Deep-Learning for Rapid Estimation of the Out-of-Field Dose in External Beam Photon Radiation Therapy – A Proof of Concept

Background and purpose: The dose deposited outside of the treatment field during external photon beam radiotherapy treatment, also known as out-of-field dose, is the subject of extensive study as it may be associated with a higher risk of developing a second cancer, and could have deleterious effects on the immune system which compromise the efficiency of combined radio-immunotherapy treatments. Out-of-field dose estimation tools developed today in research, including Monte Carlo simulations and analytical methods, are not suited to the requirements of clinical implementation because of their lack of versatility and their cumbersome application. We propose a proof of concept based on deep learning for out-of-field dose map estimation that addresses the above limitations.Materials and methods: For this purpose, a 3D U-Net, considering as inputs the in-field dose, as computed by the treatment planning system, and the patient's anatomy, was trained to predict out-of-field dose maps. The cohort used for learning and performance evaluation included 3151 pediatric patients from the [XXXX] database, treated in 5 clinical centers, whose whole-body dose maps were previously estimated with an empirical analytical method. The test set, composed of 433 patients, was split into 5 subdatasets, each containing patients treated with devices unseen during the training phase. Root mean square deviation (RMSD) evaluated only on non-zero voxels located in the out-of-field areas was computed as performance metric.Results: RMSD of 0.28 and 0.41 cGy.Gy−1 were obtained for the training and validation datasets, respectively. Values of 0.27, 0.26, 0.28, 0.30 and 0.45 cGy.Gy−1 were achieved for the 6 MV linac, 16 MV linac, Alcyon cobalt irradiator, Mobiletron cobalt irradiator, and betatron devices test sets, respectively.Conclusion: This proof-of-concept approach using a convolutional neural network has demonstrated unprecedented generalizability for this task, although it remains limited, and brings us closer to an implementation compatible with clinical routine.

An initial study on common concrete, titanium dioxide and tungsten dioxide for shielding of a standard external photon beam radiotherapy room

High-energy ionizing radiations have several applications including the treatment of various types of cancer. Advanced techniques such as IMRT (Intensity Modulated Radiation Therapy) and VMAT (Volumetric Modulated Arc Therapy) using high-energy photon beams have been largely used for radiation therapy of cancer. An important issue in radiotherapy is the evaluation of the shielding of the walls of the facility room housing a linear accelerator that produces high-energy photon beams. In this study we performed an estimation of the thicknesses of a standard radiotherapy room considering three different materials: common concrete, titanium dioxide (TiO2) and tungsten dioxide (WO2). It is assumed that patients are treated with photon beams of energies varying from 4 to 30 MeV and considering two types of treatments, only conventional and conventional plus IMRT and VMAT treatment. Results have shown an overall growth of thicknesses with energy, workload, area and type of treatment. At 1,000 Gy/week and 30 MeV photon energy the primary barrier thickness using concrete as the shielding material showed to be substantially higher as compared to TiO2 (143%) and WO2 (1,360%). Results have also indicated an overall linear increasing of volumes with the area used to construct the facility room.

Feasibility study of a scintillation sheet-based detector for fluence monitoring during external photon beam radiotherapy

PurposeThis study evaluated the properties of a scintillation sheet-based dosimetry system for beam monitoring with high spatial resolution, including the effects of this system on the treatment beam. The dosimetric characteristics and feasibility of this system for clinical use were also evaluated. MethodsThe effects of the dosimetry system on the beam were evaluated by measuring the percentage depth doses, dose profiles, and transmission factors. Fifteen treatment plans were created, and the influence of the dosimetry system on these clinical treatment plans was evaluated. The performance of the system was assessed by determining signal linearity, dose rate dependence, and reproducibility. The feasibility of the system for clinical use was evaluated by comparing intensity distributions with reference intensity distributions verified by quality assurance. ResultsThe spatial resolution of the dosimetry system was found to be 0.43 mm/pixel when projected to the isocenter plane. The dosimetry system attenuated the intensity of 6 MV beams by about 1.1%, without affecting the percentage depth doses and dose profiles. The response of the dosimetry system was linear, independent of the dose rate used in the clinic, and reproducible. Comparison of intensity distributions of evaluation treatment fields with reference intensity distributions showed that the 1%/1 mm average gamma passing rate was 99.6%. ConclusionsThe dosimetry system did not significantly alter the beam characteristics, indicating that the system could be implemented by using only a transmission factor. The dosimetry system is clinically suitable for monitoring treatment beam delivery with higher spatial resolution than other transmission detectors.

Analytical models for external photon beam radiotherapy out-of-field dose calculation: a scoping review.

A growing body of scientific evidence indicates that exposure to low dose ionizing radiation (< 2 Gy) is associated with a higher risk of developing radio-induced cancer. Additionally, it has been shown to have significant impacts on both innate and adaptive immune responses. As a result, the evaluation of the low doses inevitably delivered outside the treatment fields (out-of-field dose) in photon radiotherapy is a topic that is regaining interest at a pivotal moment in radiotherapy. In this work, we proposed a scoping review in order to identify evidence of strengths and limitations of available analytical models for out-of-field dose calculation in external photon beam radiotherapy for the purpose of implementation in clinical routine. Papers published between 1988 and 2022 proposing a novel analytical model that estimated at least one component of the out-of-field dose for photon external radiotherapy were included. Models focusing on electrons, protons and Monte-Carlo methods were excluded. The methodological quality and potential limitations of each model were analyzed to assess their generalizability. Twenty-one published papers were selected for analysis, of which 14 proposed multi-compartment models, demonstrating that research efforts are directed towards an increasingly detailed description of the underlying physical phenomena. Our synthesis revealed great inhomogeneities in practices, in particular in the acquisition of experimental data and the standardization of measurements, in the choice of metrics used for the evaluation of model performance and even in the definition of regions considered out-of-the-field, which makes quantitative comparisons impossible. We therefore propose to clarify some key concepts. The analytical methods do not seem to be easily suitable for massive use in clinical routine, due to the inevitable cumbersome nature of their implementation. Currently, there is no consensus on a mathematical formalism that comprehensively describes the out-of-field dose in external photon radiotherapy, partly due to the complex interactions between a large number of influencing factors. Out-of-field dose calculation models based on neural networks could be promising tools to overcome these limitations and thus favor a transfer to the clinic, but the lack of sufficiently large and heterogeneous data sets is the main obstacle.

On the sensitivity of PROMs during breast radiotherapy

PurposeTo investigate the sensitivity of patient-reported outcome measures (PROMs) to detect treatment-related side effects in patients with breast cancer undergoing external beam photon radiotherapy. MethodsAs part of daily clinical care, an in-house developed PROM tool was used to assess side effects in patients during a) whole-breast irradiation (WBI) to 40 Gy, b) WBI with a sequential boost of 10 Gy, and c) partial-breast irradiation (PBI) to 40 Gy. Results414 patients participated in this prospective study between October 2020 and January 2022, with 128 patients (31 %) receiving WBI, 241 (58 %) receiving WBI followed by a sequential boost, and 50 patients (12 %) receiving PBI. Significant differences in the reported toxicities (itching, radiation skin reaction, skin darkening, and tenderness and swelling) were reported between the WBI cohorts with and without boost (p < 0.001, p < 0.001, p < 0.001, and p = 0.002, respectively). The comparison of PBI with WBI (no-boost) yielded significant differences for radiation skin reaction (p < 0.001). ConclusionThe results highlight the high sensitivity of PROMs to detect treatment-related side effects in patients with breast cancer. Thus, PROMs may be a valuable tool for quality control and may support evidence-based learning from real-world data originating from daily routine care.

Long-term follow-up and health-related quality of life among cancer survivors with stage IEA orbital-type lymphoma after external photon-beam radiotherapy: Results from a longitudinal study.

We assessed the long-term outcomes and treatment-related adverse effects of patients with Stage I, "orbital-type" lymphomas that were uniformly treated with photons. All consecutive patients diagnosed with low-grade, Ann Arbor Stage IEA orbital lymphoma treated between 1999 and 2020 at our department were retrospectively reviewed. We excluded patients with exclusive conjunctival involvement, typically treated with en face electrons. In order to quantify radiotherapy related side effects we applied the CTCAE criteria, analyzed changes in visual acuity, quantified dry eye symptoms by use of the Ocular Surface Disease Index (OSDI) score and applied the EORTC QLQ-C30 questionnaire for quality of life (QoL) assessment. In total 66 eyes of 62 patients were irradiated with a median dose of 30.6Gy. The median follow-up was 43.5months. The predominant histological subtype were MALT lymphomas. No local failure occurred in this cohort. Of nine outfield relapses, six solely occurred in the contralateral eye. The 5- and 10- years distant progression free survival rates (PFS) were 81.4% and 63.5%. The 5- and 10-years overall survival rates were 85.1% and 71.9% without any tumor related death. Of the acute toxicities none was higher than CTCAE grade 1. The predominant late toxicities were dry eyes (21.2%) of CTCAE Grade <2 and radiation induced cataracts (19.7%). During long-term follow up the average visual acuity did not deteriorate. The global QoL was worst before treatment and improved significantly after 24months (p=0.007). External beam radiotherapy of "orbital-type" lymphomas with photons is an effective and gentle treatment option with excellent local control rates. From the high control rates the trend to use slightly lower total doses of 24-27Gy with conventional fractionation is supported. As non-coplanar radiotherapy techniques improved and total doses can slightly be reduced, the current status of radiotherapy as first line therapy is provided.

Quantification of Nanoscale Dose Enhancement in Gold Nanoparticle-Aided External Photon Beam Radiotherapy.

Simple SummaryGold nanoparticles (AuNPs) have become common in radiation oncology research through the past decades. Their radiosensitization effect could offer a novel approach to cancer diagnosis and radiotherapy as well. The aim of this study is the assessment of dose enhancement attributed to the incubation of AuNPs in an irradiated target. The present work is focused on investigating the impact of AuNPs properties in dose increase under different irradiation conditions using 6 MV photon beams with and without a flattening filter via Monte Carlo simulations. Results from the simulated scenarios depict a sufficient dose raise especially at close distances to AuNPs, depending on the presence of a flattening filter in the path of the photon beam, AuNPs size, the type of modeled distribution and their concentration. Therefore, the obtained enhanced dose deposition due to AuNPs presence in an irradiated region could lead to more sufficient tumor cell destruction than irradiation alone.The recent progress in Nanotechnology has introduced Gold Nanoparticles (AuNPs) as promising radiosensitizing agents in radiation oncology. This work aims to estimate dose enhancement due to the presence of AuNPs inside an irradiated water region through Monte Carlo calculations. The GATE platform was used to simulate 6 MV photon histories generated from a TrueBeam® linear accelerator with and without a Flattening Filter (FF) and model AuNPs clusters. The AuNPs size, concentration and distribution pattern were examined. To investigate different clinical irradiation conditions, the effect of field size, presence of FF and placement of AuNPs in water were evaluated. The range of Dose Enhancement Factors (DEF = DoseAu/DoseWater) calculated in this study is 0.99 ± 0.01–1.26 ± 0.02 depending on photon beam quality, distance from AuNPs surface, AuNPs size and concentration and pattern of distribution. The highest DEF is reported for irradiation using un-flattened photon beams and at close distances from AuNPs. The obtained findings suggest that dose deposition could be increased in regions that represent whole cells or subcellular targets (mitochondria, cell nucleus, etc.). Nevertheless, further and consistent research is needed in order to make a step toward AuNP-aided radiotherapy in clinical practice.

Feasibility study of radiophotoluminescent glass dosimeter for in vivo dosimetry in external photon beam radiotherapy

The purpose of this study was to check the applicability of radiophotoluminescent glass dosimeter (RPLGD) for in vivo dosimetry in external photon beams radiotherapy. The entrance dose measurements were performed. The influence of beam set-up parameters on the entrance dose was investigated using the RPLGDs. In this study, the RPLGD and 6-MV photon beam produced by a linear accelerator were used. The entrance dose calibration factors of the RPLGDs and beam energy and geometry-dependent correction factors were measured using a water phantom. For a correction factor related to the irradiation set-up, Monte Carlo (MC) simulation was performed. The entrance dose verification was performed on an Alderson Rando phantom during 64 single beam irradiations using different beam set-up. One RPLGD was attached on surface during a single beam irradiation for delivery of clinically planned dose. Additionally, during 50 irradiations, one RPLGD was placed directly inside the Alderson Rando phantom at different points in the beam path to verify the dose delivery. The mean ratio of the measured entrance dose using RPLGD on the surface of the phantom to the expected dose which was calculated by treatment planning system was equal to 1.001 ± 0.030. The mean ratio of the dose measured using RPLGDs inside the Alderson Rando phantom to the expected dose was equal to 1.019 ± 0.019. This study concludes that the RPLGD is suitable for in vivo dosimetry in external photon beam radiotherapy for dose verification within ± 5% at the clinically acceptable level.

The Potentiation of Anti-Tumor Immunity by Tumor Abolition with Alpha Particles, Protons, or Carbon Ion Radiation and Its Enforcement by Combination with Immunoadjuvants or Inhibitors of Immune Suppressor Cells and Checkpoint Molecules.

The delivery of radiation therapy (RT) for cancer with intent to cure has been optimized and standardized over the last 80 years. Both preclinical and clinical work emphasized the observation that radiation destroys the tumor and exposes its components to the immune response in a mode that facilitates the induction of anti-tumor immunity or reinforces such a response. External beam photon radiation is the most prevalent in situ abolition treatment, and its use exposed the “abscopal effect”. Particle radiotherapy (PRT), which has been in various stages of research and development for 70 years, is today available for the treatment of patients in the form of alpha particles, proton, or carbon ion radiotherapy. Charged particle radiotherapy is based on the acceleration of charged species, such as protons or carbon-12, which deposit their energy in the treated tumor and have a higher relative biological effectiveness compared with photon radiation. In this review, we will bring evidence that alpha particles, proton, or carbon ion radiation can destroy tumors and activate specific anti-tumor immune responses. Radiation may also directly affect the distribution and function of immune cells such as T cells, regulatory T cells, and mononuclear phagocytes. Tumor abolition by radiation can trigger an immune response against the tumor. However, abolition alone rarely induces effective anti-tumor immunity resulting in systemic tumor rejection. Immunotherapy can complement abolition to reinforce the anti-tumor immunity to better eradicate residual local and metastatic tumor cells. Various methods and agents such as immunoadjuvants, suppressor cell inhibitors, or checkpoint inhibitors were used to manipulate the immune response in combination with radiation. This review deals with the manifestations of particle-mediated radiotherapy and its correlation with immunotherapy of cancer.

Radiotherapy of Conjunctival Melanoma: Role and Challenges of Brachytherapy, Photon-Beam and Protontherapy

Conjunctival melanoma is a rare neoplasia, whose therapeutic management is generally of ophthalmological relevance, through radical surgical resection. The high incidence of local relapses after surgery, has made it necessary to combine various types of adjuvant treatments, which in some cases take on the role of radical treatments. Among these non-surgical treatments, those involving the application of ionizing radiation are becoming particularly important. In this review, we discuss the role of episcleral brachytherapy, external photon-beam radiotherapy, also made through stereotactic or radiosurgical modality, and of proton-beam radiotherapy. We try also to take stock of the benefits of the different irradiation modalities and the application difficulties of each.

In vivo dosimetry in external beam photon radiotherapy: Requirements and future directions for research, development, and clinical practice.

External beam radiotherapy with photon beams is a highly accurate treatment modality, but requires extensive quality assurance programs to confirm that radiation therapy will be or was administered appropriately. In vivo dosimetry (IVD) is an essential element of modern radiation therapy because it provides the ability to catch treatment delivery errors, assist in treatment adaptation, and record the actual dose delivered to the patient. However, for various reasons, its clinical implementation has been slow and limited. The purpose of this report is to stimulate the wider use of IVD for external beam radiotherapy, and in particular of systems using electronic portal imaging devices (EPIDs). After documenting the current IVD methods, this report provides detailed software, hardware and system requirements for in vivo EPID dosimetry systems in order to help in bridging the current vendor-user gap. The report also outlines directions for further development and research. In vivo EPID dosimetry vendors, in collaboration with users across multiple institutions, are requested to improve the understanding and reduce the uncertainties of the system and to help in the determination of optimal action limits for error detection. Finally, the report recommends that automation of all aspects of IVD is needed to help facilitate clinical adoption, including automation of image acquisition, analysis, result interpretation, and reporting/documentation. With the guidance of this report, it is hoped that widespread clinical use of IVD will be significantly accelerated.

Occurrence and mechanism of visual phosphenes in external photon beam radiation therapy and how to influence them

Background and purposeTwo plausible mechanisms to explain the appearance of visual phosphenes are: direct activation of the photochemicals in the retina and the generation of Cherenkov radiation in the vitreous humour. In this clinical trial we investigated the occurrence of visual phosphenes in external photon beam radiation therapy. Material and methodsLogistic regression analysis is used to examine whether seeing light flashes and seeing steady light depended on the ambient light intensity and the dose. ResultsIn total, 465 treatments of 25 patients were analysed. The odds of seeing light flashes multiply by 0,926 as the ambient light intensity increases by 10 lux. Similarly, the odds multiply by 1,604 as the dose to the retina increases by 10 cGy. The odds of seeing steady light multiply by 1,540 as the dose to the vitreous humour increases by 10 cGy. ConclusionsWe postulate that one should reduce the dose rate, instruct patients to keep the eyes open and increase the illuminance in the treatment room to reduce the probability of experiencing visual phosphenes. We hypothesize that melanopsin is involved in the visual phosphenes and that fatigue of patients might be correlated with the observation of visual phosphenes.

RETRACTED – Determination of geometrical margins in external beam radiotherapy for prostate cancer

AbstractIntroductionThe focus of this study is to find the optimal clinical tumour volume (CTV) to planning tumour volume (PTV) margins for precise radiotherapy treatment of prostate cancer. The geometrical shape of the target volume posses challenges in accurately identifying the CTV to PTV margins, especially when the organ affected by cancer demonstrates anatomical variations and the surrounding organs have high radio-sensitivity, in comparison to the organ of origin of the cancer.Materials and methodsThe geometrical margins of CTV to PTV are investigated using portal imaging, in three directions. This study is carried out on 20 patients treated by the external photon beam radiotherapy of prostate cancer using standard accelerator without stereotaxic and without prostate markers.Results and discussionBased on previous studies and the findings of our work, we propose CTV to PTV margin of 5·84 mm in the lateral direction, of 5·1 mm in the cranio-spinal direction and of 7·3 mm in the antero-posterior direction for external photon beam radiotherapy of prostate cancer.ConclusionThe proposed CTV to PTV margins ensure high radiotherapy treatment precision of prostate cancer.

In vivo dosimetry of intraoral stent using TLD during external photon beam radiotherapy of oral cavity

Introduction: Individual oral stent is a mouth-opening device that used in head and neck cancer radiotherapy with the intention of decreasing radiation dose to health tissues. The aim of this study was to check the dose delivered to both the hard palate and tongue when patient uses the stent during radiotherapy of tongue carcinoma. Methods and materials: The intraoral stents were made for 10 patients and TLD-100 dosimeters were placed at superior and inferior parts of stent at the level of hard palate and tongue, respectively. Then patients treated with 3DCRT technique using 6MV photons, the data from the TLDs and treatment planning system calculations were analyzed. Results: The results showed that the mean (±standard deviation) relative difference between measured and calculated doses for tongue and hard palate was -4.41± 6.34, 0.57± 9.6 respectively. Conclusion: This study demonstrated the value of thermoluminescent dosimetry as a treatment verification method and its usefulness departmental quality assurance program.

Dosimetry of small static fields used in external photon beam radiotherapy: Summary of TRS-483, the IAEA-AAPM international Code of Practice for reference and relative dose determination.

A joint IAEA/AAPM international working group has developed a Code of Practice (CoP) for the dosimetry of small static fields used in external megavoltage photon beam radiotherapy, published by the IAEA as TRS-483. This summary paper introduces and outlines the main aspects of the CoP. IAEA TRS-483 is a condensation of the wide range of different approaches that have been described in the literature for the reference dosimetry of radiotherapy machines with nominal accelerating potential up to 10MV that cannot establish the conventional 10cm ×10cm reference field, and for the determination of field output factors for relative dosimetry in small static photon fields. The formalism used is based on that developed by Alfonso etal. [Med Phys. 2008;35:5179-5186] for this modality. Three introductory sections describe the rationale and context of the CoP, the clinical use of small photon fields, and the physics of small-field dosimetry. In the fourth section, definitions of terms that are specific to the CoP (as compared to previous CoPs for broad-beam reference dosimetry, such as IAEA TRS-398 and AAPM TG-51) are given; this section includes a list of the symbols and equivalences between IAEA and AAPM nomenclature to facilitate the practical implementation of the CoP by end users of IAEA TRS-398 and AAPM TG-51. The fifth section summarizes the equations and procedures that are recommended in the CoP and the sixth section provides an overview of the methods used to derive the data provided in IAEA TRS-483. This is the first time an international Code of Practice for the dosimetry of small photon fields based on comprehensive data and correction factors has been published. This joint IAEA/AAPM CoP will ensure consistent reference dosimetry traceable to the international System of Units and enable common and internationally harmonized procedures to be followed by radiotherapy centers worldwide for the dosimetry of small static megavoltage photon fields.

Considerations when treating lung cancer with passive scatter or active scanning proton therapy.

Lung cancer, due to its poor clinical outcomes and significant toxicity associated with standard photon-based radiation, is a disease site that has the potential to greatly benefit from accurate treatment with proton radiation therapy. The potential of proton therapy is the ability to increase the radiation dose to the tumor while simultaneously decreasing the radiation dose to surrounding healthy tissues. For lung cancer treatment, this could mean significant sparing of the uninvolved healthy lung, which is difficult to achieve with external photon beam therapy, or decreasing the heart dose. In treating lung cancer with proton therapy, some additional considerations need to be made compared to treating patients with external photon beam radiation therapy. These include accounting for the finite range of protons in the patient, understanding temporal effects, potential dose discrepancies and choosing an appropriate treatment planning system for the task. One final consideration is differences between the different available proton therapy delivery systems-passive scattered proton therapy (PSPT) and active scanning proton therapy.

Efficacy and Safety of Three or More Courses of Radiation for Head and Neck Malignancies

Recurrent head and neck cancers are associated with significant morbidity and mortality. Salvage treatments are poorly defined and the outcomes of more than two courses have not yet been described. In this study, we investigate the safety and feasibility of delivering three or more courses of radiation with overlapping fields in patients with recurrent head and neck malignancies. A single-institution database of 3,063 patients who underwent head and neck radiation between 2011 and 2017 was queried for re-irradiation. Treatments plans were reviewed to identify patients who underwent at least three courses of overlapping radiation. Thirty-six patients were identified with three or more courses of head and neck radiation therapy with overlapping fields. Toxicity was assessed using Common Terminology Criteria for Adverse Events version 4.0. Kaplan-Meier survival analysis was performed to calculate median survival times. Thirty-six patients were identified with three or more courses of overlapping radiation to the head and neck region. Thirty-two patients underwent treatment for multiply recurrent disease and 4 patients were treated for metachronous head and neck primary tumors. Radiation therapy spanned from 1984 to 2017 and included external beam photon radiation, intraoperative brachytherapy, and proton radiation. Primary tumor sites included skin (10), oral cavity (5), nasal cavity/paranasal sinus (6), nasopharynx (4), salivary gland (4), larynx (4), oropharynx (3), and thyroid (2). Median age at the start of the third radiation course was 64 years of age. Median time intervals were 27 months between the 1st and 2nd courses of radiation, and 14 months between the 2nd and 3rd courses of radiation. Median doses for the 1st, 2nd, and 3rd courses of radiation were 64, 57, and 44 Gy, respectively. Spinal cord, brainstem, and optic nerves were the most common organs at risk that exceeded institutional guidelines. 10 patients (28%) experienced long-term toxicities of grade 3 or higher, including PEG placement (4), soft tissue necrosis (2), bleeding (2), osteoradionecrosis (1), and tracheoesophageal fistula (1). Median survival after completing three courses of radiation was 15.6 months (95% confidence interval 9.2-32.4 months). Multiple courses of re-irradiation to the head and neck region are feasible with an acceptable toxicity profile.

Abstract P2-11-09: Excellent acute toxicity outcomes with proton therapy for partial breast irradiation in early stage breast cancer: Initial results of a multi-institutional phase II trial

Abstract Background and Purpose: Partial breast irradiation (PBI) with proton therapy after lumpectomy for early stage invasive breast cancer is an area of active investigation. Advantages of this technique include a shorter treatment course and the potential for decreased morbidity versus external beam photon radiation therapy given superior sparing of the surrounding normal breast tissue. To date, multiple single-institutional studies have reported conflicting results on the acute toxicity of PBI. This prospective phase II trial investigates the feasibility, safety, and efficacy of delivering PBI with proton therapy in a multi-institutional setting. Methods: Patients over the age of 50 years with ER-positive nonlobular invasive breast cancer or ductal carcinoma in situ ≤3 cm in size who had undergone lumpectomy with at least 2 mm negative surgical margins were treated with proton therapy to a dose of 40 Gy delivered over 10 daily fractions. In this initial analysis, we assess early toxicity and treatment efficacy of proton PBI. Patients were followed up at 4 weeks post-treatment and annually thereafter, along with annual mammograms. Patient-reported quality of life and physician-reported cosmesis assessments including photographs were obtained at 1 and 3 years post-treatment. Results: Forty patients were enrolled, of which 38 were evaluable. At a median follow-up of 17.8 months (range 2-36 months), all patients had overall breast cosmesis that was scored “good” or “excellent”. Of 6 grade 2 acute adverse events that occurred, only 1 was radiation dermatitis, with others including lymphedema, hot flashes, and fatigue. One grade 3 acute toxicity occurred 3 weeks after radiation completion in the form of vascular disease requiring stent placement, highly unlikely to be attributable to radiation effects. Patient-reported quality of life outcomes were recorded using the standardized Breast Cancer Treatment Outcome Scale (BCTOS) scored from 1-4 (1: none; 2: mild; 3: moderate; 4: large), with endpoints receiving a score of 3 or 4 most frequently involving change in breast size, breast texture, nipple appearance, or scar tissue. Patients assigned a score of 4 for change in nipple appearance (n=2), breast shape (n=2), and scar tissue formation (n=2). To date, local, locoregional, and distant disease control are 100%, although one patient has developed a new hormone receptor-negative invasive ductal carcinoma of the contralateral breast. Conclusion: Proton PBI provides excellent early cancer control with acceptable cosmetic outcomes and minimal adverse effects as per patient- and physician-reported assessments. Factors Associated with Occurrence of Radiation-induced Optic Neuropathy at “Safe” Radiation Dosage

ABSTRACTBackground: Radiation-induced optic neuropathy (RION) is a rare, and often visually devastating, complication of radiation therapy (RT) near the anterior visual pathways. Methods: A retrospective case series of patients who developed RION at a tertiary medical center, followed by a case-control study comparing RION cases with matched controls who received RT. Results: Thirteen patients (18 eyes) with RION were identified. Radiation modalities included external beam photon radiation, whole brain radiation, stereotactic radiosurgery, proton beam, and unknown. Most patients received doses below published “safe” thresholds (<55 Gy; <8–10 Gy for stereotactic radiosurgery). There was no statistically significant difference in prevalence of vasculopathic factors between cases and controls; on subgroup analysis in three patients who received surprisingly low radiation doses, smoking (p=0.05) and hypertension (p=0.02) appeared more prevalent. Conclusion: RION can occur at doses below published “safe” thresholds and with different RT modalities. Smoking and hypertension might be risk factors for RION.

Dosimetric characterization of 3D printed bolus at different infill percentage for external photon beam radiotherapy

Background and purpose3D printing is rapidly evolving and further assessment of materials and technique is required for clinical applications. We evaluated 3D printed boluses with acrylonitrile butadiene styrene (ABS) and polylactide (PLA) at different infill percentage. Material and methodsA low-cost 3D printer was used. The influence of the air inclusion within the 3D printed boluses was assessed thoroughly both with treatment planning system (TPS) and with physical measurements. For each bolus, two treatment plans were calculated with Monte Carlo algorithm, considering the computed tomography (CT) scan of the 3D printed bolus or modelling the 3D printed bolus as a virtual bolus structure with a homogeneous density. Depth dose measurements were performed with Gafchromic films. ResultsHigh infill percentage corresponds to high density and high homogeneity within bolus material. The approximation of the bolus in the TPS as a homogeneous material is satisfying for infill percentages greater than 20%. Measurements performed with PLA boluses are more comparable to the TPS calculated profiles. For boluses printed at 40% and 60% infill, the discrepancies between calculated and measured dose distribution are within 5%. Conclusions3D printing technology allows modulating the shift of the build-up region by tuning the infill percentage of the 3D printed bolus in order to improve superficial target coverage.
