A CASE OF LERIGHE SYNDROME WITH PUDENDAL ULCER

Leriche syndrome is a disease with a chronic course, and the chief complaints are said to be impotence and ischemic symptoms of the lower extremities. We, however, experienced a case of the disease in which a pudendal ulcer was formed due to acute exacerbation during the clinical course. A 69-year-old man was admitted to the hospital because of pudendal ulcer and pain at rest in the both inferior limbs. With translumber aortography and abdominal CT, he was diagnosed as Leriche syndrome of the high aortic occlusion (juxta-renal aortic occlusion), with the occlusion extending to the bilateral internal and external iliac arteries. Since pelvic arteriography confirmed the patency of the external pudendal artery, thromboendarterectomy of the abdominal aorta directly below the renal artery was conducted, followed by aorto-bilateral common femoral bypass graft using a Y-shaped graft at the abdominal aorta directly below the renal artery. Postoperative course was uneventful. The pudendal ulcer rapidly cured and the pain at rest in the inferior limbs also disappeared.

Uses of Sepa Flap in Reconstruction of Defects of Hand-A Review of 25 Cases

AbstractThe SEPA flap based on superficial External Pudendal Artery, raised from lower abdomen is used to cover defects of hand. A series of 25 cases is done, out of which 5 cases were dealt in the emergency whereas 20 cases were dealt in elective operations. Both unilateral and bilateral flaps were used. The results are encouraging but vary significantly upon the severity of injury.

Penile Reconstruction with De-Epithelized Superficial External Pudendal Artery Flap

Complete loss of skin, Buck's fascia and tunica albuginea of the corpus cavernosum occurred on both sides of the penile shaft after placement of a penile prosthesis for the treatment of Peyronie's disease. We describe this unusual complication and surgical reconstruction of the penis with a de-epithelized superficial external pudendal artery axial pattern flap (January 1988). A good result was obtained.

The Blood Supply to Preputial Island Flaps

AbstractThe prepuce is formed by a combination of folding and epithelial proliferation, and separates from the glans after it has developed a blood supply. The arterial input, through 4 branches from the external pudendal arteries, is terminal and after birth supplies the outer and inner preputial surfaces in succession. Similarly, the venous return arises from small veins running transversely in the prepuce that connect to larger subcutaneous veins along the dorsal aspect of the shaft. Because the 2 preputial surfaces have a single blood supply they must be treated as 1 unit. Unfolding the prepuce leaves the former inner segment with only a terminal blood supply. The pedicle containing the superficial blood supply must remain attached to the skin flap or it will be devascularized. However, since this circulation goes exclusively to the flap, the more proximal portion of the prepuce that was raised to form the pedicle becomes ischemic when used as ventral cover. A double-faced flap avoids this complication.

Giant Pseudoaneurysm of the External Pudendal Artery Associated with Homocystinuria: A Case Report

A case of a giant pseudoaneurysm of the external pudendal artery in a fifty-three-year old man with homocystinuria is presented and discussed. As this case study demonstrates, patients with homocystinuria are prone to develop pseudonaneurysms following minor trauma to diseased vessels. Prolonged clinical follow-up of homocystinuria patients is recommended to avoid potential serious complications associated with pseudoaneurysms.

The arterial supply of the male reproductive system in the hamster.

SummaryThe major arteries, the testicular, deferential, prostatic, internal pudendal and external pudendal arteries, supply the male genital organs of the hamster. The same arterial patterns are found in three strains, except that the external pudendal artery arises from the external iliac artery or internal iliac artery in the APG strain, but only from the external iliac artery in the CBN and ACN strains. The deferential and prostatic arteries form the arterial loop on the dorsal surface of the ventrolateral lobe of the prostate. This loop may play some importance parts in the functional interaction between the epididymis and the prostate. The caudal epididymal artery arises more frequently from the spermatic cordai portion of the testicular artery than from the cranial epididymal artery, and it never arises directly from the abdominal part of the testicular artery as reported in the rat, mouse and rabbit. The deferential artery may join with the cranial vesicular artery to form the common trunk (umbilical artery), but sometimes these arteries arise directly from the internal iliac artery.

The anatomical basis of the SEPA flap

The superficial external pudendal artery (SEPA) flap was described by Dias in 1984. This paper describes the anatomical foundation of the flap, based on dissections of 12 cadavers and clinical findings in patients undergoing surgery in the groin and lower abdomen. The artery was found to be adequate in calibre and length to sustain a skin flap, on either side of the midline, extending from the symphysis pubis to the umbilicus.

Local intra-arterial infusion of growth hormone into the mammary glands of sheep and goats: effects on milk yield and composition, plasma hormones and metabolites.

Lactating goats and sheep were fitted with catheters in the external pudendal arteries supplying both mammary glands. Saline was infused continuously into one artery whereas the other artery received continuous infusions, over successive 4-day periods, of either saline or growth hormone (GH)-doses increasing twofold between successive periods from 100 to 400 micrograms/day in goats and 400 to 3200 micrograms/day in sheep. Local infusion of GH at up to 1600 micrograms/day in sheep did not affect milk yield or composition nor peripheral plasma concentrations of GH, insulin, glucose, urea and non-esterified fatty acids (NEFA). Infusion of GH at 3200 micrograms/day in sheep increased peripheral plasma concentrations of GH, tended to increase milk yield and peripheral plasma NEFA but there were no changes in peripheral plasma insulin, glucose and urea. It is concluded that GH does not exert direct effects on the mammary glands of sheep and goats in situations where the hormone is administered over short periods.

Extrinsic arterial supply of the great saphenous vein: an anatomic study

The extrinsic arterial supply to the great saphenous vein was studied by dissection following latex injection, diaphanisation and contrast roentgenograms. Arterial supply arises from the external pudendal, superficial femoral, superior genicular and posterior tibial arteries. These branches ensure the irrigation of the vasa vasorum. The role they play in the outcome of bypass with the great saphenous vein is a further argument pleading in favor of the in situ technique.

The combined use of the superficial external pudendal artery flap with a flap of the anterior rectus sheath for the simultaneous cover of dorsal and volar defects on the hand

It is not easy to provide skin cover simultaneously to both the dorsal and the volar surface of an injured hand. A case is reported in which the dorsum of the hand was covered by a superficial external pudendal artery (SEPA) axial pattern flap (Dias, 1984) and the volar surface by a flap fashioned from the anterior rectus sheath, which in turn was covered with a split-skin graft. A good result was obtained.

Blood supply and innervation of the supermedial thigh flap employed in one-stage reconstruction of the scrotum and vulva--an anatomical study.

The supermedial thigh flap is an arterial and sensate flap. Anatomical studies reveal both large and small nutrient arteries. Its largest and most consistent artery is the superficial branch of the deep external pudendal artery, which constitutes the main circulation. The secondary blood supply to the base of the flaps is contributed by musculocutaneous perforators of the medial circumflex femoral artery. Subcutaneous branches of the superficial femoral artery, when present, are an additional source of arterial supply. Branches of the ilioinguinal nerve provide sensory innervation to the flaps.

Distribution of lymphatic vessels in mammary glands of ewes.

AbstractSix multiparous ewes, three of which were lactating and the remaining three in an advanced stage of mammary involution, were used to study the distribution of lymphatic vessels in the mammary gland. The lymphatic system was distended by ligation of the regional efferent lymphatic ducts and either reconstituted blood or Latex was used to fill the blood vascular system. After fixation, distribution of lymphatic vessels was studied macroscopically and microscopically.One to three mammary nodes were situated at the postero‐dorsal aspect of each gland. Entering and leaving the nodes were 8 to 12 major afferent and 2 to 4 efferent ducts. Four or five of the afferent ducts accompanied the external pudendal artery and vein and their radicles emerged from the deep parenchyma of the gland. The remaining afferent ducts emerged from the parenchyma of the gland independent of blood vessels and their radicles drained both superficial and deep parenchyma.Lymphatic vessels were found in the connective tissue between lobes, within lobes and between lobules. Lymphatic capillaries were observed in the connective tissue within lobules and also in areas adjacent to the alveolar epithelium. Lymphatic vessels in the connective tissue between lobes and the larger vessels were supplied with valves and their walls posessed an endothelial cell lining together with smooth muscle and connective tissue layers. Finer lymphatic vessels appeared to consist of only a simple endothelial cell lining and valves were not found.
