Left atrial function after thoracoscopic left atrial appendage exclusion as standalone and as combined with epicardial atrial fibrillation ablation: echocardiographic findings at one- year follow up

Abstract Funding Acknowledgements Type of funding sources: None. Introduction Left atrial appendage (LAA) is the most common source of thrombi in patients with atrial fibrillation (AF). The Totally Thoracoscopic (TT)-LAA exclusion with epicardial clip is an effective procedure, however the effects on left atrial (LA) function remain unknown. Purpose The aim of this study was to assess the impact of TT-LAA exclusion on the LA function in patients undergoing to standalone TT-LAA exclusion and patients received combined procedure, namely: TT-LAA exclusion and Thoracoscopic Epicardial Ablation (TEA). Methods 39 AF Patients underwent to TT-LAA exclusion with the clip. Indication to standalone TT-LAA exclusion was: high risk for ischemic stroke or contraindication to long-term anticoagulant therapy. All patients were screened preoperatively with 3D CT scan, transthoracic and trans-esophageal echocardiography. Intraoperative clip positioning and LAA exclusion were guided by trans-esophageal echo. To evaluate LA function, standard transthoracic echocardiography and 2D strain of LA were performed before surgery, at discharge (3-4 days after the procedure), at 3-month and at 1 year. Routine blood tests and BNP levels were also measured. Results The study enrolled 39 consecutive patients, 25 underwent to standalone TT-LAA exclusion (mean age 78.6 ± 5.7 years, 79% males), and 14 LAA exclusion during TEA (mean age 64.5 ± 8.2 years, 82% males). The mean CHA2DS2-VASc and HASBLED scores were 3.5 and 2.1 respectively. Baseline characteristics are summarized in Figure 1. There were no major complications during the procedure. At median follow-up of 13 months, 1 non cardiovascular death, 1 ischemic stroke and 4 cardiovascular hospitalizations occurred. At 2D strain of LA, the reservoir function decreased significantly after clip implantation and recovered at 3-months follow-up; at 1 year it remained stable. Furthermore, NT-proBNP increased significantly after LAA exclusion with a return to baseline after 3 months. Changes in E/A occurred at predischarge and did not persist at follow up. Echocardiographic findings are summarized in Figure 2. Patients with indication to TEA and LAA exclusion presented a lower LA volume and a lower strain value. Conclusion The main findings of this study can be summarized as follows: 1) The amputation of LAA is a safe procedure at long term follow up. 2) LAA amputation with the clip is associated with acute elevation of BNP levels the days after the procedure with return to baseline at 3 months and one year. 3) LAA amputation impairs the LA reservoir function after the procedure, with recovery after 3 months and remains preserved at one-year. This occurs either when LA strain is measured in sinus rhythm or in AF. 4) LAA exclusion results in a change in E/A that does not persist at three months follow-up and at 1 year. 5) Patients with indication to epicardial AF ablation and LAA exclusion present a lower LA volume with a lower strain value than patients undergoing standalone TT-LAA exclusion.

Retrograde access of the left atrium for catheter ablation using robotic magnetic navigation in atrial fibrillation: a case series

Background Pulmonary vein isolation (PVI) is the cornerstone of atrial fibrillation (AF) ablation for which the left atrium (LA) is usually accessed by the antegrade femoral venous route and transseptal puncture. However, in rare cases, alternative routes must be used to overcome anatomical challenges. A retrograde approach via the femoral artery and by subsequently passing the aortic and mitral valve is virtually impossible using conventional catheters. Purpose To investigate the feasibility of a retrograde approach for AF ablation by robotic magnetic navigation (RMN) using floppy radiofrequency ablation catheters. Methods Between June 2014 and December 2021, a total of 6 consecutive patients underwent a total of 8 ablation procedures for AF by retrograde approach using a RMN at tour center. A magnetic catheter was introduced through the right femoral artery and manually advanced retrogradely to the aortic arch. Thereafter, it was further advanced to the LA using RMN by switching the magnets to navigation mode, passing through both aortic and mitral valve (Figure 1). Circumferential PVI was performed by antral point-by-point radiofrequency ablation and additional atrial lines were targeted when deemed necessary. Bidirectional conduction block was proven by loss of pulmonary vein potentials on the ablation catheter which automatically moved in a radial manner collecting 16 mapping points. Results The patient group (Figure 2) consisted of 4 females and 2 males with a mean age of 58,0±10,8years. 4 patients (67%) presented with persistent AF. The mean time between diagnosis and catheter ablation was 5,3±3,5 years. 2 patients (33%) had an acquired mechanical obstruction of the conventional transseptal route. The other 4 patients (67%) had an anatomical abnormality precluding conventional access to the LA. These 4 patients all underwent an unsuccessful ablation attempt via conventional approach elsewhere, before referral to our center. Patients 3 and 4 also underwent an epicardial ablation for AF respectively 1,3 and 6,2 years before catheter ablation using RMN. Patient 3 also underwent a failed attempt on manual catheter ablation using the retrograde route in another center. AF catheter ablation via retrograde access using RMN was possible in all patients and no complications occurred. The mean follow-up was 3,4±3,3 years. Conclusion Our findings indicate that a retrograde transaortic approach for catheter ablation in AF using magnetic navigation system and electroanatomical mapping is technically feasible and safe in patients with anatomical challenges precluding the use of a conventional antegrade transseptal approach. This technique may serve as an indispensable component of the management of this subgroup of patients in experienced centers.

Thoracoscopic epicardial ablation of atrial fibrillation: Safety, efficacy, single center experience.

Atrial fibrillation (AF) is associated with reduced quality of life and increased risk of ischaemic cerebrovascular events. The left atrial epicardial ablation procedures have evolved towards a successful and safe rhythm control strategy for patients with symptomatic drug-refractory paroxysmal, persistent or post-ablation AF or with a high risk of catheter ablation failure. The aim was to evaluate the efficacy and safety of thoracoscopic ablation at our instituiton. We observed 81 patients undergoing thoracoscopic ablation from January 2015 to December 2019. The mean age was 61.3±8.5 years and the average duration of AF was 3.1±2.6 years. The cohort consisted of 16.5% of paroxysmal AF, 36.7% persistent, and 46.8% of long-standing AF. The procedure was completed in 79 patients; during follow-up, 15 patients (19%) received radiofrequency ablation. Freedom from atrial arrhythmia recurrence was 55.7% after a follow-up (FUP) period of 3.1 ± 1.4 years. At the follow-up visit, sinus rhythm was present in 81% of patients. No relationships between arrhythmia recurrence and BMI, LVEF, left atrial dimension, gender, and AF duration were found. Major complications were noticed in 4 patients (5.0%); 2 had peripheral embolisation, 2 patients were converted to a sternotomy. At the time of the FUP visit, 25.3% of patients were using antiarrhythmic and 74.7% were still using anticoagulants. In the majority of patients, sinus rhythm remained despite a considerable atrial tachycardia recurrence rate, with a relatively low percentage of patients on antiarrythmic drugs.

Multi-spectrum robotic cardiac surgery: Early outcomes.

ObjectiveThe robotic cardiac surgery program at our current institution began in 2013 with an experienced and dedicated team. This review analyzes early outcomes in the first 1103 patients.MethodsWe reviewed all robotic procedures between July 2013 and February 2021. Primary outcomes were mortality and perioperative morbidity. Our robotic approach is totally endoscopic for all cases: off-pump for coronary and epicardial procedures, and on-pump with the endoballoon for mitral valve and other intracardiac procedures.ResultsThere were 1103 robotic-assisted cardiac surgeries over 7 years. A total of 585 (53%) were off-pump totally endoscopic coronary artery bypasses, 399 (36%) intracardiac cases (including isolated and concomitant mitral valve procedures, isolated tricuspid valve repair, CryoMaze, atrial or ventricular septal defect repair, benign cardiac tumor, septal myectomy, partial anomalous pulmonary venous drainage, and aortic valve replacement); 80 (7%) epicardial electrophysiology-related procedures (epicardial atrial fibrillation ablation, left atrial appendage ligation, lead placement, and ventricular tachycardia ablation); and 39 (4%) other epicardial procedures (pericardiectomy, unroofing myocardial bridge). Mortality was 1.2% (observed/expected ratio, 0.7). In the totally endoscopic coronary artery bypass and intracardiac groups, mortality was 1.0% (observed/expected, 0.6) and 1.5% (observed/expected, 0.87), respectively. There were 8 conversions to sternotomy (0.7%) and 24 (2.2%) take-backs for bleeding. Mean hospital and intensive care unit lengths of stay were 2.74 ± 1.26 days and 1.28 ± 0.57 days, respectively.ConclusionsThis experience demonstrates that a robotic endoscopic approach can safely be used in a multitude of cardiac surgical procedures both on- and off-pump with excellent early outcomes. An experienced surgeon and team are necessary. Longer-term follow-up is warranted.

The fate of patients after failed epicardial ablation of atrial fibrillation

BackgroundMuch debate is still going on about the best ablation strategy—via endocardial or epicardial approach—in patients with atrial fibrillation (AF), and evidence gaps exist in current guidelines in this area. More specifically, there are no clear long-term outcome data after failed surgical AF ablation.MethodsSince June 2008, 549 surgical AF ablation procedures through a right minithoracotomy were performed at our institution. From 2008 to 2011, a unipolar radiofrequency device was used (151 patients), whereas from 2011 to 2020 a bipolar radiofrequency device was used (398 patients). Patients were scheduled for surgery on the basis of the following criteria: recurrent episodes of paroxysmal or persistent lone AF refractory to maximally tolerated antiarrhythmic drug dosing and at least one failed cardioversion attempt. Besides the recommended follow-up by the local cardiologist, starting from 2021, surviving patients were asked to undergo assessment of left ventricular function and to complete a questionnaire addressing quality of life and predisposing factors for recurrent AF.ResultsAt a mean follow-up of 77 months, the rate of AF recurrence was 20.7% (n = 114). On multivariate analysis, impaired left ventricular ejection fraction (58 patients, 51%, p = 0.002), worsening of European Heart Rhythm Association (EHRA) symptom class (37 patients, 32%, p = 0.003) and cognitive decline or depression (23 patients, 20%, p = 0.023) during follow-up were found to be significantly associated with AF recurrence.ConclusionsSurgical AF ablation through a right minithoracotomy is safe, but a better outcome could be achieved using a hybrid approach. Patients after initial failed surgical AF ablation show worsening of cardiac function, clinical status and quality of life at follow-up compared to patients with successful AF ablation.

Silent strokes after thoracoscopic epicardial ablation and catheter ablation for atrial fibrillation: not all lesions are permanent on follow-up magnetic resonance imaging.

Invasive treatments for atrial fibrillation (AF) pose a risk of ischemic stroke due to periprocedural brain embolization, which may be manifest or silent. The primary aim of our study was to compare the rate of silent strokes after percutaneous catheter-based and thoracoscopic epicardial ablation for AF. The secondary aim was to evaluate the development of silent strokes over time. We included 39 subjects (aged 64.1±8.9 years) treated for persistent symptomatic AF with thoracoscopic ablation and 30 subjects (aged 64.1±10.5 years) treated for paroxysmal or persistent symptomatic AF with catheter ablation. Subjects underwent brain MRI before and early after the ablation, moreover, the surgical group underwent late MRI 6 months after therapy. On early MRI, the presence of silent strokes and their number and size were evaluated. On late MRI, transformation of previously-detected acute ischemic lesions into chronic infarction or their reversibility were assessed. Initially, different chronic ischemic findings were found in 64% of patients from the surgical group and in 70% from catheter group. Early MRI results: acute ischemic lesions were detected in 2 (6.7%) subjects (overall 3 lesions sized <5 mm) in the catheter group and in 17 (43.6%) subjects in surgical group. Most subjects in the surgical group showed multiple lesions (88%); 195 lesions were detected, a median 6 (IQR 8) lesions per case. Eighty-two percent of lesions were <5 mm, 12% 5-10 mm, 5% 10-30 mm, and 2% were large territorial ischemia. Only 1 case was symptomatic, the rest were silent strokes. On late MRI, 53.5% of all acute lesions were reversible. Lesions <5 mm were reversible in 63.1% of cases, lesions 5-10 mm were reversible in 21.7% and all lesions larger than 10 mm persisted. In 29.4% of patients all acute ischemic lesions were fully reversible. Periprocedural silent strokes were significantly more common after thoracoscopic epicardial ablation compared to catheter ablation considering both the number of affected patients and number of lesions. The majority of acute ischemic brain lesions were small, up to 5 mm in diameter, roughly half of which were reversible. Reversibility of acute ischemic lesions decreased with size. However, in 29.4% of affected patients, all lesions were fully reversible.

Epicardial access complications during electrophysiology procedures.

Percutaneous epicardial access (EA) was first described more than two decades ago. Since its initial introduction, indications for its utilization in the field of electrophysiology have expanded dramatically. Epicardial mapping and ablation in patients with ventricular tachycardia is routinely performed in tertiary electrophysiology centers around the world. Although limited by lack of randomized controlled trials, epicardial ablation for atrial fibrillation has been suggested as a conjunctive strategy in patients who have failed an initial endocardial catheter ablation attempt, and it is necessary for placement of some left atrial appendage occlusion devices as well. An accurate understanding of the cardiac anatomy is crucial to avoid complications such as inadvertent right ventricular puncture, injury to the coronary arteries, abdominal viscera, phrenic nerves, and esophagus during both EA and catheter ablation. The aim of this review is to provide a comprehensive overview of the cardiac anatomy, technical aspects to optimize the safety of epicardial puncture, recognize and avoid potential complications.

Abstract 14990: Pulmonary Vein Stenosis After Epicardial Atrial Fibrillation Ablation and Epicardial Left Atrial Appendage Occlusion: A Diagnostic Dilemma and Challenges in Management

Background: Pulmonary vein stenosis is a dreaded complication of endocardial atrial fibrillation (AF) ablation but rare after epicardial ablation and has not been reported after epicardial left atrial appendage occlusion (LAAO). Case: A 55-year-old male was referred to our tertiary hospital for management of left superior pulmonary vein (LSPV) stenosis causing dyspnea on exertion. About 2 years prior to presentation, he underwent quadruple coronary artery bypass grafting for non-ST elevation myocardial infarction along with modified Cox-Maze procedure with pulmonary vein and posterior wall isolation as well as epicardial LAAO with AtriClip for history of paroxysmal AF. At our institute, V/Q scan showed ventilation-perfusion mismatch and absent perfusion of the left upper lobe (Figure 1A). Cardiac CT showed persistent LSPV occlusion (Figure 1B). TEE showed atrial appendage occluded with a clip and no flow was observed from LSPV (Figure 1C). After multidisciplinary discussion between cardiology and cardiothoracic surgery teams, surgical approach to remove the AtriClip was deemed futile as it was placed 2 years ago and less likely to result in resolution of stenosis. Thus, an endovascular approach was attempted with left atrial and pulmonary vein angiography showing LSPV to be 100% occluded (Figure 1D). Pulmonary vein recanalization was attempted but was not successful. Conclusion: Our case highlights the importance of recognition of PVS as a possible complication after epicardial LAAO as early intervention can improve patient outcomes. PVS has been previously described with Maze procedure but that patient was successfully treated with catheter-balloon angioplasty. Given 100% occlusion and difficulty with recanalization makes epicardial ablation a less likely cause of occlusion in our case. More careful application of Atriclip protocols might be necessary to prevent this potential complication.

Sinoatrial nodal artery injury in thoracoscopic epicardial ablation for atrial fibrillation.

This study aimed to clarify the incidence of sinoatrial nodal artery (SANa) injury in thoracoscopic epicardial surgical ablation for atrial fibrillation (AF) and its impact on postoperative outcomes, which have not been previously elucidated. We enrolled 103 consecutively patients with AF who underwent thoracoscopic epicardial ablation at our institution. In these patients, we evaluated the postoperative incidence of SANa injury by using enhanced cardiac computed tomography. For patients with confirmed SANa injury, 3-day continuous electrocardiographic monitoring and exercise stress tests were performed to assess the sinus rhythm maintenance and sinus node function. Thirteen patients (12.6%) had a confirmed SANa injury (left anterior type in 6 patients, left posterior type in 2 patients and double-branch type in 5 patients). After a median follow-up of 24 months, the patients with SANa injury were not found to be associated with lower sinus rhythm maintenance (hazard ratio 1.09, 95% confidential interval 0.36-3.31) as compared with those without SANa injury after adjustment for patient characteristics. Sinus node function was evaluated in 7 patients with SANa injury who remained in sinus rhythm after the procedure, and no sinus node dysfunction was confirmed in the 3-day electrocardiographic monitoring and exercise stress tests at a median follow-up of 12 months. The prevalence of SANa injury in the patients who underwent thoracoscopic epicardial ablation for AF was relatively low, and the incidence of SANa injury was not associated with postoperative restoration of sinus rhythm and sinoatrial node dysfunction. More studies are required to better understand SANa injury.

Epicardial and Endocardial Validation of Conduction Block After Thoracoscopic Epicardial Ablation of Atrial Fibrillation.

ObjectiveIt is unknown whether epicardial and endocardial validation of bidirectional block after thoracoscopic surgical ablation for atrial fibrillation is comparable. Epicardial validation may lead to false-positive results due to epicardial tissue edema, and thus could leave gaps with subsequent arrhythmia recurrence. It is the aim of the present study to answer this question in patients who underwent hybrid atrial fibrillation ablation (combined thoracoscopic epicardial and endocardial catheter ablation).MethodsAfter epicardial ablation of the pulmonary veins (PVs) and connecting inferior and roof lines (box lesion), exit and entrance block were epicardially and endocardially evaluated using an endocardial His Bundle catheter and electrophysiological workstation. If incomplete lesions were found, endocardial touch-up ablation was performed. Validation results were also compared to predictions about conduction block based on tissue conductance measurements of the epicardial ablation device.ResultsTwenty-five patients were included. Epicardial validation results were 100% equal to the endocardial results for the left superior, left inferior, and right inferior PVs and box lesion. For the right superior PV, 85% similarity was found. Based on tissue conductance measurements, 139 lesions were expected to be complete; however, in 5 (3.6%) a gap was present.ConclusionsEpicardial bidirectional conduction block in the PVs and the box lesion corresponded well with endocardial bidirectional conduction block. Conduction block predictions by changes in tissue conductance failed in few cases compared to block confirmation. This emphasizes that tissue conduction measurements can provide a rough indication of lesion effectiveness but needs endpoint confirmation by either epicardial or endocardial block testing.

Percutaneous Epicardial Ablation of Atrial Fibrillation

The observations afforded by epicardial mapping have not only increased the appreciation of distinct epicardial structures in the left atrium but also underscore the need to address the substrate transmurally. Although epicardial access and ablation have attendant risks, comparative studies with hybrid surgical approaches are lacking. In the search to find unifying mechanisms of atrial fibrillation, a conceptual shift that emphasizes the substrate in 3 dimensions, with the epicardium distinct from the endocardium, holds promise for future investigation and evolving therapeutic tools.

Epicardial Atrial Fibrillation Ablation and Left Atrial Appendage Amputation in a Patient with Congenital Bicuspid Aortic Valve and Catheter-Induced Cardiac Tamponade.

Early recognition and rapid and appropriate treatment of cardiac tamponade are mandatory to prevent the irreversible deterioration of cerebral perfusion and other important organs. In this study, cardiac tamponade was induced by inadvertent transseptal puncture, which was managed with pericardial drainage and surgical repair in a patient with symptomatic paroxysmal atrial fibrillation. Epicardial atrial fibrillation ablation and left atrial appendage amputation were also performed at the same time.

In 2030, the European Union will include 14 to 17 million atrial fibrillation (AF) patients, with 120,000 to 215,000 new cases each year. The increase in the prevalence of this arrhythmia has led to the development of new therapeutic intervention strategies to manage the different aspects of this disease. Thus, endocavitary or epicardial ablation of AF, by radiofrequency or cryoablation, provides superior results to antiarrhythmic therapy in controlling symptoms and preventing heart failure in paroxysmal or persistent AF. In heart failure patients with advanced AF, the ablation of the atrioventricular junction associated with the implantation of a bi-ventricular pacemaker has just demonstrated its clear superiority, bringing this technique up to date. Finally, in the event of a major bleeding risk and contraindication to anticoagulants, percutaneous occlusion of the left atrium has proven its value in preventing AF-related embolic events. The future will certainly see the emergence of new technologies but also personalized strategies based on an optimal selection of the right candidates for these interventions, thanks in particular to the contribution of imaging before the procedure.

Objectives: Although preoperative left atrial size and duration of atrial fibrillation (AF) have been associated with ablation results, these factors are not sufficient to predict the outcome in long standing persistent AF (LsPeAF). The aim of this study was to investigate whether preoperative plasma Atrial Natriuretic Peptide (ANP) may reflect atrial fibrosis and has predictive value for outcome after epicardial ablation.

Collateral Damage from Catheter Ablation of Atrial Fibrillation Lessons Learnt in the Past Decade.

Catheter ablation for atrial fibrillation (AF) has been increasingly performed over the past decade. Regardless of technological advances and technique improvement, catheter ablation for AF remains a highly complex procedure and the risk of procedural complications is not negligible. This article discusses the management and the approach to avoid the serious complications of catheter AF ablation including pulmonary vein stenosis, atrioesophageal fistula, cardiac tamponade, stroke and collateral nervous damage. The management of periprocedural anticoagulation and the complications associated with epicardial AF ablation are also described.

Anatomical basis of minimally invasive epicardial ablation of atrial fibrillation

Minimally invasive atrial fibrillation surgery (MIAFS) has become a well established and increasingly used option for managing patients with stand-alone arrhythmia. Pulmonary veins (PVs) isolation continues to be the cornerstone of ablation strategies. Indeed, in most cases, atrial fibrillation (AF) is triggered in or near the PVs. Nevertheless, ectopic beats initiating AF may occasionally arise from non-PV foci. The knowledge of the anatomy and underlying morphology of PVs and non-PV foci is essential for cardiac surgeons treating AF patients with epicardial minimally invasive procedures. The anatomical structures relevant to the pathogenesis and the epicardial treatment of AF include the PVs, the pericardial space, the pericardial sinuses, the phrenic nerve, the left atrium, the retro-atrial and caval ganglionated plexuses, the ligament of Marshall, the caval veins and the left atrial appendage. In this review, we briefly describe the basic anatomy of these structures and discuss their specific correlations for cardiac surgeons interested in performing MIAFS.

Hybrid Ablation Therapy: Current Status and Future Development

Ventricular tachycardia (VT) contributes to significant morbidity and mortality for patients with structural heart disease. The implantable cardioverter-defibrillator (ICD) has been demonstrated to help in reducing mortality in this patient population. The ICD, however, cannot prevent future shocks and also contributes to significant anxiety for patients. Anti-arrhythmic drugs also have limited efficacy and are frequently not tolerated owing to various side effects. Catheter ablation of VT has now been shown to reduce recurrent VT episodes in numerous randomized trials. In addition, recent publications have suggested that an early ablation procedure prior to VT development may improve patient outcomes. Although percutaneous epicardial access has been well described and is relatively safe, patients with prior surgery frequently have pericardial adhesions which make a percutaneous approach difficult and/or not feasible. In these patients, a hybrid surgical approach may be warranted for successful ablation of VT. However, the surgical approach requires detailed planning, coordination with cardiac surgeons and an integrated multidisciplinary approach. Decreased Apoptosis following Successful Ablation of Atrial Fibrillation

Objectives: Increased apoptotic processes in tissue samples from hearts in atrial fibrillation (AF) have been previously documented in animals. Whether the restoration of sinus rhythm is associated with decreased apoptosis is not known. The aim of the present study was to establish whether successful epicardial ablation of AF leads to changes in the concentration of serum markers of apoptosis. Methods: Twenty-five patients with AF were prospectively studied. All underwent epicardial isolation of pulmonary veins. The success of the ablation was assessed clinically and with 3 Holter recordings. Blood samples were drawn before surgery, and at 3 and 6 months after. Serum concentrations of Fas (apoptosis-stimulating fragment) and TRAIL (tumor necrosis factor-related apoptosis-inducing ligand) were measured using ELISA. Results: AF was successfully ablated in 15 patients (SR group). In the other 10 patients (AF group), AF recurred during follow-up. Neither group differed with respect to age, sex, left ventricular ejection fraction, or preoperative concentrations of measured molecules. While Fas decreased in successfully ablated patients, there was no change in the Fas concentration in the AF group. Similarly, the concentrations of TRAIL decreased in the SR group, but remained unchanged in the AF group. Conclusion: The ablation of AF is associated with decreased serum markers for apoptosis.
