Endoscopic calcaneoplasty--rationale, surgical technique, and early results: a preliminary report.

We present our technique and preliminary results with endoscopic calcaneoplasty in ten patients resistant for conservative therapy for more than 6 months. All patients showed a Haglund spur on radiography; none had a cavovarus deformity. Follow-up ranged from 2 to 12 months (mean 5.2). All patients showed clinical improvement and would undergo for the procedure again. Three showed a good and seven an excellent result in Ogilvie-Harris score. Postoperative radiographic follow-up showed sufficient bone removal in all cases. Surgery lasted on average 46 min (range 28-84). There were no intra- or postoperative complications. Endoscopic calcaneoplasty is an effective minimally invasive treatment option for patients with retrocalcaneal bursitis.

Summary The purpose of the study was to present our technique and preliminary results with endoscopic calcaneoplasty. The surgical technique was first developed in 5 anatomic specimens. Endoscopic calcaneoplasty was then performed in 15 patients (8 male; 7 female). The average age was 34 years with a range from 24 to 56 years. All patients were resistant to conservative therapy for more than 6 months. All patients showed on X-ray a Haglund spur. We avoided treating patients with a cavovarus deformity. The followup ranged from 6 to 18 months with an average of 12.2 months. All patients showed clinical improvement and would choose the same procedure again. Four showed a good and 11 showed an excellent result in the Ogilvie-Harris score. Postoperative X-ray control showed sufficient bone removal in all cases. Time for surgery lasted on average 41 minutes (range from 24-84 minutes). There were no intra- or postoperative complications. Endoscopic calcaneoplasty is an effective minimally invasive treatment option for patients with retrocalcaneal Haglund-exostosis.

Endoscopic calcaneoplasty.

Endoscopic calcaneoplasty offers access to the retrocalcaneal space, thereby making it possible to remove inflamed retrocalcaneal bursa as well as the posterosuperior part of the calcaneus in applicable cases of painful hindfoot. In this study, endoscopic calcaneoplasty was performed in 21 procedures in 20 patients. All of the patients had typical complaints of inflammation of the retrocalcaneal bursa that were unresponsive to nonoperative treatment for more than 6 months. In all patients a superior calcaneal angle of more than 75 degrees and positive parallel pitch lines were present on the lateral calcaneal radiograph. The mean follow-up was 3.9 years (range, 2 to 6.5). There were no surgical complications and no postoperative infections. One patient had a fair result, 4 patients had good results, and the remaining 15 patients had excellent results. Whether this operation is performed by endoscopic or open technique, enough bone must be removed to prevent impingement of the bursa between the calcaneus and Achilles tendon. Endoscopic calcaneoplasty is a minimally invasive technique performed in an outpatient setting and combined with a functional rehabilitation program. The procedure has low morbidity. Patients have a short recovery time and quickly resume work and sports.
