Diagnostic value of FNAC combined with BRAFV600E mutation detection in Hashimoto’s thyroiditis complicated with papillary thyroid carcinoma

BackgroundThis study aimed to analyze the effect of preoperative fine needle aspiration cytology (FNAC) combined with BRAFV600E mutation detection as compared to that of fine needle aspiration cytology alone on the diagnostic performance of papillary thyroid carcinoma (PTC) combined with Hashimoto’s thyroiditis (HT).MethodPatients with thyroid nodules in Hashimoto’s thyroiditis, who underwent fine-needle aspiration cytology examination and BRAFV600E mutation detection in the puncture eluate at the outpatient clinic, were selected. Finally, 122 patients received surgical treatment and were included in the study. We used postoperative pathological results as the gold standard. Accordingly, we compared the sensitivity, specificity and accuracy of preoperative FNAC alone and FNAC combined with BRAFV600E mutation detection in for the diagnosis of PTC combined with HT.ResultsFor PTC patients with HT, the sensitivity of FNAC diagnosis was 93.69%, the specificity was 90.90% and the accuracy was 93.44%. However, the sensitivity, specificity and accuracy of FNAC combined with BRAFV600E mutation detection were 97.30%, 90.90% and 96.72%, respectively. Therefore, combined detection can improve the sensitivity and accuracy of diagnosis (p<0.05).ConclusionFNAC combined with eluent BRAFV600E mutation detection can improve the sensitivity and accuracy of diagnosis of PTC in the background of HT.

Comparison of Fine-Needle Aspiration Cytopathology with Histopathological Examination of the Thyroid Gland in Patients Undergoing Elective Thyroid Surgery: Do We Still Need Fine-Needle Aspiration Cytopathology?

Background: The thyroid gland is responsible for various functions, but it is susceptible to pathologies. The gold standard for preliminarily diagnosing thyroid abnormalities is fine-needle aspiration cytology (FNAC), although it has some limitations; thus, postoperative histopathological examination confirms the diagnosis. The aim of the present study was to compare preoperative FNAC results with postoperative histopathological examination. Methods: This study is a retrospective study based on FNAC and postoperative histopathology examination, which were compared and analyzed. Results: This study included 344 patients between 18 and 86 years old (mean age: 53.06 ± 13.89), comprising 274 females and 70 males (mean ages 52.72 ± 13.86 and 54.39 ± 14.05, respectively) with a 3.9:1 female-to-male ratio. Statistical significance between the FNAC and histopathology results was observed (p = 0.0000), and 86 (25.00%) patients were found to have been diagnosed incorrectly based on FNAC. The sensitivity of FNAC was 92.31%, and its specificity was 82.08%, with positive and negative predictive values of 68.57% and 96.08%, respectively. Conclusions: Due to many factors, FNAC may lead to over- or under-diagnosis, increasing the chances of complications associated with the selected treatment. However, we do not have any other more accurate tools; therefore, FNAC should still remain as the gold standard of preliminary examination.

Comparative Study of FNAC and Histopathology in Diagnosis of Thyroid Swelling

Background: Neck swelling is a common clinical presentation in ENT practice and it carries significant morbidity and even mortality if not managed properly. Thyroid swelling being the most common type among them. We conducted a study to assess the role of FNAC in diagnosing the nature of thyroid swelling and comparing its result with histopathology finding. The aim and objectives of this study is to observe the correlation between preoperative Fine Needle Aspiration Cytology (FNAC) and post operative histopathology in the diagnosis of thyroid lesions.
 Materials and methods: A cross sectional study was done in the Department of Otolaryngology-Head & Neck Surgery and Department of Pathology, Chittagong Medical College, Chattogram from January 2018 to December 2020. Eighty (80) patients, both male and female of different age groups were included. The selected patients were subjected to clinical examination, routine hematological investigation, thyroid hormone test, USG, thyroid Scan, FNAC and histopathological examinations of the thyroidectomy specimens. Correlation between FNAC and histopathology reports were done in those patients.
 Results: In this study, age range of the patients were 11 years to 70 years. Out of 80 patients of thyroid swelling, the most of the patients 28(35%) were 31-40 years of age groups. out of 80 patients of thyroid swelling, 26(26.25%) were male and 59(73.75%) were female and male female ratio was1:2.81. On FNAC 64(80%) were non neoplastic and 16(20%) were neoplastic. Out of 64(80%) of non neoplastic lesions, 62(77.50%) were nodular goitre and 2(2.5%) were lymphocytic thyroiditis and out of 16(20%) neoplastic thyroid swelling, 2(2.50%) were follicular lesion, 12(15%) were papillary carcinoma, 1(1.25%) was follicular carcinoma and 1(1.25%) was suspicious for malignancy. On histopathology, out of 80 patients of thyroid swelling 60(75%) were non neoplastic and 20(25%) were neoplastic. out of 60(75%) non neoplastic, 59 (73.75%) were nodular, 1(1.25%) was Lymphocytic thyroiditis and out of 20(25%) neoplastic lesions, 3(3.75%) were follicular adenoma,13(16.25%) were papillary carcinoma,3(3.75%) were follicular variant of papillary carcinoma and 1(1.25%) was follicular carcinoma.
 Conclusion: It is concluded from the present study that female are more affected in our region. Non neoplastic lesions of the thyroid were more common (Nodular goitre being the commonest) than neoplastic lesions (Papillary carcinoma being the commonest). FNAC is essential diagnostic tool for thyroid swelling but final diagnosis to rule out thyroid cancer is surgical excision and histopathology.

Comparative study of fine needle aspiration cytology and histopathology correlation of various thyroid lesions

Patients with anterior neck swelling constitute common problems encountered in the clinical practice. In all age groups, including children, thyroid cancer is the most prevalent endocrine system cancer. Initial diagnostic testing using fine needle aspiration cytology (FNAC) has been found to be quick and cost effective. Therefore, it is widely accepted and more thyroid cases are diagnosed at an early stage, but it is associated with pitfalls due to difference in correlations between FNAC and Histopathological Examination (HPE). The purpose of the study is to compare the results of FNAC with histopathological findings in order to evaluate the diagnostic efficacy of FNAC.The present study was performed over a period of two years in a tertiary care center. Included in the study were 100 cases of thyroid nodule, which underwent FNAC and the post-operative specimens of whom were subjected to histopathological examination. The results obtained by FNAC were compared with histopathological findings.Out of the 100 cases taken for the purpose of the study, 92 were benign whereas 8 were malignant. The cytological and histopathological diagnosis correlated with each-other in 89 cases. A partial correlation between the cytological and histopathological diagnoses was found in 5 cases. In 6 cases the FNAC diagnosis was not correlating with histopathological diagnosis. FNAC revealed a sensitivity of 62.50%, a specificity of 97.83%, an accuracy of 95%, a positive predictive value of 71.43% and a negative predictive value of 96.77%.FNAC of thyroid lesions is thus proved to be a simple, cost effective and accurate method to diagnose and to guide the management of palpable thyroid lesions Thus, we can see that the diagnostic accuracy of FNAC procedure is very high making it a very useful investigation in the work-up of thyroid patients. However final diagnosis and treatment pattern should be based upon histopathology.

The role of the study of the level of thyroglobulin in the washout fuid of the needle aspiration for the detection of cervical lymph node metastases in well-differentiated thyroid cancer

The study objective: to assess the diagnostic value of measuring the concentration of thyroglobulin (Tg) in the washout fluid of the fine needle aspiration (FNA) for the detection of well-differentiated thyroid cancer regional metastases.Material and Methods. The study included 64 patients with well-differentiated thyroid cancer. Ultrasound examination revealed 71 enlarged regional lymph nodes. The patients underwent simultaneous FNA cytology examination of lymph nodes and the study of Tg concentration in the washout fluid of the needle aspiration. Regional lymph node metastases were verified in 38 patients. These patients underwent surgery. Histological examination of surgical specimens was performed in 43 out of 71 cases.Results. The sensitivity, specificity, and accuracy of the FNA biopsy and the study of the concentration of thyroglobulin in the in the washout fluid of FNA were 88.5 %, 56.3 %, 76.2 % and 100 %, respectively. The rate of false-positive results of thyroid FNA biopsy was 16 %. The optimal cutoff value for FNA-Tg measurement was 4 g/ml. This made it possible to increase the sensitivity and specificity of the method to 100 %. It was found that the presence of circulating antibodies to Tg in the blood serum had no effect on the diagnostic value of FNA-Tg measurement (p=0.421). There were no statistically significant differences in the levels of thyroglobulin in the in the washout fluid of FNA between patients with Tg concentration above and below the cutoff values (3.9 [0.47; 39.9] ng/ ml vs 12.3 [0.57; 294.8] ng/ml).

Lymphatic Contrast-enhanced US to Improve the Diagnosis of Cervical Lymph Node Metastasis from Thyroid Cancer.

Background The presence of cervical lymph node (LN) metastases (LNMs) affects clinical staging and prognosis of thyroid cancer, but the role of conventional B-mode US is limited for preoperative diagnosis of LNMs. The diagnostic value of lymphatic contrast-enhanced US (LCEUS) in thyroid cancer is still being explored. Purpose To explore the diagnostic performance of LCEUS by means of thyroidal injection of contrast agent in comparison with US in detecting LNMs of suspected thyroid cancer. Materials and Methods In this single-center prospective study conducted from November 2020 to January 2021, consecutive participants with suspected thyroid cancer underwent B-mode US and LCEUS of cervical LNs before biopsy. LNMs were confirmed with fine-needle aspiration cytologic examination, thyroglobulin washout assessment, or histopathologic examination after surgery. The diagnostic performance of LCEUS for cervical LNs was compared with that of conventional B-mode US, and its association with LN size and location was evaluated. Results The final data set included 64 participants (mean age, 45 years ± 12 [SD]; 52 women) with 76 LNs. The sensitivity, specificity, and accuracy of LCEUS for LNM were 97%, 90%, and 93%, respectively, whereas they were 81%, 80%, and 80%, respectively, for LNM at conventional B-mode US. Compared with US, LCEUS had better diagnostic accuracy for the LNs smaller than 1 cm (82% vs 95%; P = .03) and for central neck LNs (level VI) (83% vs 96%; P = .04). Conclusion Lymphatic contrast-enhanced US had better diagnostic performance than conventional B-mode US for detecting cervical LN metastases in suspected thyroid cancer before surgery, especially for LNs smaller than 1 cm and central neck LNs. © RSNA, 2023 See also the editorial by Grant and Kwon in this issue.

Thyroid Gland Examination for Thyroid Nodule through Free Hand Fine Needle Aspiration Cytology (FNAC) Versus Ultrasound Guided Core Sampling

Objective: To compare the free hand fine needle aspiration cytology versus ultrasound guided core sampling for thyroid nodule examination in thyroid gland. Study Design: Retrospective study Place and Duration of Study: Department of Surgery Unit-2, Ghulam Muhammad Mahar Medical College Hospital Sukkur from 1st January 2016 to 31st December 2021. Methodology: One hundred patients were divided in two groups. Group A had 50 patients who undergo ultrasonography through Doppler scanning while Group B had equal number of 50 patients which underwent free hand fine needle aspiration cytology. The clinical details were noted and graded as un-satisfactory or lacking follicular cells or else having <6 groups epithelial cells or having sufficient number of follicular cells. Results: The mean age was 31.5±11.8 years and majority of the patients were females 54% in comparison with 46% males. Insufficient cytopathologic cells were noticed in 18% US guided thyroidal nodule examination while similar insufficiency was seen in 44% fine needle aspiration cytology examination. The adequate cytopathological cells were noticed in 82% Ultrasounds while same was only seen in 56% fine needle aspiration cytology. Conclusion: Ultrasound guided core sampling has presented higher adequate cytology cases in comparison to free hand fine needle aspiration cytology cases. CYTODIAGNOSIS OF TUBERCULOSIS USING CONVENTIONAL ZIEHL-NEELSON STAINING METHOD AND MODIFIED BLEACH METHOD ON LYMPH NODE ASPIRATES ALONG WITH CLINICAL CORRELATION IN TERTIARY CARE CENTRE.

Tuberculosis (TB) is a common public health problem and second leading cause of communicable disease world wide. Tuberculosis is one of the top ten causes of death and the leading cause from a single infectious agent (above HIV/AIDS). India accounting for almost one fth (21%) of global incidence. Lymphadenopathy accounts for the most common form of Extra-Pulmonary Tuberculosis (EPTB) 3. A prospective study was carried on 100 cases of suspected tubercular lymphadenopathy patients over a period of 18 months at Gajra Raja Medical College and JAH Group of Hospitals, Gwalior from Jan 2020 to Jun 2021. All cases of clinically suspected tubercular lymphadenopathy reporting for Fine Needle Aspiration Cytology (FNAC) examination in Department of Pathology, Gajra Raja Medical College and Jay Arogya Group of Hospitals (J.A.H.), Gwalior, Madhya Pradesh (M.P.) with proper marking for affected lymph nodes and documentation. Aim was to evaluate the efcacy of bleach method with cytological diagnosis and the conventional method. Patients with tubercular lymphadenitis were evaluated. The acid-fast positivity by routine staining was compared with modied bleach method of AFB staining. Among 100 patients, 23(23%) were positive for AFB on conventional ZN method and the smear positivity was increased to 33 (33%) by bleach method. The bleach method is simple and inexpensive. It clearly improves microscopic detection of AFB.

Analysis of causes of cervical lymphadenopathy using fine needle aspiration cytology and excision biopsy at tertiary hospital in India

Introduction: The presentation of cervical lymphadenopathy is common, but its diagnosis is sometimes difficult. Fine Needle Aspiration Cytology (FNAC) is a simple, inexpensive, rapid investigative procedure to confirm clinical diagnosis with minimal trauma and low complication rate.Aim: To determine the diagnostic accuracy of FNAC and to correlate diagnosis with FNAC and subsequent histopathological examination of excised biopsy specimens.Materials and Methods: The study was done from data collected from 2013 to 2015. Total 100 Patients with cervical lymphadenopathy were clinically evaluated and FNAC, excision biopsy were done and analysed.Results: FNAC is most sensitive in diagnosing secondary deposits in neck nodes with sensitivity of 88.9%. FNAC is least sensitive in diagnosing lymphomas with sensitivity of 78.6%.FNAC is 100% specific in diagnosing Tubercular lymphadenitis, Lymphomas and secondary deposits in neck nodes. FNAC is 96% specific in diagnosing chronic nonspecific lymphadenitis and 94% specific in diagnosing Reactive changes in cervical lymph node. Overall average sensitivity and specificity of FNAC of cervical lymphadenitis is 65% and 98% respectively.Conclusion: FNAC is a very useful diagnostic tool in patients having significant lymphadenopathy. The metastatic carcinomas, especially squamous cell carcinoma and tuberculous lymphadenopathy can be diagnosed by FNAC with a high degree of accuracy. There is significant limitation in the diagnosis of low grade Non-Hodgkin’s lymphoma from reactive hyperplasia.

Breast filariasis presenting fibroadenoma like nodules: a rare diagnosis.

India has still not eradicated filariasis, which is still endemic throughout the country. This study focuses on the breast asanon-typicalbut notuncommonsite of breast diseasecharacterizedby nematodescausingobstructionof lymphatic vesselscaused by filariasis. The case described herein involves a 26-year-old married female who presented with a subcutaneous nodule in her left breast. Clinical interrogation of this case as part of the pathological diagnosis, phlebotomy was performed by venepuncture in order to examine peripheral blood smears under a microscope. Nonetheless, the peripheral blood smear did not show microfilariae of the disease. Furthermore, fine needle aspiration cytology was also performed to identify possible things for the study. It was diagnosed that the fine-needle aspiration cytology smears of various lesions indicate the lump carried microfilariae. Conclusively the lymphatic swelling was confirmed to be caused by Wuchereria bancrofti in fine-needle aspiration cytology examination without evidence in peripheral blood smears. It was shown that the subject had filariasis, and the patient responded well to daily treatment with diethylcarbamazine citrate for 42days.

Granulomatous Lymphadenitis in a Dog Caused by Mycobacterium intracellulare

Background: Mycobacteriosis is caused by bacteria belonging to the genus Mycobacterium, with considerable zoonotic potential and risk to public health. Infection in dogs is rare and is usually associated with immunosuppression, resulting from eating meat or contact with contaminated soil or fomites. Dogs are also known as potential sources for the spread of atypical tuberculosis in humans and other animals. This paper aims to describe the clinical, cytological, histopathological, and molecular findings of a male canine seen at University Veterinary Hospital of Cuiabá, Mato Grosso, with generalized lymphadenomegaly associated Mycobacterium intracellulare infection.Case: A 2-year-old male Lhasa Apso dog was referred to the University Veterinary Hospital in Cuiabá city, located in the Midwest region of Brazil. The patient had a history of intermittent claudication of the left pelvic limb for approximately 6 months and lymphadenomegaly with progression for approximately 2 months. The dog had wheezing and generalized lymphadenopathy (submandibular, axillary, and popliteal lymph nodes); cryptorchidism was also observed. A complete blood count revealed nonspecific results, and in the serum biochemical profile, the values of urea, creatinine, albumin, and alanine aminotransferase were within the reference range. No changes were observed on the radiography of the femurotibiopatellar joints. Considering the generalised lymphadenopathy, fine needle aspiration cytology and histopathological examination through biopsy of the lymph nodes was performed. On the cytology and histopathology, numerous negative images of moderately refringent bacillary structures distending the cytoplasm from the macrophages was found. The samples were also subjected to special Ziehl-Neelsen staining, which confirmed an accentuated and diffuse granulomatous lymphadenitis associated with alcohol acid-resistant bacilli. The polymerase chain reaction (PCR) was used to amplify the DNA of the lymph node fragment for the hsp65 gene, which was subjected to genetic sequencing and construction of a phylogenetic tree, with 99.77% genetic similarity for the species M. intracellulare. As treatment, doxycycline (10 mg/kg twice a day for 60 days) and enrofloxacin (5 mg/kg once a day for 10 days) were prescribed. However, the canine suffered car trauma leading to a fractured pelvis, which motivated the owner to opt for euthanasia at another veterinary establishment.Discussion: In the reported case, it was not possible to determine the source of infection, as the owners reported that the animal lived inside the house with only sporadic access to the street. The clinical signs manifested by this dog were nonspecific, and only the signs of generalised lymphadenopathy could be correlated with the signs expected in the infection with this mycobacterium. The hematological and biochemical laboratory findings were nonspecific, and it did not demonstrate the involvement of other organs. Considering the findings in cytology and histology, mycobacterial infection can be suspected. The diagnosis was confirmed through pathological and molecular findings. In this case, the PCR technique was used with partial amplification of the hsp65 gene and subsequent genetic sequencing, making it possible to identify a species like M. intracellulare (99.77% similarity). Due to euthanasia for another reason, it was impossible to monitor the dog's treatment and investigate other changes in the post mortem examination, especially the pulmonary lesions frequently described in M. intracellulare infection in humans.Keywords: Mycobacterium avium complex (MAC), canine, infection, non-tuberculous mycobacteriosis.

T-cell-lymphoma presented as a solitary subcutaneous mass in the ventral cervical region of an adult llama- diagnostic and treatment

BackgroundNeoplasm in South American camelids (SAC) are commonly described. The most frequently reported type of neoplasm are lymphomas and difference in the age suffering from lymphomas of and llamas is seen.This report describes a case of a solitary lymphoma in a 5 years and 9 month old llama mare displaying the approach of diagnostic imaging and successful surgical treatment.Case presentationThe llama was referred to the clinic for dyspnoea and inspiratory abnormal respiratory sounds. The clinical examination comprised blood cell count, ultrasonographic and radiographic examinations, endoscopy and fine needle aspiration cytology of a mass detected in the mid cervical region. The mass was surgically removed. Histopathological examination of the surgically removed mass diagnosed a malignant T-cell- lymphoma. According to the results of the clinical, ultrasonographic and radiographic examinations no tumor invasion was apparent in distant organs and the llama was discharged from the clinic seven days after surgery.ConclusionLymphoma has been reported to be the most common neoplasia in camelids and are more often described in young alpacas and in adult llamas. To the author´s knowledge the case presented here is the first that described a broad panel of diagnostic tools including ultrasound, radiographs, endoscopy, fine needle aspiration cytology and histopathoogical examination as well as a successful surgical treatment of a solitary lymphoma in camelids.

Comparison of Fine Needle Aspiration Cytology and Histopathology in the diagnosis of lymph node pathologies at health facilities located in Hawassa: A 5-year retrospective study.

The aim of this study was to evaluate the performance of Fine Needle Aspiration Cytology in the diagnosis of lymph node pathologies in comparison with Histopathologic examination. A retrospective health facility-based cross-sectional study was carried out at health facilities located in Hawassa city among 101 patients who had both Fine Needle Aspiration Cytology and Histopathologic examination on the same site from 13 September 2016 to 30 August 2021. Background data were collected using a structured questionnaire and analyzed by SPSS version 20. A total of 3892 patients had Fine Needle Aspiration Cytology for lymphadenopathies within a specified 5-year span, out of which 101 cases had both Fine Needle Aspiration Cytology and Histopathologic examination. The overall sensitivity, specificity, positive predictive value, negative predictive value, and accuracy of fine needle aspiration cytology for the diagnosis of lymphoma were 88.2%, 92%, 91.8%, 88.5%, and 90.1%, respectively. Fine Needle Aspiration Cytology can be recommended for clinically significant lymphadenopathy as a first-line diagnostic test since it is fast, safe, cost-effective, reasonably sensitive, and specific with significant positive and negative predictive values and diagnostic accuracy.

Atypical Breast CarcinomaA Series of Six Cases

Breast cancer comprises 10.4% of all cancer among women world and it the second most common type of cancer. Increased public awareness and screening have led to earlier diagnosis at different stages. Surgical resection can be done earlier and complete cure of the disease can be achieved. As a result survival rates for breast cancer in young women have improved significantly. Six unusual type of breast carcinoma, presented to the Surgery Outpatient Department were referred to Pathology Department for further investigations. Patient age and sex, clinical characteristics of the tumour, the duration of the lesion, the size, radiological investigation, Fine Needle Aspiration Cytology (FNAC), trucut biopsy and histopathological examination were noted. Immunohistochemical examination also done in all the cases. Out of the six cases of breast carcinoma, one was the mucinous carcinoma of breast, two invasive papillary breast carcinoma, one apocrine carcinoma, one invasive Pleomorphic Lobular Carcinoma (PLC) of breast and one was fibromatosis like metaplastic carcinoma of breast. All the patients were female except, a 71-year-old male who presented with invasive papillary carcinoma. All the female patients were presented at postmenopausal age group. After careful examination of the cytology and histopathological slides the final diagnosis was made which helped the clinician for further treatment. Rare breast cancers do not differ from other types of breast cancer in their presentation. But they vary by treatment regimens and outcomes. So, proper diagnosis with imaging, FNAC, trucut biopsy and histopathological examination is a combined tool for proper diagnosis and treatment.

FINE NEEDLE ASPIRATION EXAMINATION PROFILE OF SOFT TISSUE TUMOR IN THE ANATOMICAL PATHOLOGY DEPARTMENT OF DR. SOETOMO GENERAL ACADEMIC HOSPITAL SURABAYA DURING 1 JANUARY 2014 – 31 DECEMBER 2018

Fine needle aspiration cytology is a rapid diagnostic technique and has advantages over traditional incisional biopsy. Fine needle aspiration cytology of soft tissue lesions, if supported by clinical and diagnostic data, will provide accurate results. Fine needle aspiration cytology has the advantage of significantly reducing discomfort, has a very low risk of complications, and can be performed from multiple sides, thus providing a more representative material. The aim of this study was to provide clinicopathological profile of fine needle aspiration cytology of soft tissue tumor in Anatomical Pathology Department of Dr. Soetomo General Academic Hospital during 2014 - 2018. This study used a descriptive observasional research with retrospective approach. The samples are all patient with soft tissue tumor which had fine needle aspiration cytology examination and with/without confirmation of histopathological examination at the Anatomical Pathology Department of Dr. Soetomo General Academic Hospital during 1 January 2014 - 31 December 2018. The data was obtained for five years, there were 244 cases of fine needle aspiration of soft tissue tumors, the most gender was female consist of 128 cases (52.46%). There were more benign cases (155 cases (63.52%)) than malignant cases, and there were discrepancies in the results with histopathology in 9 cases (9.18%) and with immunohistochemical examination in 5 cases (5.10%).

Cytomorphology of Castleman’s Disease of Parotid Gland: Pitfalls and Diagnostic Dilemma in a Young Adult

Fine needle aspiration cytology (FNAC) is an easy, effective diagnostic tool that makes it the procedure of choice for all accessible swellings in the body. Castleman’s disease (CD) may present as unicentric or multicentric swelling hence is subjected to FNAC very frequently. CD mainly presents mediastinal nodes, nonetheless when present in salivary glands such as parotid, it is often diagnosed clinically as a salivary gland tumor. However, due to lack of adequate literature on its cytological features and non- specific radiological findings, the condition is often misdiagnosed. There are few case series on cytological features of CD and even fewer cases of CD in the parotid gland. Histopathology serves as the gold standard of diagnosis for CD. In superficial locations, these swellings are easily subjected to direct fine needle aspiration cytology (FNAC). Radiologically guided FNA can be performed for deeper locations. Hence, the authors present a case of 19 year old male patient, who presented with a small superficial, preauricular swelling for FNAC. The initial FNAC yielded no conclusive opinion. however a repeat FNAC and histopathological examination led to the final diagnosis. This case report aims to highlight the cytomorphological and histopathological features of CD in the Parotid gland and various difficulties encountered in making the final diagnosis.

Cytomorphological study of thyroid carcinoma.

The most common neoplasm of the endocrine system is found in the thyroid gland with a significant increase in recent decades largely due to modern diagnostic methods. Thyroid tumors generally have a favorable evolution, but there are also aggressive variants with a poor prognosis. In these aggressive tumors, the most reliable method of detecting and making a differential diagnosis is represented by ultrasound-guided fine-needle cytopuncture, confirmed by histopathological examination. Although fine-needle aspiration puncture and cytological examination are considered to have a high sensitivity and specificity, diagnostic certainty is established later only by histopathological examination. Fine-needle aspiration cytopuncture of the thyroid gland correlated with histopathological examination has played a crucial role in recognizing and identifying variants of papillary carcinoma known to have aggressive biological behavior, especially in cases of poorly differentiated carcinoma. Recognition of aggressive variants of papillary carcinoma is of major importance in the prognosis and clinical management of patients. The aim of this study was to present the correlations found in a series of thyroid tumors from patients treated in surgery and oncology departments, as well as tumors accidentally detected during autopsies in the department of forensics. All the cases selected in the study benefited from a complex histopathological diagnosis adapted to each case in order to ensure maximum efficiency.

Prevalence of Malignancy in Solitary Thyroid Nodule-A Retrospective Study

Background: Solitary thyroid nodule is defined as discrete mass palpable in an otherwise apparently normal thyroid gland. Solitary nodule is the common presentation of thyroid disorders.
 Objective: This study aimed to look into the prevalence of malignancy in clinico-radiologically detected solitary thyroid nodule and to correlate the findings in pre-operative fine needle aspiration cytology(FNAC) and post-operative histopathological examination(HPE).
 Materials and Methods: A retrospective study was carried out in our Institute for a period of 6 months using the data obtained between 2018-2020 of patients who were clinically and radiologically diagnosed as solitary thyroid nodule in the Department of General Surgery.
 Results: Out of 30 cases of clinically detected solitary thyroid nodule 7(23.3%)cases was found to be malignant. The mean age of presentation was 41.2 years with male female ratio of 1:9. 25(83.3%)cases was reported as benign nodules according to pre-operative FNAC out of these 2(6.6%)cases turned out to be malignant on post-operative histopathological examination.
 Conclusion: It is concluded that from the present study the prevalence of malignancy in clinically detected solitary thyroid nodule is 23.3%. FNAC being sensitive, cost effective and reliable tool in the preoperative assessment of solitary thyroid nodules and HPE in post operative evaluation of clinical specimen both playing a vital role in management of solitary thyroid nodule thus helping in early diagnosis and proper surgical intervention.

The study of role of mammography and fine needle aspiration cytology in the diagnosis of early breast carcinoma

Background: Epidemiologically breast cancer is the most common form of cancer among women. Prevalence rate of breast cancer is higher in urban women of middle age group. Early diagnosis of breast carcinoma is required for effective treatment else it may lead to mortality if advanced to stage 3 or 4. At present, there are different diagnosis modalities for early stage breast carcinoma. Currently triple diagnostic assessment including clinical, radiological and pathological examinations with 99% sensitivity is used in diagnosing all palpable breast lumps. The aim of the study was to determine the sensitivity and specificity of mammogram and Fine needle aspiration cytology (FNAC) in diagnosing breast cancer.Methods: A cross sectional study was conducted on total 80 female patients above 30 years of age with a palpable breast lump. Patients were sent for mammography, fine needle aspiration cytology and histopathological examinations. Results of mammography, FNAC and HPR were compared and statistically correlated.Results: Sensitivity of mammogram diagnosis was observed to be 92.857%, specificity as 86.842%, positive and negative predictive values to be 88.636% and 91.667% respectively. Therefore, current study findings revealed accuracy of mammogram to be 90.000%. Sensitivity of FNAC diagnosis was observed to be 92.857% and specificity to be 100%, positive and negative predictive values were observed to be 100% and 92.683% respectively. The study results revealed that FNAC diagnosis exhibited accuracy of 96.250%.Conclusions: Current investigation findings revealed that FNAC has more accuracy in predicting breast cancer than mammogram. Also, it was concluded that there is a significant association between FNAC and mammogram diagnosis.
