Giant Extrarenal Retroperitoneal Angiomyolipoma: A Case Report and Literature Review

Extrarenal angiomyolipoma is an uncommon disease entity and is rare in the retroperitoneum. We report a case of giant extrarenal retroperitoneal angiomyolipoma mimicking liposarcoma. A 35-year-old woman presented with a history of increased abdominal circumference and urinary frequency. Abdominal computerized tomography demonstrated a 24 x 21 x 16 cm retroperitoneal fatty tumor displacing the right kidney to the left upper quadrant of the abdomen. At laparotomy, the tumor was widely excised and the right kidney was successfully preserved. Final histologic examination identified the tumor as an angiomyolipoma. At 4-month postoperative follow-up, no evidence of tumor recurrence was found and the right kidney was fully functional.

Parametrial angiomyolipoma with multicystic change

Background Angiomyolipoma is a distinctive benign neoplasm of the renal origin. Rare extrarenal instances of angiomyolipoma have been documented at various body sites. A case of extrarenal angiomyolipoma arising from the parametrial soft tissues is described in the present report. Case A woman, age 40 years, presented with a large abdominopelvic mass. CT scan study revealed a solid mass with interspersed cystic areas and suggested the tumor mass to be attached in the pelvic soft tissues of the broad ligament, partly surrounding the lower uterus. These findings were corroborated by the peroperative observations. Gross examination and the microscopic evaluation of extensively sampled tumor mass and the accompanying hysterectomy specimen showed no connection of the tumor mass to the uterine wall. The tumor possessed typical histology of the angiomyolipoma and a peculiar presence of the interspersed cystic areas. Such cystic areas were epithelial lined and had morphology consistent with the paramesonephric (mullerian) cystic remnants. Conclusion Angiomyolipoma arising from the parametrial soft tissues has not been described before in the English medical literature. The paramesonephric remnants are of common occurrence in the broad ligament soft tissues. It is highly probable that such remnants are entrapped in the tumor arising from parametrial soft tissues of the broad ligament; however, probability of the tumor’s origin from a paramesonephric remnant itself cannot be ruled out.

Angiomyolipomatous hamartoma of the tongue

Extrarenal angiomyolipoma is a rare subtype of benign lipomatous tumor composed of adipose tissue, blood vessels, and smooth muscle. We report a 60-year-old woman who developed an angiomyolipomatous tumor of the tongue and discuss the differential diagnosis along with a review of the literature. The term angiomyolipomatous hamartoma is proposed for the present lesion to distinguish it from classic oral angiomyolipoma.

The authors report a case of retroperitoneal extrarenal angiomyolipoma, an exceptional finding, and discuss the aetiological possibilities of such disease.

Extrarenal Angiomyolipomas of the Perinephric Space

We describe 2 cases of extrarenal angiomyolipoma of the perinephric space. All other cases of extrarenal angiomyolipoma of the retroperitoneum are reviewed and the clinical relevance of this unusual pathological entity is discussed.

Angiomyolipoma of the vagina

An angiomyolipoma arising in the vaginal apex and presenting as a pelvic mass is reported and compared with other reported cases of extrarenal angiomyolipoma. This case appears to be the second case of angiomyolipoma arising in the vaginal wall.

Spontaneous perinephric hemorrhage from extrarenal angiomyolipoma

We believe this is the first reported case of spontaneous perinephric hemorrhage arising from an extrarenal angiomyolipoma. To date there have been no cases reported of extrarenal angiomyolipoma associated with tuberous sclerosis. All extrarenal angiomyolipomas have been benign and treated with simple excision.

Extrarenal Angiomyolipoma: Diagnosis and Management

We review the symptoms, diagnosis and treatment of extrarenal and renal angiomyolipomas. The pathological anatomy is described and it is maintained that renal localization of angiomyolipoma is by far the most common. A case is reported of an extrarenal angiomyolipoma that was revealed by exploratory laparotomy. Treatment is always surgical, since it is difficult to establish the differential diagnosis preoperatively. Angiomyolipoma is a benign tumor and malignant degeneration has not been observed. However, there is a tendency toward recurrence if the entire tumor is not removed.
