A healthy dog and two dogs with tracheal collapse (TC) were treated using a Gianturco-Wallace type expandable metallic stent (EMS). The EMS was placed through the upper trachea in the healthy dog and at the stenotic site in the two dogs with TC under roentgenoscopy. The general condition of the healthy dog was normal after EMS placement. When the healthy dog was euthanatized after 53 days, the EMS was buried submucously. For the dogs with TC, radiographic examination after treatment revealed sufficient expansion of airway at the stenotic site in both cases, After placement of the EMS, the frequency of coughing gradually decreased and dyspnea with cyanosis disappeared completely. After about a year, however, constriction of the esophagus and recurrence of TC at the caudal site of EMS placement were observed, with death occurring suddenly in both cases. Therapy using an EMS for TC will be simple and extremely effective if materials and expansive force of the EMS can be improved.

Sequential silastic and expandable metal stenting for tracheobronchial strictures

The risks and limitations of surgical resection and reconstruction for tracheobronchial strictures demand consideration of ether therapeutic options that can alleviate the distressing symptoms of tracheobronchial obstruction. One alternative is to stent the obstructive lesion until surgical advances allow primary reconstruction or replacement of the critically diseased airway or until an ideal endoprosthesis is found. The latter requires uniformity in the distribution of expansile force, conformability and stability within the tracheobronchial tree, and ease of placement. Here we report our experience with the placement of expandable metal stents (Wallstent) used in conjunction with our Silastic (Dow Corning) endobronchial stents in 5 patients with recurrent tracheal or bronchial strictures. The major site of obstruction was the trachea in 1 patient and a main bronchus or both bronchi in 4 patients. Three patients had a benign bronchial stricture (anastomotic stricture in 2, idiopathic polychondritis in 1), and 2 patients had an obstructive airway neoplasm. Placement of the stents was performed under rigid bronchoscopic guidance. We had no complications from our technique of stenting. There has been no evidence of restenosis or occlusion within the stented segment of airway. The complementary use of expandable metal and Silastic endobronchial stents provided symptomatic and functional improvement in our patients during follow-up ranging from 5 to 24 months.

Budd-Chiari Syndrome with Long Segmental Inferior Vena Cava Obstruction: Treatment with Thrombolysis, Angioplasty, and Intravascular Stents

The authors describe a patient with Budd-Chiari syndrome caused by long segmental thrombotic obstruction of the inferior vena cava associated with paroxysmal nocturnal hemoglobinuria. The patient was successfully treated with a combination of local thrombolytic therapy, balloon angioplasty, and placement of Gianturco expandable metallic stents.

Expandable venous stents for treatment of the Budd-Chiari syndrome

The goals of treatment of the Budd-Chiari syndrome are relief of portal hypertension, relief of inferior vena cava syndrome, if present, and preservation of hepatic function. This study presents a patient with clinical resolution of the Budd-Chiari syndrome after placement of expandable metallic stents in the inferior vena cava and hepatic veins. A 26-year-old man with severe ascites and lower extremity edema but with relatively preserved hepatic function had a small gradient across a suprahepatic caval web, large gradients across an intrahepatic caval stenosis and the left hepatic vein, and an occluded right hepatic vein. Under angiographic control, web and caval stenosis were balloon-dilated, and modified Gianturco expandable metallic stents were placed in the intrahepatic vena cava. The left hepatic vein was dilated twice and a stent was placed. All gradients were completely eliminated. There were no complications and after 1 year, the stents have fully expanded without migration, edema and ascites have resolved, hepatic function has normalized, and the patient has returned to work. This new technique provides a simple, safe, effective, relatively inexpensive, and potentially long-lasting treatment for selected patients with the Budd-Chiari syndrome.

Gianturco-Rosch Biliary Stents: Preliminary Experience

Results of long-term follow-up are reported for nine patients who underwent placement of Gianturco-Rosch expandable metallic stents. Seven patients had malignant disease. The average duration of patency was 7 months. In one patient with inactive primary sclerosing cholangitis (PSC), the stents remained patent at 22 months, but recurrent stent closure was seen in a second patient with PSC. In the patient population studied, no advantages were seen with use of the stents; however, they may have an application in certain patients with benign disease.

Endoscopic Placement of Expandable Metal Stents for Biliary Strictures - A Preliminary Report on Experience with 33 Patients

The main problem with palliative treatment of extrahepatic cholestasis with an endoscopic biliary endoprosthesis is clogging. One of the factors thought to be of importance is the diameter of the stent. In order to avoid being limited by the size of the instrumentation channel of the endoscope, expandable stents have been developed. In this article we report on our preliminary clinical experience with an endoscopically placed expandable metal stent ("Wallstent") in 33 patients with extrahepatic bile duct stenoses. When fully expanded, the stent has a diameter of 30 F and a length of 6.7 cm. It was possible to successfully place a stent in every patient. Clinical improvement was achieved in all patients except one. Two patients underwent elective surgery, while one died of renal failure. Another died of septic shock after 5 weeks, but no autopsy was performed. In conclusion, our initial experience with this stent shows that at least in the short term biliary drainage was excellent, with no complications of pancreatitis or hemorrhage. Longer follow-up than our 4 weeks is necessary to establish the position of this stent in comparison with the conventional endoprosthesis in the management of obstructive jaundice.
