Preputial island flap in extensive urethral stricture repair

Seven patients with extensive urethral strictures were treated with a preputial island flap applied as a tubularised neourethra or as a patch covering the longitudinally incised urethra. This flap enables the correction of strictures as long as 9 cm. In two subjects, a double preputial island flap was employed, with the outer layer being used to replace the lacking skin. In six patients, the pedicle was based on the dorsal aspect of the penis, and in the other case is was placed on the ventral side. All of the preputial island flaps inserted had a very good blood supply. Complications developed in three patients who developed stenosis of the distal anastomosis, which was corrected by Mickulicz plasty in one case and by endoscopic urethrotomy in the two others. All subjects now void spontaneously via the urethra except for one patient, who developed a small urethrocutaneous fistula.

Endoscopic Urethrotomy versus Urethrotomy plus Nd-YAG Laser in the Treatment of Urethral Stricture

Between February 1987 and May 1988, 30 patients who presented with single, iatrogenic, annular strictures of the bulbar urethra were included in this prospective study. They were randomly divided into two groups; group 1: 15 patients who underwent direct-vision endoscopic urethrotomy (cold-knife incision at 12 o'clock) and group 2: 15 patients who underwent internal urethrotomy plus Nd-YAG laser. The results obtained were analyzed and compared at 1 and 2 years by clinical evaluation, uroflowmetry and retrograde-voiding urethrography. Group 1 obtained 80% good results at 1 year, falling to 60% at 2 years follow-up. Group 2 presented good results in 73.3% both at 1 and 2 years of follow-up.

Percutaneous pyeloplasty: an alternative to open surgery

This paper describes the management of five patients with pelviureteric junctional obstruction who were treated by a one-stage percutaneous procedure. In two patients, renal calculi were removed simultaneously. The obstructing strictured area was divided internally by means of a modified endoscopic urethrotomy knife, which was passed through a percutaneous nephrostomy track. The defect was bridged by natural regeneration around a soft splint over six weeks. Four patients achieved a successful result with a shortened hospital stay, minimal morbidity, improved radiological drainage and the avoidance of open surgery. One patient, who had undergone three previous open plastic operations, was still symptomatic, in spite of successful anatomical correction. At exploration, open surgical repair was impossible and a nephrectomy was performed. This new technique appears applicable to selected primary and secondary pelviureteric junctional obstructions in which the obstructing segment is in an anatomically dependent situation.

Posterior urethroplasty in children.

A series of 33 posterior urethral strictures in children is presented. The etiology was traumatic in 31 cases and iatrogenic in 2. Complex strictures were treated by a transpubic approach (20 cases) or by a posterior scroto-urethral inlay (1 case). Simple strictures were directly treated via the perineum: by a push-in technique (6 cases); by end-to-end anastomosis (3 cases); by a skin pedicled tube urethroplasty (1 case); by free skin graft (1 case), and by endoscopic urethrotomy (1 case). The use of omentum, employed in 19 cases, is considered a necessary step in transpubic urethroplasty. Favorable results were observed in 18 out of 20 transpubic procedures, in 8 out of 9 perineal urethroplasties and in 3 out of 4 other techniques, with an overall success rate of 87.8%.

Factors influencing recurrence of urethral strictures after endoscopic urethrotomy: the role of infection and peri-operative antibiotics.

One hundred patients who had undergone 162 urethrotomies were reviewed. Factors which influenced the outcome of the procedure are discussed. Peri-operative infection occurred in 38% of cases and significantly increased the recurrence rate unless the patients were treated with antibiotics.

Surgical treatment of urethral stricture: still a problem?

In the introduction the author stresses that the main facts which decide the choice and prognosis of therapy are localization, length and the so-called complexity of the stricture. His 15-year material from 388 operations on 290 patients is presented. Circular strictures of the anterior urethra (a total of 135 operations) are treated through endoscopic urethrotomy as the primary procedure and mostly by open resection as the secondary one. Longitudinal strictures of the anterior urethra (199 operations) are treated by marsupialization, pedicled cutaneous flap and Michalowski's "sleeve" technique as primary procedures. As the secondary procedure (after failure of the first operation) marsupialization only was performed. Complex strictures of the posterior urethra (total 54) were treated mostly by transpubic approach, which showed good results in 74% cases on 41 operations.

Endoscopic Urethrotomy: Does it Live Up to its Promises?

Endoscopic cold knife urethrotomy was done on 160 patients with urethral strictures. Of these patients 120 have been followed for 6 months to 2 years. The initially good results (56 per cent cured at 6 months) do not stand the test of time since only 25 per cent of the patients can be considered cured after 2 years. Despite these less than triumphant results endoscopic urethrotomy, which is performed easily and uncompromising to secondary urethroplasty, should be tried as a primary procedure.

Endoscopic internal urethrotomy for treatment of urethral strictures Midterm survey

Success in the management of urethral strictures can be claimed only after many years, patients sometimes faring well for ten years or more before suffering a recurrence. We used endoscopic urethrotomy as primary treatment for a variety of urethral strictures in 123 patients. Follow-up is over two years for 63 patients and more than five years for 18. Over-all success rate at five years was 76 per cent. Failures are twice as frequent in strictures, regardless of their origin, involving the anterior urethra than in the bulbar or posterior urethra. Two-thirds occurred in the first year of follow-up, but, as in other techniques, recurrence can occur after four years.

Endoscopic Urethrotomy in the Treatment of Urethral Stenosis

After reviewing the different therapeutic methods for urethral stenosis in the male, the results obtained with Sachse's urethrotomoscope (UTC) was studied in 25 patients checked within a period of between 6 and 28 months. Patients were judged cured when they maintained normal clinical, radiological and urodynamic parameters without treatment; 75% of the patients could be included in that series. Urethral stenosis (US) is a last stage common to distinct pathological causes (congenital, infectious, iatrogenic, traumatic), and lesions, together with their anatomicopathological consequences, merit special study because they contribute to bring valuable data to therapy.
