Inhibition of endothelial-to-mesenchymal transition in a large animal preclinical arteriovenous fistula model leads to improved remodelling and reduced stenosis.

Vein grafts are used for many indications, including bypass graft surgery and arteriovenous fistula (AVF) formation. However, patency following vein grafting or AVF formation is suboptimal for various reasons, including thrombosis, neointimal hyperplasia, and adverse remodelling. Recently, endothelial-to-mesenchymal transition (EndMT) was found to contribute to neointimal hyperplasia in mouse vein grafts. We aimed to evaluate the clinical potential of inhibiting EndMT and developed the first dedicated preclinical model to study the efficacy of local EndMT inhibition immediately prior to AVF creation. We first undertook pilot studies to optimize the creation of a femoral AVF in pigs and verify that EndMT contributes to neointimal formation. We then developed a method to achieve local in vivo SMAD3 knockdown by dwelling a lentiviral construct containing SMAD3 shRNA in the femoral vein prior to AVF creation. Next, in Phase 1, six pigs were randomized to SMAD3 knockdown or control lentivirus to evaluate the effectiveness of SMAD3 knockdown and EndMT inhibition 8 days after AVF creation. In Phase 2, 16 pigs were randomized to SMAD3 knockdown or control lentivirus and were evaluated to assess longer-term effects on AVF diameter, patency, and related measures at 30 days after AVF creation. In Phase 1, compared with controls, SMAD3 knockdown achieved a 75% reduction in the proportion of CD31+ endothelial cells co-expressing SMAD3 (P < 0.001) and also a significant reduction in the extent of EndMT (P < 0.05). In Phase 2, compared with controls, SMAD3 knockdown was associated with an increase in the minimum diameter of the venous limb of the AVF (1.56 ± 1.66 vs. 4.26 ± 1.71 mm, P < 0.01) and a reduced degree of stenosis (P < 0.01). Consistent with this, neointimal thickness was reduced in the SMAD3 knockdown group (0.88 ± 0.51 vs. 0.45 ± 0.19 mm, P < 0.05). Furthermore, endothelial integrity (the proportion of luminal cells expressing endothelial markers) was improved in the SMAD3 knockdown group (P < 0.05). EndMT inhibition in a preclinical AVF model by local SMAD3 knockdown using gene therapy led to reduced neointimal hyperplasia, increased endothelialization, and a reduction in the degree of AVF stenosis. This provides important proof of concept to pursue this approach as a clinical strategy to improve the patency of AVFs and other vein grafts.

Single-centre experience with an electrogram-based artificial intelligence software system to identify drivers in pulsed field ablation of persistent atrial fibrillation

Abstract Background Ablation of persistent atrial fibrillation (AF) beyond pulmonary vein isolation (PVI) remains challenging, despite increasing numbers of eligible patients. Previous studies suggest a possible role for non-PVI lesions targeting atrial drivers, but their identification can be difficult and subject to inter-operator variability. Purpose We investigated feasibility for an electrogram-based artificial intelligence software system to assist in the identification of left or right atrial dispersion sites as ablation targets for persistent AF using the penta-spline multi-electrode pulsed-field ablation (PFA) catheter, a recent technique which is more conventionally employed for PVI. Methods This is a single-centre retrospective descriptive study of a cohort of patients with persistent AF, who underwent their first-time ablation from May to November 2023 using the Farawave PFA catheter (Boston Scientific) and the VX1 system (Volta Medical). This system relies on off-line pre-training of multiparametric machine learning algorithms on a database of annotated AF intracardiac electrograms. It connects to cardiac electrophysiology recording systems via a custom data cable to receive analogue data and digitize them through an integrated converter. Its algorithms analyse signals in the cardiac chamber of interest to adjudicate the local presence or absence of dispersion and provides relevant visual cues to operators in order to target energy delivery. Results We identified 16 patients who underwent PFA for persistent AF (long-standing in 5/16, 31.3%), including PVI and further ablation guided by VX1. See table 1 for baseline population characteristics and table 2 for procedural characteristics. All underwent ablation with uninterrupted anticoagulation, general anaesthesia, use of the Rhythmia electroanatomic mapping system and its Orion mapping catheter (Boston Scientific) to feed signals into the VX1 system. VX1-guided electrogram localization and ablation mostly targeted the anterior region of the left atrium (16 segments), followed by its septal region (6 segments) and its lateral region (4 segments); the median number of applications per procedure was 88 [IQR 68-108], including a median of 76 [61-85] in the left atrium and 12 [0-15] applications in the right atrium. The median procedure duration was 123 [114-137] minutes; the median fluoroscopy time was 32 [26-40] minutes and the median radiation dose was 12.5 Gy∙cm2 [10.0-25.8]. Sinus rhythm was restored either during ablation or by electrical cardioversion (see Table 2). Two complications occurred one femoral arteriovenous fistula requiring stenting and one tamponade requiring drainage. At a three-month follow-up, 87.5% did not present AF recurrences. Conclusion Real-time detection and annotation of electrograms to track dispersion using AI-assisted technology to support PFA with a multipolar catheter is feasible. Safety and efficacy of anticoagulation therapy post atrial fibrillation ablation

Abstract Funding Acknowledgements None. Background Thromboembolic complications can be life-threatening during atrial fibrillation (AF) catheter ablation. The aim of our single center, retrospective study was to evaluate the safety and efficacy of continuous treatment using direct oral anticoagulants (DOACs) as an alternative to uninterrupted acenocoumarol for periprocedural anticoagulation. Purpose To record the safety and efficacy of the oral anticoagulants post atrial fibrillation ablation. Methods We studied 1999 patients undergoing AF catheter ablation between June 2013 and December 2021 in our center. All patients were assigned to take acenocoumarol (group 1, 194 patients) or DOACs (group 2, 1805 patients) for ≥2 months before the procedure. We compared thromboembolic and bleeding complications between the 2 groups. Results Our analysis showed no significant difference in major and minor complications between the 2 patient groups with the exception of the occurrence of groin hematoma with a clear superiority of DOACs (2.06%-0.5%, p&amp;lt;0.05). Specifically, 4 of 194 patients (2.06%) using uninterrupted acenocoumarol had a major complication (3 patients [1.55%] had transient ischemic attack resolved 8 hours later and 1 [0.52%] had pericardial tamponade, and 7 patients (3.61%) had minor complications (4 [2.06%] groin hematoma and 3 [1.55%] mild pericardial efussion). In group 2, 14 of 1805 patients (0.78%) had a major complication (11 patients had transient ischemic attack (0.61%) and 3 patients (0.17%) had pericardial tamponade). In the same group, 31 patients (1.72%) had a minor complication (3 patients [0.17%] presented with pseudoaneurysm or femoral arteriovenous fistula between the femoral artery and femoral vein, 19 [1.05%] with pericardial effusion &amp;lt;1 cm, and 9 [0.5%] groin hematoma). Conclusions DOACs and acenocoumarol have similar safety and effectiveness regarding thromboembolic complications prevention without increasing bleeding complications with the exception of the occurrence of groin hematoma with a clear superiority of DOACs.

Spontaneous femoral arterio-venous fistula in an 80-year-old female patient as an incidental finding

Spontaneous arteriovenous fistulas (AVF), in contrast to iatrogenic or post-traumatic ones, are extremely rare and only sporadically published in the literature. In the absence of exposure risk, the diagnosis of AVF can be challenging, especially if it is an incidental finding. An 80-year-old female patient presented to our vascular consultation because of swelling of the left leg due to varicosis. For years, she had also noticed that the right foot seemed to be cooler. Percutaneous catheter examinations via the groin had not been performed, and she could not remember any groin injuries. Truncal varicosis of the great saphenous vein confirmed clinically and sonographically. In addition, with peripheral pulses obtained, the right foot appeared slightly cooler but not discolored. On auscultation, a systolic-diastolic murmur accompanied by palpable buzzing was heard in the right groin. Color duplex sonography showed a coarse color mosaic pattern between the common femoral artery (AFC) and the anterior saphenous vein (VSAA) in the sense of aliasing ("confetti phenomenon"). A fistula channel between the AFC and VSAA could be visualized, in which very high systolic-diastolic flow velocities prevailed; pulsatile and turbulent flow was present in the region of the crosse-near femoral vein. Endovenous laser ablation was performed for symptomatic truncal varicosis of the left leg. Under ultrasound-guided compression of the afferent artery and fistula at the right groin, the fistula flow did not stop. The patient was reluctant to undergo a proposed interventional closure of the AVF. In follow-up over 4 years, no signs of cardiac insufficiency or critical limb ischemia developed. Spontaneous femoral AVF is a rarity. Characteristic clinical findings lead to a targeted use of color duplex sonography with correct interpretation of artifacts that can otherwise be easily missed.

Zero fluoroscopy catheter ablation of premature ventricular contractions: a multicenter experience.

Catheter ablation has become an established treatment option for premature ventricular complexes (PVCs). The use of fluoroscopy exposes patients and medical staff to potentially harmful stochastic and deterministic effects of ionizing radiations. We sought to analyze procedural outcomes in terms of safety and efficacy using a "zero fluoroscopy" approach for catheter ablation of PVCs. The present retrospective, multicenter, observational study included 131 patients having undergone catheter ablation of PVCs using "zero fluoroscopy" between 2019 and 2020 in four centers compared with another group who underwent the procedure with fluoroscopy. Median age was 51.0 ± 15.9 years old; males were 77 (58.8%). Among the study population, 26 (19.8%) had a cardiomyopathy. The most frequent PVC origin was right ventricular outflow tract (55%) followed by the left ventricle (16%), LVOT and cusps (13.7%), and aortomitral continuity (5.3%). Acute suppression of PVC was achieved in 127 patients (96.9%). At 12 months, a complete success was documented in 109 patients (83.2%), a reduction in PVC burden in 18 patients (13.7%), and a failure was recorded in four patients (3.1%). Only two minor complications occurred (femoral hematoma and arteriovenous fistula conservatively treated). The PVC ablation with a "zero" fluoroscopy approach appears to be a safe procedure with no major complications and good rates of success and recurrence in our multicenter experience.

Abstract 14956: Outcomes of Leadless Pacemaker Implantation Following Transvenous Lead Removal of Infected Cardiac Implantable Electronic Device: A Systematic Review

Background: The outcomes of leadless pacemaker (LP) implantation after transvenous lead removal (TLR) of infected cardiac implantable electronic devices (CIED) are not well-established. Objective: This study sought to describe the outcomes of LP implantation following TLR of infected CIED. Method: We conducted a literature search using PubMed and Embase for a combination of term including leadless pacemaker implantation, transvenous lead extraction, transvenous lead removal , transvenous lead explant , infected cardiac implantable electronic device . The inclusion criterion was LP implantation after TLR of infected CIED. The exclusion criterion was TLR for non-infectious reason. Study end-points were procedural complications and LP infection. Result: Of 132 publications reviewed, 13 studies with a total of 253 patients (74 ± 14 years of age, 69% males) were included. Amongst, 56.8% had systemic CIED infection and 43.2% had isolated pocket infection. Staphylococcus Aureus was the most common causative organism in 33% of the reported patients. Of 253 patients included, 105 patients (41.5%) underwent concomitant LP implantation during TLR procedure, and 36 patients (14.2%) had transvenous pacing as a bridge from TLR to LP implantation. During the LP implantation, 1 patient (0.4%) had an intra-procedural complication requiring sternotomy. Post-LP implantation, 2 patients (0.8%) developed groin hematoma, 2 patients (0.8%) developed femoral arteriovenous fistula, and 1 patient (0.4%) developed pericardial effusion requiring pericardiocentesis. During a mean follow-up of 11.3 ±10.6 months, 3 patients (1.2%) developed pacemaker syndrome, 1 patient (0.4%) developed acute on chronic heart failure exacerbation, and only 1 patient (0.4%) developed LP-related infection requiring LP retrieval. Conclusion: This study suggests that LP implant is feasible and safe post-removal of infected CIED with cumulative adverse events at 4% and a reinfection rate of 0.4%.

Percutaneous treatment of an iatrogenic femoral arteriovenous fistula from an accessory arterial branch: a case report and review of the literature.

Vascular access-related complications are an important consideration in patients undergoing cardiac catheterisation. Patients with CHD are increasingly undergoing percutaneous treatment for suitable procedures as an alternative, less invasive option to surgical intervention. As such, recognition and treatment of these complications are becoming increasingly important. We present a case of a patient with repaired Tetralogy of Fallot who developed a femoral arteriovenous fistula and femoral artery pseudoaneurysm arising from an accessory arterial branch following percutaneous Harmony valve implantation, both of which were treated endovascularly with placement of a stent.

A Systematic Review of Short-Term Outcomes of Leadless Pacemaker Implantation After Transvenous Lead Removal of Infected Cardiac Implantable Electronic Device

The inclusion criterion was LP implantation after TLR of infected CIED. The exclusion criterion was TLR for noninfectious reasons. Study end points included procedural complications and LP infection during follow-up. Of 132 publications reviewed, 13 studies with a total of 253 patients (74±14 years of age, 174 [69%] males) were included. The most common indication of the initial device implantations was a high-degree atrioventricular block (n=100 of 253, 39.5%). Of the 253 patients included, 105 patients (41.5%) underwent concomitant LP implantation during the TLR procedure, and 36 patients (14.2%) had temporary transvenous pacing as a bridge from TLR to LP implantation. Of the 148 patients with data on the type of CIED infection, 56.8% had systemic CIED infection and 43.2% had isolated pocket infection. Staphylococcus aureus was the most common causative organism in 33% of the reported patients. The LP was implanted an average of 5.4±10.7 days after TLR of infected CIED. During the LP implantation, 1 patient (0.4%) had unsuccessful implantation because of an intraprocedural complication requiring sternotomy. After LP implantation, 2 patients (0.8%) developed groin hematoma, 2 patients (0.8%) developed femoral arteriovenous fistula, and 1 patient (0.4%) developed pericardial effusion requiring pericardiocentesis. During a mean follow-up of 11.3±10.6 months, 3 patients (1.2%) developed pacemaker syndrome, 1 patient (0.4%) developed acute on chronic heart failure exacerbation, and only 1 patient (0.4%) developed LP-related infection requiring LP retrieval. Management strategy of femoral artery pseudoaneurysm combined with infectious wounds

Objective: To investigate the surgical treatment methods of femoral artery pseudoaneurysm combined with infectious wounds and to evaluate the clinical effects. Methods: The retrospective observational research method was used. Twelve patients with femoral artery pseudoaneurysm combined with infectious wounds who met the inclusion criteria were admitted to Nanjing University of Chinese Medicine Wuxi Integrated Traditional Chinese and Western Medicine Hospital (Affiliated Hospital of Jiangnan University) from October 2014 to September 2022, including 6 males and 6 females, aged from 46 to 78 years. In the primary operation, debridement, tumor resection, and artery suture/venous grafting to repair the artery/artery ligation were performed, and the wound area after tumor resection ranged from 4.0 cm×1.5 cm to 12.0 cm×6.5 cm. Wounds that could be sutured were treated with tension reduction suture and extracutaneous continuous vacuum sealing drainage (VSD), while large wounds that could not be sutured were treated with VSD to control infection. In the secondary operation, tension reduction suture was performed to repair the wounds that could be sutured; large wounds were repaired with adjacent translocated flaps with area of 9.0 cm×5.0 cm to 15.0 cm×7.0 cm. Additionally, when the length of the exposed femoral artery was equal to or over 3.0 cm, the wounds were repaired with additional rectus femoris muscle flap with length of 15.0 to 18.0 cm. The donor areas of the flaps were directly sutured. The wound with artery ligation was treated with stamp skin grafting and continuous VSD. The bacterial culture results of the wound exudate samples on admission were recorded. The intraoperative blood loss, the location of femoral artery rupture, the artery treatment method, and the wound repair method in the primary operation were recorded, and the durations of catheter lavage, catheter drainage, and VSD treatment, and the drainage volume after the operation were recorded. The repair method of wounds in the secondary operation, the durations of catheter drainage and VSD treatment, and the total drainage volume after the operation were recorded. The survivals of flap/muscle flap/stamp skin grafts were observed, and the wound healing time was recorded. Follow-up after discharge was performed to evaluate the quality of wound healing and the walking function and to check whether the pulsatile mass disappeared. B-ultrasound or computed tomography angiography (CTA) was performed again to observe potential pseudoaneurysm recurrence and evaluate the patency of blood flow of the femoral artery. Results: The bacterial culture results of wound exudate samples of all the patients were positive on admission. The blood loss was 150 to 750 mL in the primary operation. The arterial ruptures were located in the femoral artery in 8 cases, in the external iliac artery in 2 cases, and in the femoral arteriovenous fistula in 2 cases. Six cases received direct artery suture, 4 cases received autologous great saphenous vein grafting to repair the artery, 1 case received autologous great saphenous vein bypass surgery, and 1 case received artery ligation. The primary wound suture was performed in 4 cases, along with catheter lavage for 3 to 5 days, catheter drainage for 4 to 6 days, VSD treatment for 5 to 7 days, and a total drainage volume of 80 to 450 mL after the surgery. In the secondary operation, the wounds were sutured directly in 3 cases along with catheter drainage for 2 to 3 days, the wound was repaired with scalp stamp skin graft and VSD treatment for 5 days in 1 case, the wounds were repaired with adjacent translocated flaps in 2 cases with catheter drainage for 2 to 3 days, and the wounds were repaired with rectus femoris muscle flaps+adjacent translocated flaps in 2 cases with catheter drainage for 3 to 5 days . The total drainage volume after the secondary operation ranged from 150 to 400 mL. All the skin flaps/muscle flaps/skin grafts survived after operation. The wound healing time ranged from 15 to 36 days after the primary operation. Follow-up of 2 to 8 months after discharge showed that the wounds of all patients healed well. One patient who underwent femoral artery ligation had calf amputation due to foot ischemic necrosis, and the rest of the patients regained normal walking ability. The pulsatile mass disappeared in inguinal region of all patients. B-ultrasound or CTA re-examination in 6 patients showed that the blood flow of femoral artery had good patency, and there was no pseudoaneurysm recurrence. Conclusions: Early debridement, tumor resection, and individualized artery treatment should be performed in patients with femoral artery pseudoaneurysm combined with infected wounds. Besides, proper drainage and personalized repair strategy should be conducted according to the wound condition to achieve a good outcome.

Zero or minimal fluoroscopy catheter ablation: feasibility, efficacy, and safety results

Introduction Fluoroscopy has been used to guide catheter ablation since the beginning of this technique, despite its important secondary effects on both patients and operators. However, three-dimensional electroanatomic mapping systems (3D-EAMP) enable ablation of some substrates using zero or minimal fluoroscopy. The aim of this study is to explore the feasibility, efficacy and safety of a zero or minimal fluoroscopy approach in our center. Methods We performed a retrospective observational study with consecutive patients that underwent catheter ablation of atrioventricular nodal reentrant tachycardia (AVNRT), accessory pathways (AP), focal atrial tachycardia and typical atrial flutter in our center, between January 2020 and November 2022. Vascular access was obtained by ultrasound guidance. The objective was zero-fluoroscopy for all patients except those with left AP, in which minimal fluoroscopy was allowed for the transseptal access (ablations with retrograde aortic approach were excluded). Results 319 consecutive patients were included. 56% were male, mean age was 55 ± 16 years. The following substrates were included: AVNRT (43%), typical atrial flutter (41%), AP (12%) and focal atrial tachycardia (5%). We achieved zero-fluoroscopy use in 99.3% of right substrates, using fluoroscopy in just two cases to introduce catheters through femoral access. Mean fluoroscopy time in left AP was 6.1 minutes. Ablation was successful in 97% of cases, and we just experienced three complications (moderate pericardial effusion, femoral hematoma and femoral arteriovenous fistula, all of them solved without requiring intervention). After a mean follow- up of 17 ± 14 months, we noticed three recurrences, performing in all of them a successful second ablation procedure. Baseline characteristics and procedure details are shown in table 1. Conclusions Catheter ablation of simple substrates with zero or minimal fluoroscopy using 3D mapping systems is feasible, effective and safe, with high success rate and low complications rate.

Zero and near zero fluoroscopy catheter ablation of premature ventricular contractions: a multicentre experience

Methods The present retrospective, multicentre, observational study included 131 patients having undergone catheter ablation of PVCs using zero or "near zero" (less than 2 minutes) fluoroscopy between 2016 and 2020. Baseline characteristics of the population, acute success and complications derived from the procedure were evaluated, as well as recurrence rate during follow-up. Results Median age was 51.0 years old [38-63], males were 77 (58.8%). Most often cause of ablation were palpitations (90.0%), followed by reduced left ventricular ejection fraction (4.6%) and pre-syncope (14.5%). Among the study population, 26 (19.8%) had a cardiopathy. The median PVC burden before ablation was 15.1% [6%-22.4%]. The most frequent PVC origin was right ventricular outflow tract (n=72; 55.0%) followed by the left ventricle (n=21; 16.0%), LVOT and cusps (n=18; 13.7%) ,aortomitral continuity (n=7; 5.3%). A 3D electro-anatomical mapping system was used in all cases as well as a contact force catheter for mapping and ablation. The mean maximum radiofrequency power applied was 32.9 ± 4.1 W (370 s median time). Median number of applications was 4.5 and mean total procedural time was 117.3 ± 47.5 minutes. Median best prematurity was 31.5 ms. We used isoprenaline in order to induce PVCs in 42% of cases and suppression of PVC was achieved in 127 patients (96.9%). There was only 2 complication (femoral hematoma and arteriovenous fistula conservatively treated). At 12 months, a complete success was documented in 109 patients (83.2%), a reduction in PVC burden was achieved in other 18 patients (13.7%) and a failure was recorded in 4 patients (3.1%). Conclusion The PVC ablation with "zero" or "near-zero" fluoroscopy is a safe procedure with no major complications and good rates of success and recurrence in our multicentre experience.

Hybrid Ablation Versus Repeated Catheter Ablation in Persistent Atrial Fibrillation: A Randomized Controlled Trial

BackgroundAlthough catheter ablation (CA) is successful for the treatment of paroxysmal atrial fibrillation (AF), results are less satisfactory in persistent AF. Hybrid ablation (HA) results in better outcomes in patients with persistent atrial fibrillation (persAF), as it combines a thoracoscopic epicardial and transvenous endocardial approach in a single procedure. ObjectivesThe purpose of this study was to compare the effectiveness and safety of HA with CA in a prospective, superiority, unblinded, randomized controlled trial. MethodsForty-one ablation-naive patients with (long-standing)-persAF were randomized to HA (n = 19) or CA (n = 22) and received pulmonary vein isolation, posterior left atrial wall isolation and, if needed, a cavotricuspid isthmus ablation. The primary efficacy endpoint was freedom from any atrial tachyarrhythmia >5 minutes off antiarrhythmic drugs after 12 months. The primary and secondary safety endpoints included major and minor complications and the total number of serious adverse events. ResultsAfter 12 months, the freedom of atrial tachyarrhythmias off antiarrhythmic drugs was higher in the HA group compared with the CA group (89% vs 41%, P = 0.002). There was 1 pericarditis requiring pericardiocentesis and 1 femoral arteriovenous-fistula in the HA group. In the CA arm, 1 bleeding from the femoral artery occurred. There were no deaths, strokes, need for pacemaker implantation, or conversions to sternotomy, and the number of (serious) adverse events was comparable between groups (21% vs 14%, P = 0.685). ConclusionsHybrid AF ablation is an efficacious and safe procedure and results in better outcomes than catheter ablation for the treatment of patients with persistent AF. 756 ZERO OR NEAR ZERO FLUOROSCOPY CATHETER ABLATION OF PREMATURE VENTRICULAR CONTRACTIONS: A MULTICENTRE EXPERIENCE

Abstract Background Catheter ablation has become an established treatment option for premature ventricular complexes (PVCs). It is well known that the use of fluoroscopy exposes patients and medical staff to potentially harmful stochastic and deterministic effects of ionising radiations. We sought to analyse procedural outcomes in terms of safety and efficacy using a “zero” or “near zero” X rays approach for treatment of PVCs. Methods The present retrospective, multicentre, observational study included 131 patients having undergone catheter ablation of PVCs using zero or “near zero” (less than 2 minutes) fluoroscopy between 2016 and 2020 in 4 high-volume centres. Baseline characteristics of the population, acute success and complications derived from the procedure were evaluated, as well as recurrence rate during follow-up. Results Median age was 51.0 years old [38-63], males were 77 (58.8%). Most often cause of ablation were palpitations (90.0%), followed by reduced left ventricular ejection fraction (4.6%) and pre-syncope (14.5%). Among the study population, 26 (19.8%) had a cardiopathy. The median PVC burden before ablation was 15.1% [6%-22.4%]. The most frequent PVC origin was right ventricular outflow tract (n=72; 55.0%) followed by the left ventricle (n=21; 16.0%), LVOT and cusps (n=18; 13.7%),aortomitral continuity (n=7; 5.3%). A 3D electro-anatomical mapping system was used in all cases as well as a contact force catheter for mapping and ablation. The mean maximum radiofrequency power applied was 32.9 ± 4.1 W (median time: 370 sec). Median number of applications was 4.5 and mean total procedural time was 117.3 ± 47.5 minutes. Median best prematurity was 31.5 msec. We used isoprenaline in order to induce PVCs in 42% of cases and suppression of PVC was achieved in 127 patients (96.9%). There was only 2 complication (femoral hematoma and arteriovenous fistula conservatively treated). At 12 months, a complete success was documented in 109 patients (83.2%), a reduction in PVC burden was achieved in other 18 patients (13.7%) and a failure was recorded in 4 patients (3.1%). Conclusion The PVC ablation with “zero” or “near-zero” fluoroscopy is a safe procedure with no major complications and good rates of success and recurrence in our multicentre experience.

Surgical Approach of an Infected Saphenofemoral Looped PTFE Graft Arteriovenous Fistula and Arterial Reconstruction with Cryopreserved Allograft

Introduction: Autologous arteriovenous fistula is still the most popular vascular access for hemodialysis [1]. There have been few reports of saphenofemoral arteriovenous fistulas over the last 10 to 20 years because of suggestions of high morbidity, poor patencies and needling difficulties [2]. Some disadvantages of using this access are the necessity for a more extensive and deeper dissection using larger vessels and greater susceptibility to infection and steal syndrome [3, 4]. Attention should be paid to the patient's vasculature before surgery, through the "maturation" phase and chronic use [5]. Case report: A 72-year-old female with a history of arterial hypertension, diabetes mellitus and chronic renal failure requiring three-weekly hemodialysis, with a 4-day history of a painful and hot mass in the right thigh, fever, and hemoglobin decline. Physical examination: hematoma at the right inguinal region, presence of a tourniquet, femoral pulse 2/2 and not assessable distal pulses due to edema; coldness, and increased capillary refill. Aortoiliac and lower limb angiotomography showed: ruptured pseudoaneurysm of the right common femoral artery, ipsilateral superficial femoral artery injury and prosthetic femoral arteriovenous fistula infection. Open emergency approach through an infra-inguinal longitudinal incision was performed, draining a large hematoma and pus, finding an infected prosthetic PTFE graft from a saphenofemoral loop fistula with injury of the right superficial femoral artery and active bleeding at the site, plus dehiscence of saphenous anastomosis. Thrombectomy of the superficial and common femoral artery, en bloc arteriotomy of the right superficial femoral artery and arterial reconstruction with a cryopreserved arterial allograft with end-to-end anastomosis was performed. A Vacuum Assisted Therapy (VAC® ) system was created to cover the surgical site. Patient deceased from hypovolemic shock. Conclusions Unusual site AVFs (i.e., saphenofemoral loop graft AVFs) should be carefully followed-up to reduce the risk of complications and raise the awareness to diminish associated morbidity and mortality. An arterial cryopreserved allograft may be used for arterial reconstruction in the setting of an infected wound. The goal must be to spread more knowledge in this critical area of medicine that is importantly affecting medical costs of renal replacement therapies and patients' quality of life and survival rates.

Efficacy and safety of cryoballoon pulmonary vein isolation for paroxysmal and persistent atrial fibrillation: A comparison with radiofrequency ablation

BackgroundCryoballoon ablation was established as an effective and safe modality to achieve pulmonary vein isolation (PVI) in paroxysmal atrial fibrillation (PAF). However, its role in persistent atrial fibrillation (PersAF) remains unclear.ObjectiveThis study aimed to evaluate the efficacy and safety of cryoballoon PVI in PAF and PersAF comparing conventional radiofrequency catheter ablation (RFCA).MethodsTwo hundred patients undergoing cryoballoon ablation for symptomatic AF were consecutively enrolled in this retrospective study. For comparison, 210 patients undergoing RFCA in the same period were included. The primary outcome was a recurrence of any atrial tachyarrhythmias (ATas) after the index ablation. 12-lead ECG and 24-hour Holter monitoring were obtained at 1,3,6 and 9–12 months.ResultsPVI by cryoablation alone was achieved in 197 patients (98.5%). ATas-free survival at 12 months post-ablation was 72.7% in the cryoablation and 80.6% in the RFCA group (P = 0.123), respectively. The cryoablation showed comparable efficacy maintaining sinus rhythm compared with RFCA in PAF (P = 0.539), whereas in PersAF, ATas-free survival was significantly lower in cryoablation (P = 0.039). PV reconnection was observed in the majority of patients (14/16, 87.5%) who receive redo RFCA. Complications were encountered in 10 patients, including femoral arteriovenous fistula (n = 1), transient phrenic nerve palsy (n = 8), and minimal amount pericardial effusion (n = 1).ConclusionThe efficacy of cryoballoon PVI is comparable with conventional RFCA in PAF, whereas PVI alone using cryoballoon may not be insufficient to maintaining sinus rhythm in PersAF. The safety of cryoballlon PVI is tolerable.

Delayed Traumatic Superficial Femoral Artery Pseudoaneurysm and Arteriovenous Fistula Over the Left Lower Limb

A pseudoaneurysm is a pulsating, encapsulated hematoma in communication with a ruptured vessel (1); there is an accumulation of blood between the tunica media and tunica adventitia layers of the vessel. We report a case of a young male with complaints of swelling, skin discoloration, and a tingling sensation of the left lower limb. A peripheral angiography was performed, which revealed an arteriovenous fistula at the middle of the left superficial femoral artery. The patient underwent a left superficial femoral artery pseudoaneurysm excision and repair. In this case report, we discuss the presenting clinical features, methods of evaluation, and the management of the superficial femoral artery pseudoaneurysm accompanied with arteriovenous fistula.

Traumatic Superficial Femoral Arteriovenous Fistula with Pulsatile Mass and Leg Pain 60 Years after Stabbing Injury.

Post-traumatic arteriovenous fistula (AVF) is a vascular injury complication and can present with vessel dilation, forming pulsatile varices, venous hypertension, distal ischemia, and congestive heart failure. We present a case of only pulsatile mass and leg pain caused by a 60-year-old post-traumatic AVF. Computed tomography angiography showed an AVF between the superficial femoral artery and superficial femoral vein. Surgical repair with AVF ligation was successfully performed. Traumatic AVF caused vascular and heart failure in the future; therefore, post-traumatic AVF is better eliminated as soon as possible.

Cross-sectional observational study of patients submitted to percutaneous occlusion of patent foramen ovale: a 20-year retrospective analysis

Background Left-to-right shunting via the foramen ovale is a potential cause of paradoxical embolism. The presence of patent foramen ovale in more than 40% of patients with idiopathic ischemic stroke has led to deeper investigation of the impact of its closure on decreasing stroke recurrence rates. This study describes 20 years of experience with percutaneous patent foramen ovale occlusion performed by a single operator. Methods This sample comprised 527 patients with ischemic stroke submitted to percutaneous occlusion of patent foramen ovale associated to left-to-right shunting. Results The procedure was successful in all cases. The mean age was 48 years (range of 9 to 72 years), and 57% were male. Amplatzer® and Occlutech® prostheses were implanted in 295 and 232 patients (56% and 44%, respectively). There were no deaths, and complications were as follows: hematoma at the puncture site (three patients, 0.6%), femoral arteriovenous fistula with spontaneous resolution (two patients, 0.4%), cardiac tamponade resolved after puncture and drainage (two patients, 0.4%), transient supraventricular arrhythmia (22 patients, 4.2%), headache (27 patients, 5.1%), atrial fibrillation (two patients, 0.4%), large residual shunt requiring a second prosthesis (two patients, 0.4%), recurrence of ischemic stroke within 5 years (four patients, 0.8%) and hypersensitivity to nickel (one patient, 0.2%). Conclusion In this series, patent foramen ovale occlusion was a safe and effective alternative for prevention of recurrent ischemic stroke. Recurrence rates over the course of 5 years were low.

Staged Treatment of a Femoral Arteriovenous Fistula after Gunshot Injury

Thirty-four years after incurring a gunshot wound in the right thigh, a 69 year old man presented with severe heart failure caused by a femoral arteriovenous fistula (AVF). Computed tomography angiography revealed a 15 × 30 mm fistula (A, arrow) between the dilated femoral artery and vein, which was unsuitable for complete endovascular repair. An 18 mm vascular plug (LifeTech Scientific, Shenzhen, China) was deployed in the external iliac artery (asterisk) to reduce lood flow and improve the patient’s general condition. Ten days later, the defect was repaired with a linear shaped suture (B, arrow). Recovery was uneventful.Image 1

Leadless pacemaker implantation after lead extraction for cardiac implanted electronic device infection.

Cardiac implanted electronic device (CIED) pocket and systemic infection remain common complications with traditional CIEDs and are associated with high morbidity and mortality. Leadless pacemakers may be an attractive pacing alternative for many patients following complete hardware removal for a CIED infection by eliminating surgical pocket-related complications as well as lower risk of recurrent complications. To describe use and outcomes associated with leadless pacemaker implantation following extraction of a CIED system due to infection. Patient characteristics and postprocedural outcomes were described in patients who underwent leadless pacemaker implantation at Duke University Hospital between November 11, 2014 and November 18, 2019, following CIED infection and device extraction. Outcomes of interest included procedural complications, pacemaker syndrome, need for system revision, and recurrent infection. Among 39 patients, the mean age was 71 ± 17 years, 31% were women, and the most frequent primary pacing indication was complete heart block (64.1%) with 9 (23.1%) patients being pacemaker dependent at the time of Micra implantation. The primary organism implicated in the CIED infection was Staphylococcus aureus (43.6%). Nine of the 39 patients had a leadless pacemaker implanted before or on the same day as their extraction procedure, and the remaining 30 patients had a leadless pacemaker implanted after their extraction procedure. During follow-up (mean 24.8 ± 14.7 months) after leadless pacemaker implantation, there were a total of 3 major complications: 1 groin hematoma, 1 femoral arteriovenous fistula, and 1 case of pacemaker syndrome. No patients had evidence of recurrent CIED infection after leadless pacemaker implantation. Despite a prior CIED infection and an elevated risk of recurrent infection, there was no evidence of CIED infection with a mean follow up of over 2 years following leadless pacemaker implantation at or after CIED system removal. Larger studies with longer follow-up are required to determine if there is a long-term advantage to implanting a leadless pacemaker versus a traditional pacemaker following temporary pacing when needed during the periextraction period in patients with a prior CIED infection.
