Dysphagia caused by a fibrovascular polyp: a case report

A 73-year old man presented with dysphagia for liquid and solid food. Barium contrast study of the esophagus and esophagoscopy demonstrated a fibrovascular polyp. This, almost 10 cm benign esophageal tumor, was removed surgically by a cervical esophagotomy. A fibrovascular polyp is a rare benign tumor of the esophagus, which, however, may give serious complications as asphyxia resulting from laryngeal obstruction leading to sudden death.

Giant fibrovascular polyp of the oesophagus: a case report and review of the literature

IntroductionWe present a case of fibrovascular polyp, a rare submucosal tumour of the oesophagus that has been reported only sporadically in the literature. The biapproach for surgical removal of fibrovascular polyp has only been mentioned once in the literature.Case presentationA 65-year-old Greek man presented with a 9-month history of gradually progressive intermittent dysphagia. Radiologic work-up with oesophagogram and computed tomography revealed a large, sausage-shaped intraluminal polyp extending from the level of the cervical oesophagus to the level of the upper body of the stomach. The diagnosis of giant fibrovascular polyp was made radiographically and confirmed by endoscopic biopsy. The polyp was removed using a biapproach surgical technique: pharyngotomy and subsequent gastrostomy.ConclusionFibrovascular polyp is a rare submucosal tumour. Proper treatment depends on accurate assessment of the origin, size, and vascularity of the pedicle and the size of the tumour. Choice of the appropriate surgical approach depends on the correct diagnosis, which can usually be indicated radiographically by the presence of a smooth, sausage-shaped defect with a discrete bulbous tip.

RETRACTED ARTICLE: Peroral endoscopic removal: as a minimally invasive long-term surgical treatment of a regurgitated giant polisegmented fibrovascular polyp of the esophagus

Giant fibrovascular polyps (FVP) are relatively rare benign neoplasm of the upper part of the esophagus. Without previous history, their diagnosis might be difficult as the endoscopic findings are sometimes misinterpreted. The present report describes a case, in which the patient regurgitated his giant polypoid mass into his mouth and captured it between his teeth and buccal surface until the emergency endoscopic removal. Although the adequate therapy for these lesions is mainly the open surgical resection, most often via cervical esophagotomy, in our case the polyp was removed successfully by peroral endoscopic operation, as a minimally invasive surgical treatment. After 5 years of follow-up, the patient is doing well, without recurrence of his polyp.

Endoscopic Removal of a Giant Fibrovascular Polyp of the Esophagus

Giant fibrovascular polyps of the esophagus are rare benign tumors originating from the proximal esophagus. These pedunculated lesions can grow to "giant" proportions. Asphyxiation from aspiration of the regurgitated polyp is a well-described cause of death. Traditional excision has involved a transcervical vertical esophagotomy. This report describes the successful endoscopic removal of a giant fibrovascular polyp of the esophagus. A 63-year-old man with dwarfism and obstructive sleep apnea was referred for evaluation of an esophageal mass that was intermittently regurgitated into the hypopharynx. Office esophagoscopy demonstrated a 10-cm giant fibrovascular polyp originating just below the cricoid cartilage. During endoscopic removal, the base of the lesion was exposed with a Weerda bivalved laryngoscope. Bipolar cautery combined with a snare was used to transect the base with excellent hemostasis. No esophageal leak was noted on an esophagogram on postoperative day 3. The patient then resumed a liquid diet and was discharged home, resuming a regular diet within a week. Giant fibrovascular polyps of the esophagus are life-threatening because of potential airway obstruction. This report describes the successful endoscopic removal of a giant fibrovascular polyp, avoiding the potential morbidity associated with a transcervical vertical esophagotomy.

Fibrovascular polyp of the cervical esophagus

Fibrovascular polyps of the esophagus are rare. This report describes a 54-year-old man who presented with progressive dysphagia and was evaluated for an upper esophageal mass via a variety of modalities including computerized tomography (CT), esophagogastroduodenoscopy (EGD) with endoscopic ultrasound (EUS)–guided fine needle aspiration biopsy, and direct laryngoscopy with biopsies. The patient proved a diagnostic challenge, and definitive diagnosis was not established until open excisional biopsy with histopathologic examination. Before resection, CT evaluation most strongly suggested the possibility of fibrovascular polyp based on the tumor's intraluminal nature and cervical esophageal location. In this case, we illustrate the difficulty in establishing a diagnosis of fibrovascular polyp for a patient presenting with a proximal esophageal mass and nonspecific obstructive symptoms.

A case of endoscopically resected fibrovascular polyp arose from lower esophagus.

Fibrovascular polyps are very rare benign tumors almost originating from the cervical esophagus. We present the case of a 50-year-old man with a history of dizziness and dyspnea on effort for two weeks. The barium esophagogram showed a smooth and rounded polypoid tumor at EG junction. Endoscopy revealed the smooth and white polypoid tumor at EG junction, which arose from the lower esophagus, and the head of the tumor was herniated into the stomach. Because the potentially malignant tumor was suspected by endoscopic biopsy, and the tumor was small in size, we performed endoscopic resection. The final diagnosis was the fibrovascular polyp of the esophagus. This is a very rare case, because this polyp was not only small but also arose from lower esophagus.

Rhabdomyomatous well-differentiated liposarcoma arising in giant fibrovascular polyp of the esophagus

We report an example of an esophageal giant fibrovascular polyp harboring a well-differentiated liposarcoma with rhabdomyomatous differentiation in a 68 year-old man. Initial imaging studies including fiberoptic endoscopy and 2 computed tomographic examinations, and laparoscopic myotomy failed to demonstrate the polypoid intraluminal nature of the lesion. Thus, alternative diagnoses including a malignant neoplasm and achalasia were entertained. A third computed tomography with contrast showed a pedunculated mass. Surgical resection revealed a 15-cm smooth-surfaced club-shaped mass composed of a well-differentiated liposarcoma demonstrating myoglobin, muscle-specific actin, and myogenin-positive rhabdomyomatous differentiation. To date, no previously reported giant fibrovascular polyp case has described a liposarcoma with rhabdomyomatous change.

Fibrovascular polyp of the esophagus requiring esophagectomy

Fibrovascular polyps of the esophagus are rare, with only 110 cases reported in the world literature to date. Dysphagia is the most common symptom. The diagnosis is usually made by barium swallow or upper endoscopy, but almost a third of cases can be missed with these studies. Treatment is surgical. Only four cases in the literature underwent esophagectomy for removal. We present a female patient with a fibrovascular polyp of the esophagus who required a transhiatal esophagectomy to safely remove this mass.

Angiolipoma of hypopharynx: A diagnostic dilemma.

Fibro vascular polyps of esophagus are rare tumours. They usually arise close to cricopharyngeus muscle. The commonest symptom is dysphagia but regurgitation of fleshy mass into mouth, regurgitation of recently digested food and sensation of lump in throat are also reported. Several reports of polyp regurgitation and subsequent airway obstruction are mentioned. We present a 48-year-old male with fibrovascular polyp of hypopharynx. Diagnosis was established by barium swallow and oesophagoscopy.Transoral route was used to remove the polyp and histopathology reported as angiolipoma.

Giant Fibrovascular Polyp of the Esophagus Causing Sudden Dyspnea: Endoscopic Treatment

A 76-year-old man with sudden dyspnea was admitted to our department. A fleshy mass was visualized in the labial commissure, and the flexible esophagoscopy showed the polyp prolapsed into the oropharynx, partially obstructing the airway. The polyp was encircled with a large oval snare and Endoloop, which was adjusted to stalk-basis before tightening. Pure coagulation was used to transect the lesion, which was then retrieved. The polyp measured 10 x 2.5 cm. To our knowledge, our case is the first large esophageal giant fibrovascular polyps causing sudden dyspnea in the literature that has been resected endoscopically without complications.

Fibrovascular polyps of the esophagus are benign, pedunculated tumors that consist mostly of connective tissue and can reach impressive sizes. They arise from the upper third of the esophagus and may produce symptoms of dysphagia, progressive weight loss and regurgitation. The most serious clinical presentation is asphyxia secondary to laryngeal obstruction. Despite their size, diagnosis may be difficult. The location of the stalk and the vascularity makes surgical resection the preferred mode of removal of these unusual polyps.

Giant fibrovascular polyp of the esophagus: MRI is useful for diagnosis and surgical planning

In the surgical field, benign esophageal tumors are much more uncommon than esophageal malignancies. In particular, fibrovascular polyps of the esophagus are extremely rare. We present a case involving a giant fibrovascular polyp (22 cm × 9 cm) in which not only location and site of origin but also precise polyp type were diagnosed preoperatively using magnetic resonance imaging (MRI). The lesion was then resected safely using a cervical approach. MRI is useful for acquiring valuable information about fibrovascular polyps. This case demonstrates the utility and safety of transcervical esophagotomy in the treatment of giant fibrovascular polyp of the esophagus.

Giant oesophageal fibrovascular polyp (2005:12b)

The authors report a case of a giant fibrovascular polyp of the oesophagus in which CT with multiplanar reformatting gave valuable information as to both location of the lesion as well as size and anatomical attachment proximally, which was pivotal in directing surgery. It was instructive in this case that initial endoscopy was reported to be normal. The lesion was well demonstrated by barium swallow.

Giant Fibrovascular Polyp of the Hypopharynx: Surgical Treatment with the Biappoach

Fibrovascular polyps of the esophagus and hypopharynx are benign tumors of the upper digestive tract. The majority of these polyps are located in the upper part of the esophagus but the hypopharyngeal fibrovascular polyps are only rarely seen. Most of them are surgically treated and this is usually done through a cervical incision, although some of them have been removed endoscopically. The authors report here on a case of a 63-yr-old-man with a giant fibrovascular polyp of the hypopharynx that extended into the stomach; this polyp was removed through simultaneous transcervical and transabdominal approaches because of the huge size of the polyp. The man presented with progressive dysphagia of 1 yr duration. The preoperative assessment revealed a giant polyp arising from the left arytenoid and extending into the stomach. The dimension of the polyp was about 26×10×4 cm. The complete resection of the polyp with the simultaneous transcervical and transabdominal approaches was successful, and it was diagnosed as a fibrovascular polyp. The patient has been followed up without any recurrence for 6 month postoperatively.

Giant fibrovascular polyps are uncommon benign esophageal tumors almost always originating from the cervical esophagus, frequently from the upper esophageal sphincter. The case of a 74-year-old man with a long history of dysphagia and a weight loss of 9 kg is presented. Neither barium esophagogram, computed tomogram or magnetic resonance imaging correctly evidenced the lesion. Only fiberoptic endoscopy suggested the correct diagnosis because the mass fluctuated endoluminally with the spasm of vomiting. A left cervical exploratory incision with esophagotomy was performed following the experience of two previous similar cases. A giant fibrovascular polyp was observed and excised. If a malignant or benign extensive intramural tumor had been identified, a total esophagectomy would have been performed. In our opinion the possibility of the presence of a fibrovascular polyp should always be considered in the presence of an undetermined esophageal mass, and in these cases a left cervical incision is the preferred surgical access. Once the correct diagnosis is established, a major esophageal resection should always be avoided.
