Paraneoplastic cerebellar degeneration with fallopian tube adenocarcinoma

Background. Paraneoplastic cerebellar degeneration (PCD) is rarely caused by fallopian tube adenocarcinoma. Case. We present a patient with PCD and fallopian tube cancer. Anti-Yo antibody, one of an anti-neuronal antibody, was positive in serum and cerebrospinal fluid. She was also positive for HLA A24, which is common in patients with PCD. Radical surgery did not significantly ameliorate her neurological impairment, although the dysarthria improved slightly. Conclusion. This case highlights the importance of detecting the underlying malignancy in patients with subacute neurological impairment and shows that fallopian tube cancer can potentially cause PCD.

Cerebral metastasis from a primary adenocarcinoma of the fallopian tube

BackgroundPrimary adenocarcinoma of the fallopian tube is a rare disease, and cerebral metastasis from this tumor is an extremely rare event. CaseA 61-year-old female, who had been thought to be in a disease-free state after the initial treatment for stage IIIc fallopian tube adenocarcinoma, presented with severe headache, dizziness, nausea, and vomiting. Brain CT and MRI revealed solitary metastatic lesion in the cerebrum, which was the only site of recurrence. It was removed surgically, followed by platinum-based combination chemotherapy, and a clinical remission was achieved for 36 months. ConclusionAttention should be paid to the possibility of cerebral metastasis in patients who develop neurogenic signs and symptoms. Prolonged survival could be achieved with aggressive treatment containing surgical debulking and adjuvant chemotherapy for solitary cerebral metastasis.

Superior vena cava syndrome during chemotherapy for stage 3c fallopian tube adenocarcinoma

Background. Superior vena cava syndrome is most often encountered in patients with malignancies. The diagnosis constitutes an oncologic emergency with prompt treatment indicated to manage the acute symptoms. There are few reports describing the syndrome in patients with gynecologic malignancies and central venous catheters. Management has included treatment of the metastatic disease and anticoagulation/thrombolysis with catheter removal early in therapy. Case report. The case described is the first report of a patient with fallopian tube carcinoma complicated by SVC syndrome. The complication was attributed to an implanted venous access port being utilized to give adjuvant combination chemotherapy. Conclusion. Superior vena cava syndrome is rarely encountered in gynecologic oncology patients and constitutes a medical emergency. When encountered in the setting of an implanted catheter, thrombolysis and anticoagulation is an alternative to catheter removal in selected patients.

Cough in a Patient With an Infusion Port

In this case report, we describe a patient with a history of fallopian tube adenocarcinoma who had an infusion port in place for the past 4 years. During the course of her stay in the hospital for pneumonia, she developed a cough that became worse with the infusion of fluids through the port. A portogram done to investigate this problem revealed the presence of a portobronchial fistula. This is the first reported case of a portobronchial fistula. Various possibilities to explain the formation of portobronchial fistula are discussed.

Inguinal Lymph Node Metastasis: An Unusual Presentation of Fallopian Tube Carcinoma

Objectives. Inguinal metastasis is a hitherto unreported presenting feature of fallopian tube adenocarcinoma.Case. We describe a case of a 69-year-old patient whose first manifestation of fallopian tube adenocarcinoma was an enlarged inguinal lymph node. This was excised and confirmed to be metastatic adenocarcinoma. She was investigated by diagnostic laparoscopy and subsequently underwent laparotomy with total abdominal hysterectomy, omentectomy, and pelvic and para-aortic lymph node dissection. All but two of the lymph nodes extirpated were negative.Conclusions. Fallopian tube adenocarcinoma may rarely present with metastatic inguinal lymphadenopathy.

Abstract. Levite R, Fishman A, Kesler A, Altaras M, Gadoth N. Paraneoplastic cerebellar degeneration heralding fallopian tube adenocarcinoma. The objective of this paper is to describe an 81-year-old woman with subacute cerebellar degeneration due to fallopian tube adenocarcinoma. Serum anti-Yo antibodies were used to screen for pelvic malignancy. Their presence led to a meticulous search, which included bilateral salpingoophorectomy. Subsequently an occult fallopian tube adenocarcinoma was discovered. This case report highlights the diagnostic value of antineuroneal antibodies in females with subacute neurologic impairment in the form of paraneoplastic syndrome.

Metachronous Carcinoma of the Vulva and Fallopian Tube

Background. Metachronous carcinoma of the vulva and fallopian tube is an unusual co-occurrence of gynecological malignancies. A report of such a case that developed and recurred over a 7-year period is presented.Case. A 53-year-old G3P3 female presented with a verrucous carcinoma of the vulva and a serous papillary adenocarcinoma of the left fallopian tube metachronously. To investigate a possible association between the co-occurrence of the rare neoplasms and factors associated with multiple gynecological malignancies, we analyzed the status of human papillomavirus infection and DNA mismatch repair deficiency as indicated by microsatellite instability. All samples analyzed were negative for these factors.Conclusion. The present results support the possibility that metachronous carcinomas of the vulva and fallopian tube involve unknown etiological factors or arise independently.

Overexpression of p53 and HER-2/neu and c-myc in Primary Fallopian Tube Carcinoma

Primary fallopian tube carcinoma is a rare form of female cancer and the genetic basis of its carcinogenesis remains unclear. Eighteen cases of primary fallopian tube adenocarcinoma were studied. Immunohistochemical staining for p53, HER-2/neu and c-myc genes were performed. Overexpression of p53 was detected in 12 cases (67%), HER-2/neu in 16 cases (89%), and c-myc in 11 cases (61%). The potential relevance of overexpression of the three genes with clinicopathological features was examined, but no significant correlation was found. The high incidence of p53, HER-2/neu and c-myc overexpression in fallopian tube adenocarcinoma suggests these genes may play a role in its tumorigenesis.

Splenic Metastasis of a Fallopian Tube Adenocarcinoma

Clinical diagnostic and therapeutic characteristics of a case of a fallopian tube adenocarcinoma splenic metastasis are reported This case illustrates the diagnostic difficulties of this situation. These difficulties are both related to the poorly specific symptomatology and the low specificity of the current imagery. Percutaneous biopsy-aspiration guided by ultrasonography or CT-scan could be helpful. The treatment is surgery, eventually combined with adjuvant radiotherapy: it can allow a long survival time. Splenic metastasis should be systematically checked up in every tumoral context.

P53 mutation is associated with high S-phase fraction in primary fallopian tube adenocarcinoma

Fallopian tube carcinoma (FTC) is a rare but lethal gynaecological malignancy. Four out of seven FTCs were identified with three point missense mutations, one single base deletion and one silent point mutation in the p53 gene. Genital-type HPV sequences were not detected. The S-phase fraction of tumours with mutant and wild-type p53 was 25.74% (median) and 12.55% (median) respectively.

Fallopian Tube Cancer: Incidence and Role of Lymphatic Spread

Lymphatic spread pattern in 17 cases of adenocarcinoma of fallopian tube is reported. Median age of the patients was 48 years. All patients underwent surgical staging including total abdominal hysterectomy, bilateral salpingo-oopherectomy omentectomy, and appendectomy. Systematic pelvic and para-aortic lymphadenectomy was feasible in 15 cases. Majority of the patients (11 of 17 cases, 64%) had advanced disease and showed serous adenocarcinoma (83%). Lymph nodes were involved in 10 of 17 cases (59%). Node metastases rate increased significantly (P< 0.01) with intraperitoneal stage of disease and with grading. Interestingly, positive nodes were also found in 2 cases (33%) of 6 patients with disease still limited to fallopian tube. Overall, patients with negative nodes had a median survival of 76 months, compared with only 33 months if node metastases were found. In conclusion, combined pelvic and para-aortic lymphadenectomy seems to be necessary for staging and perhaps for prognosis of this disease.

Adenocarcinoma of the Bilateral Fallopian Tube Occurring after Tubal Sterilization

A case of bilateral tubal carcinoma occurring 22 years after tubal sterilization is presented. We believe that this is the first case report of bilateral tubal carcinoma that has arisen in ligated fallopian tubes. The possibility that tubal occlusion is one of the causative factors of tubal carcinoma is discussed.

Primary Fallopian Tube Adenocarcinoma: Clinical Complete Response after Salvage Treatment with High-Dose Paclitaxel

This Case Report describes a patient with primary fallopian tube cancer who had a late recurrence of pelvic disease following adjuvant chemotherapy for early-stage disease. Secondary cytoreductive surgery was suboptimal. The bulky residual disease was refractory to platinum-based therapy. Extention of the disease to the retroperitoneum resulted in obstructed venous return, ureteral obstruction, and renal infection. The latter necessitated a third laparotomy and a nephrectomy. Upon recovery from surgery and failure of platinum reinduction, the patient was treated with high-dose paclitaxel. The large pelvic mass regressed rapidly and completely. Current chemotherapy for fallopian tube cancer is briefly reviewed.

We report the first case of primary fallopian tube adenocarcinoma in situ in a patient who had received antiestrogen tamoxifen as adjuvant therapy for breast carcinoma. Clinical and in vivo animal model studies, referable to the possible estrogen agonist effect of tamoxifen on the female genital tract, are also reviewed.

Fallopian tube carcinoma: recent diagnostic approach by color Doppler imaging.

A case of primary adenocarcinoma of the Fallopian tube is presented. The diagnosis was made preoperatively by transvaginal color Doppler imaging. The tumor revealed areas of neovascularization with characteristic low impedance (resistance index, 0.35) and high blood flow velocity (peak systolic velocity, 16.7 cm/s).

Paraneoplastic Cerebellar Degeneration: Successful Early Detection and Treatment of Cancer through Characterization of the Anti-Purkinje Cell Antibody.

Paraneoplastic cerebellar degeneration (PCD) is thought to be caused by an autoantibody against both tumor and neuronal tissue. Such autoantibodies are most frequently detected in patients with gynecological or breast cancer, and are designated as anti-Yo. We report here a patient with PCD whose underlying cancer could not be detected despite extensive tumor survey. IgG in her serum and cerebrospinal fluid reacted with the cytoplasm of cerebellar Purkinje cells immunohistochemically. On immunoelectron microscopy, the endoplasmic reticulum and Golgi complex were stained. Her IgG bound to the 58 kD band on immunoblots of cerebellar proteins. A reaction was also observed with the recombinant proteins deduced from the complementary DNA clone encoding a neuronal cell antigen reported by Sakai et al (Ann Neurol 28: 692, 1990). Based on these results, successful early resection of fallopian tube adenocarcinoma was performed. It is crucially important to characterize these PCD related autoantibodies for the early treatment of underlying malignant tumors.

Posthysterectomy carcinoma of the fallopian tube presenting as vaginal adenocarcinoma: A case report

The clinical and pathological features of a case of adenocarcinoma of the fallopian tube with a unique presentation are described. The 68-year-old patient presented with vaginal bleeding 25 years after a vaginal hysterectomy. Pelvic examination revealed a 0.5-cm nodule of tumor involving the mucosa of the vaginal apex. At laparoscopy, the left fallopian tube was dilated and adherent to the vaginal vault. Pathological examination of the upper vaginectomy and bilateral salpingo-oophorectomy specimen revealed a primary papillary adenocarcinoma of the left fallopian tube that had invaded directly into the mucosa of the vaginal apex. Vaginal involvement, either at the time of presentation or subsequently in the course of the disease, is very rare in patients with carcinoma of the fallopian tube.
