Assessing competency to address ethical issues in medicine

The dissection experience as a laboratory for self‐discovery about death and dying: Another side of clinical anatomy

AbstractDissection of the human body raises questions for medical students about the source of bodies, bodily trespass, invasion of privacy, death, dying, and their own mortality. Facing and expressing the aversions, fears, and fantasies associated with human dissection help prepare the student both for academic work in the anatomy laboratory and for the emotional work implicit in patient care. With little additional curricular time within the longitudinal continuity of the dissection course we have developed a program in medical humanities that fosters both skills. This program, heavily dependent upon the arts, uses exposure to painting, film, and literature coupled with reflection, writing, and small group discussions to explore and express students' attitudes toward death and dissection as they experience the first year of medical school. The final session, a service of memoriam and thanksgiving planned and produced by the students, provides an appropriate personal closure of the dissection experience. Our students affirm that such a program, evolving over the past 12 years, has provided a foundation early in medical education for development of caring physician‐patient relationships and for continued exploration of humanistic and ethical issues in medicine. These experiences illustrate that clinical anatomy has the potential to contribute to both the art and science of medicine.

Physicians, Journalists, Ethicists Explore Their Adversarial, Interdependent Relationship

WORKING relationships among journalists, physicians, and ethicists are tenuous, often adversarial, and strangely interdependent. To explore these relationships, members of the Center for Biomedical Ethics, Case Western Reserve University School of Medicine, Cleveland, recently convened a one-day conference with two objectives: to provide a forum for these three groups to discuss how best to inform society about ethical issues in medicine and health care and to educate one another about their respective needs and perspectives. The conference had sessions on the drama and dilemma of organ transplantation, reporting on acquired immunodeficiency syndrome (AIDS), and conflicts involving mothers and fetuses. Medicine Man: A Young Doctor on the Brink of the 21st Century

Few physicians may be examining the moral consequences of the changes being brought about in medicine by the growth of technology. These changes will affect not only physicians but everyone who comes in contact with them. This book will function as a sextant by means of which we can check our present position and examine those consequences. Ethical Analysis of Clinical Medicine: A Guide to Self-Evaluation

Ethics is that branch of philosophy that deals with right and wrong and with the moral consequences of human action. A rapidly growing literature of medical ethics testifies to the importance and the immediacy of the ethical problems involved in patient care. Many physicians have an incomplete understanding of medical ethics and are often uncertain as to how to proceed in the resolution of ethical issues arising in the care of patients. This book is a response to the needs of these physicians. The book stimulates the reader to participate actively in the critical reasoning that is needed to resolve issues of medical ethics as they pertain to the care of the patient. It was written by a philosopher and two practicing physicians who have had a long interest in patient care, in the evaluation of ethical issues in medicine, and in the teaching of medical ethics. Self help in medical ethics.

The paper traces the development of programmes of lectures and symposia in most British medical schools, which have developed into a postgraduate programme of research and publication. It is asserted that a morally neutral approach is a necessary prerequisite for wide multidisciplinary involvement, as is a high level of student participation in identifying topics for discussion. Alternative possibilities for formal teaching are discussed and pitfalls highlighted.

Ethical problems in arguments from potentiality.

If potentiality of an entity is accepted as intrinsic to its reality, it will not lead to fallacious conflations and misuses that distort some of the most central ethical issues in medicine. Potentiality is usually invoked when decisions concerning human beings that are not persons are at stake: embryos, fetuses, defective children, severely deteriorated or diseased adults. Since the entity to be decided on cannot participate in the decision process, vicarious argumentation is brought in. Because there is marked reticence to place values on these human beings, one is often led to flee into argumenting from potentiality or, in the worst of cases, from a benign consideration of an unexpected future. If this argumentation from potentiality must be recurred to, it will best serve the interests of the involved human being to strictly focus on the potentiality he/she literally in-corporates. Conflating possible developments due to interactions of medical or social institutions leads to presenting a future that may very well not obtain if the multifactorial analysis does not fulfill itself.

Splicing Life, with Scalpel and Scythe

Two recent reports on the social and moral implications of recombinant DNA research and "genetic engineering" are compared: Splicing Life: A Report on the Social and Ethical Issues of Genetic Engineering with Human Beings, by the President's Commission for the Study of Ethical Issues in Medicine and Biomedical and Behavioral Research, and a shorter Study Paper of Bioethical Concerns, by the Panel on Bioethical Concerns of the National Council of Churches of Christ (NCC). The President's Commission report is seen as more sophisticated, better organized, carefully reasoned, and clearly focused on imminent clinical uses of genetic research. The NCC report is described as more diffuse, concerned with remote and dramatic applications, and heavily exhortatory.

Philosophy in Medicine: Conceptual and Ethical Issues in Medicine and Psychiatry

This relatively small (201 pages) paperback book is a gem. It may be difficult to read for those uninitiated in philosophy and logic, but it is invaluable to practitioner of medicine for understanding clearly what she is doing. There are so many issues that physicians take for granted inthe treatment of patients that Drs Culver and Gert analyze and criticize. The authors acknowledge that the book could not have been written by either of us working separately, and indeed, it is a result of collaboration of two disciplines—philosophy and medicine (psychiatry)—that is unique. The authors, professors at Dartmouth Medical School and Dartmouth College, have written extensively on philosophy and medicine and in this book provide definitions for rationality, competence, and maladies. They define and discuss these issues along with paternalism in medicine in a manner designed to alert practitioner to what he does with patients and why

A Theory of Medical Ethics

Veatch's volume now takes its place in the third generation of works on medical ethics to appear since this field was revitalized in the 1960s. The first-generation books, mostly collections of essays, called attention to the importance of ethical issues in medicine and summarized current physician thinking. The second generation, to which Veatch made notable contributions (as associate for medical ethics at the Hastings Center, before assuming his current position as professor of medical ethics at the Kennedy Institute of Ethics at Georgetown), produced single-author texts or carefully edited anthologies, isolating central principles and subjecting first-generation views to critical scrutiny. We are now in need of treatises like this one that look beneath and beyond case- and issue-oriented analyses to discern the theoretical underpinnings of a true system of medical ethics. Should the public decide?

The era of medical paternalism is past and the public must be involved in the moral debate over ethical issues in medicine. Thomas, Chairman of the British Medical Association's Central Ethical Committee, agrees with these points made by Ian Kennedy in the 1980 Reith Lectures on medical ethics, but rejects the latter's emphasis on consumerism and stricter regulation of physicians. It is the public's responsibility to provide the general moral framework within which physicians make clinical decisions, but the public must then trust the medical profession which in turn must not abuse that trust.
