If chaos in therapy is to be avoided, it is plain that new and more accurate methods of evaluation, less costly of time but also of money, must be found.

The evaluation of therapy in chronic illness.

The problem of evaluation of therapeutic agents in medicine is complicated by the demand of the sick person for some type of treatment. Since the time of Hippocrates physicians have had to manage desperately ill patients with drugs or other measures which were of very doubtful value. While perhaps few lives were saved, great solace was given these patients by the wise and kind physician. As a result physicians traditionally have not developed a highly critical sense in the evaluation of therapy. With the recent development of effective, and in some instances dangerous, drugs, the need for careful scientific evaluation of therapeutic effectiveness has become urgent. While many of the traditional modes of treatment in medicine would not withstand careful scientific scrutiny, there seems to be no urgency in discarding them. There is, however, considerable merit in evaluating new drugs and new types of therapy as they appear in the

RENAL BIOPSY IN CHILDREN

RENAL biopsy offers invaluable aid in the clinical diagnosis of kidney disease and is an important technique in research designed to clarify the etiology, pathogenesis, and evaluation of therapeutic agents, in a variety of renal diseases. The majority of the scientific reports describing renal biopsy have concerned adult patients. The few available reports of renal biopsy in children do not discuss the risks attending the procedure or the specific problems peculiar to kidney biopsy in children. A review of our experience in 150 renal biopsies in children may afford a basis for evaluation of these questions. The available techniques of renal biopsy include: 1) surgical exploration and removal of a segment of kidney cortex, and 2) percutaneous needle biopsy.

Enzymatic evaluation of therapeutic agents in cancer.

A PRINCIPLE FROM LIVER EFFECTIVE AGAINST SHOCK DUE TO BURNS

Following the development of a method for satisfactorily evaluating therapeutic activity in shock due to burns, various agents, some of which have been proposed for the treatment of shock, have been tested. Our purpose in this preliminary report is to present evidence for a principle from liver which has the activity of significantly decreasing the mortality of rats and mice subjected to severe burns and to describe briefly a method for producing standardized burn shock. Evidence is presented which demonstrates that this factor effective against burn shock, which is present in commercial liver extracts prepared to contain the anti-anemia principle, is separate and distinct from the anti-anemia activity. METHODS The evaluation of therapeutic agents in shock using the usual shock producing procedures in large laboratory animals is complicated by factors which are difficult to control adequately. It would appear that a satisfactory method should possess the following features: The degree
