with safety induced by ARV prophylaxis.

Breastfeeding and the Bahá'í Faith

The Bahá'í Sacred Writings reference breastfeeding literally and symbolically and provide guidance as to its practice. Breastfeeding is endorsed as the ideal form of infant nutrition. The importance of breastfeeding is underscored by the exemption of breastfeeding women from fasting, as well as by the identification of breastfeeding as being linked to the moral development of children. Several of the central principles of the Bahá'í Faith, such as the equality of women and men and the harmony of science and religion, may engender attitudes that support the practice of breastfeeding. The implications of the Bahá'í Writings with regard to breastfeeding are explored and summarized here.
