The Normal and Abnormal Unipolar Electrocardiogram in Infants and Children: An Analysis of the Normal QRS Complex Morphology and Ventricular Activation Times

ABSTRACT This monograph has two parts. In the first, Wasserburger presents a survey of electrocardiograms of normal infants and children based on 259,915 individual measurements of the QRS complexes of 1150 persons. The text is profusely illustrated with electrocardiograms demonstrating the variety of normal records seen in various age groups. In addition, there is a large number of charts of mean and maximum activation time and minimum and maximum amplitude of the Q, R, and S waves in leads AVR and precordial leads V3R through V6.In the second section the author uses the normal data to suggest criteria for distinguishing abnormal from normal electrocardiograms. A large number of electrocardiograms typical of each abnormality are shown, as well as tracings which present problems in interpretation. Summary Forty-six electrocardiograms taken on 21 newborn infants of diabetic mothers (30 tracings during the first week of life) fell within the limits of normal as described by Zigler except for S-T abnormalities occurring in one infant during the first day of life and one tracing with a main T axis of −20 degrees at 8 days. The ECG abnormalities occurring in a case of complete transposition of the great vessels are described separately. The evidence that electrocardiographic abnormalities described by others in newborn infants of diabetic mothers may be due to potassium depletion is discussed, and the possibility is presented that these abnormalities may be due to an exaggeration of the increased load on the left ventricle secondary to circulatory changes occurring following delivery. From this small series we would conclude that abnormal electrocardiograms are uncommon in newborn infants of diabetic mothers and that the explanation of such abnormalities remains speculative.

False coronary patterns in the infant electrocardiogram.

Electrocardiographic patterns of myocardial infarction are rarely found in infancy and suggest, in the first place, an anomalous origin of the left coronary artery. Five infants with such electrocardiograms are reported. In all coronary heart disease could be ruled out, in 4 by autopsy and in 1 by the clinical course. Comparison of these 5 cases with previously reported cases with similar electrocardiograms and autopsy control revealed that coronary patterns in infancy occur in a true and false variety. Some distinguishing features are discussed.

Paroxysmal tachycardia with a ventricular rate of 365 per minute

1. 1. A fatal case of supraventricular tachycardia in an infant with a ventricular rate of 365 per minute is presented. As far as can be learned, this is the fastest human ventricular rate ever recorded. 2. 2. The condition was unrecognized until just before death. The signs of congestive failure, which were respiratory difficulty, cyanosis, and congestion of the lungs, were misinterpreted. No evidence of organic heart disease was found at necropsy. 3. 3. Paroxysmal tachycardia is not rare in infants. The condition is amenable to treatment. A suspicion of this condition should be entertained in any child with an episodic history of cyanosis and respiratory difficulty. The more frequent use of the electrocardiogram in infants is recommended.

A note on the importance of proper technique in the recording of the precordial electrocardiogram: The description of an important source of error

The value of electrocardiography, as of any other laboratory method, may be limited by errors resulting from faulty technique as well as by errors of interpretation. One such error of technique which has been found to exert a profound effect on the form of the electrocardiogram in the precordial leads consists of the application of an excessive amount of electrode paste not only on but also between the positions for the exploring leads and results in distortion of the “true” precordial potential variations by surface conduction between the various points. This may be avoided by careful application of electrode paste so as not to establish surface continuity between the exploring leads. It is to be emphasized that the type of error described may be of great clinical and academic importance in the interpretation of the electrocardiograms of infants and children particularly.
