Digital Technology in Children’s Safeguarding Social Work Practice in the 21st Century: A Scoping Review

Abstract Digital technology plays an increasing role in children’s safeguarding social work practice, where there is a renewed emphasis on the recording and interpretation of data via digital technology, as well as the use of technology to communicate with families and colleagues, particularly since Covid-19. This scoping review delineated extant research investigating uses of digital technology in children’s safeguarding social work practice since 2000 in order to contextualise current emerging practices. The methodology used accounted for the heterogeneity of study designs whilst maintaining a rigorous approach to searching, selection and analysis. Literature searches were conducted in August and September 2022 identifying forty-three studies. Descriptive quantitative and qualitative analyses were undertaken to outline date, location, methodology, aspect of technology investigated, and key findings of included studies. The review generated two descriptive categories which reflect the included studies: electronic information systems and digital communications. These categories subsume a range of distinct platforms and applications whilst highlighting a tendency of the included studies to separate out their focus between these specific aspects of digital technology, with limited discourse between categories. Families’ perspectives are also missing from most research identified in the review.

Evaluation of Blood Transfusion Request form: The Experience in a Tertiary Health Facility in Jos, Nigeria

Background: There is a thin line between a safe blood transfusion and transfusion-related fatality hence the need to be diligent in every aspect of the blood transfusion process. Appropriate and complete documentation on a blood transfusion request form is one of the most important preanalytic activities serving as a communication tool between the clinician and the blood transfusion laboratory personnel. Aims: To evaluate compliance with appropriate and complete documentation of information on our blood transfusion request forms for a reliable preanalytic process towards an efficient blood transfusion service. Study Design: It is a retrospective study. Place and Duration of Study: Blood Bank of the Jos University Teaching Hospital between January to December 2023. Methodology: Six thousand, three hundred and sixty (6360) blood transfusion request forms from the Jos University Teaching Hospital Blood bank were evaluated for complete or incomplete documentation retrospectively and results were presented in frequencies and percentages. Results: There was 100% compliance in filling in the patients' surnames and other names as well as the laboratory number and blood groups of the patients while only 4779 (75.14%) filled in the patients' ages with 1416(22.26%) using the prefix of adult(ad) while 165(2.59%) fail to document the patients' age. There were 2829 (44.48%) males with 3522 (55.38%) females while no sex was indicated in 9 of the reviewed forms. Obstetrics history has the least cumulated documented response of 0.38% while a significant 1008 (15.85%) did not indicate either blood grouping or blood grouping with cross-match request. Conclusion: Appropriate and complete documentation of information on blood transfusion request forms is a problem among clinicians and will require continuous education on its importance, periodic auditing, provision of electronic data system and attitudinal change for a better blood transfusion compatibility service.

Temporal trends in acute coronary syndrome by subtype

Abstract Funding Acknowledgements None. Introduction Various studies reported different results in trends regarding acute coronary syndrome (ACS) subtypes over the last years. We performed a retrospective analysis to determine trends in ACS incidence, demographic characteristics of patients, invasive treatment as well as outcomes and mortality of patients with ST elevation myocardial infarction (STEMI); non-STEMI (NSTEMI) and unstable angina (UA) in our tertiary Cardiac Intensive Care Unit (CICU). Methods We collected data from medical records and our electronic hospital information system on all admissions that occured in March 2003, 2013 and 2023 and compared the data. Results Over the years, there was a significant increase in total number of patients in our CICU (74, 87 and 143 per month, respectively). Patients with ACS accounted for more than half of all patients in the investigated period, but without reaching statistical significance for increase (51.4%, 64.4% and 63.6%; p= 0.156). On the other hand, when considering patients with STEMI, there was a significant upgrowth of the number of such patients (6.8% vs 31% vs 28.7%; p<0.001) with a trend of a decrease in the number of NSTEMI (28.4%, 18.4% and 23.8%; p= 0.324) and patients with UA (16.2%, 14.9% and 11.2%; p= 0.528). Percutaneous interventional treatment significantly rose over time (35.6 vs 61.2 vs 67.8%, p<0.01), while application of ICU restricted therapies like vasopressors (1.7% vs 0 vs 6.3%; p= 0.072) and inotropes (6.8% vs 4.7% vs 8.5%; p= 0.356) showed increasing trend, but without reaching statistical significance. However, CICU length of stay (LOS) notably decreased (59h vs 43h vs 22h; p= <0,001) as well as mortality (9.5% vs 5.7% vs 2.8%; p= <0.001). Conclusion We observed a significant increase in the number of STEMI patients with a declining trend in NSTEMI and UA patients, as well as increase in coronary procedures and vasopressors and inotropes administration, all indicating growing acuity and complexity of our patients. On the other hand, LOS and mortality significantly decreased highlighting better outcomes and treatment success.

Electronic medical information systems and timeliness of care in the emergency department: a scoping review

BackgroundAlthough many emergency department (ED) settings have implemented an electronic medical information system (EMIS) and EMIS tools in an effort to improve timeliness of care outcomes, there remains a paucity of scientific information on this topic. We therefore performed a scoping review to describe the range of EMIS interventions and their impacts on time-based outcomes in the ED.MethodsWe searched four bibliographic databases to identify potentially relevant records published after 2014 in English. Two reviewers assessed records for eligibility using a two-step screening process. We then extracted data on the type of EMIS, outcomes assessed, and reported results. Findings were summarized in tabular form and grouped by time-based outcome.ResultsTwenty-five studies met the eligibility criteria, with approximately half being retrospective studies. Interventions varied among studies; they generally included a new or updated EMIS, EMIS tools related to disease diagnosis and/or management, triage tools, or health information exchange platforms. Included studies compared interventions with relevant comparator groups, such as prior versions of an EMIS, absence of an EMIS, pen-and-paper documentation, and/or communication via telephone and fax. The most common outcomes reported were length of stay (n = 17 studies) and time to medication (n = 6 studies), followed by time to order, time to provider, and time from result to disposition. Reported effects of the interventions were generally inconsistent, showing either improvements, delays, or no change in examined outcomes.ConclusionsAdditional research is needed to determine how electronic medical information may be used in the ED to improve timeliness of care. Findings from this review can be used to inform future systematic reviews that evaluate the impact of these systems and tools on specific quality of care measures.

Development and validation of a novel nomogram model for identifying risk of prolonged length of stay among patients receiving free vascularized flap reconstruction of head and neck cancer.

The aim of the present study was to build and internally validate a nomogram model for predicting prolonged length of stay (PLOS) among patients receiving free vascularized flap reconstruction of head and neck cancer (HNC). A retrospective clinical study was performed at a single center, examining patients receiving free vascularized flap reconstruction of HNC from January 2011 to January 2019. The variables were obtained from the electronic information system. The primary outcome measure was PLOS. Univariate and multivariate analyses were used to find risk factors for predicting PLOS. A model was then built according to multivariate results. Internal validation was implemented via 1000 bootstrap samples. The study included 1047 patients, and the median length of stay (LOS) was 13.00 (11.00, 16.00) days. Multivariate analysis showed that flap types ((radial forearm free flap (odds ratio [OR] = 2.238; 95% CI, 1.403-3.569; P = 0.001), free fibula flap (OR = 3.319; 95% CI, 2.019-4.882; P < 0.001)), duration of surgery (OR = 1.002; 95% CI, 1.001-1.003; P = 0.004), postoperative complications (OR = 0.205; 95% CI, 0.129-0.325; P = P < 0.001) and unplanned reoperation (OR = 0.303; 95% CI, 0.140-0.653; P = 0.002) were associated with PLOS. In addition to these variables, blood transfusion was comprised in the model. The AUC of the model was 0.78 (95% CI, 0.711-0.849) and 0.725 (95% CI, 0.605-0.845) in the primary and internal validation cohorts, respectively. The DCA revealed the clinical utility of the current model when making intervention decisions within the PLOS possibility threshold range of 0.2-0.8. Our study developed a nomogram that exhibits a commendable level of accuracy, thereby aiding clinicians in assessing the risk of PLOS among patients receiving free vascularized flap reconstruction for HNC.

IT-Related Barriers and Facilitators to the Implementation of a New European eHealth Solution, the Digital Survivorship Passport (SurPass Version 2.0): Semistructured Digital Survey.

To overcome knowledge gaps and optimize long-term follow-up (LTFU) care for childhood cancer survivors, the concept of the Survivorship Passport (SurPass) has been invented. Within the European PanCareSurPass project, the semiautomated and interoperable SurPass (version 2.0) will be optimized, implemented, and evaluated at 6 LTFU care centers representing 6 European countries and 3 distinct health system scenarios: (1) national electronic health information systems (EHISs) in Austria and Lithuania, (2) regional or local EHISs in Italy and Spain, and (3) cancer registries or hospital-based EHISs in Belgium and Germany. We aimed to identify and describe barriers and facilitators for SurPass (version 2.0) implementation concerning semiautomation of data input, interoperability, data protection, privacy, and cybersecurity. IT specialists from the 6 LTFU care centers participated in a semistructured digital survey focusing on IT-related barriers and facilitators to SurPass (version 2.0) implementation. We used the fit-viability model to assess the compatibility and feasibility of integrating SurPass into existing EHISs. In total, 13/20 (65%) invited IT specialists participated. The main barriers and facilitators in all 3 health system scenarios related to semiautomated data input and interoperability included unaligned EHIS infrastructure and the use of interoperability frameworks and international coding systems. The main barriers and facilitators related to data protection or privacy and cybersecurity included pseudonymization of personal health data and data retention. According to the fit-viability model, the first health system scenario provides the best fit for SurPass implementation, followed by the second and third scenarios. This study provides essential insights into the information and IT-related influencing factors that need to be considered when implementing the SurPass (version 2.0) in clinical practice. We recommend the adoption of Health Level Seven Fast Healthcare Interoperability Resources and data security measures such as encryption, pseudonymization, and multifactor authentication to protect personal health data where applicable. In sum, this study offers practical insights into integrating digital health solutions into existing EHISs.

Zn-Ti co-substituted LiZn microwave ferrites: Sintering characteristics and gyromagnetic properties

To meet the low-loss and high-frequency application requirements of modern electronic information systems for integrated microwave devices, Zn-Ti co-substituted LiZn microwave ferrites were prepared in this work. The results demonstrated that Zn-Ti co-substitution promoted the grain growth and densification process during sintering, leading to a decrease in the pore-induced linewidth (ΔHp) of LiZn composite microwave ferrites; the introduction of non-magnetic Zn2+ and Ti4+ ions lowered the content of Fe3+ ions in the ferrites, which reduced the magnetocrystalline anisotropy constant |K1| and thus diminished the anisotropy-induced linewidth (ΔHa). Besides, the dielectric loss of the material decreased with increasing Zn-Ti substitution amounts (x) due to the reduction of Fe2+ ion content. As a result, when x = 0.35, the FMR linewidth was decreased from ∼ 251 Oe to ∼ 97 Oe, and the dielectric loss was greatly reduced from > 100 × 10−4 to ∼ 3.82 × 10−4 in the ferrites. The material also exhibited excellent properties including high saturation magnetization (∼3913 Gauss), high Curie temperature (∼304 °C), and low coercivity (∼2.05 Oe).

Left posterior cerebral artery occlusion following minor head trauma: a case report

A 17-year-old male patient who received a punch behind the left ear 2 days ago, was admitted to the emergency room with a headache and dizziness. His neurological examination was normal. Brain CT images demonstrated diffuse hypodense areas and sulcus deletion in the left occipital lobe. On MR images, diffusion restriction for acute ischemia in the left PCA watershed area was determined, and hospitalization was recommended. However, he and his relatives left the hospital. Through the electronic information system called “e-nabız”, it was learned that the patient was hospitalized in an external center and treated with antiplatelet and anticoagulant regimens for ten days due to an acute thrombus at the left PCA P1 distal-P2 level observed on the brain CT and MR angiography images. In our clinic, his neurological examination was completely normal thirty days later. Still, a visual field examination revealed a total quadrianopsy in the upper temporal pole on the right and left eyes. An encephalomalacia in the left PCA watershed area was observed on CT images. The findings of this case showed that minor head trauma in children can cause large vessel occlusion and therefore close follow-up of these children for a certain time would be appropriate.

Assessment of antimicrobial resistance laboratory-based surveillance capacity of hospitals in Zambia: findings and implications for system strengthening

A well-established antimicrobial resistance (AMR) laboratory-based surveillance (LBS) is of utmost importance in a country like Zambia which bears a significant proportion of the world's communicable disease burden. This study assessed the capacity of laboratories in selected hospitals to conduct AMR surveillance in Zambia. This cross-sectional exploratory study was conducted among eight purposively selected hospitals in Zambia between August 2023 and December 2023. Data were collected using the self-scoring Laboratory Assessment of Antibiotic Resistance Testing Capacity (LAARC) tool. Of the assessed facilities, none had full capacity to conduct AMR surveillance with varying capacities ranging from moderate (63% (5/8)) to low (38% (3/8)). Some of the barriers of AMR-LBS were the lack of an electronic laboratory information system (63% (5/8)) and the lack of locally generated antibiograms (75% (6/8)). Quality control for antimicrobial susceptibility testing (AST), pathogen identification and media preparation had the lowest overall score among all of the facilities with a score of 14%, 20% and 44%, respectively. The highest overall scores were in specimen processing (79%), data management (78%), specimen collection, transport and management (71%), and safety (70%). Most facilities had standard operating procedures in place but lacked specimen-specific standard operating procedures. The absence of laboratories with full capacity to conduct AMR surveillance hinders efforts to combat AMR and further complicates the treatment outcomes of infectious diseases. Establishing and strengthening LBS systems are essential in quantifying the burden of AMR and supporting the development of local antibiograms and treatment guidelines.

Online Voting System using Blockchain

Blockchain technology has been presented as a support for trust needs between transactions in electronic information systems. Its successful use in cryptocurrencies has allowed it to explore its capabilities in commercial, industrial, and service systems, backed by the operational alternatives offered by Ethereum Smart Contracts and the cryptographic security of public and private key. These keys are used as a way to make online transactions anonymously, with the guarantee offered by the Blockchain network that they are executed safely. With the above in mind, this concept can be extended to the electoral processes, thus allowing its application in electronic voting systems, especially when the protocols currently used lack the trust factor between the different social actors. This document presents a proof of concept in which Blockchain and other technologies are applied, to allow interaction as an electronic voting system for the election of unique candidates. This has been achieved through the specification of an architecture designed especially for electoral processes, from which it is implemented and a simulation is carried out in order to obtain data that generates value, when evaluating Blockchain technology as an alternative to current voting systems

Application of E-Services in the Urban Local Government in Bangladesh: A Study on Rajshahi City Corporation

Public sector information and communication technology (ICT) has a long history of using mainframe operations in the 1970s and microcomputing in the 1990s. It is recognized by the Government of Bangladesh that ICT has a development impact, as it enhances access to information, the quality of public service delivery and strengthens good governance practices. Since the ending of 2019, the outbreak of the Covid-19 epidemic has revived the importance of e-governance. In the initial stage of this climacteric period when people were socially distanced, restricted in quarantine, and not interacting face-to-face with each other created a whale of a ponderosas of e-governance in Bangladesh. Without any doubt, e-governance is the prime catalyst for urban development. Rajshahi City Corporation (RCC) is one of the oldest and leading municipalities in e-governance adaptation in Bangladesh. RCC, jointly with the local government division under the ministry of local government, has introduced e-governance through initiating an electronic birth registration information system (EBRIS) in 2001. This is one of the best paradigms for adapting e-governance at the urban local government level in Bangladesh. As a part of e-governance, RCC has launched Smart Auto-Rickshaw Management System (SARMS) to reduce traffic congestion on city roads. The study, based on primary sources of information, seeks to assess the role of SARMS in reducing traffic congestion on city streets as part of e-governance. In order to properly implement e-governance in Rajshahi City Corporation (RCC), researchers have come up with two recommendations for e-service providers following two broad sections. A rich ICT infrastructure is a prerequisite for providing e-services to the recipients. This study is based on empirical research. This first section of this paper is equipped with the details of the respondents, overall opinions of the respondents on e-governance are covered by second section, the third section is depictured the status of e-services in RCC. However, the last section is enclosed by the specific opinions of the respondents on EBRIS and SARMS.

The Red Cross Red Crescent Health Information System (RCHIS): an electronic medical records and health information management system for the red cross red crescent emergency response units

BackgroundThe Red Cross and Red Crescent Movement (RCRC) utilizes specialized Emergency Response Units (ERUs) for international disaster response. However, data collection and reporting within ERUs have been time-consuming and paper-based. The Red Cross Red Crescent Health Information System (RCHIS) was developed to improve clinical documentation and reporting, ensuring accuracy and ease of use while increasing compliance with reporting standards.Case presentationRCHIS is an Electronic Medical Record (EMR) and Health Information System (HIS) designed for RCRC ERUs. It can be accessed on Android tablets or Windows laptops, both online and offline. The system securely stores data on Microsoft Azure cloud, with synchronization facilitated through a local ERU server. The functional architecture covers all clinical functions of ERU clinics and hospitals, incorporating user-friendly features. A pilot study was conducted with the Portuguese Red Cross (PRC) during a large-scale event. Thirteen super users were trained and subsequently trained the staff. During the four-day pilot, 77 user accounts were created, and 243 patient files were documented. Feedback indicated that RCHIS was easy to use, requiring minimal training time, and had sufficient training for full utilization. Real-time reporting facilitated coordination with the civil defense authority.ConclusionsThe development and pilot use of RCHIS demonstrated its feasibility and efficacy within RCRC ERUs. The system addressed the need for an EMR and HIS solution, enabling comprehensive clinical documentation and supporting administrative reporting functions. The pilot study validated the training of trainers’ approach and paved the way for further domestic use of RCHIS. RCHIS has the potential to improve patient safety, quality of care, and reporting efficiency within ERUs. Automated reporting reduces the burden on ERU leadership, while electronic compilation enhances record completeness and correctness. Ongoing feedback collection and feature development continue to enhance RCHIS’s functionality. Further trainings took place in 2023 and preparations for international deployments are under way. RCHIS represents a significant step toward improved emergency medical care and coordination within the RCRC and has implications for similar systems in other Emergency Medical Teams.

Electronic Information System Signal Noise for Noise Reduction using Variational Bayesian based Robust Adaptive Filter

The most significant issues for a thousand users is the unwanted or loud signals in sound files, it is impossible to eliminate or minimize these noise signals without knowledge their types and ranges. To overcome this issue, present an Electronic Information System Signal Noise for Noise Reduction using Variational Bayesian based Robust Adaptive Filter (EIS-SNR-VBRAF) is proposed. Initially, the data is collected from Adaptive Myriad Filter using Time-Varying Noise and Signal-Dependent Parameters dataset. Then, the data is given for Pre-processing section, Variational Bayesian based Robust Adaptive Filter (VBRAF) is used to reduce the signal noise from Gaussian Noise, Traffic noise and Audio signal back ground noise. The proposed technique is implemented and efficacy of EIS-SNR-VBRAF technique is assessed by support of numerous performances such as BER, Signal-to-Noise-Ratio (SNR), Mean Squared Error, Peak Signal-to-Noise Ratio (PSNR), Root Mean Square Error, Structural Similarity Index, Cross-Correlation and Computational Complexity is analyzed. The proposed EIS-SNR-VBRAF method attains 21.18%, 23.52% and 23.65% lower RMSE, 21.52%, 21.76% and 23.24% higher PSNR, 21.19%, 21.73% and 20.48% higher Structural Similarity Index are compared with existing methods like early detection of mechanical malfunctions in vehicles utilizing sound signal processing (ED-MMV-SSP),noise reduction in infrasound signals based on mask coefficient binary weighting generalized cross correlation non-negative matrix factorization algorithm (NRID-MCWG-NMFA) and Click-event sound detection in automotive industry using machine/deep learning (CE-SDSI-DL) respectively.

Clinical significance of elevated D-dimer in emergency department patients: a retrospective single-center analysis

IntroductionD-dimer is a marker of coagulation and fibrinolysis widely used in clinical practice for assessing thrombotic activity. While it is commonly ordered in the Emergency Department (ED) for suspected venous thromboembolism (VTE), elevated D-dimer levels can occur due to various other disorders. The aim of this study was to find out the causes of elevated D-dimer in patients presenting to a large ED in Saudi Arabia and evaluate the accuracy of D-dimer in diagnosing these conditions.MethodsData was collected from an electronic hospital information system of patients who visited the ED from January 2016 to December 2022. Demographic information, comorbidities, D-dimer levels, and diagnoses were analyzed. Statistical analysis was performed using the SPSS software. The different diagnoses associated with D-dimer levels were analyzed by plotting the median D-dimer levels for each diagnosis category and their interquartile ranges (IQR). The receiver operating characteristic (ROC) curves were calculated and their area under the curve (AUC) values were demonstrated. The optimal cut-off points for specific diseases were determined based on the ROC analysis, along with their corresponding sensitivities and specificities.ResultsA total of 19,258 patients with D-dimer results were included in the study. The mean age of the participants was 50 years with a standard deviation of ± 18. Of the patients, 66% were female and 21.2% were aged 65 or above. Additionally, 21% had diabetes mellitus, 20.4% were hypertensive, and 15.1% had been diagnosed with dyslipidemia. The median D-dimer levels varied across different diagnoses, with the highest level observed in aortic aneurysm 5.46 g/L. Pulmonary embolism (PE) and deep vein thrombosis (DVT) were found in 729 patients (3.8%) of our study population and their median D-dimer levels 3.07 g/L (IQR: 1.35–7.05 g/L) and 3.36 g/L (IQR: 1.06–8.38 g/L) respectively. On the other hand, 1767 patients (9.2%) were diagnosed with respiratory infections and 936 patients (4.9%) were diagnosed with shortness of breath (not specified) with median D-dimer levels of 0.76 g/L (IQR: 0.40–1.47 g/L) and 0.51 g/L (IQR: 0.29–1.06 g/L), respectively. D-dimer levels showed superior or excellent discrimination for PE (AUC = 0.844), leukemia (AUC = 0.848), and aortic aneurysm (AUC = 0.963). DVT and aortic dissection demonstrated acceptable discrimination, with AUC values of 0.795 and 0.737, respectively. D-dimer levels in respiratory infections and shortness of breath (not specified) exhibited poor to discriminatory performance.ConclusionThis is the first paper to identify multiple causes of elevated D-dimer levels in Saudi Arabia population within the ED and it clearly highlights their accurate and diagnostic values. These findings draw attention to the importance of considering the specific clinical context and utilizing additional diagnostic tools when evaluating patients with elevated D-dimer levels.

Bronchiectasis After Solid-Organ Transplant: A Retrospective Single Center Study.

Bronchiectasis is characterized by abnormal, persistent, and irreversible enlargement of the bronchi. Many etiological factors have been described, but there are limited data on the development of bronchiectasis after organ transplantation. Our study is the first to study evaluate the frequency of bronchiectasis in heart and liver transplants as well as kidney transplants. Our aim is to analyze the frequency of bronchiectasis development after solid-organ transplant and the characteristics of the cases and to evaluate potential relationships. We retrospectively analyzed data of patients who underwent solid-organ transplant at the Başkent University Faculty of Medicine Hospital through the hospital electronic information system. Demographic, clinical, and laboratory data and thoracic computed tomography scans were evaluated. The study included 468 patients (151 females/317 males). Kidney transplant was performed in 61.5% (n = 207), heart transplant in 20.3% (n = 95), and liver transplant in 18.2% (n = 85) of patients. Development of bronchiectasis was detected in only 13 patients (2.7%). We determined a 13.64-fold risk of developing bronchiectasis in patients with chronic obstructive pulmonary disease and 10.08-fold risk in patients with pneumonia by multivariate regression analyzes, in which all possible risk factors for the development of bronchiectasis after transplant were evaluated. The pathophysiology of transplantassociated bronchiectasis has not yet been clarified. Underlying diseases, recurrent pulmonary infections, and potential effects from immunosuppressive drugs may contribute to the pathogenesis of bronchiectasis. Further prospective studies are needed to include long-term health outcomes in transplant patients with and without bronchiectasis.

Dynamic Projection Method of Electronic Navigational Charts for Polar Navigation

Electronic navigational charts (ENCs) are geospatial databases compiled in strict accordance with the technical specifications of the International Hydrographic Organization (IHO). Electronic Chart Display and Information System (ECDIS) is a Geographic Information System (GIS) operated by ENCs for real-time navigation at sea, which is one of the key technologies for intelligent ships to realize autonomous navigation, intelligent decision-making, and other functions. Facing the urgent demand for high-precision and real-time nautical chart products for polar navigation under the new situation, the projection of ENCs for polar navigation is systematically analyzed in this paper. Based on the theory of complex functions, we derive direct transformations of Mercator projection, polar Gauss-Krüger projection, and polar stereographic projection. A rational set of dynamic projection options oriented towards polar navigation is proposed with reference to existing specifications for the compilation of the ENCs. From the perspective of nautical users, rather than the GIS expert or professional cartographer, an ENCs visualization idea based on multithread-double buffering is integrated into Polar Region Electronic Navigational Charts software, which effectively solves the problem of large projection distortion in polar navigation applications. Taking the CGCS2000 reference ellipsoid as an example, the numerical analysis shows that the length distortion of the Mercator projection is less than 10% in the region up to 74°, but it is more than 80% at very high latitudes. The maximum distortion of the polar Gauss-Krüger projection does not exceed 10%. The degree of distortion of the polar stereographic projection is less than 1% above 79°. In addition, the computational errors of the direct conversion formulas do not exceed 10−9 m throughout the Arctic range. From the point of view of the computational efficiency of the direct conversion model, it takes no more than 0.1 s to compute nearly 8 million points at 1′×1′ resolution, which fully meets the demand for real-time nautical chart products under information technology conditions.

Abstract 821: Uptake of cervical cancer screening services by women living with HIV in Kajiado County Kenya

Abstract Background: This project is an African Caribbean Cancer Consortium (AC3) facilitated study: Women living with HIV (WLHIV) are more susceptible to cancer due to their immune-compromised status. The Kenya Ministry of Health recommends that women diagnosed with HIV should be screened for cervical cancer annually by either visual inspection with acetic acid (VIA) or visual inspection with Lugol’s iodine (VILI), cytology and testing for HPV once every two years. Objective: We determined the uptake of cervical cancer screening and HPV testing by women living with HIV (WLHIV) in Kajiado County in 2022. Methodology: The electronic health information system was used to abstract data for WLHIV in Kajiado county. Kajiado county has 57 antiretroviral (ART) facilities. However, our analysis accessed data from 38 facilities whose data is captured in the health information system. Authorized health care workers from the county were requested to abstract redacted data from the health information system. No personal identifiers were collected from the system as a means of protecting the privacy of individuals and abiding with the Kenya’s data protection law. Authorization to access the county was obtained from the county’s department of health while ethical approval was obtained from Mount Kenya University’s Institutional Scientific and Ethics Review Committee. Results: A total of 16,827 people living with HIV were enrolled for care at the 38 ART selected sites in 2022. Out on the total population that was enrolled for care, 11,785 (70%) were women with an age range of 15 to 49 years. A total of 3342 WLHIV, representing 28.36% of the target population, were screened by using visual inspection with acetic acid (VIA). Data for other methods of screening and HPV testing was not available in the health information systems. Of those screened, 3240 (97%) tested negative, 78 (2.3%) had positive results while 24 (0.7%) had suspicious lesions. Conclusion: Our analysis shows low uptake of cervical cancer screening services with only 28.36% of the targeted women being screened in 2022. The only available testing method (VIA) is less robust as compared to cytology and HPV testing. We also found a gap in reporting of HPV testing in the county. We thus recommend implementation science research examining the systemic barriers to HPV testing and reporting to better inform best practice HPV-related cancer prevention and control policy and practice to reduce cancer-related morbidity and mortality among WLHIV. Laboratory Data Timeliness and Completeness Improves Following Implementation of an Electronic Laboratory Information System in Côte d'Ivoire: Quasi-Experimental Study on 21 Clinical Laboratories From 2014 to 2020.

The Ministry of Health in Côte d'Ivoire and the International Training and Education Center for Health at the University of Washington, funded by the United States President's Emergency Plan for AIDS Relief, have been collaborating to develop and implement the Open-Source Enterprise-Level Laboratory Information System (OpenELIS). The system is designed to improve HIV-related laboratory data management and strengthen quality management and capacity at clinical laboratories across the nation. This evaluation aimed to quantify the effects of implementing OpenELIS on data quality for laboratory tests related to HIV care and treatment. This evaluation used a quasi-experimental design to perform an interrupted time-series analysis to estimate the changes in the level and slope of 3 data quality indicators (timeliness, completeness, and validity) after OpenELIS implementation. We collected paper and electronic records on clusters of differentiation 4 (CD4) testing for 48 weeks before OpenELIS adoption until 72 weeks after. Data collection took place at 21 laboratories in 13 health regions that started using OpenELIS between 2014 and 2020. We analyzed the data at the laboratory level. We estimated odds ratios (ORs) by comparing the observed outcomes with modeled counterfactual ones when the laboratories did not adopt OpenELIS. There was an immediate 5-fold increase in timeliness (OR 5.27, 95% CI 4.33-6.41; P<.001) and an immediate 3.6-fold increase in completeness (OR 3.59, 95% CI 2.40-5.37; P<.001). These immediate improvements were observed starting after OpenELIS installation and then maintained until 72 weeks after OpenELIS adoption. The weekly improvement in the postimplementation trend of completeness was significant (OR 1.03, 95% CI 1.02-1.05; P<.001). The improvement in validity was not statistically significant (OR 1.34, 95% CI 0.69-2.60; P=.38), but validity did not fall below pre-OpenELIS levels. These results demonstrate the value of electronic laboratory information systems in improving laboratory data quality and supporting evidence-based decision-making in health care. These findings highlight the importance of OpenELIS in Côte d'Ivoire and the potential for adoption in other low- and middle-income countries with similar health systems.
