Refractory florid cystic endosalpingiosis: A case report with 5 years follow up and literature review.

Endosalpingiosis is characterized by the presence of ectopic, benign glands with a fallopian tube-like ciliated epithelium. Florid cystic endosalpingiosis (FCE) is a rare type of endosalpingiosis and presents with tumor-like lesions. In general, FCE has no specific clinical features. In this case, extensive pelvic multiple Müllerian cysts were first observed and removed during the patient's second cesarean section. Lesions relapsed after a year. Therefore, the patient underwent total hysterectomy and bilateral salpingectomy; pathology revealed that the patient had FCE. According to imaging studies during the follow up, recurrent and progressive multiple pelvic and extra-pelvic cysts were observed. The patient had no obvious symptoms, and the results of her laboratory tests were within normal limits. Ultrasound-guided aspiration and lauromacrogol sclerotherapy were performed, and in the past year, the cysts have stabilized without progression. This is the first reported case of recurrent FCE after total hysterectomy and bilateral salpingectomy with a 5-year follow up. A literature review and novel ideas for diagnosing and managing FCE based on this case are also presented.

Uterine Florid Cystic Endosalpingiosis with Conservative Surgery

A 44-year-old nulligravid woman presented with chronic pelvic pain, of low to moderate intensity, which lasted over a period of 6 months. Her medical history included obesity with a body mass index of 30 kg/m2, hypothyroidism, and celiac disease. Her surgical history included a laparoscopic ovarian cystectomy and a hysteroscopy for endometrial thickness and bleeding. Florid Cystic Endosalpingiosis and Adenomyosis of the Uterus Mimicking Malignancy

A 46-year-old nullipara woman presented with a large abdominal mass. She had a history of myomectomy by laparotomy, uterine embolization, and hysteroscopic resection of an International Federation of Gynecology and Obstetrics II myoma. A pelvic ultrasonography showed a large uterus (17.1 × 11.1 × 12.0 cm) associated with a right anechoic multilobed mass measuring 16.5 × 7.2 cm. The woman refused magnetic resonance imaging because of claustrophobia. The risk of ovarian malignancy algorithm score was 13.3%, showing a high risk of ovarian cancer.

Objective: To present a case of florid cystic endosalpingiosis with fertility preservation and subsequent assisted reproductive therapy yielding a term pregnancy.

Florid Cystic Endosalpingiosis (Müllerianosis) in Pregnancy.

Cystic endosalpingiosis refers to the existence of heterotopic cystic müllerian tissue resembling structures of the fallopian tubes. We report a case of florid cystic endosalpingiosis discovered in a pregnant woman during a scheduled cesarean section and review the current knowledge of this disease. A 30-year-old woman with a twin pregnancy attended the hospital day unit at term. The first twin was in a breech presentation and a cesarean section was scheduled. During the procedure the uterine fundus and part of the body were seen completely seeded with multitude of cyst-like structures resembling hydatids of Morgagni. The immunohistochemistry analysis showed a positive expression for PAX8 (Box-8), CK7, and estrogen and progesterone receptors. The lesions did not disappear after pregnancy. Cystic endosalpingiosis should be always borne in mind, even in pregnancy, when it comes to making the differential diagnosis of a pelvic or systemic multicystic mass.

Florid cystic endosalpingiosis presenting as an ovarian cyst in a postmenopausal woman

This paper presents the case of a postmenopausal woman with a rare florid cystic endosalpingiosis presenting as an ovarian cyst, together with cystic lesions in the pelvis. She had chronic pelvic pain and ultrasound-diagnosed cystic lesions in the pelvis. Surgical laparoscopic management was planned and executed successfully with the condition being diagnosed at the time of surgery and confirmed by histologic analysis. We herein describe the surgical technique used to excise these lesions and present a discussion about the possible implications endosalpingiosis has regarding high-grade serous neoplasms and the tubal origin of these malignancies.

Intramural florid cystic endosalpingiosis of the uterus: A case report and review of the literature

ObjectiveWe report a case of intramural florid cystic endosalpingiosis in the lower uterine segment of the uterus. Case reportA 43-year-old female presented with vaginal bleeding. Abdominal computed tomography suggested a leiomyoma with cystic degeneration. A total hysterectomy revealed a 4.0 cm × 3.8 cm cystic mass in the lower uterine segment. The cystic space microscopically was lined with a single layer or stratified layer of ciliated columnar cells that resembled tubal epithelium without cytologic atypia. The glandular spaces were surrounded by normal myometrium with no evidence of periglandular endometrial stroma, which was consistent with the diagnosis of florid cystic endosalpingiosis. ConclusionFlorid cystic endosalpingiosis involving the uterus is a rare and clinically unexpected finding; however, it should be considered in the differential diagnosis of a uterine mass.

Pelvic mass-like florid cystic endosalpingiosis of the uterus: a case report and a review of literature

Endosalpingiosis is a disorder of Mullerian system characterized by benign glands lined by tubal type epithelium and involves the peritoneum, subperitoneal tissues, and retroperitoneal lymph nodes. Endosalpingiosis is almost an incidental finding on microscopic examination. Seldom it appears as a cyst and it can be confused clinically for an ovarian tumor. A case report and a systematic review about pelvic mass-like florid endosalpingiosis of the uterus from PUB MED database were performed. We describe a case report of a 50-year-old woman with a pedunculated uterine neoformation, for which the preoperative exams could be compatible with an adnexal mass. Twelve patients with similar clinical history were discussed in the review. Laparoscopy with radical exeresis was performed. Microscopic exam revealed florid cystic endosalpingiosis of the uterus. Endosalpingiosis is a rare mullerian disorder and the main problem is that the symptomatology is not specific and it may be initially misinterpreted. In relation to the papillary aspect of the lesion and focal calcifications, the histological differential diagnosis could include serous adenocarcinoma, but the lack of cellular stratification in absence of mitotic activity, and the presence of slight nuclear atypia contradict the diagnosis of carcinoma. The differential diagnosis for endosalpingiosis also includes multiple peritoneal inclusion cysts (benign cystic mesothelioma). The aim of this case report has not been only to describe the rarity of this pathology, but it contributes to consider endosalpingiosis as a possible diagnostic hypothesis for which may be indicated a conservative surgical treatment.

MRI Appearance of Florid Cystic Endosalpingiosis of the Uterus: a Case Report

Endosalpingiosis is a non-neoplastic proliferation of ectopic tubal epithelium. It may be found incidentally or the patients may present with chronic pelvic pain. It may resemble a gynecologic malignancy on imaging findings and clinicians and radiologists should be aware of this benign entity to render a correct diagnosis and to avoid over-treatment. We report here the MR imaging appearance of a case of florid cystic endosalpingiosis.

Florid Cystic Endosalpingiosis Presenting as an Obstructive Colon Mass Mimicking Malignancy: Case Report and Literature Review

We report a case of transmural florid cystic endosalpingiosis of the colon with tumor-like obstruction in a 90-year-old woman with abdominal distention and obstruction. Colonoscopy examination revealed an obstructive luminal mass. Pathologic examination of the sigmoid colectomy specimen revealed a transmural, florid proliferation of mostly cystically dilated glands that were lined by epithelium that varied from flattened to ciliated tubal-type cells typical of endosalpingiosis. The pathologic diagnosis of florid cystic endosalpingiosis of the colon was made based on characteristic microscopic features and the immunoprofile. To our knowledge, this is only the second reported case in the English literature of this entity presenting as an obstructive colon mass.

Florid cystic endosalpingiosis of the uterus: a case report

Florid cystic endosalpingiosis of the spleen

A case of endosalpingiosis appearing as a huge multilocular cyst in the spleen of a 31-year-old woman is reported. The cystic lumens were smooth, and lined by fallopian tubal epithelium containing ciliated and secretory cells. The epithelial lining showed neither atypia nor foci of papillary proliferation, with only 3-5% MIB-1 labeling index. The epithelial lining was histochemically negative for mucin, but positive for MUC1 and progesterone receptor. Despite the appearance of a tumor-like mass, it was diagnosed as florid cystic endosalpingiosis as there was a normal tube-like epithelium. Regarding etiology, it was considered to derive from the secondary müllerian system of the splenic capsule.
