1.Y.1. Navigating choppy water for global health competencies, education and innovation in Europe

Abstract Global health is rapidly evolving against a backdrop of increasingly complicated, intersecting challenges and crises across health, socio-economic and ecological systems. Interconnected social justice movements call for greater acknowledgement, accountability and qualified responses to address inequities and their structural drivers. The field of global health is going through a period of reckoning and debate about how colonial legacies and structural inequities pervade the sector and how activities of global health professionals across research, policy and practice are complicit in and perpetuating them. Simultaneously, global health education (GHE) is growing, with new speciality degrees and core or elective courses integrated into degrees in medicine, health and other disciplines. Thus, GHE holds great potential as a space to train future professionals with competencies and aptitudes to confront inequalities through innovative, evidence-informed and sensitive approaches. Yet it is also a space in which inequalities may be reproduced through material and epistemic injustices. To address these paradoxical tensions, transformation in GHE is needed in terms of what is taught, how it is taught and who is involved in teaching. This Pecha Kucha workshop will contribute to this discourse by exploring perspectives on issues including: i) What key competencies and content should be included in GHE across different disciplines and Bachelor, Master or Doctoral level?; ii) How can we better practice and develop in educators and students critical perspectives and capacities to be equipped for and responsive to inequities and social injustice?; and iii) How can we ensure meaningful engagement and partnership in education with countries across varied contexts and communities with direct lived experience of marginalisation and deprivation? Four panellists with diverse backgrounds and perspectives will provide insights on their initiatives to tackles such issues within their own institutions and European alliances, followed by a short discussion with the audience. The presentations will emphasise the need to progressively build affective, practical and cognitive competencies to critically engage with historical, geopolitical and structural inequities and injustices, including how they manifest in local contexts. Panellists will highlight examples of innovative methods and approaches to build these capacities, including through co-design in education that fosters self-reflexivity in students and builds alliances and partnerships with non-academic stakeholders, such as civil society and people with lived experience. The Pecha Kucha workshop aims to inspire exchange across institutions and countries, and with the audience, about how we can collaboratively set our compasses and our sails to navigate the choppy waters of tackling pervasive colonial legacies and sailing towards an equitable, inclusive and effective future for education and action in global health. Honors Track in Global and Occupational Health: a students’ perspective from Humanitas University

Abstract Background To meet an increasing interest in global health, more opportunities for medical students should be created. Humanitas University (Milan, Italy) has debuted a new extracurricular activity called “Honors Track in Global and Occupational Health” (HT-GOH), to allow students critical insights into the field at the beginning of their career. Objectives HT-GOH is a two-year programme, led by expert professors in the field. The main aim is to circumvent limitations of global health education at individual institutions, achieved through a combined presence and online approach that allows for international collaboration with experts and attendance of online lectures and travel abroad for practical experiences. Complemented by monthly meetings, participation to workshops, seminars, conferences, and the discussion and literature review of crucial global health topics. Results The six participating students are actively promoting discussions, exchange of ideas, and literature findings on many global health issues. They audited courses in other universities and are building connections within the field. The active involvement of the students and their increasing interest in the global health debate can be seen by their participation in this abstract and the conference. The effectiveness of the programme is being evaluated through monthly feedback by students. Conclusions The programme allows for an initiation to research work, promoting collaboration between students and exploration of a variety of topics under the guidance of experts. The nature of a specific programme allows for a more individualized approach and greater development of skills unique to each student. This HT represent a valid approach to global health education, especially in settings in which this field is only taught in the context of a public health course. The European Union's Global Health Actorness: A Research Agenda for a New Age of Pandemics

AbstractWhilst the European Union's (EU's) response to health emergencies has historically been subpar, some suggest that it emerged from COVID‐19 as a stronger global health actor. This prompts two interrelated questions: how have International Relations scholars conceptualised and assessed EU ‘actorness’ in the all‐too‐often neglected field of global health, and what adjustments are required in anticipation of a potential new age of pandemics? This article revisits the longstanding notion of ‘actorness’ and dissects the few studies that apply it systematically to analyse the EU's role in global health. After identifying some key shortcomings in the literature, the article formulates a research agenda suitable for the complex and high‐stakes nature of contemporary global health endeavours. By further adapting the concept of ‘actorness’ to a multifaceted EU external action as well as to an increasingly intricate global governance architecture, this research agenda contributes to the broader scholarship on the EU's international outreach.

Addressing Challenges for Psychotherapy Supervision in Global Mental Health: Experiential Learnings From Rural Nepal.

As the field of global mental health grows, many psychotherapy trainees will work across cultures in low-resource settings in high-income countries or in low- and middle-income countries. Faculty members and mentors may face several challenges in providing supervision for psychologists in low-resource settings. As such, there is a need to develop best practices for psychotherapy supervision in global mental health. We describe the common challenges and potential strategies in psychotherapy supervision based on our research, clinical, and academic partnerships between academic institutions, a nonprofit organization, and the Nepali government. The strategies and considerations we have found helpful include focusing on therapies with strong behavioral and interpersonal (rather than emotional or cognitive) components and using locally validated therapies or standard manuals that have been endorsed by the WHO for low-resource settings. Other strategies include providing psychotherapy training for local psychiatrists who may be in supervisory roles and gaining competence in navigating different expectations of social structures and family dynamics. Supervisors face many challenges while supporting trainees and early psychologists in global mental health settings. While ensuring local adaptation, key considerations can be developed into best practices to support psychiatrists, supervisors, and trainees based in low- and middle-income countries.

A Mental Health Crisis: The Ripple Effects of COVID-19

There is little doubt that the COVID-19 pandemic has had an impact on people’s mental health all around the world. The impact has been felt by various groups, including health-care workers, patients, children, and marginalized communities. Coping strategies have been identified to mitigate the psychological effects of the crisis and promote resilience. These strategies include psychological first aid, telemental health, self-help resources, peer support, community mobilization, social protection, and mental health advocacy. There is still much work to be done in the field of global mental health about COVID-19. In addition to collaborative and interdisciplinary methods, attention to mental health fairness and human rights, and more integration of mental health into the COVID-19 response and rehabilitation, more robust and representative studies are required. Collaboration among policymakers, program implementers, donors, and researchers is crucial in addressing mental health issues and improving coping techniques during and after the COVID-19 epidemic. Priorities include tackling stigma and prejudice, expanding access to mental health services, and making that mental health is included into larger health and social policy. During and after the COVID-19 pandemic, it is imperative that we keep making mental health a top priority and investing in it. We can encourage well-being and resilience in people and communities all throughout the world.

Strategic Alliances in Global Health: Innovative Perspectives in the Era of Sustainable Development.

This article discusses current challenges in the field of global health and the World Health Organization's (WHO) strategies to address them. It highlights the importance of measuring the health impacts of global recession and globalization and the need for human-centered approaches to sustainable development. Emphasis is placed on commitment to health equity and the use of strategic partnerships for health at global, national, and local levels. Improving the health and well-being of populations, as well as public health equity, are core principles of the 2030 Agenda for the Sustainable Development Goals (SDGs). These principles are expressed in SDG 3, which promotes universal access to health services and systems and recognizes global health as a basic human right. It highlights the importance of strategic partnerships to combat emerging health crises, improve public health indices, and address the burden of chronic disease. These partnerships are contemplated in SDG 17 and are manifested in different modalities, such as network governance, cross-sector collaboration, public-private partnership, and social participation. This diversity of alliances has played an important role in scaling up and strengthening universal health systems around the world, including in Latin America and the Caribbean. The text concludes by presenting the essential characteristics of these inter-organizational and inter-institutional alliances in the field of global health.

Examining Opportunities Offered Outside of Home Country in Physical Therapy Education: Considering the Colonial History of Global Health.

Increasingly, PT programs in the United States are providing educational experiences outside of the home country (OHC) where faculty and students engage in global health initiatives. It is important to consider that the field of global health has its historical roots in colonialism, which has led to inequities and injustice. Those who are engaged in this work must assist in reversing this colonial legacy. The purpose of this position paper is to present the colonial history of global health, to provide context, and to spark critical reflection among PT educators about how OHC experiences are developed and configured. As a profession, we must educate ourselves about the history of global health and critically reflect on OHC engagements to ensure that we support equity and justice. We must start by grounding our OHC experiences in health equity and justice. This should be done by considering the colonial history of global health and the privileges we hold as members of academic institutions in the United States. Why we offer OHC experiences in PT education should be investigated and clear to both faculty and students. Excluding this colonial history and its impacts risks perpetuating the historical harms to the health and well-being of the populations we claim to be serving.

ИНИЦИАТИВЫ ВСЕМИРНОЙ ОРГАНИЗАЦИИ ЗДРАВООХРАНЕНИЯ В СФЕРЕ ПОВЫШЕНИЯ БЕЗОПАСНОСТИ ПАЦИЕНТОВ И ИХ РЕАЛИЗАЦИЯ В РОССИЙСКОЙ ФЕДЕРАЦИИ

Currently, patient safety is one of the most important areas of healthcare, and it requires global coordinated efforts. The term "patient safety" covers various aspects, including the prevention of errors and adverse events, minimising risks of infections associated with medical care, ensuring proper use of equipment and effective interaction between patients and medical staff. World Health Organization (WHO) as a specialized unit of the United Nations in the field of global health has a unique international position and authority. It collects and analyzes data from around the world, conducts research and consults member countries within its competence, including medical care safety. In the Russian Federation, the movement for patient safety is still in the process of formation, many medical organizations, when implementing modern principles of safe medical care, face a shortage of practical manuals and other methodological materials. This literature review is devoted to the analysis and systematization of WHO initiatives in the field of improving patient safety. The review pays special attention to WHO's practical recommendations for improving patient safety, which can be used in medical organizations. The review also provides an analytical assessment of the implementation of the main activities aimed to improve patient safety in the Russian Federation. Thus, this study will be interesting to a wide range of readers, especially health care managers and medical professionals who plan to implement the principles of safe medical care in their organizations.

The Impact of Dentistry on Global Health: Challenges and Research Priorities

Even though oral health has improved worldwide, there are still many obstacles to overcome, especially in high-income and low-income countries' underprivileged groups. For many people, having access to basic dental healthcare is still an asset rather than a right. The July 2019 release of the oral health issue in The Lancet sparked a buzz and raised awareness of dental services and oral health issues. To close the gap connecting dental studies and health worldwide and to make oral health a top concern in public health discourse globally, there is still more work to be done. Based on Shiffman's approach, we propose the creation of an international oral health network to promote systemic improvements and impact health policy. Four major areas of challenge—problem means, position, alliance building, and governance-have emerged from our experience in the field of global health. We provide these challenges to include stakeholders, in addition to seeking external validation. How well our profession handles these issues will influence our success concerning the Sustainable Development Goals and beyond.

Fundamental dermatology education of medical doctors at a regional hospital in Johannesburg, South Africa.

dermatology is a prevalent field of global health and dermatological conditions are amongst the most frequent complaints affecting communities, yet dermatology has become an overlooked aspect of the medical school curricula and many medical schools have failed to provide doctors with training to manage these conditions confidently and adequately. This study aimed to determine the baseline dermatological knowledge of medical doctors and determine the influence of fundamental dermatology education on hospital medical staff at a regional hospital in Johannesburg, South Africa. the knowledge and confidence of 33 medical doctors were tested using a pre-test and post-test quasi-experimental design. Participants completed an online questionnaire followed by an image-based test consisting of 20 questions to determine their level of confidence in diagnosing and managing common dermatological conditions. The participants then attended a sixty-minute educational session based on common dermatological conditions. Following this, their level of confidence and knowledge on the subject was re-assessed using the same online test. Pre and post-intervention confidence and test scores were compared. the pre-intervention test mean score was 67.11%. The post-intervention mean score was 92.50%. The difference between means (post-intervention - pre-intervention) ± SEM was 25.39 ± 4.81. The intervention significantly improved overall test scores (p-value < 0.0001). Many participants felt that their undergraduate training was insufficient in preparing them for the management of common dermatological conditions. the baseline knowledge and confidence of medical doctors in managing common dermatological conditions was poor and such educational interventions have significant value in improving the ability of medical doctors in managing common dermatological conditions. More time should be dedicated to dermatology training at an undergraduate level.

Global Mental Health Considers the Structure of the Mental Health System in the World

The World Health Organization defines mental health as a state of well-being in which each person realizes their potential and can cope with the normal stresses of life, work productively and fruitfully, and can contribute to their community. Global mental health refers to the international perception of different aspects of mental health. Taking into account cultural differences and country-specific conditions, it deals with the epidemiology of mental disorders in different countries, their treatment options, mental health education, political and financial aspects, the structure of the mental health system, and mental health human resources and human rights issues. The general goal of the field of Global Mental Health is to strengthen mental health throughout the world, providing information on the mental health situation in all countries and identifying mental health needs in order to develop economic procedures that will address these specific needs.

Relationship Between Body Mass Index and Development of Gross Motor Skills in 5-7-Year-Old Children

Background: Obesity in children and adolescents in the 21st century has become a major issue in the global health field. Previous studies showed that obese children have substantial adverse short- and long-term health consequences, including asthma, chronic low-grade systemic inflammation, diabetes, and numerous orthopedic complications. The purpose of this study was to investigate the relationship between body mass index (BMI) and the development of gross motor skills in 5-7-year-old children. Methods: This was a cross-sectional study conducted on children 5 - 7 years old in Bojnourd, Iran. The sampling method was multistage. A checklist containing the demographic data and BMI of participants was completed. Ulrich’s test of gross motor development (3rd edition) was also used to assess gross motor skills in children. Results: The age range of participants was 5 - 7 years. The mean ± standard deviation (SD) of the participants’ age was 6.24 ± 0.82 years. Furthermore, the participants’ mean ± SD weight was 24.8 ± 3.68 g (range: 17.5 - 32.2 g). There was a significant statistical difference in gross motor skills scores in the three groups of obese, overweight, and optimal weight (P &lt; 0.0001). There was also a significant difference in gross motor skills scores between the two gender groups (P &lt; 0.0001). Conclusions: Higher weight groups had lower motor skills scores. Therefore, it is necessary to adopt appropriate policies for physical activity and to reduce the prevalence of obesity and overweight in children.

Distribution and antimicrobial resistance analysis of gram-negative bacilli isolated from a tertiary hospital in Central China: a 10-year retrospective study from 2012 to 2021.

Gram-negative bacilli are one of the most common causes of various infections in clinical. The emergence and global spread of multi-drug resistant gram-negative bacilli has become a major challenge in the global public health field. A total of 51,189 non-repetitive strains of gram-negative bacilli were isolated in clinical settings. The antimicrobial susceptibility testing was conducted by using the automated VITEK 2 compact system and the matched AST susceptibility test card, complemented by the disk diffusion method. The antimicrobial susceptibility results were interpreted by CLSI. Rates of MDR and XDR in Klebsiella pneumoniae, Acinetobacter baumannii, and Pseudomonas aeruginosa were investigated. Used the chi-square test to determine whether the antimicrobial resistance rates of four major gram-negative bacilli isolated from ICU and non-ICU department have statistical differences. Escherichia coli (31.4%), Klebsiella spp. (21.2%), Acinetobacter spp. (13.8%), and P. aeruginosa (11.0%) were the most frequently isolated gram-negative bacilli. Escherichia coli was the top one organism isolated from urinary tract (68.4%), bloodstream (39.9%), body fluid (33.2%), wound and pus (37%), except for respiratory tract (8.8%). Whereas Acinetobacter baumannii and K. pneumoniae were the major isolated organisms from respiratory tract. Acinetobacter baumannii showed high resistance to fluoroquinolones, β-lactam/β-lactamase inhibitor combinations class, ceftazidime, cefepime, imipenem, and meropenem, the resistance rates reached more than 70%. Ceftazidime showed a lower resistance rate to E. coli than ceftriaxone. For E. coli, fluoroquinolones showed a high resistance rate (ciprofloxacin 61.36% and levofloxacin 53.97%), whereas amikacin, carbapenems exhibited a lower resistance rate fluctuating at 2%. Acinetobacter baumannii and K. pneumoniae showed rapid increases in carbapenem resistance whereas E. coli had the lowest resistance rate and remain stable at 2%. Acinetobacter baumannii exhibited the highest rate of MDR and XDR, reaching 60-80 and 45-55%, respectively. Compared to non-ICU departments, the resistance rates of four major gram-negative bacilli in the ICU department were much higher and the differences were statistically significant (p < 0.05). Amikacin, carbapenems, and piperacillin/tazobactam exhibited relatively high sensitivity, whereas fluoroquinolones showed high resistance rate whether they can be the first-line antimicrobials for empirical treatment of UTI should take more consideration. The gram-negative bacilli in ICU were more resistance than that in non-ICU. These findings are helpful for clinicians using antimicrobials reasonably.

Conceptions of Mental Illness and Psychopharmaceuticals in Global Health

The field of global health has prioritized the scaling up of services and treatment for mental health in low and middle-income countries. While equitable access to treatments constitutes a priority, the call for urgent action to fill the treatment gap has been advanced largely in the absence of an appeal for ethnographic attention to sociocultural knowledge of conditions and their treatment. This article argues that local knowledge of conceptions of mental illness and psychotropic medication is foundational for an informed understanding of treatment in relation to subjective experience, cultural meaning, and clinical efficacy. These issues are specifically explored in relation to scientific, clinical, and popular discourse surrounding the cultural trope of “chemical imbalance.”

Exploring perceptions of travel-eligible individuals with dementia and hotel operators

ABSTRACT Although people with dementia have leisure travel needs, no scholars appear to have empirically explored how to meet this group’s needs as hotel guests. To bridge this research gap in tourism literature, the current study employed in-depth interviews to collect first-hand data from 15 travel-eligible tourists with early-stage dementia and 15 senior luxury hotel managers in China. Specifically, six key themes were extracted from Chinese hotel guests with dementia. Hotel managers’ commentary on meeting travel-eligible tourists with dementia’ needs during leisure travel led to three themes. Innovatively, this study further sheds light on possible contributions the tourism and hospitality industry makes to the global health field using the context of a vulnerable group – travel-eligible tourists in an ageing society.

Causal relationship between blood metabolites and risk of five infections: a Mendelian randomization study

ObjectiveInfectious diseases continue to pose a significant threat in the field of global public health, and our understanding of their metabolic pathogenesis remains limited. However, the advent of genome-wide association studies (GWAS) offers an unprecedented opportunity to unravel the relationship between metabolites and infections.MethodsUnivariable and multivariable Mendelian randomization (MR) was commandeered to elucidate the causal relationship between blood metabolism and five high-frequency infection phenotypes: sepsis, pneumonia, upper respiratory tract infections (URTI), urinary tract infections (UTI), and skin and subcutaneous tissue infection (SSTI). GWAS data for infections were derived from UK Biobank and the FinnGen consortium. The primary analysis was conducted using the inverse variance weighted method on the UK Biobank data, along with a series of sensitivity analyses. Subsequently, replication and meta-analysis were performed on the FinnGen consortium data.ResultsAfter primary analysis and a series of sensitivity analyses, 17 metabolites were identified from UK Biobank that have a causal relationship with five infections. Upon joint analysis with the FinGen cohort, 7 of these metabolites demonstrated consistent associations. Subsequently, we conducted a multivariable Mendelian randomization analysis to confirm the independent effects of these metabolites. Among known metabolites, genetically predicted 1-stearoylglycerol (1-SG) (odds ratio [OR] = 0.561, 95% confidence interval [CI]: 0.403–0.780, P < 0.001) and 3-carboxy-4-methyl-5-propyl-2-furanpropanoate (CMPF) (OR = 0.780, 95%CI: 0.689–0.883, P < 0.001) was causatively associated with a lower risk of sepsis, and genetically predicted phenylacetate (PA) (OR = 1.426, 95%CI: 1.152–1.765, P = 0.001) and cysteine (OR = 1.522, 95%CI: 1.170–1.980, P = 0.002) were associated with an increased risk of UTI. Ursodeoxycholate (UDCA) (OR = 0.906, 95%CI: 0.829–0.990, P = 0.029) is a protective factor against pneumonia. Two unknown metabolites, X-12407 (OR = 1.294, 95%CI: 1.131–1.481, P < 0.001), and X-12847 (OR = 1.344, 95%CI: 1.152–1.568, P < 0.001), were also identified as independent risk factors for sepsis.ConclusionsIn this MR study, we demonstrated a causal relationship between blood metabolites and the risk of developing sepsis, pneumonia, and UTI. However, there was no evidence of a causal connection between blood metabolites and the risk of URTI or SSTI, indicating a need for larger-scale studies to further investigate susceptibility to certain infection phenotypes.

Toward a new relationship between history and global mental health

This paper explores the relationship between historical research and the field of global mental health. It identifies a gap in the current literature, and argues that an in-depth historical approach is critical for understanding and overcoming current challenges and controversies in global mental health. The authors propose that a thick historical analysis has the capacity to broaden and diversify the discussion about the core concepts in global mental health (such as illness, suffering, care or culture), and to nuance our understanding of the field’s development and impact in specific political and social contexts. The paper analyzes how a systematic historical approach is crucial for understanding colonial and post-colonial power relations embedded in the field of global mental health, and encourages researchers and practitioners to view history as a source of imagination, and of alternative ideas and initiatives in mental health that go beyond existing psychiatric frames of representations, and towards truly radical and egalitarian projects and relations. This exercise in alternative historical imagination does not need to interfere with nor disrupt the urgency of mental health practice today; on the contrary, it is meant to improve the effectiveness of interventions. It can provide practitioners with a new and enriched language to resolve long-standing clinical dilemmas (e.g. related to patient adherence or limited success of certain cultural adaptations), which could not be properly addressed previously

Current status of and future perspectives on care for cancer survivors in China.

Cancer is currently a major public health issue faced by countries around the world. With the progress of medical science and technology, the survival rate of cancer patients has increased significantly and the survival time has been effectively prolonged. How to provide quality and efficient care for the increasingly large group of cancer survivors with limited medical resources will be a key concern in the field of global public health in the future. Compared to developed countries, China's theoretical research and practical experience in care for cancer survivors are relatively limited and cannot meet the multi-faceted and diverse care needs of cancer patients. Based on the existing models of care worldwide, the current work reviews care for cancer survivors in China, it proposes considerations and suggestions for the creation of models of cancer care with Chinese characteristics in terms of optimizing top-level system design, enhancing institutional mechanisms, accelerating human resource development, and enhancing self-management and social support for patients.
