Flap Versus Flapless Implant Placement in Immediate Functional Loading: A Comparative Study

ABSTRACT Background: Dental implants are now the typical tooth replacement method. Because it speeds up therapy, quick functional loading—attaching prosthetic restorations to implants following surgery—has become popular. Flapless implant implantation reduces surgical stress and expedites recovery. Methods: A prospective comparative study of 100 flap and flapless dental implant patients was done. Patient-reported outcomes, implant survival, peri-implant parameters, and surgical complications were assessed. Statistical analysis compared the two groups’ results. Results: Both flap and flapless methods exhibited good implant survival and similar peri-implant characteristics. Patients reported equal postoperative comfort and satisfaction in both groups. Surgical complications were similar for flap and flapless surgeries. Conclusion: In immediate functional loading, flap and flapless implant insertion are equally effective and safe. Both methods yield dependable implant survival, peri-implant health, patient comfort, and surgical safety. Dental professionals should evaluate patient variables and anatomical aspects while choosing an implant implantation surgery. Implant dentistry needs further study on long-term results and treatment regimens.

Flapless Dental Implant Surgery in Bleeding Disorders

Hemostasis disorders require particular attention in dental treatment. Dental implants are a very widespread and valid method for total rehabilitation. Flapless dental implant surgery is a minimally invasive treatment that allows the implants to be placed in the jaw bones with minimal surgical trauma. The aim of this study is to report the bleeding incidence in a group of patients with bleeding disorders treated with flapless implants. A total of 52 patients with bleeding disorders (46 in anticoagulant therapy; 4 with hemophilia; 2 with von Willebrandt disease) were treated with 188 flapless implant surgeries Anticoagulants were not discontinued. Patients with hemophilia and VWD were treated following specific protocols. Four late, easy to treat bleeding complications were reported (three mild bleeding, one ecchymosis). No additional sutures or other hemostatic measures were taken, no further infusions or transfusions were reported, and no severe bleeding complications requiring more than easy on-chair treatment, were reported. In conclusion, with adequate knowledge of the procedure and the pathology, dental implantology can be safely performed in patients with bleeding disorders.

Effects of flapless and flapped implantations on soft tissue: a systematic review and Meta-analysis.

This study aimed to systematically compare the effects of flapless and flapped implantations on the surrounding soft tissues of dental implants. Nine databases were searched, including PubMed, Embase, Cochrane Library, Web of Science, Clinical Trials, OpenGrey, OpenDoar, Scopus, and Ovid, from January 1, 2013, to August 27, 2023. Randomized controlled trials comparing flapless implantation with flapped implantation for restoration of missing teeth were included. Meta-analysis was conducted on studies that met the inclusion criteria by using RevMan 5.3 and Stata 14.0. A total of 1 245 articles were retrieved, and 17 studies were ultimately included. The results of Meta-analysis showed that flapless implantation resulted in better healing of the soft tissues around the dental implants than flapped implantation. Moreover, flapless implantation showed superior changes in implant success rate [mean difference (MD)=1.06, 95% confidence interval (CI) (1.02, 1.10), P=0.004], the width of keratinized gingival changes [MD=0.10, 95%CI (0.00, 0.20), P=0.04], and probing depth [MD=-0.60, 95%CI (-0.67, -0.53), P<0.000 01], with statistically significant differences. The final combined results of modified plaque index [standardized mean difference (SMD)=-0.41, 95%CI (-0.81, 0.00), P=0.05] and modified sulcus bleeding index [SMD=-0.44, 95%CI (-0.78, -0.10), P=0.01] showed superiority over flapped implantation. The papillary presence index was higher in the flapless implantation group than in the flapped implantation group. No statistically significant differences were observed in plaque index and gingival indices changes between the two groups. Flapless implantation can achieve higher implant success rate, smaller changes in the width of keratinized gingival, and smaller probing depths than flapped implantation. It also has advantages in terms of modified plaque index, modified sulcus bleeding index, and papillary presence index.

CLINICAL EFFECTIVENESS OF GUIDED IMPLANT SURGERY IN PATIENTS WITH FULLY EDENTULOUS PATIENTS ATROPHY OF THE LOWER JAW

Objectives: The aim of this study was to evaluate the results of mandibular prosthetics based on 6 implants placed with guided surgery and immediate loading in patients with a completely edentulous mandible. Materials and Methods: 52 patients participated in this study, referred in need of full arch implant-supported reconstructions in mandible. Jaw bones patients were diagnosed with cone beam computerized tomography. Patients were evaluated according to the results of computed tomography (CT). Computer software was used to create virtual templates for 3Diagnosys, an implant treatment. Surgical guided were made for ostectomy and for implant position using a 3D printer (Stratasys. Implants were placed flapless implantation metho through the sleeves of the surgical template tightening torque of 35–45 Ncm. Patients received 312 implants (64 UV functionalised short (≤6 mm) implants, 248 standard implants) 52 implant-supported prosthesis. The following parameters were assessed: implant success, prosthetics survival and changes in peri-implant marginal bone level (MBL), probe depth (PPD), and probe bleeding (BOP). Results: After 3 months loss of the marginal bone of 0.7 ± 0.35 mm (MBL), after 12 months of observation, there was a slight loss of the marginal bone over time 1.2 ± 0.38 mm (MBL), 1,47 ± 0.42. mm (MBL), after 5 years of observation. After 5 years of observation, the average PPD was 2.42 ± 0.47 mm, the average BOP was 1.32 ± 0.85. Of the 312 implants, 3 failed to osseointegrate, 5 implants failed (peri-implantitis) after 5 years. No serious prosthetic complications have been reported. After 5 years, the effectiveness of implants was 97.4%. Conclusion: Computer-guided implant surgery and immediate loading of implants in patients with insufficient lower jaw and completely edentulous is a predictable and effective method with a minimum rehabilitation period.

Healing of Peri-Implant Tissue following Flapless Implant Surgery.

Flapless surgery for implant placement has been gaining popularity among implant surgeons. it has numerous advantages, including preservation of circulation, decreased surgical time; improved patient comfort; and accelerated recuperation. As a part of the study to evaluate crestal bone loss changes after placement of implant using the flapless technique, we placed twenty endosseous implants. Access was achieved to the crestal bone using a tissue punch. Clinical and radiographic analyses were performed second and fourth months after placement of the implant. Postoperative pain was measured by the visual analog scale at the 4th, 8th, and 24th hour. The findings of the present study demonstrate that the average crestal bone loss around the implant at 4 months using the flapless technique was 0.19 mm. No implants failed to osseointegrate, and no implants exhibited bone loss greater than 0.5 mm in the first four months. This present study shows significantly less postoperative pain in the flapless technique of implant placement. The flapless approach is a predictable procedure when patient selection and surgical technique are appropriate.

A Review of Flapless Implant Surgery

Dental implants, which are usually foreign matter to patients, are surgically inserted into or onto the jawbone. These alloplasts are used to support a single prosthetic tooth and serve as either abutments or cosmetic products for missing teeth. Several methods have been conducted to satisfy patients and surgeons. Flapless implant surgery, a more accepted procedure by patients and doctors nowadays, which uses the soft tissue punch device requiring a circumferential excision of keratinized tissue at the implant site may guarantee predictable and esthetic results. This review aims to analyze literature published in the field of implantology with flapless surgery, to compare traditional flap implant surgery and the latest flapless implant surgery, to illustrate the current scientific evidence and the new developments of this technique.is not strong enough and deformed. The second one is the most useful for needle knife instruments. More clinical study is recommended.

Effect of Anchor-Guiding Sleeve Length on the Accuracy of Computer-Guided Flapless Implant Surgery.

The objective of this study was to evaluate the effect of anchor-guiding sleeve length on the accuracy of computer-guided flapless implant surgery in edentulous cases. Twelve identical polyurethane edentulous mandibular models were equally divided into short and long anchor-guiding sleeve groups based on the type of anchor-guiding sleeve. After implant placement and scan body connections, digital impressions were taken using the intraoral scanner. Using the software's measurement function, the deviation parameters between the planned and actual position of the placed implants were calculated and compared using the Mann-Whitney U test. In the short anchor-guiding sleeve group, the median angular deviation was 4.05° (range, 2.87°-7.55°). The median linear deviation was 1.17 mm (range, 0.24-2.17 mm) for the implant apex and 0.82 mm (range, 0.43-1.67 mm) for the implant shoulder. The median deviation of the depth was 0.31 mm (range, 0.20-0.79 mm). In the long anchor-guiding sleeve group, the median angular deviation was 2.70° (range, 1.77°-4.08°). The median linear deviation was 0.88 mm (range, 0.21-1.77 mm) for the implant apex and 0.63 mm (range, 0.11-1.97 mm) for the implant shoulder. The median deviation of the depth was 0.24 mm (range, 0.09-0.53 mm). There were significant differences between the 2 groups in the angular and linear deviations at the implant apex and the shoulder and depth deviation. The accuracy of the muco-supported surgical guide was improved using the long anchor-guiding sleeve, thus providing more accurate flapless implant placement in edentulous models. However, model experiments do not always produce predictable and possible uncontrolled cause-and-effect outcomes under natural clinical conditions. Therefore, further in vivo investigations are required to determine whether the results of this study are consistent with clinical findings.

Evaluation of Crestal Bone Loss in Flap vs Flapless Implant Placement Technique: An In Vivo Comparative Study

The aims of the study were to assess the technique with-flap and flapless implant placement and to compare crestal bone heights around the implant in flapless and conventional flap technique using digital radiovisiograph, in 3 and 6 months after the surgery. A total of 20 implants were placed by flap and flapless implant technique; each patient received two implants, except for two patients who received four implants. A radiovisiograph was taken at implant placement, as well as 3- and 6-month intervals. Crestal bone level was compared between flapless and flap during these intervals and compared between intervals for each group. On evaluating the distribution, it was found to be asymmetric and hence lacked normality (K-S = 0.382; p <0.001). On mesial side, bone loss values in group I ranged from 0.40 to 1.10 units with a mean value of 0.71 and a standard deviation of 0.26 units. The median value was 0.70. On evaluating the data for normality, it was found to be symmetric and normal (K-S = 0.166; p = 0.200). This study concluded that there are not any significant differences in the crestal bone with both flap and flapless techniques. Comparatively, the flapless approach showed a lesser crestal bone height reduction, which was statistically significant. Implant dentistry is nonetheless behind when advances are concerned, we have seen the inclination toward minimal invasive implant techniques to yield better esthetic as well as improved results, thus taking care of patients' discomfort.

Accuracy of Computer-Assisted Flapless Implant Placement by Means of Mucosa-Supported Templates in Complete-Arch Restorations: A Systematic Review.

The aim of this study was to systematically review the current scientific literature regarding the accuracy of fully guided flapless implant positioning for complete-arch rehabilitations in edentulous patients and to assess if there was any statistically significant correlation between linear deviation at shoulder point, at apex point and angular deviation. The electronic and manual literature search of clinical studies was carried out using specified indexing terms. A total of 13 studies were eligible for qualitative analysis and 277 edentulous patients were rehabilitated with 1556 implants patients by means of fully guided mucosa-supported template-assisted flapless surgery. Angular deviation was 3.42° (95% CI 2.82–4.03), linear deviation at shoulder point 1.23 mm (95% CI 0.97–1.49) and linear deviation at apex point 1.46 mm (95% CI 1.17–1.74). No statistically significant correlations were found between the linear and angular deviations. A statistically significant correlation was found between the two linear deviations (correlation coefficient 0.91) that can be summarized by the regression equation y = 0.03080 + 0.8254x. Computer-assisted flapless implant placement by means of mucosa-supported templates in complete arch restorations can be considered a reliable and predictable treatment choice despite the potential effects that flapless approach could bring to the overall treatment.

Clinical Observation of Flapless Implantation in the Posterior Tooth Area under the Guidance of the Fully Guided Template.

Objective Compared with the conventional flap implantation, the postoperative effect of flapless implantation of a single posterior tooth under the guidance of the fully guided template was observed. Materials and Methods 67 cases were divided into the template group (n = 35) and the flap group (n = 32) according to the wishes and actual situation of the patients. In the template group, the fully guided template was made by rapid prototyping technology, and the flapless implantation was performed under the guidance of the template in the posterior tooth area. The flap group underwent routine flap implantation. After the operation, the template group took CBCT (cone beam computed tomography) again and fitted it with that before the operation. Neck deviation, apical deviation, depth deviation, and axial angle deviation in buccolingual and mesiodistal directions were measured to observe the accuracy of the fully guided template. At the same time, the postoperative reactions of the two surgical methods were compared by recording the operation time, pain degree 24 hours after the operation, swelling degree 72 hours after the operation, and postoperative satisfaction of patients. Results Whether in buccolingual or mesiodistal directions, the maximum values of neck deviation, apical deviation, and depth deviation were less than 2 mm. The axial angle deviation was 0.07°∼5.93° in the buccolingual direction and 0°∼4.12° in the mesiodistal direction. The guiding effect of the template was relatively reliable, and the implantation site and depth were well controlled. Although there were small deviations, the accuracy of the template could meet the clinical needs. The operation time and the VAS (visual analogue scale) score 24 hours after the operation in the template group were lower than those in the flap group (P < 0.05), and the swelling degree rating 72 hours after the operation and postoperative satisfaction of the patients were better than those in the flap group (P < 0.05). Conclusion Compared with the conventional flap implantation, the flapless implantation in the posterior tooth area under the guidance of the fully guided template could improve the accuracy of the operation, shorten the operation time, and reduce the degree of postoperative pain and swelling, which had a certain positive clinical significance.

A Split Mouth Study on Crestal Bone Architectural Changes by Flapped and Flapless Implant Surgical Procedures

Background: Rehabilitations by dental implants have often been utilized as an efficient procedure for restoring missing teeth. For implant placement two types of surgical methods are well documented. Conventional surgical approach involves raising a mucoperiosteal flap to gain access and see the underlying alveolar bone, instead there is this procedure which does not involve the reflection of the flap. Both the techniques are known to have their individual benefits and shortcomings. The present study was aimed to evaluate the longitudinal comparison and evaluation of hard tissue changes around endosseous implants placed using flapped as well as flapless surgical procedures in mandibular first molar region.&#x0D; Materials and Methods: A total of 10 healthy patients with bilateral mandibular first molar were involved in the study and 20 endosseous implants were inserted (10 in each group). Radiographic assessment was done for deviations in the marginal bone levels on both mesial and distal side and their average value was calculated at 1 month, and 3 months. All these parameters were statistically analyzed using the paired Student t test, and two-way ANOVA test and were considered to be significant if the p value was ≤ 0.05.&#x0D; Results: During the 3 months observation period, the change in crestal bone height around the implants placed by flapless and flap surgery were statistically significant. The flapless group showed less reduction in the crestal bone height compared to the flap group.&#x0D; Conclusion: Both flap and the flapless techniques of endosseous implant placement had statistically significant effect on peri-implant bone loss over the 3 month period.

Evaluation of Peri-implant Crevicular Fluid Matrix Metalloproteinase-8 levels of flapped vs. flapless dental implants: A prospective study

Objectives: The objective of this study was to compare the placement of flapped vs. flapless dental implants utilizing clinical and immunological parameters. Subjects and methods: A total of 10 patients were received 20 dental implants. One quadrant in each patient was randomly assigned to control group while other to test group with the help of coin flip. Control group with 10 flapped implants and test group with 10 flapless implants. Follow-up examinations were carried out after 1 week, 1, and 3 months. All treatments were performed under local anesthesia. Peri-implant sulcular fluid samples were collected as well as clinical parameters recorded at the follow up periods. Results: Peri-implant sulcus depth was significantly greater in flapped implants at both 4 and 12 postsurgical weeks (P < 0.005). Matrix metalloproteinase-8 values were higher to a statistically significant level in the control group at 1 (P = 0.003) and 4 weeks (P = 0.007) after placement. Conclusions: Matrix metalloproteinase 8 levels of peri implant crevicular fluid as well as pocket depth were decreased in flapless implants than conventional flap implant surgery.

The Anatomical Conditions of the Alveolar Process of the Anterior Maxilla in Terms of Immediate Implantation-Radiological Retrospective Case Series Study.

The feasibility and the level of difficulty of immediate flapless implantation depend largely on the residual alveolar bone. The purpose of the study was to determine how often immediate flapless implantation in the anterior maxilla is feasible and assess the difficulty level using cone-beam computed tomography (CBCT) scans. A radiological retrospective case series study was conducted. In total, 1200 CBCT scans from 300 consecutive patients were analyzed with dedicated planning software. Immediate flapless implants were possible in 78.33% of cases. Drilling direction was either through the apex or the palatal slope. Bimodal was conducted in 9% of the cases; only through the apex in 13.08% of the cases and in 56.25% only in the slope. In 21.67%, immediate flapless implants were excluded. The feasibility and degree of difficulty differed statistically to the disadvantage of the lateral incisors compared to the central incisors. Drilling direction caused that BASE classification reflects the difficulty level of immediate implantation. CBCT is a helpful diagnostic tool for assessing the feasibility of immediate flapless implants due to the residual bone shape and volume. BASE classification helps to determine a challenge level that may also facilitate communication and result in comparison. The alveolar bone condition allows for immediate flapless implants in most cases in the aesthetic region of the maxilla, but they should be performed by an experienced specialist with regard to the bone and soft tissue quality.

A Multilevel Analysis of Platform-Switching Flapless Implants Placed at Tissue Level: 4-year Prospective Cohort Study.

To evaluate the factors affecting peri-implant marginal bone level of single platform-switched implants with a smooth neck placed at gingival level (tissue level) using a flapless technique. Consecutive healthy patients requiring dental implant rehabilitations were enrolled in this study. Titanium implants with a zirconium-oxide-blasted surface and a platform-switch neck tulip configuration were used. Loading was performed 3 months after insertion with a provisional resin crown and after approximately 15 days with a definitive ceramic crown. Peri-implant marginal bone level (MBL) was measured on periapical radiographs at 1, 3, 6, 12, 24, 36, and 48 months by a blinded assessor. The following parameters were evaluated: location (maxillary/mandibular), position (anterior/posterior), sex (male/female), smoke (yes/no), implant placement timing (immediate, early, delayed), gingival thickness (thin/thick), endodontically treated adjacent teeth (yes/no), and diameter (3.8/4.25/5.0 mm). Multilevel analyses exploring factors associated with MBL at 36 and 48 months were performed. A total of 76 patients (42 women, 34 men; mean age: 55.6 ± 10.7 years) received 128 implant rehabilitations. The survival rate was 98.4%. MBL displayed an initial increase during the first months from insertion (preload period). Cumulative mean MBL at T48 was 0.99 ± 0.68, which was not statistically significant from the values at T24 to T36 (P > .05). Mandibular location, delayed implants, and presence of adjacent endodontically treated teeth showed higher bone loss at 36 months (P < .05). Interestingly, at 48 months, only implant placement timing showed statistically significant differences. Delayed implants showed increased bone loss compared with both early and immediate groups (P < .05). Multilevel analysis confirmed the statistical significance of implant location (P = .031; 95% CI: 0.031 to 0.659), endodontically treated adjacent teeth (P = .001; 95% CI: -1.228 to 0.859), and implant placement (P = .045; 95% CI: 0.003 to 0.337) as factors affecting MBL at 36 months. All the investigated parameters, with the only exception being the implant placement group (P = .020; 95% CI: 0.334 to 1.432), were not statistically significant at 48 months (P > .05). Platform-switched implants placed nonsubmerged with a flapless approach showed a reduced bone loss progression in the first 4 years, as MBL remained stable at longer times (36 and 48 months). Implants placed with early and immediate timing showed reduced bone loss compared with delayed implants.

Temperature Changes and SEM Effects of Three Different Implants-Abutment Connection during Debridement with Er:YAG Laser: An Ex Vivo Study.

The study aimed to evaluate a temperature increase in, and damage to, titanium implants during flapless laser debridement. The study analyzed 15 implants with various implant–abutment connections: a two-piece implant (n = 4) with a screw abutment (IA—Implant–Abutment) and a one-piece implant with a ball type fixture (BTF, n = 4) or fix type fixture (FTF, n = 4). The implants were placed in porcine mandibles 2 mm over a bone crest to imitate a peri-implantitis. The implants were debrided in contact mode for 60 s with a Er:YAG laser at fluence of 9.95 J/cm2 (G1 group: 50 mJ/30 Hz); 19.89 J/cm2 (G2 group: 100 mJ/30 Hz); 39.79 J/cm2 (G3 group: 200 mJ/30 Hz), or a scaler with a ceramic tip (G4 control group: 4 W/20 Hz). The temperature was measured with thermocouples at implant and abutment levels. The damage in the titanium surface (n = 3, non-irradiated implants from each type) was assessed using SEM (Scanning Electron Microscopy). The temperature increase at the implant level for the laser was higher at IA in contrast with FTF and BTF. (p < 0.05) The temperature change at the abutment level was lower for the scaler in contrast to Er:YAG laser at FTF. (p < 0.0002) Er:YAG laser didn’t increase the temperature by 10 °C at 100 mJ/30 Hz and 50 mJ/30 Hz. Based on SEM analysis, cracks occurred on the surface of two-piece implants and were more pronounced. Cracks and the melting of the titanium surface of two-piece implants cleaned with Er:YAG laser at 100 or 200 mJ were observed. The specimens treated with the ultrasonic scaler with a plastic curette showed the remaining dark debris on the titanium surface. We recommend using Er:YAG laser at 50 mJ/30 Hz during flapless implants debridement.

Accuracy of Computer-Guided Flapless Implant Surgery in Fully Edentulous Arches and in Edentulous Arches With Fresh Extraction Sockets.

The aim of this study was to evaluate the accuracy of computer-guided flapless implant (CGFI) surgery in edentulous jaws with fresh extraction sockets and compare it to CGFI in fully edentulous jaws. Ten patients with a completely edentulous arch (group A) and ten patients presenting natural teeth with a hopeless prognosis in the upper or lower jaw (group B) were consecutively treated with CGFI. A multipiece radiographic guide was fabricated for group B patients. The accuracy was assessed by matching the planning cone-beam computed tomography (CBCT) with a postoperative CBCT. Global coronal, global apical, angular deviation, and depth deviation were registered. The mean global coronal deviation for group A was 1.12 ± 0.5 mm, the mean global apical deviation was 1.36 ± 0.7 mm, the mean angular deviation was 3.16 ± 1.8 degrees, and the mean depth deviation was 0.51 ± 0.7 mm. The mean global coronal deviation for group B was 1.28 ± 0.6 mm, the mean global apical deviation was 1.65 ± 0.7 mm, the mean angular deviation was 3.42 ± 1.5 degrees, and the mean depth deviation was 0.53 ± 0.9 mm. Global apical deviation was significantly higher in the group B (P = 0.007). CGFI surgery in edentulous arches with fresh extraction sockets may be accurate. However, clinicians should be aware that higher apical deviation may occur in this setting.

Back ground: Microbial penetration inside the implant's internal hole creates a bacterial reservoir that is related with an area of inflamed connective tissue opposite the fixture-abutment junction and this can affect the health of the peri-implant tissue. Aims of the study: Evaluate the types aerobic and anaerobic bacterial count-percentage and difference between Aerobic and Anaerobic microflora in the implant screw hole three months after implant placement. Monitor the periodontal health status of all patients, throughout the study. Material and methods: Study methodology; Eight partially edentulous patients received 20 dental implants and these implants done with flapless surgical procedure. All patients examined clinically to determined their oral health status by examination of their plaque index, Gingival index and Bleeding on probing, each two weeks for 90 days (8visits) throughout the study period. Three months after that, the plaque sample collected from the internal hole of fixture and transfer to bacterial investigation and assessment the amount of anaerobic and aerobic bacteria. Results: Although the anaerobic viable count is higher than that of aerobic, but with statistically not significant difference between those counts (P&gt;0.05). An up to 17-year follow-up retrospective analysis of a minimally invasive, flapless approach: 18945 implants in 7783 patients.

This study investigates gender, age, jaw, implant position, loading protocol (immediate vs delayed), smoking, and type of surgery (punch vs flap) as influential factors of implant survival in a large patient collective. To evaluate the survival rates of implants in patients using a mucoperiosteal punch for flapless implantation in the majority of cases in order to evaluate its medical efficacy and safety. Between 1994 and 2015 all patients with complete data treated at the Wienerberg Dental Clinic, Vienna, Austria, were included and statistically analyzed in Cox proportional hazard (PH) models. As patients with multiple implants were included, a clustering term was added to the Cox PH model to respect pooled failures in patients. Of the initial 24282 ANKYLOS/Dentsply implants placed in 8137 patients a total of 7783 patients with 18945 implants were finally included. The mean follow-up was 2.8 ± 3.2 up to 17.9 years. Cumulative survival rates (CSRs) after 1, 3, 5, and 10 years were 98.5%, 97.7%, 96.7%, and 93.0%, respectively. Of these, 17517 (92.5%) implants were placed minimally invasive via a flapless approach by use of the ATP-Punch with comparable survival rates as observed for flap surgery. The Cox PH models proved smoking (hazard ratio [HR] = 2.2) and implant position as significant factors of implant survival. In the maxilla, canines and third molars were identified as low risk sites in comparison to the most frequently implanted first premolar site. In the mandible, the central incisor and second premolar were identified as high-risk sites, the canine as low risk site in comparison to the most frequently placed first molar site. The analyzed data concludes the safety and medical efficacy of the ATP-Punch. The CSRs using this flapless technique are comparable to the classic surgical flap approach.
