Leptospira borgptersenii and Leptospira interrogans identified in wild mammals in Rio Grande do Sul, Brazil.

Leptospira spp. are bacteria responsible for leptospirosis, a zoonotic disease with considerable impacts on the economy, animal health, and public health. This disease has a global distribution and is particularly prevalent in Brazil. Both rural and urban environments are habitats for Leptospira spp., which are primarily transmitted through contact with the urine of infected animals. Consequently, domestic and wild species can harbor these prokaryotes and serve as infection sources for other hosts. In the context of wild animals, there is a dearth of molecular studies elucidating the roles of various animal and bacterial species in the epidemiology of leptospirosis. Therefore, this study aimed to evaluate the presence of Leptospira spp. DNA in different species of free-living and captive wild animals and to assess the phylogenetic relationships of the identified microorganisms in Rio Grande do Sul, Brazil. The samples were evaluated for the presence of the gene lipL32 by polymerase chain reaction (PCR) and sequencing of the amplified fragment after which phylogenetic analyzes were carried out. DNA from Leptospira spp. was extracted from kidney tissue from wild animals (Mammalia class). Pathogenic Leptospira spp. DNA was detected in 9.6% (11/114) of the samples, originating from nine species of wild animals, including the white-eared opossum (Didelphis albiventris), skunk (Conepatus chinga), geoffroy's cat (Leopardus geoffroyi), margay (Leopardus wiedii), pampas fox (Lycalopex gymnocercus), capybara (Hydrochoerus hydrochaeris), common marmoset (Callithrix jacchus), neotropical river otter (Lontra longicaudis), and european hare (Lepus europaeus). Phylogenetic analysis revealed the presence of Leptospira borgpetersenii and Leptospira interrogans in these animals. This research is the first study contributing to the epidemiology of leptospirosis by identifying L. borgpetersenii and L. interrogans in free-living and captive wild animals in Rio Grande do Sul, Brazil, potentially acting as bacterial reservoirs. Additionally, our findings can inform sanitary measures for controlling and preventing the disease, thereby safeguarding public health.

The Role of Transplacental Infection in Leptospira spp. Epidemiology in Cattle in Caatinga Biome, Brazil.

Leptospirosis is an infectious disease that affects domestic animals, wild animals, and humans. It represents a public health problem and has an important economic impact on livestock. This study aims to investigate the importance of genital and transplacental infection in the epidemiology of leptospirosis in cows maintained in Caatinga biome conditions, Northeastern Brazil, as well as reporting organs colonized by Leptospira spp. in embryos and fetuses. Blood, urinary tract (urine, bladder, and kidney), and reproductive tract (vaginal fluid, uterus, uterine tube, ovary, and placenta) samples were collected from 15 slaughtered pregnant cows. Two embryos and 13 fetuses were sampled. Central nervous system and choroid ovoid samples were collected from embryos. Blood, central nervous system, lung, peritoneal liquid, abomasal content, liver, spleen, urine, bladder, kidney, and reproductive system samples were collected from fetuses. Diagnostic methods included the microscopic agglutination test (MAT) using a collection of 24 serovars belonging to 17 different pathogenic serogroups of five species as antigens, as well as polymerase chain reaction (PCR). Anti-Leptospira spp. antibodies were found in 9 cows (60%), while 13 cows (86.67%) had at least one organ or urine with leptospiral DNA. No fetus was seroreactive. Among the embryos and fetuses, 13 (86.67%) presented leptospiral DNA, proving a high frequency of transplacental infection (100%). For cows, the most frequent biological materials regarding Leptospira spp. DNA detection were placenta (13 out of 15 samples; 86.7%), uterus (10 out of 15 samples; 66.7%), and vaginal fluid (5 out of 15 samples; 33.3%), while, for fetuses/embryos, the most frequent PCR-positive samples were choroid ovoid (1/2; 50%), spleen (6/13; 46.2%), kidney (5/13; 38.5%), and central nervous system (5/15; 33.3%). Sequenced samples based on the LipL32 gene presented 99% similarity with L. borgpetersenii. The results indicate that transplacental infection is an efficient way of spreading Leptospira spp. in cows maintained in Caatinga biome conditions. Therefore, prevention and control strategies must include actions that interrupt transmission through this alternative route.

Genotyping of Leptospira spp. in wild rats leads to first time detection of L. kirshneri serovar Mozdok in Serbia.

This study aimed to investigate the prevalence and molecular characterization of Leptospira species in Belgrade, Serbia, an area where this disease is underexplored. Specifically, the study sought to employ molecular and multilocus sequence typing analyses to fill the gap in understanding the diversity and distribution of Leptospira species within the region. A comprehensive molecular analysis was conducted on kidney samples obtained from Norway rats (Rattus norvegicus) in the urban environment. The study utilized molecular diagnostic techniques including real-time PCR targeting the lipL32 gene and performing sequence-based typing schemes utilizing adk, icdA, lipL32, lipL41, rrs2, and secY genes. These methodologies were applied to ascertain the presence and characterize different Leptospira species and serovars, respectively. The findings revealed the presence of two Leptospira species and three separate serovars in the Belgrade area. This study identified the presence of L. kirschneri serovar Mozdok in Serbia for the first time, a significant discovery previously undocumented in the region. This pioneering investigation sheds light on the molecular diversity and prevalence of Leptospira species in Serbia. The study underscores the importance of employing molecular typing methods to gain insights into the epidemiology and characterization of Leptospira species. These findings significantly contribute to both local and global perspectives on leptospirosis epidemiology, providing vital insights for the development of effective control strategies and interventions. In our recent study, we explored the presence and performed molecular typing of the Leptospira species, the bacteria responsible for leptospirosis, in wild rats in Serbia. This was the first time such a study was conducted in the region. Leptospirosis is a serious disease that affects both animals and humans, often transmitted through contact with water contaminated by infected animals. Our focus was on understanding which types of Leptospira were present in these animals. Excitingly, we discovered a particular strain of Leptospira, known as L. kirshneri serovar Mozdok, for the first time in Serbia. This finding is significant because it sheds light on the presence and spread of different Leptospira serovars in Serbia. It also raises awareness about the potential health risks associated with this serovar, which was previously unknown in the area. Our work fits into a broader context of disease surveillance and public health. By identifying the types of Leptospira present in a specific region, we can better understand the risks to public health and take steps to prevent and control the spread of leptospirosis. This discovery is not just important for scientists studying infectious diseases; it has real implications for public health officials, veterinarians, and anyone concerned with preventing and treating leptospirosis. Our findings highlight the need for ongoing monitoring of Leptospira in wildlife and synanthropic fauna, to protect both animal and human health.

First isolation and genotyping of pathogenic Leptospira spp. from Austria

Leptospirosis is a globally distributed zoonotic disease. The standard serological test, known as Microscopic Agglutination Test (MAT), requires the use of live Leptospira strains. To enhance its sensitivity and specificity, the usage of locally circulating strains is recommended. However, to date, no local strain is available from Austria. This study aimed to isolate circulating Leptospira strains from cattle in Austria to enhance the performances of the routine serological test for both humans and animals. We used a statistical approach combined with a comprehensive literature search to profile cattle with greater risk of leptospirosis infection and implemented a targeted sampling between November 2021 and October 2022. Urine and/or kidney tissue were sampled from 410 cattle considered at higher risk of infection. Samples were inoculated into EMJH-STAFF culture media within 2–6 h and a real-time PCR targeting the lipL32 gene was used to confirm the presence/absence of pathogenic Leptospira in each sample. Isolates were further characterised by core genome multilocus sequence typing (cgMLST). Nine out of 429 samples tested positive by PCR, from which three isolates were successfully cultured and identified as Leptospira borgpetersenii serogroup Sejroe serovar Hardjobovis, cgMLST cluster 40. This is the first report on the isolation and genotyping of local zoonotic Leptospira in Austria, which holds the potential for a significant improvement in diagnostic performance in the country. Although the local strain was identified as a cattle-adapted serovar, it possesses significant zoonotic implications. Furthermore, this study contributes to a better understanding of the epidemiology of leptospirosis in Europe.

Epidemiological Status of Leptospirosis in India

A corkscrew-shaped spirochaete named Leptospira interrogans causes the infectious disease called leptospirosis. Leptospirosis, a growing public health concern worldwide, from subclinical infections to potentially lethal pulmonary hemorrhage. The disease caused by the pathogenic Leptospira, poses a threat to both humans and animals, and its transmission occurs through contact with infected animals, contaminated water, and soil. The prevalence of leptospirosis is influenced by various factors, including climate, urban development, and animal-rearing practices. It can manifest with severe symptoms in humans, making early diagnosis crucial. Diagnostic methods like microscopic agglutination test (MAT) and enzyme-linked immunoassay (ELISA) are widely used for the screening of leptospira infection. Molecular technique like PCR and qPCR offering higher sensitivity and rapidity. This paper meta-analyses the incidence of leptospirosis in various animals based on the literature published from 2005 to 2023 and provides prevalence of the disease in various animal including humans. Results suggest the significance prevalence of the disease in humans and various animal species, namely buffalo, rodents, and dogs. Coastal regions in India were particularly vulnerable to the disease. Efforts to control leptospirosis include surveillance programs and public health initiatives. Understanding the epidemiology and prevalence of leptospirosis, as highlighted in this paper, is essential for implementing effective preventive measures. Finally, a continued research, diagnostic advancements, public awareness campaigns and addressing research gaps in epidemiology of the disease are critical in mitigating the impact on human and animal health. This review provides important data for public health authorities, veterinarians, scientists and for the public, in general.

A cluster of leptospirosis cases associated with crocodile workers in the Northern Territory, Australia, 2022.

Leptospirosis is a worldwide zoonotic waterborne disease endemic in tropical and subtropical climates. Outbreaks have been observed in the Northern Territory (NT) of Australia. We briefly described the epidemiology of leptospirosis in the NT between 2012 and 2022, and undertook an investigation of a cluster of three leptospirosis cases observed in crocodile workers between January and December 2022 in the Top End of the NT. A descriptive case series was conducted to investigate the cluster; all three cases were male and non-Aboriginal with a median age of 46.5 years; none took chemoprophylaxis; only one of the three cases reported wearing appropriate protective attire; all reported receiving limited to no education about personal protective measures from their associated workplaces. Higher than average rainfall in both February and December 2022 likely contributed to the increased risk of infection in those months. Changing climate patterns are likely to result in more frequent periods of heavy rain, and risk of contracting leptospirosis in the NT may increase, particularly for those who work in wet and muddy conditions. Promoting the use of protective workplace clothing and equipment, the use of waterproof dressings for skin abrasions, regular hand hygiene, and the consideration of chemoprophylaxis in certain circumstances may prevent future cases.

Phylogenomics of Leptospira santarosai, a prevalent pathogenic species in the Americas.

Leptospirosis is a complex zoonotic disease mostly caused by a group of eight pathogenic species (L. interrogans, L. borgpetersenii, L. kirschneri, L. mayottensis, L. noguchii, L. santarosai, L. weilii, L. alexanderi), with a wide spectrum of animal reservoirs and patient outcomes. Leptospira interrogans is considered as the leading causative agent of leptospirosis worldwide and it is the most studied species. However, the genomic features and phylogeography of other Leptospira pathogenic species remain to be determined. Here we investigated the genome diversity of the main pathogenic Leptospira species based on a collection of 914 genomes from strains isolated around the world. Genome analyses revealed species-specific genome size and GC content, and an open pangenome in the pathogenic species, except for L. mayottensis. Taking advantage of a new set of genomes of L. santarosai strains isolated from patients in Costa Rica, we took a closer look at this species. L. santarosai strains are largely distributed in America, including the Caribbean islands, with over 96% of the available genomes originating from this continent. Phylogenetic analysis showed high genetic diversity within L. santarosai, and the clonal groups identified by cgMLST were strongly associated with geographical areas. Serotype identification based on serogrouping and/or analysis of the O-antigen biosynthesis gene loci further confirmed the great diversity of strains within the species. In conclusion, we report a comprehensive genome analysis of pathogenic Leptospira species with a focus on L. santarosai. Our study sheds new light onto the genomic diversity, evolutionary history, and epidemiology of leptospirosis in America and globally. Our findings also expand our knowledge of the genes driving O-antigen diversity. In addition, our work provides a framework for understanding the virulence and spread of L. santarosai and for improving its surveillance in both humans and animals.

Demystifying re-emergence zoonotic threat: A 5-year epidemiology of leptospirosis in Negeri Sembilan, Malaysia

IntroductionLeptospirosis is a re-emergence of zoonotic disease with significant morbidity and mortality. As a tropical country, there is a concerning trend of leptospirosis infection in Malaysia. However, the epidemiological study of the infection remains scarce and does not reflect the current information. Therefore, the study aimed to describe its epidemiology within the last 5-year in the state of Negeri Sembilan. MethodsA cross-sectional study was done using dataset from national electronic surveillance registry from year 2018–2022. Descriptive statistics method was applied to build the epidemiological narrative of leptospirosis infection. All data was kept confidential and abide to Helsinki declaration. ResultsA total of 1290 cases of leptospirosis was registered and included in the analysis. Majority of the cases were from adults’ group (68.4 %), male (68.4 %), of Malay ethnicity (69.8 %), unemployed (56.7 %), and from rural area (54.4 %). The possible risk exposure was from rat infestation (15.1 %), water-based activity (12.5 %), and contact with contaminated soil (7.7 %). The proportion of cases registered in the densely populated areas were comparable among those who are employed and unemployed There were overall 21 (1.6 %) mortality cases from the data. ConclusionLeptospirosis is a public health threat and need to be approached holistically. Awareness of the disease need to be continuously advocated to the public as well as to the healthcare providers. Strengthening the surveillance system at the local and national level is important as it can act as an early warning system to prevent outbreak to happen.

Genetic Evidence for a Potentially New Pathogenic Leptospira sp. Circulating in Bats from Brazilian Amazon

Leptospirosis is a neglected zoonosis frequently reported worldwide, caused by a spirochete of the genus Leptospira. It is capable of infecting domestic animals, free-living animals, and humans. Among wild animals, the role of bats in the epidemiology of leptospirosis has been investigated but is far from being elucidated. The Amazonian biome has the ideal conditions for maintaining and disseminating leptospires and, despite efforts that have been made to better understand leptospires’ occurrence in wild animals in the region, few studies aimed to explore and genetically characterize leptospires in bats. Based on this, the aim of the present study is to deeper investigate and genetically characterize leptospires detected in bats from the southwest Amazon. Animals were trapped by mist netting at five sites in the state of Acre, Brazil. Kidney samples were obtained and stored for molecular analysis. Polymerase chain reaction (PCR) was conducted first based on the LipL32 gene, and positive samples were submitted to rrs and secY-PCR and sequencing. Sequences were then submitted to phylogenetic analyses through multiple bioinformatic tools. The rrs sequences from the present study formed one single haplotype, different from any other previously deposited, grouped in a highly supported cluster with sequences from bats from Madagascar and China. The initial secY screening revealed no identity with previously deposited sequences. The phylogenetic trees revealed the sequences from the present study in an isolated branch, clearly separated from all previously known pathogenic Leptospira spp., suggesting the existence of a potentially undescribed species. The haplotype network including only leptospires from the Amazon region confirmed two new haplotypes from the same taxon unity, isolated from the others, with a probable origin of the species from L. noguchii. The characterization of this potentially new species in bats reinforces the complexity of the transmission dynamics of leptospires, including wild, periurban, and urban environments, emphasizing the need for an integrative look at leptospirosis vigilance within the context of One Health.

Introduction. 2023 marks the 225th anniversary of the birth of Karl Karlovich Seidlitz, a prominent Russian scientist, internal medicine doctor, professor of the Medical and Surgical Academy, one of the reformers of medical education and the founder of the clinical and anatomical method in Russian medicine, the founder of the department of the Faculty academic therapeutic Clinic, a close friend and biographer of the poet V.A. Zhukovsky. Аim. Is to show the role of K.K. Seidlitz in the development of clinical medicine and education in the Russian Empire. The appeal to his memory in the jubilee year allows us to preserve the continuity of generations. Material and methods. The study uses general methodological approaches for historical and medical research. The analysis of literary sources on the history of medicine is carried out, articles and materials revealing the priority and importance of the works of K.K. Seidlitz for medical science and the history of culture of Russia are studied. Results and discussion. The contribution of K.K. Seidlitz to the fight against infectious diseases in the Russian Empire is analyzed. K.K. Seidlitz was one of the first to describe the clinic and epidemiology of leptospirosis. Having headed the therapeutic clinic of the St. Petersburg Medical and Surgical (now Military Medical) Academy, he was one of the first in Russia to introduce differential diagnostics, methods of physical research, created an outpatient clinic and a laboratory at the clinic for conducting research using microscopes discharged from Paris, introduced pathoanatomical studies of deceased patients, etc. Conclusion. In modern literature, the name of K.K. Seidlitz is primarily associated with the first detailed in modern literature, the name of K.K. Seidlitz is primarily associated with the first detailed systematized biography of the poet V.A. Zhukovsky written by him. However, K.K. Seidlitz has great merits in the field of the development of clinical medicine in Russia and its teaching, which the reader should be reminded of.

Leptospirosis in Ecuador: Current Status and Future Prospects.

The location of Ecuador-an equatorial nation-favors the multiplication and dispersal of the Leptospira genus both on the Pacific Coast and in the Amazon tropical ecoregions. Nevertheless, leptospirosis epidemiology has not been fully addressed, even though the disease has been recognized as a significant public health problem in the country. The purpose of this literature review is to update knowledge on the epidemiology and geographical distribution of Leptospira spp. and leptospirosis in Ecuador to target future research and develop a national control strategy. A retrospective literature search using five international, regional, and national databases on Leptospira and leptospirosis including humans, animals, and environmental isolations of the bacteria and the disease incidence in Ecuador published between 1919 and 2022 (103 years) with no restriction on language or publication date was performed. We found and analyzed 47 publications including 22 of humans, 19 of animals, and two of the environments; three of these covered more than one of these topics, and one covered all three (i.e., One Health). Most (60%) of the studies were conducted in the Coastal ecoregion. Twenty-four (51%) were published in international journals, and 27 (57%) were in Spanish. A total of 7342 human and 6314 other animal cases were studied. Leptospirosis was a frequent cause of acute undifferentiated febrile illness in the Coast and Amazon and was associated with rainfall. All three major clusters of Leptospira-pathogenic, intermediate, and saprophytic-were identified from both healthy and febrile humans, the environment, and animals; moreover, nine species and 29 serovars were recorded over the three Ecuadorian ecoregions. Leptospira infections were diagnosed in livestock, companion, and wild animals from the Amazon and the Coast regions along with sea lions from the Galápagos Islands. Microscopic-agglutination test was the diagnostic tool most widely used. Three reviews covering national data on outpatients and inpatients determined the varied annual incidence and mortality rate, with males being more commonly affected. No human cases have been reported in the Galápagos Islands. Genomic sequences of three pathogenic Leptospira were reported. No studies on clinical ground, antibiotic resistance, or treatment were reported, nor were control programs or clinical-practice guidelines found. The published literature demonstrated that leptospirosis was and still is an endemic disease with active transmission in the four geoclimatic regions of Ecuador including the Galápagos Islands. Animal infections, distributed in mainland and insular Ecuador, pose a significant health risk for humans. Nationwide epidemiological surveys-encouraging more research on the fauna and environment with appropriate sampling design on risk factors for human and animal leptospirosis, Leptospira genotyping, increased laboratory capability, and readily available official data-are required to improve our understanding of transmission patterns and to develop effective national intervention strategies with the intention of applying One Health approaches.

Bovine Leptospirosis in Caatinga Biome, Brazil: New Insights into Diagnosis and Epidemiology.

Bovine leptospirosis causes economic losses and raises public health concerns. It is possible that there are peculiarities in the epidemiology of leptospirosis in regions with a semiarid climate, such as the Caatinga biome in Brazil, where the climate is hot and dry, and the etiological agent require alternative routes of transmission. This study aimed to close knowledge gaps to the diagnosis and epidemiology of Leptospira spp. infection in cows from the Caatinga biome, Brazil. Samples of the blood, urinary tract (urine, bladder and kidney) and reproductive tract (vaginal fluid, uterus, uterine tube, ovary and placenta) were collected from 42 slaughtered cows. Diagnostic tests included were the microscopic agglutination test (MAT), polymerase chain reaction (PCR) and bacterial isolation. Anti-Leptospira spp. antibodies were found in 27 (64.3%) of the animals analyzed using MAT at a 1:50 dilution (cut-off 50), while 31 (73.8%) animals had at least one organ/fluid where the presence of Leptospira spp. DNA was identified, and 29 animals (69%) were positive at bacteriological culture. The highest sensitivity values for MAT were obtained at the cut-off point of 50. In conclusion, even under hot and dry climate conditions, it is possible that Leptospira spp. can spread through alternative routes such as venereal transmission; moreover, a cut-off of 50 is recommended for the serological diagnosis of cattle from the Caatinga biome.

Epidemiology of Human Leptospirosis in Bantul District, Yogyakarta Province, Indonesia

Bantul District has the highest number of leptospirosis cases in Yogyakarta Province. Transmission of leptospirosis occurs primarily through exposure to water and soil contaminated with the urine of infected animals. This study aims to describe the epidemiological characteristics of leptospirosis in Bantul District, Yogyakarta. This is cross-sectional research. Leptospirosis cases were collected from active and passive detections. Selected positive cases underwent home visits for interviews about epidemiological data. Rat surveys were carried out to find out the existing rat species and successful traps in the study area. The most leptospirosis cases were found at Kasihan public health center. There were 74 suspect cases, 4 probable cases, and 15 cases positively confirmed. The cases increased from January to April and from November - December. Cases in males were more related to his work as a farmer. Fever, headache and muscle aches are the most common symptoms felt by leptospirosis sufferers. The most caught rats was Rattus tanezumi. Leptospirosis epidemiological information can be used as a basis or reference for Bantul District Health Office in prevention and control of leptospirosis in priority areas

Leptospirosis in Vietnam

Leptospirosis poses a serious public health problem in many countries because about 1 million people suffer annually from this severe, often fatal, infection. At present, its importance has been increasing due to development of integration processes, globalization and intensification of economic activities, inevitable anthropogenic transformation of landscapes as well as rise in number of people with occupational risk of pathogen exposure. The burden of human leptospirosis is expected to rise with demographic shifts and climate change that result in heavy rainfall and flooding. Regarding leptospirosis, Southeast Asia, including Vietnam, is among the most vulnerable global regions. The objective of the review was to analyze available published data on the spread of leptospirosis in Vietnam. More than 100 research articles were analyzed allowing to state that in Vietnam synanthropic rodents (rats), agricultural (buffaloes, cattle, pigs) and domestic animals (cats, dogs) are the main sources of leptospirosis infection in humans. It is essential that rats (cats and dogs also) in Vietnam are used for food. Among all farm animals, pigs are of maximal importance as a source of infection in humans. In Vietnam, Leptospira prevalence in pigs varies widely, up to 73%. It depends significantly on the location, farm size, age and gender of animals, etc. A large variety of Leptospira serovars have been identified in pigs in Vietnam: Tarassovi, Bratislava, Australis, Javanica, Autumnalis, Grippotyphosa, and some others. Official Vietnam reporting of leptospirosis cases based on clinical data does not reflect the real-life incidence rate in humans. However, the available seroprevalence studies reveal that, on average, about 10% of the Vietnamese population are infected with Leptospira. Work in agriculture is the dominant risk factor for acquiring this infection. The highest Leptospira seroprevalence rates were detected in farmers, 63.2%. Serovar diversity is typical for pathogenic Leptospira circulating in Vietnam. In humans, the most frequent are antibodies to Hebdomadis, Wolffi and Icterohaemorrhagiae, but the frequency varied markedly across the country depending on the site, possibly with relation to the environment, as well as with economic activity of the local population. In Vietnam, geographic, climatic and social differences between northern, central and southern regions contribute to the specificity of local leptospirosis epidemiology. This emphasizes the advisability of developing special measures for leptospirosis prevention taking into account regional peculiarities.

Evidence of orthohantavirus and leptospira infections in small mammals in an endemic area of Gampaha district in Sri Lanka

BackgroundOrthohantaviruses and leptospira are emerging zoonotic pathogens of high public health significance. The epidemiology of orthohantavirus infections and leptospirosis is similar and presents related clinical pictures in humans. However, a paucity of data on actual reservoir hosts for orthohantaviruses and leptospira exists. Therefore, this study aimed at determining the occurrence of orthohantaviruses and leptospira in small mammals captured in an endemic region of Sri Lanka.MethodsRodents and shrews were morphologically and/or genetically identified using morphological keys and DNA barcoding techniques targeting the cytochrome oxidase b subunit gene (Cytb). Lung tissues and sera were subsequently analyzed for the presence of orthohantavirus RNA using qRT-PCR. Sera of rats were tested for IgG antibodies against orthohantaviruses and leptospira.ResultsForty-three (43) small mammals representing: Rattus (R.) rattus (black rat) or R. tanezumi (Asian rat), Suncus murinus (Asian house shrew), R. norvegicus (brown rat) and Mus musculus (house mouse) were investigated. No orthohantavirus RNA was detected from the lung tissue or serum samples of these animals. Elevated levels of IgG antibodies against Puumala orthohantavirus (PUUV) and/or Seoul orthohantavirus (SEOV) antigens were detected in sera of 28 (72%) out of the 39 rats analysed. Interestingly, 36 (92%) of the 39 rats also showed presence of anti leptospira-IgG antibodies in their serum, representing dual infection or dual exposure in 26/39 (66.7%) of examined rats.ConclusionsThis project targets important public health questions concerning the occupational risk of orthohantavirus infections and/or leptospirosis in an endemic region of Sri Lanka. Most rats (72%) in our study displayed antibodies reacting to orthohantavirus NP antigens, related to PUUV and/or SEOV. No correlation between the orthohantavirus and leptospira IgG antibody levels were noticed. Finally, a combination of both morphological and DNA barcoding approaches revealed that several species of rats may play a role in the maintenance and transmission of orthohantavirus and leptospira in Sri Lanka.

A systematic review and meta-analysis of the epidemiology of Leptospirosis in HIV uninfected and in people living with HIV from the Southern African Development Community.

Leptospirosis is an occupational, neglected febrile disease of bacterial origin transmitted between humans and animals. In this manuscript we summarize available data on Leptospira infection in HIV uninfected and in people living with HIV from the Southern African Development Community (SADC) countries, identifying gaps in knowledge and recommend future research priorities. Articles published between 1990 and 2021 were accessed by an online search of Google Scholar and Medline/PubMed performed between February 2020 and July 2022. The STATA program was used for the Meta-analysis. Pooled prevalence values with 95% confidence intervals and heterogeneity were determined. Thirty studies from eight SADC countries, reporting the prevalence on Leptospira were reviewed. A pooled prevalence of 19% (CI: 13-25%), a heterogeneity level of 96% and index score ranging from 2 to 9 was determined. Only four (4) studies reported HIV co-infection status. Three species of Leptospira (Leptospira interrogans (4), L. kirschneri (3), Leptospira borgpetersenii (1) and 23 serogroups were identified. The most frequently reported serogroups were Icterohaemorrhagiae (13), Grippotyphosa and Australis (10) followed by Sejroe (8). Studies on human leptospirosis in the SADC region are scarce, especially in people living with HIV. Additional studies aimed at determining the prevalence and the role of the pathogen in people living with HIV, including detailed clinical, molecular and demographic data are recommended.

Global prevalence and epidemiology of leptospirosis in domestic cats, a systematic review and meta-analysis

Research worldwide describes different prevalences of leptospirosis in domestic cats. The aims of this study were: to perform a systematic review to determine epidemiologic characteristics of published observational studies on leptospirosis, and to obtain pooled measures of the global prevalence of the disease with a meta-analysis for studies using indirect diagnostic tests (M1) and direct diagnostic tests (M2). A bibliographic search to find observational epidemiological studies reporting the prevalence of leptospirosis in cats was performed. The geographic location and decade of publication of the studies, serovars, serogroups and factors associated with Leptospira infection were summarized through a systematic review. Meta-analyses were achieved to obtain the pooled global prevalence (M1 and M2), prevalences according to the continent and origin of the sampled cats. 139 articles were found, and 93 were selected. In 16 articles, Leptospira serogroups were recorded and seropositivity was more frequent for Icterohaemorragiae, Australis, Autumnalis, Pomona. In 59 articles, the serovars were reported, being the most common reactions for Pomona, Grippotyphosa and Canicola. The M1 included 83 articles; the pooled prevalence was 11.09 % (95 % CI = 8.68–13.73). The M2 comprised 23 studies and the pooled prevalence was 9.22 % (95 % CI = 4.30–15.41). A subgroup analysis revealed higher pooled prevalences in Oceania and Europe, shelter, rural and outdoor cats in studies using indirect diagnostic tests and in Asia, stray cats and veterinary hospital patients in studies with direct tests.

Prevalence of anti-leptospiral IgM and detection of pathogenic Leptospira species DNA in neonates presenting with clinical sepsis in Southwestern Uganda

BackgroundLeptospirosis is an emerging neglected zoonotic disease that presents with nonspecific signs/symptoms and it can be mistaken for other diseases. Owing to limited diagnostic capacity and unawareness, the data on human leptospirosis particularly in neonates are scarce in many sub-Saharan countries. It has been underreported hindering preventive and control measures in place. The study aimed at determining prevalence of leptospirosis as a cause of febrile illness in neonates using IgM ELISA and a quantitative real-time PCR (qPCR).MethodsThis was a descriptive cross-sectional study that included 103 neonatal sepsis cases whose parents/legal guardians gave informed consent. The data on demographic and clinical characteristics were collected using structured data collection form. EDTA whole blood sample was collected from the neonates by trained study nurses. From the samples, IgM ELISA was done using automated analyzers, DNA extracted and qPCR was performed using primers for LipL32, specific for the pathogenic leptospires.ResultsThe prevalence of anti-leptospiral IgM among the neonates as determined by ELISA was 4.3%, where all of them presented with lethargy and poor feeding. No pathogenic Leptospira species DNA was amplified by qPCR.ConclusionsEvidence of leptospirosis was demonstrated in neonatal sepsis cases in this study. The findings suggest considerations of leptospirosis in the differential diagnosis of neonates with sepsis. More data are needed on the real epidemiology, clinical features, and burden of leptospirosis in neonates. There is need to include intermediate pathogenic species of Leptospira in the diagnostic qPCR assays.

Understanding leptospirosis: application of state-of-the-art molecular typing tools with a One Health lens.

Leptospirosis is an archetypal One Health problem as described in the companion Currents in One Health article in the October 2022 issue of the Journal of the American Veterinary Medical Association by Sykes et al. A thorough understanding of leptospirosis requires a detailed analysis of the elaborate interplay among pathogenic leptospiral strains, host species, and the environment. Such an understanding is required to inform appropriate preventative measures including vaccine design, prophylaxis efforts, educational programs that help to reduce exposure to pathogenic spirochetes, as well as policy development. Because of the complex epidemiology of leptospirosis, a One Health approach as defined by the One Health Initiative Task Force is critical-an approach that calls for "the collaborative efforts of multiple disciplines working locally, nationally, and globally, to attain optimal health for people, animals and our environment." Over the last three decades, progressive advances in cutting-edge molecular typing techniques, as well as our ability to rapidly generate and share large amounts of sequence data through establishment and growth of databases, have been central to accelerating a One Health understanding of the epidemiology of leptospirosis. Nevertheless, our dependence on serotype information because of the serovar-specific nature of current vaccines means that laborious serotyping efforts continue. With the advent of new approaches such as mRNA vaccines that are based on lipopolysaccharide immunogens, sequence- and/or proteomics-based typing methods may replace these methods.

Seroprevalence of acute leptospirosis in a tertiary care hospital of western India

Background: Leptospirosis is a widespread re-emerging zoonotic disease, especially in developing countries. According to World Health Organization, despite being severe, the disease is neglected in most endemic countries because of a lack of information and awareness about the extent of the problem. Objective: The objective was to study the seroprevalence and epidemiology of leptospirosis with its biochemical correlation in the general population in Pune, Maharashtra. Materials and Methods: It is a retrospective observational laboratory-based study over 1 year from January to December 2020 in a tertiary care hospital in western India. A total of 561 blood specimens received during 1 year for the diagnosis of fever were processed for Leptospira IgM antibodies using enzyme-linked immunosorbent assay (ELISA). Results: A total of 111 of 561 blood specimens that were received during the study period tested positive for IgM antibody against Leptospira. The seroprevalence of Leptospira spp. was found to be 19.78%. In seropositive patients, the age ranged from 4 to 77 years. The highest seropositivity was observed in the age group of 21–30 years (42%). There were 34% (n = 38) males and 66% (n = 73) females, among those who tested positive. Seroprevalence in Pune was highest in the months of June–August. Renal function tests were deranged in 44 (39.63%) patients (serum creatinine > 1.5 mL). Hyponatremia and hypokalemia were observed in 37.84% and 21.62% of patients, respectively. In liver function tests, serum glutamic oxaloacetic transaminase (SGOT) and serum glutamic pyruvic transaminase (SGPT) both were deranged (SGOT > 40 U/L; SGPT > 56 U/L) in 36 (32.43%) patients, whereas SGOT alone was deranged in additional 18 patients (48.64%). Serum bilirubin was deranged in 65.45%. Coinfection was observed with chikungunya, dengue, and malaria in 7.2%, 6.3%, and 0.90% of patients, respectively. Conclusion: Because morbidity is high in this infection, early diagnosis of leptospirosis is essential because antibiotic therapy provides the greatest benefit when initiated early in the course of illness.
