A 27-Year-Old Woman With Incidental Omental Nodules

We present the case of a 27-year-old woman with an incidental finding of a lobulated, intra-abdominal, soft tissue mass, noted on a computed tomographic scan of the abdomen performed for an unrelated, self-limited process. An excisional biopsy showed multiple nodules composed of hyalinized fibrous tissue with multiple calcifications and an inflammatory infiltrate of lymphocytes and plasma cell consistent with calcifying fibrous pseudotumor of the omentum.

Fibromatous periorchitis of testis

Fibrous pseudotumors of the tunica vaginalis, epididymis, and spermatic cord are uncommon benign paratesticular masses. An extremely rare variant termed “fibromatous periorchitis” exhibits diffuse proliferative encasement of the testis and manifests grossly as an indurated testis reminiscent of malignancy. We present the case of a 25-year-old man who presented with a grossly abnormal testicular examination and benign scrotal ultrasound findings and pathologic evidence of fibromatous periorchitis. The natural history, gross and histologic findings, and management of fibrous pseudotumors and fibromatous periorchitis are discussed.

Imaging and diagnostic strategy of soft tissue tumors in children

The diagnosis of a soft tissue mass in children is a common clinical situation. Most of the lesions are benign and can be treated conservatively or by non-mutilating surgery. Nevertheless, the possibility of a malignant soft tissue tumor must be systematically considered. The most frequent benign soft tissue lesions in children are vascular lesions, fibrous and fibrohistiocytic tumors and pseudotumors, whereas rhabdomyosarcomas account for 50% of all soft tissue sarcomas. A child presenting an atypical soft tissue mass should be managed by a multidisciplinary centre, and primary resection must be proscribed until a definite diagnosis has been established. The role of imaging is essential either to confirm the benign nature of the mass or to give arguments to perform a diagnostic biopsy. Clinical examination, conventional radiography and ultrasound with Doppler represent the first-line examinations and are sometimes sufficient to assess a diagnosis. In all other situations, MRI is mandatory to establish the probable nature of the lesion and to assess local extension.

Calcifying Fibrous Pseudotumor of the Spermatic Cord

344 Calcifying fibrous pseudotumor (CFP) is an unusual benign lesion of the soft tissue. CFP is regarded as related to inflammatory pseudotumor and it may sometimes represent a late stage of inflammatory pseudotumor, in that a CFP appears to be arising within an inflammatory pseudotumor. 1 This entity occurs predominantly in young adult patients and has a good prognosis. 2 CFPs are found mostly in the extremities, neck, trunk, pleura, and mediastinum, but rarely in the spermatic cord. 3-5

症例は35歳,女性である.繰り返す右胸痛と胸水を主訴とし,経過観察中に右胸腔内腫瘤形成を認めた.胸腔鏡下生検で壁側胸膜および臓側胸膜に播種状に多発する腫瘍を認めた.生検標本にて胸膜発生多発性Calcifying Fibrous Pseudotumor(CFPT)と診断し,二期的に十分な切除断端の距離をとり完全切除した.術後6ヵ月後の現在再発を認めていない.胸膜発生多発性CFPTは非常に稀な疾患であるが,既報告例の多発発生例でも完全切除により再発を認めておらず,多発例でも完全切除を目指すべきと考える.

Fibrous Pseudotumor in the Testicular Tunica

Fibrous pseudotumor of the testicular tunics is uncommon lesion. They typically arise as painless scrotal masses that may be associated with a hydrocele or history of trauma or infection. Two-thirds involve the tunica vaginalis testis, with infrequent involvement of other scrotal structures. Once excised, these lesions behave in a benign fashion. Typically, these masses are multinodular, but in rare cases they are diffuse, band-like myofibroblastic proliferations that encase the testis. Calcifying Fibrous Pseudotumor of the Neck: Diagnostic Challenges of a Rare Benign Lesion

Neck masses, frequently encountered by physicians, comprise a vast range of diagnoses, with malignancy being the greatest concern. Calcifying fibrous pseudotumor (CFP) is a rare lesion with unknown pathogenesis, characterized pathologically by a predominance of abundant hyalinized collagenous tissue with focal lymphoplasmacytic infiltrate and psammomatous or dystrophic calcifications. We present the case of a 29-year-old woman who presented with a 4-cm left neck mass, accompanied by constitutional symptoms of vague weakness and lethargy. After the lesion failed to respond to a course of antibiotic therapy, fine-needle aspiration was performed, the pathology of which was indeterminate. The concern was that the lesion was a lymphoproliferative disorder-further workup was performed. CT of the chest, abdomen, and pelvis revealed no evidence of adenopathy or neoplasms. Subsequently, an incisional biopsy was performed, suggesting a diagnosis of CFP. Magnetic resonance imaging with contrast, performed to delineate the anatomy, revealed the lesion in the left neck, deep to the left clavicle, that extended superiorly into the supraclavicular fossa. Complete surgical removal of the lesion was successfully performed, with immunophenotyping confirming the initial diagnosis of CFP. We present a case report of cervical CFP, discuss the approach to neck masses, and review the recent literature on this rare, benign entity.

Bilateral elastofibroma dorsi: Role of MR imaging in diagnosis

Elastofibroma dorsi is slow growing, solid but ill defined, uncommon soft tissue lesion that appears in elderly individuals as a mass in the chest soft tissues, most commonly found in the periscapular region. The tumor is typically unilateral, although bilateral involvement has been reported. It is a benign chest wall lesion that is not well known to radiologists. This diagnosis is often missed prospectively. To our knowledge, there is a limited number of papers of CT and MRI findings of elastofibroma dorsi in the literature. If the radiologist is aware of this uncommon benign fibrous pseudotumor, radical or unnecessary surgery may possibly be prevent. CT and MRI appearances in a 56-year-old woman with bilateral elastofibroma dorsi are presented to alert physicians to this entity in this paper.

Reactive nodular fibrous pseudotumor involving the pelvic and abdominal cavity: a case report and review of literature

There are several entities that can present with multiple nodular lesions within the peritoneal cavity, such as "disseminated peritoneal leiomyomatosis," "peritoneal fibrosis," "calcifying fibrous pseudotumor," and the recently described lesion of "reactive nodular fibrous pseudotumor of the gastrointestinal tract and mesentery." Here we present one such lesion in a 28-year-old woman with a history of dysmenorrhea and ergotamine use for migraine attacks. Intraoperative exploration of our patient disclosed numerous nodules located throughout the pelvic and abdominal peritoneum. Histopathologically, these lesions were fibrocollagenous nodules composed of sparse wavy spindle cells within hyalinized "keloid-like" collagen surrounded by an inflammatory infiltrate. Some of the nodules were associated with small foci of endometriosis. We believe this lesion is best described by the term "reactive nodular fibrous pseudotumor" and that endometriosis and the use of ergotamine derivatives may be contributing factors.

Calcifying fibrous pseudotumor of the anterior parietal peritoneum: Treatment by laparoscopic resection*1

A 37-year-old man had a computed tomography scan of the abdomen performed as part of a workup for blunt torso trauma. A 3-cm calcified mass was incidentally noted in the left upper quadrant of the anterior abdominal wall juxtaposing the anterolateral body of the stomach (Fig 1). The patient underwent laparoscopic excision of the mass (Fig 2, A, B) and was discharged home the same day. At the 1-month follow-up, the patient was doing well and reported no complaints. Histopathologic analysis revealed a densely localized, well-demarcated collagenous nodule with irregular and psammomatous calcifications, scattered chronic inflammatory cells (Fig 2, C), and heterotopic neo-ossification (Fig 2, D) characteristic of a calcifying fibrous pseudotumor.

Calcifying fibrous pseudotumor causing thoracic outlet syndrome

Calcifying fibrous pseudotumor (CFP) is a relatively rare benign mass lesion that generally involves the soft tissues. We report a case of CFP occurring in an extremely rare location. The patient is a 34-year-old woman presenting symptoms of numbness of her right upper extremity caused by a CFP in her thoracic outlet and we discuss its clinical significance with a review of the literature.

A Pseudotumorous Nodular Lesion of the Subcutis in a Beagle Dog Comparable to Calcifying Fibrous Pseudotumor in Human

A subcutaneous mass was found in a 15-month-old male beagle dog. It grew to 6 cm in diameter, and then reduced gradually to 4 cm. Histopathologically, the mass was well circumscribed but uncapsulated and comprised of hypocellular hyalinized fibrosclerotic tissue and a variable inflammatory infiltrate, which was composed chiefly of the lymphocyte. The characteristic findings of this nodular lesion were remarkably similar to those of calcifying fibrous pseudotumor (CFP) in human, but lacking in calcification in this case. To establish the disease entity for this kind of fibrous pseudotumor in dogs, additional case reports are required.

Calcifying Fibrous Pseudotumor of the Pleura

Calcifying fibrous pseudotumor is an uncommon benign lesion that has unique histologic features. We report a case of calcifying fibrous pseudotumor of the pleura occurring in a 31-year-old woman. A computed tomographic scan revealed a pleural mass in the right anterior costophrenic angle. The excised mass was well circumscribed, nonencapsulated, solid, and firm. The tumor showed dense hyalinized collagenous tissue interspersed with spindle cells, psammomatous calcifications, and a predominantly lymphoplasmocytic infiltrate. Most spindle cells were diffusely positive for vimentin, focally positive for CD34, and negative for desmins, smooth muscle actin, S-100 protein, and anaplastic lymphoma kinase-1.

Calcifying fibrous pseudotumor of lung: A previously unreported entity

Calcifying fibrous pseudotumor (CFP) is one of the most unusual benign tumors of childhood and is located mostly in soft tissues, pleura, and peritoneum. The first case of pulmonary CFP in a 7-year-old boy is presented. The clinicopathologic features of this rare entity are discussed with special emphasis on histopathologic features and prognosis including a brief literature review. There are no presumptive clinical and laboratory findings, including tumor markers and imaging techniques, that distinguish CFP from other space-occupying lung lesions. The most important diagnostic aid is to bear this entity in mind when a child presents with an unexplained pulmonary mass. Total excision is mandatory for the appropriate diagnosis and treatment.

Reactive Nodular Fibrous Pseudotumors of the Gastrointestinal Tract: Report of 8 Cases

Eight cases of reactive nodular fibrous pseudotumor of the gastrointestinal tract are presented. The patients included 6 males and 2 females between the ages of 1 and 68 years (mean age 41.5 years). Three tumors involved the small intestine, and 5 of the investigated lesions were located in the large bowel. Of these, 2 originated in the sigmoid colon, 1 in the cecum, 1 in the appendix, and 1 in the large bowel not otherwise specified. The tumors' size varied from 3 to 10 cm in the greatest diameter (mean 6.2 cm). Histologically they were composed of stellate or spindle shaped cells resembling fibroblasts arranged haphazardly or in intersecting fascicles, embedded in a collagen-rich stroma, with sparse intralesional mononuclear cells frequently arranged in lymphoid aggregates. Immunohistochemically, the lesions were positive for vimentin (7/7), smooth muscle actin (8/8), muscle-specific actin (5/7), cytokeratins AE1/AE3 (6/7), and CAM 5.2 (1/7), and antigen CD68 (1/7). No case (0/8) reacted positively with antibody to CD117 (c-kit). Genetically no substitutions, deletions, or insertions occurred in exon 11 in all analyzed samples. Likewise, no deletions or insertions in part of exon 9 were observed. Ultrastructurally the tumor cells revealed features typical of myofibroblasts. According to the morphologic, immunohistochemical, and ultrastructural features mentioned above, especially to the positivity of low-molecular-weight cytokeratins, we propose this lesion to be related to a proliferation of multipotential subserosal cells rather than ordinary myofibroblasts or fibroblasts.

Calcifying fibrous pseudotumor of the pleura: a rare location

Calcifying fibrous pseudotumor is a rare lesion recently reported in the literature. We report a new case occurring in the pleura, which is an unusual location. The patient, a 38-year-old woman, had a left pleural mass revealed by chest roentgenograms. The tumor was resected by video-assisted thoracotomy. Only 26 cases have been reported in the literature to date, but only 4 cases have been reported in the pleura. The pathogenesis is discussed but remains uncertain. Tumor resection appears to be the best option and the clinical course is benign. However, the number of cases to date is insufficient to establish therapeutic recommendations.
