Study of the Relationship between Insulin Resistance, Iron Status Markers and Body Weight in a Sample of Egyptian Population

Abstract Background Diabetes is a group of metabolic diseases characterized by hyperglycemia resulting from defects in insulin secretion, insulin action, or both. The chronic hyperglycemia of diabetes is associated with long-term damage, dysfunction, and failure of different organs, especially the eyes, kidneys, nerves, heart, and blood vessels. Aim of the Work The aim of this study is to investigate the relationship between insulin Resistance, iron Status Markers and body Weight in a sample of Egyptian population. Patients and Methods Our study was case control study conducted on 90 subjects their age was ranging form18 to 70 from diabetes outpatient clinic, and they were divided into 3 groups: Group (1): Non-obese non-diabetic as a control group, Group (2): Obese non diabetic and group (3): Obese diabetic. Results In our study, there was no statistical significant difference found between the three studied groups regarding iron study, Also our study showed that there was statistically non significant correlation between (ferritin level and TIBC), and (ferritin level, HOMA IR), but there was statistical significant positive correlation found between TIBC and FBS (p- value 0.045),Similarly, we did not observe either abnormally high or low ferritin values in our study on subjects with obesity. We found no association between this marker and body weight, in contrast with others. In our study, the higher insulin and HOMA-IR values observed in the subjects with TSAT<20% = 4.2(1.4-11.6) than those with TSAT >20% =2.4 (1.15-6.95), but statistically non significant p- value. The present study also showed that There was statistical significant positive correlation between (T.SAT, serum ferritin), (T.SAT, S.iron) and (T.SAT,SBP) (with p-value 0.03). Our study also showed that there was no statistical significant difference found between patients with T.sat ≤ 20% and those with T.sat > 20% regarding all the studied parameters except ferritin level and iron level was found higher in patients with T.sat >20% than those with T. sat ≤20% with p-value <0.001 and <0.001 respectively. Conclusion Characteristics of iron deficiency or iron overload were not observed in subjects with overweight/obesity and diabetic patients. Our results suggest a complex metabolic dysregulation.

Pilot study: Measurement of first phase ejection fraction in a sample of Egyptian people

Abstract Background Left ventricular dysfunction (LVD) constitutes the final common pathway for a number of cardiac disorders. Ischemic heart disease (IHD) is the dominant cause of heart failure (HF). The remaining etiologies include cardiomyopathy, hypertension (HTN), valvular heart diseases (VHD) and myocarditis. Data from the Framingham heart study shows that it develops in approximately 16% of men and 18% of women who had diabetes; 12% of men and 8% of women who had HTN; and 30% of both sexes who had myocardial infarction (MI). Aim of the Work to measure the first phase ejection fraction (EF1) in a sample of Egyptian population and to determine its normal value and to assess its reliability in assessment of systolic function. Patients and Methods This study is a prospective and observational clinical that was conducted at El- Mokattam health insurance & Ain Shams University hospitals from October 2019 to March 2022. Results we found that mean EF1 was 35.41 ± 4.16 %, its minimal value was 26.9% and its maximal value was 43.4%. We found that EF1 was reliable and reproducible as there was no statistically significant intra observer nor inter observer variabilities (p-value was 0.547 & 0.602 respectively). We found that there was no significant statistical correlation between EF1 and gender or heart rate or other demographic or clinical data. Conclusion EF1 is a simple and reproducible measurement of LV systolic function that is easily measured by transthoracic echocardiography and is considered comparable to other indices that measure LV systolic function.

X-ray Repair Cross Complementing 1 (XRCC1) Gene Polymorphism in Hepatocellular Carcinoma (HCC)

Abstract Background Several risk factors for hepatocellular carcinoma (HCC) have been identified, including chronic infection of hepatitis B virus (HBV) and hepatitis C virus (HCV). Nevertheless, only a fraction of infected patients develops HCC during their lifetime suggesting that genetic factors might modulate HCC development. The human X-ray repair cross- complementing protein 1 (XRCC1) gene encodes for one of the major repair factors involved in base excision repair (BER), which is reported to be associated with the risk of several cancers. A few studies have explored the association between risk of hepatocellular carcinoma (HCC) and single-nucleotide polymorphisms (SNPs) in different DNA repair genes. Aim of the Work to investigate the possible association between XRCC1-G28152A (rs25487) single nucleotide polymorphism (SNP) and hepatocellular carcinoma (HCC) in chronic hepatitis C patients. Patients and Methods The study included 35 patients who had HCC and 15 patients who had liver cirrhosis. XRCC1 gene polymorphism was done to all participants by RT-PCR After collection of relevant clinical data and basic laboratory tests. Results A statistically higher frequency of XRCC1 (GG, GA) genotypes and increased (G) allele frequency in patients with HCC was found in comparison to cirrhotic HCV patients as well as control group but Our results revealed that the genotypic and allelic frequencies of XRCC1 gene polymorphism rs25487 didn’t show statistically significant difference between HCC patients group and liver cirrhosis control group. Conclusion The present study had demonstrated that there was no significant association between XRCC1 gene polymorphism (rs25487) and HCC in the sample of Egyptian population included in our study. Also, non-significant differences among the XRCC1 genotypes and alleles were found regarding age, sex, BMI and laboratory data including CBC, AFP, coagulation profile (PT, INR), liver function test ( total protein, albumin, AST, total bilirubin and direct bilirubin), BCLC and Milan criteria. While a statistically significant difference was observed as regards PTT and ALT between genotypes.

Iron plays a key role in the regulation of body iron homeostasis and is used as a clinical marker for iron deficiency (ID) and hemochromatosis. The idea that iron dysregulation may contribute to various metabolic diseases, such as obesity, insulin resistance, MetS, and T2DM, is a hot topic of discussion. The aim of this study is to investigate the relationship insulin resistance, iron status markers, and body weight in a sample of Egyptian population. A case control study was conducted on 90 subjects with age ranging from 18 to 70 years old from a diabetes outpatient clinic, and they were divided to three groups: Group I, non-obese- non-diabetic as the control group; Group II, obese-non-diabetic; and Group III, obese-diabetic. In our study, there was no statistically significant difference between the three studied groups regarding the different iron parameters. Similarly, we found that neither HOMA-IR nor body weight had a significant correlation with iron status markers. On the contrary, we detected significant positive correlations between the TIBC and the fasting blood glucose, between the serum iron and the LDL, between the TSAT and the systolic blood pressure, and between the HOMA-IR and hematocrit. Our study demonstrated no direct statistical significant relationship between the different iron parameters, obesity, and insulin resistance, either in the diabetic or non-diabetic subjects. This may be due to the complex metabolic dysregulation and the small number of the sample for future investigations.

Standardization of meibomian gland dysfunction in an Egyptian population sample using a non-contact meibography technique.

To develop normative data for meibomian gland dysfunction (MGD) parameters, using non-contact meibography technique of Sirius Costruzione Strumenti Oftalmici (CSO) machine, in an Egyptian population sample. Observational, cross-sectional, analytic study, in which 104 Egyptian volunteers were included. Both upper lids were examined, using "Sirius CSO" machine. Each eyelid was given a degree of meibomian gland loss (MGL), which was calculated by the software of the machine. Mean percentage MGL in right upper lid was of 30.9%±12.6%, and that of left upper lid was 32.6%±11.8%. Thirty-four volunteers (32.7%) had first-degree MGL in their right upper lid, and 67.3% had second-degree loss. One volunteer (1%) had zero-degree MGL in left upper lid, 28 (26.9%) had first-degree loss, and 75 (72.1%) had second-degree loss. Degree of MGL in right upper eyelid was not related to age, but degree of MGL in left upper eyelid increased with age. There was statistically significant difference between both genders for degree of MGL in right eye (P=0.036) and in left eye (P=0.027). Noncontact meibography is a useful non-invasive tool for diagnosing MGL. MGL is diagnosed in 100% of apparently normal individuals; 26.9%-32.7% of which have first-degree MGL, and 67.3%-72.1% have second-degree MGL.

Ossification pursue of the lower ends of radius and ulna and age determination in a sample of Egyptian population between 12 and 19 years

The epiphyseal plates of cartilage of ulna and radius is responsible for bone elongation, the fusion of their distal ends and bodies is used for age estimation. Variation exists due to many factors including ethnicity; accordingly, it is important to create a national profile for age determination using an affordable easy method. The aim of the work was to follow up the ossification of the lower end of the radius and the ulna in the Egyptian population between 12-19 years using X-ray, validating the use of Greulich and Pyle atlas. 197 subjects (103 males and 94 females) aged between 12-19 years were included. Skeletal age was determined using a plain X-ray on the lower end of the radius and the ulna comparing it to Greulich and Pyle’s atlas, and also to chronological age. In males, the mean of chronological age, skeletal age, and the difference between them were 15.35, 15.53 and +0.18 years respectively. In females, the ages were 14.77, 14.92 and +0.15. There was high correlation in all age groups. Examination of the lower end of the radius and the ulna is a suitable method for age detection in Egyptians between the ages of 12-19 years old.

Age Estimation Based on Medial Clavicular Epiphyseal Union in A Sample of Egyptian Population Using Multi-Detector Computed Tomography Scan

Background Investigating the medial clavicular epiphysis has been crucial in forensic age estimation. Multi-detector computed tomography (MDCT) scanning has been proven to be better than other radiological modalities in forensic identification. Aim To estimate the age from medial clavicular epiphyseal union using MDCT scanning in a sample of Egyptian population. Patients and methods This study was conducted on thoracic MDCT scans of 139 Egyptian participants (68 males, 71 females). Their age ranged between 10 and 35 years. The right medial clavicular epiphysis in each computed radiography (CT) image was evaluated to detect the stage of epiphyseal union according to Schmeling and colleagues (2004) classification. Results There was no significant difference between both sexes regarding the frequency of union stages of the right medial clavicular epiphysis. Stage 3 starts at ages 19–35 in males, and ages 17–24 in females while stage 4 appears at ages 25–35 in males and ages 20–33 in females, with significant differences between both sexes (P < 0.001 and 0.001 regarding stage 3 and 4, respectively). Statistically significant correlations (P < 0.001) were detected between the medial clavicular fusion stage and the chronological age in females, males, and both sexes. Univariate linear regression analyses in males, females, and all participants revealed equations for age prediction from the right medial clavicular stages. Conclusions The stages of the right medial clavicular epiphyseal union contributed significantly to age estimation in males, females, and both sexes. Regression equations have been explored, and they warrant further external validation.

Assessment of association between interleukin 6 gene variant and its serum level in chronic obstructive pulmonary disease in a sample of Egyptian population

Abstract Background Researches in chronic obstructive pulmonary disease (COPD) had shown signs of inflammation with inflammatory markers that had pivotal role in COPD. some studies proposed that elevated serum interleukin-6 (IL-6) is associated with negative impact on lung functions but up to our knowledge, no studies were performed to correlate the genotypes of IL-6 with COPD patients in Egyptian and middle east populations. Objectives To study both the correlation of serum level of IL-6 as well as its gene variant (also known as G174C)) as possible genetic marker for COPD patients in Egyptian patients. Materials and methods This study is case-control that enrolled ninety participants; it consisted of two groups. Group I included 45 patients who were diagnosed as stable COPD; Group II included 45 healthy participants. After full clinical and pulmonary functions assessment of all participants, blood samples were collected from both groups for the detection serum level of IL-6 by ELISA and the detection of IL6 variant by TaqMan Real-Time PCR. Results Our study revealed that there was statistically significant increase in serum level of interleukin-6 in COPD patients compared to normal individuals (P value <0.001) and the presence of G/G variant of interleukin 6 gene in COPD patients more than normal subjects with frequency of 84.4% and 53.3% for COPD patients and healthy controls respectively. There was significant correlation between the serum level of interleukin-6 and COPD staging (P value <0.007). Conclusion The genotype GG variant of IL6 contributes to the prediction of COPD and may be used as a marker for the prediction of COPD risk in Egyptian population. Serum level of IL6 is markedly higher in COPD patients and can be used as a parameter of correlation with COPD stages.

CBCT Based Evaluation of the Anatomical Variation of Retromolar Canal in A Sample of Egyptian Population. A Retrospective Study

The goal of this study is to estimate the prevalence of the retromolar in the Egyptian population and to enhance the allocation of the retromolar canals and foramina. Methodology: Using a Newtom Giano HR CBCT system, radiographic evaluation of 250 CBCT scans was performed. According to the manufacturer's guidelines, multiple FOVs were used with the same voxel size (0.3 mm) to produce distinct scans for different patient sizes. The presence of retromolar canal was checked for, using the CBCT scans. Linear measurements were made from the retromolar foramen to the mandibular foramen in an anteroposterior and a mediolateral directions, as well as to the lower second and third molars, and their courses was divided into three basic classes. Results: In this study, retromolar canal was absent in sides 207 (82.8%) while was presented in 43 (17%), absence of retromolar canal was significantly higher than its presence as P<0.0001. Regarding distribution, bilateral side (16.3%) was significantly lower than unilateral distribution (83.7%) as P<0.0001. In class distribution, class A (70%) was significantly the highest, while class C (6%) was significantly the lowest. Conclusion: The prevalence of retromolar canal in the Egyptian population is 17%. When evaluating the retromolar region to identify the presence of the retromolar canal and retromolar foramen to prevent problems after various dental surgical procedures, the use of CBCT is crucial.

Predictive accuracy of ultrasonography on upper ends of radius and ulna in age estimation in a sample of Egyptian population

Background Forensic age estimation is considered a prime focus of forensic anthropological analysis of living and human skeletal remains. Aim This study aimed to assess the accuracy of ultrasonography (US) in bone age estimation and to compare the predictability of age detection using ultrasound on the upper ends of the radius and ulna in a sample of Egyptian population. Patients and methods This cross-sectional study was conducted on 111 Egyptian volunteers of both sexes aged from 10 to 25 years old and subjected to the sonographic examination of the elbow joint. The volunteers were grouped into three age groups: group 1 (from 10 up to <12 years), group 2 (from 12 up to 16 years), and group 3 (> 16 up to 25 years). Results There was no statistically significant difference between both sexes concerning the age groups suggested by the stage of the epiphyseal union of the upper ends of the radius and ulna detected by US examination. The Kappa agreement test showed that there was a good agreement between the age estimated by the US examination based on the stage of the epiphyseal union of proximal ends of radius and ulna and the official age (Kappa=0.756, and 0.630, respectively). Conclusion US can be used as a suitable alternative modality for age estimation. The studied bone ends showed good agreement between the official age and the US estimated age. The proximal radius was with the highest agreement.

Role of FGF21 in Gestational Diabetes Mellitus in a Sample of Egyptian Population

Abstract Background Gestational diabetes (GDM) is defined as carbohydrate intolerance that begins or is first recognized during pregnancy. Gestational diabetes mellitus (GDM) affects approximately 5 to 7% of pregnancies and is associated with increased risk for fetal overgrowth, cesarean delivery, birth trauma, and pre-eclampsia. Objective To investigate serum FGF21 concentrations in a sample of Egyptian patients with GDM as compared with healthy pregnant controls matched for gestational age And its correlation to clinical and biochemical changes (Glucose, insulin, lipids) and fetal wellbeing using obstetric ultrasound to assess fetal size and development of congenital malformation. Methods This cross-sectional study was conducted on 80 participants collected from Obstetrics and Gynecology clinic at Ain Shams University Hospital during the period from November 2019 to March 2021. Results Shows that there is statistically significant increase in FGF21 in female pregnants with GDM as compared to female pregnants without GDM. The high FGF21 level didn’t affect fetal wellbeing. And both groups had normal fetal development with absence of macrosomia and congenital malformation as shown on results of obstetric ultrasound. In addition, there is statistically significant positive correlation between fasting FGF21 level and each of total cholesterol, LDL, HDL and TG (R +ve, P.< 0.05). Conclusion This high level of FGF21 may be FGF21 resistance or compensatory response to the underlying metabolic stress. FGF21 may have reduced an even higher level of blood glucose than the measured one, blood glucose level could have been in the Range of 400-500 mg/dl instead the measured range 200-300 mg/dl. Obstetric ultrasound results revealed normal fetal wellbeing and no change in fetal size nor development of congenital malformation between both groups.

Study of Relationship between Stool Lactobacillus acidophilus and Grave’s Disease in a Sample of Egyptian Population

Abstract Background Graves’ disease is an autoimmune disease that leads to a generalized overactivity of the entire thyroid gland (hyperthyroidism). When unrecognized, It impacts negatively on quality of life and poses serious risks of psychosis, tachyarrhythmia and cardiac failure. It affect 0.5% of the population in the US, and represents 50-80% of cases of hyperthyroidism. It occurs more commonly amongst women, smokers and patients with other autoimmune diseases or a family history of thyroid autoimmunity. Aim of the Work The study is aimed to study the relationship between stool lactobacillus acidophilus and grave’s disease in a sample of Egyptian population Patients and Methods This study is a case control study, was carried out at Endocrine outpatient clinic at Ain Shams University Hospital, on 60 Egyptian patients age 25-40 years old, from September 2020 till March 2021. Results Among group1A, 5(25%) were 25 -30 years old, 6(30%) were 30 – 35 years old, 9(45%) were ≥ 35 years old, the mean of ages was 32.75 (±4.94 SD) with range (25.0 – 40.0), there were 5(25%) males, 15(75%) females, the mean of duration of the disease was 2.95(±2.14 SD) with range (1.0 – 7.0). Among group 1B, 8(40%) were 25 -30 years old, 5(25%) were 30 – 35 years old, 7(35%) were ≥ 35 years old, the mean of ages was 31.90(±4.69 SD) with range (26.0 – 40.0), there were 3(15%) males, 17(85%) females, the mean of duration of the disease was 3.25(±2.02 SD) with range (1.0 – 7.0). Among total cases of group 1, 13(32%) were 25 -30 years old, 11(27.5%) were 30 – 35 years old, 16(40%) were ≥ 35 years old, the mean of ages was 32.33(±4.77 SD) with range (25.0 – 40.0), there were 8(20%) males, 32(80%) females, the mean of duration of the disease was 3.10 (±2.06 SD) with range (1.0 – 7.0). Conclusion Stool Lactobacillus acidophilus PCR count was insignificantly different between studied groups, which may show no relationship of lactobacillus with graves’ disease. so, further studies are needed to determine correlation or causation of this relationship.
