TREATMENT OF MEDICATION-RELATED OSTEONECROSIS OF THE JAWS WITH ULTRASONIC PIEZOELETRIC BONE SURGERY AND OXYGEN THERAPY: A CASE REPORT

A 65-year-old femal patient complained mainly of "an exposed bone that appeared after tooth extractions performed eight months earlier". Her medical history revealed breast cancer and lung metastasis, undergoing treatment with chemotherapy, radiotherapy, and Zoledronate. Intraoral examination revealed necrotic bone in the mandible with the presence of exudate. Panoramic radiographs demonstrated evidence of a radiopaque image, suggestive of bone necrosis involving the region of lower incisors and extensive area of bone sequestration spread across the body of the mandible to the region of pre-molars. Treatment included 0.12% chlorhexidine digluconate mouth rinses, antibiotic therapy, laser therapy, and hyperbaric oxygen therapy before and after surgery. Surgery was performed using vibrating tips connected to a high power ultrasonic (piezo) device, and the necrotic bone which was sent for histological analysis. The patient received followed up for two years, presenting good general health and no sign of bone exposure. الملخص يسعي هذا البحث من خلال المنهج الوصفي التحليلي إلى تفسير وتحليل ظاهرة أمنيات الموت والتخلص من الحب في الشعر الغزلي في العصر الأموي، وقد اختار الباحث هذه الظاهرة لکثرة شواهدها، وتنوع الأساليب والصورة الفنية عند شعراء الغزل. وقد اقتضت الدراسة أن يکون البحث مقسما على مبحثين قبلهما مقدمة وبعدها خاتمة. وقد ناقشنا في مقدمة البحث أسباب انتشار شعر الغزل في العصر الأموي عامة وفي الحجاز بصفة خاصة، کذلک بين البحث بعض الدلالات المهمة لانتشار شعر الغزل، کالحرية الدينية، والقيود المجتمعية، والأثر الکبير للإسلام على هذا النوع من الشعر. وفي المبحث الأول تتبع البحث ذکر الموت في أماني شعراء الغزل، وبين أسباب تمني بعض الشعراء له، وهي في الغالب بسبب ألم الفراق والبعد، ومن الظواهر التي رصدها البحث في هذا الموضوع تمني الشعراء أن يلتقوا بمحبوباتهم في القبر وفي الآخرة ، وهناک ظاهرة أخرى في هذا المبحث هي الدعاء على أعداء الحب والعاذلين بالموت. وفي المبحث الثاني وهو تمني التخلص من الحب اتخذ الشعراء مسالک عدة منها الندم على الوقوع في الحب، ودعاء القلب بالإفاقة من ضلال الحب، وفي الخاتمة خلص البحث إلى أهمية النظر إلى ظاهرة أماني الشعراء ودلالاتها المتعدة، کذلک فلسفة الشعراء في ذکر الموت Abstract This paper attempts, through descriptive analytical approach, to justify and analyze the phenomenon of ‘death wishes and getting rid of love in flirtation poetry in Umayyad Era. The researcher chose this phenomenon due to its several evidence and variety of styles and technical image used by flirtation poets. The study is divided into an introduction, two sections and a conclusion. The research introduction dealt with causes of wide spread of flirtation poetry in Umayyad Era in general, and in Higaz region in particular; it also showed some significant implications of its spread, such as religious freedom, community restrictions and the great impact of Islam on this type of poetry. Section One reviewed death as one of flirtation poets’ wishes and causes of such wishes like pain of separation. This section revealed some phenomena such as poets’ wishes to meet their beloved people in the hereafter and supplication upon love enemies and losers with death. Section Two, entitled “Wishing to Get Rid of Love”, showed poets’ several attitudes such as regret to fall in love and call for heart to get rid of love delusion. The conclusion revealed the importance of considering the phenomenon of poets’ wishes with their several implications, philosophy of mentioning death and their several ways taken to end their sufferings.

Abstract 2773: Increased tumor control boosting hypoxic regions identified with EPR pO2 imaging in 2 tumor types

Abstract Introduction: It has been known for over a century that all living systems develop radiation resistance under hypoxic conditions - low pO2. But giving extra radiation - dose painting - or boosting hypoxic tumors regions has not shown enhanced control in mammalian tumors. Pulse EPR quantitative pO2 values can be tomographically imaged as a magnetic resonance measurement with fixed stepped gradients. If there are distinct hypoxic regions, their images can then be extra radiation dose boosts. An isocentric animal irradiation system allowed conformal boosts to tiny resistant tumor subvolumes with 3D printed tungsten loaded plastic blocks. A randomized trial compared radiation boosts to hypoxic tumor vs well oxygenated tumor to determine hypoxic boost control advantage. Methods: Carcinomas (MCa4) and fibrosarcomas (FSa) in mouse legs with mean volume of 350 μl were treated with an XRAD225Cx animal irradiator to a dose that controlled 15% of tumors (TCD15), determined in separate experiments. Pulse electron paramagnetic resonance (EPR) using Spin Lattice Relaxation (SLR) and a soluble trityl spin probe has been shown to quantitatively image solution and tumor pO2. This and boost treatments were given after EPR pO2 imaging, reconstruction, registration with tumor defining T2 MRI, determination of all tumor hypoxic regions (pO2 ≤10 torr) and 3D printed tungsten loaded plastic aperture fabrication to radiate either well oxygenated or hypoxic tumor regions. Mice were followed for either 180 days (MCa4) or 90 days (FSa) sufficient for 99% of local failures to occur. Treatment of 3D hypoxic volumes was also analyzed for control dependence on surface to volume ratio (SVR). Results: Tumor control was doubled by radiating hypoxic tumor relative to well oxygenated tumor in 48 MCa4 tumors (p=0.013) and 54 FSa tumors (p=0.04). This was the first demonstration in mammalian tumors of an advantage to targeting hypoxic regions in tumors. It provides the first evidence of image based dose painting within the volume of the tumor. For MCa4 tumors, SVR values below median had significant increased survival (p=0.003) while for FSa tumors below median SVR tumors trended to increased survival (p=0.3). Clinical Relevance: Modern radiation delivery used intensity modulated radiation therapy (IMRT) for dose gradients outside the tumor volume to reduce doses to critical structures. Dose within the tumor have strict inhomogeneity limits. Allowance of higher dose variation within the tumor volume allows “hot spots” to resistant tumor volumes, sharper gradients outside the volume to protect critical structures. Muslim Businessmen and Chinese Economics in Singkawang

Instead of being known as the “City of Tasbih”, nowadays Singkawang is either known as the “City of Amoy (Chinese girls)” or the “City of Thousand ChineseTemples”. Cap Go Meh annually International Festival is another evidence of Chinese image making towards the city. This indicates a sort of Chinese domination that must be studied, economically, politically, and socially. Furthermore, the research aims to explore the dynamics of Chinese Counterpart especially the Muslim Business elites' challenge such as ironic the ongoing dominating development. To explore the dynamics, in-depth interviews were held. Key figures among Muslim businessmen, Chinese elites, government officials, even laypeople were questioned in order to obtain a clear description and balanced analysis of possible biases and perspectives of the interviewees. Quantitative data are analyzed and discussed. Methods of determining titanium dioxide nanoparticles enhance inorganic arsenic bioavailability and methylation in two freshwater algae species

We developed the effect of titanium dioxide nanoparticles (nano-TiO2) on the bioaccumulation and biotransformation of arsenic (As), which remain largely unknown. We thus exposed two freshwater algae (Microcystis aeruginosa and Scenedesmus obliquus) to inorganic As with the aim of increasing our understanding on As bioaccumulation and methylation in the presence of nano-TiO2. Direct evidence of TEM and EDX image showed that nano-TiO2 (anatase) entered the exposed algae. Thus, nano-TiO2 as carriers boosted arsenic accumulation and methylation in these two algae species, which varied with both inorganic As speciation and algae species. Specifically, nano-TiO2 could enhance markedly arsenate accumulation in M. aerugginosa and arsenite accumulation in S. obliquus. Similarly, we found higher content of As methylation in M. aeruginosa of arsenite with 2 mg L-1 of nano-TiO2 treatment and in S. obliquus of arsenate treatment. Additionally, S. obliquus exhibited higher As methylation compared to M. aeruginosa, being more sensitive to As associated with nano-TiO2 than M. aeruginosa. Due to changes in pH levels inside these exposed algae, the As dissociation from nano-TiO2 inside algal cell enhanced As methylation. Accordingly, the potential influence of nanoparticles on the bioaccumulation and biotransformation of their co-contaminants deserves more attention.•Nano-TiO2 entry is assumed to promote As accumulation into exposed algae.•Nano-TiO2 had different carrying capacities for different forms of As and algae.•As dissociation from nano-TiO2 is assumed to enhance As methylation in algae.

Mathematics education graduate students’ understanding of trigonometric ratios

ABSTRACTThis study describes mathematics education graduate students’ understanding of relationships between sine and cosine of two base angles in a right triangle. To explore students’ understanding of these relationships, an elaboration of Skemp's views of instrumental and relational understanding using Tall and Vinner's concept image and concept definition was developed. Nine students volunteered to complete three paper and pencil tasks designed to elicit evidence of understanding and three students among these nine students volunteered for semi-structured interviews. As a result of fine-grained analysis of the students’ responses to the tasks, the evidence of concept image and concept definition as well as instrumental and relational understanding of trigonometric ratios was found. The unit circle and a right triangle were identified as students’ concept images, and the mnemonic was determined as their concept definition for trigonometry, specifically for trigonometric ratios. It is also suggested that students had instrumental understanding of trigonometric ratios while they were less flexible to act on trigonometric ratio tasks and had limited relational understanding. Additionally, the results indicate that graduate students’ understanding of the concept of angle mediated their understanding of trigonometry, specifically trigonometric ratios.

Symbol and Image Worship in Jainism

The history of Jainism in Mathura has been reconstructed largely on the basis of donative inscriptions and the remains of temples, stūpas and tīrthaṅkara images unearthed in this area. The āyāgapaṭas—an integral part of Mathura art and extremely important for the understanding of Jaina art and religious practice—have been relatively neglected. The āyāgapaṭas reflect the simultaneous existence of symbol and image worship in Jainism. The practice of donating these carved slabs started in the first century BCE and continued till the third century CE. The āyāgapaṭas were intended for worship as the inscriptions specify that they were set up for the worship of arhats ( tīrthaṅkaras). This article aims to understand the āyāgapaṭa inscriptions in the context of their relief carvings. The religious imagery on these slabs represents various foci of Jaina worship, namely the stūpa, the dharmacakra and the Jina image. The evidence of the Jina image carved on the āyāgapaṭas as early as the first century BCE, points towards the prevalence of image worship in Jainism even before the first century CE. The analysis of the āyāgapaṭa inscriptions highlights the varied occupational background of donors and the active role played by women donors. The article explores the probable reasons for the discontinuation of āyāgapaṭas after the third century CE at Mathura. Finally, it clearly establishes that āyāgapaṭas were installed in temples and monasteries and were objects of worship and an important component of Jaina religious practice.

AbstractLittle more than a decade after publishing Invisible Man (1952), Ralph Ellison published “Out of the Hospital and under the Bar” (1963): an excised chapter of the novel that portrayed Mary Rambo—a black woman represented stereotypically in the published manuscript—as a necessary agent in the protagonist's emergence from the paint-factory hospital. As scholars have contested the significance of this chapter and Mary's representation in it, the Invisible Man drafts, made available to the public in 1998, reveal that this chapter is more significant than previously thought: this section of the novel underwent more revision than any other and is the sole portion of the drafts to be published as a narrative unto itself. This essay explains how the expurgated chapter reveals how voice, agency, and memory affect Invisible Man's physical and psychological ascent and confirms Ellison's deep respect for the wisdom imbedded within black folk culture. By reading Mary Rambo alongside her peers who also represent a folk past—namely Trueblood, Peter Wheatstraw, and Brother Tarp—this essay offers early evidence of Ellison's image of the modern black protagonist and prompts us to imagine what might have been had Invisible Man been able to experience the fullness of Mary's folk wisdom as Ellison had originally conceived.

International Stereotype and the Collective Construction of Destination Image

This research examines destination image as a collective construction by utilizing the context of image formation, particularly focusing on the context of structural relationships between guest and host nations. Based on international image theory that suggests overall perceptions of a foreign nation stemming from the perceived relationships between two nations or international stereotypes might be collectively formed within a nation and serve as a predictor of internation attitudes, this research investigates the effects of international stereotypes on destination image. The results indicate that people have more negative destination perceptions of a perceived unfriendly nation than a perceived friendly nation, which provides evidence of destination image as a collective construction. The results further highlight the importance of context in the process of image formation.

Black women are at high risk for obesity and obesity-related health problems (1). Nearly 50% of black women compared to 30% of white women are obese (2,3). While many women who attempt to lose weight do so through caloric restriction and/or physical activity, smoking has been identified as an alternate strategy used for weight loss among black women with weight concerns or body image concerns (4). Image fakery is barely unrecognizable because the fake image is looking so natural. Yet by using the numerical computation technique it is able to detect the evidence of fake image. This research is successfully applied the singular value decomposition method to detect image fakery. The image preprocessing algorithm prior to the detection process yields two vectors orthogonal to the singular value vector which are important to detect fake image. The result of experiment to images in several conditions successfully detects the fake images with threshold value 0.2. Singular value decomposition-based detection of image fakery can be used to investigate fake image modified from original image accurately. Diffusion-weighted MR imaging for evaluation of pediatric recurrent cholesteatomas

Objective To compare the efficiency of diffusion-weighted MR imaging (MRI) vs. high resolution CT in predicting recurrent or residual cholesteatoma in children who underwent prior middle ear surgery. Design Prospective study. Setting Tertiary care university hospital. Patients Seventeen patients (4 with 2 recurrences) aged 5–17 years (mean 11.4) previously surgically treated for a cholesteatoma of the middle ear, were included for follow-up with systematic CT scan and MRI, between 2005 and 2007. Methodology CT scan was performed on a Siemens Somaton 64 (0.5/0.2 mm slices reformatted in 0.5/0.3 mm images), parallel and perpendicular to the lateral semi-circular canal for each ear (100 mm × 100 mm FOV). MRI was undertaken on a Siemens Avanto 1.5 T unit, with an adapted protocol for young children. Diagnosis of recurrent cholesteatoma was based on the evidence of a hyperintense image at B1000 on diffusion-weighted images. Results of CT scan and MRI were compared with operative diagnosis. Results Nine patients had a positive MRI, among which 8 had cholesteatoma confirmed during revision surgery. In the 12 negative MRI cases, 5 were positive on revision surgery. None of these lesions was over 3 mm. Two of them were diagnosed on the CT scan. CT scan alone had a positive predictive value of 75%, and a negative predictive value of 58%. Conclusion Diffusion-weighted MRI is associated with a high positive predictive value for the detection of recurrent cholesteatoma. CT scan remains the first choice imaging technique. In case of doubtful CT scan, diffusion-weighted MRI could confirm a recurrence or, when negative, avoid second-look surgery.

From the electric field analysis around the needle electrode tip, we believe that the filamentlike light image expands beyond the critical electric field of SF6 gas. Thus, we concluded that these current waveforms with double peaks showed evidence of leader-type PD, leading to breakdown. Orientation Specificity: How General Is It?

The extent to which individuals show evidence of an orientation-specific map image was assessed in three experiments. Experiment 1 used a simple path map as the stimulus. Experiment 2 used a more complex map of a real campus environment and allowed repeated exposure over a 4-week period. Experiment 3 used a scale model of the campus and repeated exposures. In all three experiments evidence was found for a subgroup that exhibited no evidence of an orientation-specific map image. Group averages masked the existence of this subgroup, and instead tended to be more reflective of a smaller subgroup showing strong evidence of orientation specificity. The results caution against overgeneralizing group averages in this area of spatial cognition research.

Young children's perceptions of science and the scientist

A Draw‐a‐Scientist‐Test was done by 1143 children between the ages of 4 + and 11 + years. Attributes of the scientists and the backgrounds were recorded for each year group and compared one with another and with the drawings of a control group. There was evidence of a stereotypical image of the scientist as early as six years of age. Science tended to be depicted as an indoor activity for making new materials and artefacts, and for the study of animals. It was suggested that strategies intended to change perceptions of science may be applied too late and too narrowly to be fully effective. While the National Curriculum for Science may widen children's perceptions of the nature of science, it may have much less effect on children's image of the scientist.

Hairlike variations of filiform papillae in the human tongue

The ultrastructural characteristics of the hairlike variations of filiform papillae in the human tongue were studied by scanning and transmission electron microscopy. The scanning electron microscope gives evidence of a three-dimensional image of abnormally long secondary papillae with the epithelial cells arranged like roof tiles. These are partly adhering, partly detached, and then protruding from the core, thus forming the hairlike structure. Transmission electron microscopy of ultrathin sections shows single or partly adhering, orthokeratinized epithelial surface cells, mostly with distinct signs of degradation, surrounded by microorganisms. The pathomorphologic picture of elongated, hairlike filiform papillae was observed on the tongue coating of patients with clinically diagnosed hairy tongue but also on some small areas of the tongue of infants. These latter findings cannot be characterized by the clinical term “hairy tongue” but must be considered to be hairlike variations of filiform papillae.

Image Reconstruction with Fraunhofer Holograms*

The holograms considered are formed from opaque or transparent diffracting objects which are contained in an aperture illuminated with a coherent, collimated, quasimonochromatic beam of light. It has been shown that the intensity distribution in the near-field of the aperture, but in the far-field of the individual objects which are contained in the aperture, is given by a function whose essential term represents the interference between the Fraunhofer diffraction pattern from the object and the coherent background. The hologram thus formed is referred to as a Fraunhofer or far-field hologram because of the imposed condition. The reconstruction, which is accomplished by placing the recorded hologram in another coherent collimated quasimonochromatic beam and again going to the far-field of the individual objects, yields an intensity which is essentially the original object distribution. In the far-field region of the individual objects for which this result is valid, the reconstruction is seen to be devoid of the evidence of a virtual image. One advantage of this particular method is that the virtual image which appears in the conventional (Fresnel) hologram method creates no problem here since it reduces to a constant for the far-field approximation.
